3. History of leprosy in Mexico: from the 15th century to the 21st century.

- Abstract
Leprosy is an infectious disease transmitted by the bacterium Mycobacterium leprae. While it's not a new disease, its incidence over time has changed, and in some places, it has been eradicated. However, in other areas, it is still considered a public health problem, and Mexico is one of them. Throughout global history, diagnostic, therapeutic, prevention, control, and surveillance strategies have been implemented to reduce its incidence. Currently, our country is attached to various national and international programs for managing this disease, aiming to reduce its prevalence and eventually eradicate it. Nevertheless, many socioeconomic factors have hindered the complete achievement of these goals, which have been modified but not entirely eliminated. The purpose of this review was to conduct a retrospective analysis of leprosy management in Mexico, aiming to present the current situation to raise awareness of the potential seriousness of this disease becoming a public health priority again. This review also seeks to promote new research to identify the cause of the recent increase in cases over the past year. It includes a historical breakdown of the epidemiological trends of this disease in the country, as well as the various therapeutic and diagnostic measures and their impact in the current context. The year 2024 is the Centenary of the foundation of the Leprosy Relief Association (Lepra), formerly the British Empire Leprosy Relief Association (BELRA). The name of the organization changed to the LEProsy Relief Association (LEPRA) in 1976 but has been known as Lepra since 2008. Over the years it has worked closely with members and office holders of the Royal Society of Tropical Medicine and Hygiene. Its work has encompassed activities from the earliest initiatives to ensure appropriate living conditions for those with the disease to the development of leprosy chemotherapy. However, this has now evolved into a strong partnership between the UK- and India-based Lepra hubs, which are carrying out research and public health initiatives ranging from elimination of prejudice against those with leprosy to adopting the recently launched WHO programme for skin NTDs to facilitate integrated control and management regimens. The fight against leprosy has always been a partnership between a wide variety of disease-specific NGOs, health-care workers and international health agencies. The story of Lepra illustrates the central role of these partnerships and national as well as international collaboration. [ABSTRACT FROM AUTHOR]

Objetivo: Analizar el seguimiento de las personas afectadas de lepra en los servicios sanitarios durante la pandemia de COVID-19. Material y método: estudio exploratorio, descriptivo, con abordaje cualitativo, realizado en Unidades Básicas de Salud y en el Servicio de Referencia en Lepra de Campina Grande-Paraíba - Brasil. Recogida de datos entre octubre de 2021 y febrero de 2022. La población del estudio estaba formada por usuarios que seguían un tratamiento y eran controlados para detectar reacciones leprosas. Los datos se analizaron mediante el análisis de contenido de Bardin. Resultados: surgieron tres categorías, a saber, la falta de conocimientos de los usuarios sobre la lepra y su control, sus sentimientos sobre la enfermedad y el tratamiento, y el debilitamiento del control de la lepra en la Atención Primaria de Salud. Conclusión: Observamos fragilidad en el seguimiento de los usuarios afectados por la lepra, relacionada con la falta de medicación y de profesionales, así como con la propia percepción de la enfermedad por parte del usuario.

Objetivo: Caracterizar el perfil epidemiológico de los casos y muertes por lepra en Mato Grosso - Brasil, de 2011 a 2020. Método: estudio observacional basado en la población de todos los casos y muertes por lepra notificados entre 2011 y 2020. Posteriormente, se emparejaron los individuos declarados y se estimó la tendencia mediante la regresión de Prais-Winsten. Resultados: Se registraron 37.623 nuevos casos en los años de estudio, una tasa media de 114,7/100.000 habitantes, con tasas de incidencia más elevadas en los casos multibacilares y en los varones, así como en la proporción de muertes (58%; 88,2% respectivamente). En cuanto a la tendencia, ha aumentado con los años y en el sexo femenino. Conclusión: Los resultados sugieren que existe un perfil de más casos incidentes, posibles zonas de propagación de la enfermedad y un retraso en el diagnóstico, lo que pone de manifiesto la necesidad de utilizar indicadores como forma de seguimiento de la endemia, de reforzar la atención integral y la vigilancia de la lepra.

