Background: More than 100,000 people in the UK are living with a human immunodeficiency virus (HIV) infection. There are currently estimated to be around 4000 people newly infected in the UK per year, mostly men who have sex with men (MSM). It has become increasingly clear that antiretroviral therapy (ART) used to treat people infected with HIV also has a profound effect on infectivity. At the initiation of the programme, it was the policy in the UK to initiate ART in people when their cluster of differentiation 4 (CD4) count was approaching 350/µl. Objectives: To assess what would be the effectiveness and cost-effectiveness of a policy of immediate initiation of ART at diagnosis among MSM, taking into account the potential reductions in new infections. Design: We calibrated an individual-based model of HIV transmission, progression and the effect of ART in MSM, informed by a series of studies on sexual behaviour in relation to ART use and the transmission risk in people with viral suppression on ART, and by surveillance data collected by Public Health England. Setting, participants and interventions: The series of studies used to inform the model included (1) the Antiretrovirals, Sexual Transmission Risk and Attitudes (ASTRA) study, a cross-sectional self-administered questionnaire study of people diagnosed with HIV attending eight HIV outpatient clinics in the UK (2011–12); (2) the Cognitive Impairment in People with HIV in the European Region (CIPHER) study, a study of levels of neurocognitive impairment in HIV-positive ASTRA participants and people from HIV clinics in Rome, Copenhagen and Minsk; (3) the Attitudes to, and Understanding of, Risk of Acquisition of HIV (AURAH) study, a cross-sectional self-administered questionnaire study of individuals who have not been diagnosed as HIV-positive attending 20 genitourinary medicine clinics across the UK (2013–14); (4) a substudy of sexual behaviour among individuals enrolled in an open-label multicentre international randomised trial (from 2013) of immediate versus deferred ART (to CD4 cell counts of 350/µl) in people with CD4 cell counts of > 500/µl [the Strategic Timing of Antiretroviral Therapy (START) trial]; and (5) Partners of People on ART: a new Evaluation of the Risks (PARTNER), an observational multicentre longitudinal study of HIV serodifferent heterosexual and MSM couples, in which the HIV-positive partner is on ART (2010–14). Main outcome measures: The main outcome measures were the clinical effectiveness and cost-effectiveness of a policy of immediate initiation of ART at diagnosis. Results: Based on data from studies (i)–(v), we estimated from our modelling work that increases in condomless sex (CLS) among MSM as a whole may explain the increase in HIV infection incidence in MSM epidemics over a time when ART coverage and viral suppression increased, demonstrating the limiting effects of non-condom use on the HIV epidemic among MSM. Accordingly, an increase in the overall proportion of MSM living with HIV who are virally suppressed on ART from the current level of 500/µl, supporting ART initiation in people diagnosed with a HIV infection. Future work: There is a need for future research into the means of increasing the frequency with which MSM test for HIV. Women are still at high risk of contracting the human immunodeficiency virus (HIV) virus due to the lack of protection methods under their control, especially in sub-Saharan countries. Polyelectrolyte multilayer smart vaginal films based on chitosan derivatives (chitosan lactate, chitosan tartate, and chitosan citrate) and Eudragit® S100 were developed for the pH-sensitive release of Tenofovir. Films were characterized through texture analysis and scanning electron microscopy (SEM). Swelling and drug release studies were carried out in simulated vaginal fluid and a mixture of simulated vaginal and seminal fluids. Ex vivo mucoadhesion was evaluated in bovine vaginal mucosa. SEM micrographs revealed the formation of multilayer films. According to texture analysis, chitosan citrate was the most flexible compared to chitosan tartrate and lactate. Objetivo: Describir la incidencia de nuevos diagnósticos de infección por el virus de la inmunodeficiencia humana (VIH) y su tendencia. Métodos: Se incluyeron los nuevos diagnósticos de VIH notificados en 2009 en las 15 comunidades autónomas con sistemas de información (64% de la población española). Para analizar las tendencias durante 2004-2009 se incluyeron las nueve comunidades con datos en ese periodo. Se obtuvo información clínico-epidemiológica de las hojas de notificación de casos y se realizaron distribuciones de nuevos diagnósticos y diagnóstico tardío según distintas variables. Para evaluar las tendencias se ajustó un modelo de Poisson. Resultados: En 2009 se notificaron 2.264 nuevos diagnósticos de VIH, la mayoría en hombres (80%). La mediana de edad al diagnóstico fue de 36 años (rango intercuartílico 29-43) y el 37,6% de los casos eran inmigrantes, destacando los latinoamericanos y los subsaharianos. La categoría de transmisión más