Prostate lymphomas are very rare, with few reported cases in the literature. However, differential diagnoses should be considered. We report a hematolymphoid tumor of the prostate in a 67-year-old man who was admitted to the hospital with hematuria and obstructive urinary symptoms. The patient underwent a cystoscopy procedure to obtain a biopsy from the prostate with bipolar resection. Histologically, sheets of lymphoid tumor cells resembling non-Hodgkin lymphoma, infiltrated into the prostatic tissue, were identified. The diagnosis of diffuse large B-cell lymphoma was confirmed by immunohistochemistry. The clinicopathologic characteristics of prostatic lymphomas are discussed in the light review literature.

Summary: Chimeric antigen receptor (CAR) T‐cell (CAR‐T) therapy can provide durable remission in patients with relapsed or refractory diffuse large B‐cell lymphoma (DLBCL) after failure of chemoimmunotherapy. However, patients who are refractory or relapsing after CAR‐T therapy have poor outcomes. Multiple mechanisms of CAR‐T therapy failure have been proposed but management of these patients remains a challenge. As CAR‐T therapy moves earlier in the treatment of DLBCL, we urgently need trials focused on patients with relapse after CAR‐T therapy. Recent advances in novel immunotherapies such as bispecific antibodies, antibody–drug conjugates and next‐generation CAR‐T therapies may provide avenues for treatment. Primary pulmonary diffuse large B‐cell lymphoma is a rare entity. We describe a case of pulmonary lymphoma with multiple nodules mimicking metastases in a treated patient with rheumatoid arthritis. A 73‐year‐old man was diagnosed with rheumatoid arthritis at the age of 30. He was treated with leflunomide. He was followed up for a nontuberculous mycobacterial infection. He underwent percutaneous coronary intervention for acute myocardial infarction at the age of 70. In April 2022, routine follow‐up revealed new‐onset multiple nodules on chest computed tomography (CT). A position emission tomography/CT scan with 18F‐fluorodeoxyglucose showed a low‐high maximum standardized uptake value by multiple nodules. Pathologic examination of a video‐assisted thoracic surgical biopsy revealed pulmonary diffuse large B‐cell lymphoma. Systemic chemotherapy with rituximab, cyclophosphamide, vincristine, and prednisolone reduced and eliminated multiple nodules. To characterise the radiological features of orbital lymphoma subtypes This was a multicentre, retrospective study to analyse radiological and clinical characteristics of orbital lymphoma by histological subtype across two sites within Australia and the United States. A total of eighty-eight patients were included. The most common subtypes were extranodal marginal zone lymphoma [EMZL] (48, 54.5%), follicular lymphoma [FL] (16, 18.2%), and diffuse large B-cell lymphoma [DLBCL] (15, 17.0%). Clinically, significant associations were found between DLBCL and vision change (p <.01), pain (p <.01), extraocular movement limitation (p =.01), and optic neuropathy (p =.01). Radiologically, there was no significant difference between the individual histopathological sub-groups with respect to bone destruction (p =.30), optic nerve involvement (p =.30) and diffuse appearance (p =.84). However, if categorised as either aggressive or indolent, aggressive lymphoma subtypes were significantly more likely to demonstrate globe indentation radiologically than indolent subtypes (Odds Ratio = 5.5, 95% CI: 1.3, 23.3; p =.02). DLBCL was significantly associated with vision change, pain, extraocular movement limitation and optic neuropathy clinically. Aggressive lymphoma subtypes were significantly more likely to demonstrate globe indentation radiologically. Abstract Objective Diffuse Large B Cell Lymphoma (DLBCL) is the most common type of Non-Hodgkin Lymphoma (NHL). The aim of this study was to assess the clinico pathological characteristics of DLBCL specifically, among the affected individuals residing in Northern areas of Pakistan who had not been previously included in major lymphoma studies due to their remote location. Results Mean age of the patients was 49.7 years. Male: female ratio was 1.5:1. Primary site was lymph node in 99 (71.74%) patients, out of which, 36 (26.09%) patients had B symptoms and 19 (13.77%) patients had stage IV disease. 39 (28.26%) patients had primary extra nodal involvement, 4 (2.90%) patients had B symptoms and 3 (2.17%) had stage IV disease. Extra nodal sites involved in primary extra nodal DLBCL were gastrointestinal tract (GIT) 19 (48.72%), tonsils 6 (15.38%), spine 4 (10.26%), soft tissue swelling 3 (7.69%), parotid gland 2 (5.13%), thyroid 2 (5.13%) central nervous system (CNS) 1 (2.56), breast 1 (2.56%) and bone marrow 1 (2.56%). Our study revealed increased percentage of patients with nodal DLBCL in stage IV and with B symptoms. Few patients with primary extra nodal DLBCL had B symptoms and stage IV disease at presentation. GIT was the most common site of involvement in primary extra nodal DLBCL.

