Background and Objectives: Dentigerous cysts are one of the most frequent pathologies associated with unerupted or impacted teeth. Such cysts show a male predilection and a preference for the mandibular region. Also, they commonly occur in the second and third decades of life, with only 9% occurring in the first decade. The aim of this work is to apply and study the therapeutic algorithms developed for dentigerous cysts and their outcomes, from the early diagnostic stage to the complete healing phase of pediatric patients diagnosed with this medical condition. Materials and Methods: The study included 19 pediatric patients diagnosed with dentigerous cysts who underwent the enucleation and extraction or conservative attitude of the associated tooth. The bony healing was also followed-up 9 months after the surgery. Results: A higher incidence in the posterior area of the mandible and maxilla was observed, as well as a higher incidence in boys. The 9 months postoperative radiographic assessment showed that the bony defects were completely healed. Conclusions: A thorough understanding of the nature of the lesion backed by a good clinical history and by state-of-the-art radiographic and radiologic examinations can go a long way in helping the surgeon to choose the correct therapeutic approach and to ameliorate the medical condition in the best long-term interest of the young patient. Objective: This systematic review investigates the diagnostic, prognostic, and therapeutic implications of immunohistochemical markers in dentigerous cysts (DCs) and odontogenic keratocysts (OKCs) associated with impacted third molars. Materials and Methods: A comprehensive search strategy was employed across major databases including MEDLINE/PubMed, EMBASE, and Web of Science, from the inception of the databases to March 2024. Keywords and Medical Subject Heading (MeSH) terms such as "dentigerous cysts", "odontogenic keratocysts", "immunohistochemistry", "Ki-67", and "p53" were used. The PRISMA 2020 guidelines were followed to ensure methodological rigor. Inclusion criteria encompassed studies on humans and animals providing definitive diagnoses or specific signs and symptoms related to DCs and OKCs, with results on protein expression derived from immunohistochemistry, immune antibody, proteomics, or protein expression methods. Results: Of the 159 studies initially identified, 138 met the inclusion criteria. Our analysis highlighted significantly higher expressions of Ki-67 (22.1% ± 4.7 vs. 10.5% ± 3.2, p < 0.001), p53 (15.3% ± 3.6 vs. 5.2% ± 1.9, p < 0.001), and Bcl-2 (18.4% ± 3.2 vs. 8.7% ± 2.4, p < 0.001) in OKCs compared to DCs, indicating a higher proliferative index, increased cellular stress, and enhanced anti-apoptotic mechanisms in OKCs. Additionally, PCNA levels were higher in OKCs (25.6% ± 4.5 vs. 12.3% ± 3.1, p < 0.001). Genetic mutations, particularly in the PTCH1 gene, were frequently observed in OKCs, underscoring their aggressive behavior and potential malignancy. Conclusions: The findings emphasize the significant role of immunohistochemical markers in distinguishing between DCs and OKCs, with elevated levels of Ki-67, p53, Bcl-2, and PCNA in OKCs suggesting a higher potential for growth and recurrence. Genetic insights, including PTCH1 mutations, further support the need for personalized treatment approaches. Objectives: Odontogenic lesions evolve as a result of altered dental development. This study aimed to evaluate the prevalence and the coinfection of Epstein-Barr virus (EBV) and Kaposi sarcoma–associated herpesvirus (KSHV) in radicular cysts, dentigerous cysts, odontogenic keratocysts, and ameloblastomas. Methods: Polymerase chain reaction (PCR) was used to analyse 66 cases of odontogenic lesions for the presence of EBV-DNA and KSHV-DNA. These lesions were 15 radicular cysts, 16 dentigerous cysts, 18 odontogenic keratocysts, and 17 ameloblastomas. Results: EBV-DNA was detected in 24 (36.4%) of the studied samples as follows: 6 samples (40.0%) of radicular cysts, 4 (25.0%) of dentigerous cysts, 10 (55.6 %) of odontogenic keratocysts, and 4 (23.5%) of ameloblastomas (P = .168). KSHV-DNA was found in 16 (24.2%) of the studied samples as follows: 1 sample (6.7%) of radicular cysts, 6 (37.5%) of dentigerous cysts, 8 (44.4 %) of odontogenic keratocysts, and 1 (5.9%) of ameloblastomas (P = .001). Additionally, EBV and KSHV were positively correlated in all studied samples (P = .002). Conclusions: Both EBV and KSHV are found in odontogenic cysts and ameloblastomas. KSHV and EBV are more prevalent in odontogenic keratocysts than in other studied odontogenic lesions. Further, there is a high prevalence of EBV and KSHV coinfection in odontogenic cysts and ameloblastomas.

