Substrate reduction therapy (SRT) has been proposed as a new gene therapy for Fabry disease (FD) to prevent the formation of globotriaosylceramide (Gb3). Nanomedicines containing different siRNA targeted to Gb3 synthase (Gb3S) were designed. Formulation factors, such as the composition, solid lipid nanoparticles (SLNs) preparation method and the incorporation of different ligands, such as gold nanoparticles (GNs), protamine (P) and polysaccharides, were evaluated. The new siRNA–golden LNPs were efficiently internalized in an FD cell model (IMFE-1), with GNs detected in the cytoplasm and in the nucleus. Silencing efficacy (measured by RT-qPCR) depended on the final composition and method of preparation, with silencing rates up to 90% (expressed as the reduction in Gb3S-mRNA). GNs conferred a higher system efficacy and stability without compromising cell viability and hemocompatibility. Immunocytochemistry assays confirmed Gb3S silencing for at least 15 days with the most effective formulations. Overall, these results highlight the potential of the new siRNA–golden LNP system as a promising nanomedicine to address FD by specific SRT.

Urinary tract infections (UTIs) are extremely common and a major driver for the use of antimicrobials. Calcium fosfomycin is an old antibiotic indicated for the treatment of UTIs; however, data about its urine pharmacokinetic profile are scarce. In this work, we have evaluated the pharmacokinetics of fosfomycin from urine concentrations after oral administration of calcium fosfomycin to healthy women. Moreover, we have assessed, by pharmacokinetic/pharmacodynamic (PK/PD) analysis and Monte Carlo simulations, its effectiveness considering the susceptibility profile of Escherichia coli, the main pathogen involved in UTIs. The accumulated fraction of fosfomycin excreted in urine was around 18%, consistent with its low oral bioavailability and its almost exclusively renal clearance by glomerular filtration as unchanged drug. PK/PD breakpoints resulted to be 8, 16, and 32 mg/L for a single dose of 500 mg, a single dose of 1000 mg, and 1000 mg q8h for 3 days, respectively. For empiric treatment, the estimated probability of treatment success was very high (>95%) with the three dose regimens, considering the susceptibility profile of E. coli reported by EUCAST. Our results show that oral calcium fosfomycin at a dose level of 1000 mg every 8 h provides urine concentrations sufficient to ensure efficacy for the treatment of UTIs in women.

[EN] Interdisciplinary collaboration between health professionals is essential to improve health outcomes. The competences of pharmacists make them professionals capable of contributing to the comprehensive management of pharmacotherapy in collaboration with other healthcare professionals. The Council of Europe adopted resolution CM/Res(2020)3 on implementation of pharmaceutical care for the benefit of patients and health services in order to promote the appropriate and safe use of medicines. One of the activities to be carried out through the pharmaceutical care process is the detection of drug related problems, such as contraindications, duplications, prescription errors, interactions, etc. In many cases, it involves regular patient follow-up and needs an evaluation of the interventions performed, requiring the establishment of an appropriate interprofessional collaboration framework. This article discusses the issues to be addressed to face the change of model towards a care-based pharmacy.

En la actualidad, el uso del ARNm se ha extendido debido su alta eficacia, perfil de seguridad y versatilidad para una rápida producción de proteínas. Las partículas sólidas lipídicas (SLNs) son consideradas uno de los sistemas no virales lipídicos más eficaces para la administración de ácidos nucleicos. La terapia génica se ha propuesto como una alternativa eficaz para el tratamiento de enfermedades oculares y, en concreto, para la inflamación corneal. Los corticoides son el principal tratamiento para la inflamación corneal, pero su eficacia es limitada y presentan múltiples efectos adversos. La administración de la interleuquina 10 (IL-10) antiinflamatoria a nivel ocular se ha propuesto como una alternativa eficaz. Sin embargo, su escasa biodisponibilidad ocular y su corta vida media limitan su uso. El objetivo de la presente tesis doctoral fue el desarrollo y evaluación de sistemasde liberación de ácidos nucleicos (ADNp o ARNm) basados en SLNs para el tratamiento de la inflamación corneal mediante la administración tópica ocular. Para ello, tras la caracterización fisicoquímica y evaluación in vitro de las formulaciones en diferentes líneas celulares, se evaluó su capacidad para transfectar e inducir la expresión de IL-10 in vivo en la córnea de los ratones tras su administración en forma de colirio. Las formulaciones fueron capaces de producir IL-10 en la córnea. Este trabajo muestra el potencial de la suplementación génica no viral para el tratamiento de la inflamación corneal.

