Treating RA to the target of remission is current common practice. Cost-effectiveness of different treat-to-target (T2T) strategies, especially ones including new treatments is important for development and preference policy for treatment centers. European League Against Rheumatism (EULAR) and American College of Rheumatology (ACR) guidelines are currently unclear about preference between a JAK1/JAK2 versus a biological disease-modifying antirheumatic drug (bDMARD).Objective: The main goal of this paper was to evaluate the cost-effectiveness of baricitinib versus first biological for methotrexate inadequate responders in a T2T strategy using a Markov model that incorporates hospital costs as well as societal costs. Costs and utilities over five years were compared between the two strategies.Methods: A Monte Carlo simulation model was developed to conduct cost–utility analysis from the societal perspective over 5 years. Health states were based on the DAS28-erythrocyte sedimentation rate (ESR) categories. Effectiveness of baricitinib was retrieved from randomized controlled trials. Effectiveness of all other treatments, health state utilities, medical costs, and productivity loss were retrieved from the Dutch RhEumatoid Arthritis Monitoring (DREAM) cohorts. Annual discount rates of 1.5% for utility and 4% for costs were used. Probabilistic sensitivity analysis was employed to incorporate uncertainty and assess robustness of the results.Results: Probabilistic sensitivity analysis results showed the baricitinib strategy yielded lower costs and higher utility over a 5-year period. Scenario analyses showed the baricitinib strategy to be cost-effective in both the moderate and severe RA populations.Conclusion: Results suggest that the use of a JAK1/JAK2 inhibitor instead of a bDMARD in a T2T approach is cost-effective in csDMARD refractory RA patients.Keywords: Markov model, rheumatoid arthritis, baricitinib, cost-effectiveness, health economic model, treat-to-target

Abstract

We present the star cluster catalogs for 17 dwarf and irregular galaxies in the $HST$ Treasury Program "Legacy ExtraGalactic UV Survey" (LEGUS). Cluster identification and photometry in this subsample are similar to that of the entire LEGUS sample, but special methods were developed to provide robust catalogs with accurate fluxes due to low cluster statistics. The colors and ages are largely consistent for two widely used aperture corrections, but a significant fraction of the clusters are more compact than the average training cluster. However, the ensemble luminosity, mass, and age distributions are consistent suggesting that the systematics between the two methods are less than the random errors. When compared with the clusters from previous dwarf galaxy samples, we find that the LEGUS catalogs are more complete and provide more accurate total fluxes. Combining all clusters into a composite dwarf galaxy, we find that the luminosity and mass functions can be described by a power law with the canonical index of $-2$ independent of age and global SFR binning. The age distribution declines as a power law, with an index of $\approx-0.80\pm0.15$, independent of cluster mass and global SFR binning. This decline of clusters is dominated by cluster disruption since the combined star formation histories and integrated-light SFRs are both approximately constant over the last few hundred Myr. Finally, we find little evidence for an upper-mass cutoff ($<2\sigma$) in the composite cluster mass function, and can rule out a truncation mass below $\approx10^{4.5}$M$_{\odot}$ but cannot rule out the existence of a truncation at higher masses., Comment: 25 pages; 28 figures (Accepted to MNRAS)

Abstract

The small-signal equivalent circuit of GaN-based self-switching diodes has been obtained, which apart from the intrinsic R ‖ C branch, generally used to describe the diode performance, needs new elements to describe the low-frequency dispersion of the impedance originated by the presence of surface and bulk traps. The proposed model allows us to reproduce not only the high-frequency results (extracted from S-parameter measurements in the 40 MHz–43.5 GHz range) at room temperature, but also the low-frequency impedance measurements (75 kHz–30 MHz) at cryogenic temperatures down to 70 K. These new elements are a self-inductance associated to the effect of surface states (typical of a device with a high surface-to-volume ratio) and an extra series R – C branch modeling the influence of the bulk traps. Abstract

In this paper, we report an analysis of reverse current mechanisms observed in GaN Schottky barrier diodes leading to hysteretic behavior of the I–V curves at low temperature. By means of DC measurements from 33 to 475 K, we demonstrate the presence of two leakage mechanisms when comparing the experiments with the results obtained using a unified model to predict the ideal reverse current of the diode. Poole–Frenkel emission is the dominant mechanism for temperatures above 200 K, while trap-assisted tunneling prevails for lower temperatures, where also, hysteresis cycles are revealed by means of DC dual-sweep voltage measurements. The energy of the corresponding traps has also been determined, being around 0.2 and 0.45 eV, respectively. The hysteresis phenomenon is attributed to the bias-induced occupancy of the energy states originating the leakage-current processes, which leads to the reduction of the reverse current after a high negative voltage is applied to the diode. Abstract