Background: Dentigerous cysts are commonly seen in association with third molars and maxillary canines. Only 5–6% of dentigerous cysts are associated with supernumerary teeth. We report a rare case of dentigerous cyst associated with an impacted anterior maxillary supernumerary tooth (mesiodens). Case Presentation: A 30-year-old male reported to our Department of Oral Medicine, School of Dentistry at Hamedan University of Medical Sciences, with chief complaint of a painless swelling in the anterior upper jaw (in the region of incisors) for a duration of 3 month. At the time of his presentation, his medical history was unremarkable, with no systemic problems and no report of pain. Conclusion: Although the association of dentigerous cyst with an impacted supernumerary tooth (mesiodens) is rare, prevention of harmful complications as developmental cyst, early diagnosis and treatment is necessary. Dentigerous cysts (DC) in the maxillary sinus are rare and pose challenges for effective treatment. Despite various available surgical techniques, a definitive approach remains debated. This study introduces a markerless Augmented Reality Assisted Surgery (ARAS) system that utilizes tooth image recognition and surgical simulation to enhance the precision of maxillary sinus DC extractions. Using advanced technology, such as 3-dimensional (3D) intraoral scanning and CT imaging for accurate data capture, the system aligns virtual models with patient anatomy without external markers, demonstrating a minimally invasive surgical solution. The ARAS system enabled precise surgical planning and realization of a DC extraction in the maxillary sinus by creating a bone window in direct contact with the cyst, assisting in complete removal with minimal risk to adjacent structures. OBJECTIVES: The current study was conducted to analyze immunohistochemical appearance of P53 protein in odontogenic cysts. METHODOLOGY: Thirty paraffin blocks of confirmed case were prepared to investigate the immunohistochemical appearance of P53 protein. RESULTS: Sixteen out of thirty odontogenic cysts (53.3%) showed P53, four out of ten dentigerous cyst (40%) had P53, twelve out of fifteen odontogenic keratocysts (80%) expressed P53 while none of the five radicular cysts (0%) showed P53 protein. CONCLUSION: Reclassification of OKC as keratocystic odontogenic tumor was supported by the present study and its findings.

A dentigerous /follicular cyst is an odontogenic cyst associated with the crown of a non-erupted or partially erupted tooth. The exact histogenesis of this cyst remains unknown. Multiple signaling pathways and cellular modification are involved in its formation. The aim of this article is to report a case of a 9-year-old boy with a de novo EDA mutation, having an extreme oligodontia accompanied by the formation of a follicular cyst. Dentigerous cysts are defined as developmental odontogenic cysts, which surround the crown of an impacted tooth. It is caused by fluid accumulation between the reduced enamel epithelium and the enamel surface, resulting in a cyst with the tooth crown located within the lumen. This paper reviews the pathogenesis, clinical features and the behaviour of dentigerous cyst. It also presents a case of a slow growing, painless swelling in the maxillary incisal region of 3 years duration. Detailed clinical, radiological and histopathological investigations aided the definitive diagnosis of dentigerous cyst. The cystic mass was enucleated with the impacted tooth in situ under local anesthesia. The operative site healed uneventfully and a removable partial denture was fabricated to improve aesthetics, speech and masticatory function.

Objective: Midkine is a heparin-binding growth factor whose expression is increased in most tumors, namely ameloblastomas. This study aimed to compare Midkine expression in different odontogenic lesions. Material and Method: This analytical cross-sectional study was performed on 52 definitely diagnosed odontogenic lesions including 15 dentigerous cysts, 13 odontogenic keratocysts, and 17 unicystic and 5 multicystic ameloblastomas archived from 1997 to 2015. Midkine expression was examined in tissue samples through immunohistochemistry. The nonparametric Kruskal-Wallis and Mann-Whitney tests were run as appropriate (P < 0.05). Results: The frequency of Midkine expression was < 20% in 7.7%, 20-50% in 25%, and > 50 % in 67.3% of the samples, indicating significant differences among the groups (P = 0.002). Moreover, the expression intensity was strong in 63.5%, moderate in 23.1%, and weak in 13.5% of odontogenic lesion samples (P = 0.071). The total staining score was weak in 3.8%, moderate in 48.1%, and strong in 48.1% of the cells, displaying significant differences between the study groups in this regard (P = 0.043). Conclusion: Midkine can be considered as both a differentiating factor and a molecular-targeted therapy in odontogenic lesions. Yet, further studies are required to approve the role of this cytokine in different biological and pathological stages of the tumors.