Standard treatment of primary central nervous system lymphoma (PCNSL) in countries with limited resources remains conventional chemotherapy, with or without whole brain radiotherapy (WBRT). To evaluate the treatment outcomes, prognostic factors and costs in patients with PCNSL treated with high-dose Methotrexate, vincristine and procarbazine, plus Rituximab (MVP-R) followed by consolidation with reduced dose (rd) WBRT and Cytarabine chemotherapy. We conducted an institutional audit of the first line treatment of patients with PCNSL, who were treated with MVP-R regimen, WBRT, or both between September 2011 and January 2020. Long term neuro-cognitive toxicity was recorded on follow up. The 5-year overall survival (OS) was the primary end point. Of 54 patients, 42 were evaluable [median age: 54 years (19–73 years)]. The commonest subtype was activated B-cell subtype (90%). At presentation, multiple and deep brain lesions were reported in 38 and 73% patients, respectively. Combined chemoimmunotherapy was given to 41 patients and WBRT to 29 patients. 27 patients (65%) achieved a complete response, and 22 received rdWBRT. 7 patients with partial response received conventional dose WBRT. Among tested prognostic factors, response to treatment was the single most significant determinant. At a median follow-up of 58 months, the 5-year progression free survival was 42%, and 5-year OS was 60%. The median direct hospital costs incurred by most patients for investigations and treatment were $1976.45 and $12,078.49, respectively. The OncoCollect Registry was set up in 2017 to address the challenges in the collection of retrospective data through chart review, recording access to anthracycline and rituximab-based treatment, and to study outcomes and any improvement in the patient follow-up. Methodology: The OncoCollect Lymphoma group registry was set up at a national level with 9 participating centers. Lymphoma patients registered at these centers between 2011 and 2017 were included. The clinical features, prognostic stratification, associated comorbidities, response to first-line treatment, and 3-year outcomes of adult patients with DLBCL were analyzed. Results: Of the 5,886 lymphoma patients registered in the OncoCollect registry, 2,581 (44%) had DLBCL. A total of 1,961 were evaluable for frontline therapy. The median age at presentation was 57 years. Gender ratio was 1.6:1. At presentation, 43% were early stage, 70% had low and low intermediate IPI, 53% had extranodal disease, and 30.9% had one or more comorbidities (data available for 1,136 patients). The commonest extra nodal site was gastro-intestinal (23.98%) followed by head and neck (19.24%). The overall response rate was 79.29%. Complete remission was seen in 61.75%, partial response in 17.5%, stable disease in 4.3%, and progression in 7.9%. Patients who received anthracycline-based therapy (86.7%) and rituximab-based therapy (83.7%) had a 3-year event-free survival (EFS) of 69.67% and 68.48%, respectively. With a median follow-up of 33 months, the 3-year overall Survival (OS) and EFS were 75.37% and 66.58%, respectively. Conclusions: DLBCL remains the commonest (44%) lymphoma subtype and is curable with standard anthracycline- and rituximab-based therapies. Primary pulmonary diffuse large B cell lymphoma (PPDLBCL) is extremely rare, with fewer than 40 cases reported to date and a lack of systemic analysis. Herein, we present a case of PPDLBCL mimicking metastasis in a heavily treated patient with breast cancer. To our knowledge, this is the first reported case of PPDLBCL in a patient with breast cancer. A 66-year-old Chinese female diagnosed with breast cancer 7.5 years previously and multiple bone metastases 31 months later presented with a new-onset subpleural nodule in the inferior lobe of left lung detected by routine follow-up in November 2017. A 18F-fluorodeoxyglucose (FDG) positron emission tomography-computed tomography scan showed that the pulmonary nodule was hypermetabolic with a maximum standard uptake value of 14.9, consistent with lung metastasis in view of her history of breast cancer and multiple bone involvement. Surprisingly, pathologic investigation revealed primary lung DLBCL, staged IEA. Systemic chemotherapy with R-CDOP (rituximab, cyclophosphamide, vindesine, doxorubicin