Abstract Background Psychiatric emergencies pose a special challenge for emergency physicians. It is known from other areas of medicine that the influence of a doctor’s gender can have an impact on the type of treatment and quality of patient care. However, this has not yet been investigated in the context of prehospital care in psychiatric emergencies. Objectives To identify whether the gender of the prehospital emergency physicians has an influence on the “on-scene” time, treatment strategy and on the potential escalation of interventions for patients with a psychiatric diagnosis. Methods A retrospective cohort analysis of emergency missions with a psychiatric diagnosis was performed between January 1, 2015 and December 31, 2021 at the Department of Emergency Medicine, Department of Anesthesiology and Intensive Care Medicine, University Hospital Ulm, Germany. Results 2882 emergency missions with a psychiatric indication/prehospital psychiatric diagnosis were studied and divided into: intoxication (n = 1343, 46.6%), suicidal behavior (n = 488, 16.9%), exceptional mental situation (n = 282, 9.8%), agitation (n = 262, 9.1%), anxiety and panic disorders (n = 262, 9.1%) and “psychiatric miscellaneous” (n = 245, 8.5%). Inpatient hospitalization occurred in 67.9% (n = 1958) of emergency missions. Of these, 20.3% (n = 392) were admitted directly to a psychiatric hospital. Male emergency physicians had a slightly longer "on-scene" time for psychiatric emergencies than female emergency physicians (p = 0.024). However, the variance in "on-scene" time for all interventions was significantly greater for female emergency physicians than for male emergency physicians (p = 0.025). Male emergency physicians were significantly more likely than their female counterparts to administer intravenous hypnotics in prehospital psychiatric emergencies (p = 0.001). For psychiatric patients who refused medically indicated inpatient psychiatric admission (“involuntary psychiatric admission”), male and female emergency physicians were equally likely to take the required action (p = 0.522). However, male emergency physicians were significantly more likely to administer an intravenous hypnotic to enforce involuntary admission (p = 0.009). Conclusions Similar to other medical specialties where the influence of physician gender on patient care is certainly relevant, the gender of prehospital emergency physicians also appears to influence their prehospital management strategy in psychiatric emergencies. The influence of gender is sometimes subtle and limited to specific aspects, such as the administration of hypnotics. Prospective study designs are needed to thoroughly investigate the influence of the gender of the prehospital emergency physician on the quality of care in psychiatric emergencies. Approximately 36% of EM reported not to be adequately qualified to handle psychiatric emergencies (p = 0.0001), while around 50% of respondents were neutral in their assessment in how to deal with psychiatric emergencies. 80% of EM reported a negative interaction (rejection of patients) with PS when referring a psychiatric emergency patient to the acute psychiatric unit. The most common reasons for refusal were intoxication (EM: 78.8%, PS: 88.2%), emergency physician therapy (EM: 53.8%, PS: 63.5%), and not resident in the catchment area of the hospital (EM 68.8%, PS: 48.2%). In the casuistry presented, most respondents would choose "talk down" for de-escalation (EM: 92.1%, PS: 91.3%). With respect to drug therapy, benzodiazepine is the drug of choice (EM: 70.4%, PS: 78.8%). More EM would choose an intravenously (i.v.) or a Mucosal Atomization Device (MAD) administration as an alternative to oral medication (i.v.: EM: 38.8%, PS: 3.8%, p = 0.001, MAD: EM: 36.7%, PS: 10.6%, p = 0.006). Significantly more EM would seek phone contact with the acute psychiatric hospital (EM: 84.7%, PS: 52.9%, p = 0.0107). A psychiatric emergency plan was considered useful in this context by more than 90% of respondents. The need for further training for EM with regard to treating psychiatric clinical syndromes was considered important by all respondents. In particular, the topics of "psychogenic seizure," "intoxication," and "legal aspects of psychiatric emergencies" were considered important (Mann-Whitney U test, p < 0.001).Conclusion: The interaction-related problems identified in the emergency medical care of pre-clinical psychiatric patients relate to non-modifiable, structural problems, such as insufficient admission capacity and non-existent or inadequate monitoring capabilities in acute psychiatric hospitals. However, factors such as the education and training of EM and communication between EM and PS can be improved. Background and Purpose: Dentists treat a wide range of patients, including patients