Background: The incidence and frequency of odontogenic cysts and tumors depends on the different geographic location. Odontogenic cysts and tumors are lesions that arise from the tooth apparatus or its remnants. The purpose of this study was to determine the prevalence of odontogenic cysts and tumors over a period of 5 years 9 months and to compare our results with other published studies. Aim and Objective: The purpose of this study was to evaluate the prevalence of odontogenic cysts and tumors diagnosed at the Department of Oral Pathology, M.M. College of Dental Sciences & Research, Mullana (Ambala) and to compare the results with findings in the literature. Materials and Method: Data of odontogenic cysts and tumors diagnosed during 2014 to 2019 (till September) w4ere collected from the files of the Oral Pathology Department, M.M. College of Dental Sciences & Research, Mullana (Ambala). Results: Out of the total 71 cases of odontogenic cysts, most prevalent odontogenic cysts were found to be radicular cysts (n=42, %= 59.1%), followed by dentigerous cysts (n=17, %=23.9%) and odontogenic keratocyst (n=12, %=16.9%). Among the odontogenic tumors out of 49 cases reported, the most prevalent was ameloblastoma (n=28, %=57.1%) followed by, ameloblastic fibroma (n=06, %=12.2%), odontoma (n=05,%=10.2%), adenomatoid odontogenic tumor (n=03, %=6.1%), peripheral odontogenic fibroma (n=02, %=4.08%), ameloblastic carcinoma (n=02, %=4.08%), odontogenic fibroma (n=02, %=4.08%) and odontogenic myxofibroma (n=01, %=2.04%). The anterior and posterior portion of the body of mandible was the most common site for both odontogenic cysts and tumors. The age group for odontogenic cysts ranged from 07 to 72 years of age, whereas for the odontogenic cysts the range was 10 to 75 years of age. The male to female ratio for the odontogenic cysts was 1:1.2, whereas for the odontogenic tumors it was found to be having more female predilection with the ratio of 1:1.3. Conclusion: The prevalence of odontogenic cysts was similar to that reported in the literature, with incidence of radicular cysts seen most frequently. Objective: To investigate the expression of CD10 in the epithelium of Odontogenic keratocyst (OKC), Dentigerous and radicular cyst. Study Design: Cross sectional analytical study. Place and Duration of Study: Armed Forces Institute of Pathology, Rawalpindi, from Jan 2017 to Dec 2017. Methodology: In this study, total of sixty cases were included with 20 of each Radicular cyst, Dentigerous cyst and Odontogenic keratocyst. Sections were stained with Haematoxylin and eosin (H&E) followed by Immunohistochemistry (IHC) staining for CD10 antibody. SPSS version 20 was used to analyse the results. Expression of CD10 was evaluated. Results: Out of total 60 cases, 41 (68.3%) were male and 19 (31.7%) were female patients with age ranges from 11 to 75 years. Mandible was reported in 38 (63.3%) of the cases followed by maxilla in 22 (36.7%) of the cases. In this study 7 (38%) cases of Odontogenic keratocysts showed negative epithelial CD10 expression as compared to Dentigerous cysts and Radicular cysts (45% & 25%). In Odontogenic keratocysts 9 (45%) of the cases showed basal layer of positivity of epithelium. While in dentigerous cyst 11 (55%) of the cases showed CD10 expression in the superficial layer of epithelium and the entire epithelium was positive in almost all 15 (75%) cases of radicular cyst. Intensity of staining was moderate in maximum number of the cases of Odontogenic keratocyst and radicular cyst (40% & 35%) as compared to dentigerous cyst (20%). There was a statistically significant association between odontogenic cysts and epithelial CD10 expression (p=0.001). Conclusion: Majority of odontogenic cysts showed positivity for CD10 marker. Background: The goal of the study was to create a histopathology image classification automation system that could identify odontogenic keratocysts in hematoxylin and eosin-stained jaw cyst sections. Methods: From 54 odontogenic keratocysts, 23 dentigerous cysts, and 20 radicular cysts, about 2657 microscopic pictures with 400× magnification were obtained. The images were annotated by a pathologist and categorized into epithelium, cystic lumen, and stroma of keratocysts and non-keratocysts. Preprocessing was performed in two steps; the first is data augmentation, as the Deep Learning techniques (DLT) improve their performance with increased data size. Secondly, the epithelial region was selected as the region of interest. Results: Four experiments were conducted using the DLT. In the first, a pre-trained VGG16 was employed to classify after-image augmentation. In the second, DenseNet-169 was implemented for image classification on the augmented images. In the third, DenseNet-169 was trained on the two-step preprocessed images. In the last experiment, two and three results were averaged to obtain an accuracy of 93% on OKC and non-OKC images. Conclusions: The proposed algorithm may fit into the automation system of OKC and non-OKC diagnosis. Utmost care was taken in the manual process of image acquisition (minimum 28–30 images/slide at 40× magnification covering the entire stretch of epithelium and stromal component). Further, there is scope to improve the accuracy rate and make it human bias free by using a whole slide imaging scanner for image acquisition from slides.

