3. Alta satisfacción en pacientes con dolor de cadera en atención conjunta con enfermera de práctica avanzada en un centro ambulatorio docente asistencial

Introducción: En Chile, la Enfermería de Práctica Avanzada (EPA) es un rol emergente. El propósito del estudio es comparar la satisfacción de los pacientes atendidos por una EPA en conjunto con un traumatólogo subespecialista de cadera, con pacientes atendidos en la consulta estándar del traumatólogo de cadera. Material y método: Estudio piloto cuasi-experimental. Se incluyeron pacientes mayores de 18 años que consultaron en un centro ambulatorio docente-asistencial entre los años 2020-2023. Se evaluaron dos grupos, en el grupo experimental la modalidad de atención fue mixta (EPA y un traumatólogo subespecialista de cadera) y en el control los pacientes recibieron atención estándar de un traumatólogo subespecialista de cadera. Se evaluó satisfacción mediante una encuesta de diez preguntas (escala numérica continua de 0 a 100). El instrumento midió la calidad de la atención, diagnóstico y tratamiento, si el paciente volvería a ser tratado bajo esta modalidad de atención y si lo recomendaría a otras personas. Se utilizó las prueba de chi-cuadrado y Mann-Whitney. Resultados: Se evaluaron 108 pacientes. El 49% de los pacientes fueron asignados al grupo de intervención (n=53). La mediana del puntaje de satisfacción total fue de 96,5 puntos (rango 79-100), sin diferencias significativas entre ambos grupos (p 0,26). No se observaron diferencias significativas en las puntuaciones de satisfacción en ninguna de las diez preguntas. Conclusión: No hubo diferencias en los puntajes de satisfacción entre ambas modalidades de atención. Esta es la primera experiencia reportada en Chile sobre la atención otorgada por una enfermera de práctica avanzada, en ortopedia.

Objetivo. Conocer la calidad de vida y la satisfacción con los cuidados que expresan los pacientes de 65 o más años, con diagnóstico reciente de cáncer, que reciben quimioterapia de forma ambulatoria en Hospital de Día de Adultos del Hospital Universitario Son Espases. Comparando la atención habitual de cuidados estándar, con la incorporación de una enfermera experta, denominada Enfermera de Práctica Avanzada. Metodología. El diseño del estudio es cuasiexperimental pre-post. Tiene 2 grupos aleatorios que realizarán 2 cuestionarios específicos para pacientes con cáncer validados para hospitales españoles: cuestionario de calidad de vida de pacientes con cáncer (Quality of life questionnaire), QLQ-C30 (versión 3) y cuestionario de valoración de la satisfacción con la atención expresada por pacientes ambulatorios con cáncer que reciben quimioterapia (Cancer Out-Patient Satisfaction with Care Questionnaire), OUT-PATSAT35 CT. Los cuestionarios se pasarán a ambos grupos antes del inicio de los tratamientos, y pasados 3-4 meses del inicio. Relevancia. Este proyecto pretende poner en valor la aportación de la figura de Enfermera de Práctica Avanzada en la mejora de la Satisfacción de los Cuidados recibidos por el paciente de edad avanzada con cáncer en el servicio de Hospital de Día de Adultos, así como la mejora en la Calidad de Vida de los mismos, para poder extrapolar la utilidad de esta figura en la coordinación de los cuidados de otros pacientes dentro de la propia unidad y de otras unidades hospitalarias. ABSTRACT Objective. To assess the quality of life and satisfaction with care reported by patients aged 65 or older with a recent cancer diagnosis receiving ambulatory chemotherapy at the Adult Day Hospital of the Son Espases University Hospital. This study compares standard. care with the incorporation of an expert nurse, referred to as an Advanced Practice Nurse.Methodology. The study design is a quasi-experimental pre-post approach. It consists of two randomly groups that will complete two specific questionnaires for cancer patients validated for Spanish hospitals: the Quality of Life Questionnaire for Cancer Patients (QLQ-C30, version 3) and the Cancer Out-Patient Satisfaction with Care Questionnaire (OUT-PATSAT35 CT). Both groups will complete questionnaires before the start of treatments and after 3-4 months after initiation. Relevance. This project aims to highlight the contribution of the Advanced Practice Nurse in improving the satisfaction with care received by elderly cancer patients in the Adult Day Hospital service, as well as enhancing their quality of life. This may help extrapolate the utility of this role in coordinating care of other patients within the unit and in other hospital units.

