A 38-year-old lifelong smoker woman with a diagnosis of lung adenocarcinoma was referred to us to perform an endoscopic ultrasound with fine-needle aspiration (EUS-FNA) procedure in order to define liver lesions suspicious for metastases. She had already performed a percutaneous procedure with ultrasound guidance, which turned out to be not diagnostic. Plasma cell dyscrasias (PCDs) are characterized by a bone marrow plasma cell clone producing a monoclonal component (MC). This heterogeneous group comprehends malignant diseases as multiple myeloma (MM), conditions requiring only observation, like monoclonal gammopathy of undetermined significance (MGUS), and rarer entities where the MC causes organ dysfunction in absence of malignant PCD [(i.e. monoclonal gammopathy of clinical significance (MGCS)]. The evaluation of urine proteins has a key role in the management of PCDs, particularly quantification of proteinuria and albuminuria in the diagnostic workflow and clinical management of MGCS with renal involvement. In immunoglobulin light chain (AL) amyloidosis, proteinuria (cut-off: 0.5 g/24h) and urine albumin/creatinine ratio (UACR; cut-off: 300 mg/g) are used for diagnosis of renal involvement. These biomarkers are also included in staging systems that stratify the severity of renal involvement (proteinuria cut-off: 5 g/24h; UACR cut-off: 3 600 mg/g) and in renal response criteria (decrease of proteinuria/UACR >30% from baseline). In light chain deposition disease (LCDD), relatively low grade proteinuria (cut-off: 2 g/24h) results in higher risk of end-stage renal failure and its reduction after treatment (>50% from baseline) associates with better outcome. Finally, the presence of a MC and tubular proteinuria raises suspicion of light chain proximal tubulopathy (LCPT) with acquired Fanconi syndrome, especially in presence of other abnormalities at the urine examination (e.g. glycosuria, phosphaturia, uricosuria). The clinical laboratory, with the determination of biomarkers in urine provides crucial data in PCD management: for the diagnostic workflow, assessment of severity of the disease and evaluation of treatment effectiveness. Background. The need to contain the spread of the SARS-CoV-2 pandemic has forced national and local organizations to define and implement targeted emergency response and management measures. As the knowledge about the infection grew, a wider range of organizational measures were deployed. Methods. This study involves the SARS-CoV-2 infected people managed by the Local Health Authority of Rieti (Italy). Diagnostic test waiting times and hospital admission rates in the Province of Rieti are investigated as the pandemic evolved. Trends were analyzed in relation to the tempora spreading of SARS-CoV-2, to the organizational actions taken by the Local Health Authority of Rieti, and to the deployment of actions across the territory. A municipalities classification of the province of Rieti was conducted after a cluster analysis based on the diagnostic test waiting times and the hospital admission rates. Results. Our findings show a declining trend, thus indicating a possible positive effect of the measures taken to contain the pandemic. The cluster analysis of the municipalities of the Province of Rieti makes evident an inhomogeneous geographical distribution of examined parameters (diagnostic test waiting times and the hospital admission rates), demonstrating the capability of Local Health Authority of Rieti to reach even the most disadvantaged areas and implying that the differences are due to the demographical variabilities. Conclusion. Despite some limitations, this study outlines the importance of management measures in response of the pandemic. These measures should adapt to social, cultural and geographical nature of the territory involved. Coronavirus disease (COVID-19) as an emerging disease decreases security among people from different countries. Sense of security can be raised via quick diagnosis of COVID-19, and its management and control using clinical decision support systems (CDSS) to prevent further spread of the disease. So, the aim of this study is to identify and introduce the applications of a CDSS in the diagnosis, management, and control of COVID-19. This cross-sectional study was conducted to identify and introduce the applications of CDSS in the diagnosis, management, and control of COVID-19. Based on the results of some meetings with infectious disease specialists and a general practitioner as well as reviewing the related literature, information about COVID-19 and CDSS was obtained. Then based on the information obtained, a questionnaire was designed electronically and distributed in a two-round Delphi method among 19 experts in the three fields of medical informatics, health information management, and infectious disease specialists. According to the literature and expert opinions, 35 applications of CDSS applications were identified in the four main groups of “diagnosis”, “medication”, “monitoring”, and “health services”. Eventually, a collective agreement was reached on 30 applications in the first and second rounds of Delphi. Among all the applications, the highest means were assigned to “monitoring the vital signs” and “helping diagnose infections and damaged lung tissue through CT scan”. Introducing these applications can provide general, basic knowledge of the design and implementation of clinical decision support systems in the real world to prevent irreversible complications and even many people’s death.