liposome, and prednisone) achieved complete remission with mild side effects. At the latest follow-up in August 2019, the patient had disease-free survival of 21 months. The findings from this case indicate that primary pulmonary lymphoma should be included in the differential diagnostic checklist of pulmonary occupancy, even in solid tumor patients treated with multiple modalities. When a newly developed lung nodule is identified in such patients, clinicians should not take for granted that it is lung metastasis. In late 2017, the Food and Drug Administration (FDA) approved CD19 CAR T-cell therapy for the treatment of relapsed or refractory diffuse large B-cell lymphoma (DLBCL) after two or more lines of systemic therapy. Since 2017, two anti-CD19 CAR T-cell therapy products, axicabtagene ciloleucel and tisagenlecleucel, have been approved by the FDA for the treatment of relapsed or refractory large B-cell lymphoma after the failure of two lines of systemic therapy (Nair & Neelapu, [15]). CAR T-cell therapy's potential for the treatment of autoimmune diseases such as APLS is based on its ability to specifically target the B cells that produce the autoantibodies causing these diseases. In conclusion, we report a patient with refractory DLBCL who was also undergoing lifelong anticoagulation therapy for APLS, and whose DLBCL and APLS remissions are likely attributable to CAR T-cell therapy. Lymphomas, the malignant neoplasms of the reticuloendothelial and lymphatic system, are classified based on characterisitic morphological pattern, immunophenotypic pattern and sometimes distinctive chromosomal aberrations. Most of the intraosseous lymphomas should be metastatic, and primary intraosseous lymphoma is a rare type of lymphoma, which accounts for only about 5% extranodal lymphoma. It accounts for approximately 3% of all primary bone malignancies.We report a case of 54 years lady presented with pain over right thigh since 3 to 4 months which suddenly increased. The clinico-radiological differential diagnosis was fibrous dysplasia, plasmacytoma or metastasis.The histological findings and immunohistochemistry being positive for CD20 and BCL2 negative are those of Lymphoma- Diffuse large B cell type. Radiotherapy and/or chemotherapy is the preferred modality of management.

Context: Primary extranodal lymphoma (pENL) refers to group of disorders arising from tissues other than lymph nodes. The incidence of pENL is increasing and is probably due to better diagnostic immunophenotyping and imaging modalities. Hence, this study was undertaken to ascertain the incidence, distribution, and histological subtypes of extranodal non-Hodgkin lymphoma (NHL) in a tertiary care institute in South India. Subjects and Methods: This was a retrospective study of patients diagnosed to have histologically proven NHL. The demographic and clinical features, laboratory parameters, imaging findings, histopathology, and immunophenotyping were documented. The lymphomas were grouped as extranodal and nodal. The data were tabulated in a Microsoft Excel sheet, and descriptive analysis was done. Results: Primary extranodal NHLs constituted 35.96% (41/114) of all NHLs. The B symptoms were less common in pENL compared to nodal NHL. Gastrointestinal tract (GIT) constituted the most common extranodal site (19/41, 46.34%), and diffuse large B-cell lymphoma (DLBCL) was the most common histological subtype. Majority (40/41, 97%) of the patients with pENL were immunocompetent. 31/41 (75%) patients were in Stage I-II compared to 58/73 (79.4%) patients in Stage III-IV in nodal NHL. Conclusions: Primary extranodal NHL constituted about one-third of patients diagnosed to have NHL at our center with the GIT being the most common site of presentation and DLBCL being the most common histology. A strong suspicion of NHL at an extranodal site with appropriate pathological and immunophenotyping evidence is needed to establish the diagnosis of a pENL. [ABSTRACT FROM AUTHOR]

