Movement-evoked pain is an understudied manifestation of musculoskeletal conditions that contributes to disability, yet little is known about how the neuromuscular system responds to movement-evoked pain. The present study examined whether movement-evoked pain impacts force production, electromyographic (EMG) muscle activity, and the rate of force development (RFD) during submaximal muscle contractions. Fifteen healthy adults (9 males; age = 30.3 ± 10.2 yr, range = 22–59 yr) performed submaximal isometric first finger abduction contractions without pain (baseline) and with movement-evoked pain induced by laser stimulation to the dorsum of the hand. Normalized force (% maximal voluntary contraction) and RFD decreased by 11% (P < 0.001) and 15% (P = 0.003), respectively, with movement-evoked pain, without any change in normalized peak EMG (P = 0.77). Early contractile RFD, force impulse, and corresponding EMG amplitude computed within time segments of 50, 100, 150, and 200 ms relative to the onset of movement were also unaffected by movement-evoked pain (P > 0.05). Our results demonstrate that movement-evoked pain impairs peak characteristics and not early measures of submaximal force production and RFD, without affecting EMG activity (peak and early). Possible explanations for the stability in EMG with reduced force include antagonist coactivation and a reorganization of motoneuronal activation strategy, which is discussed here. NEW & NOTEWORTHY: We provide neurophysiological evidence to indicate that peak force and rate of force development are reduced by movement-evoked pain despite a lack of change in EMG and early rapid force development in the first dorsal interosseous muscle. Abstract Background Thumb carpometacarpal osteoarthritis (CMC OA) is characterized by chronic progressive degeneration of the joint cartilage, with high prevalence. Patients present with pain at the base of the thumb, morning stiffness, and muscle weakness, symptoms that affect hand function and therefore interfere in activities and social participation. Movements that involve grip or lateral pinch are the most affected and directly impact independence, self-care, and leisure activities. In the literature consulted, several protocols with exercises for these patients were found. However, most do not compare the same intervention modality and only provide basic methodological information, with no consistent information on training load and load progression. In addition, most protocols only address the strengthening of the abductor and extensor thumb muscles and pinching or grasping exercises. However, some biomechanical and electromyographic studies have demonstrated the important role of the first dorsal interosseous muscles as stabilizers of the thumb carpometacarpal joint. Methods This is a randomized, controlled, double-blind, and parallel clinical trial that will include 56 participants, over 40 years old, with radiographic evidence of thumb base osteoarthritis. Participants will be randomly allocated into two groups: control and intervention. The following evaluations will be conducted: the Australian/Canadian Hand Osteoarthritis Index, Canadian Occupational Performance Measure, Nine-Hole Peg Test, grip and pinch strength associated with muscle activation assessment, and Bilateral Upper Limb Function Test at four different times: baseline, session 13, session 18, and follow-up. Treatment will take place over 6 weeks, with reassessments in the fourth and sixth weeks and 3 months after the end of the intervention (follow-up). Qualitative variables will be expressed as frequency and percentage, and quantitative variables as mean and standard deviation. Introduction/Aims: Age can affect hand muscles non‐uniformly. We investigated the influence of age on the compound muscle action potential (CMAP) amplitude of the hand muscles and the derived split‐hand index (SHI). Methods: We studied 244 subjects investigated for suspected myasthenia gravis but without neuromuscular disorders. Abductor pollicis brevis (APB), first dorsal interosseous (FDI), and abductor digiti minimi (ADM) CMAPs were obtained by supramaximal stimulation at