The critical issues of the most widespread categorical diagnostic models, such as the ICD and the DSM, elicit some reflections: Clinicians must interface with the way psychopathology is considered and how the psychopathology model influences the diagnostic phase and the indication/contraindication to treatment. Recent research has emphasized the importance of embedding the signs and symptoms with neurobiological functioning and a developmental perspective. Recent classification systems [i.e., the Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood - DC: 0-5TM and the Research Domain Criteria - RdoC (Insel et al., 2010)] aim to identify criteria that are useful to the clinician and that overcome the limitations of traditional models. The article outlines the main points of the most up-to-date models of psychopathology, particularly how they integrate different components of the person's functioning with the developmental and neurobiological levels and the environment. These radical changes make clinicians reflect on how to use psychodiagnostic instruments: Testing was born when different models of psychopathology prevailed. The paper provides the scientific community and the clinic practitioners dealing with neurodevelopmental disorders with a list of open questions about research and diagnosis. Such questions are elicited by the evidence coming from neuroscience investigations and from the outline of a different framework to understand disorders with onset in the developmental period. The broad heterogeneity that characterizes the diagnostic categories of neurodevelopmental disorders has favored the emergence of constructs referable to complexity, multidimensionality, and comorbidity. The reflection is carried-out according to the Research Domain Criteria (RDoC) approach. RDoC is a dimensional diagnostic system with the aim of shifting the focus of research and clinical practice away from the existing diagnostic categories towards dimensions of observable behavior and neurobiological measures. The transdiagnostic perspective of the RDoc approach is used here both as a possible criticism of the dominant research paradigms and as a challenge to rethink neurodevelopmental disorders and as a potential solution to intercept the specificities of the phenotypes. It is important to be able to measure the range of clear focus in clinical practice to advise on presbyopia correction techniques and to optimise the correction power. Both subjective and objective techniques are necessary: subjective techniques (such as patient reported outcome questionnaires and defocus curves) assess the impact of presbyopia on a patient and how the combination of residual objective accommodation and their natural DoF work for them; objective techniques (such as autorefraction, corneal topography and lens imaging) allow the clinician to understand how well a technique is working optically and whether it is the right choice or how adjustments can be made to optimise performance. Techniques to assess visual performance and adverse effects must be carefully conducted to gain a reliable end-point, considering the target size, contrast and illumination. Objective techniques are generally more reliable, can help to explain unexpected subjective results and imaging can be a powerful communication tool with patients. A clear diagnosis, excluding factors such as binocular vision issues or digital eye strain that can also cause similar symptoms, is critical for the patient to understand and adapt to presbyopia. Some corrective options are more permanent, such as implanted inlays / intraocular lenses or laser refractive surgery, so the optics can be trialled with contact lenses in advance (including differences between the eyes) to better communicate with the patient how the optics will work for them so they can make an informed choice.

- Abstract
Introduction: Neuroendocrine neoplasms (NENs) represent a complex group of tumors arising from neuroendocrine cells, characterized by heterogeneous behavior and challenging diagnostics. Despite advancements in medical technology, NENs present a major challenge in early detection, often leading to delayed diagnosis and variable outcomes. This review aims to provide an in-depth analysis of current diagnostic methods as well as the evolving and future directions of diagnostic strategies for NENs. Area covered: The review extensively covers the evolution of diagnostic tools for NENs, from traditional imaging and biochemical tests to advanced genomic profiling and next-generation sequencing. The emerging role of technologies such as artificial intelligence, machine learning, and liquid biopsies could improve diagnostic precision, as could the integration of imaging modalities such as positron emission tomography (PET)/magnetic resonance imaging (MRI) hybrids and innovative radiotracers. Expert opinion: Despite progress, there is still a significant gap in the early diagnosis of NENs. Bridging this diagnostic gap and integrating advanced technologies and precision medicine are crucial to improving patient outcomes. However, challenges such as low clinical awareness, limited possibility of noninvasive diagnostic tools and funding limitations for rare diseases like NENs are acknowledged.

