A patient presents with worsening of his general condition, chills and dyspnoea on exertion. With a history of aortic valve replacement, infective endocarditis is suspected, but due to negative imaging by transesophageal echocardiography and negative blood cultures cannot be confirmed. Finally, Cutibacterium acnes prosthetic valve endocarditis is diagnosed after culture of C. acnes during an extended incubation period of blood cultures, the presence of embolic complications and a characteristic finding on PET-CT scan. Approximately 25% of all ischemic insults are caused by cardiac emboli in atrial fibrillation. Therefore, in patients with atrial fibrillation, oral anticoagulation is initiated or transesophageal echocardiography is performed before electroconversion to exclude blood clots in the heart. This shows that virtually all cardiac thrombi (>90%) are localized in the atrial ear. Therefore, for many decades surgeons have been removing the atrial tube during open heart surgery to eliminate it as a source of emboli. As a minimally invasive alternative, various screens for percutaneous closure of the atrial ear have been developed in the past 10–20 years. Trees may migrate northward in response to climate change and become exposed to new photoperiod and soil moisture regimes. This study assessed the impacts of photoperiod and its interaction with soil moisture and carbon dioxide concentration ([CO2]) on the hydraulic conductivity in jack pine ( Pinus banksiana Lamb.) and its vulnerability to xylem embolism. Seedlings were exposed to 400 vs. 950 μmol·mol−1 [CO2], 60%-70% vs. 30%-40% (of field capacity) soil moisture, and photoperiods of seed origin and 5° and 10° north of seed origin in greenhouses. Cavitation vulnerability curves were measured for determining the xylem pressure at which 50% hydraulic conductivity was lost (ΨPLC50). It was found that elevated [CO2] significantly increased hydraulic conductivity, whereas low soil moisture decreased it. Under elevated [CO2], the xylem became progressively more vulnerable to embolism with changes in photoperiod regime from the seed origin to 10° north of the seed origin, as indicated by the progressively less negative ΨPLC50. However, no such a trend was detected under the ambient [CO2]. The results suggest that the species may become less resistant to drought as the atmospheric [CO2] increases, hindering the northward migration or seed transfers. Even within its current natural distribution range, trees near its northern boundary of the range may be more vulnerable to embolism as the atmospheric [CO2] increases even without any change in moisture conditions. Atrial fibrillation (AF) is the most frequently encountered sustained arrhythmia with a prevalence of 0.5–10%, depending predominantly on age. The arrhythmia is associated with significant morbidity and mortality, mainly due to thromboembolic events including stroke and systemic embolisms. These complications can be effectively prevented with anticoagulation therapy either with vitamin K antagonists (VKA) or with non-vitamin K antagonists (NOAC). VKA therapy is effective in preventing strokes but these medications are difficult to use, are associated with significant bleeding risk, and have pharmacokinetic/dynamic properties that make their use cumbersome. NOACs—either factor II or factor Xa inhibitors—have been developed over the past two decades and have been tested against VKA in large randomized controlled trials. This trial evidence was complemented more recently by increasing real-world data comprising several 100,000 patients. Finally, NOACs have been examined for their use in specific clinical situations, for example, in patients undergoing cardioversion, catheter ablation, or coronary interventions. In all of these clinical scenarios, NOACs have been similarly effective or—in many instances—even superior to treatment with VKA. Recent guidelines, therefore, recommend NOAC therapy for stroke prevention in AF as first-line therapy. [ABSTRACT FROM AUTHOR]