común (42,5%) fue la de hombres que tienen sexo con hombres, seguida de la heterosexual (34,5%) y la parenteral (8,1%). In 2008, Scarlet Alliance, the Australian Sex Workers Association, carried out a needs assessment among sex workers living with HIV in Australia. The research showed that HIV positive sex workers experience discrimination from within the community, are criminalised for sex work and subject to disclosure laws in some states and territories, and face stigma perpetrated by the media. Supported by legislation, they have an almost insurmountable lack of access to policy development due to disclosure and confidentiality issues, and have expressed ongoing frustration at the lack of leadership on the intersecting issues of HIV and sex work. A high profile prosecution of a sex worker living with HIV coincided with the duration of the needs assessment project. The research gave a voice to sex workers living with HIV and highlighted the levels of institutionalised marginalisation and stigmatisation they experience. Criminalisation of sex work, of people living with HIV, and of sex workers living with HIV is at the core of this discrimination and must be challenged. Scarlet Alliance advocates for the decriminalisation of sex work across all jurisdictions in Australia. This will deliver rights to sex workers living with HIV and create a more equitable and productive environment for HIV prevention and public health generally. Our research explored the reproductive desires of HIV-negative women and their HIV-positive partners who underwent assisted conception based on sperm-washing and intrauterine insemination in Italy. Twenty-two semi-structured interviews were conducted with former patients (6 women, 5 men and 11 couples). Desire for children, perceived risk from treatment and acceptability of the insemination technique were some of the issues explored. Participants had the treatment for a mean duration of 3.5 years, with a mean number of cycles of 5.3. They were highly motivated to conceive a biological child to bring purpose to their lives, and strongly desired options to conceive safely. Most rejected spontaneous conception. Those who successfully conceived reported a positive impact on their quality of life, fulfilling their desire to be parents and restoring their sense of "normalcy". Participants strongly supported extending assisted conception services to all HIV negative women living with HIV positive partners who wished to become pregnant and felt that withholding such treatment was not ethically justified. They perceived assisted conception services to be a safe and effective alternative to spontaneous conception, and felt that society has a moral obligation to provide such services. Nous avons étudié les souhaits de femmes séronégatives au VIH et de leurs partenaires séropositifs qui s'étaient soumis à une procréation assistée avec lavage du sperme et insémination intra-utérine en Italie. Vingt-deux entretiens semi-structurés ont été menés avec d'anciens patients (6 femmes, 5 hommes et 11 couples). Le désir d'enfant, la perception du traitement et l'acceptabilité de la technique d'insémination figuraient parmi les questions abordées. Les participants ont suivi le traitement en moyenne pendant 3,5 ans, avec un nombre moyen de 5,3 cycles. Ils étaient très motivés pour concevoir un enfant biologique afin de donner un sens à leur vie, et demandaient des options pour procréer en toute sécurité. La plupart rejetaient la conception spontanée. Ceux qui ont conçu un enfant ont indiqué que cela avait eu des retombées positives sur leur qualité de vie, en satisfaisant leur désir d'être parents et leur redonnant un sentiment de « normalité ». Les participants soutenaient énergiquement l'élargissement des services de procréation assistée à toutes les femmes séronégatives vivant avec des partenaires séropositifs et qui souhaitaient avoir un enfant, et ils pensaient qu'il était moralement injustifié de refuser ce traitement. Ils estimaient aussi que la procréation assistée était une option sûre et efficace à la conception spontanée, et jugeaient que la société avait l'obligation morale de fournir de tels services. Se realizaron 22 entrevistas semiestructuradas con antiguos pacientes (6 mujeres, 5 hombres y 11 parejas). Algunos de los aspectos explorados fueron: el deseo de tener hijos, el riesgo percibido del tratamiento y la aceptación de la técnica de inseminación. Los participantes recibieron el tratamiento por una duración media de 3.5 años, con un número medio de ciclos de 5.3. Estaban muy motivados a concebir un hijo biológico para darle sentido a sus vidas, y muy deseosos de tener opciones para concebir de manera segura. La mayoría rechazó la concepción espontánea. Aquéllos que lograron concebir informaron un impacto positivo en calidad de vida, realizar su deseo de ser padres y restaurar su sentido de "normalidad". Los participantes apoyaron enfáticamente extender los servicios de concepción asistida a todas las mujeres VIH-negativas que viven con parejas VIH-positivas, que desean quedar