Abstract Background Castleman disease (CD) is a rare benign disorder characterised by hyperplasia of lymphoid tissue that may develop at a single site or throughout the body. The etiology of this disorder is unclear, although the histopathological presentation can be differentiated into a hyaline vascular variant, a plasma cell variant and a mixed variant. Clinically, it has been recorded that 3 manifestations of CD are characterized: a localized unicentric type, a generalized multicentric type and a mixed form. Surgery remains the main treatment for resectable unicentric CD, since removal of the large node is possible without further complications. No consensus has been reached concerning the most adequate treatment for irresectable unicentric CD. Methods Case report of a 67 year old woman. Results This report, describes the case of a 67-year-old woman with unicentric Castleman disease located in the right lower abdomen. The patient had symptoms of fatigue, dyspnoea and pain in the right lower abdomen. Computed tomography (CT)- examination revealed a tumour, which had grown to form a close relationship with the common iliac vessels and the sacral bone. A Laparotomy procedure revealed that the tumour was an irresectable mass. Neo-adjuvant radiotherapy (40 Gy) was administered in order to downsize the tumour. Six weeks later a new CT-scan revealed a major reduction of the tumour, which enabled a successful radical resection of the tumour to be performed. Histopathological analysis of the tumour showed the hyaline vascular type of CD. Conclusions Neo-adjuvant radiotherapy should be considered in case of an irresectable unicentric CD.

Examples of reasoning problems such as the twins problem and poison paradox have been proposed by legal scholars to demonstrate the limitations of probability theory in legal reasoning. Specifically, such problems are intended to show that use of probability theory results in legal paradoxes. As such, these problems have been a powerful detriment to the use of probability theory - and particularly Bayes theorem - in the law. However, the examples only lead to 'paradoxes' under an artificially constrained view of probability theory and the use of the so-called likelihood ratio, in which multiple related hypotheses and pieces of evidence are squeezed into a single hypothesis variable and a single evidence variable. When the distinct relevant hypotheses and evidence are described properly in a causal model (a Bayesian network), the paradoxes vanish. In addition to the twins problem and poison paradox, we demonstrate this for the food tray example, the abuse paradox and the small town murder problem. Many methods have been suggested for evaluating the evidential value of a matching Y-chromosomal DNA profile obtained from a biological stain associated with a crime scene and the Y-chromosomal DNA profile of a suspect. Most of these methods are based on estimating the population frequency of the Y-profile. The common independence assumption between loci for autosomal DNA profiles cannot be used for Y-chromosomal DNA profiles. In this paper we reconsider the problem of population frequency estimation by application of Bayesian networks and the Chow-Liu algorithm to model dependencies between loci. We found that the method based on the Chow-Liu algorithm performs almost as well as the discrete Laplace method. We have also made comparisons to the independence model and we have demonstrated once again that the independence method for Y-profiles cannot be supported., (Copyright © 2018 Elsevier B.V. More often than not, the source of DNA traces found at a crime scene is not disputed, but the activity or timing of events that resulted in their transfer is. As a consequence, practitioners are increasingly asked to assign a value to DNA evidence given propositions about activities provided by prosecution and defense counsel. Given that the dispute concerns the nature of the activity that took place or the identity of the actor that carried out the activity, several factors will determine how to formulate the propositions. Determining factors are (1) whether defense claims the crime never took place, (2) whether defense claims someone other than the accused (either an unknown individual or a known person) performed the criminal activity, and (3) whether it is claimed and disputed that the suspect performed an alternative, legitimate activity or has a relation to the victim, the object, or the scene of crime that implies a legitimate interaction. In de Zoete et al. (2015) a framework for the evaluation of evidence when an individual is a suspect of two separate offenses (based on Evett et al., 2006) is implemented using a Bayesian network. Here, we extend this to situations with multiple offenders. When we have multiple offenders, new questions arise: (1) Can we distinguish between the offenders, even if we do not know their identity? (2) Do we know that certain pieces of evidence originate from the same person? (3) Do we know the number of offenders? With the aid of a mock case example, we show that such subtle differences between situations can lead to substantially different conclusions in terms of posterior probabilities of a certain suspect being one of the offenders in a particular crime. We reach our conclusions by constructing appropriate Bayesian networks for each situation. Although we find it undesirable that Bayesian networks are demonstrated in court, they can be very helpful in guiding expert and legal reasoning, identifying pitfalls and assist in preventing them. In forensic casework, evidence regarding the type of cell material contained in a stain can be crucial in determining what happened. For example, a DNA match in a sexual offense can become substantially more incriminating when there is evidence supporting that semen cells are present. Besides the question which cell types are present in a sample, also the question who donated what (association) is very relevant. This question is surprisingly difficult, even for stains with a single donor. The evidential value of a DNA profile needs to be combined with knowledge regarding the specificity and sensitivity of cell type tests. This, together with prior probabilities for the different donor-cell type combinations, determines the most likely combination. We present a Bayesian network that can assist in associating donors and cell types. A literature overview on the sensitivity and specificity of three cell type tests (PSA test for seminal fluid, RSID saliva and RSID semen) is helpful in assigning conditional probabilities. The Bayesian network is linked with a software package for interpreting mixed DNA profiles. This allows for a sensitivity analysis that shows to what extent the conclusion depends on the quantity of available research data. This can aid in making decisions regarding further research. DNA profiles can be used as evidence to distinguish between possible donors of a crime stain. In some cases, both the prosecution and the defence claim that the cell material was left by the suspect but they dispute which cell type was left behind. For example, in sexual offense cases the prosecution could claim that the sample contains semen cells where the defence argues that the sample contains skin cells. In these cases, traditional methods (e.g. a phosphatase test) can be used to examine the cell type contained in the sample. However, there are some drawbacks when using these methods. For instance, many of these techniques need to be carried out separately for each cell type and each of them requires part of the available sample, which reduces the amount that can be used for DNA analysis. Another option is messenger RNA (mRNA) evidence. mRNA expression levels vary among cell types and can be used to make (probability) statements about the cell type(s) present in a sample. Existing methods for the interpretation of RNA profiles as evidence for the presence of certain cell types aim at making categorical statements. Such statements limit the possibility to report the associated uncertainty. Some of these existing methods will be discussed. Most notably, a method based on a 'n/2' scoring rule (Lindenbergh et al.) and a method using marker values and cell type scoring thresholds (Roeder et al.). From a statistical point of view, a probabilistic approach is the most obvious choice. Two approaches (multinomial logistic regression and naïve Bayes') are suggested. All methods are compared, using two different datasets and several criteria regarding their ability to assess the evidential value of RNA profiles. We conclude that both the naïve Bayes' method and a method based on multinomial logistic regression, that produces a probabilistic statement as measure of the evidential value, are an important improvement of the existing methods. Besides a better performance, they are flexible and can be adapted to other situations. When two or more crimes show specific similarities, such as a very distinct modus operandi, the probability that they were committed by the same offender becomes of interest. This probability depends on the degree of similarity and distinctiveness. We show how Bayesian networks can be used to model different evidential structures that can occur when linking crimes, and how they assist in understanding the complex underlying dependencies. That is, how evidence that is obtained in one case can be used in another and vice versa. The flip side of this is that the intuitive decision to "unlink" a case in which exculpatory evidence is obtained leads to serious overestimation of the strength of the remaining cases., (Copyright © 2014 Forensic Science Society. When a Y-chromosomal and a (partial) autosomal DNA profile are obtained from one crime sample, and both profiles match the suspect's profiles, we would like to know the combined evidential value. To calculate the likelihood ratio of observing the autosomal and Y-chromosomal DNA profiles combined, we need to know the conditional random match probability of the observed autosomal DNA profile, given the Y-chromosomal match. We examine this conditional probability in two ways: (1) with a database containing data of 2,085 men and (2) using a simulation model. We conclude that if the Y-chromosomal DNA profiles match, we can still regard the autosomal DNA profile as independent from the Y-chromosomal DNA profile if the matching person is not a descendant of the father of the donor of the (crime) sample. The evidential value can, in that case, be computed by multiplying the random match probabilities of the individual profiles.

