Background: The objective of this study was to evaluate the pain and inflammatory response in soft tissues using healing and prosthetic abutments of different diameters and lengths. Methods: The study population was rehabilitated with Astra Tech EV single implants (Dentsply Sirona, Atlantis, Dentsply Sirona S.A., Barcelona, Spain) of 4.2 and 4.8 millimetres in diameter in the upper and lower maxilla and loaded with custom abutments digitally designed using Dentsply Sirona's Virtual Atlantis Design software (Atlantis WebOrder, Dentsply Sirona S.A., Barcelona, Spain), version 4.6.5. The custom abutments had a larger diameter than the healing abutments to evaluate for biomarkers through ELISA. Results: Rehabilitations in the mandible and with healing abutments with diameters less than 4.29 mm and rehabilitators with diameters less than 2.18 mm elicited a higher pain and inflammatory response and, in turn, higher interleukin-1β values. Conclusions: Greater inflammation was evident in cases in which healing abutments with reduced diameter were used compared to the same subsequent rehabilitation with prosthetic abutments with larger diameters. Background. A key factor for the success or failure of an implant is how the stresses are transferred to the surrounding bone. The implant‒abutment connection (IAC) is paramount for implant success. The purpose of this finite element analysis (FEA) study was to evaluate the stress distribution in and around three different implant‒abutment interfaces with platform-switched and platform-matched abutments using the finite element method (FEM). Methods. Three distinct types of IAC were selected: tri-channel internal connection, conical connection, and internal hex connection. Six models were generated, three in platform-switched and three in non-platform-switched configuration. Computer-Aided Three-Dimensional Interactive Application (CATIA) V5 R20 software was used to generate virtual models of the implants and the mandible. The models were transferred to Analysis of Systems (ANSYS) 15.0 software, in which the models were meshed and underwent FEA. Results. On the crestal bone, the highest von Mises stresses in platform-switched abutments were noticed in the internal hex implant‒abutment system (370 MPa), followed by the tri-channel implant‒abutment system (190 MPa) and conical implant‒abutment system (110 MPa). On the implant and the abutment screw, the highest von Mises stresses were observed in the internal hex implant‒abutment system, followed by the conical implant abutment system and tri-channel implant‒abutment system. Platform-switched implants had a more favorable stress distribution on crestal bone. Conclusion. Within the constraints of the current study, the internal hex connection exhibited the highest stress. In contrast, the conical abutment connection with platform switching configuration had more favorable stress distribution in crestal bone than other implant abutment systems.

This study investigated the effects of antirotational titanium bases on the mechanical behavior of CAD/CAM titanium bases used for implant-supported prostheses. The aim was to assess the impact on the marginal fit, fatigue behavior, stress concentration, and fracture load of implant-supported CAD/CAM zirconia crowns. Forty titanium implants were divided into two groups: those with antirotational titanium bases (ARs) and those with rotational titanium bases (RTs). Torque loosening and vertical misfit were evaluated before and after cyclic fatigue testing (200 N, 2 Hz, 2 × 106 cycles). Fracture resistance was assessed using a universal testing machine (1 mm/min, 1000 kgf), and failed specimens were examined with microscopy. Three-dimensional models were created, and FEA was used to calculate stress. Statistical analysis was performed on the in vitro test data using two-way analysis of variance and Tukey's test (α = 0.5). Results show that the presence of an antirotational feature between the implant and titanium base reduced preload loss and stress concentration compared to rotational titanium bases. Aim: The conometric concept was proposed as a possible connection between the abutment and the prosthetic coping. This research aimed to review the features and possible clinical uses of this connection in an implant-supported fixed prosthesis. Methods: An electronic search was conducted on an online database for the topic in object; articles published in international literature were considered and the research gave 17 results, and 6 parameters were analyzed. Results: This connection eliminated the possibility of cement residues in the subgingival region, reducing the risk of inflammation of peri-implant soft and hard tissues; not having to remove the cement residues, it is possible to place the margins in more apical portions, improving the aesthetics outcomes of the rehabilitations. It is also known that the retention by means of a screw causes a weakening of the restoration. The retentive force is adequate for fixed rehabilitation even after a high number of insertion–disengagement cycles; in vitro studies have also shown a high bacterial sealing. Implant rehabilitation using preformed components, such as conometric hoods, is helpful for CAD/CAM, so a digital workflow is possible. Several types of prosthesis were presented, all of which demonstrated adequate clinical performance in the follow-up observation. Conclusions: This type of connection seems to be suitable to support fixed implant rehabilitations, but long-term clinical studies are needed to validate this system.

