Abstract

Introduction: A non-interventional, multicenter, European, prospective evaluation of the effectiveness, tolerability, and safety of a topical preservative-free tafluprost (0.0015%) and timolol (0.5%) fixed-dose combination (PF tafluprost/timolol FC) in adults with open-angle glaucoma (OAG) and ocular hypertension (OHT) demonstrating insufficient response to topical beta-receptor blockers or prostaglandin analogue (PGA) monotherapy. Methods: Mean intraocular pressure (IOP) change from baseline was measured at study visits following a switch to PF tafluprost/timolol FC. Primary endpoint was absolute mean IOP change at month 6. Change from baseline concerning ocular signs and symptoms was also explored. Results: Analyses included 577 patients (59.6% female). Mean age (SD) was 67.8 (11.67) years. Mean (SD) IOP reduction from baseline was significant at all study visits; 5.4 (3.76) mmHg (23.7%) at week 4, 5.9 (3.90) mmHg (25.6%) at week 12, and 5.7 (4.11) mmHg (24.9%) at month 6 (p < 0.0001 for all visits). At month 6, 69.2%, 53.6%, 40.0%, and 25.8% were responders based on ≥ 20%, ≥ 25%, ≥ 30%, and ≥ 35% cutoff values for mean IOP, respectively. Significant reductions were observed concerning corneal fluorescein staining (p < 0.0001), dry eye symptoms, irritation, itching, and foreign body sensation (p < 0.001 for each parameter). Conjunctival hyperemia was significantly reduced at all study visits (p < 0.0001 at each visit). Overall, 69 treatment-related adverse events (AEs) were reported, one of which was serious (status asthmaticus). Most AEs were mild to moderate in severity, and the majority had resolved or were resolving at the end of the study period. Conclusion: In clinical practice, PF tafluprost/timolol FC provided statistically and clinically significant IOP reductions in patients with OAG and OHT insufficiently controlled on or intolerant to PGA or beta-receptor blocker monotherapy. The full IOP reduction appeared at week 4 and was maintained over the 6-month study period. Key symptoms of ocular surface health improved. Abstract

The authors would like to acknowledge missing data for all prior latanoprost users as a whole. The following sentence has been added to the manuscript (page 8, first paragraph): Mean (SD) reduction from baseline at Month 6 for users of all latanoprost formulations (preserved and PF) was 6.1 (4.25) mmHg (25.9%; p\0.0001). Abstract

Introduction: The VISIONARY study demonstrated statistically significant intraocular pressure (IOP) reductions with the preservative-free fixed-dose combination of tafluprost 0.0015% and timolol 0.5% (PF tafluprost/timolol FC) in open-angle glaucoma (OAG) or ocular hypertension (OHT) patients, sub-optimally controlled with topical prostaglandin analogue (PGA) or beta-blocker monotherapy. Current subanalyses have examined these data according to the baseline monotherapy. Methods: A European, prospective, observational study included adults (aged ≥ 18 years) with OAG or OHT, who were switched to the PF tafluprost/timolol FC from PGA or beta-blocker monotherapy. Treatment outcomes were reported according to prior monotherapy subgroup: beta-blocker, preserved latanoprost, PF-latanoprost, bimatoprost, tafluprost, and travoprost. Endpoints included the mean change from baseline regarding IOP, conjunctival hyperemia, and corneal fluorescein staining (CFS) at Week 4 and Week 12, and at Month 6. 5. Strategies to Reduce Oxidative Stress in Glaucoma Patients

Abstract

Background: Primary open-angle glaucoma (POAG) is a multifactorial pathology involving a variety of pathogenic mechanisms, including oxidative/nitrosative stress. This latter is the consequence of the imbalance between excessive formation and insufficient protection against reactive oxygen/nitrogen species. Objective: Our main goal is to gather molecular information to better managing pathologic variants that may determine the individual susceptibility to oxidative/nitrosative stress (OS/NS) and POAG. Method: An extensive search of the scientific literature was conducted using PUBMED, the Web of Science, the Cochrane Library, and other references on the topic of POAG and OS/NS from human and animal model studies published between 2010 and 2017. Finally, 152 works containing relevant information that may help understanding the role of antioxidants, essential fatty acids, natural compounds and other similar strategies for counteracting OS/NS in POAG were considered. Results: A wide variety of studies have proven that antioxidants, among them vitamins B-3, C and E, Coenzyme Q10 or melatonin, omega-3/omega-6 fatty acids and other natural compounds (such as coffee, green tea, bear bile, gingko biloba, coleus, tropical fruits, etc.,) may help regulating the intraocular pressure as well as protecting the retinal neurons against OS/NS in POAG. Conclusion: Based on the impact of antioxidants and omega-3/omega-6 fatty acids at the molecular level in the glaucomatous anterior and posterior eye segments, further studies are needed by integrating all issues involved in glaucoma pathogenesis, endogenous and exogenous risk factors and their interactions that will allow us to reach newer effective biotherapies for preventing glaucomatous irreversible blindness.

