Objective: to analyze the experience of the patient during hospitalization, focusing on the co-production of care related to patient safety protocols. Method: qualitative study, whose data were collected through the triangulation of multiple sources: document analysis, observation of 10 professionals in the provision of care and 24 interviews with patient-families from 12 clinical and surgical inpatient units of a hospital. Thematic analysis was carried out, based on the concept of co-production. Results: safety protocols according to the experience of the patient portrayed the role of patient-families as co-producers of safe care. It was found an alignment between perceptions of the patients, institutional definitions and basic national and international patient safety protocols. However, these protocols are not always followed by professionals. Conclusion: co-production was perceived in the protocols for safe surgery and prevention of injuries resulting from falls. In patient identification, hand hygiene and medication process, it was found that co-production depends on the proactive behavior of patient-families, as it is not encouraged by professionals. The research contributes with subsidies to leverage the participation of the patient as an agent of their safety, highlighting the co-production of health care as a valuable resource for advancing patient safety. Objetivo: analisar a experiência do paciente durante a internação hospitalar, com foco na coprodução de cuidados relacionados aos protocolos de segurança do paciente. Método: estudo qualitativo, cujos dados foram coletados por meio da triangulação de múltiplas fontes: análise documental, observação de 10 profissionais na prestação de cuidados e 24 entrevistas com pacientes-famílias de 12 unidades de internação clínicas e cirúrgicas de um hospital. Procedeu-se à análise temática, ancorando-se no conceito de coprodução. Resultados: protocolos de segurança segundo a experiência do paciente retrataram o papel dos pacientes-famílias como coprodutores do cuidado seguro. Constatou-se alinhamento entre as percepções dos pacientes, as definições institucionais e os protocolos básicos de segurança do paciente, nacionais e internacionais. No entanto, esses protocolos nem sempre são seguidos pelos profissionais. Conclusão: a coprodução foi percebida nos protocolos de cirurgia segura e de prevenção de lesões decorrentes de queda. Na identificação do paciente, higienização de mãos e processo de medicação, verificou-se que a coprodução depende do comportamento proativo de pacientes-famílias, pois não é estimulada pelos profissionais. A pesquisa contribui com subsídios para alavancar a participação do paciente como agente de sua segurança, destacando a coprodução de cuidados em saúde como valioso recurso para o avanço na segurança do paciente. Método: estudio cualitativo, cuyos datos fueron recolectados a través de la triangulación de múltiples fuentes: análisis de documentos, observación de 10 profesionales en la atención y 24 entrevistas con pacientes-familias de 12 unidades de hospitalización clínica y quirúrgica de un hospital. Se realizó un análisis temático, basado en el concepto de coproducción. Resultados: los protocolos de seguridad de acuerdo con la experiencia del paciente retrataron el papel de las familias de pacientes como coproductores de atención segura. Se encontró una alineación entre las percepciones de los pacientes, las definiciones institucionales y los protocolos básicos nacionales e internacionales de seguridad del paciente. Sin embargo, estos protocolos no siempre son seguidos por profesionales. Conclusión: la coproducción se percibió en los protocolos de cirugía segura y prevención de lesiones resultantes de caídas. En la identificación del paciente, la higiene de las manos y el proceso de medicación se descubrió que la coproducción depende del comportamiento proactivo de las familias de pacientes, ya que no es alentada por los profesionales. La investigación contribuye con subsidios para aprovechar la participación del paciente como agente de su seguridad, destacando la coproducción de la atención médica como un recurso valioso para avanzar en la seguridad del paciente.