Objectives: To evaluate the association between different vertical levels of the abutment margin and residual cement prevalence in cement‐retained implant restorations with customized abutments. Methods: One hundred and nine single‐unit cement‐retained implant restorations with a screw‐access channel were included. The crowns were intraorally cemented on the abutments, and excess cement was removed. The abutment–crown complex was unscrewed, and the abutment–crown complex and peri‐implant tissue were photographed. Residual cement presence was recorded by dividing the abutment–crown complex and peri‐implant tissue into four quadrants: mesial, distal, buccal, and lingual. The prevalence of residual cement was compared according to the height of the custom abutment margin of the corresponding quadrant. A multilevel model was used for statistical analysis (α =.05). Results: Cement remnants were discovered on 72.48% of the dental implants. When the restoration quadrants were compared, cement remnants were present on 51.38%, 39.45%, 20.18%, and 17.43% of the mesial, distal, buccal, and lingual surfaces, respectively (p <.01). Regarding the abutment margin level, cement residues were found in 60.22% and 61.4% of the 0.5 mm subgingival and ≥1 mm subgingival margin groups, respectively, which were significantly more than those in the supragingival (23.65%) and equigingival (26.59%) margin groups (p <.01). After adjustment for confounding factors, the adjusted odds ratio (with 95% confidence interval) for residual cement in the subgingival margin groups was 3.664 (1.71, 7.852) when compared to the supragingival and equigingival margin groups. Conclusions: The risk of residual cement occurrence was 3.66‐fold higher with a subgingival abutment margin than with supragingival and equigingival abutment margins. [ABSTRACT FROM AUTHOR]

Objective: To assess the marginal and bacterial microleakage in zirconia and CAD‐CAM or cast Co‐Cr implant abutments. Methods: Sixty‐four conical connection implants with their respective abutments were divided into four groups (Co‐Cr (milled, laser‐sintered, and cast) and Zirconia (milled)). All specimens were subjected to a chewing simulation and thermocycling. After aging process, specimens were submerged in a 0.2% methylene blue solution with Porphyromonas gingivalis (P.g) for 48 h. The marginal microleakage was measured using a 40× optical microscopy at the internal part of the implant, and when positive microleakage was observed, a DNA isolation with a polymerase chain reaction (PCR) test was used. The microbiological assessment was based on colony forming units (CFUs). Results: Thirty (47%) implant‐abutments presented microleakage and the PCR was performed on those specimens (1 Zirconia, 1 Co‐Cr milled, 14 Co‐Cr laser‐sintered and 14 cast). Seven specimens (1 Co‐Cr laser‐sintered and six cast) presented values below the PCR detection limit (< 100 CFUs). The lowest CFUs count occurred in the Co‐Cr milled group (5.17E+02 CFUs/ml) followed by zirconia (7.70E+03 CFUs/ml). The Co‐Cr cast (9.39E+03 CFUs/ml) and laser‐sintered (2.4E+05 CFUs/ml) groups had higher bacterial count. The CFU count comparison performed between Co‐Cr cast and laser‐sintered resulted in a statistically significant differences in favor of Co‐CrCL (p <.05). Conclusions: The abutment material and fabrication technique affected the implant‐abutment microleakage. Background: Little is known regarding the interaction of dental implant surface nanotubes and oral soft and hard tissues. The purpose of this study was to evaluate both histologically and radiographically the qualitative and quantitative effects of dental implant surface nanotubes on hard and soft tissue in a canine model.Methods: Three subgroups consisting of a combination of test and control implants and abutments (Group A: control implant/control abutment, Group B: control implant/test abutment: Group C: test implant/test abutment) were placed in edentulous mandibles of six large-breed canines. Implants and abutments were placed on one side at baseline, and on the opposite side of the mandible at week 10; sacrifice occurred at week 12. Quantitative and qualitative analyses were used to measure newly formed hard and soft tissues histologically and radiographically.Results: The mean radiographic change in marginal bone level from weeks 0 to 12 between implant groups was not statistically significant (P > 0.05). Mean soft tissue contact (junctional epithelium + connective tissue) for Groups A, B, and C were 2.29, 2.33, and 2.31 mm, respectively, with no statistically significant difference (P > 0.05) between the groups. All connective tissue fibers were oriented