Abstract

Objective: To evaluate the effects of intraocular lidocaine hydrochloride (HCI) on the corneal endothelium and of blood-aqueous barrier (BAB) permeability in ultrasound phacoemulsification., Design: Nonrandomized, comparative trial., Participants: Sixty patients who underwent uneventful, single-surgeon, clear cornea phacoemulsification were studied prospectively., Methods: Sub-Tenon's anesthesia was administered to 30 patients (group 1), and intraocular lidocaine hydrochloride was administered to an additional 30 patients (group 2). An endothelial study of at least 120 cells per patient, using a noncontact specular microscope and a digital image analysis system, was performed before surgery and 1 month after surgery. Blood-aqueous barrier permeability was evaluated preoperatively and postoperatively (48 hours, 1 week and 1 month) using a laser flare meter., Main Outcome Measures: Endothelial parameters (cell density, hexagonality, and coefficient of variation in cell area) and BAB permeability., Results: No significant differences were found between groups when comparing the postoperative changes produced in endothelial cell density (95% confidence interval [CI], 0.46, 4.7; P = 0.10), hexagonality (95% CI, -2.5, 3.5; P = 0.72), and coefficient of variation in cell area (95% CI, -8.3, 4.6; P = 0.57). A multivariate study detected no significant differences in mean flare values between groups during follow-up (P = 0.40). No clinically significant differences were found between mean preoperative and 1 month postoperative flare values in either group., Conclusions: The recovery of BAB permeability 1 month after surgery seems to suggest that no additional inflammation was induced by intraocular lidocaine HCI. The fact that no differences in postoperative changes in endothelial parameters were found between groups indicates that no further corneal endothelial damage (other than that caused by surgical manipulation) was incurred. According to our results, intracameral lidocaine HCI appears to be safe for the average phacoemulsification patient in the absence of ocular pathologic conditions.

Abstract

Since 1992, when our department acquired a flexible fiberscope with a working channel, we have used it in surgery on the upper respiratory and digestive tract. We call our technique laryngeal fiberscopic surgery. We report the results of four years of experience in this field. A total of 368 surgical interventions were performed, almost half to remove benign lesions of the vocal cords (polyps, nodules, etc). This technique is a major advance in our field of study. Its main advantages are avoidance of general anesthesia and hospitalization, reduction of surgical risk, and savings of time and money. We consider it to be a useful alternative to traditional direct laryngoscopy (Kleinsasser technique) for many applications.

Abstract

A case is presented of cervical sarcomatoid carcinoma that was initially confused with malignant fibrohistiocytoma. The similarity in their histological features and compatible immunohistochemical tests make it necessary to include sarcomatoid carcinoma in the differential diagnosis malignant fibrohistiocytoma. Many atypical presentations of malignant fibrohistiocytoma may be true sarcomatoid carcinomas. Our case is special because the patient had a recent spinocellular carcinoma of the lip, in which the undifferentiated portion had originated lymph node metastasis.

Abstract

Thygeson's superficial punctate keratitis (Thygeson's SPK) is a distinct clinical entity, characterized by round conglomerates of discrete, granular, white-gray, fine intraepithelial dots without conjunctival involvement. The only effective treatment with regard to relieving symptoms and diminishing lesions has been topical corticosteroids, but their prolonged use can be associated with severe side-effects. The purpose of this study is to present the long-term results of the use of 2% topical cyclosporin A in olive oil in Thygeson's SPK. Eight patients diagnosed as having Thygeson's SPK were included. All the patients were treated with 2% cyclosporin dissolved in olive oil four times a day for three months, and two times a day for one month before withdrawing therapy. The follow-up period ranged from twelve to twenty-five months. The number of corneal lesions varied between 5 and 15 before treatment. After cyclosporin treatment, no corneal lesion was observed and the cornea remained clear after the follow-up period. In conclusion, 2% cyclosporin in olive oil is a safe alternative to corticosteroids in the treatment of Thygeson's SPK, and resulted in satisfactory control of the condition.

We present 4 cases of adenoid cystic carcinoma from different locations (parotid, submaxillary, larynx and auditory duct) and we underline the supraglottic origin of one of them. We point out the good clinical behaviour of this carcinoma, with a long tested survival, more than 5 years, in 2 of our cases. All of them showed morphological features independently of the sitting.

Abstract

We have studied a 67 years old woman suffering from a 40 years evolution Pleomorphic adenoma. The specimen weighed 250 g, and showed several areas displaying carcinoma. This report confirms the relationship between long-term evolution and malignant transformation.
