6. Síndrome de vasoconstricción cerebral reversible.

- Abstract
BACKGROUND: Reversible cerebral vasoconstriction syndrome is a disease whose incidence is unknown, in which segmental and reversible vasoconstriction of the cerebral arteries occurs, and is characterized by severe thunderclap headache, which may or may not be accompanied by neurological deficit. CLINICAL CASE: A 52-year-old female patient, with a history of 2 pregnancies and 2 deliveries and recent onset of climacteric, with dysregulation of the menstrual cycle and obesity. She presented a headache of 4 days of evolution, of severe intensity, palpitations, photophobia, sweating, nausea and vomiting. The neurological examination did not reveal any neurological deficit. Brain CT showed right fronto-parietal subarachnoid hemorrhage. The arteriography revealed multiple segmental stenosis in all vascular territories. During her admission, nimodipine was administered, and controls were performed with transcranial echo-Doppler, showing favorable clinical evolution. CONCLUSIONS: Reversible cerebral vasoconstriction syndrome constitutes a diagnostic challenge, given the different forms of presentation and the different causes and factors that can trigger it. The mechanisms involved in its production are partly unknown; however, within the reported associations it has been suggested that sex hormones play an important role in its pathogenesis, but more scientific evidence is needed to clarify this association. - Abstract
Arcuate ligament syndrome (ALS) is caused by extrinsic compression of the celiac trunk by the median arcuate ligament. It has an estimated incidence of 2 per 100,000 people, preferentially affecting female individuals in the age range of 30 and 50 years. The clinical feature of SLA can manifest with chronic postprandial abdominal pain, nausea and/or vomiting, weight loss and abdominal murmur, although there are patients who do not develop symptoms due to increased collateral circulation. The diagnosis of SLA is based on the presence of clinical symptoms, imaging findings, and the exclusion of other causes of abdominal pain. Within imaging techniques, magnetic resonance angiography (MRA) and computed tomography angiography (AngioCT) are the modalities of choice for the diagnosis of SLA. The management of SLA is surgical with the purpose of restoring the flow in the celiac trunk and whose approaches are described in this article. Finally, this pathology is a challenge for the specialist since the symptoms can be non-specific and overlap with other abdominal disorders, making diagnosis and management difficult. Recent advancements in medical technology have propelled the field of arterial and cerebral blood pressure management into a new era of precision and foresight. This discourse delves into contemporary methodologies and technologies utilized in the diagnosis and prognosis of arterial and cerebral blood pressure conditions, alongside their potential implications for future health prognoses. Notably, the discourse underscores the merits of cutting-edge technologies, including machine learning, artificial intelligence, genomic sequencing, and nanotechnology, in enhancing diagnostic precision, early detection of pathological markers, and tailoring personalized treatment regimens. Moreover, ethical considerations and confidentiality concerns pertinent to the adoption of these technologies are carefully scrutinized. To this end, an exhaustive review of contemporary literature pertaining to medical diagnostic technologies, encompassing machine learning, artificial intelligence, genomic sequencing, and nanotechnology, was conducted. The collaborative efforts of interdisciplinary teams comprising medical professionals, engineers, computer scientists, and ethicists are advocated as pivotal for realizing the full potential of modern diagnostic technologies in clinical settings. Los avances recientes en la tecnología médica han impulsado el campo del manejo de la presión arterial y cerebral hacia una nueva era de precisión y previsión. Este discurso profundiza en las metodologías y tecnologías contemporâneas utilizadas en el diagnóstico y pronóstico de las condiciones de presión arterial y cerebral, junto con sus posibles implicaciones para los pronósticos de salud futuros. En particular, el discurso subraya los méritos de las tecnologías de vanguardia, incluido el aprendizaje automático, la inteligencia artificial, la secuenciación genómica y la nanotecnologia, para mejorar la precisión del diagnóstico, la detección temprana de marcadores patológicos y la adaptación de regímenes de tratamien- to personalizados. Además, se examinan cuidadosamente las consideraciones éticas y las cuestiones de confidencialidad pertinentes a la adopción de estas tecnologías. Para ello, se realizó una revisión exhaustiva de la literatura contemporánea relacionada con las tecnologias de diagnóstico médico, que abarca el aprendizaje automático, la inteligencia artificial, la secuenciación genómica y la nanotecnologia. This report described the use of acoustic radiation force impulse (ARFI) elastography in the diagnosis of two cases of canine malignant peripheral nerve sheath tumors. Both patients presented with lameness in the thoracic limb and no response to previous clinical treatment or trauma history. Physical examination revealed muscular atrophy in one of the thoracic limbs, in both dogs. B-mode axillary ultrasound showed the presence of a predominantly hypoechogenic, heterogeneous, irregular, nonencapsulated nodule with adjacent reactivity, measuring approximately 20.1 mm x 15.4 mm in the first case and 18.3 mm x 10.7 mm in the second. ARFI elastography was performed, showing areas of significant stiffness, with an average shear wave velocity of 4.12 m/s (case 1) and 4.35 m/s (case 2), suggesting malignancy. In both cases, the affected limb was amputated, and the tumors sent for histopathological analysis, which were; subsequently, diagnosed as malignant tumors of the peripheral nerve sheath. The management of urinary tract infection (UTI) in infants and children has changed significantly over the past few decades based on scientific evidence that questioned the efficacy of strategies used to prevent kidney injury and subsequent progression to chronic kidney disease, which is very unlikely in most paediatric cases. However, there is still substantial heterogeneity in its management and uncertainty regarding the diagnosis, indication of imaging tests, treatment or follow-up in these patients. The Spanish clinical practice guideline has been updated through the review of the literature published since 2009 and a rigorous evaluation of current clinical practice aspects, taking into account the evidence on the benefits of each intervention in addition to its risks and drawbacks to attempt to provide more precise recommendations. Resumen: En las últimas décadas, se han producido profundos cambios en el manejo de la infección urinaria en pediatría, a partir de estudios que cuestionaban la eficacia de las estrategias para prevenir el daño renal y la evolución posterior a enfermedad renal crónica, muy poco probable en la mayoría de casos. No obstante, todavía existe una gran variabilidad clínica en el manejo e incertidumbre en cuestiones relativas al diagnóstico, la indicación de una exploración radiológica, el tratamiento o el seguimiento de estos niños. La actualización de la guía de práctica clínica española ha examinado en la literatura publicada desde el año 2009 y de modo riguroso aspectos de la práctica actual, considerando la evidencia de los beneficios de cada intervención, junto con los riesgos asociados y desventajas, para intentar delimitar las pautas de actuación más precisas.

