This paper summarizes the practical of medical integration model in the Traditional Chinese Medicine nursing ward round for a patient with lumbar disc herniation. Based on the review of patient’s clinical data, responsible nurse had reported the case analysis, and doctors had comprehensively analyzed the illness condition of the patient. The key nursing contents were discussed and nursing measures were jointly formulated. Through the integration of medical and nursing rounds, the professional complementarity between medical and nursing can be rebuilt, and a mode consisting of medical treatment, nursing care and patient participation can be achieved. (总结1例腰椎间盘突出患者的医护一体化中医护理查房实践经验。分析患者临床资料, 由责任护士汇报病例, 医生分析病情, 医护联合对重点护理内容进行讨论, 协同制定护理措施。通过医护一体化查房, 医护之间专业互补, 重建医、护、患三位一体的治疗和护理模式, 达到实施整体护理的效果。)

Objective To improve the risk assessment and prevention of perioperative venous thromboembolism (VTE) in gynecological patients by using integrated information management system of medical care. Methods A quality management team was established to design the key issues of VTE prevention and formulate a standard procedure of evaluating, alarming and processing. The early warning risk management, evaluated by the structure-process-result system, was established to continuously improve the quality of preventive monitoring and management of VTE. Totally 407 patients with hospital admission between Jan and June 2019 were included as the control group, and another 343 patients with hospital admission between July and December 2019 were recruited as the observation group. The incidence of VTE was compared between two groups. Results The preliminary screening rate in the observation group was 100. 00%(343/343), which was significant higher than 74. 94%(305/407)in the control group (P＜0. 01). The implementation rates of preventive measures for low-risk patients and moderate-to high risk patients were increased significantly after using integrated information management system (P＜0. 01). There were 2(0. 58%) cases of VTE in low-risk patients in the observation group. There were 5(1. 23%) cases of VTE in low-risk patients and 7(1. 72%) cases in moderate-to-high risk patients in the control group. Conclusion The integrated medical care management system can optimize the medical communication channels, promote the standardization and homogenization of the prevention and control system of VTE for gynecological patients during the perioperative period, which can ensure the safety of patients and improve the medical quality. (目的 依托医护一体化信息管理系统, 改善妇科围术期患者静脉血栓栓塞症(VTE)风险评估及预防措施落实。方法 建立科室医护一体化VTE质量管理小组, 设计重点环节节点控制, 形成VTE评估-提醒-处置标准化预防流程, 完善预警风险管理, 以结构-过程-结果为评价体系, 对科室VTE预防监测管理和持续质量改进。以医护一体化信息管理系统在病区实施时间为节点, 将2019年1月—6月住院的407例患者设为对照组, 2019年7月—12月343例患者设为观察组。对比两组VTE发生率。结果 观察组住院患者VTE风险评估初筛率为100. 00%(343/343), 相较于对照组的74. 94%(305/407)上升, 差异有统计学意义(P＜0. 01)。实施医护一体化信息管理后, VTE低危、中危及以上患者预防措施落实率均较实施前提升(P＜0. 01)。观察组中危及以上患者VTE发生2(0. 58%)例; 对照组低危患者VTE发生5(1. 23%)例, 中危及以上患者VTE发生7(1. 72%)例。结论 医护一体化管理系统能够优化医护沟通渠道, 促进妇科围术期患者VTE防控体系标准化和同质化, 保障患者安全, 提升医疗质量。)

To review the clinical data of a patient with myelodysplastic syndrome and summarize the experience of treatment and nursing under the integrated mode of medical care. In the integrated medical and nursing ward rounds, doctors carried out symptomatic diagnosis and treatment, and used bedside physical examination combined with Traditional Chinese medicine. Nurses gave care based on syndrome differentiation, intradermal acupuncture treatment and moxibustion treatment intervention, and improved psychological evaluation and health education. In addition, patients were encouraged to actively cooperate with treatment and care. The implementation of integrated ward rounds can effectively improve the adverse reactions of chemotherapy in patients with myelodysplastic syndrome, improve treatment compliance, promote rehabilitation, and enhance the quality of nursing services. The integrated medical and nursing ward rounds is beneficial for the enhancement of medical care cooperation, sense of participation among nursing workers, and is conducive to the improvement of nursing quality. (本文回顾1例骨髓增生异常综合征患者的临床资料, 总结医护一体化模式下的治疗和护理经验。医护一体化模式下, 医生开展对症诊疗, 采用床旁查体联合中药方剂治疗; 护士实施辨证施护, 给予皮内针治疗及艾条灸治疗干预, 并完善心理评估和健康教育。此外, 鼓励患者积极配合治疗和护理。实施医护一体化查房, 能有效缓解骨髓增生异常综合征患者化疗的不良反应, 提高治疗依从性, 促进康复, 提升护理服务质量。医护一体化查房加强了医护合作, 可提升护理工作者的参与感, 进而有利于护理质量的提升。)