with compromised health conditions. While rendering treatment, various medical emergencies can and do occur. To help increase the knowledge required to manage such emergencies, dental students must be trained while in dental school. This study aims to assess the level of medical emergency preparedness and knowledge among dental students at four dental schools. Material and Methods: The participating dental schools were IUSD, Case Western Reserve University School of Dentistry, Marquette University School of Dentistry, and the University of Alabama School of Dentistry. Groups were designed to include 20 dental students from Years 1 to 4. Students were asked to fill out a survey and were then tested on 10 clinical medical emergency scenarios. Results: A total of 331 dental students participated in the study. The scores based on 10 case scenarios presented with a range of 4.35–8.02. There was no statistically significant difference in the level of preparedness when dental schools were compared. However, Year 1 and Year 2 dental students had significantly lower total scores than those of Years 3 and 4. The students in Years 1 and 2 demonstrated less confidence in their current knowledge to manage medical emergencies. Satisfaction with the training received ranged from 38% to 84%. Conclusion: The results from this study indicate that students' preparedness to manage medical emergencies at these four dental schools is statistically similar. As it is well known, any incompatible blood transfusion can generate adverse reactions in feline patients, because cats naturally have plasmatic alloantibodies. Even so, recent scientific studies have shown that transfusions with heterologous blood (canine) to cats, were indeed followed by mild hemolysis states, in cases of severely anemic patients. A repeated transfusion however will cause anaphylactic shock with fatal outcome. Based on the data we have studied, we consider xenotransfusion, even though still controversial, to be considered as an emergency therapeutic procedure when superior, homologous blood cannot be acquired for felines. Through this review we hope to offer clinicians the necessary information to consider xenotransfusion as an alternative in emergency therapy, when this is rigorously sustained by blood compatibility tests and when all other options have been excluded only.

Algorithm of selection of compounds and formulations for development of multi-purpose CNS targeted medicinal products for emergency field and civil therapy is justified on the example of potent analgesics. Key criteria and quantitative parameters for selection of substances and dosage forms are singled out. The advantages of strong synthetic opioids vs traditionally used morphine, its derivatives and analogues are demonstrated. Advantages of multi-target formulations are shown. Choice of transmucosal dosage forms of analgesics, antiemetics and antidotes for urgent therapy is substantiated. Approaches to development of universal pharmacological platform for creating drugs acting on CNS are proposed. The current situation in the field of potent analgesics and other limited use medicines in the Russian Federation is described, problems are noted and possible solutions are suggested.

Background: Mechanical chest compression devices are commonly used providing a constant force and frequency of chest compression during cardiopulmonary resuscitation. However, there are currently no recommendations on ventilation during cardiopulmonary resuscitation with a mechanical chest compression device using continuous mode. An effective method for ventilation in such scenarios might be a triggered oxygen-powered resuscitator.Methods: We report seven cardiopulmonary resuscitation cases from the Munich Emergency Medical Service where mechanical chest compression devices in continuous mode were used with an oxygen-powered resuscitator. In each case, the resuscitator (Oxylator) was running in automatic mode delivering a breath during the decompression phase of the chest compressions at a frequency of 100 per minute. End-tidal carbon dioxide and pulse oximetry were measured. Additional data was collected from the resuscitation protocol of each patient.Results: End-tidal carbon dioxide was available in all cases while oxygen saturation only in four. Five patients had a return of spontaneous circulation. Based on the end-tidal carbon dioxide values of each of the cases, the resuscitator did not seem to cause hyperventilation and suggests that good-quality cardiopulmonary resuscitation was delivered.Conclusions: Continuous chest compressions using a mechanical chest compression device and simultaneous synchronized ventilation using an oxygen-powered resuscitator in an automatic triggering mode might be feasible during cardiopulmonary resuscitation.