Objective To evaluate the effect of decompression treated large cystic lesions of the jawsand analyze the influencing factors. Methods With the panoramic radiographs, the cysts size were measured before and after de⁃ compression in 6 dentigerous cysts (DC), 9 keratocystic odontogenic tumors (KTOC) and 10 unicystic ameloblastomas (UA). The reduction rate in the three cystic groups was calculated. Relationships between the age of the patient, the ini⁃ tial size of the cyst and the reduction rate were also analyzed. Results The reduction size of the three types of cysts at 6 months after decompression: DC group 60.73% ± 7.15%, KTOC group 60.99% ± 4.00%, UA group 59.25% ± 6.81%. There was no difference in their reduction rate between the three types of cysts (P＞0.05). However, there was a statisti⁃ cally meaningful relationship between the initial size of the lesion and the absolute reduction rate in the UA and KOTC group (UA group: P＜0.01, R＝0.99; KTOC group: P＜0.01, R＝0.86). There was also a significant relationship be⁃ tween the age of the patient and the absolute reduction rate in the DC and UA groups (DC group: P＜0.01, R＝0.99; UA group: P＜0.01, R＝0.99). Conclusion There was no difference in the reduction in size due to decompression be⁃ tween the three types of cysts. However, the age of the patient and the initial size of the lesion showed a significant rela⁃ tionship with the reduction rate.

The scientific article contains a review of various methods of surgical treatment of follicular cysts of the jaws in children, both in the historical aspect, and in modern recommendations. Cases of 134 patients of Department of Maxillofacial Surgery of the Nizhny Novgorod Regional Children's Hospital evaluated the surgical techniques used in the clinic, viewed a permanent tooth preservation problem. There was made a conclusion about the indications for the choice of the surgical method, preservation rudiment of permanent tooth, and the necessity of early detection of follicular cysts of the jaws in children.

Large Unilocular radiolucent lesions of the jaws often present a dilemma to both, the Oral Pathologist and the Maxillofacial surgeon with regards to their accurate diagnosis as well as their most appropriate treatment modality. A precise identification as to whether the lesion is a cyst or a tumor is imperative before any treatment is instituted. Once the correct diagnosis and likely prognosis are established, a management protocol can be planned which will completely eliminate the lesion, while at the same time, ensure least possible morbidity for the patient, such as pathological jaw fractures, persisting neurological deficits, esthetic deformity, functional debility, recurrence/persistence of the lesion, etc. To establish the value of Immunohistochemistry (IH) as a Diagnostic marker and Prognostic indicator for extensive Unilocular radiolucent lesions of the jaws. To assess its role as an adjunct to Histopathological Examination (HPE) in distinguishing Odontogenic tumours from the cysts, by identifying the former using IH Tumor Markers; and in aiding in selection of the most appropriate and effective treatment option for each of such ambiguous lesions, based on their prognosis as indicated by the expression of lH Cell Proliferation Markers. Thirty cases of large Unilocular Radiolucent lesions of the jaws (Maxilla/Mandible) were managed over a period of three years. Histopathological examination (HPE) and Immunohistochemical (IH) analysis were carried out of the biopsy specimens in all the cases. Calretinin, an Immunohistochemical Tumor marker, was used to distinguish between Odontogenic cysts and tumours. Ki-67 and Proliferating Cell Nuclear Antigen (PCNA), Immunohistochemical Cell Proliferation markers, provided information on the aggressive potential of the lesions. On the basis of the above information, an appropriate management protocol was established for each of these different lesions. Nerve sparing enucleation and curettage was employed for the established cases of Odontogenic Cysts; Enucleation and curettage, peripheral ostectomy, followed by chemical cauterization was employed for the Unicystic Ameloblastomas and other Odontogenic tumours with a low Ki-67 and PCNA Proliferation Index (PI)/Labelling index (LI ≤ 3); Marginal resection was carried out for the tumours with a higher Labelling Index (LI >3 ≤5), and Segmental resection (including partial/complete Maxillectomy, Hemimandibulectomy with/without disarticulation) for the aggressive pathologies with high Labelling Index (LI > 5). Of the thirty cases of large Unilocular radiolucent lesions of the Maxilla and Mandible, thirteen were diagnosed as Dentigerous cysts, one as Dentigerous cyst showing Ameloblastomatous transformation; two as Unicystic Ameloblastomas, one as the Mural variant of Unicystic Ameloblastoma; four as Follicular Ameloblastomas, two as Plexiform