the wrist, recording with surface electrodes while checking the best recording site. We applied Tukey's HSD and Kruskal–Wallis one‐way analysis of variance for comparing age groups defined by median and interquantile ranges (IQRs). Spearman's rank correlation coefficient and linear regression were used for testing age‐dependence of measurements. Results: Median age was 61.5 y (first IQR, 44.5; third IQR, 72.0; range 18–89). Age and neurophysiological measurements were similar between genders. APBCMAP, FDICMAP, ADMCMAP, and SHI were correlated with age (P <.001). Median and cutoff values were significantly different between age groups. APBCMAP, FDICMAP, and ADMCMAP decreased by 0.8/0.7/0.3 mV/y, respectively, and SHI decreased 0.15/y. Discussion: The CMAP amplitudes of hand muscles and derived SHI were strongly age‐dependent, although this effect was less in ADM. This represents a physiological phenomenon. Incomplete spinal cord injury (SCI) may result in muscle weakness and difficulties with force gradation. Although these impairments arise from the injury and subsequent changes at spinal levels, changes have also been demonstrated in the brain. Blood-oxygen-level dependent (BOLD) imaging was used to investigate these changes in brain activation in the context of unimanual contractions with the first dorsal interosseous muscle. BOLD- and force data were obtained in 19 individuals with SCI (AISA Impairment Scale [AIS] C/D, level C4–C8) and 24 able-bodied controls during maximal voluntary contractions (MVCs). To assess force modulation, participants performed 12 submaximal contractions with each hand (at 10, 30, 50, and 70% MVC) by matching their force level to a visual target. MVCs were weaker in the SCI group (both hands p < 0.001), but BOLD activation did not differ between SCI and control groups. For the submaximal contractions, force (as %MVC) was similar across groups. However, SCI participants showed increased activity of the ipsilateral motor cortex and contralateral cerebellum across all contractions, with no differential effect of force level. Activity of ipsilateral M1 was best explained by force of the target hand (vs. the non-target hand). In conclusion, the data suggest that after incomplete cervical SCI, individuals remain capable of producing maximal supraspinal drive and are able to modulate this drive adequately. Activity of the ipsilateral motor network appears to be task related, although it remains uncertain how this activity contributes to task performance and whether this effect could potentially be harnessed to improve motor functioning. The current study aimed to investigate the remote effects of upper trapezius (UT) Dry needling (DN) and ischemic pressure (IP) on the pain intensity and threshold of the MTrPs in the extensor carpi radialis longus (ECRL) and the first dorsal interosseous (FDI) muscles. Twenty-eight subjects were allocated into two groups in a randomized clinical trial study undergoing DN and IP interventions on the MTrPs of the UT muscle for 3 sessions. Pressure pain intensity was assessed by visual analog scale (VAS) and pressure pain threshold (PPT) before and after interventions. Upper limb functions were evaluated by the disabilities of the arm, shoulder, and hand (DASH) questionnaire. There were significant interactions between muscle, group, and time for PPT, muscle and group, group and time, and muscle and time for VAS (P < 0.05). In DN group, PPTs and VAS of the ECRL and FDI were changed after treatment in all sessions (P < 0.05). In ECRL and FDI muscles, the DN was more effective than IP (P < 0.05). Comparing the pain intensity and threshold of the ECRL, FDI, and UT muscles it was revealed that the improvement in symptoms of the ECRL muscle was the most in the DN group (P < 0.05). There was a significant interaction between group and time for DASH (P < 0.05). In both groups, the DASH score was reduced (P < 0.001). UT muscle DN demonstrated significantly better remote effects in alleviating symptoms of the ipsilateral distal muscles than IP. Both interventions were effective in improving the upper limb function in myofascial pain syndrome patients. • This