The anti-inflammatory cytokine Interleukin-10 (IL-10) is considered an efficient treatment for corneal inflammation, in spite of its short half-life and poor eye bioavailability. In the present work, mRNA-based nanomedicinal products based on solid lipid nanoparticles (SLNs) were developed in order to produce IL-10 to treat corneal inflammation. mRNA encoding green fluorescent protein (GFP) or human IL-10 was complexed with different SLNs and ligands. After, physicochemical characterization, transfection efficacy, intracellular disposition, cellular uptake and IL-10 expression of the nanosystems were evaluated in vitro in human corneal epithelial (HCE-2) cells. Energy-dependent mechanisms favoured HCE-2 transfection, whereas protein production was influenced by energy-independent uptake mechanisms. Nanovectors with a mean particle size between 94 and 348 nm and a positive superficial charge were formulated as eye drops containing 1% (w/v) of polyvinyl alcohol (PVA) with 7.1–7.5 pH. After three days of topical administration to mice, all formulations produced GFP in the corneal epithelium of mice. SLNs allowed the obtaining of a higher transfection efficiency than naked mRNA. All formulations produce IL-10, and the interleukin was even observed in the deeper layers of the epithelium of mice depending on the formulation. This work shows the potential application of mRNA-SLN-based nanosystems to address corneal inflammation by gene augmentation therapy.

Throughout history, the pharmacy has undergone major transformations. Previously, pharmacy was based on the preparation and dispensing of medicines, but due to the changes in society, the pharmaceutical profession has taken a more care path. Thus, in recent decades the development of pharmaceutical care and healthcare pharmacy has been prompted. Pharmaceutical care can be defined as the pharmac ist’s professional practice of caring for the patient’s needs by detecting, preventing, and resolving drug-related problems in a continu ous, systematic, and documented manner, in collaboration with the patient and other health care professionals to achieve accurate patient quality of life. Related to pharmaceutical care and healthcare pharmacy, Professional Pharmaceutical Care Services (PPCS) have been developed; so that community pharmacies offer a wide range of services to all citizens. Nowadays, in almost every country in the world, new services are being designing and implementing with the aim to also integrate them into the activity of community pharmacy. PPCSs fall currently into two groups. On the one hand, the Pharmaceutical Care Services and on the other hand, the Services related to Community Health. A few years ago, and in conjunction with the Basque Pharmacists’ Council, Basque pharmacies began offering PPCSs. These services, as important tools for change in the profession, do not only benefit patients, but also the collective of pharmacists and pharmacists themselves. En las últimas décadas se ha detectado un aumento de casos diagnosticados de rosácea, afectando aproximadamente a un 10% de la población. La rosácea es una enfermedad inflamatoria crónica caracterizada por la aparición de reacciones eritematosas transitorias o permanentes en la zona central del rostro. También es común la aparición de telangiectasias, pápulas, pústulas, fimas y lesiones oculares. Su etiopatogenia todavía no es muy conocida y su sintomatología puede afectar psicológicamente a quien la padece, haciendo que su calidad de vida disminuya. Por este motivo es importante empezar cuanto antes un tratamiento adecuado que ayude a minimizar su clínica, aunque todavía no se ha dado con un tratamiento capaz de curarla. El tratamiento se basa en informar al paciente sobre una serie de medidas generales que debe seguir, además de una terapia farmacológica administrada de forma tópica o sistémica. También se ha demostrado que son eficaces otra serie de terapias como son el láser o la luz pulsada intensa. Un asesoramiento adecuado desde la oficina de farmacia puede ser de gran ayuda ya que el personal de la farmacia puede aportar una serie de consejos dermofarmacéuticos para combatir los signos de la rosácea y mejorar el estado de la piel.

25 p. : il. -- Bibliogr.: p. 20-21, Hoy en día el asma y la EPOC son enfermedades prevalentes en España por lo que el estudio de dichas enfermedades y de su tratamiento es un tema de actualidad clínica. Según la OMS (Organización Mundial de la Salud) el asma se define como una enfermedad crónica que se caracteriza por ataques recurrentes de disnea y sibilancias, que varían en severidad y frecuencia de una persona a otra. Asimismo, esta misma organización define la EPOC (Enfermedad Pulmonar Obstructiva Crónica) como un concepto general que designa diversas dolencias pulmonares crónicas que limitan el flujo de aire en los pulmones. Principalmente, la terapia que utilizan los pacientes con asma y EPOC es la terapia inhalada. En los estudios relativas a la terapia inhalada se aprecian diferentes errores relacionados con el tipo de inhalador, el paciente, la técnica de inhalación, etcétera. El objetivo de este trabajo es desarrollar un protocolo de actuación para pacientes con asma y EPOC en colaboración con los datos recogidos en la oficina de farmacia Isabel Rivero de Vitoria consiguiendo así una mayor efectividad y seguridad en el tratamiento de dichas patologías.

