Abstract Psoriasis is a chronic skin disease that negatively impacts on patient’s life. A holistic approach integrating well-being assessment could improve disease management. Since a consensus definition of well-being in psoriasis is not available, we aim to achieve a multidisciplinary consensus on well-being definition and its components. A literature review and consultation with psoriasis patients facilitated the design of a two-round Delphi questionnaire targeting healthcare professionals and psoriasis patients. A total of 261 panellists (65.1% patients with psoriasis, 34.9% healthcare professionals) agreed on the dimensions and components that should integrate the concept of well-being: emotional dimension (78.9%) [stress (83.9%), mood disturbance (85.1%), body image (83.9%), stigma/shame (75.1%), self-esteem (77.4%) and coping/resilience (81.2%)], physical dimension (82.0%) [sleep quality (81.6%), pain/discomfort (80.8%), itching (83.5%), extracutaneous manifestations (82.8%), lesions in visible areas (84.3%), lesions in functional areas (85.8%), and sex life (78.2%)], social dimension (79.5%) [social relationships (80.8%), leisure/recreational activities (80.3%), support from family/friends (76.6%) and work/academic life (76.5%)], and satisfaction with disease management (78.5%) [treatment (78.2%), information received (75.6%) and medical care provided by the dermatologist (80.1%)]. This well-being definition reflects patients’ needs and concerns. Therefore, addressing them in psoriasis will optimise management, contributing to better outcomes and restoring normalcy to the patient’s life.

Resumen: La gripe es una amenaza constante que en ocasiones puede producir cuadros clínicos graves. Las mayores tasas de incidencia por grupos de edad ocurren en los menores de 15 años, especialmente en los menores de 5 años, en los que además las tasas de hospitalización son similares a las de las personas de 65 años en adelante. Además, los niños son los principales vectores de transmisión de la enfermedad. En España están autorizadas cinco vacunas antigripales en la edad pediátrica: tres inactivadas tetravalentes procedentes de cultivos en huevos embrionados, una inactivada tetravalente procedente de cultivos celulares, y una atenuada tetravalente de administración intranasal, que para la temporada 2024-2025 pasará a ser trivalente, al no incluir el linaje B Yamagata, como recomienda la OMS. El CAV-AEP recomienda la vacunación sistemática en: niños de 6 a 59 meses, niños y adolescentes pertenecientes a grupos de riesgo, personas que puedan transmitir la gripe a los grupos que tienen riesgo de gripe complicada, y convivientes o entorno familiar de menores de 6 meses. A partir de los 2 años de edad se recomienda preferentemente la vacuna atenuada intranasal por su mayor aceptabilidad y por facilitar la mejora de las coberturas. El CAV-AEP considera también que la vacunación antigripal de niños y adolescentes de 5-18 años sanos es una medida recomendable, por cuanto les proporciona protección individual y favorece la protección familiar y comunitaria. Es especialmente importante la vacunación antigripal de todos los profesionales sanitarios y la vacunación de las gestantes en cualquier momento del embarazo. Abstract: The flu is a constant threat that can sometimes cause severe forms of disease. The highest incidence rates by age group occur in children under 15 years of age, especially in those under 5 years, in whom the rate of hospitalization is also similar to the population aged 65 years and older. In addition, children are the main transmitters of the infection. In Spain, five influenza vaccines are authorized for the paediatric age group: three inactivated tetravalent vaccines harvested from fertilised eggs, one tetravalent inactivated vaccine obtained from cell cultures, and one attenuated tetravalent vaccine for intranasal administration, which will become trivalent in the 2024-2025 season by excluding the B Yamagata lineage as recommended by the WHO. The CAV-AEP recommends systematic vaccination in children aged 6 to 59 months, children and adolescents belonging to risk groups, people who can transmit the flu to groups at risk of complicated flu, and household contacts or close family of infants under 6 months. From 2 years of age, the intranasal attenuated vaccine is preferred due to its greater acceptability and thus contribution to greater vaccination coverage. The CAV-AEP also considers that vaccination against influenza of healthy children and adolescents aged 5 to 18 years is advisable, as it provides individual protection and promotes protection at the family and community levels. It is especially important to vaccinate all health care professionals against influenza as well as pregnant women at any time during the pregnancy.

Urban habitats represent new opportunities to study natural processes such as ecological succession. Our work focused for the first time on the changes in ecosystem services occurring in the ecological succession from annual to perennial grasslands in Mediterranean peri-urban greenspaces. Student’s t test was used to analyse the influence of the grassland type on plant and soil features, and showed that soil under perennial grassland had a significantly higher C content and arylamidase and arylsulfatase activity than annual grasslands. In contrast, annual grasslands showed a significantly higher plant-species richness, and their soils had higher bulk density and phenoloxidase and arylamidase activity compared to perennial grasslands. Neither the labile nor recalcitrant fractions of the organic matter showed any significant difference between communities. When all the soil factors were included together, Redundancy Analysis revealed a significant gradient in soil phenoloxidase activity and organic matter distinguishing perennial from annual grasslands. We conclude that when annual grasslands give way to perennial grasslands through natural succession in Mediterranean greenspaces, certain ecosystem services such as soil carbon storage and water regulation improve, while biodiversity maintenance declines. Thus, Mediterranean urban greenspaces where natural succession occurs should be handled in such a way as to preserve both type of habitats in order to improve a wider range of ecosystem services.