El interés por la Enfermería de Práctica Avanzada (EPA) sigue creciendo al mismo ritmo que la confusión en torno al concepto. El Consejo Internacional de Enfermería (CIE) reconoció esta situación y lanzó en 2020 las “Directrices de Enfermería de Práctica Avanzada” (1). Lejos de clarificarla, un análisis rápido de los constructos que se exponen muestra la incompatibilidad entre sí de algunos de ellos, existiendo algunos fallos de lógica. Por ejemplo, no se explica, pero se incluye, el término “avanzado” en las definiciones, lo que las convierte en definiciones circulares. Las Directrices del CIE, de forma fortuita, son un reflejo de diferentes formas de entender qué es la EPA en el mundo.

Objective: To reflect on the contributions of nursing to the elimination of viral hepatitis in the Unified Health System. Methods: This was a reflexive study with a qualitative approach based on the practice of nursing in health systems, organized into three categories. Results: In the category Expansion of the scope of practice: path to strengthen the role of nurses in the Health Care Network, the strategic agenda of the Ministry of Health and the Federal Council of Nursing is addressed in providing conditions for expanding access through management, care, teaching and research. In the category subsidies of care management and coordination to guide nursing practice, the individual and family, professional, organizational, systemic and societal dimensions are addressed, with recognition of the need to broaden reflection. In the category Nurses in the management of programs to combat viral hepatitis, intrinsic aspects built since training are highlighted, with emphasis or management and management, in a real dynamic of technical responsibility in programs, services and teams. Conclusion: The reflection on the contribution of nurses is related to the management and coordination of care, a practice already established for professionals, but which requires guidelines and investment for hepatitis through continuing education. Objective: To map the contributions and strategies to implement advanced practice nursing in primary health care. Materials and methods: This scoping review was carried out following the guidelines of the Joanna Briggs Institute in six databases, namely: LILACS, MEDLINE, WoS, Embase, CINAHL, and Scopus, in addition to a bibliographic repository. A total of 3,076 studies were found, from which 12 were selected. Results: The contributions mentioned by the studies regarding advanced practice nursing were divided into three subcategories, as follows: care, educational or management, and preventive practices. They concern autonomy, specialized clinical skills, therapeutic counseling, and patient-centered interaction. Some strategies to implement advanced practice are related to continuing education, practice management, self-care, and disease management. Conclusions: There is a need to improve strategies for advanced practice nursing, especially in primary care, to address gaps in specialized care, the development of indicators, and therapeutic objectives. El avance en el desarrollo profesional de la enfermería española, el aumento de su nivel formativo, las disputas por delimitar los espacios competenciales de los distintos profesionales de la salud y los procesos organizacionales encaminados a la disminución de los costes sanitarios constituyen el contexto en el que se ha popularizado en España el término enfermería de práctica avanzada. Un término empleado para dar cuenta de tareas y desarrollos profesionales heterogéneos que coinciden en precisar de una formación y una experiencia más amplias que las de las enfermeras generalistas y que la profesión enfermera está utilizando como herramienta simbólica desde la que proyectar sus aspiraciones y desarrollar sus estrategias de ampliación de sus fronteras competenciales.

