Background: Malaria remains a significant concern for children under five in malaria-endemic regions. Rwanda's successful efforts in malaria treatment reduced nationwide cases, but high mortality persists in the Eastern and Southern provinces. This study aimed to investigate the clinical features of severe malaria and to identify the death risk factors among under-five children in Eastern province Hospitals. Methods: This cross-sectional study assessed severe malaria-related mortality and associated factors among children aged under five years in hospitals of Eastern Province, Rwanda, from 2017 to 2021. Data were collected from hospital records, and descriptive statistics and logistic regression were used for data analysis. Results: Factors associated with severe malaria-related mortality included coma (aOR: 10, 95% CI: 1.2–82.5, p = 0.03), Vomiting (aOR: 5.2, 95% CI: 1.0–26.0, p = 0.04), four or more days of illness before consultation (aOR: 30.9, 95% CI: 8.7–109.9, p < 0.01). On the other hand, a low parasitaemia level (aOR: 0.01, 95% CI: 0.0–0.1, p < 0.01). Conclusion: In conclusion, coma, vomiting, and healthcare delays increased mortality in children with severe malaria. Prompt treatment, proper assessments, protocol adherence, and caregiver education on early symptom recognition are crucial for improving outcomes. [ABSTRACT FROM AUTHOR]

Risk factors for interpersonal violence-related injury (IPVRI) in low-income and middle-income countries (LMICs) remain poorly defined. We describe associations between IPVRI and select social determinants of health (SDH) in Cameroon. We conducted a cross-sectional analysis of prospective trauma registry data collected from injured patients >15 years old between October 2017 and January 2020 at four Cameroonian hospitals. Our primary outcome was IPVRI, compared with unintentional injury. Explanatory SDH variables included education level, employment status, household socioeconomic status (SES) and alcohol use. The EconomicClusters model grouped patients into household SES clusters: rural, urban poor, urban middle-class (MC) homeowners, urban MC tenants and urban wealthy. Results were stratified by sex. Categorical variables were compared via Pearson's χ2 statistic. Associations with IPVRI were estimated using adjusted odds ratios (aOR) with 95% confidence intervals (95%CI). Background: Depression is associated with a 60% increased risk of type 2 diabetes mellitus, and diabetes doubles the likelihood of depression. This study was conducted to estimate the prevalence of depression and assess the association between glycemic control and depression in diabetic patients. Methods: A total of 100 patients with type 2 diabetes mellitus were included in this cross-sectional hospital study. The study protocol included a detailed clinical history, examination, questionnaire administration and examination. Fasting plasma glucose, postprandial plasma glucose, HbA1c, lipid profile, renal function test and electrolytes were determined in these subjects. The Becks Depression Inventory (BDI) scale was used to diagnose and classify the severity of depression in these patients. Results: Of 100 diabetic patients, depression was present in 39% of individuals, of which 16% had mild depression, 10% had borderline depression, 8% had moderate depression, 3% had severe depression, and 2% had extreme depression.. The prevalence of depression in patients with glycated hemoglobin levels = 6.4 was found to be 29%, 6.5 to 7 was 33% and = 7.1% was 62%. Conclusions: Depression was found to be more common in diabetics compared to the general population. The prevalence of depression was higher in patients with long-term diabetes, female sex, Muslim religion, substance abuse, diabetes-related complications, and poor glycemic control. Background: Stroke is a neurological emergency with high prevalence, especially in developing countries where it assumes an epidemic proportion. It is globally the second most common cause of death after ischemic heart disease, and it is projected to remain so by 2030. It had a global prevalence of 1,300.6 per 100,000 people in 2017., Objectives: This study compared the socio-demographic, clinical and laboratory characteristics of acute Haemorrhagic and Ischaemic stroke patients managed over nine years in a private tertiary hospital in Abakaliki, Nigeria., Methods: This is a Retrospective and comparative Hospital-based study conducted at a tertiary hospital in Abakaliki Nigeria from January 2014 to December 2022. Relevant data were retrieved from the case notes and the sociodemographic, clinical, and laboratory parameters of Haemorrhagic stroke victims were compared with that of Ischaemic stroke patients., Results: Out of the 172 (males- 56.98%, females- 43.02%) patients that fulfilled the inclusion criteria, 53 (30.81%) had a Haemorrhagic stroke while 119 (69.19%) had an ischemic stroke. The overall admission mortality rate was 15%, and it was more preponderant among Haemorrhagic stroke victims. Haemorrhagic stroke was statistically associated with admitting hyperglycaemia , anaemia, hypercholesterolemia, neutrophilic leucocytosis, hypernatremia, alteration in consciousness, low educational attainment, and higher mortality., Conclusions: Haemorrhagic stroke is more prevalent among female folks, and it is associated with higher morbidity and mortality. Abstract Background Post-stroke depression (PSD) has worse functional outcomes and quality of life. Despite the extensive literature on this topic, there is no agreement on the frequency or risk factors for post-stroke depression. Objectives To establish the frequency and risk factors of post-stroke depression and its impact on quality of life. Patients and methods One hundred three stroke patients were recruited from the out-patient clinic of Qena University Hospital who satisfied the WHO definition for stroke, together with a control group of 50 age- and sex-matched healthy volunteers. A complete history, neurological examination, and CT brain were obtained for each patient. DSM-IV TR criteria were used for diagnosis of depressive disorders which was scored with the Hamilton depression rating scale (HAM-D); Barthel Index (BI), and quality of life were also measured. Results Thirty-eight (36.9%) stroke patients had PSD which was significantly higher than in the normal population (control group 12%). Statistically significant risk factors for PSD included low educational level, low socioeconomic status, smoking, and post-stroke functional impairment. Post-stroke depression has an impact on quality of life. Conclusion Post-stroke depression is a relatively common complication of stroke and can affect the quality of life. Low educational level and socioeconomic status, as well as smoking and functional impairments, were considered as risk factors for the occurrence of post-stroke depression. Early detection of predictors of post-stroke depression may improve the outcome of stroke and prevent the psychiatric consequences.