study evaluated the remote effects of most common treatments on myofascial trigger points. • Dry needling showed therapeutic effects on pain alleviation in terms of both immediate and short term in the extensor carpi radialis longus muscle. • First dorsal interosseous pain threshold and intensity improved following dry needling of the upper trapezius while it is not connected to the upper trapezius muscle myofascially. The purpose of this study was to examine possible differences in motor unit action potential amplitudes (MUAP AMPS) and firing rates of the first dorsal interosseous (FDI) in male and female children aged 8–10 years. Eight male (mean ± SD, age = 8.8 ± 0.7 yrs; BMI = 16.5 ± 1.3 kg/m2) and eight female (age = 9.3 ± 0.9 yrs; BMI = 16.1 ± 1.5 kg/m2) children volunteered to complete isometric trapezoidal muscle actions of the first dorsal interosseous at 50% of maximal voluntary contraction (MVC). Electromyographic signals were decomposed to yield MUAP AMPS and mean firing rates (MFR) at the targeted force. An exponential model was fitted to the MUAP AMPS vs. recruitment threshold (RT) while linear models were fitted to the MFRs vs. RT relationships for each subject. Ultrasonography determined the muscle cross-sectional area (CSA) of the FDI. Independent samples t -tests were used to examine possible differences between the male and female children for MVC strength, CSA, and the coefficients from the MU relationships. There were no differences in MVC strength, CSA, or the MUAP AMP vs. RT relationships between the male and female children (P < 0.05). Males, however, had greater MFRs of lower-threshold MUs as evident by significantly larger y-intercepts (P = 0.019) and more negative slopes (P = 0.004) from the MFR vs. RT relationships. Despite no differences in muscle strength, CSA, and MUAP AMPS , differences in firing rates existed between male and female children aged 8–10 years. Patients with mild traumatic brain injury (mTBI) are frequently affected by fatigue. However, hardly any data is available on the fatigability of the motor system. We evaluated fatigue using the Fatigue Severity Scale (FSS) and Modified Fatigue Impact Scale (MFIS) questionnaires in 20 participants with mTBI (>3 months post injury; 8 females) and 20 age- and sex matched controls. Furthermore, index finger abduction force and electromyography of the first dorsal interosseous muscle of the right hand were measured during brief and sustained maximal voluntary contractions (MVC). Double pulse stimulation (100 Hz) was applied to the ulnar nerve to evoke doublet-forces before and after the sustained contraction. Seven superimposed twitches were evoked during the sustained MVC to quantify voluntary muscle activation. mTBI participants reported higher FSS scores (mTBI: 5.2 ± 0.8 SD vs. control: 2.8 ± 0.8 SD; P < 0.01). During the sustained MVC, force declined to similar levels in mTBI (30.0 ± 9.9% MVC) and control participants (32.7 ± 9.8% MVC, P = 0.37). The decline in doublet-forces after the sustained MVC (mTBI: to 37.2 ± 12.1 vs. control: to 41.4 ± 14.0% reference doublet, P = 0.32) and the superimposed twitches evoked during the sustained MVC (mTBI: median 9.3, range: 2.2–32.9 vs. control: median 10.3, range: 1.9–31.0% doubletpre, P = 0.34) also did not differ between groups. Force decline was associated with decline in doublet-force (R2 = 0.50, P < 0.01) for both groups. Including a measure of voluntary muscle activation resulted in more explained variance for mTBI participants only. No associations between self-reported fatigue and force decline or voluntary muscle activation were found in mTBI participants. However, the physical subdomain of the MFIS was associated with the decline in doublet-force after the sustained MVC (R2 = 0.23, P = 0.04). These results indicate that after mTBI, increased levels of self-reported physical fatigue reflected increased fatigability due to changes in peripheral muscle properties, but not force decline or muscle activation. Additionally, muscle activation was more important to explain the decline in voluntary force (performance fatigability) after mTBI than in control participants.