Introduction - Papillary fibroelastoma is the most common cardiac tumor with valvular localization, involving more frequently the aortic or mitral valve and much rarely the tricuspid or pulmonary valve. It is the second most common primary cardiac tumor, after myxoma. Many patients are asymptomatic, but the tumor represents a potential source of embolism which explains its clinical importance. Case presentation - We report the case of a 60-year-old woman with nonspecific symptoms in whom a mass attached to the tricuspid valve was found during a routine transthoracic echocardiography, as part of her cardiac evaluation for an elective orthopedic surgical procedure. Multimodality imaging including transesophageal echocardiography and cardiac MRI was afterwards performed. Since the patient had a high global risk of pulmonary embolism, the tumor was surgically removed with preservation of the tricuspid valve and an uneventful postoperative course. The histopathological examination confirmed the diagnosis of papillary fibroelastoma. Conclusion - This case emphasizes the importance of echocardiography as a first-line imaging modality in the diagnosis of cardiac tumors. The embolic risk for papillary fibroelastomas is high and even if the discovery of the tumor is incidental, excision is advisable as surgery has excellent long-term results. [ABSTRACT FROM AUTHOR]

This article reports two cases of patients with coronavirus disease 2019 (COVID-19) in which occlusion of large cerebral arteries occurred. These occurred in a female patient in the early stage of COVID-19 and in the second case in the late stage. One female patient could be successfully treated with i.v. thrombolysis and mechanical thrombectomy. Coagulopathy in the course of COVID-19 can result in severe stroke with poor outcome even in younger patients. With respect to the etiology of arterial occlusions (COVID-19-induced hypercoagulopathy, cardiomyopathy, vasculitis) there is a necessity for further research.Zwei Fälle von Patienten mit der Coronavirus-19-Erkrankung (COVID-19) werden berichtet, bei denen Verschlüsse großer Hirnarterien vorlagen. Diese traten bei einer Patientin in der Früh- als auch im 2. Fall in der Spätphase der COVID-19 auf. Eine Patientin konnte erfolgreich mithilfe der i.v.-Thrombolyse und mechanischer Thrombektomie behandelt werden. Gerinnungsstörungen im Rahmen der COVID-19 können auch bei jüngeren Patienten zu fulminanten Hirninfarkten mit schlechtem Outcome führen. Bezüglich der Ätiologie dieser Gefäßverschlüsse (COVID-19-induzierte Hyperkoagulopathie, Kardiomyopathie, Vaskulitis) besteht weiterer Forschungsbedarf.

Es wird uber den Fall eines 28-jahrigen Patienten mit einem Niereninfarkt aufgrund eines embolisierenden Aneurysmas nach traumatischer Dissektion einer Nierensegmentarterie berichtet. Er stellte sich 1,5 Jahre nach einem Motorradunfall mit Abdominal- und Flankenschmerzen vor. C-reaktives Protein und Laktat-Dehydrogenase waren erhoht. Die Diagnose wurde mittels computertomographischer Angiographie gestellt. Weitere Ursachen eines Niereninfarkts wurden ausgeschlossen. Nach interdisziplinarer Besprechung entschieden wir uns bei jungem und sportlich aktivem Patienten fur ein interventionelles Coiling, um eine langfristige Antikoagulation zu vermeiden.

Nous rapportons le cas d'un myxome de l'oreillette gauche compliqué de syndrome coronarien aigu et de multiples accidents vasculaires cérébraux ischémiques. Le diagnostic d'accidents emboliques est retenu devant l'association d'une tumeur intracardiaque emboligène, des AVCI et un SCA en l'absence de lésions d'athérosclérose et l'absence de thrombus intracardiaque. Le patient a bénéficié d'une exérèse chirurgicale de la tumeur avec des suites opératoires simples. Nous illustrons à travers cette observation le potentiel embolique du myxome qui rend cette tumeur redoutable et nous insistons sur l'intérêt d'une prise en charge chirurgicale urgente.

La maladie à coronavirus 2019 (COVID-19) déclarée en Chine en fin 2019 s´est rapidement généralisée aux autres continents. Son diagnostic se fait par test PCR (Polymerase Chain Reaction) sur des prélèvements naso-pharyngés. Ce test bien que spécifique est d´une sensibilité moindre comparé à la TDM thoracique. Nous rapportons le cas d´un patient testé négatif à deux reprises et chez qui la TDM retrouvait des signes typiques de COVID-19 et une embolie pulmonaire. Et ce n´est qu´après un troisième test PCR qu´il a été positif. Ce qui montre l´intérêt de répéter plusieurs fois les tests PCR mais également de considérer les signes scanographiques comme argument diagnostic devant induire une prise en charge adéquate.