Abstract Objective We aimed to evaluate epilepsy management at the regional hospital center (RHC) of Tahoua (Niger) to determine the intrahospital deficiencies to optimize to improve the management of people with epilepsy. Methods A descriptive retrospective study was carried out at the Psychiatric Unit of the RHC of Tahoua between January 1, 2016 and December 31, 2019. We collected from the registers of consultation all patients followed by nurse technicians in mental health for epilepsy whose diagnosis was made by nonspecialist physicians or internists. The study included patients with epilepsy who had a medical follow‐up at least 6 months. Results Of the 2022 patients seen during the period of the study, 246 patients were consulted for epilepsy with a hospital frequency of 12.2%. The mean age was 22.38 years, with a slight predominance of men (57.7%). Only generalized tonic‐clonic seizures (95.1%) and focal‐aware seizures (4.9%) were reported. The main etiologies were cerebral malaria (18.7%), bacterial meningitis (8.1%), and head trauma (4.9%). In 60.2% of cases, the etiology was undetermined, but the etiological diagnosis investigation was incomplete (CT scan not done). Only the first‐generation antiepileptic drugs were used. Only 2.8% of the patients had drug‐resistant epilepsy, and 97.2% of the patients had controlled seizures. Conclusion The study shows a predominance of infectious causes, particularly cerebral malaria. We found a high proportion of epilepsies with unknown etiologies with incomplete workup. The RHC of Tahoua should facilitate access to the CT scan for people with epilepsy to improve etiological diagnosis investigation.

Abstract Background and objective The crude prevalence rate of stroke in Qena, Egypt, is high (922/100,000). For the first time, we estimate the frequency of aphasia following the first-ever ischemic stroke and its relationship to the site of lesion in Arabic-speaking countries. Methods The study was conducted on 180 acute or subacute ischemic stroke patients. Aphasia was diagnosed with the short form of the Boston Diagnostic Aphasia Examination after establishment of the cases (at least 10 days of stroke onset). Computed tomography and/or magnetic resonance imaging were performed for topographic localization of stroke, and routine laboratory investigations were done to evaluate the possible stroke risk factors. Results The frequency of post-stroke aphasia was recorded in 20%, and atrial fibrillation (AF) represents a high-risk factor for ischemic stroke with aphasia. The following categories were observed: global aphasia in 50%, motor aphasia in 27.7%, sensory aphasia in 11.1%, nominal aphasia in 2.8%, and conductive aphasia in 8.3%. Global aphasia was mainly associated with complete middle cerebral artery (MCA) territories infarction. Motor aphasia was associated mainly with frontal or frontoparietal operculum lesions as well as basal ganglionic lesions. Sensory aphasia was associated with temporal lobe lesions, inferior parietal lobe, and basal ganglia. Conductive aphasia showed lesions in the deep white matter of the frontal and parietal lobes. Anomic aphasia was associated with inferior parietal lobe lesion. Conclusion Our data are consistent with other reports as regards the frequency and risk factors of post-stroke aphasia. Atrial fibrillation represents a high-risk factor for ischemic strokes with aphasia. Global and motor aphasia are the major subtypes. The lesion locations are predictive of aphasia subtypes.

Abstract Objectives Epilepsy constitutes a major public health concern in the world particularly in developing countries, especially in sub‐Saharan African countries. We designed this study to evaluate epilepsy management at a tertiary referral center in Niger to obtain a comprehensive understanding to determine the intrahospital deficiencies to improve and to make recommendations in terms to improve epilepsy management in Niger. Methods We conducted a retrospective study at the Neurology Outpatient Clinic of the National Hospital of Niamey (Niger) between May 2013 and May 2018 (5 years), collecting all cases of patients diagnosed with epilepsy by the neurologists. From the registers of consultation, we collected for each patient the demographic, clinical, etiological, and therapeutic data, as well as the outcomes during follow‐up visits. Results Of the 4576 patients seen during the period of the study, 1350 patients consulted for epilepsy with a hospital frequency of 29.5%. The mean age of the patients was 18.55 ± 17.15 years (range: 3 months to 83 years) with a predominance of the male sex (sex ratio at 1.5). Patients younger than 20 years were the most represented (61.6%). All patients underwent EEG. Only 463 patients (35.2%) underwent brain imaging. Generalized tonic‐clonic seizures were the most frequent (50%) followed by typical absences seizures (11.8%). Cerebrovascular disease, central nervous system infections, and head injuries were the main etiologies. First‐generation AEDs were the most prescribed (99%). The proportion of patients with drug‐resistance was 9.6%. Significance Our study shows limited access to newer generation AEDs and diagnostic tests of epilepsy in Niger. Considerable efforts should be made to facilitate for people living with epilepsy the accessibility to diagnostic tests and newer generation AEDs in order to improve the quality of epilepsy management in Niger.

BACKGROUND: The present study determines to understand the maternal health-care infrastructure in Meghalaya, a state of Northeast India. The state has large number of population of 2,964,007 (Census 2011), however, the state has only two public District Hospitals related to maternal health-care services – one in East Khasi Hills District and the other one in West Garo Hills District. Furthermore, according to the National Rural Health Mission (2015–2016), the maternal mortality ratio of Meghalaya is 211/100,000 live births which is higher than the national average. MATERIALS AND METHODS: A hospital-based mixed method study has carried out to identify the problems faced by the women during their maternity period and to assess the quality and availability of maternal care infrastructure in Meghalaya. To conduct the interview, the present study covered all 80 pregnant women who were admitted in the major two district maternal health-care hospitals of Meghalaya for maternity care (i.e., delivery and postdelivery period) during the period of hospital visit in October 2019. RESULTS: The study shows that majority of the admitted women came from different villages of different districts of Meghalaya as those remote areas have no adequate maternal health-care institutions nearby other than dispensary or subcenter. The study also revealed that women who came to get treatment in hospital during pregnancy had to face difficulties like to travel a long distance, use public transport, bad road conditions, and financial problems. CONCLUSION: To improve the health scenario of health-care system in the remote village areas, the Government should give more focus on infrastructure development in terms of availability of adequate facilities for handling any emergency cases of maternal care in remote areas of Meghalaya. OBJECTIVES: To explore the risk factors of essential hypertension with hyperhomocysteinemia (H-type hypertension) and design a nomogram to predict this risk. METHODS: A hospital-based study was conducted on 1,712 individuals, including 282 patients with H-type hypertension, 105 patients with simple hypertension, 645 individuals with hyperhomocysteinemia, and 680 healthy controls. Logistic regression and nomogram models were applied to evaluate the risk factors. RESULTS: Logistic regression showed that advanced age, male sex, high body mass index (BMI), high total cholesterol levels, high glucose levels, and high creatinine levels were risk factors of H-type hypertension in the healthy population and were integrated into the nomogram model. Advanced age, male sex, high BMI, high total cholesterol levels, and high glucose levels were shown to be risk factors of H-type hypertension in the hyperhomocysteinemia population. Male sex and high creatinine levels were shown to be risk factors of H-type hypertension in the hypertension population. Nomogram analysis showed that the total factor score ranged from 106 to 206, and the corresponding risk rate ranged from 0.05 to 0.95. CONCLUSIONS: Men are more likely to have H-type hypertension, and advanced age, high BMI, high total cholesterol levels, and high glucose levels are risk factors of H-type hypertension in healthy and hyperhomocysteinemia populations. Furthermore, high creatinine level is a risk factor of H-type hypertension in healthy and hypertension populations. Nomogram models may be used to intuitively evaluate H-type hypertension risk and provide a basis for personalized interventions.

