TIME series analysis , *LEISHMANIASIS - Abstract
Objective. To analyze the dynamics of the spatial and temporal distribution of visceral leishmaniasis in Brazil from 2007 to 2020. Method. This ecological study focused on the spatial pattern of visceral leishmaniasis cases in Brazil from 2007 to 2020 using data from the Information System for Notifiable Diseases (SINAN). The variables analyzed were the incidence rate of visceral leishmaniasis and the visceral leishmaniasis composite indicator (VLCI), from which triennial thematic maps were constructed. The global Moran index was calculated to assess the existence of spatial autocorrelation, and the local Moran index was used to identify areas with higher and lower risk of visceral leishmaniasis in Brazil. Results. During the study period, there were 48 705 cases of visceral leishmaniasis, with an incidence rate of 25.53 cases per 100 000 population. There was spatial autocorrelation in all triennial blocks, with municipalities in the North and Northeast regions having an incidence equal to or greater than 50 cases per 100 000 population. Regarding the VLCI there was an increase in the number of municipalities classified as low risk for transmission, and a growing presence of state capitals with a classification of very high risk. Conclusion. The incidence of visceral leishmaniasis varied over the trienniums. The border region between the states of Tocantins, Maranhão, and Pará, along with the state of Ceará, stood out in the spatial distribution of the disease incidence and risk stratification by VLCI. These areas should be a priority for surveillance and control efforts for the disease. [ABSTRACT FROM AUTHOR]

TUBERCULOSIS , *INFLECTION (Grammar) , *DEATH rate , *MORTALITY , *TIME series analysis - Abstract
Objective. To analyze the temporal trend of tuberculosis incidence and mortality rates in Brazil between 2011 and 2019. Methods. This was an ecological time series study of tuberculosis incidence and mortality rates in Brazil between 2011 and 2019. Data were extracted from the Notifiable Disease Information System and the Mortality Information System, and population estimates were from the Brazilian Institute of Geography and Statistics. Trends were analyzed by Joinpoint regression, which recognizes inflection points for temporal analysis. Results. The average incidence rate of tuberculosis in Brazil in the period was 35.8 cases per 100 000 population. From 2011 to 2015, this coefficient had an annual percentage change of --1.9% (95% CI [--3.4, --0.5]) followed by an increase of 2.4% (95% CI [0.9, 3.9]) until 2019. The average mortality rate between 2011 and 2019 was 2.2 deaths per 100 000 population, with an average annual percentage change of --0.4% (95% CI [--1.0, 0.2]). Amazonas was the only state with an increase in the annual average percentage variation for the incidence rate (3.2%; 95% CI [1.3, 5.1]) and mortality rate (2.7%; 95% CI [1.0, 4.4]) over the years, while Rio de Janeiro state had an increasing inflection for incidence from 2014 to 2019 (2.4%; 95% CI [1.4, 3.5]) and annual average of decreasing percentage variation (--3.5%; 95% CI [--5.0, --1.9]). Conclusions. During the period analyzed, a decreasing trend in incidence was observed between 2011 and 2015, and an increasing trend for the period from 2015 to 2019. On the other hand, no change in the trend for mortality was found in Brazil. [ABSTRACT FROM AUTHOR]