ABSTRACTSince the introduction of Haemophilus Influenzae type b (Hib) conjugate vaccines, invasive Hib disease has strongly declined worldwide, yet continued control of Hib disease remains important. In Europe, currently three different hexavalent combination vaccines containing Hib conjugates are marketed. In this phase IV, single-blind, randomized, controlled, multi-country study (NCT04535037), we aimed to compare, in a 2 + 1 vaccination schedule, the immunogenicity and safety and show non-inferiority, as well as superiority, of DTPa-HBV-IPV/Hib (Ih group) versus DTaP5-HB-IPV-Hib (Va group) in terms of anti-polyribosylribitol phosphate (PRP) antibody geometric mean concentrations (GMCs) and proportion of participants reaching anti-PRP antibody concentrations greater than or equal to a threshold of 5 µg/mL. One month after the booster vaccination, the anti-PRP antibody GMC ratio (Ih group/Va group) was 0.917 (95% CI: 0.710–1.185), meeting the non-inferiority criteria. The difference in percentage of participants (Ih group – Va group) reaching GMCs ≥5 µg/mL was -6.3% (95% CI: -14.1% to 1.5%), not reaching the predefined non-inferiority threshold. Interestingly, a slightly higher post-booster antibody avidity was observed in the Ih group versus the Va group. Both vaccines were well tolerated, and no safety concerns were raised. This study illustrates the different kinetics of the anti-PRP antibody response post-primary and post-booster using the two vaccines containing different Hib conjugates and indicates a potential differential impact of concomitant vaccinations on the anti-PRP responses. The clinical implications of these differences should be further studied.

AbstractBackground Determining tumor necrosis factor-alpha inhibitors (anti-TNF-α) failure is still a challenge in the management of moderate-to-severe psoriasis. Thus, our comprehensive systematic literature review aimed to gather information on the criteria used to define anti-TNF-α failure. We also aimed to discover the main reasons for anti-TNF-α failure and define subsequently administered treatments.Materials and methods We conducted a systematic review following review and reporting guidelines (Cochrane and PRISMA). International (Medline/PubMed and Cochrane Library) and Spanish databases (MEDES, IBECS), and gray literature were consulted to identify publications issued until April 2021 in English or Spanish.Results Our search yielded 58 publications. Of these, 37 (63.8%) described the criteria used to define anti-TNF-α primary or secondary failure. Criteria varied across studies, although around 60% considered Psoriasis Area and Severity Index (PASI)-50 criteria. Nineteen (32.8%) reported the reasons for treatment failure, including the lack or loss of efficacy and safety-related problems, mainly infections. Finally, 29 (50%) publications outlined the treatments administered after anti-TNF-α: 62.5% reported a switch to another anti-TNF-α and 37.5% to interleukin (IL)-inhibitors.Conclusion Our findings suggest a need to standardize the management of anti-TNF-α failure and reflect the incorporation of new targets, such as IL-inhibitors, in the treatment sequence.KEY MESSAGESIn the treatment of psoriasis, the primary and secondary anti-TNF-α failure criteria differ widely in the scientific literature.The strictest efficacy criteria for defining anti-TNF-α failure, or those recommended by guidelines such as PASI75, were underused both in clinical trials and observational studies.Most studies failed to consider patient-reported outcomes in assessing psoriasis treatment efficacy, which contrasts with recent recommendations on the inclusion of patient-reported HRQoL as a supporting criterion when considering clinical outcomes.

During pregnancy, controlling nutrition is crucial for the health of both mother and foetus. While polyphenols have positive health effects, some studies show harmful outcomes during pregnancy. This study evaluated polyphenol intake in a cohort of mother–child pairs and examined its effects on foetal anthropometric parameters. Polyphenol intake was assessed using food frequency questionnaires (FFQs) and 24-h dietary recalls, and analysed with the Phenol-Explorer database. Gestational age and birth measurements were retrieved from medical records. Statistical analyses validated dietary records and assessed polyphenol impact using multivariate generalised linear models. The study found that mean gestational age was 39.6 weeks, with a mean birth weight of 3.33 kg. Mean total polyphenol intake by FFQ was 2231 mg/day, slightly higher than 24-h recall data. Flavonoids and phenolic acids constituted 52% and 37% of intake, respectively, with fruits and legumes as primary sources. This study highlights the use of FFQs to estimate polyphenol intake. Furthermore, the study found associations between polyphenol consumption and anthropometric parameters at birth, with the effects varying depending on the type of polyphenol. However, a more precise evaluation of individual polyphenol intake is necessary to determine whether the effects they produce during pregnancy may be harmful or beneficial for foetal growth.