Summary: Motor unit action potential size (MUAPsize) versus recruitment threshold (RT) relationship analysis provides a non‐invasive measure of motor unit (MU) hypertrophy; however, this method's ability to identify MU atrophy is unknown. This investigation sought to determine if MUAPsize versus RT relationship slope (APslope) comparison could identify evidence of MU atrophy in older individuals. Surface electromyography signals were recorded from the first dorsal interosseous (FDI) of fourteen young (YG, age = 22·29 ± 2·79 years) and ten older (OG, 61·0 ± 2·0 years) subjects during a 50% maximal voluntary contraction (MVC) isometric trapezoidal muscle action. The signals were decomposed to yield a MUAPsize and RT for each MU. For each subject, the MUs recruited between 10% and 50% MVC were linearly regressed as a function of RT to calculate an individual APslope. FDI cross‐sectional area (CSA) and echo intensity (EI) were quantified via ultrasonography. The mean APslope was lower for OG (0·033 ± 0·010 mV %MVC−1) than YG (0·056 ± 0·019 mV %MVC−1). OG and YG possessed similar CSAs (OG: 2·09 ± 0·31 cm2; YG: 2·08 ± 0·41 cm2); however, OG (53·25 ± 7·56 AU) had greater EI than YG (43·87 ± 7·59 AU). The lower OG mean APslope was due to smaller MUAPsizes of higher‐threshold MUs, likely due to atrophy of muscle fibres that comprise those MUs. In support, similar CSA with greater EI indicated increased adipose and fibrous tissue and reduced contractile tissue in OG. Background: Therapy programs to treat thumb carpometacarpal (CMC) arthritis may engage selective activation and reeducation of thenar muscles, particularly the first dorsal interosseous (FDI) and opponens pollicis (OP) to reduce subluxation of the joint. We describe the effect of simulated selective activation of the FDI and OP muscles upon radiographic subluxation of the thumb CMC joint. Methods: In a cadaver model of CMC subluxation, loads were applied to the FDI, the OP, and then concomitantly at 0%, 25%, 50%, 75%, and 100% maximal loads and radial subluxation of the joint and reduction in subluxation was measured. Results: Selective activation of the OP, alone, improved the subluxation ratio (SR) in a dose-dependent manner. Selective activation of FDI, alone, demonstrated minimal effects on SR. Concomitant activation of OP and FDI improved the SR across all loading states, and activation of 75% and greater, when compared with FDI activation alone, resulted in a statistically significant improvement in SR to within 10% of the presubluxed joint. Conclusions: Concomitant activation of the FDI and OP acts to reduce subluxation of the thumb CMC joint in a dose-dependent fashion. The OP is likely the predominant reducing force. Hand therapy programs that focus on selective strengthening programs likely function in part to encourage patients to activate the easily palpable and easily understood FDI. Concomitant coactivation of the OP may be the major reducing force to elicit clinical and radiographic reduction of subluxation, improved thumb positioning, and reduction of pain and arthritic symptoms. The amplitude of motor-evoked potentials (MEPs) elicited with transcranial magnetic stimulation (TMS) varies from trial-to-trial. Synchronous oscillations in cortical neuronal excitability contribute to this variability, however it is not known how different frequencies of stimulation influence MEP variability, and whether these oscillations are rhythmic or aperiodic. We stimulated the motor cortex with TMS at different regular (i.e., rhythmic) rates, and compared this with pseudo-random (aperiodic) timing. In 18 subjects, TMS was applied at three regular frequencies (0.05 Hz, 0.2 Hz, 1 Hz) and one aperiodic frequency (mean 0.2 Hz). MEPs (n = 50) were recorded from three intrinsic hand muscles of the left hand with different functional and anatomical relations. MEP amplitude correlation was highest for the functionally related muscle pair, less for the anatomically related muscle pair and least for the functionally- and anatomicallyunrelated muscle pair. MEP correlations were greatest with 1 Hz, and least for stimulation at 0.05 Hz. Corticospinal neuron synchrony is higher with shorter TMS intervals. Further, corticospinal neuron synchrony is similar irrespective of whether the stimulation is periodic or aperiodic. These findings suggest TMS frequency is a crucial consideration for studies using TMS to probe correlated activity between muscle pairs. [ABSTRACT FROM AUTHOR]

