Haiman pään poistoleikkaus on vaativimpia vatsakirurgisia toimenpiteitä. Leikkausta käytetään erityisesti haiman ja pohjukaissuolen alueen syövissä pyrittäessä parantavaan hoitoon. Vaikka haimasyövän ennuste on edelleen huono, leikkaus antaa ainoan mahdollisuuden parantavaan hoitoon. Toinen tärkeä leikkauksen aihe on vaikea krooninen haimatulehdus, jossa kipuja ja muita ongelmia ei saa muilla keinoilla hallintaan. Haiman pään poistoleikkauksessa tehtävään haiman ja ohutsuolen liitokseen liittyy huomattava vaikeiden varhaisongelmien, jopa varhaiskuolleisuuden riski, minkä vuoksi vielä 80-luvulla pohdittiin vakavasti, pitäisikö toimenpiteestä kokonaan luopua. Tuolloin kuitenkin alettiin julkaista myös huomattavasti parempia varhaistuloksia. Nykyään hyväksytään laajalti käsitys, että haiman pään poistoleikkaus on turvallinen toimenpide, kun se tehdään sairaaloissa, joissa on laaja kokemus näistä toimenpiteistä, sellaisten kirurgien toimesta, jotka tekevät näitä toimenpiteitä riittävän usein. Haima-ohutsuoliliitoksen aukipysymisestä ja toimimisesta myöhäisvaiheessa on vähän tietoa. Ahtautuessaan se voi aiheuttaa kipuja, laihtumista ja ripulia. Tässä tutkimuksessa selvitettiin haiman pään poistoleikkausten varhaistuloksia suomalaisissa sairaaloissa. Tutkimuksen havainnot tukivat aiempia havaintoja siitä, että leikkauksessa tehtävän haimasuoliliitoksen hyvällä paranemisella on keskeinen merkitys leikkauksesta toipumisessa. Edelleen tutkimus vahvisti pääosin ulkomaisia havaintoja siitä, että näiden leikkausten keskittäminen ns. suuren volyymin sairaaloihin parantaa hoitotuloksia. Haimaleikattujen seurantatutkimuksessa havaittiin, että merkittävällä osalla potilaita leikkauksessa tehty haimasuoliliitos oli myöhemmin ahtautunut, mikä johtaa haiman erittämien ruuansulatusentsyymien vajaukseen. Toisaalta osalla potilaita tämä leikkauksen jälkeinen haiman vajaatoiminta johtui itse jäljellä olevan haiman alentuneesta toiminnasta mihin leikkausliitoksella ei voida vaikuttaa. Vaikka osalla potilaita oli leikkauksen jälkeisiä haittoja, ei haimaleikattujen potilaiden yleisessä elämänlaadussa kuitenkaan todettu merkittävää eroa ikävakioituun suomalaiseen väestöön verrattuna. Eräs keino parantaa haimaleikkauksessa tehtävän haimasuoliliitoksen toimivuutta ja sitä kautta jopa leikkauksen pitkäaikaistuloksia saattaisi olla elimistössä hajoavan sisäisen tuen (proteesin) käyttö haimaliitoksen suojana. Tutkimuksessa todettiin tällaisen haimasuoliliitoksessa käytettävän biohajoavan proteesin valmistaminen mahdolliseksi. Samoin tutkittiin biohajoavasta materiaalista tehdyn spiraalin käyttäytymistä sappi- ja haimanestettä muistuttavissa olosuhteissa ja todettiin mm. ympäristön happamuudella olevan vaikutusta proteesimateriaalin hajoamisnopeuteen. This study addresses early and long-term complications that may be linked to technical problems of pancreaticojejunostomy. Given the importance of the leakage of the pancreaticojejunostomy in increasing postoperative mortality and morbidity, a study was performed in which it was found that biliary leakage usually occurs together with pancreatic leakage. This gave rise to a hypothesis of a possible common aetiologic factor, which may be postoperative pancreatitis. To investigate the effect of surgeon and hospital volume on postoperative morbidity and mortality, a nation-wide study was performed including 33 Finnish hospitals, where resections of the head of the pancreas (average 2.1 resections per year per hospital) were performed during a five-year period (1990-1994). There were 98 surgeons performing these resections (average 0.7 resections per year per surgeon). Most of the complications that resulted in death were a result of technical problems. Both surgeon and hospital volume affected postoperative morbidity and mortality. To reduce postoperative morbidity, mortality and hospital stay pancreatic head surgery needs to be centralized in fewer hospitals with adequate perioperative care and performed by surgeons professing special interest in pancreatic surgery. Long-term results, however, were related to the nature of the