Cardiac tamponade occurs when fluid accumulates in the pericardial sac and intrapericardial pressure increases, becoming greater than the normal cardiac diastolic pressure. The condition can be suspected on the basis of clinical signs and physical examination; laboratory examinations, electrocardiogram and chest X-ray can suggest the diagnosis and help identify causes and complications, but definitive and essential for the diagnosis is the echocardiographic exam. Cardiac tamponade is an emergency: the patient's life depends on the rapid and accurate diagnosis and on the timely completion of pericardiocentesis, this work being the only effective treatment in case of cardiac tamponade. Background: A primary splenic torsion occurs when the organ rotates around its vascular pedicle in the absence of any other concomitant disease. The occurrence of primary splenic torsion is rare, with a prevalence lower than 1%. Splenic torsion is more often related to dilated gastric volvulus syndrome, and it has no clear etiology despite reports that associate it with congenital abnormalities or traumatic ruptures of the gastrosplenic or splenocolic ligaments. This study reports a case of splenic torsion in an American Pit Bull terrier dog, its related symptomatology, and the treatment adopted. Case: A 3-year-old male American Pit Bull terrier dog weighing 32 kg was admitted to the hospital with symptoms of acute abdomen, lethargy, increased abdominal volume, discomfort, restlessness, and with a history of collapse three days prior to admission. Clinical examination revealed lethargy, pale mucous membranes, reduced capillary refill time, moderate dehydration, and hyperthermia. Abdominal palpation revealed increased abdominal volume in the epigastric and mesogastric areas. CBC and serum biochemistry profile revealed hypochromic normocytic anemia, leukocytosis with absolute neutrophilia and eosinopenia, mild hypoalbuminemia, and thrombocytopenia. PCR for detection of Babesia sp. and Ehrlichia sp., which returned negative results, and measurement of clotting time, which was prolonged, were performed to exclude immune-mediated hemolytic anemia. Radiographic and ultrasonographic images showed a normal stomach, but revealed presence of splenomegaly. An exploratory laparotomy allowed direct observation of the spleen, which exhibited an increased volume, a blackened, cold, hyperemic appearance, and a complete torsion of the splenic vascular pedicle; the surgical team opted to perform total splenectomy rather than undoing the pedicle twist. The animal returned to the hospital after 10 days exhibiting an excellent clinical condition. Healing of the incisional wound was satisfactory, with formation of epithelial tissue throughout its extension. Thirty days after the surgery, the hematological parameters were within normal ranges. The animal was discharged, and its owner received information on adequate nutrition. Discussion: Clinical evaluation of symptoms of discomfort and increase in abdominal volume, especially on the left side, together with the information of the anamnesis, led to a suspicion of splenic torsion in this case. Splenic torsion can exhibit numerous clinical signs; here, ultrasound imaging allowed diagnosis of this condition. The anemia observed by means of the hematological tests could be explained by sequestration of red blood cells by the rotated spleen, with and inflammatory leukogram resulting from inflammation of the poorly perfused parenchyma, local peritonitis, and, possibly, increased release of neutrophils from the bone marrow. Thrombocytopenia and prolonged clotting time may be secondary to splenic torsion, as shown in this case. Splenectomy is the treatment of choice in cases of acute splenic torsion. This procedure is relatively easy to perform. Additionally, merely undoing the torsion does not prevent its recurrence, and may allow necrotic debris to reach the circulation. No intercurrences or involvement of adjacent organs were observed in this patient. This is due to factors such as early diagnosis, preoperative stabilization of the patient, and the short time between diagnosis and the surgical procedure, reiterating its importance in emergency conditions such as splenic torsion. Emergency patients have acute health problems. The task of the emergency physicians are goal-oriented examinations to diagnose the life- or limb-threatening problems, to stabilize the patient and take him safely to the hospital, in a family practice or in an emergency department of a hospital the doctors fear that symptoms are misinterpreted and a life-threatening diagnosis is missed. Abdominal pain, shortness of breath or acute neurologic disabilities are some of the most-often observed symptoms in patients visiting a doctor. Which diagnoses should not be missed? Hymenoptera venom (HV) is injected into the skin during a sting by Hymenoptera such as bees or wasps. Some components of HV are potential allergens and can cause large local and/or systemic allergic reactions (SAR) in sensitized