embarazadas y opinan que la denegación de dicho tratamiento no se justifica éticamente. Percibieron los servicios de concepción asistida como una alternativa segura y eficaz a la concepción espontánea, y afirmaron que la sociedad tiene la obligación moral de proporcionar dichos servicios. Sexual health is defined in terms of well-being, but is challenged by the social, cultural and economic realities faced by women and men with HIV. A sexual rights approach puts women and men with HIV in charge of their sexual health. Accurate, accessible information to make informed choices and safe, pleasurable sexual relationships possible is best delivered through peer education and health professionals trained in empathetic approaches to sensitive issues. Young people with HIV especially need appropriate sex education and support for dealing with sexuality and self-identity with HIV. Women and men with HIV need condoms, appropriate services for sexually transmitted infections, sexual dysfunction and management of cervical and anogenital cancers. Interventions based on positive prevention, that combine protection of personal health with avoiding HIV/STI transmission to partners, are recommended. HIV counselling following a positive test has increased condom use and decreased coercive sex and outside sexual contacts among discordant couples. HIV treatment and care have reduced stigma and increased uptake of HIV testing and disclosure of positive status to partners. High adherence to antiretroviral therapy and safer sexual behaviour must go hand-in-hand. Sexual health services have worked with peer educators and volunteer groups to reach those at higher risk, such as sex workers. Technological advances in diagnosis of STIs, microbicide development and screening and vaccination for human papillomavirus must be available in developing countries and for those with the highest need globally. La santé génésique, définie en termes de bien-être, est menacée par les réalités sociales, culturelles et économiques des femmes et des hommes séropositifs. Il faut donner aux femmes et aux hommes séropositifs la maîtrise de leur santé génésique. Les pairs et les personnels sanitaires formés à des approches empathiques sont les mieux à même de transmettre des informations exactes pour permettre des choix éclairés et des relations sexuelles sûres et agréables. Les jeunes séropositifs ont spécialement besoin d'éducation sexuelle et de soutien pour vivre leur sexualité et leur identité par rapport au VIH. The swelling studies showed a moderate water uptake (<300% in all cases), leading to the sustained release of Tenofovir in simulated vaginal fluid (up to 120 h), which was accelerated in the simulated fluid mixture (4–6 h). The films had high mucoadhesion in bovine vaginal mucosa. The multilayer films formed by a mixture of chitosan citrate and Eudragit® S100 proved to be the most promising, with zero toxicity, excellent mechanical properties, moderate swelling (<100%), high mucoadhesion capacity, and Tenofovir release of 120 h and 4 h in vaginal fluid and the simulated fluid mixture respectively. Three randomised clinical trials, in South Africa, Uganda and Kenya, have shown a substantial reduction in female-to-male transmission of HIV to men who had been circumcised as compared to men who had not, during a follow-up period of up to 24 months. The question of what to do with this evidence is currently being debated around the world. WHO and UNAIDS took the lead by developing technical, policy and programmatic guidance following a series of consultations with a range of stakeholders in the field, using the research evidence as the basis. Consensus on many aspects of this matter is far from being achieved, however. Many questions and different points of view are emerging – clinical, public health, sociological, anthropological and cultural; in relation to priority setting in HIV prevention and delivery of health services; and in relation to sexuality, ethics, gender and rights. On 28 March 2007, WHO and UNAIDS published a set of conclusions and recommendations regarding the research implications for HIV policy and programming. The introduction and excerpts from these follow below. They are, in turn, followed by a roundtable of nine papers which contain a range of critical thinking and analysis of these issues. Male circumcision is generating debate across the globe. This can only be a good thing, as it is a complex matter and far more than a straightforward public health intervention. We hope these papers will help to inform that debate. Le 28 mars 2007, l'OMS et l'ONUSIDA ont publié des conclusions et recommandations relatives aux conséquences de la recherche sur les politiques et les programmes, dont nous présentons ci-après l'introduction et quelques extraits. Ils sont suivis d'une table ronde de neuf articles qui reflètent différentes pensées et analyses critiques. La circoncision suscite un débat dans le monde, ce qui ne peut être que positif, car c'est une question complexe et bien plus qu'une simple intervention de santé publique.