- Abstract
In diagnostic imaging departments, there has been a technological evolution in recent years. Progress made in interventional processes and the development of new treatments, as well as an increase in the number of diagnostic and therapeutic tests, has generated a growing and exponential need for specific care. Furthermore, the people who come to these departments are often in a vulnerable state and out of their comfort zone. The implementation of advanced practice nursing (APN) in the context of metabolic therapies and diagnostic imaging provides expert care with a holistic approach to patient care. It uses the nursing care process as a working method which, through research, logical analysis and critical reasoning, enables nursing care to be developed and applied. This approach contributes to safety management in the different processes in which it is engaged. Objective: to report the experience of leading nurses in coping with COVID-19 in a university hospital in southern Brazil. Method: experience report of the performance of leading nurses in the fight against the new Coronavirus, from March to June 2020, in a reference hospital in southern Brazil. Results: the report was based on the actions of advanced practice in nursing services, encompassing research, education, care practice and management. Some approaches were identified in the performance of the nurse-leader in the COVID Intensive Care Unit and in the Non-Reference Inpatient Unit, such as: creation of protocols and flows, training of nursing teams, dimensioning/ relocation, adequacies in care and different feelings experienced. Conclusion: the nurse’s leadership in the face of the pandemic has been permeated by numerous challenges, and it is necessary to use their skills and competences to provide a quality, safe and healthy work environment. Objective: To describe nurses' experience in implementing the copper Intrauterine Device (IUD) insertion service in Primary Health Care (PHC) in Florianópolis, SC. Methods: Experience report presenting the following steps: (a) Theoretical-Practical Material elaboration; (b) Nurse facilitators training; and (c) process monitoring and evaluation. Results: In total, 115 nurses were trained, including permanent professionals and residents. Regarding the expansion of access to the contraceptive method, a quantity of 2,024 IUD insertions by nurses was recorded since the implementation of the practice, in just over 3 years (390 insertions in 2018; 728 in 2019; 547 in 2020 and 359 insertions until May of 2021). These data reflect a 60% increase in access to the method to women. Conclusion: The nurse, by offering the insertion of the IUD in the PHC responsibly and based on scientificity, has contributed to reducing bureaucracy in access to the method. In this sense, the training process positively influences the qualification of care, and the reported practice has demonstrated efficacy and safety, in addition to surpassing models, which until then were hegemonic and centered on the doctor. Objective: To contextualize the professional experience of nurses from the perspective of Systematization of Nursing Care, with a focus on strengthening Primary Health Care and expanding access to resolving and quality. Methods: This is a descriptive study, with a qualitative approach, of the experience report type, on the experience of nurses in the participatory implementation of nursing protocols in the city of Florianópolis - Santa Catarina, Brazil. Results: The experience reported comprised the following phases: Definition of Stakeholders, Conceptual Alignment, Application of a Questionnaire - to survey the expertise of professionals, Establishment of Premises, Validation of Protocols, Monitoring and Evaluation of the Process, General Provisions. It is a practice with great possibility of replication. It is sustainable insofar as the Nursing Commission is strengthened, bringing together professionals with a network vision and a focus on patient care. Conclusion: It was observed the advance of the nurse's work and the expansion of access to the health service. In addition, the use of protocols can help in decision making, focusing on patient safety and support for the professional based or ethical principles of the profession. [ABSTRACT FROM AUTHOR]

Objective: To describe the implementation of advanced practice nursing in Latin America. Methods: Integrative literature review, using five databases as the research source and based on the guiding question: "How has the implementation of Advanced Practice nursing in Latin America been happening?" Results: 176 studies were identified, however, the final sample had 11 articles published between the years 2016 and 2020. The selected studies were aggregated into three categories according to thematic similarity: the initiatives, potentials and challenges for implementation of advanced practice nursing in Latin American countries. Conclusion: There is still much to be discussed about the EAP implementation process, as establishing the exercise of advanced practice with autonomy and independence for clinical judgment, diagnosis and prescriptive actions and the formation of new profiles of health professionals will be a challenge. - Abstract
National, the exploratory, qualitative, and quantitative study aimed to know the opinion of coordinators of undergraduate courses regarding the introduction of advanced nursing practices within the scope of Primary Health Care (PHC) in Brazil. Data were collected through face-to-face (n = 16) and telephone (n = 94) interviews, between 2015 and 2016, and analyzed using descriptive and inferential statistics, and qualitatively regarding their content. Most agree that PHC nurses could perform actions attributed to doctors, increasing the resolution of this level of care. However, the actions cited by them as likely to be incorporated are already part of the role of nursing, suggesting that the practice of nurses in PHC is limited. Opinions contrary to advanced practices are based on the perception that nurses have strayed from their real duties, or that they perform many tasks, without the prospect of social and salary recognition. - Abstract
