Purpose — to investigate the vitamin D supply in children with chronic viral hepatitis B (HBV) depending on the activity of hepatitis and the stage of liver fibrosis. Materials and methods. Fifty children with HBV were examined. All children underwent a comprehensive examination according to the recommendation and elastography of the shear wave of the liver parenchyma to determine the stage of fibrosis. Serum 25(OH)D concentration was determined to verify the diagnosis of vitamin D deficiency. The peculiarities of vitamin D supply in children with HBV, depending on gender, age, hepatitis activity and stage of liver fibrosis, were evaluated. The study included children who did not receive calcium and vitamin D for 6 months. Results. Children of senior scholl age predomnanted among the surveyed. HBV was more often registered in boys 66.0% (n=33), while in girls — only 34.0% (n=17) (χ2=10.24; p=0.01). Among the examined children significantly more often 66.0% (n=33) we observed HBeAg-positive chronic hepatitis, 18.0% (n=9) children were diagnosed with HBeAg-positive chronic infection, HBeAg-negative chronic infection were determined in 14.0% (n=7) of children. In the vast majority (70.0%) of patients with HBV, the concentration of vitamin D in the serum was reduced (χ2=16.0; p=0.01). The average concentration of 25(OH)D was 59.85 [13.4–181] nmol/l and was in the zone of insufficiency. The optimal concentration of 25(OH)D was found in 30.0% (n=15) of children, insufficiency in 42.0% (n=21), vitamin D deficiency was diagnosed in 14 children (28.0%). Gender, age and biochemical activity of hepatitis do not affect the level of vitamin D in the examined children with HBV (p>0.05). The analysis of vitamin D supply depending on the stage of fibrosis did not reveal differences in the median concentration of 25(OH)D between groups of children, but in all groups the number of patients with low concentrations of vitamin D prevailed. Conclusions. HBV in children is characterized mainly by a chronic course, with a predominance of the parenteral route of infection. In most patients with HBV, the concentration of 25(OH)D in the serum is reduced. There was no statistically significant difference between the levels of vitamin D in children with HBV depending on gender, age, and biochemical activity of hepatitis. All children with cirrhosis of the liver were deficient in vitamin D. The research was carried out in accordance with the principles of the Helsinki declaration. Background & Aims Primary sclerosing cholangitis (PSC) is a rare cholangiopathy of unknown aetiopathogenesis. The aim of this study was to evaluate cellular senescence (CS) marker expression in cholangiocytes of patients with PSC and their correlation with clinical–pathological features and prognosis. Methods Thirty-five patients with PSC with at least 1 available liver sampling were included. Clinical laboratory data at the time of liver sampling were collected. The endpoints were survival without liver transplantation (LT), time to LT, and survival without LT or cirrhosis decompensation. Histological grading and staging were assessed according to Nakanuma. Immunohistochemical stains for CS markers, p16INK4A (p16) and p21WAF1/Cip1 (p21), were performed and scored by a 3-tier scale based on positivity extent in native bile duct (NBD) and ductular reaction (DR). Results: p16 expression in NBD and DR was directly correlated with fibrosis (p ≤0.001 for both) and stage (p = 0.006 and p, Graphical abstract, Highlights • Cholangiocyte senescence was previously described in end-stage PSC. • Cholangiocyte senescence is present in all stages of PSC and may represent an early pathogenic event. • Cholangiocyte senescence is associated with histological and clinical severity in patients with PSC. • Cholangiocyte senescence is independently associated with patients’ outcome in PSC.

Objective: to study features of vitamin D status of children with chronic hepatitis C depending upon age, gender, seasonality, place of residence and activity of hepatitis. Materials and methods. In total 28 children with chronic hepatitis C were examined during the period of 2015–2018. A complex of laboratory tests in accordance with the protocol was conducted. To verify the vitamin D status, the 25(OH)D3 serum concentration was determined. The peculiarities of vitamin D status in children depending upon gender, age, seasonality, place of residence were studied, as well as the activity of hepatitis. Results. In the majority of children that made up 60.7% (n=17) a vitamin D level decrease was detected, in 28.6% (n=8) of which insufficiency was diagnosed and deficiency – in 32.1% (n=9). The optimum vitamin D level was found in 39.3% (n=11) of children. The average vitamin D level in children with chronic hepatitis C constituted 23.05 [15.44–38.05] ng/mL and showed its insufficiency. It was found that the optimum vitamin D level was significantly more frequently observed in children with inactive hepatitis as compared to the group of children with ALT-3N (χ2=3.97, р=0.04). A significant negative correlation was found between the activity of hepatitis and vitamin D concentration in the blood serum of children with chronic hepatitis С (R0.05) revealed. Conclusions. In the majority (60.7%) of children with chronic hepatitis C, a reduced 25(OH)D in serum was revealed. It is shown that vitamin D status in children with this disease had an inverse correlation with the biochemical activity of hepatitis (according to transaminases level). There was no statistically significant difference between the vitamin D status of children with chronic hepatitis C, depending upon gender, age, seasonality and place of residence.

Background Because of the high prevalence and absence of cure for infection, chronic hepatitis B virus (HBV) infection has been acknowledged as a pressing public health issue. Toll-like receptors (TLRs) activate the human innate immune system and the polymorphisms in TLRs may alter their function. The present study aimed to investigate the association between TLR polymorphisms and disease progression of chronic HBV infection. Methods During the study period, 211 treatment-naïve patients with chronic HBV infection were recruited, and blood samples were collected from each individual. Matrix-assisted laser desorption/ionization time of flight mass spectrometry was employed to genotype the selected TLR polymorphisms after human genome extraction. In addition, HbsAg, TNF-α, and IL-6 levels were quantified using enzyme linked immunosorbent assay (ELISA). Statistical analyses were conducted to investigate the association between TLR polymorphisms and hepatitis activity, liver function parameters, HbsAg level, and cytokine level. Results We did not observe any mutations in rs4986790, rs4986791, and rs5743708 among all study subjects. A logistic regression revealed that mutations in rs3804099 and rs4696480 were associated with milder hepatitis activity. Consistent with the logistic regression, improved liver function parameters and reduced level of both HbsAg and cytokines were also correlated with the mutant carriers of rs3804099 and rs4696480. Conclusions TLR mutations were significantly associated with milder hepatitis activity among patients with chronic HBV infection. Therefore, we conclude that the activation of TLR pathways may further intensify the inflammation of hepatocytes, and leads to progression of disease.

OBJECTIVEWe aimed to evaluate the role of elevated perioperative alanine aminotransferase (ALT) as a surrogate marker of hepatitis activity in determining the risk of recurrence and survival in hepatitis B-related hepatocellular carcinoma (HCC) after curative hepatectomy.METHODSA retrospective review of the hepatectomy database was performed and 142 patients were found who had hepatitis B-related HCC from January 2001 to March 2006. Their ALT levels preoperatively and 1 month, 3 months, and 6 months postoperatively were recorded. The risk factors for recurrence and prognostic factors of survival were analysed.RESULTSAn elevated perioperative ALT level (p = 0.021), multiple tumour nodules in the resected specimen (p < 0.001), and a tumour size greater than 5 cm (p = 0.001) were significant independent risk factors for tumour recurrence. The latter two factors were also independent prognostic factors for overall survival and disease-free survival. An elevated ALT level was an independent prognostic factor for disease-free survival (p = 0.025).CONCLUSIONAn elevated perioperative ALT level, which reflects increased hepatitis activity, is an independent risk factor for intrahepatic recurrence of hepatitis B-related HCC. It is also associated with a poorer disease-free survival rate.