Abstract Background and aims Diabetes mellitus is likely to have a major effect on vision, and adequate knowledge of its ocular manifestations is of substantial importance to diabetic patients. The study aimed to assess the ocular knowledge and practices among Type 2 diabetic patients of Bangladesh. Methods This cross-sectional study included 122 diabetic patients from the outpatient department (OPD) of the apex diabetic healthcare hospital of the country under the sponsorship of the Diabetic Association of Bangladesh (BIRDEM). A questionnaire was used for collecting data on knowledge on and practices relating to diabetes mellitus with particular emphasis on ocular issues. A predefined score was used for categorizing levels of knowledge and practices as poor, average, and good. Results Of the 122 respondents, 63%, 55%, 40%, 44%, and 30% reported, blindness, retinopathy, cataracts, glaucoma, and double vision respectively, as complications of diabetes mellitus. About 50% were aware of the need for eye screening for people with the complications. Only 8% monitored their blood glucose levels daily, 15% monitored weekly, and 10% reported checking their blood pressure daily and 43% took their medications as prescribed. The level of diabetic knowledge was poor, moderate and good, respectively, among 24%, 56%, and 20% of the respondents, whereas the practice standards showed that 47%, 31%, and 22% had poor, average, and good levels respectively. The knowledge score was significantly associated with the practice score (r = 0.460, p = 0.001). Conclusion The results indicate that the ocular knowledge and practices among diabetic patients attending a tertiary-care hospital in Bangladesh is average. Health and eye-care practitioners need to expand diabetic health education and promotion among diabetic patients.

Abstract Background Human strongyloidiasis is a chronic and persistent gastrointestinal disease caused by infection with soil-transmitted helminths of the genus Strongyloides. The aim of this research was to obtain diagnostic prevalence regarding strongyloidiasis in northeast Thailand through a hospital-based study. Methods Patients’ demographic data and the results of stool examinations conducted using the formalin ethyl acetate concentration technique were collected from the parasitology laboratory records at Srinagarind Hospital in Khon Kaen, Thailand. The relevant information from years 2004 to 2014 was collected and descriptively analyzed. Results Of a total of 22,338 patients, 3889 (17.4%) had stool samples that tested positive for Strongyloides larvae. The highest prevalence was 22.8% (95% CI = 19.6–26.2%) in the year 2004. This percentage progressively decreased, reaching 11.2% (95% CI = 10.2–12.4%) in 2013 and remaining stable at 12.9% (95% CI = 11.8–14.1%) in 2014. Males (2741 cases) had double the positivity rate of females (1148 cases). The prevalence of infection was highest (25.9%; 95% CI = 24.5–27.3%) among patients that were 51–60 years of age. Conclusions Areas endemic for strongyloidiasis should be emphasized under the national helminth control program and health education campaigns. Nationwide assessments should also be performed regarding Strongyloides infection, including risk factors, treatment, and prevention. The diagnostic laboratory data presented here identify the geographical focus of disease to be the northeastern region of the country. Further targeted surveillance using more sensitive methods will almost certainly reveal a higher individual disease burden than found in this report.

Background: The purpose of this study was to explore the stroke types and the risk factors of the first-time stroke in the holy city of Madinah Munawarah. Methods: A prospective hospital-based research study was carried out over the year of 2014. The patients with the diagnosis of the cerebrovascular accident were enrolled in the study. The details of patients’ history, demographic data, stroke type, and risk factors were collected. Results: First-time stroke were determined in 164 patients (91 male and 73 female) during the entire periods of this study with a mean age of 67.52 ± 2.31 years. The prevalence of types of stroke was ischaemic strokes in 133 patients (% 81.1), intercerebral hemorrhage in 24 patients (14.63%), and subarachnoid hemorrhage in one patient (0.6%). The most common risk factors recorded were hypertension (86.6%), diabetes mellitus (63.4%), and ischaemic heart disease (41.5%). There was no significant relationship between the frequency of ischaemic stroke and intercerebral hemorrhage stroke ( p> 0.05). Conclusion: The study indicates that hypertension, diabetes, and ischaemic heart disease are critical risk factors for developing stroke and fairly commensurate with the global reported risk factors. Further research is needed to investigate stroke patterns and other possible risk factors concurrently with Saudi national prevention programs.