French Lavender and Lavandin crops are subject to severe dieback, resulting from the interaction between Stolbur's disease and increasingly intense and frequent droughts. Improving the resistance of Lavender plantations is a challenge to safeguard this sector with a strong economic, tourist and social impact on its territory. A project involving partners from research and the Lavender sector aims to increase crop resistance through new practices. Within this framework, the objectives of the thesis were i) to identify physiological indicators of the state of stress, and in particular a physiological threshold of non-recovery, following severe droughts in Lavender and Lavandin and ii) to evaluate the effect of the agro-ecological practice of inter-row cultivation on the response of Lavandin to summer drought. In addition, a study on climate forecasting focused on production departments revealed an increase in summer and autumn temperatures and a decrease in autumn rainfall on the near horizon (2021-2050), confirming the interest of our work for the sector. To continuously monitor the physiological condition of the plants during a drought, PépiPIAF-type dendrometers, which measure variations in stem diameter, were used on these species. Two indices related to the intensity of the drought and the damage caused were thus identified: percentage loss of diameter (PLD) and loss of rehydration capacity (PLRC). Plants did not recover from a drought (PLRC = 100%) when the PLD had reached a maximum value of about 21%. These results mean that the inability to recover occurs when the elastic water storage are empty, thus demonstrating their importance during intense droughts. During dehydration, acoustic measurements and electrolyte leakage showed that this inability was associated with significant cell damage and not a high level of embolism. These two indices developed under controlled conditions also made it possible to diagnose the stress state of lavandins in the field. Dendrometers on lavandins revealed water stress exerted by an inter-crop vegetation cover during the growing season and summer drought. The water stress was earlier with this cover but the intensity of the drought was identical to that with bare soil. In addition to the importance of this work in fundamental research, in particular on the mechanisms of plant mortality, these results open perspectives on the anticipation of mortality in Lavender plantations, and even on the management of irrigation of these crops, for a reasoned control of the hydric stress necessary for the production of essential oils but harmful to the plants according to its intensity and the period.; Les cultures françaises de Lavande et Lavandin sont soumises à un sévère dépérissement, résultant de l’interaction entre la maladie du Stolbur et des sécheresses de plus en plus intenses et fréquentes. Ces résultats signifient que l’incapacité à récupérer survient lorsque les réservoirs élastiques d’eau sont vides et démontrent ainsi leur importance lors de sécheresses intenses. Lors des déshydratations, des mesures acoustiques et de fuites d’électrolytes ont montré que cette incapacité était associée à des dégâts cellulaires importants, et non à un haut niveau d’embolie. Ces deux indices développés en conditions contrôlées ont aussi permis de diagnostiquer l’état de stress de lavandins en champ. Les dendromètres sur les lavandins ont révélé une contrainte hydrique exercée par un couvert végétal en inter-rang pendant la période de croissance et de sécheresse estivale. Le stress hydrique fut plus précoce avec ce couvert, mais l’intensité de la sécheresse identique à celle avec un sol nu en inter-rang. Outre l’importance de ces travaux pour la recherche fondamentale, en particulier sur les mécanismes de mortalité des plantes, ces résultats ouvrent des perspectives sur l’anticipation des mortalités en lavanderaie, voire sur le pilotage de l’irrigation de ces cultures, pour un contrôle raisonné de la contrainte hydrique nécessaire à la production d’huiles essentielles, mais dommageables pour les plants selon son intensité et la période.

Left Atrial Appendage Closure- An Alternative to Continuous Anticoagulation for Atrial Fibrillation? Abstract. Approximately 25% of all ischemic insults are caused by cardiac emboli in atrial fibrillation. Therefore, in patients with atrial fibrillation, oral anticoagulation is initiated or transesophageal echocardiography is performed before electroconversion to exclude blood clots in the heart. This shows that virtually all cardiac thrombi (>90%) are localized in the atrial ear. Therefore, for many decades surgeons have been removing the atrial tube during open heart surgery to eliminate it as a source of emboli. As a minimally invasive alternative, various screens for percutaneous closure of the atrial ear have been developed in the past 10-20 years. The requirements, techniques, and results will be discussed in the following article.