individuals. During their lifetime, ~ 3% of the general population will develop SAR following a Hymenoptera sting. This guideline presents the diagnostic and therapeutic approach to SAR following Hymenoptera stings. Symptomatic therapy is usually required after a severe local reaction, but specific diagnosis or allergen immunotherapy (AIT) with HV (VIT) is not necessary. When taking a patient's medical history after SAR, clinicians should discuss possible risk factors for more frequent stings and more severe anaphylactic reactions. The most important risk factors for more severe SAR are mast cell disease and, especially in children, uncontrolled asthma. Therefore, if the SAR extends beyond the skin (according to the Ring and Messmer classification: grade > I), the baseline serum tryptase concentration shall be measured and the skin shall be examined for possible mastocytosis. The medical history should also include questions specific to asthma symptoms. To demonstrate sensitization to HV, allergists shall determine concentrations of specific IgE antibodies (sIgE) to bee and/or vespid venoms, their constituents and other venoms as appropriate. If the results are negative less than 2 weeks after the sting, the tests shall be repeated (at least 4 - 6 weeks after the sting). If only sIgE to the total venom extracts have been determined, if there is double sensitization, or if the results are implausible, allergists shall determine sIgE to the different venom components. Skin testing may be omitted if in-vitro methods have provided a definitive diagnosis. If neither laboratory diagnosis nor skin testing has led to conclusive results, additional cellular testing can be performed. Therapy for HV allergy includes prophylaxis of reexposure, patient self treatment measures (including use of rescue medication) in the event of re-stings, and VIT. Following a grade I SAR and in the absence of other risk factors for repeated sting exposure or more severe anaphylaxis, it is not necessary to prescribe an adrenaline auto-injector (AAI) or to administer VIT. Under certain conditions, VIT can be administered even in the presence of previous grade I anaphylaxis, e.g., if there are additional risk factors or if quality of life would be reduced without VIT. Physicians should be aware of the contraindications to VIT, although they can be overridden in justified individual cases after weighing benefits and risks. The use of β-blockers and ACE inhibitors is not a contraindication to VIT. Patients should be informed about possible interactions. For VIT, the venom extract shall be used that, according to the patient's history and the results of the allergy diagnostics, was the trigger of the disease. If, in the case of double sensitization and an unclear history regarding the trigger, it is not possible to determine the culprit venom even with additional diagnostic procedures, VIT shall be performed with both venom extracts. The standard maintenance dose of VIT is 100 µg HV. In adult patients with bee venom allergy and an increased risk of sting exposure or particularly severe anaphylaxis, a maintenance dose of 200 µg can be considered from the start of VIT. Administration of a non-sedating H1-blocking antihistamine can be considered to reduce side effects. The maintenance dose should be given at 4-weekly intervals during the first year and, following the manufacturer's instructions, every 5 - 6 weeks from the second year, depending on the preparation used; if a depot preparation is used, the interval can be extended to 8 weeks from the third year onwards. If significant recurrent systemic reactions occur during VIT, clinicians shall identify and as possible eliminate co-factors that promote these reactions. If this is not possible or if there are no such co-factors, if prophylactic administration of an H1-blocking antihistamine is not effective, and if a higher dose of VIT has not led to tolerability of VIT, physicians should should consider additional treatment with an anti IgE antibody such as omalizumab as off lable use. For practical reasons, only a small number of patients are able to undergo sting challenge tests to check the success of the therapy, which requires in-hospital monitoring and emergency standby. To perform such a provocation test, patients must have tolerated VIT at the planned maintenance dose. In the event of treatment failure while on treatment with an ACE inhibitor, physicians should consider discontinuing the ACE inhibitor. In the absence of tolerance induction, physicians shall increase the maintenance dose (200 µg to a maximum of 400 µg in adults, maximum of 200 µg HV in children). If increasing the maintenance dose does not provide adequate protection and there are risk factors for a severe anaphylactic reaction, physicians should consider a co-medication based on an anti-IgE antibody (omalizumab; off-label use) during the insect flight season. In patients without specific risk factors, VIT can be discontinued after 3 - 5 years if maintenance therapy has been tolerated without