This study analyzes the work situations experienced by professionals in the sanitary fiscalization of medicines at the Brazilian Health Regulatory Agency, during the COVID-19 pandemic. This is a single-case study, qualitative in nature, with an ergological approach. The data were collected from 2020 to 2021. During this period, 18 interviews and 3 online work meetings were conducted. The discursive practices were systematized in ergodialogical maps, resulting in three categories: the works experienced in the sanitary enforcement of medicines; the variabilities of the organizational context and the renormalizations facing the pandemic. The fragilities of the information systems, the outdated regulatory framework, the performance model, the fragmentation of the work, among others, have caused barriers to the professional’s activity. The conformation of a new way of performing the sanitary fiscalization of medicines involves the combination of interventions aimed at prevention, detection, and response to the consumption of medicines that are harmful to health. The necessary transformation goes beyond the restricted function of control and punishment and consolidates itself as a new proactive and dynamic action model, based on health risk management and the implementation of Good Fiscalization Practices. Primary Health Care (PHC) intends to rearrange services to make it more effective. Nevertheless, effectiveness in PHC is quite a challenge. This study reviews several articles regarding the effectiveness improvements in PHC between 2010 and 2020. Ninety out of 8,369 articles found in PubMed and the Virtual Health Library databases search were selected for thematic analysis using the Atlas.ti® 9.0 software. There were four categories identified: strategies for monitoring and evaluating health services, organizational arrangements, models and technologies applied to PHC. Studies concerning the sensitive conditions indicators were predominant. Institutional assessment programs, PHC as a structuring policy, appropriate workforce, measures to increase access and digital technologies showed positive effects. However, payment for performance is still controversial. The expressive number of Brazilian publications reveals the broad diffusion of PHC in the country and the concern on its performance. These findings reassure well-known aspects, but it also points to the need for a logical model to better define what is intended as effectiveness within primary health care as well as clarify the polysemy that surrounds the concept. We also suggest substituting the term "resolvability", commonly used in Brazil, for "effectiveness". [ABSTRACT FROM AUTHOR]

Objective. To identify the strategies employed by regulatory systems for the market surveillance and control of substandard, falsified, and unregistered medicines at the regional-global levels, especially regarding drug recall procedures. Method. An integrative literature review was performed. Searches were performed in MEDLINE via PubMed, Embase, and SciELO to select articles published from 2007 to 2019 in English, Portuguese, and Spanish, covering national regulatory system initiatives, with a focus on the recall of substandard, falsified, and unregistered medicines. Results. Of 483 articles initially identified, 21 global, regional, or national scope studies were selected. Prevention, detection, and response strategies, including drug recall, were grouped according to two broad market surveillance and control models (passive-reactive and proactive) used by regulatory systems. These models seem to combine passive and proactive, complementary or concurring actions that varied according to country development level and regulatory capacity. Although considered the most effective response for protection of populations, medicine recall was not implemented in a uniform manner in different regulatory systems as indicated by the studies. Conclusions. Addressing the complexity and magnitude of the problem of substandard, falsified, and unregistered medicines will demand effort, investment, and profound changes in the approaches, processes, and capacity of regulatory systems, with market surveillance and control strategies possibly converging toward a hybrid, multisectoral, multidisciplinary, global, and systemic model of human health protection. This article analyzes federal guidance on the organization of primary healthcare work processes and the modes of production of care aimed at promoting universal access to services. A qualitative document analysis was undertaken of documents related to the National Health Policy, National Program for Improving Primary Care Access and Quality, More Doctors Program, and National Tele health Networks Brazil Program. Five thematic categories were defined for content analysis: staff training, valuing staff and work, organization of health actions for/with people and communities, complexity of healthcare and multiprofessional work, and collective construction of health as a right. The results show that advances were made in promoting universal access, particularly in the documents issued before 2015, with actions related to staff training, welcoming, the establishment of evaluation processes, and expansion of the scope of actions. However, there has been a shift towards the universal coverage model, as shown by measures encouraging reductions in the functional diversity of health teams and the flexibilization of working hours, hindering the implementation of the SUS as an inclusive social policy. This study was multicenter with a qualitative approach, which sought to identify the elements that can increase or reduce the workloads of the Family Health nurse. Forty nurses were interviewed, from 36 teams from five regions of Brazil, considered successful according to the requirements of the National Policy of Primary Care and with good evaluation in the National Program for Improvement in Primary Care Access and Quality. Data collection was performed by instrument triangulation, using a semi-structured interview, observation and documentary study, from 2013 to September 2016. The findings were analyzed based on the Thematic Content Analysis and categorized with the help of Atlas.ti software. It was identified that the elements that most influence the increase of the workloads are the precariousness and deficits in the work environment, materials and equipment, added to the numerical deficit of the workforce and the excess of health care demand. However, teamwork, recognition for the performed work, bonding with users and good interpersonal relationships, all contribute to reduce the workloads. We highlight the dialectic present in the elements of the work process. They can increase or decrease workloads influenced by objective conditions, the moment and way they are handled. The objective is to analyze the insertion of the pharmacists work in primary health care in Brazil. The search was performed on BVS, SciELO, Lilacs e Medline databases from 1998 to 2016. From the 157 articles found, excluding the duplicates, theses, dissertations and reviews, after the complete reading the review included 9 articles dealing with the pharmacist's work describing experiences, attributions, potentialities, difficulties and challenges. Results show incipient production and predominance of qualitative studies starting in 2007. The insertion in the team is the central topic of the studies, pointing out the challenges and the difficulties related to the recognition and acceptance of the pharmacists interventions. The potentialities reside in the area of actions directed to the client, the families and the team, as well as in the professional training field as well as in the dissemination of the results of the pharmaceutical actions. The pharmacists' isolation in the primary care prevails, albeit there are perspectives of strengthening the integration in the team that has been stimulated by the recent institutional and regulatory transformations in the national scenario. Beyond the present difficulties and experiences reported, the researchers focus on the potentialities for the professional practice, glancing at the construction of the future. [ABSTRACT FROM AUTHOR]

The Brazilian National Telemedicine Program in indigenous healthcare assists health professionals working in remote areas in strengthening the healthcare provided to indigenous populations. The current study aimed to analyze the implementation of the National Telemedicine Program in indigenous people's health, from the perspective of health administrators. This was a qualitative exploratory descriptive study using document analysis and open-ended interviews with 10 administrators. Content analysis resulted in two categories: the program's process implementation and the potential of telemedicine for indigenous people's healthcare. The results emphasize the importance of dialogue between all institutions involved and the construction of a democratic forum for evaluating this process and related decision-making: resumption of the program's implementation and subsequently its expansion and improvement of the available resources and the search for other applicable strategies for indigenous people's health. A broad discussion on this topic is recommended that involves the indigenous people's strategies for social control.