La ansiedad, es un estado emocional desagradable que cursa con nerviosismo y una serie de respuestas tanto, psicológicas, conductuales, cognitivas, sociales como físicas. Esta patología, si se prolonga en el tiempo puede causar problemas de salud más graves. Su tratamiento se basa en paliar la sintomatología con la que cursa, bien sea con tratamiento psicológico o farmacológico. En el 2020, debido al colapso de los centros de atención primaria, se ha producido un incremento en el número de consultas en las oficinas de farmacia, por parte de pacientes en busca de tratamientos que no requieran prescripción médica y consejos farmacéuticos para el control de determinadas patologías. En la farmacia Alfonso Eguinoa López de Uralde, las consultas más prevalentes en este periodo fueron las relacionadas con la ansiedad y los problemas para conciliar el sueño. En esta misma farmacia, se ha producido, durante el año de la pandemia, un aumento de casi el 16% en el número de fármacos para el tratamiento de la ansiedad frente al 2019 y un incremento de más del 38% en el caso de productos que no necesitan prescripción médica para el tratamiento de esta patología. Es por ello, que es importante destacar el papel fundamental que puede tener la farmacia asistencial dentro del sistema sanitario a la hora de optimizar los resultados en salud de los y las pacientes a través de servicios profesionales farmacéuticos asistenciales como es el de indicación terapéutica.

The development of safe and effective nucleic acid delivery systems remains a challenge, with solid lipid nanoparticle (SLN)-based vectors as one of the most studied systems. In this work, different SLNs were developed, by combination of cationic and ionizable lipids, for delivery of mRNA and pDNA. The influence of formulation factors on transfection efficacy, protein expression and intracellular disposition of the nucleic acid was evaluated in human retinal pigment epithelial cells (ARPE-19) and human embryonic kidney cells (HEK-293). A long-term stability study of the vectors was also performed. The mRNA formulations induced a higher percentage of transfected cells than those containing pDNA, mainly in ARPE-19 cells; however, the pDNA formulations induced a greater protein production per cell in this cell line. Protein production was conditioned by energy-dependent or independent entry mechanisms, depending on the cell line, SLN composition and kind of nucleic acid delivered. Vectors containing 1,2-dioleoyl-3-trimethylammonium-propane (DOTAP) as unique cationic lipid showed better stability after seven months, which improved with the addition of a polysaccharide to the vectors. Transfection efficacy and long-term stability of mRNA vectors were more influenced by formulation-related factors than those containing pDNA; in particular, the SLNs containing only DOTAP were the most promising formulations for nucleic acid delivery.

221 p., El proceso inflamatorio de la superficie ocular subyace en la base de importantes enfermedades que pueden provocar incluso la pérdida de la visión en los casos más graves. Actualmente, la terapia de elección de la inflamación corneal o keratitis, además de presentar eficacia limitada, conlleva importantes efectos adversos. La interleuquina-10 (IL-10) juega un papel fundamental en la resolución de la inflamación corneal, pero su aplicación terapéutica es inviable debido a su corta semivida de eliminación y baja biodisponibilidad ocular. La terapia génica representa una nueva estrategia terapéutica, mediante la que se podría abordar el tratamiento de la queratitis de una manera efectiva y segura induciendo la producción de IL-10 en la córnea. En este sentido, las nanopartículas sólidas lipídicas (SLNs) son uno de los sistemas más prometedores tanto para la administración ocular de fármacos como para la preparación de vectores no virales para terapia génica. El objetivo de la presente tesis doctoral fue el diseño, desarrollo y evaluación in vitro, ex vivo e in vivo de vectores no virales basados en SLNs para el tratamiento de la inflamación corneal mediante terapia génica. Los sistemas se formularon en forma de colirio para inducir la producción de novo de la potente citoquina antiinflamatoria IL-10 en las célulascorneales, tras su administración por vía tópica. Se diseñaron y caracterizaron vectores formados por diferentes SLNs combinadas con ligandos, como protamina y polisacáridos. Se evaluaron in vitro en células humanas de epitelio corneal (HCE-2) y en un modelo ex vivo de córnea de conejo. Los vectores más eficaces se formularon en forma de colirio empleando como agente viscosizante alcohol polivinílico. Los sistemas lipídicos son los de uso más extendido debido a que presentan propiedades óptimas de biocompatibilidad y biodegradabilidad. Los liposomas fueron los primeros en ser utilizados como sistemas de administración de diferentes tipos de sustancias activas y son los más conocidos, pero actualmente existen otros tipos más avanzados como las nanopartículas sólidas lipídicas (SLN), los transportadores lipídicos nanoestructurados (NLC) y los conjugados de un lípido y un fármaco (LDC). Las nuevas generaciones de nanopartículas lipídicas se han ido desarrollando como respuesta a los problemas detectados en las estudiadas anteriormente. Existen numerosos métodos de preparación de nanopartículas lipídicas entre los cuales se debe seleccionar el más adecuado para cada caso. Por ello, en este trabajo se recogen las ventajas e inconvenientes de los principales métodos. Las nanopartículas lipídicas son estructuras coloidales complejas que requieren métodos especializados para su caracterización física y estructural, por lo que también se describen los principales parámetros a medir. Las características de los diferentes tipos de nanopartículas lipídicas les han permitido ser utilizadas como sistemas de administración de sustancias activas tales como fármacos poco solubles, péptidos y proteínas y ácidos nucleicos, además de aplicaciones cosméticas. Se trata de una estrategia en continuo desarrollo, ya que la formulación de nanopartículas lipídicas ha abierto nuevos horizontes para desarrollar estrategias innovadoras para la administración de sustancias activas que, hasta ahora, a pesar de su conocida actividad terapéutica, no habían encontrado un sistema de administración adecuado.