Background: The sleeve gastrectomy is a surgical technique to treat morbid obesity by both restrictive and probably hormonal action. Originally developed as a first stage to gastric bypass, it is more and more performed as a sole procedure. Therefore it is important to report results on weight loss and reduction in co-morbidity., Methods: A consecutive series of 74 morbid obese patients were evaluated. Parameters were operative variables, complications, weight loss and the need for medication for co-morbidity at least six months postoperatively., Results: Six procedures included the removal of a band and twice a vertical banded gastroplasty was performed previously. Median operating time diminished over time to 71 min. Three procedures were converted into open approach. Major complications were rhabdomyolysis (2), bleeding (2) and leakage (4). Four days was the mean hospital stay. The median follow-up was 12 months (range 6-33). The median percentage of excess weight loss was 49.6% (range 22-96%EWL). The median loss in BMI points was 23.1% (range 9-50%BMIL). Three quarters of the patients were able to diminish or stop their medication for diabetes, hypertension and hyperlipedemia., Conclusion: The laparoscopic gastric sleeve is effective in reduction of both weight and co-morbidity and has potential as a sole procedure. A 77-year-old man was admitted with an ileus. At physical examination a nodule was observed in the umbilicus, 'Sister Mary Joseph's nodule'. At laparotomy, this turned out to be a metastasis of obstructing left-sided colonic cancer.

For a publisher of a scientific journal around 1850 there were various possibilities with regard to illustration technique. The ultimate choice was determined by considerations of necessity, clarity but particularly also, price. In the Nederlandsch Tijdschrift voor Geneeskunde the last-mentioned aspect appears to have been very weighty, especially in the beginning. Newly introduced techniques permitting better detail were often applied relatively late and infrequently. Photography proved to be an important aid in illustrating, although the printed reproduction was initially primitive and of poor quality. Improvement of the financial situation of the Journal was immediately reflected in an increase of the number of illustrations.
