Being highly effective, non-invasive and easy to perform, SGUS has become increasingly popular among specialists in assessing salivary gland (SG) abnormalities, including those commonly found in primary Sjögren’s syndrome (pSS). SGUS may be useful in the assessment of pSS and its complications, the most serious being the development of non-Hodgkin’s lymphoma (NHL). SGUS may also be useful in the characterization and differential diagnosis of diffuse and focal abnormalities commonly associated with pSS, and may act as a guide for core-needle biopsy (CNB), an established, safe, and feasible technique, which provides enough viable tissue for the diagnosis and assessment of lymphoproliferative diseases of the SG. The combination of SGUS with other tools, such as sonoelastography and artificial intelligence (AI), could further improve the usefulness of SGUS in the management of pSS. Abstract

INTRODUCTION: In the 330-patient ADVOCATE trial of avacopan for the treatment of antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis, in which 81% of patients had renal involvement, estimated glomerular filtration rate (eGFR) increased on average 7.3 ml/min per 1.73 m(2) in the avacopan group and 4.1 ml/min per 1.73 m(2) in the prednisone group (P = 0.029) at week 52. This new analysis examines the results in the patient subgroup with severe renal insufficiency at enrollment into the trial, i.e., eGFR ≤20 ml/min per 1.73 m(2). METHODS: eGFR was determined at baseline and over the course of the trial. Changes in eGFR were compared between the 2 treatment groups. RESULTS: In ADVOCATE, 27 of 166 patients (16%) in the avacopan group and 23 of 164 patients (14%) in the prednisone group had a baseline eGFR ≤20 ml/min per 1.73 m(2). At week 52, eGFR increased on average 16.1 and 7.7 ml/min per 1.73 m(2) in the avacopan and prednisone groups, respectively (P = 0.003). The last eGFR value measured during the 52-week treatment period was ≥2-fold higher than baseline in 41% of patients in the avacopan group compared to 13% in the prednisone group (P = 0.030). More patients in the avacopan group versus prednisone group had increases in eGFR above 20, 30, and 45 ml/min per 1.73 m(2), respectively. Serious adverse events occurred in 13 of 27 patients (48%) in the avacopan group and 16 of 23 patients (70%) in the prednisone group. CONCLUSION: Among patients with baseline eGFR ≤20 ml/min per 1.73 m(2) in the ADVOCATE trial, eGFR improved more in the avacopan group than in the prednisone group.

Abstract

Cryoglobulinemic vasculitis (CV) or mixed cryoglobulinemic syndrome (MCS) is a systemic small-vessel vasculitis characterized by the proliferation of B-cell clones producing pathogenic immune complexes, called cryoglobulins. It is often secondary to hepatitis C virus (HCV), autoimmune diseases, and hematological malignancies. CV usually has a mild benign clinical course, but severe organ damage and life-threatening manifestations can occur. Recently, evidence in favor of rituximab (RTX), an anti-CD 20 monoclonal antibody, is emerging in CV: nevertheless, questions upon the safety of this therapeutic approach, especially in HCV patients, are still being issued and universally accepted recommendations that can help physicians in MCS treatment are lacking. A Consensus Committee provided a prioritized list of research questions to perform a systematic literature review (SLR). A search was made in Medline, Embase, and Cochrane library, updated to August 2021. Of 1227 article abstracts evaluated, 27 studies were included in the SLR, of which one SLR, 4 RCTs, and 22 observational studies. Seventeen recommendations for the management of mixed cryoglobulinemia with rituximab from the Italian Study Group of Cryoglobulinemia (GISC) were developed to give a valuable tool to the physician approaching RTX treatment in CV.

Abstract

Introduction: In the 330-patient ADVOCATE trial of avacopan for the treatment of antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis, in which 81% of patients had renal involvement, estimated glomerular filtration rate (eGFR) increased on average 7.3 ml/min per 1.73 m2 in the avacopan group and 4.1 ml/min per 1.73 m2 in the prednisone group (P = 0.029) at week 52. This new analysis examines the results in the patient subgroup with severe renal insufficiency at enrollment into the trial, i.e., eGFR ≤20 ml/min per 1.73 m2. Methods: eGFR was determined at baseline and over the course of the trial. Changes in eGFR were compared between the 2 treatment groups. Results: In ADVOCATE, 27 of 166 patients (16%) in the avacopan group and 23 of 164 patients (14%) in the prednisone group had a baseline eGFR ≤20 ml/min per 1.73 m2. At week 52, eGFR increased on average 16.1 and 7.7 ml/min per 1.73 m2 in the avacopan and prednisone groups, respectively (P = 0.003). The last eGFR value measured during the 52-week treatment period was ≥2-fold higher than baseline in 41% of patients in the avacopan group compared to 13% in the prednisone group (P = 0.030). More patients in the avacopan group versus prednisone group had increases in eGFR above 20, 30, and 45 ml/min per 1.73 m2, respectively. Serious adverse events occurred in 13 of 27 patients (48%) in the avacopan group and 16 of 23 patients (70%) in the prednisone group. Conclusion: Among patients with baseline eGFR ≤20 ml/min per 1.73 m2 in the ADVOCATE trial, eGFR improved more in the avacopan group than in the prednisone group.