Impact ionization originated by the buffer leakage current, together with high electric fields ( > 3 MV cm−1) at the anode corner of the isolating trenches, has been identified as the failure mechanism of shaped planar GaN Gunn diodes when biased above 20 V, so that no evidence of Gunn oscillations in fabricated devices has been observed yet. In order to avoid the avalanche, we propose the addition of a Schottky substrate terminal, which, by means of Monte Carlo simulations, has been confirmed to be able to suppress such not-desired leakage current when applying a negative substrate bias. When the substrate bias is positive, impact ionization is also reduced due to the lower electric field at the hotspot, but a vertical cathode-substrate current degrades the device operation. In order to avoid such current, we propose the use a MIS configuration for the substrate contact, which is the optimal solution. [ABSTRACT FROM AUTHOR]

Abstract

We report on the measurements of the two main figures of merit for microwave detection, namely, responsivity and noise equivalent power (NEP), in HEMTs based on two of the most broadly used material systems AlGaN/GaN and AlInAs/InGaAs. A comparison between their performances as RF detectors in a wide temperature range is provided by means of experiments made under probes with direct connection to the drain contact. InGaAs HEMTs exhibit much better responsivity and NEP, which are further improved when lowering the working temperature. Moreover, we analyze the possibility of optimizing the current-mode detection of the transistors by improving the impedance matching conditions through an adequate choice of the device width W. Abstract

Introduction: Baricitinib, a Janus kinase (JAK) 1/2 inhibitor, is an approved treatment for rheumatoid arthritis (RA), atopic dermatitis (AD), and alopecia areata (AA). Further characterisation of adverse events of special interest (AESI) for JAK inhibitors in at-risk populations will improve benefit-risk assessment for individual patients and diseases. Methods: Data were pooled from clinical trials and long-term extensions in moderate-to-severe active RA, moderate-to-severe AD, and severe AA. Incidence rates (IR) per 100 patient-years of major adverse cardiovascular event (MACE), malignancy, venous thromboembolism (VTE), serious infection, and mortality were calculated for patients with low risk (younger than 65 years with no specified risk factors), and patients at risk (≥1 of: aged 65 years or older, atherosclerotic cardiovascular disease, diabetes mellitus, hypertension, current smoking, HDL cholesterol Results: Datasets included baricitinib exposure up to 9.3 years with 14744 person-years of exposure (PYE) (RA), 3.9 years with 4628 PYE (AD), and 3.1 years with 1868 PYE (AA). In patients with low risk (RA: 31%, AD: 48%, AA: 49%), IRs for MACE (0.05, 0.04, 0), malignancies (0.20, 0.13, 0), VTE (0.09, 0.04, 0), serious infection (1.73, 1.18, 0.6), and mortality (0.04, 0, 0) in the RA, AD, and AA datasets, respectively, were low. In patients at risk (RA: 69%, AD: 52%, AA: 51%), IRs were for MACE (0.70, 0.25, 0.10), malignancies (1.23, 0.45, 0.31), VTE (0.66, 0.12, 0.10), serious infection (2.95, 2.30, 1.05), and mortality (0.78, 0.16, 0) for RA, AD, and AA datasets, respectively. Conclusion: Populations with low risk have low incidence of the examined JAK inhibitor-related AESI. In the dermatologic indications, incidence is also low for patients at risk. Considering individual disease burden, risk factors, and response to treatment is relevant to make informed decisions for individual patients treated with baricitinib.

Abstract

In this paper, the occupancy of sidewall surface states having a clear signature in the performance of AlGaN/GaN-based self-switching diodes (SSDs) is analyzed using a semi-classical Monte Carlo (MC) simulator in a wide temperature (T) range, from 100 to 300 K. Experimental I – V curves show an unusual current decrease at low temperature attributed to surface trapping. The dependence on T of the negative surface charge density σ at the etched sidewalls of the SSDs is essential to explain the measurements. Two devices with different widths (80 and 150 nm) have been characterized and simulated in detail paying especial attention to the modeling of the surface states. At room temperature, MC simulations with a position-independent value of σ are able to qualitatively reproduce the I – V curves. However, a more complex approach is required to correctly replicate the values and shape of the DC experimental curves at low temperature, below 220 K. An algorithm where σ depends not only on T but also on the applied bias V is proposed to successfully fit the current values at every bias point. The model is able to explain the physics of the unexpected dependence of the resistance with the channel width and the sign change in the bowing coefficient, the parameters that govern the detection capabilities of the diodes. Abstract

ObjectiveCOVID-19 vaccines have a favorable safety profile in patients with autoimmune rheumatic diseases (AIRDs) such as idiopathic inflammatory myopathies (IIMs); however, hesitancy continues to persist among these patients. Therefore, we studied the prevalence, predictors and reasons for hesitancy in patients with IIMs, other AIRDs, non-rheumatic autoimmune diseases (nrAIDs) and healthy controls (HCs), using data from the two international COVID-19 Vaccination in Autoimmune Diseases (COVAD) e-surveys.MethodsThe first and second COVAD patient self-reported e-surveys were circulated from March to December 2021, and February to June 2022 (ongoing). We collected data on demographics, comorbidities, COVID-19 infection and vaccination history, reasons for hesitancy, and patient reported outcomes. Predictors of hesitancy were analysed using regression models in different groups.ResultsWe analysed data from 18 882 (COVAD-1) and 7666 (COVAD-2) respondents. Reassuringly, hesitancy decreased from 2021 (16.5%) to 2022 (5.1%) (OR: 0.26; 95% CI: 0.24, 0.30, P ConclusionVaccine hesitancy has decreased from 2021 to 2022, long-term safety concerns remain among patients with IIMs, particularly in Caucasians and those with poor physical function.

