Living donor liver transplantation is an important treatment option in countries like ours where cadaver donors are insufficient. This surgery is a major operation for the donor as well as the recipient. We should minimize the damage to donors who come for organ donation in a healthy way and minimize the risks of such a major surgery. Endometriosis is increasingly diagnosed in women of reproductive age and is estimated to affect up to 10% of women. Endometrioma in rare cases occurs with an acute abdominal picture. However, adnexal torsion with endometrioma is very rare in such patients. A 38-year-old female patient was admitted to the outpatient clinic of the maternity department urgently with intermittent left groin pain lasting for four hours. The patient had intermittent pain and concomitant vomiting. Biochemical examination was evaluated within normal ranges. There were no abnormalities in the vital signs of the case. Ultrasound (US) imaging revealed endometrioma and heterogeneous images in the left adnexal space. Considering endometrioma torsion, emergency surgery was decided, and laparoscopy was planned. During the operation, endometrioma and tubal-ovarian torsion were observed in the left ovary. During the operation, it was detorsioned. Endometrioma cyst excision was performed. The patient was discharged from the hospital in good health. Torsion is very rare in cases with endometrioma. Endometriomas are often tightly adherent to neighboring structures, and therefore seem less likely to cause adnexal torsion, although there is insufficient data on the torsion rate in the cysts. With this case report, we considered contributing to the literature on torsion in endometrioma. It should be kept in mind that patients with endometrioma have tubal-ovarian torsion, which can be seen very rarely if they present to the emergency department with an acute abdomen. Background/Aims: To evaluate in which cases Retrograde Intrarenal Surgery (RIRS) should be combined with other techniques in pediatric patients undergoing retrograde intraluminal endoscopic surgery, RIRS combined with extracorporeal shock wave lithotripsy, and RIRS combined with open/laparoscopic surgery for urolithiasis in our clinic. Materials and Methods: This study conducted a retrospective analysis of data from 302 pediatric patients undergoing RIRS, this technique in conjunction with extracorporeal shock wave lithotripsy (ESWL), open stone surgery, and laparoscopic stone surgery for urolithiasis at our clinic from January 2013 to October 2020. The patient's demographic data, the position, and size of the stones discovered using imaging techniques, surgical interventions for stone extraction, stone-free results, and the condition of postoperative hydronephrosis were evaluated. Results: A total of 302 pediatric patients were included in this study. The mean age was 89.719 months (SD 51.447, range 6-216). 160 patients (53%) were male, whereas 142 patients (47%) were female. The mean size of the treated stones was 7.964 mm (SD 3.516, range 2.2-25 mm). Stone-free rate was achieved in 262 (86.75%) of the patients during surgical follow-up. Imaging indicated the absence of stones in 235 participants (77.81%). In the follow-up, it was shown that hydronephrosis completely resolved in 278 (92.05%) patients. Conclusion: Choosing minimally invasive surgical procedures is advisable, particularly for pediatric patients. Aim: This study aimed to investigate the impact of education on nurses’ compliance with standardization and isolation precautions. Methods: The study employs a pre-test post-test design. The study was conducted between September 2022 and September 2023 with 100 nurses in the intensive care and isolation units of a public hospital where patients with confirmed cases of SARS-CoV-2 infection were admitted. Data were collected utilising the Standard Precautions Compliance Scale and the Isolation Precautions Compliance Scale. The compliance of nurses with standard and isolation methods was examined in both time periods. Results: The mean total score of the Compliance to Isolation Measures Scale was higher after the education (T1=55.39±7.39, T2=77.50±9.68; t=8.372, p<0.001). The mean total score of the Compliance to Standard Precautions Scale was higher after the education (T1=10.38±2.46, T2=14.43±4.29; t=6.298, p<0.001) Conclusion: Education on the importance of using personal protective equipment, isolation, and compliance with standard precautions led to an increase in the variety and usage rates of nurses’ protective equipment. Objective: Laparoscopic Total Extraperitoneal (TEP) hernia repair is frequently performed around the world. Ourgoal here was to evaluate preoperative radiologic findings, intraoperative and postoperative conditions of cases, together with their demographic characteristics, in unilateral, bilateral primary and recurrent cases undergoing hernia operation. Methods: Between 2011 and 2021, 512 patients were operated for hernia with laparoscopic TEP method. Age, gender, BMI, hernia type, radiologic or peroperative type of hernia, defect size measured during surgery, hernia side, content of hernia, postoperative complications(seroma) of these patients were evaluated retrospectively. Additionally to the expressive statistical procedures (mean, standard deviation) in interpriting and calculating the input, the allocations of the variables were analyzed by the Shapiro-Wilk normality test. Also the independent t test, chi-square test, and logistic regression analysis used. The outcome spicked up were analyzed with p<0.05 meaningful level. Results: Of 512 cases, 484 (94.53%) underwent unilateral laparoscopic TEP hernia repair and 28 (5.47%) underwent bilateral laparoscopic TEP hernia repair. 467 of the cases were primary and 45 were recurrent. The average ages was47.28±12.41 years. 321 were men (62.70%) and 191 were women (37.30%). The differentiation related to allocation of BMI of Recurrence(-) and Recurrence(+) groups was meaningful (p=0.019). The distinction absolutely was not remarkable between the side allocation of the Recurrence(-) with Recurrence(+) groups(p = 0.217). The presence of indirect hernia (ID) was mostly seen in the Recurrence(+) group(p=0.014). The mean defect size(mm) of the Recurrence(+) group was measured statistically more than the Recurrence(-) group (p=0.015). Aim: We aimed to compare the surgical outcomes between manual free vaginal morcellation and no vaginal morcellation for uterine extraction during total laparoscopic hysterectomy (TLH) performed in a tertiary center. Materials and Methods: A total of 397 TLH either with or without oophorectomy, for a benign indication, surgeries performed at our tertiary hospital between March 2022 and May 2024 were analyzed in our study. Patients who underwent manual free vaginal morcellation were defined as Group I (n=85, 21.4%) and patients who did not undergo vaginal morcellation were defined as Group II (n=312, 78.6%). Demographic characteristics, laboratory values, operation characteristics and histopathological reports were accessed from the hospital database. Results: The median uterine weight of the morcellation group was greater. Median morcellation time was 9 minutes. While the median operation time of Group I was 80 minutes, it was 70 minutes for Group II (p<.01). The groups were similar with regard to postoperative urinary tract, intraoperative urinary tract and intraoperative bowel injuries. Intraoperative vaginal or perineal laceration was detected at a greater rate in the morcellation group. Conclusion: For large uteruses, TLH is still the best option for suitable patients when performed by experienced surgeons. It has been revealed that manual free vaginal morcellation, performed without the need for additional incisions, is safe, low-cost and practical in total laparoscopic hysterectomies with benign indications. Aim: This study aims to analyze the experience of a tertiary health center about the complications of hysterectomy within a period of five years. Materials and Methods: This is a retrospective review of 1311 hysterectomies which were performed for both benign and malignant indications between 1 January 2017 and 1 January 2023. Results: Complications occurred in 218 patients (16.6%) and the mortality rate was 0.5% (n=6). The rate of conversion into laparotomy was 2.8%. Bladder injury, colon injury, ileus and incisional hernia were significantly more frequent in women who underwent laparotomy (p=0.007, p=0.026, p=0.027 and p=0.010 respectively). Logistic regression analysis indicated hysterectomy technique as an independent prognostic factor for its complications (p=0.001). Ureter injury, bladder injury and colon injury were significantly less frequent in patients who had hysterectomy for gynecological malignancy (p=0.001, p=0.001 and p=0.015 respectively). Hospital stay was significantly longer in patients undergoing hysterectomy for laparotomy and gynecological malignancy (p=0.001 for both). Conclusion: Minimally invasive techniques such as laparoscopy or vaginal approach