Ameloblastomas; four as Acanthomatous Ameloblastomas; one as Ameloblastic Fibroma and two as Adenomatoid Odontogenic Tumours. The predictive and prognostic indication of the Immunohistochemical markers correlated well with the post treatment findings. In cases of extensive Unilocular lesions of the jaws, where ambiguity often exists in both diagnosis and appropriate treatment plan to be employed, Immunohistochemistry can serve as an invaluable tool in establishing the precise diagnosis, guiding the treatment plan, as well as indicating the likely prognosis of these lesions. • This extensive study establishes a correlation between the histopathological typing and Immunohistochemical analysis of extensive unilocular radiolucent lesions of jaws. • IH Tumour markers helped to arrive at confirmatory diagnosis. • IH Cell Proliferation markers provided information on the aggressive and invasive potential of the pathologies. • On the basis of the above, the most appropriate treatment modality could be applied to each case so as to achieve the most ideal outcome. Objective: Inflammatory dentigerous cysts (IDCs) are the most common cysts of inflammatory origin involving immature unerupted premolars or incisors. These cysts develop as a result of intrafollicular spread of periapical inflammation from an overlying nonvital primary tooth. Recently, many case reports have been reported in the literature indicating their presence at an early age. The objective of this study was to evaluate the frequency and demographic profile of IDCs in Central Indian population. Materials and Methods: Retrospective observation of the study was made on 112 IDCs from 6625 new children reporting to the Department of Pediatric Dentistry. Following variables were recorded: Frequency, demographic profile, radiographic features, treatment, and risk factors. Descriptive analysis was made of study variables using the SPSS Version 21.0. Results: The frequency of IDC was found to be 1.7% in Central Indian population. There was male predominance with a mean age of 8.8 years. Posterior mandibular region was the most common site followed by maxillary posterior. Marsupialization was the most commonly used treatment modality. Conclusion: Oral healthcare professionals should be aware of the frequency, demographic profile, and radiographical features of IDCs so that early diagnosis and definitive treatment of these lesions can be planned.

Background: Geographic differences are seen in prevalence and pattern oforofacial cysts and tumors. The purpose of this study was to determine the prevalence of cysts and tumors in the oral cavity. Material and Method: The present study was conducted to assess the prevalence of Odontogenic cysts and tumors diagnosed histopathologically in 260 patients over the period of 6 months. Data regarding age, gender, location, size, histopathological evaluation and treatment done were gathered from the clinical records. Data was analyzed using the SPSS - 20 (Chicago, IL-USA). The level of significance was set at P < 0.05. Results: In the present study 53.46% were male and 46.53% were females. The gender distribution showed a slight male predilection in Odontogenic Cysts, while the Odontogenic Tumors showed female predilection. Dentigerous cysts were most commonly occurring cysts than the other odontogenic cysts and shows male predilection. Ameloblastoma was most commonly occurring tumor than the other odontogenic tumors and shows female predilection. Conclusion: Our study concluded that odontogenic cysts were more prevalent than odontogenic tumors and the gender distribution in odontogenic cysts showed a slight male predilection, while the Odontogenic Tumors showed female predilection. Objective: Midkine is a heparin-binding growth factor whose expression is increased in most tumors, namely ameloblastomas. This study aimed to compare Midkine expression in different odontogenic lesions. Material and Method: This analytical cross-sectional study was performed on 52 definitely diagnosed odontogenic lesions including 15 dentigerous cysts, 13 odontogenic keratocysts, and 17 unicystic and 5 multicystic ameloblastomas archived from 1997 to 2015. Midkine expression was examined in tissue samples through immunohistochemistry. The nonparametric Kruskal-Wallis and Mann-Whitney tests were run as appropriate (P < 0.05). Results: The frequency of Midkine expression was < 20% in 7.7%, 20-50% in 25%, and > 50 % in 67.3% of the samples, indicating significant differences among the groups (P = 0.002). Moreover, the expression intensity was strong in 63.5%, moderate in 23.1%, and weak in 13.5% of odontogenic lesion samples (P = 0.071). The total staining score was weak in 3.8%, moderate in 48.1%, and strong in 48.1% of the cells, displaying significant differences between the study groups in this regard (P = 0.043). Conclusion: Midkine can be considered as both a