Acute pancreatitis may be the first manifestation in systemic lupus erythematosus or occur during evolution. It is a rare complication, which is often associated with other visceral manifestations. Outcome is usually favorable but can be serious. We report a case of a 17‐year‐old girl with a past history of systemic lupus erythematosus who developed acute pancreatitis revealed by abdominal pain. Elevated serum amylase and lipase levels and pancreatic enlargement on tomography confirmed the diagnosis. Systemic lupus erythematosus (SLE) is a multisystem autoimmune disease. Although gastrointestinal (GI) involvement is reported in the literature, GI manifestations of lupus are not common in the early disease course. GI symptoms can be related to several factors other than lupus; however, systemic lupus per se can be the responsible factor. Although the typical presentation is a gradual onset of progressive symptoms, acute abdominal pain is frequently reported and is the most serious presentation. It can reflect gastroenteritis, cholecystitis, hepatitis, pancreatitis, peritonitis, and abdominal vasculitis. When SLE diagnostic criteria for GI manifestations are lacking, the decision to implicate lupus as the cause of these manifestations is difficult, especially in the primary presentation. Early diagnosis and the initiation of immunosuppressive agents are associated with a better outcome. In this case, we introduce a patient who presented with acute abdominal pain secondary to acute liver failure as the first manifestation of lupus. [ABSTRACT FROM AUTHOR]

Abstract: Purpose: The occurrence of acute pancreatitis in systemic lupus erythematosus (SLE) is known but rare, and is exceptionally the presenting manifestation. Its pathogenesis is multifactorial, and it is difficult to separate what belongs to vasculitis, thrombotic phenomena in the context of an associated antiphospholipid syndrome, or iatrogenic complications. We report on six cases of lupus pancreatitis. Methods: This is a retrospective monocenter study of 110 patients with SLE. The diagnosis of lupus pancreatitis was established after exclusion of other causes of pancreatitis. Results: Five women and one man (5.4%) with a mean age of 36.3years presented with lupus pancreatitis. In four patients the pancreatitis was concurrent with the diagnosis of SLE and it occurred later during an exacerbation of the disease in the two remaining patients. In all patients, pancreatic manifestations were associated with other organ involvement. Clinical manifestations were: abdominal pain (n =6), vomiting (n =3), and fever (n =3). Elevated pancreatic enzyme was noted in all cases. All patients were treated by high doses of glucocorticoids. The outcome was favorable in five patients, and one patient died. Conclusion: Pancreatitis may be the presenting manifestation of SLE. Its pathogenesis is often multifactorial. The outcome is usually favorable with corticosteroids. [Copyright &y& Elsevier]

Pancreatitis as a manifestation of systemic lupus erythematosus (SLE) is rare and rates are less than 5% in published series in Europe and United States. Diagnosis relies on clinical, serological and imaging findings, and the exclusion of other common causes of pancreatitis. The mechanism by which a patient with SLE develops pancreatitis is not fully understood, but it is considered multifactorial, being vasculitis and microthrombosis the main pathogenic mechanisms involved. Lupus pancreatitis commonly presents in patient with moderate to severa SLE activity and portends a worst prognosis. Nonetheless, it is highly responsive to steroids, thus, a timely diagnosis is paramount. We present a case of lupus pancreatitis as the inaugural manifestation of SLE. [ABSTRACT FROM AUTHOR]