disease. Given that most patients undergoing resection of the pancreatic head show severe signs of pancreatic insufficiency, a dynamic secretin-stimulated magnetic resonance cholangiopancreatography (D-MRCP) examination was performed to examine whether these symptoms were a result of anastomotic stricture or due to malfunction of the pancreatic gland. This study was performed on 26 patients who 3-76 (median 52) months earlier had undergone pancreaticoduodenectomy with end-to-end invaginated anastomosis, which was the method of choice in the 1990s. All those patients had severe exocrine insufficiency as measured by faecal elastase-1. D-MRCP failed in three patients, but revealed severe glandular malfunction in seven patients (30 %), total obstruction of the pancreaticojejunostomy in five patients (22 %), partial obstruction in six patients (26 %) and an open anastomosis in five patients (22 %), these five patients having the highest faecal elastase-1 values. Better long-term functional results may require better surgical technique. To study postoperative quality of life in patients who had undergone pancreaticoduodenal resection, 15D, a generic instrument was used which enabled comparison of the results to healthy population. In the early stage (24 months postoperatively), postoperative health-related quality of life was significantly poorer than in general population, but there was no difference in the late stage (110 months postoperatively). As a possible solution to protect the pancreaticojejunal anastomosis, biodegradable, barium sulphate containing stents were manufactured to be inserted in the pancreatic duct during the operation. The degradability of these stents was followed-up in vitro in different pH and enzyme milieus and the stents were tested in vivo in two pilot patients. Degradation of the stents occurred after 24-52 weeks of incubation. Alkaline milieu together with the presence of pancreatic enzyme degraded the stents twice as fast as when either alkaline milieu or enzymes alone were present. In the milieu resembling pancreatic juice barium sulphate had no effect on the degradation time. Neither of the pilot patients experienced any postoperative complications.

Haiman kirurgisessa käsittelyssä vaurion välttämiseen kiinnitettävä erityistä huomiota. Haimakasvainten kirurginen hoito on haastavaa ja leikkauksiin liittyy huomattava komplikaatioiden riski. Haima on erityisen herkästi vaurioituva elin, jonka vauriomekanismit ovat kuitenkin melko huonosti tunnettuja. Suurin osa komplikaatioista liittyy kirurgisesti rakennettuun haima-suoliliitokseen. Väitöstutkimuksessa selvitettiin kokeellisesti eläinmalleja käyttäen haima-ohutsuoliliitoksen rakentamiseen liittyvien kirurgisten vaiheiden: haiman katkaisun, ompelun ja liimaamisen sekä biohajoavan stentin käytön vaikutusta haimavaurion kehittymiseen. Kokeellisten töiden pohjalta kehitettiin tekniikka haima-ohutsuoliliitoksen rakentamiseksi. Päämetodeina kokeellisissa töissä käytettiin seerumin amylaasiaktiivisuutta ja haimahistologiaa. Uusi biohajoava röntgenpositiivinen stentti osoittautui kudosyhteensopivaksi sekä rotan haimaparenkyymissä että sian haimatiehyessä ja sen hajoamista oli mahdollista seurata röntgenkuvauksilla. Haiman leikkaaminen käyttäen perinteistä veistä sai aikaan vähemmän kudosvauriota kuin leikkaaminen ultraäänisaksilla tai diatermialla riippumatta käytetystä hemostaasimenetelmästä. Haiman ompelu oli vähiten kudosta vaurioittavaa, kun käytettiin ohuempaa lankaa, vähemmän ompeleita ja pienempää solmunkireyttä. Syanoakrylaattipohjaiset kudosliimat aiheuttivat merkittävää haimavauriota sekä tulehduksen että asinussoluvaurion muodossa. Fibriinipohjaiset kudosliimat aiheuttivat lievemmän, mutta selkeän, haimavaurion. Liimoilla aikaansaadun haima-suoliliitoksen vetolujuuksissa oli selviä eroja. Kokeellisten osatöiden yhteinen löydös oli, että niin haiman leikkaus, haiman ompelu kuin suolen liimaaminen haiman pintaan aiheuttivat laajamittaisen tulehdus- ja vaurioreaktion kudoksessa. Histologisia muutoksia oli sekä interventiokohdassa että toisesta päästä haimaa otetuissa