The current demand on health services requires that nurses play a key role, by adapting their competencies to different fields and complexity levels. The approach of situations presented by critically ill patients underpins the need for development of specialised competencies in specific areas such as patient safety, prevention and control of healthcare-associated infections, performance of specific techniques and interventions, autonomous medication management or the use of technology, among others. Spain relies on a specialist training programme that is unique worldwide. Training admission is managed through a contract as a "Resident Nurse Intern" (EIR, Enfermera Interna Residente), provided by regional healthcare services. Only 6 specialities have been established and developed, in an uneven manner and with a short provision of places, annually. Given that the specialization in critical care nursing does not exist, nurses usually self-fund their postgraduate training to enhance their opportunities career development. The development of a speciality for critical care nursing is a priority. The models proposed advocate for creating nursing roles that could cover the systemic gaps through the expansion of their competencies and the introduction of procedures that fit nursing into advanced practice, which could be achieved through Advanced Accreditation Diplomas. Simultaneously, it would be convenient to analyse how and why such a dynamic discipline in some countries became stuck in anachronistic models of the Spanish healthcare system. Lecciones que deja la pandemia por Covid-19 Lições que a pandemia da COVID-19 deixa La falta de visión y de recursos de los sistemas de salud de las naciones de América Latina y El Caribe no ha permitido enfrentar de manera adecuada la pandemia por el nuevo coronavirus SARS-CoV-2, salvo honrosas excepciones. El énfasis en los sistemas curativos, tanto en infraestructura como en recursos humanos y financieros, en desmedro de la atención primaria de salud es uno de los principales ejemplos de esta falta de visión histórica, que disminuye la posibilidad de poder controlar una pandemia, efectivamente desconocida, pero de la cual ya se tenían los antecedentes necesarios para enfrentarla desde la prevención y la promoción de la salud en las poblaciones vulnerables de cada país. Se sumó a lo anterior la ausencia —en un número adecuado y de talento en la gobernanza— de enfermeros, por lo cual el escenario fue propicio para la diseminación de la pandemia y la consecuente afectación de los sistemas sanitarios, sociales, económicos y políticos de los países de América Latina. Klippel-Trenaunay-Weber syndrome is a rare congenital disease characterized by a triad of capillary malformations (hemangioma), venous malformations (arteriovenous fistula), and bone and soft tissue hypertrophy in the extremities. A clinical case is presented of a 54-year-old man with said syndrome and the presence of a 120-day-old ulcer in the lower limb that has worsened, causing severe pain, sick leave from work, and difficulty walking. The patient requires a referral to the advanced practice clinic for complex chronic wounds, which after a holistic assessment and treatment with enzymatic alginate gel manages to heal the ulcer in 90 days., El síndrome de Klippel-Trenaunay es un trastorno congénito complejo que se caracteriza por una tríada de alteraciones: malformación capilar, malformación venosa y crecimiento excesivo de las extremidades. Se expone un caso clínico de un hombre de 54 años con dicho síndrome y presencia de úlcera de evolución tórpida de 120 días de evolución, en miembro inferior que se ha agravado, produciendo dolor severo, baja laboral y dificultad para caminar. El paciente requiere de una derivación a la consulta de práctica avanzada en heridas crónicas complejas, que tras una valoración holística y un tratamiento combinado de terapia compresiva y gel de alginato enzimático, se logra cicatrizar la úlcera en 90 días.

Objective: To map the contributions and strategies to implement advanced practice nursing in primary health care. Materials and methods: This scoping review was carried out following the guidelines of the Joanna Briggs Institute in six databases, namely: LILACS, MEDLINE, WoS, Embase, CINAHL, and Scopus, in addition to a bibliographic repository. A total of 3,076 studies were found, from which 12 were selected. Results: The contributions mentioned by the studies regarding advanced practice nursing were divided into three subcategories, as follows: care, educational or management, and preventive practices. They concern autonomy, specialized clinical skills, therapeutic counseling, and patient-centered interaction. Some strategies to implement advanced practice are related to continuing education, practice management, self-care, and disease management. Conclusions: There is a need to improve strategies for advanced practice nursing, especially in primary care, to address gaps in specialized care, the development of indicators, and therapeutic objectives., Objetivo: mapear as contribuições e as estratégias para implementar as práticas avançadas de enfermagem na atenção primária a saúde. Materiais e método: trata-se de uma scoping review, realizada conforme as recomendações do Instituto Joanna Briggs, em seis bases de dados: Lilacs, Medline, WoS, Embase, Cinahl e Scopus, além de um repositório bibliográfico. Foram encontrados 3076 estudos, dos quais 12 foram selecionados. Resultados: as contribuições apontadas pelos estudos a respeito da prática avançada de enfermagem foram divididas em três subcategorias: práticas assistenciais, educativas ou de gestão e preventivas. Elas versam sobre autonomia, habilidades clínicas especializadas, aconselhamento terapêutico e interação centrada no paciente. Algumas estratégias para efetivar práticas avançadas relacionam-se à educação continuada, à gestão da clínica, ao autocuidado e ao manejo de doenças. Conclusões: diante do exposto, observa-se a necessidade de aprimorar estratégias voltadas à prática avançada de enfermagem especialmente no âmbito da atenção primária com vistas a sanar lacunas para um atendimento especializado, formulação de indicadores e metas terapêuticas., Objetivo: mapear los aportes y las estrategias para implementar la enfermería de práctica avanzada en la atención primaria de salud. Resultados: los aportes enumerados en los estudios respecto a la enfermería de práctica avanzada se dividieron en tres subcategorías: prácticas asistenciales, educativas o de gestión y preventivas, las cuales versan sobre la autonomía, las habilidades clínicas especializadas, el asesoramiento terapéutico y la interacción centrada en el paciente. Algunas estrategias para implementar la práctica avanzada se relacionan con la educación continua, la gestión de la práctica, el autocuidado y la gestión de enfermedades. Conclusiones: se observa la necesidad de perfeccionar estrategias direccionadas a la enfermería de práctica avanzada, en especial en el marco de la atención primaria, con el fin de cerrar brechas en atención especializada, formulación de indicadores y metas terapéuticas.