Background/aim: Diffuse large B-cell primary gastric lymphomas (DLBC-PGLs) are treated with different therapies. Their optimal treatment is not well documented. Materials and methods: We retrospectively analyzed the data of 51 patients diagnosed with DLBC-PGL in the previous 10 years. All patients were treated with R-CHOP as first line. Radiotherapy was added to chemotherapy in 8 patients. Surgery was performed in 5 patients. Results: The median follow-up time of the 51 patients was 45.5 (range 5-144) months and the complete response (CR) rate was 90.2%. CR was achieved in 34 (89.4%) of 38 patients treated with single chemotherapy, in all (100%) 5 patients treated with chemotherapy plus surgery, and in 7 (87.5%) of 8 patients treated with chemotherapy plus radiotherapy. The 5-year overall survival (OS) and event-free survival (EFS) rates were 85.8% and 89.6%, respectively. The 5-year OS and EFS rates were not significantly different between patients treated with single chemotherapy or chemotherapy plus radiotherapy/surgery (P > 0.05). Conclusion: R-CHOP chemotherapy is as effective as R-CHOP plus radiotherapy/surgery in the treatment of DLBC-PGL patients. Recent advances have led to major progress in the classification of lymphomas with better understanding of the cellular immunophenotyping and molecular biology. The expression of CD5 and presentation in the leukemic phase in patients diagnosed with diffuse large B-Cell lymphoma are usually stigmata of an aggressive disease with a poor prognosis. Controversy arises concerning the management of these aggressive forms of DLBCL with the absence of clear recommendations guiding the oncologist's choice. Overall survival in diffuse large B-cell lymphoma (DLBCL) has significantly improved in the last decade, especially after the incorporation of rituximab. Involvement of the central nervous system (CNS) at presentation or at recurrence is an uncommon event, but carries a dismal prognosis with median survival of less than 6 months. Although prophylactic CNS directed therapy is a widely used approach to prevent this complication, randomized clinical trials have been very limited. CNS prophylaxis has inherent toxicities; therefore, identifying the population of patients who would receive most benefit is of utmost importance. From an extensive review of current literature, we report the incidence of CNS relapse in DLBCL and describe the role of CNS prophylaxis in the post-rituximab compared to the pre-rituximab era. We also review the current modalities of CNS prophylaxis and attempt to identify the high-risk patients who would benefit. The breast is an uncommon site of involvement in non-Hodgkin lymphoma, and primary breast lymphoma (PBL) is a disease localized to one or both breasts with or without regional lymph nodes involvement. The objectives of the study were to review the clinical profile, epidemiological parameters and assess the outcomes exclusively in women with primary diffuse large B cell lymphoma (DLBCL) of breast. This was a retrospective observational study done at Kidwai Memorial Institute of Oncology, Bangalore, India. We studied 6 consecutive female patients, diagnosed with primary DLBCL of breast between January 2007 and December 2011. Median age at diagnosis was 45 years (range 33-56 years). B symptoms were present in 3 patients. One patient had central nervous system involvement with high risk International Prognostic Index (IPI). 3 patients underwent lumpectomy and 3 core biopsy. All received anthracycline based chemotherapy, with rituximab in one patient and 3 received involved field radiotherapy. Three patients achieved complete response; one is disease free at 15 months. Two relapsed at 8 and 53 months and both were alive with disease. One achieved partial response, one had progressive disease and response was not assessed in one (but died due to toxicity). Primary breast DLBCL is a rare entity and multi modality combination therapy involving chemotherapy and radiation can give a longer overall survival and thus avoiding the morbidity of mastectomy. Primary breast lymphoma (PBL) is an uncommon type of extranodal non-Hodgkin's lymphoma (NHL), accounting for approximately 1% of all extranodal NHL. Diffuse large B cell lymphoma (DLBCL) is the most frequent histology. Both primary mucosa-associated lymphoid tissue (MALT) lymphoma of the breast and its relapse have been documented previously. We report on a 63-year-old woman with primary DLBCL of the breast which was diagnosed 8 years after gastric MALT lymphoma. Following chemotherapy, complete response was obtained. We suggest that for women presenting