Several possible mpox reinfections have been reported, however, the debate on whether these are confirmed reinfections remains open.A 30-year-old male living with HIV and a history of single-dose mpox vaccination, first diagnosed with mpox in September 2022, presented with genital ulcers in March 2023, testing positive for mpox virus. Real-time polymerase chain reaction revealed the presence of viral DNA with cycle threshold values of 24 and 25, respectively. Whole genome sequencing and phylogenetic approach allowed us to classify these viruses as Clade IIb lineage B.1 and Clade IIb lineage B.1.4, respectively. Twelve nucleotide differences were identified. The observed difference was higher than the estimate of mutations/genome/year described.These data confirm that mpox reinfection is possible and reinforces current vaccination campaigns.

Abstract Key message Abies alba Mill.–Pinus uncinata Ramond. ecotone dynamics are examined along both altitudinal and protection level gradients by combining field inventories and Landsat data. An upward expansion of A. alba to the subalpine belt is observed in the last decades as a result of stand maturation after logging cessation. Context High-mountain forests constitute sensitive locations to monitor the impacts of global change on tree-species composition and ecotone dynamics. In this study, we focus on the Spanish Pyrenees where silver fir (Abies alba Mill.) coexists with mountain pine (Pinus uncinata Ramond.) forming montane-subalpine ecotones. Aims The main goal of this study is to assess the spatiotemporal dynamics of the silver fir–mountain pine ecotone and its underlying driving factors. Methods We reconstructed the spatial distribution and dynamics of the species by combining remote sensing imagery and field plot data from 1989 to 2015, employing support vector machine techniques for image classification. Using variance analysis and mixed effects models, we then analyzed the evolution of basal area and replacement index, a measure of relative change in species composition, over time and altitude range. Additionally, we explored their relationship with site factors and protection level (National Park vs. protection buffer zone). Results Silver fir has expanded its distribution in both the National Park and the protection buffer zone, whereas mountain pine has remained stable. Both species exhibit increased basal area associated with stand maturation and a higher level of protection. The replacement index indicates a rise in silver fir in the understory on North-facing slopes, attributed to stand densification. These findings are particularly noticeable in the area with the highest level of protection. Conclusion The cessation of traditional land uses has led to ongoing stand densification, promoting succession and favoring the increased abundance of silver fir at its uppermost locations, where this species outcompetes mountain pine.

Resumen: Los indicadores de calidad son una herramienta clave como garantía de calidad y homogenización de la asistencia sanitaria. En este contexto, la Academia Española de Dermatología y Venereología ha diseñado el proyecto CUDERMA (Certificación de unidades de dermatología), una iniciativa que busca definir indicadores de calidad para certificar unidades de dermatología en distintos ámbitos, entre los que se seleccionaron psoriasis y dermato-oncología de forma inicial. Este estudio tuvo por objetivo consensuar los aspectos a evaluar por los indicadores, siguiendo un proceso estructurado para la revisión bibliográfica y elaboración de un set preliminar de indicadores, revisado por un grupo multidisciplinar de expertos, para su evaluación mediante un Consenso Delphi. Un panel de 28 dermatólogos evaluó los indicadores y los clasificó como «básicos» o «de excelencia», generando un conjunto de 84 indicadores consensuados que serán estandarizados para diseñar la norma con la que certificar las unidades de dermato-oncología. Abstract: Quality indicators are crucial for standardizing and guaranteeing the quality of health care practices. The Spanish Academy of Dermatology and Venereology (AEDV) launched the CUDERMA Project to define quality indicators for the certification of specialized units in dermatology; the first 2 areas selected were psoriasis and dermato-oncology. The aim of this study was to achieve consensus on what should be evaluated by these indicators using a structured process comprising a literature review and selection of an initial list of indicators to be evaluated in a Delphi consensus study following review by a multidisciplinary group of experts. The selected indicators were evaluated by a panel of 28 dermatologists and classified as either «essential» or «of excellence». The panel agreed on 84 indicators, which will be standardized and used to develop the certification standard for dermato-oncology units.

IntroductionStress is characterized as a challenging occurrence that triggers a physiological and/or behavioral allostatic response, alongside the demands typically encountered throughout the natural course of life. A sustained state of stress gives rise to secondary effects, including insomnia and neck pain. Also, the risk of musculoskeletal problems in the cervical and lumbar spine can be increased due to a sustained state of stress. The present study main objective is to study the association between orofacial and pelvic floor muscles in women in Spain.MethodologyAn observational, cross-sectional, retrospective analytical study was designed and carried out in the laboratories of the European University of Madrid. Sixty-five participants were recruited with a mean age of 29.9 ± 7.69. Measurements were taken by myotonometry on natural oscillation frequency (Hz), dynamic stiffness (N/m), elasticity (N/m2), mechanical stress relaxation time (ms) and creep, for the following muscles: right and left masseter, right and left temporalis and central fibrous nucleus of the perineum (CFPF). On the other hand, the subjects completed the following questionnaires: perceived stress scale (PSS-14), anxiety scale (STAI), self-reported bruxism questionnaire (CBA), Fonseca Anamnestic Index and the Pittsburgh Sleep Quality Scale (PSQI).ResultsSignificant correlations were found in several parameters between the right temporalis and CFPF (pConclusionThe results support the proposed hypothesis. To the author's knowledge, this is the first study which relates both locations, and it is suggested to continue with the research and expand the knowledge of it.