Background: Esophageal cancer (EC) causes more than 400 thousand deaths per year, and half of them occur in China. There are discrepancies regarding the survival of EC patients between population-based surveillance studies and hospital-based studies.Objectives: We aimed to synthesize the survival data from hospital-based EC studies in the Chinese population from 2000 to 2018 and to compare the survival rates between EC patients with different clinical classifications.Methods: The protocol of this systematic review was registered in PROSPERO (CRD-42019121559). We searched Embase, PubMed, CNKI, and Wanfang databases for studies published between January 1, 2000 and December 31, 2018. We calculated the pooled survival rates and 95% confidence intervals (CIs) by Stata software (V14.0).Results: Our literature search identified 933 studies, of which 331 studies with 79,777 EC patients met the inclusion criteria and were included in meta-analyses. The pooled survival rates were 74.1% (95% CI: 72.6–75.7%) for 1-year survival, 49.0% (95% CI: 44.2–53.8%) for 2-years survival, 46.0% (95% CI: 42.6–49.5%) for 3-years survival, and 40.1% (95% CI: 33.7–46.4%) for 5-years survival. An increased tendency toward EC survival was verified from 2000 to 2018. In addition, discrepancies were observed between EC patients with different clinical classifications (e.g., stages, histologic types, and cancer sites).Conclusions: Our findings showed a higher survival rate in hospital-based studies than population-based surveillance studies. Although this hospital-based study is subject to potential representability and publication bias, it offers insight into the prognosis of patients with EC in China.

Background: Few studies have examined the concurrent effects of physical disease and systemic inflammation on suicide risk in patients with depression. The authors investigated the independent contributions of chronic physical disease and systemic inflammation as indexed by C-reactive protein (CRP), on risk of suicide attempt. Methods: In this case–control study, 1468 cases of suicide attempters and 14 373 controls, both aged 18–65 years with a diagnosis of depression during 2011–2015, were identified from the hospital-wide database. Regression models were implemented to identify separate effects of physical diseases and systemic inflammation indexed by CRP, on risk of suicide attempt. Results: Compared with having no physical disease, having one, two, and three or more physical diseases was associated with a 3.6-, 6.4-, and 14.9-fold increase in odds of making a suicide attempt, respectively, after adjusting for age, sex, and race/ethnicity. In a sub-sample of cases and controls with available CRP values, patients with high CRP (>3 mg/L) had 1.9 times the odds of suicide attempt compared with patients with low CRP (<1 mg/L). This association was no longer significant when controlling for the effect of physical disease. Conclusions: The presence of physical disease is an important risk factor for suicide attempt among patients with depression. Systemic inflammation is likewise associated with increased risk for suicide attempt, however, this association appears to be accounted for by the presence of physical disease among patients receiving care in a medical center setting. Healthcare providers should consider the risk of suicide attempt in depressed patients burdened with multiple comorbidities. [ABSTRACT FROM AUTHOR]

Purpose: To investigate dry eye-associated risk factors in a hospital-based population of China. Methods: It is a multi-center cross-sectional study involving 94 tertiary hospitals all over China. Outpatients in these hospitals were asked to complete McMonnies Questionnaire and undergo dry eye examinations. A total of 31124 participants were finally selected. Results: Of the 31124 participants, 17937 (57.6%) participants were clinically diagnosed as dry eye. The percentage of diagnosed dry eye was higher among women than among men (61.9% versus 53.2%). Dry eye was diagnosed more often with higher age (under 25 years, 47.6%; 25–45 years, 56.6%; over 45 years, 65.6%). In multivariate analysis, independent risk factors for dry eye were female sex (odds ratio [OR] = 1.44; 95% confidence interval [CI], 1.37–1.51), higher age (compared to age<25 years, 25–45 years: OR = 1.42; 95%CI, 1.34–1.51; compared to age<25 years, over 45 years: OR = 2.09; 95%CI, 1.96–2.24), arthritis (OR = 1.51; 95%CI, 1.38–1.65), thyroid diseases (OR = 1.41; 95%CI, 1.27–1.56), refractive surgery (OR = 1.23; 95%CI, 1.15–1.31), antihistamine eye drops (OR = 1.34; 95%CI, 1.23–1.47), diuretics (OR = 1.13; 95%CI, 1.01–1.27), sleeping tablets (OR = 1.21; 95%CI, 1.09–1.34), tranquilizers (OR = 1.60; 95%CI, 1.40–1.82), medication for duodenal ulcer (OR = 1.88; 95%CI, 1.61–2.21) and medication for digestive problems (OR = 1.14; 95%CI, 1.05–1.25). Diabetes was not found to be significantly associated with dry eye in multivariate model (p =.836). Conclusions: Dry eye was commonly diagnosed among ophthalmic outpatients in mainland China. Objectives The aim of this study is to evaluate the management of epilepsy in the elderly at a tertiary referral center in Niger to obtain a comprehensive understanding to determine the intrahospital deficiencies to improve and to make recommendations in terms to improve the management of epilepsy in the elderly in Niger. Materials and Methods We conducted a retrospective study at the Neurology Outpatient Clinic of the National Hospital of Niamey (Niger) over a period of 5 years from May 2013 to May 2018, collecting all cases of patients aged 60 years or over diagnosed with epilepsy by neurologists. From the registers of consultation, we collected and analyzed for each patient the demographic, clinical, etiological, and therapeutic data, as well as the outcomes during follow-up visits. Results Of the 4,576 patients of all ages seen during the period of our study, we included 62 patients aged 60 years or over diagnosed with epilepsy with a hospital frequency of 1.35%. The mean age of patients was 65.82 ± 5.72 years (range: 60 and 83 years) with a predominance of the male sex (sex ratio at 1.6). Patients aged 60 to 64 years were the most represented (43.5%). Generalized tonic–clonic seizures were the most frequent (41.9%), followed by focal to bilateral tonic-clonic seizures (25.8%). All patients underwent electroencephalogram. Only 30 patients (48.4%) underwent brain imaging, and mainly brain computed tomography scan. The etiologies included poststroke epilepsy (25.8%), brain tumors (3.2%), cerebral toxoplasmosis (3.2%), and cerebral meningioma (1.6%). We found 41 cases (66.1%) of epilepsy without definite etiology and with an incomplete workup. Carbamazepine and phenobarbital were the only two antiepileptic drugs (AEDs) used. Conclusion The present study shows limited access to newer generation AEDs and diagnostic tests of epilepsy in Niger. Considerable efforts should be made to facilitate for people living with epilepsy the accessibility to diagnostic tests and the newer generation AEDs to improve the quality of epilepsy management in Niger. Background: Esophageal cancer (EC) causes more than 400 thousand deaths per year, and half of them occur in China. There are discrepancies regarding the survival of EC patients between population-based surveillance studies and hospital-based studies. Objectives: We aimed to synthesize the survival data from hospital-based EC studies in the Chinese population from 2000 to 2018 and to compare the survival rates between EC patients with different clinical classifications. Methods: The protocol of this systematic review was registered in PROSPERO (CRD-42019121559). We searched Embase, PubMed, CNKI, and Wanfang databases for studies published between January 1, 2000 and December 31, 2018. We calculated the pooled survival rates and 95% confidence intervals (CIs) by Stata software (V14.0). Results: Our literature search identified 933 studies, of which 331 studies with 79,777 EC patients met the inclusion criteria and were included in meta-analyses. The pooled survival rates were 74.1% (95% CI: 72.6–75.7%) for 1-year survival, 49.0% (95% CI: 44.2–53.8%) for 2-years survival, 46.0% (95% CI: 42.6–49.5%) for 3-years survival, and 40.1% (95% CI: 33.7–46.4%) for 5-years survival. An increased tendency toward EC survival was verified from 2000 to 2018. In addition, discrepancies were observed between EC patients with different clinical classifications (e.g., stages, histologic types, and cancer sites). Conclusions: Our findings showed a higher survival rate in hospital-based studies than population-based surveillance studies. Objectives: Epilepsy constitutes a major public health concern in the world particularly in developing countries, especially in sub‐Saharan African countries. We designed this study to evaluate epilepsy management at a tertiary referral center in Niger to obtain a comprehensive understanding to determine the intrahospital deficiencies to improve and to make recommendations in terms to improve epilepsy management in Niger. Methods: We conducted a retrospective study at the Neurology Outpatient Clinic of the National Hospital of Niamey (Niger) between May 2013 and May 2018 (5 years), collecting all cases of patients diagnosed with epilepsy by the neurologists. Patients younger than 20 years were the most represented (61.6%). All patients underwent EEG. Only 463 patients (35.2%) underwent brain imaging. Generalized tonic‐clonic seizures were the most frequent (50%) followed by typical absences seizures (11.8%). Cerebrovascular disease, central nervous system infections, and head injuries were the main etiologies. First‐generation AEDs were the most prescribed (99%). The proportion of patients with drug‐resistance was 9.6%. Significance: Our study shows limited access to newer generation AEDs and diagnostic tests of epilepsy in Niger. This study aims to determine the prevalence, risk factors and perception associated with e-cigarette use among those attending government hospitals and health clinics in Malaysia. Methods A cross-sectional hospital-based study was conducted in seven public hospitals and health clinics in Malaysia, which were selected through a twostage cluster sampling. A validated questionnaire was used to obtain data from the selected participants. Multivariable logistic regression was employed to determine the association between sociodemographics and perceptions of e-cigarette use. Results Almost three-quarters (73.6%; n=923/1254) of participants were aware of e-cigarettes and 13.2% (n=122/923) reported having ever used e-cigarettes. The prevalence was significantly higher among males (18.1%), smokers (21.4%), and younger age group 18–34 years (30.2%). Care for the critically ill patients is often considered synonymous with a hospital having an intensive care unit. However, a focus on Essential Emergency and Critical Care (EECC) may obviate the need for much intensive care. Severe COVID-19 presented a specific critical care challenge while also being an exemplar of critical illness in general. Our multidisciplinary team conducted research in Kenya and Tanzania on hospitals' ability to provide EECC as the COVID-19 pandemic unfolded. Important basic inputs were often lacking, especially sufficient numbers of skilled health workers. However, we learnt that higher scores on resource readiness scales were often misleading, as resources were often insufficient or not functional in all the clinical areas they are needed. By following patient journeys, through interviews and group discussions, we revealed gaps in timeliness, continuity and delivery of care. Generic challenges in transitions between departments were identified in the receipt of critically ill patients, the ability to sustain monitoring and treatment and preparation for any subsequent transition. While the global response to COVID-19 focused initially on providing technologies and training, first ventilators and later oxygen, organisational and procedural challenges seemed largely ignored. Yet, they may even be exacerbated by new technologies. Efforts to improve care for the critically ill patients, which is a complex process, must include a whole system and whole facility view spanning all areas of patients' care and their transitions and not be focused on a single location providing 'critical care'. Abstract Purpose This study aims to estimate the computed tomography (CT) and Magnetic resonance imaging (MRI) uptake, stroke subtypes and 30-days case-fatality in Nigeria. Methods Stroke diagnosis and mortality data were identified from relevant databases. A random effect meta-analysis was conducted to obtain the pooled percentage uptake of CT/MRI, including 30-days case fatality and a meta-regression-like epidemiological model was applied on all data points. Findings A total of 24 studies involving 5874 stroke patients conducted in predominantly tertiary referral hospitals met the inclusion criteria. The pooled CT/MRI uptake in the last seven years was 46.66% (95% CI = 15.35 to 77.98, 8 studies). There were significant variations in the prevalence of stroke subtypes. The pooled prevalence ischemic stroke was highest (55.32%, 95% CI 48.67 to 61.97, 16 studies), followed by intracerebral haemorrhage (ICH) (32.69%, 95% CI 25.54 to 39.83, 16 studies), subarachnoid haemorrhage (SAH) (3.76%, 95% CI 2.30 to 5.22, 14 studies). In addition, the stroke of undetermined aetiology was found to be 16.57% (95% CI, 7.44–25.70, 8 studies). Overall, the 24-h, one-week and 30-days case-fatality from stroke were 10.84% (95% CI, 4.48–17.20), 24.62% (95% CI, 17.20–32.04) and 33.28% (95% CI, 27.80–38.77), respectively. There was a moderate negative correlation between prevalence of brain imaging uptake and ischaemic stroke, albeit not statistically significant (Spearman rho = 0.333, p -value =.412). Conclusion Uptake of CT/MRI procedure for stroke is poor in Nigeria. Although poverty, inaccessibility and influence of major risk factors remain pronounced, scaling up of effective strategies for stroke prevention and management should be a major public health policy priority in Nigeria. Most of the ever e-cigarette users were male, current smokers, young adults and those