recurrent anaphylactic events. Prolonged or permanent VIT can be considered in patients with mastocytosis, a history of cardiovascular or respiratory arrest due to Hymenoptera sting (severity grade IV), or other specific constellations associated with an increased individual risk of recurrent and/or severe SAR (e.g., hereditary α-tryptasemia). In cases of strongly increased, unavoidable insect exposure, adults may receive VIT until the end of intense contact. The prescription of an AAI can be omitted in patients with a history of SAR grade I and II when the maintenance dose of VIT has been reached and tolerated, provided that there are no additional risk factors. The same holds true once the VIT has been terminated after the regular treatment period. If attacked by bees or wasps, protect the head with arms or clothing. The retreat must not be hectic, but very slow. Insects release pheromones when stinging, which also motivate other insects to sting. Therefore, the sting site should be covered with the hand in the event of a sting.– Try to selectively inform bystanders about the sting event and its possible consequences.– Immediately remove any stinger remaining in the skin. When doing so, do not squeeze the sting apparatus with your fingers, but scrape it away to the side.Emergency medication in case of mild reactions limited to the skin:– If venom-specific immunotherapy has not yet been administered, oral medication is taken immediately after the sting, even in the absence of symptoms, according to the doctor‘s instructions: – Antihistamines – Steroids– After a successful allergen-specific immunotherapy*, medication should only be taken if, contrary to expectations, systemic symptoms do occur. For symptoms limited to the skin, oral medications are used first, and for more extensive reactions, the adrenaline auto-injector is used.Emergency measures in case of shortness of breath, swelling in the mouth/throat region or of circulatory problems:– Inject adrenaline laterally into the lateral thigh– In case of asthma, inhale 2 puffs of the emergency spray– Correct positioning (shortness of breath→ raised upper body, circulatory problems head-down position, unconsciousness→ stable side position)– Take oral medications only if swallowing is possible without problems– Alert an emergency doctor immediately!*Your allergist has confirmed that success of an allergen-specific immunotherapy is highly likely based on a tolerated sting provocation or field sting. In patients with a history of a severity grade I reaction, and in the absence of other risk factors, the prescription of an AAI is not required. However, the AAI can be prescribed in special situations (e.g., high risk of exposure, long distance to medical care, limitation of quality of life).Consensus22. In patients with a history of anaphylaxis (grade II – IV) or of a severity grade I reaction in combination with a high risk of re-exposure, an emergency kit including an AAI shall be prescribed until allergy diagnosis and assessment are complete.Consensus23. After successful initiation of VIT and reaching the maintenance dose at the maintenance interval, the prescription of an AAI can be waived in patients with a history of a systemic sting reaction (severity grade I – II) and in the absence of other risk factors for VIT failure.Consensus24. Wash hands and wipe mouth after eating.– Do not drink from bottles or beverage cans, cover drinking glasses, use straws.– Do not scare insects away from food sources, especially not with hectic movements.– Keep skin largely covered by clothing (at least when gardening). Do not walk barefoot, or use open foot wear. When riding a motorcycle, wear gloves and motorcycle clothing close to the skin. Open bicycle helmets are to be provided with a net.– Be especially careful on days with hot and humid weather, as insects are aggressive during such weather.– Avoid wearing loose-fitting, light garments, e.g., loose skirts or dresses with dark colors; try to wear dresses with light colors.– Keep apartment windows closed during the day or secure them with insect nets. No light in the evening when windows are open, as hornets are nocturnal and then prefer to fly towards light sources.– Watch for hidden insects (especially in bed or shoes).– Beehives must be avoided. Nests near a permanent residence must be removed (by beekeeper or fire department).– Wasp traps or repellent sprays can be helpful.