Background: Elderly individuals over 65, along with children under 5, are the most affected by invasive pneumococcal diseases (IPDs). Monitoring vaccination coverage and conducting surveillance are essential for guiding evidence-based prevention campaigns and public health measures. Methods: Since 2007, the Veneto Region has relied on three sources for surveillance of invasive bacterial infections, contributing to an increase in reported IPD cases. This study analyzed notifications related to individuals aged ≥65 years from 2007 to 2023. Results: A total of 1527 cases of IPDs in elderly individuals were reported between 2007 and 2023. The notification rate significantly increased from 5.61 to 14.63 per 100,000 inhabitants, despite underreporting during the COVID-19 pandemic. Cases associated with sepsis increased from 3.89 to 9.58 per 100,000, while notifications of meningitis and case fatality rates remained stable at 1.5 per 100,000 and 11.8%, respectively. Serotyping was not performed in 52% of the notifications. The most common serotypes were 3 (21.6%), 8 (11.1%), and 19A (5.0%), with fluctuations over time. There was a significant decline in serotypes covered by PCV7 and PCV13 and an increase in non-vaccine serotypes. Conclusions: The regional surveillance system allows for an increasingly comprehensive profile of the epidemiological landscape of IPDs in Veneto. However, the surveillance of pneumococcal infections still presents challenges. The currently available data are likely to be underestimated, mainly referring to the most severe cases, and the serotyping necessary to identify the etiological agent is still not often performed. Objective: To determine the antimicrobial susceptibility and serotypes of Neisseria (N.) meningitidis and Streptococcus (S.) pneumoniae in Sri Lankan patients. Methods: We retrospectively analyzed 11 blood culture specimens from suspected patients with invasive meningococcal disease and 26 S. pneumoniae clinical isolates. We tested 6 antimicrobials against N. meningitidis and 12 antimicrobials against S. pneumoniae. Meningococcal serogroup was determined by realtime PCR and Quellung serotyping was used for pneumococcal analysis. Results: N. meningitidis serogroup B was the most common in this study. Intermediate-susceptibility to penicillin was seen in 75.0% (6/8) of strains. Susceptibility to ciprofloxacin, levofloxacin and cotrimoxazole was 62.5% (5/8), 62.5% (5/8) and 87.5% (7/8), respectively. Excellent susceptibility was seen in cefotaxime and meropenem. In S. pneumoniae, the most common serotype was 19F in both invasive and non-invasive pneumococcal diseases. The majority of strains showed multidrug resistance. Penicillin non-susceptibility in non-meningeal strains were 13.6% and all meningeal strains were penicillin resistant. Erythromycin was highly resistant in both groups. Amoxicillin showed excellent susceptibility in non-invasive pneumococcal diseases strains. Linezolid, levofloxacin and vancomycin showed 100.0% susceptibility in all pneumococcal isolates. Conclusions: Implementation of vaccines should be considered, especially for children and high-risk populations. This may contribute to reducing pneumococcal and meningococcal invasive disease burden and help prevent emergence of antimicrobial resistant strains. [ABSTRACT FROM AUTHOR]

There is difference in the pattern and aetiology of IBD according to the geographical variation and the antimicrobial susceptibility of causative organisms are also variable. There is lack of data on patterns of IBD from central India. Aim: To generate the data on the burden of pneumonia, meningitis and sepsis among under five children in central India and the microorganisms associated with them. Materials and Methods: This prospective observational study was conducted from August 2019 to July 2020 at the Department of Paediatrics at Mahatma Gandhi Memorial Medical College and MY Hospital, indore, India on children of 1-59 months of age admitted with pneumonia, meningitis and sepsis. A detailed history was recorded for all the participants and thorough physical examination was done along with chest X-ray , routine laboratory examination and blood cultures were performed. Latex Agglutination Test (LAT) and Antibiotic Susceptibility Testing (AST) were performed on Cerebrospinal Fluid (CSF) samples. All the data collected were recorded in Microsoft (MS) excel sheet and frequency (n), percentages (%) and chi-square test for significance level, were calculated to analyse the collected data. Results: Of the total 450 cases enrolled, 283 (62.8%) had confirmed IBD. Out of the confirmed cases, 183 (64.6%) had pneumonia, 52 (18.3%) had meningitis and 48 (16.9%) had sepsis. Highest case fatality rate was seen with sepsis followed by meningitis and pneumonia. The most common organism isolated was Staphylococcus aureus. Overall, gram negative bacilli were found to be more resistant to the conventional antibiotics than gram positive organisms. An 40 (83%) of Staphylococcus aureus were susceptible to methicillin whereas only 12 (45%) of Klebsiella pneumoniae were susceptible to amikacin. Conclusion: The present study highlights the predominance of staphylococcus aureus and adverse outcomes of IBD in children with Severe Acute Malnutrition (SAM) and incomplete immunisation. Emergence of resistant bacterial strains to conventional antibiotics warrants the need to consider the locally prevailing antimicrobial susceptibility patterns for the effective management of these diseases.