Abstract

Two-terminal devices based on an AlGaN/GaN nanochannel exhibit a significant hysteresis in their current–voltage curve due to their large surface-to-volume ratio. Surface effects at the sidewalls of the trenches defining the nanochannel are the key for such behavior. By applying large positive/negative voltages, it is possible to trap/de-trap electrons in/from the surface states, thus modulating the conductance of the channel. In this work, the characteristic times of the involved slow processes (of the order of tens or hundreds of s) are studied from 70 up to 200 K, and the possible memory applications due to the charge retention are explored. 50. Sr isotope stratigraphy across the Paasivaara PGE reef of the Penikat intrusion, Northern Finland:insights into the genesis of reef-type PGE mineralization

Layered mafic-ultramafic intrusions host the largest reserves of feasible PGE commodities. Worldwide rare in the geological record, these deposits are well-constrained in a few large igneous provinces. Despite extensive research on layered intrusions and related PGE ores, the models regarding the PGE ore-formation are diverse, involving both magmatic and hydrothermal processes, and the subject remains largely debatable. In Europe, research efforts around the Baltic Large Igneous Province (BLIP) has doubled due to the new geopolitical strategy of the European Union in their search for new sources of critical metals. Global politics have placed Finnish mafic-ultramafic layered intrusions in the spotlight. Penikat is a Paleoproterozoic 2.44 Ga layered intrusion part of the east-west trending Tornio-Näränkävaara belt in northern Finland. The exposed intrusion is 23 km long and 1.5 km to 3.5 km on the current erosional level. Based on seismic and gravimetric data, it extends to 2.5 km deep. The intrussion is split into five westward-dipping blocks: Sompujärvi, Kilkka, Yli-Penikka, Keski-Penikka, and Ala-Penikka. Traditionally, Penikat is divided into five megacyclic units (MCU I-MCU V), hosting three main PGE reef horizons: the Sompujärvi Reef (SJ), located to the basal part of MCU IV; the Ala-Penikka Reef (AP), occurring within the lower proportion of unit IV; and the Paasivaara Reef (PV), which defines the hanging wall contact from the MCU IV to the MCU V. The present study comprises 700 m of stratigraphy in the Ala-Penikka block and focuses on the Paasivaara reef (PV). This platinum-enriched reef appears at the Transition Zone (TZ), along the MCU IV-V boundary. To gain new insight into Penikat’s petrogenesis, we utilize fourteen thick section samples from historic drill cores, both intersecting the PV reef, and two additional samples made after hand specimens. The sections were reviewed using petrologic microscopy before subjecting them to geochemical analyses. From these sections, we obtained new in-situ Sr isotope data in plagioclase grains by using the LA-MC-ICP-MS laser ablation technique at the GTK facilities in Espoo, Finland. The procedure can track sub-grain scale heterogeneities from core to rim. As radiogenic ratios are rarely affected by melting or crystallization, subgrain analyses are a powerful tool for tracing the contributors in a magmatic system. In addition, EPMA was used to analyze the plagioclase composition, with dedicated attention to the anorthite content in the assayed grains, at the University of Oulu, Finland. The anorthite content analysis was then compared to the LA-MC-ICP-MS results to assess the magmatic differentiation undergone by the plagioclase crystals. The results support crystallization stability in most of MCU IV (Sr(i) ~ 0.7027 ± 0.0005) and MCU V (Sr(i) 0.7032 ± 0.0005 ). Nonetheless, data record a systematic, gradual increase in radiogenic Sr signature in the sparsely 38 m between MCU IV and V. In addition, rim to core distributions register more than a single Sr(i) signature at a subgrain scale in TZ samples. The variability in the Sr signature at the Transition Zone converges with a sharp An86 content spike at the PV reef boundary, arguing for a primitive magma contributor. The results confirm affinity between the SHMB type magma, related to plume magmatism, and the parental magma of Penikat. The Transition Zone would be a product of binary and asymmetric magma mixing, each with contrasting isotopic lineages and varying degrees of crustal contamination. The discussion supports magma ascension from a staging chamber as a likely origin of the PV reef, thus supporting the new magma pulse hypothesis. Finally, the present study is compared with previous Sr(i) stratigraphy in Bushveld, showing similarities between the UPZ-MZ transition and the Transition Zone in Penikat. The detection of similar Sr(i) may lead to new PGE discoveries in the future.