should be attempted whenever it is possible and feasible. Performing hysterectomy for benign pathologies should not be considered as a distraction from meticulous work during the preoperative preparation and postoperative monitorization periods. Objective: To describe the characteristics of pregnant women diagnosed with adnexal masses and review our experience with diagnosis, management and treatment modalities in this area for which there are no definitive treatment guidelines. Methods: All women with adnexal masses during pregnancy whose management and follow-up was completed at a large tertiary referral center between January 2018 and September 2023 were retrospectively evaluated. After applying the exclusion criteria, a total of 19 women who underwent antepartum laparoscopic adnexal surgery (group 1) and 34 women who underwent expectant management (group 2) were included. Results: Gestational age at birth, cesarean section rates, birthweight and APGAR-5 scores were similar, while the largest mass diameter and CA-125 levels were significantly higher in group 1 (p<0.001 and p=0.014, respectively). Of the patients in group 1, 15 (78.9%) had unilateral complex cysts and 4 (21.1%) had unilateral simple cysts, while there were no bilateral masses. Emergency laparoscopy was performed in 6 (31.6%) of patients for adnexal torsion and in 7 (36.8%) for cyst rupture, while elective laparoscopy was performed in 6 (31.6%) asymptomatic patients with suspected malignancy. While functional cyst was the most common histopathologic finding, malignancy was found in 2 cases (granulosa cell tumor and endometrioid adenocarcinoma). Preterm births occurred in 8 (41.1%) women, of whom 5 (26.3%) gave birth before 34 weeks' gestation and 3 (15.7%) between 34-36 weeks and 6 days. Conclusion: Parallel to the increase in ultrasound examinations in early pregnancy, both the overdiagnosis and early detection of adnexal masses during pregnancy have increased. Given the high rate of spontaneous regression in most lesions and the fact that bleeding in ruptured cases is usually self-limiting, an expectant approach would be appropriate in cases without hemodynamic instability, acute abdomen and/or suspected malignancy. Introduction: Heterotopic pregnancy is a rare condition accompanied by intrauterine and ectopic pregnancy. Case: Intrauterine pregnancy and a suspicious ectopic focus were observed in a thirty-seven-yearold patient with no risk factors who presented with menstrual delay. The ectopic pregnancy was successfully removed by surgery to preserve the uterine pregnancy. Discussion: This case report discusses the importance of early diagnosis and management of heterotopic pregnancy to avoid both fetal and maternal morbidity and mortality. Conclusion: It is important to evaluate the adnexia along with the uterus in determining pregnancy in patients presenting with menstrual delay. Background: Acute appendicitis is the most common nonobstetric cause of acute abdomen in pregnant women. In this study, we aimed to evaluate pregnant patients who were operated on for acute appendicitis in our center, which has a large pregnant population, in the light of the literature. Materials and Methods: The data of patients operated on for acute appendicitis between November 2015 and August 2022 were retrospectively examined and recorded electronically. The patients' complaints, physical examination findings, radiological data, operation type, and pathological data were recorded and examined in the light of the literature. Results: The average age of 37 patients who underwent appendectomy was 27.7 (20-42) years. In these patients, acute appendicitis was diagnosed most frequently in the 2nd trimester (48.6%). The most common presenting complaint of the patients was abdominal pain, and the most common physical examination finding was abdominal tenderness. Considering the admission time of the patients, it was observed that a delay of more than 24 hours increased the risk of perforation. Although the most common histopathological finding was acute appendicitis, malignancy was detected in one patient and enterobius vermicularis was detected in another. Conclusions: Acute appendicitis should be kept in mind in patients with new-onset right lower quadrant abdominal pain during pregnancy. This article discusses a rare case of primary pancreatic hydatid disease, a parasitic infection. The diagnosis of pancreatic hydatid cysts can be challenging, and surgical skill is required to prevent the spread of cyst contents during