Abstract

The purpose of this work is to identify the guidelines for the development and diversification of the cultural touristic offer achievable at the Reale Osservatorio Vesuviano (ROV). Founded in 1841, it is the oldest volcano observatory in the world and it has always had the vocation for scientific-naturalistic and formative tourism. The present study investigates the connections between the sociology of tourism and the ROV’s sustainability and competitiveness as a tourist destination. Here it is performed a study of the tourist flow, based on the sociology of tourism and on the correct processing of data sets, distinguishing the ''statistical information on tourism" from "tourism statistics", as such a study is the basis of a proper market strategy in different sectors. Tourism remains, in fact, an important phenomenon in world economy: despite the economic crisis of recent years, international tourist arrivals show a positive development of the trend in all the major sub-areas into which the large international areas are traditionally divided. The principles for a future marketing plan were outlined after collecting all the relevant information. The plan was designed to enhance the overall touristic offer of the ROV, never ceasing to consider the integration of natural-scientific aspects and cultural events to be offered within the observatory itself., El objetivo de este trabajo es identificar las pautas para el desarrollo y diversificación de la oferta de turismo cultural realizable en el Reale Osservatorio Vesuviano (ROV). Fundado en 1841, es el observatorio de volcanes más antiguo del mundo y siempre ha tenido vocación de turismo científico-naturalista y formativo. El presente estudio investiga las conexiones entre la sociología del turismo y la sostenibilidad y competitividad de ROV como destino turístico. Aquí se lleva a cabo un estudio de los flujos turísticos, basado en la sociología del turismo y en el correcto tratamiento de los conjuntos de datos, distinguiendo la "información estadística sobre el turismo" de las "estadísticas turísticas"; como tal estudio es la base de una estrategia de mercado adecuada en diferentes sectores el turismo sigue siendo, de hecho, un fenómeno importante en la economía mundial. A pesar de la crisis económica de los últimos años, las llegadas de turistas internacionales muestran una evolución tendencial positiva en todas las principales subzonas en las que tradicionalmente se dividen las grandes áreas internacionales. Tras recopilar toda la información pertinente, se esbozaron los principios de un futuro plan de marketing. El plan se diseñó para mejorar la oferta turística global del ROV, considerando al mismo tiempo la integración de los aspectos científico-naturales y los eventos culturales que se ofrecerán dentro del propio observatorio., O objetivo deste trabalho é identificar as orientações para o desenvolvimento e diversificação da oferta turística cultural realizáveis no Reale Osservatorio Vesuviano (ROV). Fundada em 1841, é o mais antigo observatório vulcão do mundo e sempre teve a vocação para o turismo científico-naturalista e formativo. O presente estudo investiga as conexões entre a sociologia do turismo e sustentabilidade do ROV e competitividade como um destino turístico. Aqui é realizado um estudo do fluxo turístico, com base na sociologia do turismo e sobre o tratamento correto de conjuntos de dados, distinguindo “informações estatísticas sobre o turismo" de "estatísticas do turismo"; como um tal estudo é a base de um estratégia de mercado adequada em diferentes setores do turismo continua a ser, de facto, um fenómeno importante na economia mundial. Apesar da crise económica dos últimos anos, as chegadas de turistas internacionais mostram uma evolução positiva da tendência em todas as principais sub-áreas em que as grandes áreas internacionais são tradicionalmente divididas. Os princípios para um plano de marketing futuro foram delineados após a recolha de toda a informação relevante. O plano foi projetado para melhorar a oferta turística global do ROV, não deixando de considerar a integração dos aspetos naturais-científicos e eventos culturais a serem oferecidas dentro do próprio observatório.