- Abstract
Background When a hematological malignancy is diagnosed, the whole family carries the burden of the disease; parents often try to protect minor children from suffering by avoiding communication about their disease. Since 2009, patients with minors at the Adult Hematology Division at San Gerardo Hospital (Monza) can take part in the "Emanuela Project": children can visit parents and talk with psychologists and hematologists, who explain the disease through simple metaphors.Materials and Methods The EMY STUDY aimed to evaluate the impact of illness-related communication on children's behavior, comparing Monza's experience with other Hematology Units, where the communication is delegated to parents or psychological support. Questionnaires exploring the children's main behaviors (school performance, appetite, sleeping patterns, attachment to family figures, and family dialogue) were administered to both sick (SP) and healthy (HP) parents. From 2017 to 2021, 32 patients were enrolled, 20 from Monza and 12 from other hospitals; 84 questionnaires were globally collected.Results In Monza's group, no major changes in children's behavior were observed and an open dialogue about the disease was often possible. Disease communication is considered crucial and perceived as a responsibility of parents together with a professional figure, mainly the hematologist. Patients were satisfied with "Emanuela Project," reporting positive effects on doctor-patient relationship. Difficulties in separation were significantly higher at other hospitals (P = .019) than in Monza. While at other centers communication is considered parents' responsibility, Monza's patients emphasize the role of professional figures (P = .007). Differently from other hospitals, the role of the hematologist is crucial to Monza's patients (P = .001).Conclusion Disease communication to patients' offspring is a crucial moment in the process of care, and the hematologist can play a major role in this difficult task, with

- Abstract
Autism sibling recurrence in prospective infant family history studies is ~20% at 3 years but systematic follow-up to mid-childhood is rare. In population and clinical cohorts autism is not recognized in some children until school-age or later. One hundred and fifty-nine infants with an older sibling with autism underwent research diagnostic assessments at 3 years and mid-childhood (6 to 12 years (mean 9)). We report the autism sibling recurrence rate in mid-childhood and compare developmental and behavioral profiles at mid-childhood and 3 years in those with earlier versus later recognized autism, and those who had, or had not, received a community autism diagnosis. The autism recurrence rate in this sample in mid-childhood was 37.1%, 95% CI [29.9%, 44.9%] and higher in boys than girls. Around half of those diagnosed with autism in mid-childhood had not received a diagnosis at 3 years. Later, diagnosis was more common in girls than boys. While some had sub-threshold symptoms at 3, in others late diagnosis followed a largely typical early presentation. Sibling recurrence based on community clinical diagnosis was 24.5%, 95% CI [18.4%, 31.9%]. Those who also had a community diagnosis tended to be older, have lower adaptive function and higher autism and inattention symptoms. Notwithstanding limitations of a single site study, modest sample size and limits to generalisability, autism sibling recurrence in family history infants may be higher in mid-childhood than in studies reporting diagnostic outcome at 3 years. Findings have implications for families and clinical services, and for prospective family history studies.

- Abstract
Objectives: The high incidence of prostate cancer causes prostatic samples to significantly affect pathology laboratories workflow and turnaround times (TATs). Whole-slide imaging (WSI) and artificial intelligence (AI) have both gained approval for primary diagnosis in prostate pathology, providing physicians with novel tools for their daily routine. Methods: A systematic review according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines was carried out in electronic databases to gather the available evidence on the application of AI-based algorithms to prostate cancer. Results: Of 6290 articles, 80 were included, mostly (59%) dealing with biopsy specimens. Glass slides were digitized to WSI in most studies (89%), roughly two-thirds of which (66%) exploited convolutional neural networks for computational analysis. The algorithms achieved good to excellent results about cancer detection and grading, along with significantly reduced TATs. Furthermore, several studies showed a relevant correlation between AI-identified histologic features and prognostic predictive variables such as biochemical recurrence, extraprostatic extension, perineural invasion, and disease-free survival. Conclusions: The published evidence suggests that AI can be reliably used for prostate cancer detection and grading, assisting pathologists in the time-consuming screening of slides. Further technologic improvement would help widening AI's adoption in prostate pathology, as well as expanding its prognostic predictive potential.