differentiating factor and a molecular-targeted therapy in odontogenic lesions. The dentigerous cysts, like any other cystic lesion of the jaws, are benign lesions that may present different clinical signs depending on the location and stage of development. À dentigerous cyst or follicular cyst is a cyst odontogenic linked to abnormal development of the dental organ, These are the most frequent odontogenic cysts and they are generally related to the crowns of the teeth that have not erupted [1]. The cyst cavity is lined by epithelial cells derived from the reduced enamel epithelium during the formation of the enamel organ. The pathogenesis of dentigerous cyst is related to the pressure exerted by an erupting tooth on the follicle-clogging blood circulation and inducing accumulation of exudate between the reduced enamel epithelium and the crown of the tooth. We prospectively investigated in adults the decompression of dentigerous cysts associated with impacted third molars at high risk of iatrogenic damage during extraction. The computerised calculated volumetric postoperative reductions measured by cone-beam computed tomography at four and eight months were correlated with the duration of treatment, initial volume, and patient’s age. We prospectively monitored 14 adult patients with 14 dentigerous cysts that were associated with impacted third molars, for eight months after decompression. The mean (SD) reduction in volume during the first eight months was 71 (14) % (51% after the first four months and 20% after the second four months), with a monthly reduction rate of 9%. Both the total decrease in volume (a) and the monthly rate of reduction (b) were significantly associated (p < 0.01) and linearly correlated with the duration of decompression [r (a) = −0.89, r (b) = −0.61] and initial volume [r (a) = r (b) = 0.92], but not with the patient’s age (p = 0.49) [r (a) = r (b) = 0.04]. Background and Objective: Odontogenic cysts and ameloblastomas are the most commonly occurring lesions arising from odontogenic epithelium in the oral cavity. Clinical and radiographical findings have their limitations in correctly diagnosing these lesions which may significantly affect the treatment and prognosis of patients. Hence, histological diagnosis along with their classification is mandatory in these cases. This study was designed to see the relative frequency of histologically diagnosed odontogenic lesions in comparison to their provisional clinical diagnosis. Methods: A total of 114 blocks containing odontogenic lesions along with their clinical records were acquired from Morbid Anatomy and Histopathology department at University of Health Sciences Lahore and a local private laboratory from year 2011 - 2015. Hematoxylin and Eosin staining was performed on section for final diagnosis. Results: In a total of 114 patients, 32 (28.02%) differed in their initial clinical and final histological diagnosis. From a total of 32 lesions, 4 unicystic ameloblastomas were provisionally diagnosed as dentigerous cysts, 8 OKCs were provisionally diagnosed as multicystic ameloblastoma, 7 dentigerous cysts were provisionally diagnosed as radicular cysts, 4 OKCs were provisionally diagnosed as dentigerous cysts and 9 ameloblastomas were provisionally diagnosed as OKCs. Conclusion: Owing to the propensity for misdiagnosis, all the cystic lesions from the oral cavity should be sent to the laboratory for their final diagnosis for better treatment and prognosis in these patients. Objective: To determine the pattern and presentations of odontogenic jaw cysts in patients reporting at the Armed Forces Institute of Dentistry. Study Design: Descriptive study. Place and Duration of Study: Armed Forces Institute of Dentistry, from Jan to Dec 2007. Material and Methods: Hundred patients including 70 males and 30 females with the age range 5-65 years were included in the study. History, clinical examination, radiographic examination and histopathologic examination of lesion were carried out for each patient. A proforma was filled for each patient for all relevant information, presentation and pattern. Diagnosis was confirmed by histopathology. Data were analyzed using SPSS version 10. Results: Out of total 100 patients, 58% were diagnosed with radicular cysts, 25% with dentigerous cysts, 15% with odontogenic keratocyst, 1% patient with calcifying epithelial odontogenic cyst and 1% patient was diagnosed with eruption cyst. Conclusion: The study demonstrates that radicular cyst was the most common odontogenic cysts followed by dentigerous and odontogenic keratocysts respectively in our study sample. Here, we present an interesting case of dentigerous cyst in a 4-year-old boy, which developed around an unerupted maxillary first premolar. Histological examination revealed a cyst with 1-2-cell-thick epithelial lining and presence of inflammatory cells in the connective tissue.