Objectives Two commonly-used methods for setting stimulus intensities in transcranial magnetic brain stimulation studies were compared to determine which best approximated a motor evoked potential (MEP) of 50% of the maximal MEP amplitude (SI 50 ); a suprathreshold intensity relative to resting motor threshold (rMT) or adjusting the intensity to evoke an MEP amplitude of 1 mV. Methods Corticomotor stimulus–response curves and rMT for the right first dorsal interosseous (FDI) muscle of 176 subjects (aged 10–74 years) were retrospectively analysed. Results Regardless of subject age or sex, SI 50 occurred at 127.5 ± 11.3% rMT. Except in young children, MEPs of 1 mV were significantly smaller than those evoked at SI 50 . Conclusions In the inactive FDI muscle, a stimulus intensity of 127–128% rMT consistently gives the best approximation of SI 50 in most subjects, except perhaps young children. Significance Setting TMS stimulus intensities relative to rMT provides a less variable inter-subject comparator, with respect to individual differences in corticomotor input–output characteristics, than adjusting the stimulator output to give an absolute MEP magnitude. Introduction: Hand therapists selectively strengthen the first dorsal interosseus (FDI) to stabilize arthritic joints yet the role of the FDI has not yet been radiographically validated. Purpose: To determine if FDI contraction reduces radial subluxation (RS) of the thumb metacarpal (MC). Methods: Fluoroscopy was used to obtain true anterior-posterior radiographs of non-arthritic CMC joints: 1) at rest, 2) while stressed and 3) while stressed with maximal FDI contraction. Maximal FDI strength during CMC stress and thumb MC RS and trapezial articular width were measured. The ratio of RS to the articular width was calculated. Results: Seventeen participants (5 male, 12 female) participated. Subluxation of a stressed CMC significantly reduced and the subluxation to articular width ratio significantly improved after FDI activation. Conclusions: Contraction of the FDI appears to radiographically reduce subluxation of the healthy thumb CMC joint. In the context of sexual and natural selection, an allocation trade-off for carotenoid pigments may exist because of their obligate dietary origin and their role both in the antioxidant and immune systems and in the production of coloured signals in various taxa, particularly birds. When birds have expended large amounts of carotenoids to feather growth such as after autumn moult, bird health and oxidative status might be more constrained. We tested this hypothesis in a bird species with carotenoid-based plumage colour, by manipulating dietary carotenoids and physical activity, which can decrease antioxidant capacity and increase reactive oxygen metabolite (ROM) concentration. Great tits were captured after moult and kept in aviaries, under three treatments: physical handicap and dietary supplementation with carotenoids, physical handicap and control diet, and no handicap and control diet. We measured plasma composition (antioxidant capacity, ROM concentration, and vitamin A, vitamin E and total carotenoid concentrations), immune system activation (blood sedimentation) and stress response (heterophil/ lymphocyte ratio) and predicted that handicap treatment should influence these negatively and carotenoid supplementation positively. Coloration of yellow feathers was also measured. Carotenoid supplementation increased total plasma carotenoid concentration, decreased feather carotenoid chroma and marginally increased ROM concentration. Handicap increased blood sedimentation only in males but had no clear influence on oxidative stress, which contradicted previous studies. Further studies are needed to investigate how physical activity and carotenoid availability might interact and influence oxidative stress outside the moult period, and their combined potential influence on attractiveness and reproductive investment later during the breeding season. Muscle produces force by forming cross-bridges, using energy released from ATP. While the magnitude and duration of force production primarily determine the energy requirement, nearly a century ago Fenn observed that muscle shortening or lengthening influenced energetic cost of contraction. When work is done by the muscle, the energy cost is increased and when work is done on the muscle the energy cost is reduced. However, the magnitude of the 'Fenn effect' and its mirror ('negative Fenn effect') have not been quantitatively resolved. We describe a new technique coupling magnetic resonance spectroscopy with an in vivo force clamp that can directly quantify the Fenn effect [E=I+W, energy liberated (E) equals the energy cost of isometric force production (I) plus the work done (W)] and the negative Fenn effect (E=I-W) for one muscle, the first dorsal interosseous (FDI). ATP cost was measured during a series of contractions, each of which occurred at a constant force and for a constant duration, thus constant force-time integral (FTI). In all subjects, as the FTI increased with load, there was a proportional linear increase in energy cost. In addition, the cost of producing force greatly increased when the muscle shortened, and was slightly reduced during lengthening contraction. These results, though limited to a single muscle, contraction velocity and muscle length change, do quantitatively support the Fenn effect. Hemispheric brain injury resulting from a stroke is often accompanied by muscle weakness in contralateral limbs. In neurologically intact subjects, appropriate motoneuronal recruitment and rate modulation are utilized to optimize muscle force production. In the present study, we sought to determine whether weakness in an affected