This correspondence discusses the recent findings by Straalen et al., highlighting molecular similarities and distinctions between palmoplantar pustulosis (PPP) and dyshidrotic palmoplantar eczema (DPE). The study emphasizes shared proinflammatory pathways and T-cell–related gene upregulation while detailing unique features such as neutrophil involvement in PPP and lipid antigen processing in DPE. We elaborate on histopathological differences, especially intraepidermal vesicle formation in PPP linked to IL–1–mediated pathways and the absence of hyaluronan expression, contrasting with Th2 cytokines-driven spongiosis in DPE. By addressing IL-4, hyaluronan synthases, and keratinocyte adhesion molecules, this correspondence aims to deepen understanding of PPP and DPE pathophysiology.

[Display omitted] Palmoplantar pustulosis (PPP) is an inflammatory disease characterized by relapsing eruptions of neutrophil-filled, sterile pustules on the palms and soles that can be clinically difficult to differentiate from non–pustular palmoplantar psoriasis (palmPP) and dyshidrotic palmoplantar eczema (DPE). We sought to identify overlapping and unique PPP, palmPP, and DPE drivers to provide molecular insight into their pathogenesis. We performed bulk RNA sequencing of lesional PPP (n = 33), palmPP (n = 5), and DPE (n = 28) samples, as well as 5 healthy nonacral and 10 healthy acral skin samples. Acral skin showed a unique immune environment, likely contributing to a unique niche for palmoplantar inflammatory diseases. Compared to healthy acral skin, PPP, palmPP, and DPE displayed a broad overlapping transcriptomic signature characterized by shared upregulation of proinflammatory cytokines (TNF, IL-36), chemokines, and T-cell–associated genes, along with unique disease features of each disease state, including enriched neutrophil processes in PPP and to a lesser extent in palmPP, and lipid antigen processing in DPE. Strikingly, unsupervised clustering and trajectory analyses demonstrated divergent inflammatory profiles within the 3 disease states. These identified putative key upstream immunologic switches, including eicosanoids, interferon responses, and neutrophil degranulation, contributing to disease heterogeneity. A molecular overlap exists between different inflammatory palmoplantar diseases that supersedes clinical and histologic assessment. This highlights the heterogeneity within each condition, suggesting limitations of current disease classification and the need to move toward a molecular classification of inflammatory acral diseases. In the treatment process of atopic dermatitis, pompholyx may occurs on palms and soles. However, there is limited knowledge about which patients are more likely to develop pompholyx and when it is expected to occur. Therefore, this study investigated the occurrence of pompholyx after the start of induction remission treatment. We collected medical information from48 AD patientswho visited Hiroshima University Hospital for the first time between January 2019 and December 2020, from the time of their first visit to 12 months later. In the initial group of 48 patients, 10 individuals developed pompholyx within 1 year after their first visit. There was no clear connection between the occurrence of pompholyx and treatment received. Most of the patients who developed pompholyx were above the age of 30. Serum thymus and activation-regulated chemokine (TARC) levels tended to be higher at the first visit in the group that later developed pompholyx compared to the group that did not. In conclusion, patients over the age of 30 with higher serum TARC levels may develop pompholyx while their AD is improving. It is crucial to provide these patients with information about the possibility of developing pompholyx during the course of their AD treatment and that it would be cured in the course of treatment. In the present case, a 48-year-old Chinese female patient exhibited a decade-long history of palmoplantar erythema, which clinically resembled Dyshidrotic Eczema. Histological examination suggested the presence of ESFA. BACKGROUND: Dyshidrotic dermatitis, dyshidrosis and dyshidrotic eczema have been widely used over the years to refer to an entity that manifests as vesicles on the hands and feet. It is usually related to hyperhidrosis, which is why it was thought that the sweat glands were affected and hence the term dyshidrosis. However, it is now known that the principal histopathological manifestation is spongiotic dermatitis without alteration of the acrosyringium, which is why the term dyshidrotic dermatitis should be discarded when referring to this pathology. METHODS: We retrieved the files of 30 patients of two centers with the clinical or histopathological diagnosis of "dyshidrosis or dyshidrotic dermatitis or even dyshidrotic eczema". RESULTS: Thirty cases, 15 with the histopathological report of "dyshidrotic dermatitis or dyshidrosis" and 15 more with the clinical diagnosis of "dyshidrotic dermatitis or dyshidrosis" were studied under a light microscope. No alterations in the acrosyringium were observed on any of the slides of the 30 patients. CONCLUSIONS: We propose abandoning these terms and using the diagnosis of spongiotic dermatitis in the palms and soles to avoid clinical confusion. [ABSTRACT FROM AUTHOR]