Hypertension urgency and emergency represents a challenging condition in which clinicians should determine the assessment and/or treatment of these patients. Whether the elevation of blood pressure (BP) levels is temporary, in need of treatment, or reflects a chronic hypertensive state is not always easy to unravel. Unfortunately, current guidelines provide few recommendations concerning the diagnostic approach and treatment of emergency department patients presenting with severe hypertension. Target organ damage determines: the timeframe in which BP should be lowered, target BP levels as well as the drug of choice to use. It's important to distinguish hypertensive emergency from hypertensive urgency, usually a benign condition that requires more likely an outpatient visit and treatment.

Hepatic diffuse conditions and focal liver lesions represent two of the most common scenarios to face in everyday radiological clinical practice. Thanks to the advances in technology, radiology has gained a central role in the management of patients with liver disease, especially due to its high sensitivity and specificity. Since the introduction of computed tomography (CT) and magnetic resonance imaging (MRI), radiology has been considered the non-invasive reference modality to assess and characterize liver pathologies. In recent years, clinical practice has moved forward to a quantitative approach to better evaluate and manage each patient with a more fitted approach. In this setting, radiomics has gained an important role in helping radiologists and clinicians characterize hepatic pathological entities, in managing patients, and in determining prognosis. Radiomics can extract a large amount of data from radiological images, which can be associated with different liver scenarios. Thanks to its wide applications in ultrasonography (US), CT, and MRI, different studies were focused on specific aspects related to liver diseases. Even if broadly applied, radiomics has some advantages and different pitfalls. This review aims to summarize the most important and robust studies published in the field of liver radiomics, underlying their main limitations and issues, and what they can add to the current and future clinical practice and literature.

Despite the increasing number of radiological case reports, the majority lack a standardised methodology of writing and reporting. We therefore develop a reporting guideline for radiological case reports based on the CAse REport (CARE) statement. We established a multidisciplinary group of experts, comprising 40 radiologists, methodologists, journal editors and researchers, to develop a reporting guideline for radiological case reports according to the methodology recommended by the Enhancing the QUAlity and Transparency Of health Research network. The Delphi panel was requested to evaluate the significance of a list of elements for potential inclusion in a guideline for reporting mediation analyses. By reviewing the reporting guidelines and through discussion, we initially drafted 46 potential items. Following a Delphi survey and discussion, the final CARE-radiology checklist is comprised of 38 items in 16 domains. CARE-radiology is a comprehensive reporting guideline for radiological case reports developed using a rigorous methodology. We hope that compliance with CARE-radiology will help in the future to improve the completeness and quality of case reports in radiology.