Resumo Objetivo Descrever o perfil sociodemográfico, o acesso e interesse em receber informações on-line sobre a disfunção vesical e intestinal, bem como compreender a vivência da família de crianças e adolescentes acometidos por essa disfunção. Métodos Trata-se de estudo multi-metodológico realizado em um ambulatório de Prática Avançada de Enfermagem em Uropediatria de um hospital de ensino da região centro-oeste do país. Resultados A vivência da família da criança com disfunção vesical e intestinal aponta para um impacto negativo no cotidiano podendo estar relacionado às condições sociodemográficas, a falta de conhecimento das famílias sobre os sintomas e aos estigmas associados à sua manifestação. Para tanto, o acesso a informações on-line apresenta-se como potencial ferramenta de apoio para melhorar a experiência da família da criança com os sintomas. Conclusão Os resultados expressam a caracterização sociodemográfica da criança e sua família e o acesso e interesse em receber informações pela internet sobre a disfunção vesical e intestinal, que podem ser importantes para a adesão e percepção de melhora nos sintomas refletindo na vivência familiar. Portanto, o enfermeiro que atua no contexto de cuidado em uropediatria precisa inovar em sua abordagem e implementar novas modalidades de assistência ou intervenções em saúde, principalmente por meio da incorporação de tecnologias baseadas na internet, visando à melhora da qualidade de vida tanto da família quanto da criança com disfunção vesical e intestinal. Resumen Objetivo Describir el perfil sociodemográfico, el acceso y el interés en recibir información digital sobre la disfunción vésico-intestinal, así como comprender la vivencia de la familia de niños y adolescentes acometidos por esta disfunción. Métodos Se trata de un estudio multimetodológico realizado en consultorios externos de Práctica Avanzada de Enfermería en Urología Pediátrica de un hospital universitario de la región Centro-Oeste del país. Resultados La vivencia de la familia de niños con disfunción vésico-intestinal indica un impacto negativo en la cotidianidad, lo que puede estar relacionado con las condiciones sociodemográficas, la falta de conocimiento de las familias sobre los síntomas y los estigmas asociados a su manifestación. Para eso, el acceso a la información digital se presenta como una potencial herramienta de apoyo para mejorar la experiencia de la familia de niños con los síntomas. Conclusión Los resultados expresan la caracterización sociodemográfica de los niños y su familia y el acceso e interés en recibir información por internet sobre la disfunción vésico-intestinal, que puede ser importante para la adhesión y percepción de mejora de los síntomas y puede reflejarse en la vivencia familiar. Por lo tanto, los enfermeros que actúan en el contexto de cuidado en urología pediátrica necesitan innovar su enfoque e implementar nuevas modalidades de atención o intervenciones en salud, principalmente mediante la incorporación de tecnologías basadas en internet, con el objetivo de mejorar la calidad de vida tanto de la familia como de los niños con disfunción vésico-intestinal. Abstract Objective To describe the sociodemographic profile, access and interest in receiving online information about bladder and bowel dysfunction as well as understand the experience of families of children and adolescents affected by this dysfunction. Methods This is a multi-methodological study carried out in an outpatient clinic of Advanced Nursing Practice in uropediatrics of a teaching hospital in midwestern Brazil. Results The experience of families of children with bladder and bowel dysfunction points to a negative impact on everyday life, which may be related to sociodemographic conditions, lack of knowledge of families about the symptoms and stigmas associated with its manifestation. To this end, access to online information is a potential support tool to improve the experience of children’s families with the symptoms. Conclusion The results express children’s sociodemographic characterization and their family and the access and interest in receiving information on the internet about bladder and bowel dysfunction, which may be important for compliance and perception of improvement in symptoms, reflecting on family experience. Therefore, nurses who work in the context of uropediatrics care need to innovate in their approach and implement new care modalities or health interventions, mainly through the incorporation of internet-based technologies, aimed at improving the quality of life of both the families and children with bladder and bowel dysfunction.