A 45-year-old male with multiple severe vesicles, bullae and pustule on the palmar aspects of both hand and foot diagnosed dyshidrotic eczema by pathology was treated with dupilumab at an initial dose of 600 mg subcutaneously, followed by 300 mg every 2 weeks. The physician’s assessment of the patient revealed an excellent response to the treatment with dupilumab; the lesions and symptoms achieved dramatic improvement on the third day, and at 6 weeks, the hands and feet became completely normal without relapse in the past 1.5 years of discontinuation. Systematic literature searches were performed, and 6 case reports, 5 case series, 2 prospective observational studies and 1 retrospective review with a total of 150 patients were identified to describing the evaluation of efficacy and safety of dupilumab treatment for hand and foot eczema. Dupilumab appears to be safe and well tolerated with clinical benefit in recalcitrant hand and foot eczema. Larger randomized controlled trials using validated outcome measures and detailed hand eczema type and population classification are needed before dupilumab can be applied in clinical settings.Keywords: dupilumab, hand eczema, dyshidrotic eczema, treatment, quick

Chronic hand and foot eczema (CHFE) is a common inflammatory disorder that generally lasts for over 3 months. If it is intractable to topical agents, systemic immunomodulators can be considered; however, they are not suitable for long‐term management because of their adverse effects. Baricitinib is an oral Janus kinase inhibitor that has been approved for the treatment of moderate‐to‐severe atopic dermatitis. However, its effect on CHFE has rarely been described. Herein, we report nine cases of recalcitrant CHFE that were treated with baricitinib after an inadequate response to low‐dose ciclosporin. Dyshidrotic palmoplantar eczema or pompholyx is considered to be a part of the spectrum of atopic dermatitis with a significant impact on the quality of life and limited treatment options. Tralokinumab is a new fully human monoclonal antibody which neutralizes interleukin 13, a chief cytokine in itch pathogenesis and skin barrier defects. Tralokinumab is FDA‐approved for the treatment of atopic dermatitis in adults and EMA‐approved for the treatment of atopic dermatitis in adults and adolescents. We, hereby, report a 40‐year‐old female with severe dyshidrotic palmoplantar eczema who was successfully treated with tralokinumab. 16. Low Dose Oral Alitretinoin With Narrowband Ultraviolet B Therapy for Chronic Hand Dermatitis.

- Abstract
Background: Chronic hand dermatitis (CHD) is difficult to treat and has high individual and societal burdens. Phototherapy and oral alitretinoin are safe monotherapies for CHD, but their combination has not been assessed. Objective: To assess the effectiveness and safety of low dose oral alitretinoin combined with phototherapy versus high dose oral alitretinoin for CHD refractory to topical corticosteroids. Methods: This retrospective study of adult patients with CHD refractory to topical corticosteroid therapy compared low dose oral alitretinoin (10 mg three times weekly) combined with narrowband ultraviolet B therapy (three times weekly; LDA-UVB) to high dose oral alitretinoin (30 mg daily; HDA) for 16 weeks. Outcomes were improvement in disease severity measured by the Physician's Global Assessment and quality of life measured with the Dermatology Life Quality Index. Results: The mean age of the study population (n = 64) was 41.25 years and 57.8% were male. Both cohorts experienced improvements in disease severity and quality of life after 16 weeks, however, significantly more participants who received LDA-UVB (n = 21/33, 63.6%) achieved "clear" or "almost clear" assessments compared to those who received HDA (n = 12/31, 38.7%; P <.05). Adverse effects were significantly more prevalent in the HDA group (P <.0001) and included headache, elevated cholesterol, and dry lips. Conclusions: The combination of low dose oral alitretinoin with narrowband-UVB therapy was more effective and had fewer adverse effects compared to high dose oral alitretinoin for participants with CHD refractory to topical corticosteroid therapy. The emergence of the novel SARS-CoV-2 coronavirus and rapid spread of the COVID-19 disease it causes in late 2019 and early 2020 has since led to a global pandemic announced by the World Health Organization (WHO). The pandemic is a steadily growing social, economic, psychological, and health burden. The infection rates worldwide are climbing. COVID-19 is considered a systemic disease, potentially resulting in acute respiratory distress and cardiovascular failure leading to death. Yet, SARS-CoV-2, a novel and still under-studied virus, has been documented to cause a large variety of symptoms, in many cases including cutaneous manifestations. This paper is a review of medical literature available to date regarding the cutaneous manifestations in the course of SARS-CoV-2 infection. It is also aimed at discussing the significance of dermatological findings for improved diagnosis and treatment of COVID-19 patients. Considering the gravity of the novel coronavirus pandemic, an improved dermatological approach would aid timely diagnosis and effective management of COVID-19, and would facilitate classification of the cutaneous abnormalities observed. Objective To study the etiological pattern of contact sensitizers in patients of dyshidrotic eczema. Methods 50 patients of suspected and provisionally diagnosed as having dyshidrotic eczema were subjected to patch test with Indian Standard Battery developed by Contact and Occupational Dermatoses Forum of India (CODFI). Results were read after 48 hours of application and second reading, if required, was taken after 72-96 hours. Results Patch test analysis of these patients revealed that out of 50 patients 26 (52%) showed positive result with patch test. Paraphenylenediamine (20%) and nickel sulphate (10%) were the commonest allergens, followed by parthenium (8%), fragrance mix (8%) and potassium dichromate (8%). Conclusion Careful history and examination of the patients helps to ascertain the probable relevant allergen responsible for the disease flare up. The patients of pompholyx with positive patch test results can be advised to avoid these allergens. [ABSTRACT FROM AUTHOR]