Dentigerous cysts are benign odontogenic cysts that are accompanying with the crown of permanent teeth. They are typically single in incidence and are situated in the mandible. They are usually associated with impacted mandibular third molar and maxillary canines. Here we report a very rare presentation of dentigerous cyst in anterior maxilla accompanying with ectopically impacted maxillary second premolar. The dentigerous cysts are odontogenic cysts that are most frequently found in individuals in the age group between 20 and 40 years. A case of dentigerous cyst in a 9-year-old male patient was studied. On physical examination, temporary teeth persistence and swelling of the alveolar process with firm consistency were observed with no handling pain, and erasure of the vestibular sulcus was noted in the right upper jaw. On an occlusal radiograph, a great area radio lucid image involving the whole right alveolar ridge and maxillary sinus with the teeth presence was observed inside. Surgical access to the maxillary sinus was gained under general anesthesia, and then the rehabilitation with denture was made. The dentigerous cysts are frequently found in our practice, so it is crucial to follow-up with research to avoid complications and decide on the best treatment according to the individual case of each patient. Objectives: Odontogenic lesions evolve as a result of altered dental development. This study aimed to evaluate the prevalence and the coinfection of Epstein-Barr virus (EBV) and Kaposi sarcoma-associated herpesvirus (KSHV) in radicular cysts, dentigerous cysts, odontogenic keratocysts, and ameloblastomas., Methods: Polymerase chain reaction (PCR) was used to analyse 66 cases of odontogenic lesions for the presence of EBV-DNA and KSHV-DNA. These lesions were 15 radicular cysts, 16 dentigerous cysts, 18 odontogenic keratocysts, and 17 ameloblastomas., Results: EBV-DNA was detected in 24 (36.4%) of the studied samples as follows: 6 samples (40.0%) of radicular cysts, 4 (25.0%) of dentigerous cysts, 10 (55.6 %) of odontogenic keratocysts, and 4 (23.5%) of ameloblastomas (P = .168). KSHV-DNA was found in 16 (24.2%) of the studied samples as follows: 1 sample (6.7%) of radicular cysts, 6 (37.5%) of dentigerous cysts, 8 (44.4 %) of odontogenic keratocysts, and 1 (5.9%) of ameloblastomas (P = .001). Additionally, EBV and KSHV were positively correlated in all studied samples (P = .002)., Conclusions: Both EBV and KSHV are found in odontogenic cysts and ameloblastomas. KSHV and EBV are more prevalent in odontogenic keratocysts than in other studied odontogenic lesions. Among inflammatory cells, mast cells can be associated with their pathogenesis. The aim of this study was to analyze mast cells in periapical cysts, dentigerous cysts, and keratocystic odontogenic tumors. Methods: Tissue sections were submitted to toluidine blue staining and immunohistochemistry with antibody anti-tryptase (clone G3). Mast cells were quantitated using Image-Pro Plus software to obtain the mean number of mast cells in three regions: epithelial, superficial portion of the fibrous wall and deep portion of the fibrous wall from 20 periapical cysts, 20 dentigerous cysts (six non-inflamed and 14 inflamed) and 20 keratocystic odontogenic tumors (four non-inflamed and 16 inflamed). Results: The mean number of mast cells detected per lesion by immunohistochemistry (4.1) was higher than by histochemistry (1.5) ( P < 0.0001). Inflamed dentigerous cysts and keratocystic odontogenic tumors showed a higher mean number of mast cells than non-inflamed lesions in all regions. The deep region from all cysts showed the highest mean number of degranulated mast cells, except for non-inflamed keratocystic odontogenic tumors analyzed by immunohistochemistry. Conclusions: Immunohistochemical staining detected higher number of mast cells than histochemistry. The higher number of mast cells observed in inflamed lesions could indicate the participation of these cells in the inflammatory response in odontogenic lesions. Abstract Aim To evaluate parathyroid hormone/parathyroid hormone-related peptide receptor 1 (PTHR1) expression in odontogenic cystic lesions and to compare immunoexpression between the lesions. Methodology Thirty-five radicular cysts, 22 dentigerous cysts and 17 keratocystic odontogenic tumours were evaluated. Immunohistochemical reactions against PHTR1 were carried out in 3-μm histological sections, and the expression and the intensity of PTHR1 expression were evaluated. For statistical analysis, the Fisher exact test was used, with a significance of 5%. Results The intensity of expression in the epithelial lining was significantly weaker in the radicular cyst ( P = 0.007). However, in the fibrous capsule, the radicular cyst presented higher positivity for PTHR1 ( P = 0.04). Conclusions The probable co-expression of PTHrP and PTHR1 in odontogenic cystic lesions may eventually have an autocrine and/or paracrine stimulus in the epithelial and mesenchymal cells, inducing proliferation and lesion growth. A dentigerous cyst is a developmental odontogenic cyst that develops when fluid accumulates between the reduced enamel epithelium and the tooth crown of an unerupted tooth. Treatment modalities range from marsupialization to enucleation and are based on the premise that the pathological process can be controlled locally with minimal injury to the adjacent host structures. The purpose of this case report was to describe the diagnosis of a large expansible dentigerous cyst in an 11-year-old boy. Abstract: Dentigerous cysts are benign odontogenic cysts that arise from the dental follicle of an unerupted or developing tooth. They are the second most common