This article analyzes and summarizes the incidence, harm, diagnosis methods, conservative treatment, surgical methods for relieving obstruction and follow-up continuous treatment measures of symptomatic urinary calculi during pregnancy, aiming to provide reference for further study on long-term medical treatment and nursing intervention pattern of the patients with symptomatic urinary calculi during pregnancy. The multidisciplinary collaboration and standardized diagnosis and treatment based on the evidence-base medicine may increase the safety during pregnancy and living quality of pregnant women with symptomatic urinary calculi, moreover provide more theoretical perspectives and research ideas. STUDY QUESTION Does treatment selection for cervical lesions affect the outcome of IVF/ICSI? SUMMARY ANSWER There was no difference in pregnancy outcome between treated and untreated groups, or between different types of IVF/ICSI treatment. WHAT IS KNOWN ALREADY Human papillomavirus (HPV) infection and HPV-induced cervical lesions are associated with decreased fertility, and cervical intraepithelial neoplasia (CIN) treatment may increase the risk of adverse pregnancy outcomes. STUDY DESIGN, SIZE, DURATION Between 2018 and 2020, 190 women with infertility who had abnormal HPV screening or cytology results prior to IVF/ICSI, and were diagnosed with CIN2/CIN3 by colposcopy biopsy at a tertiary hospital, were enrolled in a retrospective cohort study with follow-up until 31 December 2021. PARTICIPANTS/MATERIALS, SETTING, METHODS Patients with infertility who were diagnosed with CIN2/CIN3 by colposcopy biopsy were divided into the treatment and expectant management groups. The treatment group was divided into two intervention subgroups: the ablative therapy group and the surgical treatment group. The baseline data, number of oocytes retrieved, and rates of fertilization, high-quality embryos, positive serum HCG, clinical pregnancy, abortion, live birth, and cumulative pregnancy were compared among groups. MAIN RESULTS AND THE ROLE OF CHANCE Among the 190 patients included in the study, 152 were diagnosed with CIN2, and 38 patients had CIN3. There was no significant difference in the baseline data between the treatment and expectant groups. The time from confirmed lesions to the onset of gonadotrophin administration in the surgical treatment group was significantly longer than in the ablative therapy group and the expectant group (P  = 0.007 and P  = 0.024, respectively). For the treatment and expectant groups, respectively, the average number of oocytes retrieved (12.95 ± 8.77; 13.32 ± 9.16), fertilization rate (71.01 ± 23.86; 64.84 ± 26.24), and high-quality embryo rate (48.93 ± 30.72; 55.17 ± 34.13) did not differ, and no differences were detected between the different treatment subgroups. There were no differences among groups in rates of HCG positivity, clinical pregnancy, miscarriage, live birth, or cumulative pregnancy. The live birth rate in the surgical treatment group was slightly higher than that in the expectant groups (77.78% versus 66.67%), but the difference was not statistically significant. The 3-year cumulative pregnancy rates in the surgical treatment and expectant groups were 58.19% and 64.00%, respectively. LIMITATIONS, REASONS FOR CAUTION This is a retrospective study, which by nature can include selection bias, and the number of cases in the expectant group was <30, which may result in a false-negative result owing to the small sample size. WIDER IMPLICATIONS OF THE FINDINGS For patients with CIN2/CIN3, the treatment of cervical lesions does not affect the outcome of IVF/ICSI. Patients with CIN2 can enroll for IVF/ICSI cycles, with close follow-up to prevent the progression of cervical lesions, in order to avoid further delay in starting ART. For patients with CIN3, ovulation induction and embryo cryopreservation can be initiated as soon as possible after cervical lesions are treated, and frozen–thawed embryo transfer can be carried out 9–12 months later. Background: Missed miscarriage patients with residual tissue in the uterine cavity after medical abortion can be treated by surgery and expectant therapy. Because surgery may cause damage to the endometrium, expectant treatment is preferred clinically. However, the individual probability of expulsion of residual tissue from the uterus remains unelucidated. The present study used logistic regression analysis and nomogram to create a scoring system for quantitatively evaluating the individual probability of residual tissue expulsion in patients with missed miscarriage. Methods: A cohort of 126 patients with missed miscarriage who had residual tissue after medical abortion were included in the trial after receiving their informed consent. Of these patients, 102 and 24 patients had successful and failed expectant treatment, respectively. Results: Logistic regression analysis showed that gravidity, residual tissue size, and residual tissue with blood supply significantly affected the probability of expulsion of residual tissue after medical abortion. A special nomogram was constructed for predicting the probability of successful expulsion. At the maximum Youden index of 0.696, the area under the curve (AUC) of the receiver operating characteristic curve (ROC) was 0.849; this corresponded to a score of 77.78 and probability of residual tissue expulsion of 71%. Conclusion: A scoring system was established based on a nomogram. The success rate of residual tissue expulsion from the uterine cavity was significantly improved at the total score of 77.78 points after assigning influence weights to each influencing factor and an AUC of 0.849.