Abstract Background Invasive bacterial diseases (IBD) are a serious cause of hospitalization, sequelae and mortality. Albeit a low incidence, an increase in cases due to H. influenzae was registered in the past 4 years and, in the Tuscany region, an excess of cases due to N. meningitidis since 2015 is alarming. The purpose of this study is to deepen the knowledge of IBD epidemiology in Tuscany with particular attention to temporal trends. Methods Tuscan residents hospitalized for IBD from January 1st 2000 to March 18th 2016 were selected from the regional hospital discharge database based on ICD-9-CM codes. Age-specific and standardized hospitalization rates were calculated together with case-fatality rates (CFRs). A time-trend analysis was performed; whereas, prognostic factors of death were investigated through univariable and multivariable analyses. Results The average standardized hospitalization rates for invasive meningococcal diseases (IMD), invasive pneumococcal diseases and invasive diseases due to H. influenzae from 2000 to 2016 were 0.6, 1.8, and 0.2 per 100,000, respectively. The average CFRs were 10.5%, 14.5% and 11.5% respectively with higher values in the elderly. Older age was significantly associated with higher risk of death from all IBD. A significant reduction in hospitalization rates for IMD was observed after meningococcal C conjugate vaccine introduction. The Annual Percentage Change (APC) was -13.5 (95% confidence interval (CI) -22.3; -3.5) in 2005–2013 but has risen since that period. Furthermore, a significant increasing trend of invasive diseases due to H. influenzae was observed from 2005 onwards in children 1–4 years old (APC 13.3; 95% CI 0; 28.3). Conclusions This study confirms changes in the epidemiology of invasive diseases due to H. influenzae and IMD. Furthermore, attention is called to the prevention of IBD in the elderly because of the age group’s significantly higher rate of hospitalizations and deaths for all types of IBD.

Introduction: Haemophilus influenzae type b, Streptococcus pneumoniae, and Neisseria meningitidis were the leading causative agents of invasive bacterial diseases (IBD) among children of less than 5 years of age prior to the introduction of the Haemophilus influenzae type b (Hib) conjugate vaccine. A significant decrease of invasive Haemophilus influenzae diseases (IHD) occurred in nations that implemented routine Hib vaccination in their national immunization program (NIP). Aim: The goal of this study was to describe the frequency and characteristics of pediatric patients with IBD before and after routine Hib vaccination was implemented into the Croatian national immunization program (NIP). Subjects and methods: A retrospective analysis of patient records from administrative databases for patients aged 0 to 18 with etiologically proven IBD at University Hospital for Infectious Diseases "Dr. Fran Mihaljević". Results: A total of 1.112 pediatric patients were analyzed. The patients were divided into three periods: one prevaccinal (reference) and two postvaccinal periods. During the second period, a statistically significant change in proportion occurred for Neisseria meningitidis (p<0.05), Streptococcus agalactiae (p<0.05) and Haemophilus influenzae (p<0.0001). Haemophilus influenzae (p<0.0001) maintained this trend during the third period as well. Additionally, a decrease in Hib meningitis cases was noted (p<0.0001). Conclusion: The implementation of Hib vaccine in the Croatian NIP led to a significant decrease of IHD. Streptococcus pneumoniae and Neisseria meningitidis are still the leading causative agents of IBD, and thus the introduction of adequate pneumococcal and meningococcal vaccine in Croatian NIP should be considered. Introduction. The International Circumpolar Surveillance (ICS) project is a population-based surveillance network. Since 2000, Canada has participated in the ICS Invasive Bacterial Disease Working Group's surveillance of invasive disease due to Haemophilus influenzae (Hi). Methods. A standardized case report form containing demographic and clinical information was completed for all reported Hi cases in the study regions. Isolates were sent to a reference laboratory for confirmation and serotyping. Analysis was conducted on all Hi serotype a (Hia) cases reported from 2000 to 2010. The northern Canadian population was estimated using Statistics Canada information. Results. Of the 130 Hi cases reported from 2000 to 2010, 72 (56% of cases with serotype information) were due to Hia. The number of Hia cases reported each year ranged from 2 in 2008 to 13 in 2010. The average Hia incidence over the 11 years was 4.6 cases per 100,000 population per year. The majority of Hia occurred in infants less than 2 years of age (73% of cases). This age group had an average annual incidence of 87.5 cases per 100,000 population. Among cases for which ethnicity was indicated, 91% of Hia cases reported Aboriginal status with the average incidence being 6.9 cases per 100,000 population per year. The most common clinical presentation was meningitis (reported in 37% of cases), followed by bacteraemia (34%) and pneumonia (27%). More than 90% of cases were hospitalized, and there were 4 deaths, resulting in a case fatality ratio of 5.6%. Conclusion. In the last decade, Hia has become an important cause of morbidity and mortality in the Canadian North. More detailed surveillance information from a national perspective is needed. Summary The Kilifi Health and Demographic Surveillance System (KHDSS), located on the Indian Ocean coast of Kenya, was established in 2000 as a record of births, pregnancies, migration events and deaths and is maintained by 4-monthly household visits. The study area was selected to capture the majority of patients admitted to Kilifi District Hospital. The KHDSS has 260 000 residents and the hospital admits 4400 paediatric patients and 3400 adult patients per year. At the hospital, morbidity events are linked in real time by a computer search of the population register. Linked surveillance was extended to KHDSS vaccine clinics in 2008. KHDSS data have been used to define the incidence of hospital presentation with childhood infectious diseases (e.g. rotavirus diarrhoea, pneumococcal disease), to test the association between genetic risk factors (e.g. thalassaemia and sickle cell disease) and infectious diseases, to define the community prevalence of chronic diseases (e.g. epilepsy), to evaluate access to health care and to calculate the operational effectiveness of major public health interventions (e.g. conjugate Haemophilus influenzae type b vaccine). Rapport with residents is maintained through an active programme of community engagement. A system of collaborative engagement exists for sharing data on survival, morbidity, socio-economic status and vaccine coverage. [ABSTRACT FROM AUTHOR]