Dyshidrotic eczema (DE) or acute palmoplantar eczema is a common cause of hand and foot dermatitis in adults. It is a recurrent vesicular eruption affecting the soles, palms, or both. It is very pruriginous and generally appears suddenly. It creates vesicles that, on physical examination, can look similar to "tapioca pudding", which is the characteristic clinical feature of this disorder. It is more common in young adults and affects men and women equally. In this report, we present the case of a 56-year-old man with no relevant past medical history who presented to the hospital with vesicular lesions in his hands and maculopapular lesions in his arms and legs. The patient had characteristic lesions in his right hand consistent with DE and negative workup for bullous pemphigoid, scabies, and bacterial, fungal, and viral infections.

Background. It is unclear whether the respiratory tract is involved in eliciting or aggravating eczematous lesions in patients with vesicular hand eczema. Objectives. To investigate the effect of inhalation of house dust mite (HDM) on vesicular hand eczema. Methods. Eighteen patients with vesicular hand eczema and HDM allergy received inhalation challenges with four concentrations of HDM in a randomized, double-blind, placebo-controlled, cross-over study. Early asthmatic reactions and late asthmatic reactions were defined as a placebo-corrected fall of 15% or more from baseline of forced expiratory volume in 1 second. Hand eczema was scored according to the Dyshidrotic Eczema Area and Severity Index (DASI) at baseline, and 1, 6, 24 and 48 hr. Results. The median DASI increased significantly as compared with baseline at 6 and 48 hr after HDM inhalation. This increase was significantly different between the provocations at 6 hr. The median vesicles score increased significantly from baseline at 24 and 48 hr. Patients with a placebo-corrected increase in the number of vesicles at 24 hr and 48 hr had significantly more often late asthmatic reactions than those without an increase in the number of vesicles. Patients with a placebo-corrected increase of the DASI score at 24 hours had as a group a higher mean total IgE level than those without an increase of the DASI score. Conclusion. Hand eczema increased significantly more after HDM provocation than after placebo provocation. An increase in the number of vesicles was preceded by late asthmatic reactions. 31. Retrospective long-term follow-up in patients with chronic palmoplantar dermatoses after good response to bath PUVA therapy.