– When approached by insects or being near the nest, avoid hectic or flapping movements, pull back slowly! In individual cases, VIT may be applied despite the presence of contraindications. This concept shall include a thorough weighing of the benefits and risks. Autoimmune diseases, severe cardiovascular or pulmonary diseases, and malignant diseases shall be optimally treated, shall be in remission before initiation of VIT, and shall be closely monitored during the course of VIT.Strong Box 15.Recommendations on venom selection for Hymenoptera VIT. In case of bee venom allergy and increased risk of sting or risk of particularly severe anaphylaxis, starting VIT with a maintenance dose of 200 µg may be considered.Consensus Box 16.Recommendation on the maintenance therapy of Hymenoptera VIT. The maintenance dose should be administered at 4-week intervals in the 1st year and, taking into account the manufacturer’s information, can be administered every 5 – 6 weeks from the 2nd year onwards, depending on the preparation used, and every 8 weeks from the 3rd year onwards if a depot preparation is used.Consensus Box 17.Recommendation on the reduction of side effects in Hymenoptera VIT. Strength of consensus38. A non-sedating antihistamine can be administered as a preventive measure during updosing, which can be continued in the further treatment if required. In case of reactions in the area of the injection site, local cooling measures shall be used.Consensus Box 18.Recommendations on the management of repeated systemic allergic adverse events in Hymenoptera VIT. Strength of consensus39. Possible risk factors of systemic side effects of VIT shall be identified and eliminated as appropriate.Majority40. Concomitant therapy with an H1-blocking antihistamine should be performed. The last tolerated dose should be continued for 3 months and, subsequently, a new updosing should be attempted.Consensus41. If risk factors for systemic side effects are present and cannot be eliminated, and if concomitant therapy with an H1-blocking antihistamine is not effective, concomitant treatment with an anti-IgE antibody (omalizumab; off-label use) should be performed.Majority42. Inappropriate seizure management may result in high morbidity and mortality. We assessed the adherence of health professionals in southern Rwanda to a national protocol for pharmacological management of seizures in children. A questionnaire featuring a 5-year-old child with generalized prolonged seizures was administered. The questions focused on the choice of initial treatment and the sequence of management following failure of the initial treatment choice. Benzodiazepine was chosen as initial therapy by 93.7% of physicians and 90.9% of nurses. Only 49.2% of physicians and 41% of nurses would repeat the initial treatment in case of failure of the first dose and 47% of doctors would wait 30 min to intervene. In case of refractory status epilepticus, 34% of physicians would give three doses of benzodiazepine, whereas 19% did not know what to do. These results suggest poor adherence to national protocol. Patients with insulin-treated type 1 diabetes (T1D) are exposed to hypoglycaemia, which may be serious. Serious cognitive impairment (including coma and seizure) that requires the help of a third party is a medical emergency. Besides the intravenous injection of glucose by a health care provider, its treatment consists of the subcutaneous or intramuscular injection of glucagon which may be performed by a family member. However, such an injection is not easy and puts off some people, which retards the initiation of a potentially life-saving therapy. The intranasal administration of 3 mg glucagon has been shown as efficacious as the subcutaneous or intramuscular injection of 1 mg glucagon in controlled studies carried out in both adult and youth patients with T1D. Classical congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency leads to glucocorticoid and mineralocorticoid deficiency. Management should be viewed as a process of care which requires input from an interdisciplinary team. Glucocorticoid therapy should take the form of hydrocortisone in a starting dose of 15mg/m2/day (divided into three doses), and the dose should be titrated to blood or urine profiles of cortisol and 17-hydroxyprogesterone. Mineralocorticoid replacement (9α-fludrocortisone) requires higher doses in infancy and childhood than in adolescence. The starting dose should be 150μg/m2/day, and the dose thereafter titrated to plasma renin activity and blood pressure. Despite adequate glucocorticoid substitution and concordance with medical therapy, control can be difficult during puberty due to alterations in the clearance of hydrocortisone, and dosing schedules may need to be adjusted to account for this. Diabetes mellitus, known as the most widespread disease in the world, along with four other chronic diseases, involves major expenditures and significant human resources for care, thus representing a burden on any type of health care system especially due to its rapid evolution of acute and chronic complications. For the emergency department (ED), the requirements of patients with acute complications of diabetes, determine expenses which are three times higher than those for non-diabetic patients and their hospitalizations are four times more frequent. The acute complications for which patients with diabetes most frequently require the ED are hypoglycemic, hyperosmolar, or ketoacidosis coma as well as alterations of the general condition that is typical of hypoglycemia, diabetic ketoacidosis (DKA), hyperglycemic hyperosmolar state and new-onset hyperglycemia. Hypoglycemia