hand muscle in stroke survivors is partially attributable to alterations in the control of muscle activation. Specifically, our goal was to characterize whether the surface EMG amplitude was systematically larger as a function of (low) force in paretic hand muscles as compared to contralateral muscles in the same subject. We tested a multifunctional muscle, the first dorsal interosseous (FDI), in multiple directions about the second metacarpophalangeal joint in ten hemiparetic and six neurologically intact subjects. In six of the ten stroke subjects, the EMG-force slope was significantly greater on the affected side as compared to the contralateral side, as well as compared to neurologically intact subjects. An unexpected set of results was a nonlinear relation between recorded EMG and generated force commonly observed in the paretic FDI, even at very low-force levels. We discuss possible experimental as well as physiological factors that may contribute to an increased EMG-force slope, concluding that changes in motor unit (MU) control are the most likely reasons for the observed changes. In the present study, we investigated whether weakness in contralesional hand muscle in stroke survivors is partially attributable to alterations in motor unit activation, including alterations in firing rate modulation range. The afterhyperpolarization (AHP) potential of a motoneuron is a primary determinant of motoneuron firing rate. We examined differences in AHP duration in motoneurons innervating paretic and less impaired (contralateral) limb muscles of hemiparetic stroke survivors as well as in control subjects. A novel surface EMG (sEMG) electrode was used to record motor units from the first dorsal interosseous muscle. The sEMG data were subsequently decomposed to derive single-motor unit events, which were then utilized to produce interval (ISI) histograms of the motoneuron discharges. A modified version of interval death rate (IDR) analysis was used to estimate AHP duration. Results from data analyses performed on both arms of 11 stroke subjects and in 7 age-matched control subjects suggest that AHP duration is significantly longer for motor units innervating paretic muscle compared with units in contralateral muscles and in units of intact subjects. These results were supported by a coefficient of variation (CV) analysis showing that paretic motor unit discharges have a lower CV than either contralateral or control units. This study suggests that after stroke biophysical changes occur at the motoneuron level, potentially contributing to lower firing rates and potentially leading to less efficient force production in paretic muscles. Subject specific musculoskeletal models typically base some or all of their parameters on a source other than the subject being modeled. Evidence demonstrates that cadaveric measurements do not always scale appropriately to every subject, yet many musculoskeletal models still rely heavily on cadaveric based data. This study focused on the First Dorsal interosseous (FDI) given its unique function as the sole abductor of the second metacarpophalangeal joint. There were two purposes to this study: (1) to describe the procedures that can be used in vivo to determine the properties of a model of the FDI. (2). To determine the model parameters required to characterize the FDI for a group of four subjects. Parameters were determined using ultrasound imaging and a custom-built finger dynamometer. Some parameters were measured directly while other parameters had to be estimated using a least-squares criterion. For example, the parameters for the force-length properties were determined by fitting a model to experimentally determined data, with maximum isometric force values ranging from 86 to 102 N, and optimum lengths from 41 to 53 mm. It was shown that full characterization is possible for the FDI with parameters that are physiologically reasonable, but which showed variability between subjects. Abstract: The purpose of this study was to compare force variability and the neural activation of the agonist muscle during constant isometric contractions at different force levels when the amplitude of respiration and visual feedback were varied. Twenty young adults (20–32years, 10 men and 10 women) were instructed to accurately match a target force at 15% and 50% of their maximal voluntary contraction (MVC) with abduction of the index finger while controlling their respiration at different amplitudes (85%, 100% and 125% normal) in the presence and absence of visual feedback. Each trial lasted 22s and visual feedback was removed from 8–12 and 16–20s. Each subject performed three trials with each respiratory condition at each force level. Force variability was quantified as the standard deviation of the detrended force data. The neural activation of the first dorsal interosseus (FDI) was measured with bipolar surface electrodes placed distal to the innervation zone. Relative to normal respiration, force variability increased significantly only during high-amplitude respiration (∼63%). The increase in force variability from normal- to