with a breast mass who received cytotoxic treatment for the other lymphoma, PBL should be considered in the differential diagnosis of the breast mass together with primary breast carcinoma. Objective: To investigate the clinical manifestation, imaging and histological features of different histological subtypes of non-Hodgkin's lymphoma of nasal cavity and paranasal sinuses. Method: Fifteen NHL patients of the sinonasal region were collected from the Department of Otolaryngology of Peking University Third Hospital from 2010 to 2016. HE staining and immunohistochemical staining were performed. The clinical characteristics and imaging features of different subtypes were described and analyzed. Result: :We analyzed a total of 6 patients with localized sinonasal diffuse large B cell lymphoma and 9 patients with localized sinonasal extranodal NK/T cell lymphoma. The age distribution for these two subtypes is very distinct. The median age of the patients with localized sinonasal extranodal NK/T cell lymphoma was 39 years. There were 5 males and 4 females. Nine sinonasal NHLs were NK/T-cell lymphoma, nasal type, all of which were infected with EBV. The median age of the patients with localized sinonasal diffuse large B cell lymphoma was 64 years. There were 3males and 3 females. Symptoms for patients with SN-DLBCL and SN-ENKTL were significantly different in epiphora, proptosis, diplopia and nasal congestion ( P =0.18, 0.004, 0.18, 0.18). Imaging features for patients with SN-DLBCL and SN-ENKTL were significantly different in tumor extended to orbit and inferior turbinate ( P >0.05). Positive staining for CD 56 was detected in 9 patients, for CD 3 in 9 patients, for EBER in 9 patients. The Hans algorithm identified 1 patient with the germinal center B-cell (GCB) subtype and 5 with the non-GCB subtype.Compared with the control group, the observation group was significantly better than the control group ( P < 0.01). Conclusion: Early symptoms of epiphora, proptosis, diplopia, and images finding with orbital invasion should be highly suspected of diffuse large B cell lymphoma. Lymphomas may be broadly divided into non-Hodgkin (90%) and Hodgkin (10%) types. Most lymphomas (90%) are of B cell origin but can also be T cell or natural killer cell. Clinical management of indolent and aggressive lymphomas is different. Aggressive lymphomas are more dangerous if left untreated yet a higher cell proliferation rate also renders them more chemosensitive, so they are managed with curative intent. Objective: Non-Hodgkin lymphomas (NHL) in the palate are very rare. We aimed to investigate the CT appearance of lymphoma in the palate to improve diagnostic quality and review the literature on NHL., Materials and Methods: The study retrospectively included patients with histopathologically confirmed lymphoma who were treated at our hospital between 2008 and 2015. We examined CT features, including tumor location, appearance, margins, and involvement of the surrounding tissues., Results: Records were available for eight cases of lymphoma over the study period. Of these, two were in the soft palate and six in the hard palate. Median age at presentation was 63 years. Seven of the eight were diffuse large B cell lymphoma and one was T cell lymphoma. Hard palate lymphoma on CT showed bone destruction and spread to the maxillary arch and maxillary sinuses, whereas soft palate lymphoma spread to the surrounding parapharyngeal area and did not show bone destruction., Conclusion: We describe CT findings of palate lymphoma, which are important for treatment planning.

The non-Hodgkin lymphomas (NHLs) are a heterogeneous group of diseases with variable clinical outcomes. Autologous hematopoietic stem-cell transplantation (ASCT) as frontline, consolidative therapy has been evaluated based upon histological subtype of NHL. In this review, we summarize the major clinical trials guiding the use of frontline ASCT in NHL. With the constantly changing landscape of upfront therapy and multiple promising novel agents, the ability to conduct randomized trials to evaluate the benefit of consolidative ASCT is not only challenging but may be considered by some an inept utilization of resources. Our recommendation for consolidative ASCT is based on analyzing the current available data.