näytteissä. Interventiot aiheuttivat myös väliaikaisen amylaasiarvon kohoamisen seerumissa. Myös kontrollimielessä tehdyssä sarjassa, jossa jäljiteltiin interventioryhmien leikkausoperaatiota haiman käsittelyssä mutta ilman interventiota, havaittiin lievä haiman tulehdusreaktio ja amylaasimuutokset. Lopuksi kehitettiin leikkaustekniikka, jota varten valittiin kokeellisten töiden perusteella vähiten haimaa vaurioittavat kirurgiset menetelmät. Tekniikassa haima-suoliliitos rakennettiin käyttäen biohajoavaa stenttiä haimatiehyessä ja ompelematta haimakudosta. Tekniikka todettiin mahdolliseksi toteuttaa kolmella potilaalla. Johtopäätöksenä voidaan todeta, että haima on erittäin herkkä elin ja pienikin interventio voi aiheuttaa siihen laajamittaista tulehdusta ja kudosvaurioita. Haimakirurgian korkean morbiditeetti vuoksi uusille leikkaustekniikoille on tarvetta. Väitöstutkimuksessa kuvattu leikkaustekniikka tulee jatkossa tutkia turvallisuudeltaan ja teholtaan kliinisissä sarjoissa. Complications originating from reconstructed pancreaticojejunal anastomosis after pancreaticoduodenectomy are relatively common. The pancreas is known to be an easily irritated and vulnerable organ. However, the development of pancreatic injury and the association of the injury with complications are not comprehensively known although several factors have been identified in the course of injury response. The aim of this thesis was to experimentally investigate pancreatic injury response to the various methods used in the major surgical procedures during pancreaticoduodenectomy including resecting, suturing and gluing the pancreas, and the use of a biodegradable pancreatic stent. The biocompatibility of the new biodegradable polylactide stent was investigated both when a stent fibre was punctured into the pancreatic parenchyma of rat and as a braided stent when inserted into the pancreatic duct of swine. The stent material was non-toxic in the parenchyma and the duct but a widespread inflammatory response was observed after parenchymal puncture regardless of whether the punctured material was same size of stent fibre or reference stainless steel needle. This finding led to studies where several surgical methods were investigated and compared regarding the pancreatic injury response they induce. Resection of the pancreatic tissue with a surgical knife, independent of the used haemostasis method, was found to induce less histological injury than resection with ultrasonic scissors or electrocautery. Suturing the pancreatic parenchyma was least injurious when thinner thread, fewer suture passes and less force at the suture loops were used. Gluing a loop of the jejunum on the side of the pancreas using cyanoacrylate derivatives induced severe inflammation and acinar cell damage in the tissue. Fibrin glues induced similar but milder histological changes in the pancreas. Only mild changes were seen in the bowel wall. Tensile strength of the created junction increased by time exceeding 1 newton with most of the tissue adhesives tested, but the harmful tissue effects observed compromise their use in pancreatic surgery. The common finding from the experimental Studies I and III-V was that each type of intervention and also sham-operation, where the surgical procedure was mimicked without the intervention itself, induced temporary hyperamylasaemia and widespread injury response in the pancreas. This spreading was identified from observation of similar but milder histological changes in tissue specimens taken remote from the primary intervention site with each type of intervention. On the basis of the five experimental studies, a modified technique for pancreaticojejunostomy was developed by choosing the least injurious methods in each major surgical procedure. A modified pancreaticojejunostomy with the biodegradable pancreatic duct stent and without sutures through the pancreatic parenchyma was successfully tested in three human pilots and was found to be an applicable technique. This modified anastomosis needs to undergo further phase I and phase II trials to test its safety and efficacy.
