Introduction. Physical training is the most important element of the combat readiness of the troops, its goals and objectives are defined by the Manual on Physical Training in the Armed Forces of the Russian Federation. The solution of these tasks should contribute to the formation of military personnel's readiness to endure extreme physical and mental stress during the preparation and conduct of combat operations. Comprehensive provision of physical training in the troops requires the hard work of various services, including the medical service.The purpose of the study. Identify problematic issues of medical support for physical training and sports events in the Armed Forces of the Russian Federation and identify ways to solve them.Materials and methods. The article uses the materials of the work of medical and nursing teams in the medical provision of physical training of units of the Southern Military District from 2016 to 2019. Additionally, the data of an in-depth medical examination of the personnel of some units of the district subordination for 2018 were used.Results and discussion. When analyzing the work of visiting medical and nursing teams during the medical support of passing the standards for physical training, a number of problematic issues were identified. First, there are always military personnel who regularly do not engage in physical training and during the delivery of standards make excessive efforts and work at the limit of their capabilities. Secondly, some exercises are very dangerous, either in terms of injuries or the likelihood of developing complications from the cardiovascular system. All this applies to the medical support of the competition. As an example, the championship of the Armed Forces of the Russian Federation in fire and rescue sports, held from 3 to 6 September 2019 in Rostov-on-Don on the territory of the garrison fire brigade, is given. The program of the competition included passing the obstacle course, climbing the training tower, extinguishing the fire, etc., all very traumatic. Therefore, for the medical support of these competitions, a medical and nursing team of the resuscitation profile was allocated on a reanimobile with specialized stowage, respiratory and oxygen equipment.Conclusion. It is necessary to strictly monitor the correctness of the definition of the group of physical training and the gradual increase in physical activity in order to avoid complications and further progression of diseases or even deaths in military personnel. It is impossible to approach formally the conduct of an in-depth medical examination of military personnel.

Введение. Актуальность проблемы обусловлена тем, что по данным информационного ресурса COVID-19, число пациентов, перенесших коронавирусную инфекцию, составило 6 161 835 человек за период с января по декабрь 2020 г., из них: средняя степень тяжести – 480 692 человека, тяжелая степень тяжести – 110 178 человек, крайне тяжелая – 78 669 человек, легкая и др. состояния – 5 492 296. В настоящее время по данным Всемирной организации здравоохранения (ВОЗ) осложнениями после перенесенной ковидной инфекции страдают не менее 10% переболевших людей [1]. По данным ВОЗ, не менее 10% людей, переболевших COVID-19, страдают постковидным синдромом. Около 40% из них жалуются на ощущение нехватки воздуха, одышку. У 17% сохраняется кашель. Каждый десятый испытывает боли в суставах и мышцах. Также пациентов может беспокоить учащенное сердцебиение. Каждый пятый жалуется на ощущение тумана в голове или усиление выпадения волос. Проблемы со сном испытывает каждый третий пациент. 60% пациентов после COVID-19 испытывают признаки астении (синдром хронической усталости), в том числе мышечную слабость. Еще 4% страдают от головокружений, а 2% отмечают онемение в стопах и кистях рук. У 4% пациентов наблюдаются боли в животе, тошнота, диарея [1]. Цель ‒ изучить последствия заболевания у пациентов, перенёсших новую коронавирусную инфекцию. Материалы и методы ‒ анализ, классификация, сравнение, анкетирование, методы статистической обработки данных. Результаты. Выборку исследования составили 83 человека, перенёсших новую коронавирусную инфекцию. Среди них 41 женщина и 42 мужчины в возрасте от 17 до 66 лет. Было выявлено, что у женщин позднее, чем мужчин, начинают проявляться признаки постковидных осложнений; а у мужчин тяжелее проходит период реабилитации сразу после острой фазы заболевания. Так же нам удалось выявить связь между степенью тяжести перенесенного заболевания и постковидным синдромом. Пациенты, перенесшие заболевание в тяжелой степени, чаще испытывают на себе симптомы постковидных последствий, чем пациенты, перенесшие заболевание в легкой форме. Анализ наличия последствий у исследуемых по прошествии 12 месяцев и более выявил наличие