odontogenic cysts after radicular cysts. Multiple dentigerous cysts have been reported on occasion in association with syndromes such as cleidocranial dysplasia, mucopolysaccharidosis and basal cell nevus syndrome. We report the clinical and radiologic findings of bilateral maxillary dentigerous cysts in a nonsyndromic 10-year-old boy, a condition that, to our knowledge has only been reported twice. Abstract: The p53 is a well-known tumor suppressor gene, the mutations of which are closely related to the decreased differentiation of cells. Findings of studies on immunohistochemical P53 expression in odontogenic cysts are controversial. The present study was carried-out to investigate the immunohistochemical expression of P53 protein in odontogenic cysts. Thirty paraffin blocks of diagnosed odontogenic cysts were processed to determine the immunohistochemical expression of P53 protein. Nine of the 11 odontogenic keratocysts (81.8%) expressed P53, one of three dentigerous cyst cases expressed P53, while none of the 16 radicular cysts expressed P53 protein. AIM: To present a series of clinical cases of children treated for large dentigerous cysts with a conservative and tissue preserving surgical approach. METHODS: A follow-up study of 11 children aged 5-11 years old treated for large dentigerous cysts with decompression. The children were followed from the first appointment when the cysts were diagnosed until the full eruption of the involved and displaced teeth. The treatment procedure is described step by step. Three of the 11 cases are presented in detail. RESULTS: All permanent teeth displaced by the dentigerous cysts, except a wisdom tooth and a maxillary canine, erupted in a correct position and in good occlusion. No facial disfigurations or defects of oral tissues occurred. CONCLUSIONS: A decompression approach, as described in this paper, is rewarding for the patient and the surgeon. It would be unethical and destructive to the maxillofacial bone structure not to treat large dentigerous cysts in children by decompression. [ABSTRACT FROM AUTHOR]

Background: The goal of the study was to create a histopathology image classification automation system that could identify odontogenic keratocysts in hematoxylin and eosin-stained jaw cyst sections. Methods: From 54 odontogenic keratocysts, 23 dentigerous cysts, and 20 radicular cysts, about 2657 microscopic pictures with 400× magnification were obtained. The images were annotated by a pathologist and categorized into epithelium, cystic lumen, and stroma of keratocysts and non-keratocysts. Preprocessing was performed in two steps; the first is data augmentation, as the Deep Learning techniques (DLT) improve their performance with increased data size. Secondly, the epithelial region was selected as the region of interest. Results: Four experiments were conducted using the DLT. In the first, a pre-trained VGG16 was employed to classify after-image augmentation. In the second, DenseNet-169 was implemented for image classification on the augmented images. In the third, DenseNet-169 was trained on the two-step preprocessed images. In the last experiment, two and three results were averaged to obtain an accuracy of 93% on OKC and non-OKC images. Conclusions: The proposed algorithm may fit into the automation system of OKC and non-OKC diagnosis. Utmost care was taken in the manual process of image acquisition (minimum 28–30 images/slide at 40× magnification covering the entire stretch of epithelium and stromal component). Further, there is scope to improve the accuracy rate and make it human bias free by using a whole slide imaging scanner for image acquisition from slides. [ABSTRACT FROM AUTHOR]

This paper presents a case of a dentigerous cyst accompanied by a history of inflammation, resorption of the roots of the first molar and the lingual aspect of the distal root of the second molar, and devitalization of the 2 premolars. The treatment option chosen was marsupialization of the cyst, extraction of the involved teeth, endodontic treatment of both premolars, and implant insertion in the area of the first mandibular molar at a later stage. Twelve months postsurgery the area of interest was almost flattened and the radiographic examination revealed total disappearance of the radiolucency. An implant of 15 x 4 mm was inserted in the area of the first mandibular molar. Despite the excellent implant stability achieved, a thin central zone of the defect remained void of bone. After complete excision of soft tissue the defect was filled with an alloplastic bone substitute. Eight months later (20 months postsurgery), the implant was uncovered and the restorative procedure completed. The implant and the crown have been functioning sign and symptom free for 48 months. Marsupialization was preferred instead of enucleation due to (1) proximity of the cyst to the mandibular canal; (2) need for apicectomy of both premolars to gain good access for enucleation; (3) refusal of the patient to undergo a second surgery for bone harvesting; (4) lower probability for postoperative contamination; (5) need of small quantity of bone substitute; (6) easier extraction of the impacted second molar due to its coronal movement. The disadvantages of the treatment were the long healing period and the discomfort of the patient at the early stages of marsupialization. It was judged that the advantages outweighed the disadvantages in this case. We discuss the findings of other authors who support the conservative approach to jaw cysts in a young population, and the concerns that exist in the literature about the ability of osseoconductive graft materials to generate vital bone and achieve implant-to-bone contact.