Introducción: La lepra es una enfermedad infectocontagiosa crónica con incidencia en varios países del mundo. En Cuba, deja de ser un problema de salud desde 1993; pero aún se reportan casos nuevos, con un promedio de 200 al año. Objetivo: Caracterizar la incidencia de la lepra en Cuba, durante el decenio del 2012 al 2022. Método: Se realizó un estudio observacional, descriptivo de corte transversal. Para la recolección y análisis de los datos se accedió al Anuario Estadístico de Salud, en formato digital desde la edición del año 2013 al 2023, las variables fueron: la incidencia, según los años, el sexo, el grupo de edad y las formas clínicas de la enfermedad. Se utilizó la estadística descriptiva para el procesamiento de la información. Resultados: Entre los años del 2012 al 2022, se diagnosticaron 2024 personas con la enfermedad; los años de mayor incidencia fueron el 2012 y el 2013 con 258 y 232 casos en ese orden, el 55,53 % de las personas enfermas fueron del sexo masculino con 1130 casos; hubo mayor representación del grupo de mayores de 15 años ,1962 96,94 %; la forma clínica que sobresalió en la incidencia de la lepra fue la lepromatosa ,789 con 39 %. Conclusiones: La tendencia a la disminución de los casos, el predominio del sexo masculino, del grupo de mayores de 15 años y la forma clínica lepromatosa como la más sobresaliente, caracterizan la lepra en Cuba, en los años del estudio.

11. Datos epidemiológicos de la casuística de lepra en un hospital de tercer nivel de 1990 a 2023.

OBJECTIVE: To analyze the epidemiological data of patients with leprosy treated in the Dermatology Service of the General Hospital Dr. Manuel Gea González, in order to provide a reference on its incidence in a group of Mexican patients. MATERIALS AND METHODS: Retrospective, observational, descriptive and crosssectional study in which data were collected from the Hospital General Dr. Manuel Gea González, Mexico City, from 1990 to August 2023. RESULTS: A total of 117 patients with a diagnosis of leprosy were found. The most common age group was of 40 to 59 years (n = 59, 50.4%). A higher incidence was observed in men (n = 75, 64.1%), male:female ratio of 1.7:1. The most common variant of leprosy was lepromatous leprosy (n = 78, 66.6%). Mexico City was the location with the highest concentration of cases (n = 37, 31.6%). CONCLUSIONS: The data collected in this analysis contribute to the objective of knowing the frequency of leprosy cases in a referral hospital and by having verifiable data we will have a notion of the current problem in Mexico and thus try to participate in the Global Leprosy Strategy 2021-2030 that aims to further reduce the burden of this disease worldwide. To analyze the epidemiological profile of new cases of leprosy with physical disability in the Northeast region of Brazil between 2018 and 2022. Epidemiological, ecological, cross-sectional study of a descriptive and retrospective nature, with a quantitative approach. The sample was formed by leprosy cases diagnosed with some degree of physical disability in the Northeast region of Brazil, reported in the Compulsory Notification Diseases Information System. 13,928 cases of leprosy with some degree of disability were reported. A higher prevalence of disabilities was seen in men (62.9%), mixed race (64.0%), incomplete primary education (39.6%), aged 50 to 59 years (20.4). Clinical variables showed a prevalence of multibacillary cases (90.1%), with more than five lesions (47.1%), without a reactional episode (54.1%) and use of 12 doses of MDT (88.6%). Data demonstrated that late diagnosis is the main factor that leads to the appearance of disabilities. [ABSTRACT FROM AUTHOR]