parallel to the abutment regardless of surface treatment.Conclusions: The findings of this study suggest that healing of hard and soft tissues around implants and abutments is similar when comparing grit-blasted surfaces to machined, turned surfaces with nanotubes. The objective of this study was to perform a quality analysis of systematic reviews with meta-analyses that focused on the comparison of platform-switching (implant-abutment mismatching) and platform-matched (PM) implants. The assessment of multiple systematic reviews (AMSTAR) and Glenny (Checklist) Scales were used to qualify the studies. PubMed, Scientific Electronic Library Online (SciELO), Web of Science (formerly ISI Web of Knowledge), and Cochrane databases were searched, by topic, for systematic reviews on dental implants with switching platforms. A total of 8 systematic reviews, including 7 studies with meta-analyses, were selected. The AMSTAR scale indicated a high (n = 6) to moderate (n = 2) score for the included studies. The quantitative analysis indicated that platform-switching implants preserved more bone tissue when compared with platform-matched implants (6 meta-analyses; P <.001, smaller mean difference: −0.29 mm, 95% CI: −0.38, −0.19 and greater mean difference: −0.49 mm, 95% CI: −0.73, −0.26). Quantitative analysis based on 7 systematic reviews with meta-analysis indicated positive peri-implant bone preservation for implants restored with an implant-abutment mismatching (PSW). Further, there is evidence to improve the design of current systematic reviews. Future systematic reviews in this thematic area should consider searches in gray literature and different databases and include only randomized controlled clinical studies. [ABSTRACT FROM AUTHOR]

Small-diameter dental implants are associated with a higher risk of implant failure. This study used both three-dimensional finite-element (FE) simulations and in-vitro experimental tests to analyze the stresses and strains in both the implant and the surrounding bone when using one-piece (NobelDirect) and two-piece (NobelReplace) small-diameter implants, with the aim of understanding the underlying biomechanical mechanisms. Methods: Six experimental artificial jawbone models and two FE models were prepared for one-piece and two-piece 3.5-mm diameter implants. Rosette strain gauges were used for in-vitro tests, with peak values of the principal bone strain recorded with a data acquisition system. Implant stability as quantified by Periotest values (PTV) were also recorded for both types of implants. Experimental data were analyzed statistically using Wilcoxon's rank-sum test. In FE simulations, the peak value and distribution of von-Mises stresses in the implant and bone were selected for evaluation. Results: In in-vitro tests, the peak bone strain was 42% lower for two-piece implants than for one-piece implants. The PTV was slightly lower for one-piece implants (PTV = −6) than for two-piece implants (PTV = −5). In FE simulations, the stresses in the bone and implant were about 23% higher and 12% lower, respectively, for one-piece implants than those for two-piece implants. Conclusion: Due to the higher peri-implant bone stresses and strains, one-piece implants (NobelDirect) might be not suitable for use as small-diameter implants.

Aim: The purpose of the study was to evaluate and compare the stress distribution at the implant abutment interface in a platform switched Morse taper and pseudo platform switched internal hex connection implant system under simulated load using a 3D finite element analysis. Materials and Method: In the study ANKYLOS which is a Morse taper platform switching implant system (Group A) and XiVE having an internal hex connection which forms a pseudo platform switched implant system (Group B) were used. The geometric properties of implant systems were modeled using 3D finite element analysis. The masticatory forces of 200 N, 500 N and 1000 N were applied axially to both the implant systems with the abutment screw tightening torque of 110 Nm2, 320 Nm2 and 550 Nm2. Von Mises stress distribution pattern was analyzed considering the objectives of the study. In order to interpret the result, the data generated by finite element analysis (FEA) were statistically analyzed. Results: There was better stress distribution for Group B at the neck of implant as compared to Group A but on screw Group A showed more decrease in force concentration and therefore better stress distribution. Conclusion: Stress concentration on connecting screw of pseudo platform switch implant was more and may lead to fracture or loosing of screw than platform switched Morse taper implant.