high-amplitude respiration was strongly associated with amplified force oscillations from 0 to 3Hz (R 2 ranged from .68 to .84, p < .001). Furthermore, the increase in force variability was exacerbated in the presence of visual feedback at 50% MVC (vision vs. no-vision: .97 vs. .87N) and was strongly associated with amplified force oscillations from 0 to 1Hz (R 2 = .82) and weakly associated with greater power from 12 to 30Hz (R 2 = .24) in the EMG of the agonist muscle. Our findings demonstrate that high-amplitude respiration and visual feedback of force interact and amplify force variability in young adults during moderate levels of effort. Evidence from human and animal studies suggests that motor neuron pool organization is not uniform for all motor tasks. Groupings of motor units within a muscle may be recruited differentially for a given task based on principles beyond anatomical or architectural features of the muscle alone. This study aimed to determine whether: (1) there was differential activation across locations of the first dorsal interosseous (FDI) muscle during a given task, (2) the differential activation was related to directional requirements and/or end goal of the task, and (3) there was an anatomical pattern to the differential activation. Twenty-six healthy right-handed participants carried out isometric finger/hand contractions in sitting while surface EMG was collected from 4 bipolar sites on the FDI muscle simultaneously. The tasks included: abduction, flexion, diagonal, 30% abduction + 30% flexion, 30% flexion + 30% abduction, key pinch, and power grasp. Mean peak integrated EMG for each task was normalized to site and task specific mean M waves. Differential activation was evident across FDI sites based on movement direction, order of directional components within a combination condition, and end goal of the task. There was greatest activation in the distal ulnar site for all tasks. Additionally there was a trend toward an ordering effect in the amount of activation at each site: distal ulnar > distal radial > proximal radial > proximal ulnar. [ABSTRACT FROM AUTHOR]

The article presents a study which showed that corticospinal transmission to the motoneuron pool of first dorsal interosseous muscle can be changed by maximal voluntary contractions. Procedures were authorized by the local human research ethics committee and carried out according to the Declaration of Helsinki. Fifteen volunteers were excluded due to the lack of response to magnetic corticospinal tract stimulation at the cervicomedullary junction. It is mentioned that premotoneuronal mechanisms have a role besides altered motoneuron excitability.

Abstract: Accurate in vivo estimation of muscle volume is important as it indicates the amount of power a muscle can produce. By tracking muscle volume changes in vivo, a muscle''s response to disease or rehabilitation training can be quantified. The purpose of this study was to validate the use of imaging ultrasound to estimate the volume of a small muscle, specifically the First Dorsal Interosseous (FDI) muscle. The perimeter of the FDI was imaged using ultrasound in 22 cadaver hands. For each FDI, serial cross-sectional areas were determined by manual digitization, volumes were then estimated using the Cavalieri principle. The muscles were then dissected from the cadavers, and muscle volume was determined via the water displacement method. The water displacement measures of muscle volumes were used as the criterion, and compared with those estimated via ultrasound. A Bland–Altman plot illustrated that all measures fell within the 95% confidence interval, with no statistical evidence of changes in measurement accuracy with size of specimen, or of a constant deviation in the accuracy of estimated volumes. For superficial muscles these results indicate that ultrasound imaging is an accurate method for determining muscle volumes in vivo even for a relatively small muscle (volume ∼4mL). Muscle architecture is considered to reflect the function of muscle in vivo, and is important for example to clinicians in designing tendon-transfer and tendon-lengthening surgeries. The purpose of this study was to quantify the architectural properties of the FDI muscle. It is hypothesized that there will be consistency, that is low variability, in the architectural parameters used to describe the first dorsal interosseous muscle because of its clear functional role in index finger motion. The important architectural parameters identified were those required to characterize a muscle adequately by modeling. Specifically the mass, cross-sectional area, and length of the tendon and muscle were measured in cadavers along with the muscle fiber optimum length and pennation angle, and the moment arm of the first dorsal interosseous at the metacarpophalangeal joint. These parameters provide a characterization of the architecture of the first dorsal interosseous, and were used to indicate the inherent variability between samples. The results demonstrated a large amount of variability for all architectural parameters measured; leading to a rejection