18. Aristotelian and other auctoritates in the works of Enrique de Villena, and their possible sources

My objective has been to examine, selectively, 'auctoritates' which Villena, who was working during the early days of the emergence of 'vernacular humanism' in Castile, used in his body of work, and to identify their possible sources. My approach assigns all authoritites used by Villena to seven key groups (e.g., classical, biblical) and eight categories of possible source works (e.g., 'florilegia', encyclopaedias), for which I determine relationships. In what I consider the key component, I review the Latin extracts used by Villena in 'Consolación', 'Lepra' and 'Aojamiento' in an extensive exercise in which I identify more than 300 possible primary references. I find that approximately one third of these references can be attributed to Vincent de Beauvais's 'Specula' and his briefer works. This figure can be extended to more than 400 examples when secondary possible sources are taken into account. Other significant possible sources I identify include 'Auctoritates Aristotelis', popular in the Middle Ages, 'Compendium moralium notabilium' (using the 1505 printed version 'Epytoma sapientie'), Pietro Alighieri's 'Commentum', and the 'Verona florilegium' (of 1329). I have also made an assessment of the extent to which 'Auctoritates Aristotelis' might be a possible source throughout Villena's body of work, not solely for Aristotle, but also for Boethius and Seneca. In a third evaluation, I assess the possible sources of what I identify as a 'double authority' in 'Glosas' 35 (Plato/Boethius and Vegetius). As a result, I have formed the opinion that a conjectural notebook, similar in scope to the Count of Haro's 'Vademecum', a collection of excerpts distilled from several other collections of excerpts, including, for example, 'Auctoritates Aristotelis', could have been compiled for Villena's use, obviating his need to access more than 20 source works or necessitate his dependence on complete texts of the works to which he makes reference in his writings. Overall, I judge that my work is significant in the study of Villena insofar as it establishes a substantially enhanced understanding of Villena's use of authorities and his possible sources that is not dependent on the work of Cotarelo.

Objetivo: evaluar la calidad de vida, la funcionalidad y la autoestima de las personas después del alta de la lepra. Métodos: Se trató de un estudio epidemiológico transversal realizado en una institución de referencia para lepra en Paraíba, Brasil, de mayo a agosto de 2021. Se utilizaron cuatro instrumentos para caracterización sociodemográfica y clínica, evaluación de la calidad de vida, funcionalidad y autoestima. Los datos se analizaron mediante las pruebas chi-cuadrado de Pearson, exacta de Fisher, Mann-Whitney y Kruskal-Wallis. Resultados: Participaron 131 personas, de las cuales 86 (65,6%) eran hombres y 72 (55%) tenían entre 41 y 60 años. Se observó un impacto negativo en la calidad de vida relacionada con la salud entre las personas sin estudios y con bajos ingresos familiares. Limitación de la funcionalidad en los que no practican actividad física, afectados por neuritis y con algún grado de discapacidad física. Sin embargo, alta autoestima. Conclusión: Es fundamental formar a los profesionales y organizar medidas asistenciales de cara a la atención longitudinal e interdisciplinar en esta fase terapéutica.

Introducción: La lepra o enfermedad de Hansen es una enfermedad infecciosa bacteriana que durante largo tiempo se ha considerado como un estigma. Objetivo: Caracterizar la preparación del personal médico de la Atención Primaria de Salud sobre enfermedad de Hansen. Métodos: Se realizó un estudio observacional descriptivo, corte transversal, durante los meses de julio a septiembre de 2022. El universo de estudio estuvo conformado por 102 médicos de familia que se encontraban laborando en los 42 consultorios pertenecientes al Policlínico Docente ¨Ernesto Guevara¨. Las variables analizadas fueron: nivel de conocimiento del personal médico sobre enfermedad de Hansen y los factores relacionados con la preparación profesional para el manejo de la enfermedad en la Atención Primaria de Salud. El número absoluto y el porcentaje fueron las medidas de resumen estadístico utilizadas. Resultados: El 61.77 % del personal médico presentó un conocimiento adecuado sobre la realización del examen dermatoneurológico, seguido del dominio de los síntomas y signos de la enfermedad, 50.98 %. Entre los factores relacionados con la preparación profesional para el manejo de la enfermedad predominaron las fuentes de información actualizadas en el 59.80 % y la motivación por el tema en el 56.86 %. Conclusiones: La preparación del personal médico que labora en la Atención Primaria de Salud del municipio de Niquero sobre la enfermedad de Hansen, representa una fortaleza en la detención temprana y el adecuado control epidemiológico.