with favourable perceptions towards e-cigarettes. Therefore, effective health educational activities regarding safe usage of e-cigarettes targeting those group identified in this study are warranted to reduce the negative outcomes from the use of this product. Objective: To analyze the clinical and epidemiological features of patients with infantile hypertrophic pyloric stenosis (IHPS) so as to provide scientific evidence for diagnosis and prevention of IHPS.Methods: We performed a retrospective study of infants with IHPS diagnosed from 2012 to 2015 at Anhui Provincial Children's Hospital. Demographic characteristics and clinical data were collected.Results: Three hundred four patients (264 males and 40 females) were studied, of which 94.7% were full term and 75.7% were bottle fed or mixed fed; 16.8% of the patients had other congenital malformations in combination with IHPS. The proportion of IHPS cases with hyponatremia, hypokalemia, and hypochloremia was 18.4%, 12.5%, and 53.9%. A negative correlation was found between duration of disease and serum electrolytes. The mean pyloric muscle thickness, pyloric length, and diameter were 4.8 ± 0.7 mm, 19.4 ± 2.5 mm, and 13.3 ± 1.8 mm, respectively. There were significant differences in muscle thickness, pyloric length, and diameter between short (≤14 d) and long (>14 d) durations of disease. All patients underwent pyloromyotomy, and postoperative recovery was good.Conclusions: IHPS occurs mainly in male, full-term, bottle-fed or mixed-fed infants. Patients with long duration of disease were more likely to develop electrolyte disorder and thicker muscle layer. Background and Aims: Diabetes mellitus is likely to have a major effect on vision, and adequate knowledge of its ocular manifestations is of substantial importance to diabetic patients. The study aimed to assess the ocular knowledge and practices among Type 2 diabetic patients of Bangladesh.Methods: This cross-sectional study included 122 diabetic patients from the outpatient department (OPD) of the apex diabetic healthcare hospital of the country under the sponsorship of the Diabetic Association of Bangladesh (BIRDEM). A questionnaire was used for collecting data on knowledge on and practices relating to diabetes mellitus with particular emphasis on ocular issues. A predefined score was used for categorizing levels of knowledge and practices as poor, average, and good.Results: Of the 122 respondents, 63%, 55%, 40%, 44%, and 30% reported, blindness, retinopathy, cataracts, glaucoma, and double vision respectively, as complications of diabetes mellitus. About 50% were aware of the need for eye screening for people with the complications. Only 8% monitored their blood glucose levels daily, 15% monitored weekly, and 10% reported checking their blood pressure daily and 43% took their medications as prescribed. The level of diabetic knowledge was poor, moderate and good, respectively, among 24%, 56%, and 20% of the respondents, whereas the practice standards showed that 47%, 31%, and 22% had poor, average, and good levels respectively. The knowledge score was significantly associated with the practice score (r = 0.460, p = 0.001).Conclusion: The results indicate that the ocular knowledge and practices among diabetic patients attending a tertiary-care hospital in Bangladesh is average. The WHO classification that defines subtypes of acute myeloid leukaemias (AMLs) is relatively unexplored at the population-based level. This study aimed to examine the frequency of acute promyelocytic leukaemia (APL or AML-M3) in Brazil. Data were extracted from 239 cancer centres (2001-2012) and categorized according to the International Classification of Diseases for Oncology (CID-O 3.0) and WHO classification (n = 9116). CID-O3 code 9866 identified 614 APL patients. AML not otherwise specified (NOS) was frequent, and the APL group represented the main subtype specified. The mean age of APL was lower than that of other AMLs (31.5, standard deviation (SD) 18.6 versus 40.9, SD 24.6; p < 0.001); there was a high frequency of APL in the 13-21-year-old (11.8 %) and ≤12.9-year-old (6.4 %) age groups. Time taken to begin treatment (as ≤14 days versus >14 days) and induction death rate were lower in APL than in other AML subtypes (p < 0.001). Background: Human strongyloidiasis is a chronic and persistent gastrointestinal disease caused by infection with soil-transmitted helminths of the genus Strongyloides. The aim of this research was to obtain diagnostic prevalence regarding strongyloidiasis in northeast Thailand through a hospital-based study.Methods: Patients' demographic data and the results of stool examinations conducted using the formalin ethyl acetate concentration technique were collected from the parasitology laboratory records at Srinagarind Hospital in Khon Kaen, Thailand. The relevant information from years 2004 to 2014 was collected and descriptively analyzed.Results: Of a total of 22,338 patients, 3889 (17.4%) had stool samples that tested positive for Strongyloides larvae. The highest prevalence was 22.8% (95% CI = 19.6-26.2%) in the year 2004. This percentage progressively decreased, reaching 11.2% (95% CI = 10.2-12.4%) in 2013 and remaining stable at 12.9% (95% CI = 11.8-14.1%) in 2014. Males (2741 cases) had double the positivity rate of females (1148 cases). The prevalence of infection was highest (25.9%; 95% CI = 24.5-27.3%) among patients that were 51-60 years of age.Conclusions: Areas endemic for strongyloidiasis should be emphasized under the national helminth control program and health education campaigns. Nationwide assessments should also be performed regarding Strongyloides infection, including risk factors, treatment, and prevention. The diagnostic laboratory data presented here identify the geographical focus of disease to be the northeastern region of the country. Background: Awareness of glaucoma is low in spite of being the second leading cause of preventable blindness in the world. Aim: To study the awareness and knowledge levels of glaucoma in a tertiary eye care hospital in a tier-2 city. Materials and Methods: This is a hospital-based crosssectional study. A total of 704 patients attending ophthalmology outpatient department of Sri Dharmasthala Manjunatheshwara College of Medical Sciences and Hospital (SDM) Dharwad, aged 40 years and above were administered a questionnaire after a written informed consent to assess their awareness and knowledge levels of glaucoma. Subjects "having heard of glaucoma" even before they were recruited for the study were defined as "aware", and subjects having some understanding of the eye disease were defined as "knowledgeable". Results: Out of 704 subjects recruited for the study, only 34 (4.8%) subjects were aware of glaucoma and 22 (3.1%) had some knowledge of glaucoma. Determinants of glaucoma awareness were higher levels of education, diabetes, or glaucoma and family history of glaucoma. Conclusion: Awareness and knowledge of glaucoma was very low among the subjects. The findings of the study stress the need for health awareness for effective prevention of blindness due to glaucoma.