excision. The case presented involves a 19-year-old male patient with recurrent abdominal pain in the epigastric and left upper quadrant. Imaging and laboratory tests confirmed the diagnosis of hydatid disease, and the patient underwent laparoscopic distal pancreatectomy. It is important to consider primary pancreatic hydatid disease as a differential diagnosis in patients with newly formed pancreatic cysts, especially in endemic areas. Objective: Iatrogenic ureteral injuries are critical consequences of pelvic and vaginal surgeries that can lead to severe morbidity and even death. Therefore, the purpose of this study was to evaluate the outcomes of primary laparoscopic repair of ureteral injuries during laparoscopic hysterectomy (LH). Methods: Patients who underwent LH between November 2018 and October 2021 were evaluated retrospectively. Medical records of all patients with ureter injury were reviewed and their causes of injury, incidence, treatment and follow-up were evaluated. Results: A total of five patients had ureter injury during laparoscopic surgery. All ureter injuries immediately repaired during the same operation without conversion to laparotomy or additional trochar insertion. Conclusion: Identification of intra-operative urinary tract injury during gynecological operations allows for appropriate and immediate repair. The result of the study showed that gynecologists experienced in endoscopic surgery can successfully and effectively managed ureteral injury during laparoscopy. [ABSTRACT FROM AUTHOR]

A 29-year-old primigravid pregnant women was admitted to our tertiary center with complaints of abdominal pain, nausea, and vomiting at the 29 weeks of gestation. Ultrasonography revealed a 6 cm diameter cystic mass in the right adnexa, and there was a high suspicion of adnexal torsion. On magnetic resonance imaging, the right ovary appeared normal and there was a 6 cm cystic mass adjacent to the right ovary, and the findings supported isolated fallopian tube torsion. Emergency laparoscopy revealed that the right fallopian tube and paratubal cyst were torsioned by 720°, and the fallopian tube was necrotic. This torsion did not include the ovary. Since the color tone of the fallopian tube did not recover after detorsion, the right salpingectomy was performed. After the patient was discharged, routine obstetric follow-ups were continued and 3375 g male infant was delivered by cesarean section at 39 weeks of gestation. [ABSTRACT FROM AUTHOR]

Abstract Objectives: To present the data generated at our hospital by comparing the operative characteristics and surgical results of patients who applied to the clinic and emergency room due to ovarian cyst and underwent laparoscopy or laparotomy. Materials and Methods: In this retrospective study, patients who underwent cystectomy, oophorectomy, and hysterectomy salpingo-oophorectomy due to ovarian cysts were compared in two groups, comprising laparoscopy and laparotomy. Evaluated retrospectively in this study were 443 patients operated on due to benign ovarian cyst diagnosis. Data in the patient files were analyzed in terms of age, cyst size, postoperative hemoglobin, postoperative white blood cell count, operating time, hospital stay, and surgical site infection. Results: Postoperative surgical site infection was significantly higher in the laparotomy group. The risk of surgical site infection was RR= 4. 5 (1.74–11.67) times higher in those who underwent laparotomy when compared to laparoscopy. The duration of hospital stay was lower in the laparoscopy group for all operation types (oophorectomy, cystectomy, and hysterectomy salpingo-oophorectomy). The cyst sizes of the patients who underwent hysterectomy salpingo-oophorectomy were significantly more significant in the laparotomy group. The duration of hospital stay in the patients who underwent hysterectomy salpingo-oophorectomy was significantly longer when compared to the laparoscopy group, while no significant difference was found in the oophorectomy and cystectomy patients. The need for blood transfusion was significantly lower in the laparoscopy group for all operation types. Conclusions: It was concluded that the duration of hospital stay, surgical site infection, need for blood transfusion, and operating time was less in patients who underwent laparoscopy. Laparoscopic surgery methods can be safely recommended for rapid and effective treatment of benign ovarian cysts with cystectomy, oophorectomy, and laparoscopic hysterectomy about hospital stay and complications.