Background: The aim of this study is to investigate peri-implant and intraconnection microflora of healthy implants restored with cemented and screwed superstructures.Methods: Patients with two to three implants restored with cemented or screwed restorations and 5 years of follow-up were recruited. Samples were taken from peri-implant sulci, adjacent teeth, and the inner portion of connections. Prevalence of positive sites and bacterial loads for 10 microorganisms were obtained with quantitative real-time polymerase chain reaction. Implant connection permeability to the studied microorganisms was estimated using a standardized bacterial contamination index. Statistical analysis was performed using a generalized estimating equations model, Wald χ(2) test, and the least significant difference test.Results: The final sample consisted of 18 patients (55 implants) in the cemented group and 22 patients (46 implants) in the screw-retained group. Regarding prevalence of positive sites, significant differences between groups were only found for Tannerella forsythia, which was 8.7 times more frequent at peri-implant sulci of cemented than screw-retained prostheses. Bacterial loads of Porphyromonas gingivalis, T. forsythia, Parvimonas micra, and total bacterial load were significantly higher at peri-implant sulci for the cemented group; at the inner portion of connections, values were significantly higher for P. micra and Fusobacterium nucleatum for the screw-retained group. Contamination index values demonstrated higher permeability to most microbes in the cemented group.Conclusions: Internal implant surfaces were microbiologically contaminated for both cemented and screw-retained superstructures. Differences were found between the two methods of prosthetic retention: the cemented group presented significantly higher bacterial loads in the peri-implant sulcus but significantly lower bacterial loads at the inner portion of the implant connection. Background and aims. Because of compromised angulations of implants, the abutments are sometimes prepared. The purpose of this study was to investigate the effect of removing one wall of the implant abutment on the retention of cement-retained crowns. Materials and methods. Four prefabricated abutments were attached to analogues and embedded in acrylic resin blocks. The first abutment was left intact. Axial walls were partially removed from the remaining abutments to produce abutments with three walls. The screw access channel for the first and second abutments were completely filled with composite resin. For the third and fourth abutments, only partial filling was done. Wax-up models were made by CAD/CAM. Ten cast copings were fabricated for each abutment. The copings of fourth abutment had an extension into the screw access channel. Copings were cemented with Temp Bond. The castings were removed from the abutment using an Instron machine, and the peak removal force was recorded. A one-way ANOVA was used to test for a significant difference followed by the pairwise comparisons. Results. The abutments with opened screw access channel had a significantly higher retention than the two other abut- ments. The abutment with removed wall and no engagement into the hole by the castings exhibited the highest retention. Conclusion. Preserving the opening of screw access channel significantly increases the retention where one of the axial walls of implant abutments for cement-retained restorations is removed during preparation. Purpose: The aim of the present study was to evaluate if a smaller morse taper abutment has a negative effect on the fracture resistance of implant-abutment connections under oblique compressive loads compared to a conventional abutment., Materials and Methods: Twenty morse taper conventional abutments (4.8 mm diameter) and smaller abutments (3.8 mm diameter) were tightened (20 Ncm) to their respective implants (3.5 × 11 mm) and after a 10 minute interval, implant/abutment assemblies were subjected to static compressive test, performed in a universal test machine with 1 mm/min displacement, at 45° inclination. The maximum deformation force was determined. Data were statistically analyzed by student t test., Results: Maximum deformation force of 4.8 mm and 3.8 mm abutments was approximately 95.33 kgf and 95.25 kgf, respectively, but no fractures were noted after mechanical test. Statistical analysis demonstrated that the evaluated abutments were statistically similar (P=.230)., Conclusion: Abutment measuring 3.8 mm in diameter (reduced) presented mechanical properties similar to 4.8 mm (conventional) abutments, enabling its clinical use as indicated.