Objective: To report the experience of creating an online extension course, focusing on Advanced Nursing Practice in the promotion ofchild continence. Method: An experience report on the creation of a course for nursing students, carried out in the second half of 2021 at a federal university in Brazil. It was based on the Meaningful Learning Theory, Instructional Design, and on the Digital Storytelling strategy. Results: The course was planned to be offered online, including knowledge about childhood continence, Advanced Nursing Practice, urinary and intestinal symptoms, and nurses’ work in the context of pediatric urology. Final considerations: The authors proposed an innovative online course based on their experience to promote the teaching of child urological care in nursing education. RESUMEN Objetivo: Relatar la experiencia de creación de un curso de extensión online, cuyo foco es la Práctica Avanzada de Enfermería en la promoción de la continencia infantil. Método: Informe de experiencia sobre la creación de un curso para estudiantes de enfermería, realizado en el segundo semestre de 2021 en una universidad federal de Brasil. Se basó en la Teoría del Aprendizaje Significativo, en el Diseño Instruccional, y en la estrategia de Digital Storytelling. Resultados: El curso fue planificado para ser ofrecido online, incluyendo conocimiento sobre la continencia infantil, la Práctica Avanzada de Enfermería, los síntomas urinarios e intestinales, y la actuación de los enfermeros en el contexto de la uropediatría. Consideraciones finales: Se propuse un curso innovador basado en la experiencia de los autores, con el objetivo de promover la enseñanza del cuidado urológico infantil en la formación de enfermeros. Método: Relato de experiência sobre a criação de curso para estudantes de enfermagem, realizada no segundo semestre de 2021 em uma universidade federal do Brasil. Baseou-se na Teoria da Aprendizagem Significativa, no Design Instrucional e na estratégia Digital Storytelling. Resultados: O curso foi planejado para ser ofertado na modalidade online, contemplando conhecimentos sobre a continência na infância, Prática Avançada de Enfermagem, sintomas urinários e intestinais, e atuação do enfermeiro no contexto de uropediatria. Considerações finais: Evidencia-se a proposição de um curso online inovador a partir da experiência das autoras, com vistas a promover o ensino do cuidado urológico infantil na formação do enfermeiro.

Objective. to determine the evidence-based practice (EBP) competence of Spanish and Latin-American nurses participating in professional forums on the Internet and estimate the influence of socio-demographic and professional factors on their competence, which was defined as knowledge of, attitude towards, and implementation of EBP. Methodology: An online survey was administered to a convenience sample of nurses active in Internet forums, comprising validated Spanish versions of the Evidence-Based Practice Questionnaire (EBPQ) and Practice Environment Scale of the Nursing Work Index (PES-NWI) and socio-demographics and professional variables. Progress in the professional development of Spanish nursing, an increase in its level of training, disputes over defining the jurisdictions of different health professionals and organisational processes aimed at reducing healthcare costs constitute the context in which the term advanced practice nursing has become popular in Spain. A term used to account for heterogeneous professional tasks and developments that coincide in requiring more extensive training and experience than those of general nurses, and which the nursing profession is using as a symbolic tool to project its aspirations and to develop strategies for expanding its jurisdictional borders., El avance en el desarrollo profesional de la enfermería española, el aumento de su nivel formativo, las disputas por delimitar los espacios competenciales de los distintos profesionales de la salud y los procesos organizacionales encaminados a la disminución de los costes sanitarios constituyen el contexto en el que se ha popularizado en España el término enfermería de práctica avanzada. Health systems worldwide are in constant transformation that leads to restructuring in order to meet the needs of the population. These changes offer health personnel the opportunity to meet hospital demands, especially nursing personnel. Advanced nursing practice is a field that broadens and expands the boundaries of the scope of nursing practice, contributes to knowledge and promotes the advancement of the profession. It is characterized by the integration and application of a wide range of theoretical, evidence-based knowledge that is produced as part of graduate nursing education. In this sense, an advanced practice nurse is a person who has specialized, acquiring through additional education, either postgraduate or master's degree, the knowledge and skills necessary for complex decision-making and clinical competencies according to the context for which it has