No período de julho de 1986 a julho de 1993, 22 pacientes portadores de dissecção de aorta tipo A foram tratados cirurgicamente utilizando-se parada cardiocirculatória (PCC) total sob hipotermia profunda (18ºC), dos quais 15 apresentavam dissecção aguda e 7 dissecção crônica. Em 14 casos (64%) a aorta ascendente foi reconstruída utilizando-se enxerto reto de Dacron, com troca valvar aórtica em 5 pacientes e ressuspensão valvar aórtica em 2; a reconstrução do arco aórtico foi empregada em 8 casos (36%), nos quais a dissecção se extendia ou se originava no mesmo, havendo necessidade de reimplante dos ramos supra-aórticos em 3 pacientes. O tempo médio do PCC foi de 43 minutos, a mortalidade hospitalar foi de 18%, e a complicação pós-operatória mais freqüente foi a infecção respiratória. Cinco pacientes (22,7%) apresentaram dano neurológico pós-operatório, sendo, em 4 casos, reversível e atribuível a edema cerebral; 1 caso (4,5%) apresentou acidente vascular hemisférico estabelecido. Concluímos que a técnica de PCC sob hipotermia profunda deve ser utilizada sempre no reparo das dissecções de aorta tipo A, independentemente de sua extensão ao arco aórtico, pois oferece uma proteção cerebral segura, permite a inspeção ampla da zona dissecada e do local de rotura da íntima, e evita o trauma produzido pelo clampeamento da aorta acometida.From July 1986 to July 1993, 22 consecutive patients with Type A Aortic Dissecation were surgically treated using total cardiovascular arrest with deep hypothermia (18ºC). Fifteen cases had acute dissecations and 7 were chronic cases. In 14 cases (64%), the ascending aorta was reconstructed using a straight Dacron graft. In 5 of these, the aortic valve was repaired. The aortic arch was reconstructed in 8 cases (36%) and in 3 of these, the arch branches were reimplanted. The average time of the cardiovascular arrest was 43 minutes and the hospital mortality was 18%. The most frequent complication was respiratory infection, 5 patients (22.7%) showed neurologic damage, 4 of them transient and reversible. In summary, the cardiovascular arrest with deep hypothermia technique for the treatment of Type A Aortic Dissection can be used with reasonable mortality and morbility allowing a safe cerebral protection, good exposure of the dissected aorta and prevents damage to the aortic wall by the cross clamping.

Paciente de 15 meses sometido a resección de masa suprarrenal derecha. Estudio de extensión negativo e histología de tumor adrenocortical. Dos meses después presenta recidiva tumoral en vena cava inferior (VCI) a nivel de venas suprahepáticas que se reseca bajo hipotermia profunda y parada circulatoria (19 min) (Fig. 1). Se extrae una masa friable de 25 × 20 mm que ocluye la VCI, sin evidencia de resto tumoral, aunque se considera resección incompleta al estar infiltradas las paredes de VCI. La histología confirma carcinoma suprarrenal (Fig. 2). A los 5 meses sigue quimioterapia sin datos de nueva invasión de VCI.

ResumenSe presenta el caso clínico de un paciente de 3 años con Persistencia del Conducto Arterioso (PCA) tratado previamente con dispositivo septal migrado a aorta torácica descendente. El caso fue atendido en nuestra institución un año después de presentarse la complicación, por lo cual se requirió del manejo quirúrgico bajo complejas técnicas de resección y reconstrucción aórtica y manejo según protocolos de hipotermia profunda. En este artículo se describen las técnicas y los tiempos utilizados para lograr el objetivo propuesto con una evolución satisfactoria del paciente.AbstractClinical case of a three years old male case with persistent ductus arteriosus, previously treated with Septal Occluder Device, migrated to descendent thoracic aorta, this case were treated in our hospital after one year of complication; so it was required under complex surgical management techniques aortic resection and reconstruction and application of deep hypothermic protocol. This article describes time and technics used to get therapeutic objectives with a patient favorable evolution.

OBJETIVO: Monitoramento do tronco cerebral através da análise dos traçados do BERA em cirurgia cardíaca com hipotermia profunda e parada circulatória total (PCT), registrando a recuperação funcional (eletrofisiológica) do tronco cerebral. FORMA DE ESTUDO: Coorte Longitudinal. MATERIAL E MÉTODO: Foram estudados o comportamento do traçado do BERA de 8 pacientes submetidos a cirurgia cardíaca com hipotermia profunda e PCT para correção de aneurisma de aorta torácica. Etapas: resfriamento corporal até 18ºC C; PCT por até 60 minutos; reaquecimento. Registros do BERA: 35ºC, 32ºC, 26ºC, 18ºC, 27ºC, 35ºC. RESULTADOS: Os traçados iniciais (35ºC) se apresentavam normais. Aos 26ºC há o desaparecimento de todas as ondas. Aos 18ºC o traçado do BERA mostrava um padrão isoelétrico. Aos 27ºC, recuperação das ondas I, III e V. A partir de 35ºC as latências das ondas retornam ao normal. CONCLUSÃO: O BERA constitui-se em competente instrumento de monitoramento da integridade funcional do tronco cerebral em cirurgia cardíaca com hipotermia profunda e PCT. As ondas desaparecem no período de resfriamento corporal, atingem um padrão isoelétrico a 18ºC, e reaparecem durante o reaquecimento. No final do procedimento, os médicos anestesistas não dispõem de exame clínico fidedigno para acessar a integridade funcional do tronco cerebral, devido ao uso de drogas depressoras do SNC. O reaparecimento das ondas do BERA é o parâmetro proposto para este fim. A manutenção do padrão isoelétrico após o reaquecimento corporal sugere grave comprometimento funcional do tronco cerebral.AIM: The authors propose the use of ABR as an instrument for the detection of brainstem dysfunction in the trans- and post-operatory period of surgeries with total circulatory arrest (TCA) and deep hypothermia for correction of aneurysms of the thoracic aorta. STUDY DESIGN: Longitudinal Cohort. MATERIAL AND METHOD: Eight adult patients that underwent surgery for correction of aneurisms of thoracic aorta had their brainstem monitored through ABR. The patients had their body temperature lowered to 18ºC. At this moment, TCA was performed for a period of up to 60 minutes. The recordings were done at the following: before lowering the body temperature, during the cooling process, at the time of TCA (18ºC), and during the re-warming process. RESULTS: The initial tracings (35ºC) were normal. At 26ºC, ABR waves disappeared. At 18ºC and TCA, ABR showed isoelectric tracings. Waves I, III and V reappeared at 27ºC, although with prolonged latencies. When temperature reached 35ºC, latencies were back to normal. CONCLUSIONS: The ABR seems to be a useful monitor for evaluating brainstem function during surgeries with TCA and deep hypothermia. Our experience showed the extreme usefulness of the evoked auditory potentials in such procedures as a noninvasive, reliable and objective method for the monitoring of variations in the neurophysiological pattern of the brainstem. This can be of great value in presuming the patient's neurological outcome in a moment when the neurological status cannot be clinically accessed due to the several drugs used during surgery.