This paper summarized the practice of medical-nursing integrated model and PDCA management in Traditional Chinese Medicine nursing ward rounds for a patient with chronic obstructive pulmonary disease(COPD). An integrated management team consisted of doctors and nurses was established, and PDCA management was adopted in Traditional Chinese Medicine nursing ward rounds for the patient. The aim of implementing medical-nursing integrated model and PDCA management is to provide patients with professional, comprehensive and effective high-quality TCM nursing services through integrated medical and nursing TCM nursing rounds. (本文总结PDCA模式下1例慢性阻塞性肺疾病患者的医护一体化中医护理查房经验。成立医护一体化管理小组, 实施PDCA管理模式, 通过医护一体化中医护理查房, 为患者提供专业、全面、有效的优质中医护理服务。)

The integrated medical and nursing ward rounds contributes greatly to quality improvement of clinical diagnosis and treatment, better meets the needs of patients, and promotes the harmonious development of the doctor-patient relationship. With the continuous development of modern medicine, medical science is becoming increasingly specialized, and increasing attention has being paid to the inter-professional cooperation. The implementation of medical and nursing integrated ward rounds is an important measure to promote medical and nursing cooperation. In order to promote the in-depth development, sustainability and standardization, this article briefly discussed the practice and quality improvement of the integrated medical and nursing ward rounds, and provided a reference basis for its clinical application. This paper reviewed the literature related to the mode of integrated medical and nursing ward rounds, and summarized the concept of integrated medical and nursing ward rounds, the implementation method, the application status in clinical nursing, the problems existing in the implementation process and the countermeasures, so as to provide reference for the improvement and popularization of integrated medical and nursing ward rounds in the clinical setting. (本文对医护一体化查房模式的相关文献进行归纳和总结, 从医护一体化的概念、医护一体化查房模式的实施方法、在临床护理的应用现状、实施过程中存在的问题及应对措施几个方面进行综述, 以期为临床医护一体化查房工作模式的完善和推广使用提供参考依据。)

This paper summarized the integrated traditional Chinese and western medicine nursing based on integration of medical care for pain management in a patient with diabetic foot disease complicated with erysipelas. An integration team of medical care specialists was established for consultation instruction, evaluation, diagnosis, treatment and nursing. Based on the syndrome differentiation, integrative nursing measures included Chinese herbal medicine application, Traditional Chinese Medicine collapse stains, acupuncture and health education before discharge from hospital were carried out to relieve the pain and improve the quality of life of the patient. Training of Traditional Chinese Medicine (TCM) nursing talents fundamentally affects the development of TCM nursing. By theoretical teaching, mentor-guided clinical practice and skills training, the design of master-apprentice system is aimed to multi-dimensionally explore the path to sustainable development of TCM nursing, seek opportunities for development, and improve quality of the TCM nursing technique and service. This paper summarized the current situation in TCM nursing talents training and practice of master-apprentice system, in order to provide reference for training of high quality TCM nursing talents of integration of medical care. To investigate the effect of integration of medical care for pain control of a patient with diabetic foot gangrene complicated with infection. The clinical data of a patient with diabetic foot gangrene complicated with infection were retrospectively analyzed. Based on the integration of medical care, integrated Traditional Chinese and western medicine nursing interventions such as integration nursing ward round, Traditional Chinese Nursing technique, emotion nursing and health education were carried out to relieve the pain caused by diabetic foot gangrene and improve the patients’ quality of life. ObjectiveThe objective of this study was to explore the nursing effect and negative psychological stress response of elderly patients with femoral neck fracture by applying the high-quality nursing mode of medical care.MethodsA total of 130 elderly patients with femoral neck fractures hospitalized in our hospital from January 2020 to June 2021 were randomly divided into the control group and observation group, with 65 patients in each group. The control group adopted the conventional nursing mode, while the observation group adopted the high-quality nursing mode of medical care. The observation indexes selected in this study are nursing satisfaction, hip flexion activity on the 1, 15, and 30 days after the operation, the time when the affected limb was lifted off the bed actively, and the anxiety and depression of patients.ResultsOn the 1, 15, and 30 days after the operation, there were statistically significant differences between the two groups in hip flexion activity and the time when the affected limb was lifted off the bed (P < 0.05). The nursing satisfaction of the observation group was 95.38%, which was statistically significant compared with the 80.00% of the control group (P < 0.05). After treatment, the self rating depression scale (SDS) and self rating anxiety scale (SAS) scores in the observation group were lower than those in the control group (P < 0.05).ConclusionThe high-quality nursing model of medical care can effectively promote the rehabilitation of elderly patients with femoral neck fracture, reduce the negative psychological stress reaction of patients, and improve nursing satisfaction, which has important application value and guiding significance for the nursing of patients with femoral neck fracture.