Chronic pain is a complex phenomenon with a multidimensional nature that involves both biology and psychology, as well as socio-cultural influences. While its primary function is to protect us from environmental and endogenous dangers, ensuring the survival of the species, when it becomes chronic it can be threatening to us, causing important physical and mental burden. In this complex phenomenon, psychology may play two important roles: the first is to provide a key to understanding the mechanisms beyond the chronification by exploring the various domains related to cognition, behaviours and psychological functions. The second role deals with non-invasive and non-pharmacological therapeutic approaches that, also by mean of psychology, can contribute to improving patients' conditions and the socio-economic management of the burden of chronic pain. In this framework, my research focused on both of these aspects. The first chapter will introduce the context of chronic pain, the specific characteristics of the different clinical conditions characterised by it, and provide a brief description of the main pathways connecting the central nervous system with the periphery, and vice versa. Furthermore, the neural correlates typical of chronic pain conditions will be briefly presented, and the main therapeutic approaches, currently known and applied in the broad field of chronic pain, will be illustrated. The second chapter will be focused on presenting the different experimental studies constituting my research activity. In particular, the first paragraph will present four research works focused on cognitive, behavioural and psychological functioning of three different populations affected by chronic pain, namely (i) small fibre neuropathy, a specific painful peripheral neuropathy, (ii) chronic primary pain conditions, and (iii) cluster headache. Objectives: To assess the ability of the International Endometrial Tumor Analysis (IETA)-1 polynomial regression model to estimate the risk of endometrial cancer (EC) and other intracavitary uterine pathology in women without abnormal uterine bleeding. Methods: This was a retrospective study, in which we validated the IETA-1 model on the IETA-3 study cohort (n = 1745). The IETA-3 study is a prospective observational multicenter study. It includes women without vaginal bleeding who underwent a standardized transvaginal ultrasound examination in one of seven ultrasound centers between January 2011 and December 2018. The ultrasonography was performed either as part of a routine gynecological examination, during follow-up of non-endometrial pathology, in the work-up before fertility treatment or before treatment for uterine prolapse or ovarian pathology. Ultrasonographic findings were described using IETA terminology and were compared with histology, or with results of clinical and ultrasound follow-up of at least 1 year if endometrial sampling was not performed. The IETA-1 model, which was created using data from patients with abnormal uterine bleeding, predicts four histological outcomes: (1) EC or endometrial intraepithelial neoplasia (EIN); (2) endometrial polyp or intracavitary myoma; (3) proliferative or secretory endometrium, endometritis, or endometrial hyperplasia without atypia; and (4) endometrial atrophy. The predictors in the model are age, body mass index and seven ultrasound variables (visibility of the endometrium, endometrial thickness, color score, cysts in the endometrium, non-uniform echogenicity of the endometrium, presence of a bright edge, presence of a single dominant vessel). We analyzed the discriminative ability of the model (area under the receiver-operating-characteristics curve (AUC); polytomous discrimination index (PDI)) and evaluated calibration of its risk estimates (observed/expected ratio). Il presente lavoro analizza la funzione del reato nell’autismo ad alto funzionamento esaminando le molteplici aree psico-emotive coinvolte in questo processo. Si approfondiscono gli aspetti di diagnosi differenziale e comorbilità psichiatrica con alcuni disturbi maggiormente correlati all’autismo di livello 1. Si evidenziano, dunque, le conseguenti implicazioni sul piano clinico e forense. L’elaborato parte dall’esame di come il sistema giuridico italiano si interfaccia con il disturbo mentale. È messo in luce il ruolo che il sistema giuridico attribuisce alle neuroscienze nell’analisi del profilo di responsabilità di un soggetto con disturbo mentale ed, in particolare, di un soggetto con autismo di livello 1 che commette un reato. A tale proposito è esaminata la funzione dei reati più comuni. Si evidenziano le differenze dell’autismo di livello 1 con il disturbo antisociale e schizofrenico. L’attenzione è sull’esplorazione della sintomatologia clinica in termini di funzione. Attraverso un’analisi critica degli studi evidenziati in letteratura, è descritto il fattore fondamentale per la corretta individuazione della diagnosi differenziale e, di conseguenza, della comorbilità psichiatrica. Esso è rappresentato dall’esame del processo difensivo dal quale i sintomi hanno origine. Una stessa manifestazione sintomatologica può, infatti, rappresentare problematiche e patologie differenti. Il focus è dunque posto sull’importanza di una valutazione clinico forense particolarmente approfondita in cui sono esaminati i meccanismi di difesa. La finalità di questo articolo è evidenziare l’opportunità di applicare nel contesto giuridico nuovi e più completi strumenti di valutazione e diagnosi.