Objective To identify the factors affecting expectant management of early-onset preeclampsia, and evaluate the correlation between expectant treatment and foetal growth restriction.Materials and Methods The retrospective study included 72 women who were admitted for early-onset preeclampsia between February 2018 to April 2021. Data included maternal clinical parameters, demographic and maternal and neonatal outcomes, which were analysed for correlation.Results Multiple logistic regression analysis demonstrated that the time interval from the onset of 24-h proteinuria to termination of pregnancy showed a strong correlation with the expectant treatment; Univariate logistic analysis confirmed that there was no correlation between expectant treatment and foetal growth restriction.Conclusion There was a negative correlation between the duration of 24-h proteinuria and the expectant treatment of patients with early-onset preeclampsia; Expectant treatment could not improve the development of foetal growth restriction in patients with early-onset preeclampsia.KEY MESSAGESThe duration of 24-h proteinuria affects the effectiveness of expectant management of early-onset preeclampsia.Expectant management can reduce adverse neonatal outcomes due to iatrogenic preterm delivery, but it cannot improve the occurrence of foetal growth restriction.

IntroductionCurrently, the treatment strategies for angular pregnancy in the first trimester after assisted reproduction technology (ART) are unclear. Improper treatment will cause unnecessary losses to patients, especially infertile patients, after ART. The purpose of this study was to clarify the pregnancy outcomes of expectant treatment for angular pregnancy post-ART and to provide a basis for the formulation of clinical treatment strategies.MethodThis retrospective case series study was performed at the Reproductive Medicine Center of a university hospital. Maternal data and pregnancy outcomes were collected and analyzed for all patients diagnosed with angular pregnancies after ART between January 2016 and August 2021. The outcomes included live birth, term birth, premature birth, early pregnancy loss, fetal death, placental abruption, uterine rupture, maternal death, and hysterectomy.ResultsA total of 78 patients were analyzed in this study, of whom 54 (69.2%) had live births, 44 (56.4%) had term births, 21 (26.9%) had an early pregnancy loss, 1 (1.3%) had mid-trimester missed abortion, 1 (1.3%) underwent mid-trimester labor induction due to fetal malformation, and 1 (1.3%) underwent uterine rupture. There were no cases of maternal death, placental abruption, or hysterectomies.DiscussionAngular pregnancy after ART is not as dangerous as that described in previous studies; most cases could be treated expectantly under close-interval follow-up and obtain live birth.

ObjectiveThe COVID-19 lockdown extended premature rupture of membranes (PROM) expectant time among nulliparas and increased the risk of term neonatal complications. This study investigated the impact of term nulliparas with PROM delays at home on neonatal outcomes during the COVID-19 lockdown period, considering the clinical diagnostic application of maternal C-reactive protein (CRP).MethodsThis study collected 505 term nulliparous women who underwent PROM at home from five provinces in a non-designated hospital of China in 2020. We analyzed PROM maternal information at home and neonatal complications in the COVID-19 regional lockdown and compared related information in the national lockdown. Poisson regression models estimated the correlation of PROM management at home, maternal CRP, and neonatal morbidity. We constructed two diagnostic models: the CRP univariate model, and an assessed cut-off value of CRP in the combined model (CRP with PROM waiting time at home).ResultsIn the regional lockdown, PROM latency at home and the severity of neonatal complications were extended and increased lower than in the nationwide lockdown, but term neonatal morbidity was not reduced in the COVID-19 localized lockdown. Objectives: The aim of our study was to analyze the treatment and pregnancy outcome of tubal heterotopic pregnancy (HP) patients with a viable intrauterine pregnancy (IUP) in our center.Study Design: This was a retrospective analysis of 81 patients with tubal HP and a viable IUP. Patients were divided into either an expectant treatment group (29 patients) or a surgical treatment group (52 patients, 36 laparoscopy and 16 laparotomy). Data related to the basal clinical characteristic of all patients, rescue treatment and ectopic pregnancy (EP) rupture rate in the expectant treatment group, operation details in the surgical treatment group and pregnancy outcomes were collected and analyzed. Subgroup analyses were also performed.Results: In the expectant treatment group, the abortion rate, EP rupture rate and rescue treatment rate were 10.34 % (3/29), 21.14 % (7/29) and 34.48 % (10/29), respectively; subgroup analysis revealed that the rescue treatment rate in patients with EP mass enlargement ≥50 % was 71.43 % (5/7), which was significantly higher