There was, and possibly still is, potential for COVID-19 to disrupt power inequities and contribute to positive transformation in global health research that increases equity. While there is consensus about the need to decolonise by transforming global health, and a roadmap outlining how we could approach it, there are few examples of steps that could be taken to transform the mechanics of global health research. This paper contributes lessons learnt from experiences and reflections of our diverse multinational team of researchers involved in a multicountry research project. We demonstrate the positive impact on our research project of making further steps towards improving equity within our research practices. Some of the approaches adopted include redistributing power to researchers from the countries of interest at various stages in their career, by involving the whole team in decisions about the research; meaningfully involving the whole team in research data analysis; and providing opportunities for all researchers from the countries of interest to voice their perspectives as first authors in publications. Although this approach is consistent with how research guidance suggests research should be run, in reality it does not often happen in this way. Introduction Surgical site infection (SSI) remains a common and widespread problem, which contributes to significant morbidity and mortality, prolongs hospital stays, and consequently increases healthcare costs. The current study aimed to assess the prevalence of SSI and its associated risk factors among patients who underwent any surgical intervention in a tertiary care center in Trichy, Tamil Nadu, India. Methodology This was a hospital-based, cross-sectional study that was carried out over a period of one year in Trichy, Tamil Nadu, India. All adult patients of both genders older than 16 years who underwent surgery were included. Patients who underwent second surgery at the same site for any reason, patients on immunosuppressant therapy or immunodeficiency disease, patients on antibiotics already, and patients with infection elsewhere were excluded. After 48 hours of surgery, if there was evidence of wound infection, then the patient was considered to have SSI. The data obtained were analyzed using SPSS (Statistical Package for the Social Sciences) version 21 (IBM Corp., Armonk, NY). Results A total of 2076 patients underwent different types of surgeries. The prevalence of SSIs during the study period was 5.6% (n = 2076). SSIs were more common in abdominal surgeries (61.2%). Patients aged 16-24 years have a higher risk of getting SSI than other age groups (p = 0.040). Males have a higher risk of getting SSI than females (p = 0.022). Patients who underwent emergency surgery have a higher risk of getting SSI than those who underwent elective surgery (p = 0.025). Those with diabetes had a higher risk of getting SSI than those who were non-diabetics (p ≤ 0.0001). Conclusion SSIs were more common in abdominal surgeries. Background: Uterine prolapse is a common problem among women in developing countries. It is known to cause physical and psychosocial problems affecting the quality of life of patients. This study was done to determine the risk factors, clinical features, and management practices in uterine prolapse (UP). Methods: A review of 350 case records of UP cases admitted between 2009 and 2014 was done in tertiary care hospitals. Results: Mean age at presentation of UP was 52.8 ± 13.2 years. Majority of cases were manual laborers [232 (78.6 %)]. Obstetric factors like parity ≥5 times [78 (22.3 %)], age at last pregnancy between 30 and 39 years (57.2 %), inadequate birth spacing (57.8 %), home deliveries [162 (58.3 %)], deliveries conducted by untrained personnel (25.3 %), vaginal deliveries (89.7 %), prolonged duration of labor (21.6 %), and heavy work in post natal period (29.8 %) were observed among cases. Correlation between age of presentation of prolapse with age at first and last pregnancy was significant. Most common associated complaint among UP cases was pain abdomen [55 (15.7 %)] and difficulty in micturition [51 (14.6 %)]. Majority were cases of third-degree prolapse [269 (76.8 %)]. Most common associated organ prolapse was cystocele [261 (74.6 %)]. The most common operative procedure done was vaginal hysterectomy, and conservative procedure was ring pessary application. Conclusion: Public awareness on reduction in family size, support for institutional-based delivery by trained personnel, and adequate rest and exercises in early post natal period is required to minimize the occurrence of UP. Background Newborn screening for urinary cytomegalovirus (CMV) and early introduction of antiviral treatment are expected to improve neurological outcomes in symptomatic congenital CMV-infected infants. This cohort study prospectively evaluated neurological outcomes in symptomatic congenital CMV-infected infants following the introduction of hospital-based newborn urinary CMV screening and antiviral treatment. Subjects/methods Following institutional review board approval and written informed consent from their parents, newborns were prospectively screened from 2009 to 2014 for urinary CMV-DNA by PCR within 1 week after birth at Kobe University Hospital and affiliated hospitals. CMV-positive newborns were further examined at Kobe University Hospital, and those diagnosed as symptomatic were treated with valganciclovir for 6 weeks plus immunoglobulin. Clinical neurological outcomes were evaluated at age ⩾12 months and categorized by the presence and severity of neurologic sequelae. Results Urine samples of 6348 newborns were screened, with 32 (0.50%) positive for CMV. Of these, 16 were diagnosed with symptomatic infection and 12 received antiviral treatment. Four infants developed severe impairment (33%), three developed mild impairment (25%), and five developed normally (42%). Conclusions This is the first Japanese report of neurological assessments in infants with symptomatic congenital CMV infection who received early diagnosis and antiviral treatment. Context: Acute Myeloid leukemia has been of special interest to innumerable workers in the field of cancer research since the blood and haematopoietic tissue can be easily and repeatedly sampled. Aim: • To know the relative incidence of Acute myeloid leukemia among the patients referred for complete haemogram at the Department of Pathology, JJMMC, Davangere, India. • To study the clinical manifestations and their correlation with various types of acute myeloid leukemia. • To study the haematological profiles in acute myeloid leukemia. Settings and Design: This was a hospital based study conducted at haematology unit, Department of Pathology, JJM Medical College, Davangere, India. Materials and Method: The present study was done during the period of June 2006 to May 2008 at haematology unit department of Pathology, JJM Medical College, Davangere, India. Cases from Chigateri General Hospital, Bapuji Hospital and other private hospitals situated in and around Davangere were included for the study. The case selection was based on the clinical features and supported by laboratory evidence. Bone marrow aspiration was subsequently carried out after obtaining written consent from the patient or the guardian. Statistics: The results were expressed in percentage. Results and Conclusion: A total of 1039 patients who were referred to the Department of Haematology out of which 50 patients were diagnosed as Acute Myeloid Leukemia. The present study highlighted that light microscopic features of peripheral smear and bone marrow will still remain mainstay in the diagnosis of acute leukemias.