21. Intervención Farmacéutica Educativa en pacientes que finalizaron el tratamiento contra la lepra

Introducción: En su roll de educador sanitario el farmacéutico realiza Intervenciones puntuales en enfermedades crónicas como la Intervención Farmacéutica Educativa (IFE) que puede ser aplicada en la Unidad de Salud de la Familia. (USF) . Objetivo: Evaluar la IFE en pacientes con lepra que finalizaron el tratamiento farmacológico entre 2011 y 2017 en Unidades de Salud de la Familia (USF) de cinco distritos del departamento central. Método: Estudio cuasi experimental, participaron 34 pacientes que finalizaron el tratamiento farmacológico contra la lepra. Se realizaron tres entrevistas domiciliarias con cada uno de ellos. Se utilizó un cuestionario de elaboración propia para medir conocimientos sobre la enfermedad, tratamiento y transmisión, mientras que para medir calidad de vida se utilizó el cuestionario SF-36 antes y después de la IFE. Resultados: En la tercera entrevista los pacientes mejoraron conocimientos, se obtuvieron mayores porcentajes de respuestas correctas en ítems como, reacciones de la lepra 100%, causas de las reacciones 82,4 %, frecuencia de control médico posterior al tratamiento 61,8%, duración del seguimiento 97%, manifestación de signos de lepra en contactos hasta 10 años después del contagio 97,1% y evaluación de contactos por personal de salud 97,1%. En el item estado de salud general mejoró la puntuación. Conclusiones: Los resultados han demostrado que los pacientes que finalizaron el tratamiento contra la lepra intervenidos, mejoraron sus conocimientos en ciertos aspectos. Esto evidencia la utilidad de la IFE como estrategia de educación sanitaria y podría implementarse en Unidades de Salud de la Familia.

A doença de Hansen, vulgarmente conhecida como lepra, é uma doença infecciosa causada por Mycobacterium leprae. Sendo rara nos países desenvolvidos, configura uma doença de importação cada vez mais frequente considerando o fluxo migratório de países onde é endémica. Apresentamos o caso de um homem de 21 anos que recorreu ao serviço de urgência por poliartralgias de caráter aditivo envolvendo grandes articulações, pápulas e placas eritematosas nos membros com bolhas e necrose central e febre com calafrio com uma semana de evolução. Foi realizada biópsia cutânea que revelou infiltrado neutrofílico com granulomas de distribuição perineural e baciloscopia com deteção de bacilos ácido-álcool resistentes. Foi estabelecido o diagnóstico de DH multibacilar com reação lepromatosa tipo 2 (eritema nodoso leproso), apresentando melhoria clínica sob corticoterapia. O eritema nodoso leproso cursa habitualmente com lesões dolorosas, configurando uma apresentação atípica de lepra, sobretudo na presença de bolhas e necrose, tornando este diagnóstico altamente desafiante. O estigma social é frequentemente limitativo na aceitação da doença e adesão ao tratamento.

The genus Varicellaria was originally erected for a single species that was similar to the species then included in Pertusaria, which all had simple ascospores, but differed in having 1-septate ascospores. However, a molecular analysis has shown that septate ascospores is not a genus-level trait in the group and that Varicellaria was better characterised by the thallus containing lecanoric acid, and several species lacking a septum have subsequently been transferred to Varicellaria from Pertusaria. Here, Varicellaria cacuminum is described as new to science from rocks on the summits of several mountains on Campbell Island/ Motu Ihupuku, New Zealand. It is characterised by the thallus lacking soralia, the presence of lecanoric acid, and huge ascospores up to 315 µm long. The new species is compared with other members of the genus and Pertusaria otagoana, a species with similar sized ascospores. Pertusaria otagoana, which has only been reported from its type locality near Dunedin, is also reported from Campbell Island and South Island, New Zealand. The status of the New Zealand species of Pertusaria transferred to Lepra is discussed and it is concluded that Pertusaria erubescens and P. muricata should be retained in Pertusaria, whereas it is not currently possible to determine the correct generic placement of the other three species. 27. Intervención Farmacéutica Educativa en pacientes que finalizaron el tratamiento contra la lepra

