Purpose. This study investigated the relationship between serum immunoglobulin A (IgA) concentration and epileptic seizures in children. Methods. Pediatric patients with epilepsy were included in this cross-sectional investigation. Serum IgA levels were analyzed using a biochemical analyzer during admission for initial diagnosis before any medical treatment was administered. Results. Among the 94 patients, 19.1% had decreased serum IgA levels, with a median value of 0.57 g/L. No significant relationship existed between serum IgA concentration and gender, age, epilepsy type, seizure duration or epilepsy waves. Children with low IgA levels experienced more spastic episodes and had a higher seizure frequency compared to those without decreased IgA levels. Changes in serum IgA levels were significantly associated with seizure frequency after three months of treatment. Initially, the occurrence of seizures in the low IgA group was 10.5 times lower than in the non-low IgA group, but, after three months, the low IgA group had a significantly higher seizure rate (72.2%) compared to the non-low IgA group (19.7%). After six months, the low IgA group had a higher proportion of children without seizures (100% versus 84.8%), although the difference was not statistically significant. Conclusion. Serum IgA concentration plays a crucial role in diagnosing and predicting outcomes for pediatric epilepsy.

This study aims to determine whether supplementing newborn mice with eight different strains of Bifidobacterium longum subsp. infantis might regulate their IgA levels. The strains were gavaged to BALB/C female (n = 8) and male (n = 8) dams at 1–3 weeks old. Eight strains of B. longum subsp. infantis had strain-specific effects in the regulation of intestinal mucosal barriers. B6MNI, I4MI, and I10TI can increase the colonic IgA level in females and males. I8TI can increase the colonic IgA level in males. B6MNI was also able to significantly increase the colonic sIgA level in females. B6MNI, I4MI, I8TI, and I10TI regulated colonic and Peyer's patch IgA synthesis genes but had no significant effect on IgA synthesis pathway genes in the jejunum and ileum. Moreover, the variety of sIgA-coated bacteria in male mice was changed by I4MI, I5TI, I8TI, and B6MNI. These strains also can decrease the relative abundance of Escherichia coli. These results indicate that B. longum subsp. infantis can promote IgA levels but show strain specificity. Objective: To determine the frequency of Celiac disease in patients with Type-1 Diabetes Mellitus (DM) and its association with socio-demographic parameters. Study Design: Cross-sectional study. Place and Duration: Medical Departments of Dr Ruth K. M. Pfau, Civil Hospital, Karachi Pakistan, from Apr to Sep 2019. Methodology: Patients diagnosed with type-1 DM were enrolled in the study. Patients underwent blood testing for Celiac Serology. IgA level greater than 10U/m1 was taken as positive for celiac disease. If IgA levels were low, then Anti-tTG IgG was tested. Results: One hundred seventy-seven patients diagnosed with type-1 diabetes mellitus were included in this study. The average age of patients was 36.39±6.81 years. There were 109(61.58%) males and 68(38.2%) females. The frequency of celiac disease in patients with type-1 DM was observed at 8.47%. The frequency of celiac disease was not statistically significant among different age groups (p=0.644), gender and disease duration (p>0.05). However, celiac disease was more frequent in patients with a family history, but it was not statistically significant (25% vs 7.7%; p=0.086). Conclusion: This study showed a higher frequency of celiac disease in patients with T1DM than in the general population in our country, and the data lend support to recommend regular screening for Celiac disease in all patients with Type-1 DM.