OBJECTIVES: To document the frequency of anemia in all patients who presented to out-patient and/or in-patient in a tertiary level hospital in Peshawar PAKISTAN and to find the frequency of various grades of anemia in male and female patients. METHODOLOGY: This retrospective, chart-based, descriptive, institution-based study was conducted in Department of Pathology, Hayatabad Medical Complex, Peshawar. A total of 16,000 'full blood count' reports were reviewed for this study which were performed from 15th March, 2012 to 15th June, 2012. Simple random sampling was done to select data of 6104 male patients and 1578 female patients. Hemoglobin level <13 mg/dL for males and <12 mg/dL for females was considered as anemic. RESULTS: Out of a total of 7682 patients, 66.8% (n=5133) were found to be anemic while 33.2% (n=2549) patients had normal hemoglobin levels. There were 4115 male (67.4%) and 1018 female (64.5%) which were found to be anemic. Mean hemoglobin level was 11.64+2.5536 gm/dl for male patients and 11.10+2.1024 gm/dl for female patients. Moderate anemia was more common in female patients (18.5%) as compared to males (16.5%) while mild and severe anemia were more common in male patients as compared to females patients (43% vs. 39%) and (7.6% vs. 7%)] respectively. CONCLUSION: Frequency of anemia was high in patients presenting to tertiary care hospital in Peshawar. The frequency was slightly higher amongst male patients compared to female patients. Background Febrile illness is the most common symptom experienced by patients suspected of having malaria, especially in the malaria endemic areas of northeast India. Hospital-based data from two districts of upper Assam, namely Golaghat and Tinsukia, were analysed to assess the prevalence of malaria among inpatients who reported to health centres with febrile illness. Methods A total of 16 hospitals were selected for the study. These included six government hospitals (three in rural and three in urban areas) and 10 private hospitals, which included four tea-garden hospitals. Selection was dependant on the availability of patients and inpatient treatment facilities. During the study, a total of 350 inpatients reported to the selected hospitals with fever; 324 were suspected of having malaria and were interviewed and included in the study. Results The average age of patients was 24.2±15.2 years and 63.3% (205/324) of them were male. The majority of patients (77.5%; 251/324) belonged to the lower household income category (<5000 Indian rupees) and travelled to the nearest town or city for treatment. Overall, the prevalence of malaria (as diagnosed by clinical examination and blood tests) across the different categories such as location, types of hospitals and household income was found to be 29.9% (97/324). Multivariate analysis revealed that the adjusted odds ratios (AOR) of malaria cases was higher based on location (2.13), type of health centre (1.75) and the distance travelled to a health centre (2.09). The mean duration of hospital treatment was 4.2±3.6 days and the delay in reporting to hospital was 3.9±2.6 days. Conclusion The study emphasises the need to strengthen and improve the treatment facilities for malaria in government hospitals, and to create more awareness among people regarding early treatment, especially in the rural periphery villages of the endemic areas. Background: The aim was to compare the severity of glaucoma among newly diagnosed patients presenting to a hospital-based glaucoma care centre (HBGS: Sankara Nethralaya, Medical Research Foundation) with that of age matched subjects from the population-based Chennai Glaucoma Follow-up Study (CGFS). Methods: Newly diagnosed subjects with primary glaucoma from HBGS and age- and gender-matched subjects from the urban arm of CGFS examined during the same time period were included. All subjects underwent comprehensive ocular examinations including Humphrey visual field (HVF: 24-2 SITA Standard). Glaucoma was defined as: an intraocular pressure (IOP) of 22 or greater mmHg in either eye; vertical cup-to-disc ratio (VCDR) of 0.7 or greater or asymmetry 0.2 or more or the presence of focal thinning, notching or a splinter haemorrhage. All subjects had a minimum of three follow-up visits and reliable visual fields. The IOP, vertical cup-to-disc ratio, mean deviation (MD) and pattern standard deviation (PSD) of the Humphrey field measurements at the third follow-up visit of CGFS were compared for assessing the severity of glaucoma with the HBGS group. Results: Forty-seven age-matched subjects from both the study populations were selected. Significantly higher (p = 0.04) IOP was noted in the HBGS population than the CGFS, with a difference in mean IOP of 2.80 mmHg (95% CI of diff: 0.14 to 5.46). The mean ± SD of the mean deviation and pattern standard deviation were -6.92 ± 6.53 dB and 6.05 ± 4.20 dB among the HBGS and -4.47 ± 4.19 dB and 3.26 ± 2.69 dB among the CGFS population, respectively, the difference in the mean deviation (p = 0.036) and pattern standard deviation (p = 0.0001) were statistically significant. The mean vertical cup-to-disc ratio did not vary between populations (p = 0.14). Conclusion: Patients from the HBGS group had higher IOP and more severe visual field defects than the CGFS group. The prevalence of Parkinson’s disease (PD) in sub-Saharan Africa is still a matter of debate. The few studies that have been conducted have shown prevalences lower than those in the western world. Whether this represents a genuine finding is unclear to date. In northern Tanzania, we have conducted a hospital-based evaluation and a community-based door-to-door study to assess the prevalence of Parkinsonism, including that of PD. Over a period of 8 months, all patients admitted to a mid-size rural hospital were screened for PD. In parallel, 1,569 people aged ≥50 years were recruited from the communities and assessed for PD with standard questions. Sampling was performed according to the method of “multistage cluster sampling.” The questions had previously been tested in a pilot study prior to the survey. People who screened positive were examined by a specialist neurologist. In the hospital, eight of 740 people with neurological diagnoses had Parkinsonism, of whom three patients had a diagnosis of PD. In the community-based study, 18 people answered positively to least one of the 12 screening questions. However, the diagnosis of PD could not be confirmed by further examination in any of them. The prevalence of PD in northern Tanzania was found to be very low. This result would need confirmation in studies with larger populations, ideally of different African ethnicities.