Background: Numerous studies have confirmed the short-term effectiveness of 8-methoxypsoralen bath PUVA therapy in patients with chronic palmoplantar dermatoses; however, little is known about long-term results. Patients and methods: In this retrospective study we examined the long-term results in 79 patients (mean age: 48 years) with chronic palmoplantar dermatoses who were treated with bath PUVA three times a week over an 8-year period. A good clinical response (a reduction of more than 50% of the skin lesions) occurred after a mean of 23 treatments and a mean cumulative UVA dose of 39 J/cm
2 in 51 patients (65%). In 2007 a questionnaire was sent to these 51 patients to assess the long-term outcome. Results: With bath PUVA treatment, the best results were found in patients with hyperkeratotic eczema (17/22; 77% good clinical response) followed by patients with palmoplantar psoriasis (26/41; 63%) and patients with dyshidrotic eczema (8/16; 50%). Thirty-four patients (67%) answered the questionnaire after a mean follow-up interval of 4.3 years (10-87 months). Among these patients, 36% reported an improved course of disease after PUVA therapy with reduced frequency and/or intensity of the skin rash, and 29% of patients reported continued complete clearance. 79% of our patients reported a long-term reduction in the use of topical corticosteroids during the follow-up period (mean: 4.3 years). In addition, 67% of patients reported a lasting improvement in quality of life. Conclusions: These data show that bath PUVA may have a long-term, beneficial influence on the course of disease in a majority of patients with recalcitrant chronic palmoplantar dermatoses. Introduction: Intravenous immunoglobulin (IVIG) is used to treat many inflammatory and autoimmune disorders and although generally well tolerated, cutaneous side effects occur. Objective: We reviewed reports of pompholyx and eczematous reactions associated with IVIG. Methods: A literature search was performed using the PubMed and MEDLINE databases with the search terms “intravenous immunoglobulin pompholyx,” “intravenous immunoglobulin eczema,” “intravenous immunoglobulin cutaneous adverse effects,” “intravenous immunoglobulin cutaneous effects,” “intravenous immunoglobulin skin effects,” and “intravenous immunoglobulin adverse effects.” Relevant English-language articles or articles in other languages cited in English-language articles were included. Results: We identified 64 cases of eczematous reactions associated with IVIG therapy, including a patient treated on our inpatient consult service. In reported cases, the majority of patients (62.5%) had pompholyx alone or a combination of pompholyx on the hands or feet and two or fewer additional body surfaces involved. The majority of reported cases (75%) experienced the eczematous reaction after their first IVIG treatment. Neurologic conditions were the most common (85.9%) diseases for which IVIG was used. Most patients responded well to topical steroids or did not require treatment. Limitations: Some reported cases had insufficient descriptions to be included in this review. A literature review may underestimate the frequency of eczematous reactions to IVIG because these reactions are often limited and may not be reported. Context: Pompholyx (called dyshidrosis by some) is one of the most common conditions and its immune response is presently poorly understood. Case report: We describe a 58 year old African American female with a clinical history of rheumatoid arthritis and type II diabetes who presented a chronic five-year, itchy vesicular/blistering rash involving her hands and feet. A lesional skin biopsy was taken for hematoxylin and eosin (H & E) analysis. In addition, a multicolor direct immunofluorescence (MDIF) and immunohistochemistry (IHC) studies were performed. The major findings to be reported were: the H & E examination revealed spongiotic dermatitis and pompholix. IHC and MDIF studies demonstrated focally deposits of positive CD45, CD3, CD8, anti myeloperoxidase (MPO), and anti-human IgE, C3C, C3D and anti-humanfibrinogen within the epidermal spongiotic process, as well as around the blood vessels surrounding the inflammatory process especially at the sweat glands and respective ductus. The patient began mycophenolate mofetil therapy, with successful clearing of the palms and soles. Conclusion: The significance of our findings indicates a complex immunological process including complement, MPO and T-cell immune response. In addition, possibly a secondary allergic process for the presence of IgE immune response and possibly aggravation by application of other medicines. Dyshidrotic eczema (DE) is a chronic and/or recurrent vesicular eczema of the palmar sides of the hands and sometimes the soles of the feet. The major aetiologies to be considered are atopic DE, contact DE, systemic contact DE and DE due to fungal or microbial allergy. To clarify the role of occupational contact allergens, 59 workers with DE were examined to determine whether they had atopy, environmental contact sensitization and/or contact allergy coupled with work activity. For all, patch testing was carried out using the European standard series and the material brought in by the patients. No significant difference was found between the rate (42.4%) of history of atopy among the 59 workers with DE and that (30.6%) observed among 160 workers with other forms of hand eczema (HE). Positive patch test reactions were found in 72.8% of dyshidrotic patients. A relationship to occupational activity was strong for 18 (30.5%) of them but this rate was significantly lower than that (55%) observed among the 160 with other types of eczema. Results of the present study showed that contact sensitization is as much involved in workers with DE as in workers with other form of HE, even though the relationship to work is significantly less frequent for DE. In some cases that have been diagnosed as contact allergy to nickel, there are repeated cutaneous eruptions of pompholyx, even in areas with no direct contact with the metal. The possible alimentary origin of dyshidrotic eczema should be considered when deciding on therapy. We have collected the clinical data for 24 patients with dyshidrotic eczema caused by nickel, to evaluate the benefit of a low-nickel diet versus treatment with oral disodium cromoglycate, comparing both objective and subjective symptoms. A low nickel diet does not improve these patients but those treated with DSCG reacted better, from both objective and subjective point of view, than either the controls or the patients treated by diet. We next did intestinal permeability tests before therapy and after 15 days of treatment. We found that nickel uptake diminishes simultaneously with the reduction of absorption through the smaller aqueous "pores". This phenomenon was greatest after DSCG. We suggest that DSCG can help selected cases of pompholyx. [ABSTRACT FROM AUTHOR]