than that in patients with EP mass enlargement <50 % (15.00 %, 3/20), with P = 0.011. In the surgical treatment group, the abortion rate of all patients was 15.38 % (8/52); the abortion rate was 22.22 % (8/36) in the laparoscopy subgroup, which was significantly higher than that in the laparotomy subgroup (0.00 %, 0/16), with P = 0.038.Conclusions: Surgical treatment is a safe treatment option for tubal HP with a viable IUP, and laparoscopic surgery may be a potential risk factor for abortion. As the age of pregnancy grows, the rate of cervical cancer during pregnancy is increasing and the management of cervical cancer during pregnancy is also becoming more difficult in clinic. During treatment, safety of both pregnant women and offspring should be taken into account. Management of patients wishing to preserve of her pregnancy aim at fewer adverse effects on fetuses, less tumor progress and similar outcomes with non-pregnancy women with cervical cancer. Current options to treat pregnant patients with cervical cancer desired to preserve pregnancy contains expectant treatment, surgery and chemotherapy. Radical trachelectomy, simple trachelectomy and conization are common surgical methods and most common chemotherapy is based on platinum，with or without other agents. Commonly, patients with stage ⅠA1 cervical cancer are eligible for expectant treatment. Surgical treatment is option for patients with stage ⅠA2-ⅠB1 cervical cancer and neoadjuvant chemotherapy is used in patients with stage ⅠB1 or greater cervical cancer. Objective To evaluate the efficacy of combined use of traditional Chinese and western medicine in combination with conservative treatment in elderly patients with osteoporotic proximal humerus fractures． Methods 100 patients with osteoporotic proximal humerus fractures were randomly divided into treatment group ( n = 50) and control group ( n = 50) ． All patients were given a manual reduction combined with an adjustable upper limb extubation treatment． The treatment group additionally received alendronate sodium combined with Xianlinggubao treatment for 12 months． Healing time，complication，bone metabolism and bone mineral density were compared before and after treatment． Ｒesults 1，3，6 and 12 months after treatment，the scores of NＲS in the treatment group were significantly lower than those in the control group ( P ＜ 0. 05) ，and the shoulder function was significantly better in the treatment group than that the control group at 12 months ( P ＜ 0. 05) ． The incidence of postoperative complications in the treatment group was significantly lower than that in the control group ( P ＜ 0. 05 ) ，and the time of fracture healing was significantly shorter in the treatment group than that in the control group ( P ＜ 0. 05) ． After treatment for 6 and 12 months，BMD of the lumbar spine ( L2-4 ) ，left femoral neck and serum BGP were significantly higher in the treatment group than in the control group, while the level of serum β-CTX in the treatment group was significantly lower than that in the control group，and the difference was statistically significant ( P ＜ 0. 05) ． There was no significant difference in serum N-MID level between the two groups before and after treatment ( P ＞ 0. 05) ． Conclusion Alendronate combined with Xianlinggubao was safe and effective in the treatment of osteoporotic proximal humerus fractures in elderly patients． It could effectively relieve pain，accelerate fracture healing，improve the shoulder function of patients and reduce the complications incidence after treatment，thus is worthy of clinical promotion． [ABSTRACT FROM AUTHOR]

Background: We studied the possible advantages of feed-forward control nursing model in the treatment of placenta previa. Methods: We enrolled 60 pregnant women who were receiving treatment for expectant placenta previa between January 2010 and January 2016 and randomly divided them into the control group and the observation group with 30 cases in each group. In the control group, we offered specialist nursing which included examination, body positioning, vaginal bleeding record, psychological consultation and medication observation. Feed-forward control nursing was applied in the observation group which included establishing feed-forward control nursing improvement team, conducting quality control of nursing defects and putting forward ideas for improvements and verifying improvement outcomes. Results: The observation group got significantly higher success rate and lower complication rate compared with control group. Gestational age and fetal weights improved apparently in the observation group. When we compared the amount of postpartum bleeding and pregnancy bleeding in two groups we did not find any statistically significant difference (P>0.05). Patients’ satisfaction rate toward our nursing services was much higher in the observation group and the rate of nursing errors was significantly lower in this group. Cesarean scar pregnancy (CSP) is the rarest kind but serious complication of ectopic pregnancy. We presented a 35 year-old woman gravida 4, para 3, with a prior history of cesarean