and the Somogyi phenomenon are the most common complications of type 1 diabetes but they can also occur in patients with type 2 diabetes who are treated with insulin through its overdose. DKA can occur in type 1 and 2 diabetes either by administering inadequate doses of insulin or due to the existence of precipitating factors such as stress, acute myocardial infarction, infections, sepsis, and/or gastrointestinal bleeding. Hyperosmolar hyperglycemic status is the most common complication in patients with type 2 diabetes and DKA. Treating the acute complications of diabetes in the ED involves, besides taking immediate measures to assess and maintain vital functions, monitoring patients, assessing blood sugar, electrolytes, urea, creatinine, and bicarbonate, and applying appropriate immediate therapeutic measures for each type of acute diabetes complication. Glutaric acidemia type II and carnitine palmitoyltransferase type II deficiency are rare, but potentially treatable, inherited metabolic diseases. Hallmarks of the early onset form of both conditions are renal abnormalities and neonatal metabolic crisis. In this article, we report on two newborns with cystic renal dysplasia as a leading sign of these metabolic diseases. We focus on the clinical presentation and discuss the diagnostic tests and the available therapeutic options. We conclude that prenatal diagnosis of cystic renal dysplasia should alert the physician to the possibility of these metabolic diseases. Abstract: Objective: To compare treatment with beta 2 agonist delivered either by a spacer device or a nebulizer in children with severe or potentially severe acute asthma. Methods: In this randomized trial, children 4 to 15 years, cared for in the emergency department for severe or potentially severe acute asthma, received 6 times either nebulizations of salbutamol (0.15mg/kg) or puffs of a beta 2 agonist (salbutamol 50μg/kg or terbutaline 125μg/kg). The primary outcome was the hospitalization rate. Secondary outcomes included percentage improvement in Bishop score, in PEF, SaO2, respiratory and heart rates, side effects, length of stay and relapses 10 and 30 days later. Results: Groups did not differ for baseline data. There were no significant differences between the 2 groups (nebulizer N =40, spacer N =39) for baseline characteristics before emergency department consultation except for length of acute asthma in the spacer group. Clinical evolution after treatment, hospitalization rate, relapse were similar including the more severe subgroup. In the spacer group, tachycardia was less frequent (P <0.02). The overall length of stay in the emergency department was significantly shorter (148±20 vs 108±13min, P <10−9). Conclusions: The administration of beta 2 agonist using a metered-dose inhaler with spacer is an effective alternative to nebulizers for the treatment of children with severe or potentially severe acute asthma in the emergency department. Abstract: Emergency resuscitation of ferrets can be done using medications and equipment readily available in small animal hospitals. Cardiopulmonary resuscitation mechanics in the ferret are similar to those in other companion mammals. This article reviews methods of cardiac massage, intubation and ventilation, fluid support, and drugs and dosages appropriate for use in the ferret emergency patient. A presurgical preparation regimen to prevent cardiac arrest and anesthetic and post anesthetic complications is also presented. Treatments for resuscitation of the ferret in vaccine induced anaphylaxis (vaccine reaction) are also discussed. Objective: To compare the efficacy of long-acting IM methylprednisolone to tapering oral methylprednisolone in adult asthmatic patients discharged from the emergency department (ED). Methods: Randomized, double-blind, placebo-controlled trial of a single IM dose of 160 mg depot methylprednisolone vs 8-day tapering of a total dose of 160 mg oral methylprednisolone in adult asthmatic patients (age range, 18 to 45 years) who were discharged from the ED following standardized treatment for an acute exacerbation. The primary end point was relapse, which was defined as the need to seek unscheduled care at a doctor's office, clinic, or ED for symptoms of persistent or worsening asthma within 10 days of ED discharge. Results: Of 190 patients enrolled into the study, 180 completed the study and the follow-up at 10 days (96%). The relapse rate was nearly identical for the two treatment groups (IM administration, 14.1% [13 of 92 patients]; oral administration, 13.6% [12 of 88 patients]; difference, 0.5% [95% confidence interval, -9.6 to 10.6%]). Conclusions: Single-dose IM methylprednisolone administered to adult asthmatic patients at ED discharge appears to be a viable therapeutic alternative to a course of oral methylprednisolone. Clinicians may choose to base the route of administration of corticosteroids on concerns about nonadherence to therapy or on the ability of a patient to afford a prescription for outpatient medication.