The school and its clients, pupils, and families are facing critical change. The presence of foreign students and cultural diversity is growing. These changes are accompanied by an exponential growth in the use of diagnoses, with the production of an almost infinite descriptive taxonomy: disabilities, learning difficulties, and cultural differences blend, all becoming deficits. Without a clear definition of the problems that the school context, as a system of productive relationships, is facing, one moves towards confusion that leads to placing the cause of every complex educational relationship on the individual pupils, with the production of an infinite number of deficit certificates. In this scenery, diagnosis becomes a tool for activating resources, with the imagination - starting from those - of being able to solve a vast range of problems which, upon closer inspection, are problems of coexistence. This dynamic makes it difficult for the school to think about its complexity, causing it lose sight of its mandate: to train to the wider social context, grasp its changes, and promote them. In this work, an intervention proposal is made which uses the reference to a psychology of the individual-context relationship, based on the hypothesis of the unconscious collusive dynamic that organizes the social and organizational relationship in the school, useful for reading the changes taking place and promoting its objectives of development. Introduction. Asthma is one of the most common chronic diseases in both pediatric age and adulthood, often underdiagnosed and undertreated. Over the past 20 years, numerous guidelines have been published with the aim of translating evidence into recommendations for good clinical practice; however, their impact was modest. The multilevel training program “Revolution in asthma” was created with the dual objective of improving the knowledge of the main guidelines on asthma and investigating the level of agreement of the participants with the recommendations of the guidelines, as well as their real-life application. Materials and methods. The survey was conducted from February to October 2021 on a representative sample of 400 Italian general practitioners, pulmonologists and algologists. Four guidelines (BTS/SIGN, NICE, AEPP-NHLBI-EPR and GINA) were compared and the concordances/ discrepancies between the recommendations of the guidelines relating to seven thematic areas were analyzed. Agreement/disagreement with the recommendations was measured with a Likert scale. The level of adherence in clinical practice was also assessed through questions aimed at estimating participants’ “opinions/beliefs” (as indicator of self-efficacy, i.e., confidence that the recommendations can be applied in daily practice). Results and conclusions. This e-learning project increased the participants’ guidelines knowledge (at the end of the course, 80% of the participants thought it was more useful to use several guidelines than one). The analysis highlighted the areas of agreement with the guidelines, and the “grey areas”, i.e. the areas with moderate agreement, such as patients monitoring and PEF use. Besides, barriers that interfere with the application of the recommendations into clinical practice were identified. In conclusion, the project allowed the participants to re-elaborate their own “mental guidelines” and to compare them with the published ones, thus driving a paradigm shift in everyday clinical practice. In early 2021, the ISarcoPRM – the special interest group on sarcopenia of the International Society of Physical and Rehabilitation Medicine (ISPRM) – proposed a new diagnostic algorithm that enables effective and early diagnosis of sarcopenia. The diagnosis is made through four tests, all of which can be administered in both inpatient and outpatient settings. A validated diagnostic algorithm for sarcopenia contributes to the standardization of clinical practices and promotes better communication among health professionals. This allows to obtain consistent and comparable results in different healthcare facilities, facilitating clinic research and the identification of the best therapeutic approaches to counteract this condition. Coronavirus disease (COVID-19) as an emerging disease decreases security among people from different countries. Sense of security can be raised via quick diagnosis of COVID-19, and its management and control using clinical decision support systems (CDSS) to prevent further spread of the disease. So, the aim of this study is to identify and introduce the applications of a CDSS in the diagnosis, management, and control of COVID-19. This cross-sectional study was conducted to identify and introduce the applications of CDSS in the diagnosis, management, and control of COVID-19. Based on the results of some meetings with infectious disease specialists and a general practitioner as well as reviewing the related literature, information about COVID-19 and CDSS was obtained. Then based on the information obtained, a questionnaire was designed electronically and distributed in a two-round Delphi method among 19 experts in the three fields of medical informatics, health information management, and infectious disease specialists. According to the literature and expert opinions, 35 applications of CDSS applications were identified in the four main groups of "diagnosis", "medication", "monitoring", and "health services". Eventually, a collective agreement was reached on 30 applications in the first and second rounds of Delphi. Among all the applications, the highest means were assigned to "monitoring the vital signs" and "helping diagnose infections and damaged lung tissue through CT scan". Introducing these applications can provide general, basic knowledge of the design and implementation of clinical decision support systems in the real world to prevent irreversible complications and even many people's death.