section before 6 months ago was admitted to our department with minimal vaginal bleeding and pelvic pain. Ultrasonographic evaluation revealed a regular well-defined gestational sac (GS) located in the lower segment of anterior uterine wall, adjacent to the vesicouterine space, 12*16*11 mm in size (5week 6 day). The case was managed by dilatation and curettage but procedure was unsuccessful. Patient was followed up by expectant management. The hCG resolution time was 40-50 days. Normal uterin anatomy was achieved after 103 days without complications. About 6 months later the patient has conceived intrauterin pregnancy. CSP should be diagnosed and treated as soon as possible to reduce morbidity and preserve fertility. Expectant management can be performed for selected patients detected in early first trimester. Background and Objective: About 25-50% of women with infertility and 3-10% in reproductive age are with endometriosis. The objective of this study was to determine the prevalence of endometriosis, stage of endometriosis and planning for treatment base on the stage of endometriosis. Methods: This study carried out on 221 infertile patients after laparoscopy in Babol Fatemeh Zahra center during 1998-2001. Findings: Of these patients, 30 (13.6%) had endometriosis. From these, 63%, 20% and 16.7% were with mild, moderate and severe endometriosis, respectively. The mean age±SD was 5.74±5.13 years. The pregnancy rate with expectant treatment was 23.3%. From these, 16.7%, 3.3% and 3.3% were with mild, moderate and severe endometriosis, respectively. Conclusion: Prevalence of endometriosis is related of marital age, parity and genetic factors. Patients with mild endometriosis can be first managed expectantly.

Objective: Pregnancy in previous cesarean scar is the rarest form of ectopic pregnancy. Little is known about its natural history and optimal management. All except one reported case in the literature that were treated expectantly either become unsuccessful or complicated. We aimed to present two cases of cesarean scar pregnancy (CSP) that were managed expectantly to discuss the value of this management and have a glimpse of the natural courses of certain CSPs. Cases: We present two cases of parous women with complaint of amenorrhea and mild bleeding. Sonography and magnetic resonance imaging (MRI) revealed the diagnosis of CSP without viable gestational sac. The patients opted for expectant treatment. We closely monitored the patients with a detailed plan. Results: In case 1, the patient had mild bleeding during monitoring and her serum βHCG dropped quickly to normal range after 7 weeks. She was fully recovered with total absorption of the mixed mass in the scar of the anterior wall of the uterus. In case 2, the patient had laparoscopy after expectant management for 16 weeks because her serum βHCG reached a plateau between 30 and 40 mIU/mL for 4 weeks and there was no obvious resorption of the mixed mass within the lower anterior wall of the uterus. The histopathology revealed decidualized villa within organizing tissue and the patient recovered uneventfully. Conclusions: CSP with no viable gestation sac and rapidly decreased serum βHCG level can be expectantly treated. The natural history of such CSPs may be naturally demised. [ABSTRACT FROM AUTHOR]

This single center retrospective study reviews the management and outcomes of 117 consecutive patients with humeral shaft fractures and associated radial nerve palsy (RNP) treated over a 20-year period (1986–2006). A total of 101 fractures were managed conservatively and 16 fractures underwent external fixation for poor bony alignment. Sixteen grade 1 and 2 open fractures underwent wound toileting alone. No patients underwent initial radial nerve exploration or opening of the fracture sites. All patients achieved clinical and radiological bony union at a mean of 8 weeks (range 7–12 weeks). There were no complications or pin tract infections in the operated patients. A total of 111 cases had initial spontaneous RNP recovery at a mean of 6 weeks (range 3–24 weeks) with full RNP recovery at a mean of 17 weeks (range 3–70 weeks) post-injury. Fourteen patients had no clinical/EMG signs of nerve activity at 12 weeks and 6 subsequently failed to regain any radial nerve recovery; 2 had late explorations and the lacerated nerves underwent sural nerve cable neurorraphy; and 4 patients underwent delayed tendon transposition 2–3 years after initial injury, with good/excellent functional outcomes. Humeral fractures with associated RNP may be treated expectantly. With low rates of humeral nonunion, 95% spontaneous nerve recovery in closed fractures and 94% in grade 1 and 2 open fractures, one has the opportunity of waiting. If at 10–12 weeks there are no clinical/EMG signs of recovery, then nerve exploration/secondary reconstruction is indicated. Late tendon transfers may also give good/excellent functional results. ObjectiveTo evaluate the relative safety and efficacy of conservative surgical, medical and expectant management. DesignRelevant studies were identified through a computer Medline