The aim of study was to assess the safety and effectiveness of 3 different commercial iliac branch devices (IBDs): the Zenith Branch Iliac Endovascular Graft; the Gore Excluder Iliac Branch System and the E-liac Stent Graft System for the treatment of aorto-iliac or iliac aneurysms.

Venous aneurysms are long-term complications of arteriovenous fistula (AVF) for hemodialysis with an estimated incidence rate of around 5–6%. The purpose of our study is to investigate the role of immunosuppressive therapy in the development of AVF aneurysms in renal transplant patients, and to determine whether AVF closure following transplantation is necessary.

The aim of this study is to retrospectively analyze the early and long-term outcomes of endovascular treatment of Trans-Atlantic Inter-Society Consensus II class C and D (TASC II) aortoiliac occlusive disease with an expanded polytetrafluoroethylene–covered stent graft.

Ruptured mycotic aneurysms are an extremely rare complication of intravesical Bacillus Calmette-Guerin (BCG) immunotherapy. Several cases involving various arterial sites, mostly in the thoracic or abdominal aorta, have been described in the literature. BCG immunotherapy rarely causes false aneurysms and open surgical repair using an in situ prosthetic graft is most commonly performed. Further to this, targeted antituberculous treatment is required for at least one year following surgery.

The antiphospholipid syndrome (APS), either primary or secondary form, is considered an autoimmune disease with the presence of at least 1 clinical and 1 laboratory abnormalities as defined by the Sydney criteria. Clinical criteria include vascular thrombosis of venous, artery, small vessel in any organ, and recurrent pregnancy pathologies. Mesenteric ischemia is a rare and threatening manifestation of APS. We herein report a case of a 34-year-old pregnant woman referred to our Emergency Room with thoracic and abdominal acute pain. Her past medical history was remarkable for positivity to antiphospholipid antibodies, deep vein thrombosis of the right lower limb, chronic occlusion of celiac trunk, and superior mesenteric artery and recurrent abortions. Imaging revealed acute occlusion of inferior mesenteric artery (IMA). Both a surgical and endovascular thrombectomy were ineffective to obtain durable IMA patency and so the patient underwent antegrade aorta-inferior mesenteric bypass with saphenous vein and resection of an ischemic ileal loop. The medical treatment at discharge was lifelong oral anticoagulant associated with double antiplatelet therapy. To the best of our knowledge, this is the first case reporting a young pregnant woman with APS and mesenteric ischemia submitted to several attempts of revascularization. Aggressive oral anticoagulant and antiplatelet long-term therapy is advised. Moreover, we recommend strict follow-up in those patients in order to early diagnose thrombotic recurrence.

To assess the etiology, management and outcome of iliac pseudoaneurysms following renal transplantation.

The in vivo and ex vivo effects of macrophage colony-stimulating factor (M-CSF) were studied in a profoundly neutropenic rabbit model in order to determine its potential to augment pulmonary host defence againstAspergillus. M-CSF (100–600μg/kg/d) was administered prophylactically to neutropenic rabbits with pulmonary aspergillosis starting three days pre-inoculation and then throughout neutropenia. Rabbits receiving M-CSF had significantly increased survival (P=0.01) and decreased pulmonary injury, as measured by decreased pulmonary infarction (P=0.004), when compared with untreated controls. Microscopic studies demonstrated greater numbers of activated pulmonary alveolar macrophages (PAMs) in lung tissue of rabbits receiving M-CSF, in comparison to controls (P<0.001). PAMs harvested from rabbits treated with M-CSF had a significantly greater percent phagocytosis ofAspergillus fumigatusconidia than did PAMs from controls (P=0.04). These data indicate that prophylactic administration of M-CSF augments pulmonary host defence against A. fumigatusand suggest a potential role for this cytokine as adjunctive therapy in the treatment of pulmonary aspergillosis in the setting of profound neutropenia.

Enhanced burning of plasma‐ignited propellants has been deduced in recent years from closed bomb pressure traces. The question remains, however, whether the enhanced burning is inherent or possibly the result of propellant grain fracture. To find out, interrupted and non‐interrupted burning tests were conducted with cylindrical perforated grains of JA2 and M30 in a closed bomb with a loading density of approximately 0.2 g/cm3. Both, conventional black powder and plasma igniters were used, and a few tests were carried out with the propellants cooled to −20 °C. The plasma igniter was an ablating capillary, and the electrical energy density was about 0.7 kJ/g of propellant. The diameters of the collected grains yielded the actual burn distance at the time of the interrupted burning. The experimental pressure traces and the conventional burn rate coefficients of the propellants were used to calculate the theoretical depth burned assuming no plasma‐induced burn rate modifications. From overlapping pressure traces at several interrupted pressures, and from comparison to the calculated versus measured burn distances, it was found that there is burn rate enhancement during the plasma pulse–but not much once the pulse has ended. In contrast to the JA2 burn rates, both ambient and cold M30 burn rates, deduced from the non‐interrupted tests using the BRLCB code, were enhanced even after the plasma turn‐off, thus contradicting the interrupted‐tests results. However, vivacity analysis of the non‐interrupted tests indicated that the M30 grains exhibited increased surface area (possible fragmentation) because of the plasma interaction–an effect that would cause erroneous results from the BRLCB. Indeed, simulation of the non‐interrupted M30 tests using the XNOVAKTC code, and assuming partial fragmentation of the propellant charge, yielded vivacities that mimicked the experimental ones.