The article reports on the outcome of an open pilot study on the effectiveness of Khellin, a furanochrome chemically resembling the structure of methoxypsoralens, for the treatment of recalcitrant palmoplantar pompholyx. Khellin (Kh) has been used as an either topically or orally administered photochemotherapy agent for vitiligo and psoriasis. Subjects were treated with a daily topical application of a commercial Kh preparation one hour before exposure to natural sunlight and the results were positive. Kh-PCT can be a valuable treatment but reliable candidates to this outpatient regimen should be selected with caution.

Dyshidrotic eczema, also known as dyshidrotic dermatitis or pompholyx, is characterized by pruritic, tense, deep-seated vesicles mainly on the palms and lateral surfaces of the fingers. In the chronic phase, scaling, desquamation, fissuring, and, sometimes, lichenification may be seen. The peak age of onset is between 20 and 30 years of age. The sex incidence is approximately equal. Most cases are idiopathic. Predisposing factors include atopy, contact allergens, contact irritants, dermatophyte infection, allergy to ingested metal, hyperhidrosis, prolonged use of protective gloves, intravenous immunoglobulin, psychological stress, and smoking. Although the disease is benign, it tends to run a chronic and relapsing course. Successful treatment requires a systemic multipronged approach that consists of avoidance of triggering factors, optimal skin care, pharmacotherapy during acute exacerbations, and education of patients/caregivers. Ultrapotent topical corticosteroids are the mainstay of pharmacotherapy.

Background: Dyshidrotic eczema of the palms and soles is a common condition, which can be rather resistant to treatment. Therapy studies and their comparability are of clinical importance. Objective: As standardized assessment methods for the severity of this particular form of eczema are lacking, we developed a severity index for dyshidrotic eczema. Methods: The Dyshidrotic Eczema Area and Severity Index (DASI) is based on the severity grade of single items – number of vesicles per square centimetre (V), erythema (E), desquamation (S) and itch (I) – and the extension of the affected area (A) and is calculated with defined score points (p) as: DASI = (pv + pE = pS + pI) × pA. Results: In two treatment studies on dyshidrotic hand eczema, the DASI was found to be a simple and useful tool to assess the severity of dyshidrotic eczema and the effect of therapy. Conclusion: The DASI needs to be further validated in larger cohorts.

UVA 1 therapy is a relatively new form of light therapy for atopic dermatitis. We describe that after 4 weeks of UVA-1 patients were better capable to maintain clinical improvement than after 3 weeks of therapy. Furthermore, UVA 1 therapy proved to be better than placebo therapy in patients with dyshidrotic eczema. Also, we described the favorable effect of UVA 1 therapy in chronic, generalized lichen ruber planus. Because of deep penetration of UVA 1, it could not only influence dermal inflammatory infiltrates in T-cell mediated diseases mentioned above, but also reach circulating activated B-cells in dermal capillaries in SLE. We investigated the effect of 6 and 12 J/cm2 UVA 1 in SLE patients, in a double-blinded, placebo-controlled, cross over study. Three weeks of 12 J/cm2 resulted in improved disease activity and proved to be more effective than placebo. Furthermore, a decrease of auto-antibody titers was observed in several patients. In vitro we showed that 40% of UVA 1 reached the dermis and increasing numbers of PBMCs died after rising doses of UVA-1. Also, immunoglobulin production by activated B cells decreased after increasing doses of UVA 1. It would be interesting to investigate UVA 1 therapy for other auto immune diseases.

Bakalářská práce se zabývá onemocněním kůže postihující bříška prstů a dlaně. Pro tuto práci jsou vybrány nemoci dyshidrotický a hyperkeratotický ekzém. Otisky s těmito nemocemi z databáze výzkumné skupiny STRaDe jsou analyzovány. Je navržen postup, jak generovat tyto nemoci do otisků prstů vytvořených pomocí generátoru syntetických otisků prstů Anguli. Tento postup je následně implementován a vyhodnocen otestováním pomocí algoritmů na porovnání otisků prstů., This bachelor thesis deals with skin diseases affecting fingers and palms. For this thesis dyshidrotic eczema and hyperkeratosis has been chosen. Fingerprints with these diseases from database of research group STRaDe is analyzed. Procedure, how to generate this diseases into fingerprints with generator Anguli, is proposed. Procedure is implemented and evaluated by tests of algorithms for fingerprints comparison.