19. Análisis de perfiles neuropsicológicos en niños con Déficit de Atención e Hiperactividad y Tempo Cognitivo Lento

El actual estudio nace de la inquietud que ha surgido en los últimos años sobre el Trastorno de Déficit de Atención e Hiperactividad (TDAH) y el Tempo Cognitivo Lento (TCL). En especial sobre cuan independientes son estos trastornos entre sí, con diferentes causas, síntomas y características. Para ello hemos analizado las diferencias en perfiles cognitivos de 105 participantes (74 niños y 31 niñas) de entre 6 y 13 años (M = 8,97; DT = 1,97) separados en dos grupos según su diagnóstico: 57 sujetos diagnosticados con TDAH y 48 sujetos diagnosticados con TCL. Para analizar el perfil neuropsicológico de los participantes se empleó la Escala de Inteligencia de Weschler para niños (WISCIV) el cual consta de cuatro áreas: la Comprensión Verbal, el Razonamiento Perceptivo, la Memoria de Trabajo y la Velocidad de Procesamiento. Se empleó un diseño transversal y los datos fueron analizados en SPSS a través de la prueba de contraste U-Mann Whitney. Tras el análisis de datos se pusieron de manifiesto diferencias significativas entre ambos perfiles, mostrando al grupo con TCL con puntuaciones más bajas en memoria de trabajo, pero mejores en pruebas de velocidad de procesamiento. Aún con sus similitudes, parece tratarse de trastornos diferentes, lo cual se traduce en el ámbito nosológico a la necesidad de un estudio con unas herramientas diagnósticas propias y de un tratamiento diferente para cada uno de los trastornos. Se requiere de otros estudios con mayor muestra y diferentes instrumentos para profundizar en las similitudes y diferencias entre el TDAH y el TCL.