Introducción: En su rol de educador sanitario el farmacéutico realiza Intervenciones puntuales en enfermedades crónicas como la Intervención Farmacéutica Educativa (IFE) que puede ser aplicada en la Unidad de Salud de la Familia. (USF) . Objetivo: Evaluar la IFE en pacientes con lepra que finalizaron el tratamiento farmacológico entre 2011 y 2017 en Unidades de Salud de la Familia (USF) de cinco distritos del departamento central. Método: Estudio cuasi experimental, participaron 34 pacientes que finalizaron el tratamiento farmacológico contra la lepra. Se realizaron tres entrevistas domiciliarias con cada uno de ellos. Se utilizó un cuestionario de elaboración propia para medir conocimientos sobre la enfermedad, tratamiento y transmisión, mientras que para medir calidad de vida se utilizó el cuestionario SF-36 antes y después de la IFE. Resultados: En la tercera entrevista los pacientes mejoraron conocimientos, se obtuvieron mayores porcentajes de respuestas correctas en ítems como, reacciones de la lepra 100%, causas de las reacciones 82,4 %, frecuencia de control médico posterior al tratamiento 61,8%, duración del seguimiento 97%, manifestación de signos de lepra en contactos hasta 10 años después del contagio 97,1% y evaluación de contactos por personal de salud 97,1%. En el item estado de salud general mejoró la puntuación. Conclusiones: Los resultados han demostrado que los pacientes que finalizaron el tratamiento contra la lepra intervenidos, mejoraron sus conocimientos en ciertos aspectos. Esto evidencia la utilidad de la IFE como estrategia de educación sanitaria que podría implementarse en Unidades de Salud de la Familia.

The case report of a patient infected by the human immunodeficiency virus in early symptomatic phase is described, who was seen at the Dermatology Service of the Dr. León Cuervo Rubio Teaching Clinical Surgical Hospital of Pinar del Río for presenting infiltrated erythematous lesions of three months of evolution in the right lower limb, with altered pain and thermal sensation. A basiloscopic and histopathologic study was indicated, which confirmed the presumptive diagnosis of borderline tuberculoid leprosy, and therefore polychemotherapy combined with antiretroviral therapy was started, which favored the patient's evolution. BACKGROUND: Leprosy or Hansen’s disease is caused by Mycobacterium leprae; clinically it occurs from skin lesions to severe neural lesions due to peripheral nerve involvement and sensory dysfunction. CLINICAL CASE: A 55-year-old male patient, with no significant medical history, who consulted the outpatient service due to the initial appearance of multiple nodular, erythematous lesions with burning pain and itching, in who lepromatous leprosy was diagnosed. CONCLUSIONS: The recognition of skin lesions in primary healthcare approach, and the ability to generate differential diagnoses allow an adequate clinical and paraclinical approach, having a favorable impact on medical management. Analisamos os planos da construção do prédio, principalmente levando em consideração a teoria dos miasmas; concepções sobre as terapêuticas da lepra na época, sobretudo quanto à transmissibilidade da doença; e, por fim, a idealização do médico acerca das rotinas de tratamento, trabalho, lazer e recuperação dos doentes.

Se describe el caso clínico de un paciente infectado por el virus de la inmunodeficiencia humana en fase sintomática precoz, quien fue asistido en el Servicio de Dermatología del Hospital Clínico Quirúrgico Docente Dr. León Cuervo Rubio en Pinar del Río por presentar lesiones eritematosas infiltradas de tres meses de evolución en el miembro inferior derecho, con alteración de la sensibilidad térmica y dolorosa. Se le indicó baciloscopia y estudio histopatológico que confirmaron el diagnóstico de sospecha de lepra dimorfa tuberculoide, por lo que se comenzó a aplicar poliquimioterapia combinada con terapia antirretroviral, lo cual favoreció la evolución del paciente.