search. ResultsLaparoscopy is not mandatory for diagnosis of ectopic pregnancy, which can be diagnosed by the use of transvaginal scan and estimation of quantitative βhCG. Laparoscopic salpingectomy or salpingostomy is preferred to laparotomy in a haemodynamically stable patient and should be used more frequently. The role of expectant management in ectopic pregnancy is very limited. Spontaneous resolution may occur only in a selected group of unruptured ectopic pregnancies with an initial βhCG of <200 mIU/ml and declining hCG level. Systemic methotrexate administration is a promising treatment in patients with early and unruptured ectopic pregnancy. Several prospective studies have shown success rates of 85–94% following single-dose systemic methotrexate treatment. Single-dose methotrexate is associated with fewer side-effects but is as effective as multiple-dose regimes. Recurrent ectopic pregnancy and intrauterine pregnancy rates following systemic methotrexate are comparable to those following laparoscopic salpingostomy. ConclusionSystemic methotrexate is a viable alternative to laparoscopic salpingostomy for women wishing to preserve their fertility. Objective: To describe unusual clinical and radiological features in patients with chronic recurrent multifocal osteomyelitis (CRMO).Design and Subjects: Based on radiographic and microscopic findings, three patients were prospectively diagnosed as having chronic recurrent multifocal osteomyelitis (CRMO). They form the basis of this report because of either the unusualness of the clinical presentation, hitherto undescribed associated diseases or the unusual age of presentation and sites of lesions.Results: One patient developed pyoderma gangrenosum at the site of one of the skeletal lesions and then went on to develop ulcerative proctitis. A second patient presented with a soft tissue mass, which on MRI mimicked a sarcoma. The final patient presented with lesions in the wrist and phalanges of the toes at the unusual age of 38. None of the patients was treated with steroids or antibiotics for the skeletal lesions. Steroids were administered to one patient for treatment of pyoderma gangrenosum.Conclusions: The pattern and distribution of skeletal lesions in CRMO are well recognized in the pediatric age group. The unusual clinical and/or radiological features discussed herein suggests that this is a disease that continues to evolve with a broader spectrum of features than recognized. In search of guidelines for the management of traumatic intracerebral haematomas (TICHs) with slight mass effects on computed tomography (CT) scans, the author reviewed the records of 29 patients who did not undergo surgery and 11 patients who did. It is found that patients with a TICH volume of less than 15 ml, a midline shift of less than 5 mm, an open perimesencephalic cistern on CT scans, a Glasgow Coma Scale (GCS) score of 12 or more, and an absence of lateralizing signs may be treated conservatively and expected to make a good recovery. On the other hand, with zero mortality and satisfactory outcomes, the patients undergoing early surgery tended to have a TICH volume of more than 15 ml, a midline shift of more than 5 mm, an obliterated perimesencephalic cistern on CT scans, a GCS score of less than 12, and the presence of lateralizing signs. However, the position of such features as the criteria of early operation for a TICH is weakened by the retrospective nature of this study because some surgical patients, free of lateralizing signs in particular, might have managed to do well without craniotomy. The present study was planned to explore the relationship between miRNA and VEGF expression in serum as well as placenta tissue with the outcome of early-onset severe preeclampsia (EOSP) in patients receiving expectant treatment. Sixty EOSP patients who had expectant treatment indications were divided into the success group (n = 46) and the failure group (n = 14) according to the pregnancy outcomes. miR-210 and miR-155 expression levels were studied in serum, ante partum, and in placenta tissue. The vascular endothelial growth factor (VEGF) and soluble VEGF receptor 1 (sFlt-1) expression levels were also explored. miR-210 and miR-155 expression levels in serum and placenta tissue before treatment, ante partum, and after accouchement of the success group were significantly lower than those of the failure group. Further, VEGF expression levels in serum and placenta tissue before treatment, ante partum, and after accouchement of the success group were significantly higher than those of the failure group. However, sFlt-1 expression levels in the success group showed a decrease in comparison to the failure group. Patients' satisfaction rate toward our nursing services was much higher in the observation group and the rate of nursing errors was significantly lower in this group. All differences were statistically significant ( P <0.05)., Conclusion: Application of feed-forward control nursing model in the expectant treatment of placenta previa can improve treatment success rate, decrease complications and upgrade nursing quality.