Neste artigo, apresenta-se e fundamenta-se uma experiência formativa para o diagnóstico do Pensamento Profissional Docente (PPD) sob a concepção do pensamento como atividade cognoscitiva da personalidade do professor e o método de seu estudo, de P. Ya. Galperin. Discute-se o significado do construto pensamento profissional docente, objeto do diagnóstico, na base de sua essência, sua estrutura e suas funções. O texto também trata da metodologia da experiência formativa, elaborada e testada experimentalmente, que permite o referido diagnóstico sob determinadas condições que demarcam possibilidades e limitações da experiência formativa. Os resultados apontam para uma contribuição teórico-metodológica aos estudos sobre o PPD como parte da formação e do desenvolvimento profissional de professores.

O Neuroblastoma é um tumor das células primitivas do sistema nervoso simpático com idade média de diagnóstico de 17 meses, representando 10% das neoplasias da infância e sendo responsável por 15% das mortes por cancro na infância. Os sintomas clínicos são variáveis conforme a localização, e tem também uma gravidade e prognóstico muito variáveis. Descrevemos um caso de uma criança de dois anos que recorre ao serviço de urgência (SU) com queixas iniciais de gemido intermitente e desconforto ocasional, acabando por lhe ser diagnosticado um neuroblastoma cervical.

A discinesia ciliar primária é uma síndrome autossómica recessiva, de clínica muito variável. Se associada a situs inversus, sinusite crónica e bronquiectasias, denomina-se Síndrome de Kartagener. A sua clínica pode ser muito variável e inespecífica, incluindo dificuldade respiratória neonatal. Descrevemos um caso de Síndrome de Kartagener num bebé de um ano, que se manifestou aos seis dias com dificuldade respiratória, de agravamento progressivo. Este caso representa o paradigma da importância da atuação do Médico de Família, na valorização dos sinais e sintomas, que permitiu o diagnóstico precoce de uma síndrome pouco prevalente.

Introduction: Neurological complications are the most frequent type of extracardiac complications of infective endocarditis (IE), and can be the initial manifestation. The objectives of this study were to determine the prevalence of neurological complications in patients with IE and to evaluate whether initial presentation with neurological symptoms causes a diagnostic delay. Material and methods: We conducted a retrospective observational study of patients with IE admitted to a tertiary hospital between 2003 and 2020. Results: The study included 222 patients with IE (67% men; mean [SD] age, 66.4 [14.2] years). Neurological complications occurred in 21.2% of patients, with ischaemic stroke (74.5%) and intracerebral haemorrhage (23.4%) being the most frequent. No differences in diagnostic delay were found between the group of patients in whom the disease manifested with neurological complications and the rest of the patients (4.4 vs 4.5; P = .76). Conclusions: A total of 21.2% of patients with IE presented neurological complications, with ischaemic stroke being the most frequent. Neurological symptoms as the initial manifestation of IE did not lead to a delay in diagnosis. Resumen: Introducción: Las manifestaciones neurológicas son las complicaciones extracardíacas más frecuentes de la endocarditis infecciosa (EI) y pueden ser el síntoma inicial. Los objetivos del estudio fueron determinar la prevalencia de complicaciones neurológicas en los pacientes con EI y evaluar si la clínica neurológica como debut condiciona un retraso diagnóstico. Material y métodos: Estudio de cohortes retrospectivo de pacientes con EI ingresados en un hospital de tercer nivel en el periodo entre 2003 y 2020. Resultados: Se han revisado 222 pacientes con EI (67% varones, edad 66,4 ± 14,2 años). El 21,2% presentaron complicaciones neurológicas durante el ingreso, siendo el ictus isquémico (74,5%) y la hemorragia cerebral (23,4%), las más frecuentes. No se encontraron diferencias entre el tiempo medio desde el ingreso hasta el diagnóstico en el grupo de pacientes que debutó con focalidad, en comparación con el resto de pacientes (4,4 vs. 4,5; p = 0,76). Conclusiones: Un 21,2% de los pacientes con EI presentaron complicaciones neurológicas durante el ingreso, siendo el ictus isquémico la más frecuente. La clínica neurológica como debut de la EI no conllevó un retraso diagnóstico.