Infertility, defined as failure to achieve pregnancy within 12 months of unprotected intercourse or therapeutic donor insemination in women younger than 35 years or within 6 months in women older than 35 years, affects up to 15% of couples. An infertility evaluation may be offered to any patient who by definition has infertility or is at high risk of infertility. Hysterosalpingography, hysterosalpingo-contrast sonography, saline infusion sonohysterography, hysteroscopy, laparoscopy, and bacteriological and endocrinological examinations that will be made after these studies aim to focus more on the causes of infertility. With the development of assisted reproductive technology, the need for major reproductive surgery, which may be necessary for the primary treatment of infertility, has decreased over the years. Surgical methods are mainly considered as laparoscopic and hysteroscopic techniques. However, laparotomy is also rarely required and may be needed in cases such as adhesions, mass lesions, unsuccessful surgeries, or emergency surgery. When a surgical treatment is planned for infertile patients who required surgery, it is very crucial that the procedure should be performed by experienced surgeons. In this review, pathologies that require surgery for infertility treatment and surgical methods that can be applied to infertile patients were discussed, rather than medical treatments.

Purpose: Every procedure, no matter how difficult, has a learning curve. However, no one discusses the learning curve for simple operations. This idea constantly comes up when there is a demanding surgical method that challenges habits. The aim of this study is to use cumulative sum analysis to estimate the learning curve of the demanding laparoscopic complete mesocolic excision+central vascular ligation technique for right sided colon cancer. Materials and Methods: This retrospective study involved patients older than 18 years and with right-sided colon cancer admitted to Ankara University School of Medicine, Department of General Surgery between January 2015 and June 2022. The cumulative sum (CUSUM) chart based on operation duration and quantity of lymph nodes harvested was utilized to determine the learning curve. Results: 53 patients included in this study. CUSUM analysis based on operation duration showed that Surgeon had passed the learning phase and was considered proficient after 21 cases. And the CUSUM analysis based on the lymph nodes harvested revealed that surgeon gained proficiency in 25th case. In terms of demographic, oncological, and operative data, there was no difference between the the groups defined by the phases according to the CUSUM analysis based on both operation duration and lymph node count. Conclusion: Surgeons can become familiar with laparoscopic complete mesocolic excision for right sided colon cancer and perform it after approximately 21 to 25 operations.

Background: The aim of our study is to compare laparoscopic and abdominal myomectomies performed in our clinic and to evaluate the feasibility and safety of laparoscopic myomectomies. Materials and Methods: In this study, we retrospectively evaluated 314 cases who were threated with myomectomy operation in our clinic between 2012-2022. All of the cases were divided into two groups with respect to the surgical procedure performed. 314 patients were in the abdominal myomectomy group, 47 patients were in the laparoscopic myomectomy group and the groups were compared. Results: There was no statistically significant difference between the groups in terms of demographic data (p>0.05). When the groups were evaluated in terms of postoperative white blood cell(WBC), hemoglobin, hematocrit, platelet values, operation time, hospitalisation time, postoperative blood transfusion and perioperative complications, no statistically significant difference was observed (p>0.05). There were no statistically significant difference in terms of preoperative WBC and CRP values between the groups, but postoperative WBC and CRP values were lower in the laparoscopic myomectomy group (p<0.05). Conclusions: Laparoscopic myomectomy has a short recovery time and aesthetic advantage. However, equipment and the need of experienced surgeons limit its applicability. With the development of laparoscopic technologies and increasing the experience of surgeons with laparoscopic surgery, it will provide a serious advantage over the abdominal method in terms of less blood loss, less hospital stay, shorter operation times, less trauma and better cosmetic results. Objective: In laparoscopic distal colon surgery, diverting ileostomy is often used to improve the anastomosis leakage and eliminate the need for secondary surgery. However, complications related to the stoma and its closure have led to new searches. The rectal tube has started to be used as another method to reduce the anastomosis leakage by reducing intestinal pressure and providing drainage. In this study, we aimed to compare the effect of the rectal tube and diverting ileostomy on the outcomes of patients who underwent laparoscopic pelvic subperitoneal surgery. Methods: Retrospective information was obtained from 66 patients who underwent laparoscopic pelvic subperitoneal surgery in our clinic between 2013 and 2019. The patients were evaluated in two groups: ileostomy and rectal tube. Demographic data, operation findings, pathological results, and follow-up information were evaluated. Results: Fourty-one of 66 patients who underwent colorectal pelvic peritoneal surgery were in the rectal tube group and 25 were in the ileostomy group. The majority of the patients in the ileostomy group were males (22-88% vs 23-56%, p=0.007); however, there was no difference between the two groups in terms of age, body mass index, comorbidity, and the previous abdominal operation. In the rectal tube group, the number of patients who were diagnosed as having rectal tumors and consequently underwent low anterior resection was higher. In the ileostomy group, the operation time was longer (476±130 vs. 341±114 mins, p=0.0001) and amount of peroperative bleeding was higher (261±260 vs 128±98 mL, p=0.02). Intraoperative and postoperative complications of the patients were similar in both groups. The pain scale on the postoperative second day was lower in the rectal tube group (2.8±1.5 vs. 4.7±2.9, p=0.008). Although the pain scales on day 1 and day 3 were lower in the rectal tube group, there was no statistically significant difference between the two groups. The oncological results were similar during the average follow-up period of 28.3±18.5 months. The cosmetic scores of the patients were better in the rectal tube group (9.3±0.9 vs. 7.2±1.9, p=0.001). Conclusion: In laparoscopic pelvic subperitoneal colorectal surgeries; it has been observed that the results of the rectal tube are not worse than ileostomy, it can be preferred in appropriate cases and reduce stoma-related problems. Aim: Endometrial carcinoma is the most frequent gynecologic malignancy in Turkey, among United States and Europe. Life time risk of a patient to have endometrial carcinoma is 3-4% Endometrial carcinoma is staged and treated the mostly preferred surgical method is open surgery. Aim of this study is to compare open surgery and laparoscopic surgery for treatment and surgical staging of endometrial carcinoma. Materials and Method: We included a total number of 57 patients with the diagnosis of endometrial carcinoma, including 29 cases operated with laparoscopic surgery and 28 cases operated with open surgery. In this study, we compared the groups in terms of age, body mass index, perioperative and postoperative complications, total lymph node number, operation time, length of hospital stay, pre and post-operative hemoglobin values. Results: There were no significant differences between the two groups among demographic and anthropometric measures. Parity was higher among the open surgery arm and this was statistically significant. There were no significant differences between the two groups in terms of operation time, blood loss, pre and post-operative hemoglobin values, intra and post operative complications. Length of hospital stay was significantly shorter in the laparoscopically managed cases than the open surgery arm. However, total number of lymph nodes dissected were significantly higher in the open surgery group. Conclusion: Laparoscopic surgery is a reliable and a feasible surgical technique in cases with the diagnosis of endometrial carcinoma. Shorter stay at hospital seems to be advantageous in teims of quality of life. Despite that, laparoscopy seems to be disadvantageous for cases with high stage, high grade, agressive type disease, because of lower nimbers of dissected lymph nodes. [ABSTRACT FROM AUTHOR]