Resumo O artigo analisa uma experiência de cura da lepra com assacu (Hura crepitans L.), realizada em Santarém, Pará, em 1847, por um indígena chamado Antonio Vieira dos Passos. A experiência passou a ser realizada nas demais províncias do Brasil e também no exterior. Por essa razão, o artigo estabelece relações com práticas médicas realizadas em outras partes do país, tendo como foco o diálogo entre a medicina oficial e a medicina indígena. A análise de matérias de jornais e documentos oficiais revelou que os saberes indígenas sobre o uso de plantas medicinais eram amplamente reconhecidos e utilizados pelos médicos com a intenção de incorporá-los em seu repertório terapêutico.

A partir del registro de las campañas médicas se vislumbra las condiciones de higiene y salud de la población, así como diversos aspectos que el Estado buscó erradicar y controlar en diversos sectores de la población con el fin de mejorar la calidad de vida de sus habitantes. El trabajo analiza mediante fuentes de archivo los esfuerzos de las autoridades posrevolucionarias por enseñar y promover una cultura de la prevención e higiene principalmente en las áreas rurales.

Objetivo. Describir el comportamiento epidemiológico de la lepra en varios países latinoamericanos durante 2011-2020, tomando como base los indicadores de la Organización Mundial de la Salud (OMS). Método. Estudio transversal, descriptivo y cuantitativo con datos oficiales sobre incidencia y prevalencia en la población en general, niños, forma clínica y casos de discapacidad de grado 2 registrados por la OMS entre 2011 y 2020. Se seleccionaron los ocho países latinoamericanos con más casos y se realizaron análisis de estadística descriptiva simple y comparativa entre las variables. Resultados. Entre 2011-2020 se reportaron 301 312 casos de lepra en los países seleccionados: Argentina, Brasil, Colombia, Cuba, México, Paraguay, República Dominicana y Venezuela. Brasil fue el único con una prevalencia mayor a 1 por 10 000 habitantes y representó 93,77% de casos. En Brasil y República Dominicana se observó un aumento de la prevalencia durante 2011-2019, mientras que en los demás países esta tendió a disminuir. La enfermedad fue más frecuente en hombres, y los casos multibacilares superaron significativamente a los paucibacilares. En Brasil se encontraron las mayores incidencias de casos de lepra infantil y discapacidad de grado 2 durante el período evaluado. Conclusión. A pesar de que en América Latina la lepra solo es considerada un problema de salud pública en Brasil, la mayoría de los países de la región continúa reportando casos cada año, lo que revela una falta de atención médica adecuada. El estudio confirmó la importancia de la vigilancia activa, el diagnóstico temprano y la planificación de acciones contra la enfermedad en todos los países evaluados, con el propósito de disminuir o detener su transmisión.

We describe a case of a male patient with systemic lupus erythematosus (SLE) and lupus nephritis. A patient who was initially diagnosed with multibacillary leprosy, an infectious disease, with clinical symptoms for two years. However, after hospitalization and investigation, his diagnosis was revoked and replaced with SLE. The aim of this study is to emphasize the importance of knowing the most important and significant clinical changes in SLE and thus allowing an accurate diagnosis, preventing disease progression with target organ involvement, and allowing better clinical management. This work aimed to analyze the formation and the role of the nursing professional of the Family Health Strategy in the care of people with leprosy in Basic Health Units in hyper-endemic territory. This is a qualitative investigation, which used semi-structured questionnaire and content analysis as a method of data collection. The testimonies showed that humanization in contact with the patient and the integrated work of the interprofessional team maximize the possibility that the patient with leprosy will adhere to the treatment and persist in the journey of selfcare. The treatment was a central aspect in the speech of the nurses participating in the research, indicating a certain limitation of resources for prophylaxis and additional resources. The protocols of the Ministry of Health proved to be useful, and the initial and permanent training of health professionals to treat leprosy is a central element for conclusions and considerations in this investigation.