- Abstract
Introduction: Mortality from HIV/AIDS is one of the main indicators to monitor this public health problem. Objective: Analyze the changes in the trend in mortality and in the number of people infected with HIV/AIDS in Peru 2000-2021. Material and Methods: Ecological study based on open data on people living with HIV/AIDS in the period 2000-2021. Mortality rates were obtained from the Single National Health Information Repository. Data on new cases of HIV/AIDS were obtained from CDC Peru sources. A joinpoint regression analysis estimated trends using mean percentage change (APC); considering sex, life stages and regions, p values less than 0.05 being significant. Results: The general mortality trend had no significant changes (APC=-1.3 [95% CI: - 2.6; 0.1]); nine regions presented increasing mortality trends; mortality in women decreased (APC=-2.2 [95% CI: -3.7; -0.6]); an increasing trend was observed in adolescent mortality (APC=6.3 [95% CI: 2.3; 10.3]). An increasing trend was demonstrated in the number of people infected with HIV (APC=3.73 [95% CI: 2.6; 4.8]) and the trend of AIDS cases decreased from 2004 (APC=-2.54 [95% CI: -4.3; -0.7]). Conclusion: It was found that there are increasing trends in mortality from HIV/AIDS in adolescents and in a significant number of regions of the Peruvian highlands, being a key priority to deepen the management and approach of these populations. - Abstract
The Instituto Mexicano del Seguro Social (IMSS) developed and implemented epidemic monitoring and modeling tools to support the organization and planning of an adequate and timely response to the COVID-19 health emergency. The aim of this article is to describe the methodology and results of the early outbreak detection tool called COVID-19 Alert. An early warning traffic light was developed that uses time series analysis and a Bayesian method of early detection of outbreaks from electronic records on COVID-19 for suspected cases, confirmed cases, disabilities, hospitalizations, and deaths. Through Alerta COVID-19, the beginning of the fifth wave of COVID-19 in the IMSS was detected in a timely manner, three weeks before the official declaration. The proposed method is aimed at generating early warnings before the start of a new wave of COVID-19, monitoring the serious phase of the epidemic, and supporting decisionmaking within the institution; unlike other tools that have an approach aimed at communicating risks to the community. We can conclude that the Alerta COVID-19 is an agile tool that incorporates robust methods for the early detection of outbreaks. [ABSTRACT FROM AUTHOR]

STOMACH cancer , *CANCER-related mortality , *DEATH certificates , *DISEASE risk factors , *CAUSES of death - Abstract
Objective. To identify nationwide temporal trends and spatial patterns of gastric cancer--related mortality in Brazil. Methods. An ecological study was performed using death certificates registered from 2000 to 2019 in which gastric cancer was recorded as any cause of death (an underlying or associated cause). Trends over time were assessed using joinpoint regression models. Spatial and spatiotemporal clusters were identified by Kulldorff's space--time scan statistics to identify high-risk areas. Results. In 276 897/22 663 091 (1.22%) death certificates gastric cancer was recorded as any cause of death. Age-adjusted gastric cancer--related mortality increased significantly over time (annual percentage change [APC]: 0.7, 95% confidence interval [CI]: 0.5 to 0.8). The increase in mortality was more pronounced in the less-developed North and Northeast Regions (North Region, APC: 3.1, 95% CI: 2.7 to 3.5; Northeast Region, APC: 3.1, 95% CI: 2.5 to 3.7). Eight spatiotemporally associated high-risk clusters of gastric cancer--related mortality were identified in the North, South, Northeast and Central--West Regions, as well as a major cluster covering a wide geographical range in the South and Southeast Regions of Brazil during the first years of the study period (2000 to 2009). Conclusions. More recently, during 2010 to 2019, clusters of gastric cancer have been identified in the Northeast Region. The nationwide increase in mortality in this analysis of 20 years of data highlights the persistently high burden of gastric cancer in Brazil, especially in socioeconomically disadvantaged regions. The identification of these areas where the population is at high risk for gastric cancer--related mortality emphasizes the need to develop effective and intersectoral control measures. SICK leave , *TIME series analysis - Abstract
Objective: We describe time trend incidence (2009-2018) of non-work related Sickness Absence (SA) segmented by duration of episodes in Spain. Methods: We used SA cases from a health insurance company ("mutua") in Spain. Overall non-work related SA incidence and incidence by duration of episodes (1-3 days, 4-15 days, 16-30 days, 31-90 days and> 90 days) were obtained. A time series ecological study was carried out with an inflection point in 2013. The annual percentage of change and 95% confidence interval were obtained. The analyses were stratified by age and sex. Results: Overall incidence went from 35.3 cases per 100 workers-years in 2009 to 25.2 in 2013. From 2014, there is a sustained rise in the incidence of ITCC, ending 2018 with 34.1 cases per 100 workers-years. The overall incidence is determined mainly by processes less than 16 days in young population. The decrease in 2009-2013 occurred in all the duration segments, especially in 4 to 15 (APC=-11,2; 95% CI=(-14,1 a -8,2)) and more than 90 days (APC =-9,4; 95% CI =(-15,5 a -2,8)), mainly in young people. The rise in 2013-2018 was observed in all the segments, with the largest significant increase in sections of 1-3 days (younger workers: APC =18,9; 95% CI =(14,8 a 23,2)) and in more than 90 days (mainly in older ones). Time trend of SA showed similar pattern in both sexes. Conclusions: Time trend analysis of SA incidence by duration segments offers a detailed information of SA. These results are useful for professionals in the prevention and management of SA. [ABSTRACT FROM AUTHOR]