Objective: Laparoscopic colorectal surgery has become a widely used method in many centers today. In our study, it was aimed to compare the postoperative clinical and oncological results in patients who underwent laparoscopic and open colorectal surgery and to compare them with the literature. Material and Method: The data of patients who underwent colorectal surgery between January 2012 and December 2018 at the Dışkapı Yıldırım Beyazıt Training and Research Hospital General Surgery clinic were analyzed retrospectively. Results: A total of 544 patients were included in our study, of which 438 (80.5%) underwent open resection, 73 (13.4%) laparoscopic, 33 (6.1%) underwent resection with conversion. In the series, the number of cases that were operated urgently was 158 (29.0%), and the number of elective cases was 386 (71.0%). A total of 156 (28.7%) postoperative complications were detected. When evaluated according to the groups; There were 20 (27.4%) postoperative complications in the laparoscopic surgery group, 126 (28.8%) in the open surgery group, and 10 (30.3%) in the conversion group. The mean number of lymph nodes removed in rectal cancers is 19.0±9.7 in the open procedure and 22.3±11.6 in the laparoscopic procedure (p: 0.198). A positive circumferential surgical margin was found in 1 (2.3%) and 8 (5.9%) patients with rectal cancer in the laparoscopic and open surgery groups, respectively (p: 0.456). The mean distal surgical margin was 45.1±39.8 mm in the open group and 41.1±37.7 mm in the laparoscopic group (p: 0.608). Conclusion: Laparoscopic colorectal cancer surgery has comparable clinical and oncological outcomes to the open procedure. Given the many known advantages of minimally invasive surgery, laparoscopic surgery will likely become the standard treatment for colon cancer. [ABSTRACT FROM AUTHOR]

Purpose: The aim of this study is to evaluate the incidence of pathologies causing biliary stasis together with xanthogranulomatous cholecystitis (XGC) and its effect on conversion to open cholecystectomy. Materials and Methods: This retrospective study reviewed the medical records of 11840 patients who underwent cholecystectomy at Seyhan State Hospital between January 2010 and June 2020. After examining the medical records, 40 (0.33%) of 11840 cholecystectomy cases were diagnosed as XGC pathologically. Results: In the last decade, 40 (0.33%) patients out of a total of 11840 patients who underwent cholecystectomy were diagnosed with XGC. The mean age of the cases was 58 (34-88). In 26 (65%) of 40 patients, surgery was completed laparoscopically. Open surgery was performed in 5 (12.5%) of them. Nine (22.5%) patients were converted from laparoscopic cholecystectomy to open surgery. In all cholecystectomy cases, the conversion cholecystectomy(CC) rate was 412/11840 (3.4%). Endoscopic retrograde cholangiopancreatography(ERCP) was performed in 13 (32.5%) patients for diagnosis and therapeutic purposes related to bile ducts. The gallbladder wall's average thickness was found to be 8.0 millimeters (± 3.87) on histopathological examination. 38 (95%) patients had an increase in gall bladder wall thickness of 4 millimeters or more. CC rate was found to be high in the subgroup analysis of pathology causing bile stasis and in patients who underwent emergency cholecystectomy. Conclusion: This study demonstrates that the pathologies causing bile stasis are observed more frequently in XGC patients than other cholecystitis patients, suggesting that these pathologies may be associated with XGC.