Narcolepsy is a primary disorder of the central nervous system resulting from genetic, environmental, and immunological interactions defined as excessive daytime sleepiness plus cataplexy, hallucinations, sleep paralysis, and sleep fragmentation. The pathophysiology is not entirely known, but the interaction among genetic predisposition, environmental exposition, and immune component with consequent hypocretin-1 deficiency is the model to explain narcolepsy type I. The mechanism of narcolepsy type II is less understood. There is a delay of over ten years for the diagnosis of narcolepsy around the world. Patients with narcolepsy have many comorbidities with a negative impact on quality of life. The treatment of narcolepsy must contain an educational approach for the family, coworkers, and patients. Scheduled naps and sleep hygiene are essential to minimize the dose of medications. Much progress has been seen in the pharmacological treatment of narcolepsy with new stimulants, different presentations of oxybate, and recent studies with orexin agonists. Narcolepsy is a rare disease that needs to be more understood and highlighted to avoid delayed diagnosis and severe disabilities in patients.

In a competitive business environment, understanding and improving organizational performance is essential for long-term success. This qualitative study focuses on a prefabricated construction company in Celaya, Guanajuato, Mexico, with the goal of conducting a detailed organizational diagnosis and proposing a strategic intervention to optimize its performance. The research is based on a theoretical framework that encompasses financial, operational, and managerial evaluations, highlighting the importance of a comprehensive view for continuous improvement. Inspired by Lewin's theory and the action research approach, the intervention is conceived as a planned process carried out by external experts. This process aims not only to address current issues but also to strengthen the company’s ability to face future challenges. The methodology employs a qualitative approach, utilizing the Socioeconomic Management Method (SAGE) and action research as primary strategies. Semi-structured interviews and detailed organizational analyses will be conducted to gain a deep understanding of the internal and external dynamics affecting the company. Data triangulation from various sources will help identify significant patterns and relationships. The diagnosis at Structainer identified key issues: noise in engineering, excessive physical strain, barriers in communication and coordination, and planning difficulties due to external factors like steel price volatility. The main issue is ineffective communication, which negatively impacts productivity. To improve, it is crucial to implement integrated strategies that address these problems by enhancing communication, managing materials, reducing noise, handling personnel turnover, and balancing the workload. Dual training is an educational model that combines both classroom learning and practical training in the workplace. The objective of this research is to analyze the impact of dual training on the development of work skills in students of Third Year of Baccalaureate, professional accounting figure at the Emigdio Esparza Moreno Educational Unit in the city of Babahoyo, province of Los Ríos, which is It was carried out using a non-experimental, descriptive and exploratory design and with a mixed approach, which is based on inductive-deductive methods. To obtain data, the technique of surveys and documentary analysis was used. For the diagnosis of the development of work skills, 75 students from three different courses participated. It was observed that the students have developed work competencies and technical and interpersonal skills, which allows them to function in the workplace and face the challenges of today's world. In addition, it is noted that dual education is a promising strategy to develop work skills that facilitate a transition between school and work, promotes a close link between educational institutions and companies, this union guarantees that graduates have the skills required in companies, optimizing their employability, economic and social development. Tooth decay is a widespread disease that damages the tooth and causes pain. Its development is influenced by a complex interplay of factors, including the consumption of cariogenic foods, inadequate oral hygiene practices, and the presence of specific microorganisms. These factors are particularly pronounced in populations with lower socioeconomic status and cultural differences. However, despite various strategics aimed at reducing its prevalence, including those influenced by socioeconomic factors, the effectiveness of these efforts is limited. This results in a decrease in life quality for patients. Therefore, the objective of this research was to explore the effectiveness on diagnosis and early intervention in controlling tooth decay. Through a narrative review of PubMed publications between 2014 and 2023. Using keywords such as diagnosis, intervention, caries, and strategics, we selected twenty articles that met the criteria. The literature analyzed provides evidence of the effectiveness of early intervention in managing the disease, emphasizing the importance of a multidisciplinary approach involving all healthcare personnel, even before the eruption of the tooth. We conclude that clinical visual and tactile diagnosis, combined with early disease management, constitute one of the most accurate strategies for controlling dental decay. [ABSTRACT FROM AUTHOR]