Mesothelioma mortality and its socio-demographic and temporal patterns in Argentina from 1980 to 2013 were estimated using data from death certificates obtained from the Vital Statistics System of Argentina’s National Ministry of Health. There were 3,259 mesothelioma deaths corresponding to an age-adjusted mortality of 3.1/1,000,000 in 1980 and 5.7/1,000,000 in 2013, an average increase of 84.1% in 34 years. This raising trend became clearer after 1997. Males had higher mortality estimates compared with women in every year of the series; these findings suggest past exposure to asbestos. It is plausible that the asbestos exposure was mostly occupational, which is more common among men. Actions related to reinforcing the asbestos ban already in place and strengthening health surveillance directed at workplaces, previously exposed workers, and the population in general are recommended.

Objective. To describe the temporal distribution and epidemiologic characteristics of congenital syphilis (CS) cases in the city of Niterói, southeastern Brazil, from 2007 to 2016. Method. This descriptive time series analysis of the incidence of CS used data from the Notifiable Diseases Information System (SINAN) and the Live Birth Information System (SINASC). The sample included all notified cases. A probabilistic matching was performed between SINAN and SINASC data to recover ignored information. The time series was estimated using logarithmic regression according to sociodemographic and prenatal care variables. Results. There were 754 identified cases of CS in the study period (mean incidence: 11.9 cases/1 000 live births). The incidence was higher in younger women (10 to 19; 20 to 24 years) and in those with black skin, low schooling, and without prenatal care. Of the overall group, only 57.6% received a diagnosis of syphilis during prenatal care. Treatment was not adequate in 87,7%, and only 12.2% of partners were treated. SC incidence presented a growing trend of 16%/year, reaching 23.2 cases/1 000 living births in 2016. This growth was especially marked in female adolescents (25.2%/year), brown race/skin color (16.8%/year), women with low schooling (57.1%/year) and women who received prenatal care (17.3%/year); and, from 2012 to 2016, in women without information on skin color. Conclusions. Social inequalities were linked to CS in the present sample. Also, increasing CS incidence was detected in youth. Health care professionals must be trained to manage gestational syphilis, and public policies must effectively address the social determinants of this condition. [ABSTRACT FROM AUTHOR]

Objective: to analyze the trend in hospitalization rates for mental and behavioral disorders due to alcohol use (ICD-10: F10.0), Brazil and its five regions, 2010-2020. Methods: this was an ecological time-series study using data from the Hospital Information System of the Brazilian National Health System; to calculate the trend in hospitalizations, we used the Prais-Winsten generalized linear regression model and the Stata Statistical Software 14.0; a decreasing trend was considered when p-value < 0.05, and the regression coefficient was negative. Results: in the period analyzed, there were 423,290 hospitalizations for mental and behavioral disorders due to alcohol use in the country; the data analyzed showed a decreasing trend in adult hospitalization rates for this cause in the country (p-value < 0.001; 95%CI -0.094;-0.079) and in all its regions, for both sexes (p-value < 0.001). Conclusion: hospitalizations for mental and behavioral disorders due to alcohol use have reduced in Brazil and in its macro-regions.