Objective:Postoperative pulmonary complications (PPC) are one of the most important causes of postoperative morbidity and mortality. The ARISCAT Risk Index is a seven-variable regression model that classifies patients as low, intermediate, and high risk and is used to estimate the overall incidence of postoperative pulmonary complications. In our study, we aimed to evaluate the effectiveness of ASA and ARISCAT index in predicting pulmonary complications in after laparoscopic abdominal cancer surgery. Materials and Method:After the approval of Hospital Ethics Committee, 100 patients scheduled to have major laparoscopic abdominal cancer surgery were included in the study. Preoperative ASA and ARISCAT score, comorbidities, intraoperative fluid amount, ventilation parameters, pneumoperitoneum pressure and duration of surgery were recorded. Patients were followed up for pulmonary complications until discharge from hospital. The length of hospital stay was recorded. Results:Pulmonary complications developed in 11 patients postoperatively. PPC incidence was found similiar within ASA and ARISCAT risk groups (p> 0.05). There was no significant correlation between ASA and ARISCAT risk index and PPC (p= 0.23 and p= 0.89, respectively). The number of fluids administered intraoperatively was significantly higher in patients who developed PPC (p= 0.018). There was a significant correlation between sugical site and PPC development (p= 0.012). The duration of surgery and length of hospital stay was longer in patients with PPC (pË,0.05). Conclusion:We found that both ASA score and ARISCAT risk index are not sufficient to predict postoperative pulmonary complications in laparoscopic major abdominal cancer surgery, and the surgical site, long operation time and the volume of fluid used intraoperatively are important independent risk factors for pulmonary complications. Objective: Today, laparoscopic hernia repairs are on the way to becoming the gold standard. However, the cost of dissection balloon and mesh fixer in laparoscopic total extraperitoneal (TEP) inguinal hernia repairs is an obstacle to performing these surgeries. The aim of this study is to examine the feasibility of these surgeries with standard laparoscopic instruments without using costly materials with surgical experience. Methods: Patients who underwent laparoscopic TEP inguinal hernia repair without the use of a dissection balloon and mesh stabilizer between March 2019 and April 2021 were retrospectively reviewed. Age, gender, intraoperative and postoperative complications, operation time, hospitalization time, return to daily activities, recurrence parameters were evaluated. Results: The study was conducted on the results of TEP repair applied to 220 hernia patients, 86 right, 76 left, 44 bilateral. The mean age is 52.55±18.02 years. 186 of them are men and 34 of them are women. The mean time to return to daily activities was 3.98±1.68 days, and the mean operative time was 39.55±11.32 minutes. The mean hospital stay was 0.78±0.16 days. Intraoperative complications were observed in 23 of the cases and seroma in 5%. Recurrence was observed in 3 patients at the 6th month follow-up. Conclusion: Laparoscopic TEP inguinal hernia repair without dissection balloon and mesh stabilizer; Compared to other laparoscopic repairs in the literature, it is recommended to be used widely because of the complication rates, hospitalization and return to daily activities are similar, and it is a less costly method. Abstract Background: The COVID-19 pandemic has significantly affected healthcare service systems and personnel worldwide. Important decisions have been made such as surgical activity changes, recommendations to cancel elective surgery, and shifting surgeons to other medical activities. However, following the collection of new data and acceptance that this pandemic would last for at least a few years, firstly elective cases and then laparoscopic operations were started again. While it was reported that the COVID-19 virus was detected in the abdominal cavity in studies published in the form of series of a few cases, there is no clear information about the virulence of this detected virus yet. In guidelines published as recommendations by surgical associations, it is stated that it could be performed by taking the necessary precautions, but open surgery needs to be preferred in positive cases. We aimed in this study to show minimally invasive surgery is safe with all the precautions made preoperatively for both patients and surgical crew. Material and Method: We retrospectively examined the laparoscopic cases included between August 2020 and December 2020 at the General Surgery Clinic of the Hitit University Erol Olçok Research and Training Hospital from the hospital’s information system. We collected the ages of the found cases, their preoperative COVID diagnostic tests, and their COVID polyclinic visitation data in their postoperative follow-up. We also examined whether or not the surgical team involved in the surgeries of these patients was diagnosed with COVID-19 within 2 weeks following these surgeries. Results: A total of 124 laparoscopic cases were included at our clinic between August 2020 and December 2020. Most of these cases (68.5%) were laparoscopic cholecystectomy cases. The surgical team comprised the surgical nurse and operating room personnel under the leadership of the surgeon. Based on verbal questioning, it was determined that no person from the team involved in these surgeries was infected with SARS-CoV-2. Conclusion: In our study, as a result of our application of all these recommendations in the literature, we did not encounter any contamination in the 124 laparoscopic surgery cases we examined. In conclusion, we think that laparoscopic surgery may be performed safely with taking the necessary precautions during the COVID-19 pandemic period.