Most wrought aluminium alloys of the 2000 series are difficult to manufacture by laser powder bed fusion (L-PBF) due to the formation of cracks during building. To date, the effects of processing regimes on crack formation are still not well understood. In this study we performed a detailed microstructural characterisation of crack development in the AlCuMg alloy AA2024 to quantify the extent of cracking and porosity arising from a range of different process parameters. Two samples, produced with different build parameters, were selected for in-depth study by scanning and transmission electron microscopy; these had similar low levels of porosity but high (H - 2.6±0.4mm/mm2) and low (L - 1.5±0.3mm/mm2) crack densities respectively. Based on distinct morphological features and characteristic length, we differentiate hot cracks from solid state cracks. Hot tears form at high angle grain boundaries and are associated with micron-sized gas pores as well as intermetallic phases in both samples. The solidification and cracking behaviour are modelled with the aid of a Scheil-Gulliver model that includes solute trapping. This approach predicts differences in hot-crack-susceptibilities, due to different solidification velocities, in line with experimental observations. The sample H, of high crack density, also experiences the higher cooling rate, and hence strain rate, which contributes to the greater propagation of cold cracks in the low fracture toughness AA2024. The observation of cracking associated with microporosity and the use of a Scheil-based solidification model, including solute trapping, provide new insights into the complex problem of hot tearing during L-PBF.

Interleukin B (IL-B) (ref. 1), formerly termed BEF (B-cell-derived enhancing factor)2,3or IL-B4 (ref. 4), was originally described as a non-immunoglobulin regulatory factor spontaneously produced by B lymphocytes and B-cell lines that enhances the in vitro antigen-driven antibody response of unfractionated spleen cells stimulated by thymus-dependent antigens2,3. Since then we have examined the function of interleukin B in a number of immune reactions, both in vitro and in vivo5, and found that it inhibits the activation of suppressor T lymphocytes3,6–9. We report here the production of two monoclonal antibodies (mAb) that specifically inhibit interleukin B activity. The use of these mAb in the purification and characterization of IL-B is described. IL-B from both normal and transformed B cells consists of two subunits of similar size and amino-acid composition. The structure of interleukin B and its specific behaviour in biological assay distinguish it from many other known lymphokines.

To reduce operative mortality in elderly patients, a system of preoperative staging, based on invasive monitoring, was developed. All of the 148 consecutive patients studied had been cleared for surgery by standard assessment, but only 13.5% had normal measured, hemodynamic, respiratory, and oxygen transport function. Mild physiologic aberrations, not requiring a delay in surgery, or more severe abnormalities, indicative of high operative risk, were found in 63.5% of the patients. Advanced and incorrigible functional defects found in the remaining 23% made them unacceptable risks for major surgery under general anesthesia, and all who underwent the planned operation in spite of the warning died. Invasive preoperative assessment of elderly patients discloses a high percentage of serious physiologic abnormalities requiring a delay in some and cancellation of the operation in others.(JAMA 243:1350-1355, 1980)

Today the only investigation considered valid for the diagnosis of reactions to food is the oral stimulation test following a period of diet during which the suspected food item has been withdrawn. This test is, however, not without a certain risk, due to the possibility of the onset of an unexpected anaphylactic reaction. Researchers are therefore focusing attention on new, more reliable and less risky diagnostic techniques. The aim of this investigation was to evaluate, by means of ELISA, the serum levels of specific IgG and IgA antibodies and of their subclasses for casein, alpha lactalbumin, bovine beta-lactoglobulin and ovalbumin in 72 children with gastointestinal disorders or atopic dermatitis due to food. The diagnosis of food intolerance was made following a double blind oral stimulation test: 16 healthy age-matched children were used as controls. The results of the investigation demonstrated that specific IgA levels for all the food antigens were statistically higher compared with those of controls, irrespective of the clinical features, whereas the IgG class antibodies were statistically higher only for casein in the children with atopic dermatitis. Studies on the antibody subclasses, showed a statistically significant increase particulary of IgG, but also of IgA1, especially in children with gastointestinal disorders. In conclusion, the results show that total specific IgA may play a role, although modest, in the diagnosis of food intolerance. The IgG subclasses, and to a lesser extent the IgA subclasses, appear to be useful in the diagnostic work-up, particularly in children with gastrointestinal symptoms.

This article describes an invasive hemodynamic monitoring method that evaluates actual functional abnormalities in the surgical patient and quantitates them in the preoperative period. The various modalities of re-establishing physiologic equilibrium to an optimal state are discussed as well as an elaboration of risk assessment as applied to subspecialty surgical areas.

Abstract:  Previous studies have defined two different peptide binding motifs specific for HLA-A * 0101. These motifs are characterized by the presence of tyrosine (Y) at the C-termini of 9-mer and 10-mer peptides, and either a small polar or hydrophobic (S, T, M) residue in position 2, or a negatively charged (D or E) residue in position 3. In this study, the structural requirements for peptide binding to A * 0101 have been further analyzed by examining the binding capacity of large sets of peptides corresponding to naturally occurring sequences which bore one or the other of these two A * 0101-specific motifs. By correlating the presence of specific residue types at each position along the peptide sequence with increased (or decreased) binding affinity, the prominent influence of secondary anchor residues was revealed. In most cases, the two anchors in positions 2 and 3 appear to act synergistically. With the exception of the DE3 submotif in 9-mer peptides, a positive role for aromatic residues in position 1 and the center of the peptide (positions 4 or 5 of 9- or 10-mer peptides, respectively), and proline at C-3, were also consistently detected. However, secondary anchor residues also appear to differ significantly between the two different submotifs, demonstrating that A * 0101 can utilize alternative modes in binding its peptide ligands. According to these analyses, specific refined submotifs were also established, and their merit verified by independent sets of potential A * 0101 binding peptides. Besides providing useful insight into the nature of the interaction of the A * 0101 allele with its peptide ligands, such refined motifs should also facilitate accurate prediction of potential A * 0101-restricted peptide epitopes.