been accredited. Their role is tied to that of a Clinical Nurse Specialist (CNS) capable of providing expert care and advice based on established diagnoses in specialized clinical practice fields, coupled with a systems approach to practice as a member of the healthcare team., Los sistemas sanitarios a nivel mundial se encuentran en una constante transformación que los lleva a la reestructuración con el fin de satisfacer las necesidades de la población. Estos cambios ofrecen al personal de la salud, la oportunidad de satisfacer las demandas hospitalarias especialmente al personal de enfermería. La práctica avanzada de enfermería, es un campo que amplía y expande los límites del ámbito de la práctica de enfermería, contribuye al conocimiento y promueve el avance de la profesión. Se caracteriza por la integración y aplicación de una gran gama de conocimientos teóricos, basados en evidencia que se producen como parte de la educación de posgrado en enfermería. Objective: To present the definition of “symptom cluster” in cancer patients and to reflect on the theory, assessment, outcomes, and interventions for symptom management, based on the perspective of advanced practices in oncology nursing. Method: Theoretical-reflective study that presents and discusses possibilities for managing “symptom clusters” through advanced practices in oncology nursing. Results: The term “symptom cluster” can be defined as a set of two or more related symptoms. The theoretical concepts and models that can help in its understanding are: Theory of Unpleasant Symptoms, Theory of Symptom Management, concept of self-efficacy and Theory of symptom self-management. Advanced practice nurses have the skills to manage “symptom clusters,” optimizing outcomes and positively influencing the quality of life of cancer patients. Conclusion: Advanced practice nurses have the essential characteristics to design, to implement and to evaluate intervention protocols aimed at the management of “symptom clusters” in cancer patients. RESUMEN Objetivo: Presentar la definición de “clúster de síntomas” en pacientes oncológicos y reflexionar sobre los modelos teóricos, evaluación, resultados e intervenciones para el manejo de los síntomas desde la perspectiva de la enfermería de práctica avanzada oncológica. Método: Estudio teórico-reflexivo que presenta y discute posibilidades para el manejo de “clústeres de síntomas” a través de prácticas avanzadas en enfermería oncológica. Resultados: El término “clúster de síntomas” se caracteriza por un conjunto de dos o más síntomas relacionados entre sí. Los siguientes conceptos y modelos teóricos pueden auxiliar su comprensión: Teoría de los Síntomas Desagradables, Teoría del Manejo de los Síntomas, concepto de autoeficacia y teoría del autocontrol de los síntomas. Los enfermeros de práctica avanzada tienen habilidades para manejar los “clústeres de síntomas”, que optimiza los resultados e influye positivamente en la calidad de vida de los pacientes con cáncer. Conclusión: Los profesionales de la enfermería de práctica avanzada reúnen las características fundamentales para elaborar, diseñar, aplicar y evaluar protocolos de intervención dirigidos al manejo de “clústeres de síntomas” en pacientes con cáncer. Teleconsultation can be classified as an advanced practice nursing that requires nurses’ clinical reasoning based on a consistent theoretical framework to use in the nursing process. Our study conducted a theoretical-reflective analysis, based on Callista Roy’s Adaptation Model of Nursing and Chick-Meleis’ Transition Theory, about the contribution of teleconsultation as an advanced practice nursing in the care of older adults with chronic diseases during the COVID-19 pandemic. We reflect on this in two moments: “nursing theories and dealing with COVID-19” and “ways of adapting to new care models and advanced practice nursing,” based on communication and information technologies. The worsening of the pandemic in Brazil changed life cycles, health/disease and organizational processes, demanding the development of an adaptive-transactional state by users and health care providers. Thus, information and communication technologies combined with advanced practice nursing can relieve social distancing and its repercussions on health care. RESUMEN La teleconsulta es una herramienta de la enfermería de práctica avanzada que requiere un razonamiento clínico de los profesionales de enfermería basado en un marco teórico consistente para aplicarse en el proceso de enfermería. Este estudio realizó un análisis teórico-reflexivo, basado en el Modelo de Adaptación de Callista Roy y en la Teoría de las Transiciones de Chick-Meleis, sobre el aporte de la teleconsulta como herramienta de la enfermería de práctica avanzada a la atención de pacientes adultos mayores y de aquellos con enfermedades crónicas en el contexto de la pandemia del Covid-19. La reflexión se presenta en dos momentos: “teorías de enfermería y enfrentamiento al Covid-19” y “modos de adaptación a los nuevos