симптомов «постковидного синдрома», что может говорить о длительном течении нового явления «ковидный хвост», что требует более длительного наблюдения за пациентами. Исследования показали, что постковидный синдром может влиять на жизнь широкого спектра людей с COVID-19 ‒ от тех, у кого острая форма протекает легко, до лиц с наиболее тяжело протекавшим заболеванием. Постковидный синдром может затрагивать множество органов, влиять на целый ряд систем и проявляться в виде усталости, одышки, нарушениями работы сердца, когнитивных функций, сна. Обсуждения. Наиболее часто отмечаемый респондентами симптом ‒ астения. Ее возникновение и стойкий характер могут быть обусловлены воздействием вируса на нервные клетки, а также перенесенной гипоксией и развитием микротромбозов. Влияние SARS-CoV-2 на когнитивную деятельность, а также на возникновение постковидной астении является серьезной проблемой как у мужчин, так и у женщин. Слабость после коронавируса с сопутствующими головными болями, сонливостью, физической слабостью может возникать из-за нехватки кислорода в связи с поражением легких. В отличие от обычной усталости постковидная астения является патологическим состоянием и не устраняется отдыхом. Такое состояние может вести к снижению работоспособности, нарушению привычной жизнедеятельности и создавать почву для более тяжелых психических и соматических нарушений. Выводы. COVID-19 – мультисистемное заболевание с разнообразными симптомами. Развитие продолжающегося симптоматического (подострого) и постковидного синдрома не зависит от степени тяжести заболевания. У перенесших острый COVID-19 могут быть различные симптомы и со временем они могут меняться. Постковидный синдром ‒ опасное состояние, которое может влиять на физическое и психологическое состояние человека., Introduction. Seventeen percent have a persistent cough. One in ten experience joint and muscle pain. Patients may also be disturbed by palpitations. One in five have a foggy feeling in the head or increased hair loss. And one in three patients experience sleep problems. 60% of patients after COVID-19 experience signs of asthenia (chronic fatigue syndrome), including muscle weakness. Another 4% suffer from dizziness, and 2% report numbness in the feet and hands. Abdominal pain, nausea, and diarrhea occur in 4% of patients [1]. Purpose to study ‒ the consequences of the disease in patients who had a new coronavirus infection. Materials and methods. Questionnaire, analysis, classification, comparison method, statistical processing methods. Results. As a result of the study, we were able to find out that women later than men begin to show signs of post-acute complications, and men have a harder period of rehabilitation immediately after the acute phase of the disease. We also managed to identify a correlation between the severity of the disease and postcoid syndrome. Patients who had the disease in a severe degree were more likely to experience symptoms of postcoidal sequelae than patients with a mild degree of severity. Analysis of the presence of sequelae in study subjects after 12 months or more revealed the presence of "postcoid syndrome" symptoms, which may suggest a long-term course of the new "covid tail" phenomenon, and requires longer follow-up of patients. Studies have shown that post-covid syndrome can affect the lives of a wide spectrum of people with COVID-19, from those with the acute form going mildly to those with the most severe disease. COVID-19 can affect multiple organs and affect a range of systems, including fatigue, shortness of breath, cardiac dysfunction, cognitive damage, and sleep disturbances. Discussion. The most frequently reported symptom by respondents is asthenia. Its occurrence and persistence may be due to the effect of the virus on nerve cells, as well as to hypoxia and the development of microthrombosis. The effect of SARS-CoV-2 on cognitive performance as well as the occurrence of post-coronary asthenia is a serious problem in both men and women. Weakness after coronavirus with associated headaches, drowsiness, and physical weakness can occur due to lack of oxygen due to lung damage. Unlike normal fatigue, post-coronary asthenia is a pathological condition and is not corrected by rest. Such a condition can lead to decreased performance, disruption of habitual activities, and set the stage for more severe mental and somatic disorders. Conclusions. COVID-19 is a multisystem disease, with a variety of symptoms. The development of ongoing symptomatic (subacute) and post-acute syndrome is independent of the severity of the disease. Persons with acute COVID-19 may have a variety of symptoms and may change over time. Post-covid syndrome is a dangerous condition that can affect a person's physical and psychological well-being.