Abstract: As digital ulcers usually reflect focal ischemia due to a microangiopathy, digital gangrene is more likely to be the consequence of a macroangiopathy which can also sometimes be associated with a microangiopathy. Digital gangrene is a common skin manifestation of connective tissue diseases, especially systemic sclerosis. Other multiple causes are possible: Buerger''s disease, occupational diseases (particularly hypothenar hammer syndrome) or obstructive vascular disease due to subclavian artery embolism or cardiac embolism. More rarely vasculitis, cryoglobulinemia, polycythemia vera and essential thrombocythemia or solid cancer are associated with digital gangrene, sometimes as a first manifestation. Drug-induced digital gangrene should in all cases be checked (particularly bleomycine and interferon alpha). A thorough clinical examination is frequently enough to evoke a specific cause. Cardiac and upper limbs echo-Doppler, biological examination looking for an inflammatory syndrome, an abnormal blood formula, the presence of a cryoglobulinemia, antineutrophil and antinuclear antibodies is mandatory. In some cases, upper limb arteriography is needed to identify a mechanism such as subclavian aneurism or atherosclerotic plaques or to confirm digital arteritis as shown in Buerger''s disease. Limited amputation is frequently needed, but may sometimes occur spontaneously. The main treatment is to treat the cause. [Copyright &y& Elsevier]

Résumé L’embolie paradoxale est une entité rarement démontrée, souvent évoquée, et le diagnostic est présomptif dans la plupart des cas. Le foramen ovale perméable (FOP) est un facteur anatomique prédisposant important pour une telle complication. Nous décrivons le cas d’une patiente ayant un volumineux thrombus enclavé dans le FOP diagnostiqué par l’échocardiographie incluant la modalité 3D, dans un contexte d’embolie pulmonaire aiguë massive. Le traitement n’est pas codifié dans ce contexte, et le thrombus a été extrait avec succès par chirurgie associé à la fermeture du FOP et l’anticoagulation efficace. Paradoxical embolism is rarely demonstrated, often suggested, and the diagnosis has been largely presumptive in most cases. The patent foramen ovale (PFO) is an important predisposing anatomic factor for such a complication. We describe a case where a voluminous thrombus straddling the PFO was diagnosed by echocardiography including the 3D modality, in the setting of acute massive pulmonary embolism. The treatment is not codified in this setting, and the thrombus was successfully removed by surgery, associated with PFO closure, and anticoagulation. [ABSTRACT FROM AUTHOR]

Abstract: Sap is transported under tension (i.e. negative pressure) in trees, according to the tension-cohesion theory. Since water is physically instable under negative pressure, a risk of cavitation is possible. Techniques have been developed during the past two decennia to study cavitation in trees. Trees appear remarkably immune to cavitation events. Cavities form only when extreme water stresses occur or when sap freezes. Nucleation is heterogeneous in trees, presumably caused by the aspiration of air bubbles through conduit walls. Threshold xylem pressures for cavitation vary greatly between species, in concordance with the great functional and ecological diversity of trees. To cite this article: H. Cochard, C. R. Arterial complications following total knee arthroplasty are rare but carry a high risk of significant morbidity and mortality. Herein, the authors report a case of pseudoaneurysm in a branch of a high-division anterior tibial artery caused by laceration by the retractor following primary total knee arthroplasty in a 72-year-old woman. The patient presented with an unexplained hematoma and an enlarging swelling, with pallor and acute hemorrhage on the first postoperative day. The diagnosis was confirmed by arteriography and the patient was successfully treated using endovascular embolization without long-term complications. Because of the high risk of progression to a potentially limb- or life-threatening condition, it is important to discuss the risk factors for this complication, as well as its early diagnosis and treatment methods.