One of the main challenges in gene therapy is the issue of delivery, and it is especially relevant for the success of gene therapy in the cornea. In the present work, eye drops containing biocompatible non-viral vectors based on solid lipid nanoparticles (SLNs) as gene delivery systems to induce the expression of interleukin 10 (IL-10) were designed to address the treatment of corneal inflammation. Two kinds of SLNs combined with different ligands (protamine, dextran, or hyaluronic acid (HA)) and formulated with polyvinyl alcohol (PVA) were prepared. SLN-based vectors were characterized in terms of size, adhesiveness, viscosity, and pH, before topical administration to wild type and IL-10 knock out (KO) mice. The formulations showed a homogenous particle size below 400 nm and a positive surface charge to favor bioadhesion; the incorporation of PVA improved the corneal penetration. After three days of treatment by topical instillation, SLN-based vectors mainly transfected corneal epithelial cells, HA-formulations being the most effective ones. IL-10 was capable of reaching even the endothelial layer. Corneal sections showed no histological change and formulations seemed to be well tolerated after repeated topical administration. These promising results highlight the possible contribution of non-viral gene augmentation therapy to the future clinical approach of corneal gene therapy.

The use of messenger RNA (mRNA) in gene therapy is increasing in recent years, due to its unique features compared to plasmid DNA: Transient expression, no need to enter into the nucleus and no risk of insertional mutagenesis. Nevertheless, the clinical application of mRNA as a therapeutic tool is limited by its instability and ability to activate immune responses; hence, mRNA chemical modifications together with the design of suitable vehicles result essential. This manuscript includes a revision of the strategies employed to enhance in vitro transcribed (IVT) mRNA functionality and efficacy, including the optimization of its stability and translational efficiency, as well as the regulation of its immunostimulatory properties. An overview of the nanosystems designed to protect the mRNA and to overcome the intra and extracellular barriers for successful delivery is also included. Finally, the present and future applications of mRNA nanomedicines for immunization against infectious diseases and cancer, protein replacement, gene editing, and regenerative medicine are highlighted.

Gene silencing targeting proangiogenic factors have been shown to be a useful strategy in the treatment of corneal neovascularization (CNV). Among interference RNA (RNAi) molecules, short-hairpin RNA (shRNA) is a plasmid-coded RNA able to down-regulate the expression of the desired gene. It is continuously produced in the host cell, inducing a durable gene silencing effect. The aim of this work was to develop a solid lipid nanoparticle (SLN)-based shRNA delivery system to downregulate metalloproteinase 9 (MMP-9), a proangiogenic factor, in corneal cells for the treatment of CNV associated with inflammation. The nanovectors were prepared using a solvent emulsification-evaporation technique, and after physicochemical evaluation, they were evaluated in different culture cell models. Transfection efficacy, cell internalization, cell viability, the effect on MMP-9 expression, and cell migration were evaluated in human corneal epithelial cells (HCE-2). The inhibition of tube formation using human umbilical vein endothelial cells (HUVEC) was also assayed. The non-viral vectors based on SLN were able to downregulate the MMP-9 expression in HCE-2 cells via gene silencing, and, consequently, to inhibit cell migration and tube formation. These results demonstrate the potential of lipid nanoparticles as gene delivery systems for the treatment of CNV-associated inflammation by RNAi technology.