Abstract

ObjectiveTo analyse how the potential exposure to air pollutants can influence the key components at the time of diagnosis of Sjogren's phenotype (epidemiological profile, sicca symptoms, and systemic disease). MethodsFor the present study, the following variables were selected for harmonisation and refinement: age, sex, country, fulfilment of 2002/2016 criteria items, dry eyes, dry mouth, and overall ESSDAI score. Air pollution indexes per country were defined according to the OECD (1990-2021), including emission data of nitrogen and sulphur oxides (NO/SO), particulate matter (PM2.5 and 1.0), carbon monoxide (CO) and volatile organic compounds (VOC) calculated per unit of GDP, Kg per 1000 USD.ResultsThe results of the chi-square tests of independence for each air pollutant with the frequency of dry eyes at diagnosis showed that, except for one, all variables exhibited p-values <0.0001. The most pronounced disparities emerged in the dry eye prevalence among individuals inhabiting countries with the highest NO/SO exposure, a surge of 4.61 percentage points compared to other countries, followed by CO (3.59 points), non-methane (3.32 points), PM2.5 (3.30 points), and PM1.0 (1.60 points) exposures. Concerning dry mouth, individuals residing in countries with worse NO/SO exposures exhibited a heightened frequency of dry mouth by 2.05 percentage points (p<0.0001), followed by non-methane exposure (1.21 percentage points increase, p=0.007). Individuals inhabiting countries with the worst NO/SO, CO, and PM2.5 pollution levels had a higher mean global ESSDAI score than those in lower-risk nations (all p-values <0.0001). When systemic disease was stratified according to DAS into low, moderate, and high systemic activity levels, a heightened proportion of individuals manifesting moderate/severe systemic activity was observed in countries with worse exposures to NO/SO, CO, and PM2.5 pollutant levels. Abstract

Objective To analyse how the key components at the time of diagnosis of the Sjogren's phenotype (epidemiological profile, sicca symptoms, and systemic disease) can be influenced by the potential exposure to climate-related natural hazards. Methods For the present study, the following variables were selected for harmonisation and refinement: age, sex, country, fulfilment of 2002/2016 criteria items, dry eyes, dry mouth, and overall ESSDAI score. Climate-related hazards per country were defined according to the OECD and included seven climate-related hazard types: extreme temperature, extreme precipitation, drought, wildfire, wind threats, river flooding, and coastal flooding. Climatic variables were defined as dichotomous variables according to whether each country is ranked among the ten countries with the most significant exposure. Results After applying data-cleaning techniques and excluding people from countries not included in the OECD climate rankings, the database study analysed 16,042 patients from 23 countries. The disease was diagnosed between 1 and 3 years earlier in people living in countries included among the top 10 worst exposed to extreme precipitation, wildfire, wind threats, river flooding, and coastal flooding. A lower frequency of dry eyes was observed in people living in countries exposed to wind threats, river flooding, and coastal flooding, with a level of statistical association being classified as strong (p<0.0001 for the three variables). The frequency of dry mouth was significantly lower in people living in countries exposed to river flooding (p<0.0001) and coastal flooding (p<0.0001). People living in countries included in the worse climate scenarios for extreme temperature (p<0.0001) and river flooding (p<0.0001) showed a higher mean ESSDAI score in comparison with people living in no-risk countries. Abstract

Objective Baricitinib, an oral Janus kinase (JAK) 1-2 inhibitor, is currently used along biologic DMARDs (bDMARDs) after the failure of methotrexate (MTX) in rheumatoid arthritis (RA). We investigated the efficacy and safety of baricitinib in real life. Methods We prospectively enrolled 446 RA patients treated with baricitinib from 11 Italian centres. Patients were evaluated at baseline and after 3, 6, and 12 months. They were arrayed based on previous treatments as bDMARD-naïve and bDMARD-insufficient responders (IR) after the failure or intolerance to bDMARDs. A sub-analysis differentiated the effects of methotrexate (MTX) and the use of oral glucocorticoids (OGC). Results Our cohort included 150 (34%) bDMARD-naïve and 296 (66%) bDMARD-IR patients, with 217 (49%) using baricitinib as monotherapy. Considering DAS-28-CRP as the primary outcome, at 3 and 6 months, 114/314 (36%) and 149/289 (51.6%) patients achieved remission, while those in low disease activity (LDA) were 62/314 (20%) and 46/289 (15.9%), respectively; finally at 12 months 81/126 (64%) were in remission and 21/126 (17%) in LDA. At all-timepoints up to 12 months, bDMARDs-naïve patients demonstrated a better clinical response, independently of MTX. A significant reduction in the OGC dose was observed at 3 and 12 months in all groups. The serum positivity for both rheumatoid factors (RF) and anti-citrullinated protein antibodies (ACPA) conferred a lower risk of stopping baricitinib due to inefficacy. Fifty-eight (13%) patients discontinued baricitinib due to adverse events, including thrombotic events and herpes zoster reactivation. Conclusion Real-life data confirm the efficacy and safety profiles of baricitinib in patients with RA and provide evidence that drug survival is higher in bDMARDs-naïve and seropositive patients.