modelos de asistencia y las prácticas avanzadas en enfermería”, fundamentados en las tecnologías de la información y la comunicación. El avance de la pandemia en Brasil trajo cambios relacionados con los ciclos de vida, los procesos de salud/enfermedad y procesos organizacionales, que requirieron el desarrollo de un estado adaptativo-transaccional por parte de los usuarios y los profesionales de la salud. En este contexto, las tecnologías de la información y la comunicación, sumadas a la enfermería de práctica avanzada, jugaron un papel clave para mitigar el distanciamiento social y sus repercusiones en la asistencia sanitaria. RESUMO A teleconsulta pode ser definida como uma prática avançada de enfermagem que requer do enfermeiro raciocínio clínico fundamentado em um arcabouço teórico consistente para aplicação no processo de enfermagem. No presente estudo, realizou-se uma análise teórico-reflexiva, fundamentada no Modelo de Adaptação de Callista Roy e na Teoria de Transição de Chick-Meleis, sobre a contribuição da teleconsulta como prática avançada de enfermagem no atendimento a pacientes idosos e com doenças crônicas no contexto da pandemia de covid-19. A reflexão é apresentada em dois momentos: “teorias de enfermagem e o enfrentamento da covid-19” e “formas de adaptação a novos modelos de assistência e as práticas avançadas em enfermagem”, norteados pelas tecnologias de comunicação e informação. O agravamento da pandemia no Brasil trouxe mudanças relacionadas aos ciclos de vida, aos processos saúde/doença e organizacionais, demandando o desenvolvimento de um estado adaptativo-transacional por parte dos usuários e profissionais de saúde. This is a reflective study aiming to analyze the implementation of Advanced Practice Nursing (APN) and provide perspectives regarding the role of these professionals in the Region of the Americas, particularly in Brazil. Several countries in the world have introduced advanced nursing roles to improve quality of care and reduce health care costs. However, these roles have not been fully implemented in Latin American countries, although some countries have held advanced discussions about it and proposed graduated courses. Brazil fulfills all conditions necessary for the implementation of the education of advanced practice nurses. The investment in advanced practice nurses will result in positive outcomes for the health of the population in a short period of time, with lower costs for governments than other possible actions. It is also a strategic way to address the persistent pressure on health services, whether related to the COVID-19 pandemic or other health needs of the population, and it can be a viable strategy for the provision of care to vulnerable populations and those living in remote areas. RESUMEN Estudio reflexivo que pretende analizar la implementación del rol de la Enfermera de Práctica Avanzada (EPA) y las perspectivas de actuación de este profesional en la Región de las Américas, sobre todo en Brasil. Varios países han introducido roles avanzados para los profesionales de la enfermería con el fin de mejorar la calidad de la atención, además de reducir los costos en salud. Pero este rol no está totalmente introducidos en los países de América Latina, aunque algunos de ellos avanzaron en las discusiones sobre el tema y en la propuesta de cursos de posgrado. Brasil dispone de plenas condiciones para poner en práctica la enfermería de práctica avanzada. Esta es una inversión que generará resultados para la población a corto plazo y con menores costos para los gobiernos que otras acciones. También constituye una estrategia para hacer frente a la presión sobre los servicios de salud, ya sea por las demandas de la pandemia del COVID-19, o por las demás necesidades de salud de la población; y puede ser una estrategia viable en la atención a las poblaciones vulnerables y en las regiones remotas. RESUMO Estudo reflexivo com objetivo de analisar a implementação da Enfermagem de Práticas Avançadas (EPA) e as perspectivas de atuação deste profissional na Região das Américas, com enfoque no Brasil. Diversos países têm introduzido papeis avançados para enfermeiros no intuito de melhorar a qualidade dos cuidados e para reduzir custos em saúde. Este papel não está totalmente implementado nos países da América Latina, ainda que alguns países tenham avançado nas discussões sobre o tema e na proposição de cursos de pós-graduação. O Brasil tem plenas condições para implementação de enfermeiros de prática avançada. O investimento em enfermeiros de prática avançada implicará em resultados positivos para a saúde da população em um curto período de tempo e com menores custos para os governos, em comparação com outras possíveis ações. Também se configura como estratégia de enfrentamento da pressão nos serviços de saúde, seja em decorrência da pandemia de COVID-19, como para as demais necessidades de saúde da população; e pode ser uma estratégia viável na atenção de populações em situação de vulnerabilidade e em regiões remotas.