of the hypothesis. Ratios designed to describe the functioning of the muscles in vivo, for example the ratio of tendon to fiber optimum lengths, also demonstrated a large variability. The results suggest that function cannot be deduced from form for the first dorsal interosseous, and that subject-specific architectural parameters may be necessary for the formulation of accurate musculoskeletal models or making clinical decisions. Abstract: We investigated the effects of immobilization on the maximal motoneuronal firing rate recorded from the first dorsal interosseous (FDI) during voluntary isometric contraction. In five human subjects, the middle finger, index finger, and thumb were immobilized for 1 week in a fiber-glass cast, which kept FDI in a shortened position. During a maximal voluntary contraction, single muscle-fiber action potentials were recorded using a tungsten microelectrode, and mean firing rate was calculated for each action-potential train. Three recording sessions were held: before immobilization (pre), after immobilization (post), and after a 1-week recovery period (recovery). The mean firing rate of FDI motoneurons during maximal voluntary contraction was decreased immediately after the 1-week immobilization (pre: 39.0±3.2Hz, number of detected spike trains (n)=353; post: 33.1±1.5Hz, n =285; p <0.05), and there was a return to control after the recovery period (40.2±3.4Hz, n =236). This suggests that the maximal motoneuronal firing rate achieved during maximal voluntary contraction is reduced after short-term immobilization. The functional implications and the contribution of this phenomenon to the immobilization-induced reduction in maximal voluntary force are discussed. [Copyright &y& Elsevier]

Interhemispheric inhibition between motor cortices is reduced in musicians. In the present study we have assessed intracortical inhibition (ICI) and facilitation (ICF) within ipsilateral motor cortex in 15 musicians and 15 non-musician controls. Transcranial magnetic stimulation (TMS) was used to elicit muscle evoked potentials (MEPs) from left first dorsal interosseous (FDI) muscle at rest, and during voluntary index finger abduction (0.5 N). Paired TMS with subthreshold conditioning was used to test early ICI with interstimulus intervals (ISIs) 1–5 ms, and ICF with ISIs 8–15 ms. Suprathreshold conditioning was used to test late ICI with ISIs 100–200 ms. TMS thresholds were similar in musicians and controls both at rest and with weak voluntary activation of FDI, indicating that postsynaptic excitability of corticospinal neurons was similar in both groups. ICI was less effective in musicians with FDI at rest and active, but only with an ISI of 3 ms. ICF was less effective in musicians under both rest and active conditions, and this was independent of ISI. There were no differences in late ICI between musicians and controls. We conclude that ICI and ICF circuits which are activated by weak TMS have less influence on corticospinal neuron excitability in musicians. Because of the dependence on ISI, the most likely explanation for the reduced ICI in musicians is an alteration of the interaction between the ICI circuit and neural elements responsible for the later I-waves evoked in corticospinal neurons by TMS. Excitability of the neural elements producing early and late ICI is not altered in musicians. Reduced ICF in musicians could be due to reduced excitability of neurons responsible for ICF, or an altered balance of excitatory inputs to corticospinal neurons which favours neurons that are not acted upon by the ICF circuit. The reduced influence of ICI and ICF circuits on corticospinal neuron excitability in musicians is likely to reflect a training-induced adaptation. It is not clear at present whether these differences represent an adaptive change related to their extraordinary control of finger movements, or alternatively a maladaptive change induced by "overuse" of the hands from extensive training. Objectives – To evaluate the use of fixed distance side to side comparison of abductor digiti minimi (ADM) and first dorsal interosseous (FDI) compound muscle action potential (CMAP) studies in the diagnosis of distal ulnar neuropthy. Materials and methods – Thirty normal controls underwent ADM and FDI CMAP studies bilaterally at a fixed ADM recording to stimulating point distance of 6 cm. Side to side mean latency differences to both muscles were calculated. Twenty patients with suspected unilateral distal ulnar neuropathy from routine nerve conduction studies had positive results when compared with controls. Results – The upper limit of normal for side to side mean latency difference at 3 SD above the mean was 0.394 and 0.474 ms for ADM and FDI, respectively. All 20 patients had side to side mean CMAP latency difference above 3 SDs in the ADM, FDI or both muscles. Fifty percent of cases had involvement of the superficial sensory branch. Conclusions – Fixed distance side to side CMAP latency comparison is a useful electrodiagnostic adjunct for distal ulnar neuropathy. Trauma was the most common aetiology in the 20 reported cases.
