The aim of this study was to evaluate the quality of prenatal care offered in 12 Basic Health Units (BHU) in the city of Sao Paulo, Brazil, through a review of medical and nurse charts, before and after the municipalization of the public health system. The indicator used considered excellence in care as: starting prenatal care in the first quarter of pregnancy; at least six medical visits; at least two results of blood screening for syphilis and one for HIV; returning to BHU up to 42 days after delivery, This indicator was not present in any care delivered in 2000, and only 7.7% of the care delivered in 2004 obtained it (1.1% to 30% of the care per unit assessed). Although there was an evident improvement in care during the period, the low proportion of excellent prenatal care shows an urgent need to improve this care in the BHU of São Paulo city. Objective: The objective of the study was to assess the nutritional status and metabolic alterations in HIV-exposed uninfected (HIVe) children compared with HIV-unexposed (HIVn) children. Methods: A cross-sectional study was carried out with 76 children distributed into two groups: HIVe (n = 31) and HIVn (n = 45). Biochemical data (hematologic test, lipid profile, insulin resistance, hepatic profile, and C-reactive protein) were evaluated. Anthropometric parameters and body composition analyses were performed. Results: The groups were similar regarding body mass index-for-age z-scores (P = 0.297) and height-for-age z-scores (P = 0.666). HIVe had a higher dyslipidemia prevalence (38.7% versus 11.1%; P = 0.010), altered total cholesterol (TC) values (19.4% versus 2.2%; P = 0.016) higher LDL-C mean levels (97.8 mg/dl versus 86 mg/dl; P = 0.028), borderline low-density lipoprotein cholesterol (LDL-C) (40% versus 14%; P = 0.011) and TC (41.9% versus 20%; P = 0.038) compared with HIVn. Conclusion: Despite the similar nutritional status between groups, our data clearly demonstrated a higher prevalence of dyslipidemia, altered TC, higher LDL-C levels and also LDL-C and TC borderline values in HIVe children. Background: HIV infection can have important although sometimes unexpected consequences, such as contributing to enlargement of the pool of rubella-susceptible children., Methods: At the Federal University of São Paulo, Brazil, we assessed response to rubella immunization at 15 months of age in 15 human immunodeficiency virus type 1 (HIV)-infected children, 20 seroreverted children (SR) and 18 healthy control children born to HIV-seronegative mothers (CON). Blood samples were collected before and 3 months after vaccination. All HIV-infected children had started highly active antiretroviral therapy during their first 6 months of life. Serum samples were tested with a rubella IgG enzyme-linked immunosorbent assay kit., Results: HIV children in immunologic categories 2/3 had lower rubella antibody titers (geometric mean, 33 IU/mL) than those from CON (125 IU/mL) and SR group (236 IU/mL) (Tukey, P = 0.01). Antibody values after vaccination were positively associated with CD4 T cell numbers and negatively associated with HIV viral load assessed immediately before vaccination. The percentage of children with protective antibodies after vaccination (above 10.0 IU/mL) was also significantly different among groups (Fisher's exact test, P = 0.013): CON, 94%; SR, 100%; HIV category 1, 100%; HIV category 2/3, 62%., Conclusions: HIV-infected children with a preserved immune system at measles-mumps-rubella immunization can have a good response to rubella vaccine. In contrast, those in more advanced categories for HIV infection respond poorly.