39. Complicaciones en aplicación estética de ácido hialurónico en el territorio facial: revisión de alcance.

Introduction: Dentists have increasingly focused on aesthetic treatments using HA, leading to a rise in associated complications and a greater need to understand its effects and management. Objective: To describe the different complications associated with the use of HA in non-surgical aesthetic procedures, as well as their management and prevention. Materials and Methods: We conducted an exploratory systematic review of existing literature on complications related to HA use in the facial area and their treatments during August 2023. PubMed, Ebsco, and Scopus were utilized for this purpose. Studies reporting complications and HA management were included, while in vitro, ex vivo, and animal studies were excluded. Results: A total of 31 articles were obtained. After screening based on titles and abstracts, 14 met the criteria for inclusion. For filler migration and lesions resembling xanthelasma, small volumes of HA are recommended. In cases of facial paralysis, hialuronidase combined with systemic or topical corticosteroids is suggested. Treatment options for late infection include laser surgery, hyperbaric oxygen therapy, drainage, broad-spectrum antibiotics, and hialuronidase, which is also used for immediate vascular complications. However, specific treatment protocols for ecchymosis, the Tyndall effect, contour irregularities, diplopia, nodules, and certain ocular complications are not well-documented in the literature. Conclusions: The most prevalent complications reported include hematoma, edema, xanthelasma-like lesions, nodules, filler migration, and erythema. Among the more complex complications, contour irregularities, ocular complications, infections, immune reactions, persistent inflammation, and vascular involvement are described. In veterinary medicine, particularly in diagnostics, drug administration, and reproductive biology, nanotechnology has made significant contributions. By enabling the precise delivery of unstable or insoluble substances, reducing systemic toxicity, and prolonging the action of the active ingredient at the desired site, nanoparticles enhance the effectiveness of treatments. This translates to fewer doses, reduced costs, and less waste from animal-derived products. Furthermore, nanotechnology aids in diagnostics by detecting pathogens and mapping molecular profiles related to diseases. The sensitivity of tests, increased by nanoparticles, allows for more effective detection of contaminants and microbial agents in animal-derived products, which is crucial for food safety. Nanoparticles and nanotubes are also used in reproductive biology to improve fertility, extend sperm longevity, and facilitate semen purification for artificial insemination. Additionally, implanted nanotubes can monitor hormone levels, aiding in the identification of estrus and the selection of breeders. Despite these advancements, it is crucial to monitor the risks and environmental impacts of nanotechnological technologies to ensure their safety and effectiveness in veterinary medicine. Background: In many countries, midwives and nurses play a vital role in supporting effective perinatal mental health (PMH) care, including peripartum depression (PPD) management. In our study, we aimed to evaluate the effectiveness of PMH training in increasing certified midwives (CMs) and certified nurse–midwives (CNMs)’ knowledge about PPD management in primary care settings. The training was a part of postpartum depression (PPD) prevention programme implemented by Polish Ministry of Health. Additionally, we investigated the link between the CMs and CNMs perception of systemic difficulties, attitudes toward work with patients with mental health problems, and the adherence to PPD screening procedures. Method: A total of 379 CMs and CNMs from primary health centres and hospitals who participated in a PPD management training completed the Test of Antenatal and Postpartum Depression Knowledge before and after PMH training and the REASON scale after the training. Results: Before the training, only 17% (n = 66) of CMs and CNMs considered themselves to be educated for PPD management. The participants’ knowledge about PPD significantly increased after the training, by three points on average. The effect size was very strong, d = 0.84. However, the prevalence of perinatal mental disorders and the suicide rate among persons with PPD remained misestimated. We also found that age had a substantial influence in controlling the relationship between perceived educational preparation for PPD management, job tenure, and actual knowledge about PPD management. Self-reported higher levels of perceived systemic problems in the workplace and having more negative attitudes toward working with persons experiencing emotional difficulties were associated with declared lower frequency of measuring the severity of PPD, using screening tools, counselling, and making referrals to specialists. Discussion: Even though the training turned out to be effective, a tendency to underestimate the negative consequences of PPD was observed. The participants’ perception of systemic problems and personal attitudes were associated with an adherence to PPD screening procedures. The implementation of an obligatory PPD screening may not be enough to change the everyday practice of CMs and CNMs. Further training, taking into account personal attitudes of health care professionals, and systemic changes are required to ensure an optimal management for persons with PPD.