Abstract

For many decades, the clinical unmet needs of primary Sjögren's Syndrome (pSS) have been left unresolved due to the rareness of the disease and the complexity of the underlying pathogenic mechanisms, including the pSS-associated lymphomagenesis process. Here, we present the HarmonicSS cloud-computing exemplar which offers beyond the state-of-the-art data analytics services to address the pSS clinical unmet needs, including the development of lymphoma classification models and the identification of biomarkers for lymphomagenesis. The users of the platform have been able to successfully interlink, curate, and harmonize 21 regional, national, and international European cohorts of 7,551 pSS patients with respect to the ethical and legal issues for data sharing. Federated AI algorithms were trained across the harmonized databases, with reduced execution time complexity, yielding robust lymphoma classification models with 85% accuracy, 81.25% sensitivity, 85.4% specificity along with 5 biomarkers for lymphoma development. To our knowledge, this is the first GDPR compliant platform that provides federated AI services to address the pSS clinical unmet needs. © 2022 The Author(s)

Abstract

Objective Non-specific musculoskeletal pain is common in subjects destined to develop psoriatic arthritis (PsA). We evaluated psoriatic patients with arthralgia (PsOAr) compared with psoriasis alone (PsO) and healthy controls (HCs) using ultrasonography (US) to investigate the anatomical basis for joint symptoms in PsOAr and the link between these imaging findings and subsequent PsA transition. Methods A cross-sectional prevalence analysis of clinical and US abnormalities (including inflammatory and structural lesions) in PsOAr (n=61), PsO (n=57) and HCs (n=57) was performed, with subsequent prospective follow-up for PsA development. Results Tenosynovitis was the only significant sonographic feature that differed between PsOAr and PsO (29.5% vs 5.3%, p<0.001), although synovitis and enthesitis were numerically more frequent in PsOAr. Five patients in PsOAr and one in PsO group developed PsA, with an incidence rate of 109.2/1000 person-years in PsOAr vs 13.4/1000 person-years in PsO (p=0.03). Visual Analogue Scale pain, Health Assessment Questionnaire, joint tenderness and US active enthesitis were baseline variables associated with PsA development. Conclusion Tenosynovitis was associated with arthralgia in subjects with psoriasis. Baseline US evidence of enthesitis was associated with clinical PsA development in the longitudinal analysis. These findings are relevant for enriching for subjects at risk of imminent PsA development.

Abstract

The volcanic system of Ischia is characterized by an intense hydrothermal activity, documented since the early 16th century by the study of Iasolino (1588), which represents the first systematic analysis of the thermal springs of the island for therapeutic purposes. Later studies partially contributed to the enhancement of knowledge on the volcanic, hydrogeological and hydrothermal features of the island, highlighting the strong interaction between hydrothermal flowpaths and volcano- tectonic processes. The reconstruction of the interplay between hydrothermal and magmatic system becomes, therefore, a fundamental element for territorial planning, not only in terms of management of the huge water and geothermal resource, but also in a perspective of prevention and mitigation of volcanic risk. Thermal springs, fumaroles and mud deposits give beginning clues about deep hydrothermal conditions. Till now at Ischia, the local geochemical characterization of fluids and groundwater has been used for the definition of the origin and structure of the hydrothermal system as a whole, as the hydrogeological information is incomplete. However, volcanic hydrothermal systems, such as that characterizes the island of Ischia, are particularly difficult to analyze and outline, as the groundwater resources are the result of a complex and dynamic mixing among meteoric water, sea water and deep fluids. In such cases, the need for an interdisciplinary approach is evident, involving knowledge and research methods ranging from geology to volcanology, geophysics, geochemistry and hydrogeology. With particular reference to the functional and structural representation of the geothermal system of the Ischia island and the resulting correlations with the volcano-tectonic processes, the examination of previous information highlights the need to update and improve the knowledge on groundwater hydrodynamics and mineralization processes. Therefore, the present study aims at a strongly interdisciplinary action that, starting from the design and implementation of a database on the existing geological/volcanological and hydrogeological information, will contribute to highlight the critical issues, define an operating scheme of the hydro-geo-thermal system of the island of Ischia, and upgrade its hydrogeological, geochemical and volcanic monitoring system. The preliminary results improved the definition of the hydrogeological complexes and advanced knowledge of the groundwater flow conditions.

However at Ischia the geochemical characterization of fluids and groundwater, performed through sampling and analysis of a discrete number of scattered springs, wells and fumaroles, has been used for the definition of the origin and structure of hydrothermal systems as a whole, as the hydrogeological information is incomplete. Moreover, volcanic hydrothermal systems, such as that characterizes the island of Ischia, are particularly difficult to analyze and outline, as the groundwater resources are the result of an articulated and dynamic interaction among meteoric water, sea water and fluids of deep origin. In such cases, the need for an interdisciplinary approach is evident, involving knowledge and research methods ranging from geology to volcanology, geophysics, geochemistry, mineralogy and hydrogeology.