Objetivo: Analisar o uso de instrumentais oftalmológicos durante os procedimentos cirúrgicos e propor um método de gerenciamento de materiais. Método: Estudo de método misto, exploratório sequencial, realizado entre janeiro e junho de 2015, em hospital universitário no sul do Brasil. Primeiramente, fez-se uma abordagem qualitativa a partir de brainstorming e observação de campo. Os temas foram agrupados em categorias temáticas. Por conexão, a etapa quantitativa aconteceu por arranjos de matrizes e programação linear, culminando à proposta de gerenciamento de instrumentais. Resultados: As categorias - reorganização do instrumental conforme tempo do procedimento cirúrgico e necessidade de peças para cada procedimento - orientaram a definição das restrições existentes e aplicação dos modelos matemáticos. Verificou-se uma redução média de 13,10% no número de peças por bandejas e aumento de 17,88% da produção cirúrgica. Considerações finais: Esta proposta permitiu a racionalização e otimização de instrumentais oftalmológicos, favorecendo a sustentabilidade da organização Aim: Analysis of the use of ophthalmic instruments during surgical procedures in order to propose a material management method. Objective: analyze the falls of adult hospitalized patients and their repercussions on the Nursing worker as the second victim. Method: an exploratory, descriptive and qualitative study conducted in two stages - identification of falls with moderate to severe damage and the modeling of falls using the software Functional Resonance Analysis Method; and analysis of the repercussions on the worker as the second victim by means of semi-structured interviews submitted to Content Analysis, with 21 Nursing workers. Results: a total of 447 falls of adult patients were identified, 12 of which with moderate to severe damage, occurred in the absence of the companion, while using sleep inducing, hypotensive or muscle strength altering medications. The modeling identified 22 functions related to the Standard Operating Procedure, to the fall prevention protocol and to the post-event actions. Of these, eight presented variability in their execution. In the second step, the categories "The complexity of care to prevent falls" and "Feelings of the second victim" emerged. Conclusion: falls are complex phenomena and prevention requires involvement of the patient, family and multi-professional team. The professionals involved are second victims and experience feelings of guilt, fear, anguish and helplessness. This study can contribute to the multi-professional spirit and to the early approach to second victims. [ABSTRACT FROM AUTHOR]

Objective: to report the experience of planning and executing a mixed method research in stricto sensu postgraduate nursing. Method: research experience report linked to the Nursing Graduate Program of Paraná and group of studies in the area of health management and nursing. The process of planning and execution of the investigation took place from March 2015 to December 2017. Results: the study was systematized by the mixed method in the sequential explanatory strategy. The experience was described in three moments/stages: Preconception, in which a primary approach to the methodological aspect was done; Research project planning, phase in which there was intense immersion in the study and object design, resulting in the research design, and; Execution, where the procedures and planned investigation phases were operationalized. 17. Environment of the professional Nursing practice in Latin American countries: a scoping review.

- Abstract
Objective: To identify and map the Latin American literary production about the nursing work environment in the hospital setting based on the Nursing Working Index-Revised and on the Practice Environment Scale. Method: A scoping review in five databases and other sources on the topic. The sample included 20 articles, nine Theses and five Dissertations published in the last decade. Result: In the sample, Brazil presented the largest number of publications in journals. The environments were favorable with the application of the Nursing Working Index-Revised and unfavorable with the Practice Environment Scale. Among the domains with the worst scores, control over the environment and adequacy of the team and resources stand out. Conclusion: The literary production has gained prominence only in the last decade and points to the association between favorable practice environments and better care results. [ABSTRACT FROM AUTHOR]

18. Challenges of safety culture in Surgical Center: mixed methods study.

Objective: to analyze the safety attitudes of health and support areas professionals working in Surgical Center. Method: sequential explanatory mixed methods study. The quantitative stage covered 172 health and support professionals in eight Surgical Centers of a hospital complex. The Safety Attitudes Questionnaire/Surgical Center was applied. In the subsequent qualitative stage, 16 professionals participated in the Focus Group. Photographic methods were used from the perspective of ecological and restorative thinking, and data analysis occurred in an integrated manner, through connection. Results: the general score, by group of Surgical Centers, based on the domains of the Safety Attitudes Questionnaire/Surgical Center, reveals a favorable perception of the safety climate, with emphasis on the domains Stress Perception, Communication in the Surgical Environment, Safety Climate and Perception of Professional Performance. The overall analysis of the domain Communication and Collaboration between Teams appears positive and is corroborated by data from the qualitative stage, which highlights the importance of interaction and communication between healthcare teams as fundamental for daily work. Conclusion: the perception of safety attitudes among health and support professionals was positive. 19. Protocols for Postanesthesia Care Unit Handoff and Patient Safety: A Scoping Review.

Aim: To map studies that tested an intervention orienting patient transfer to Postanesthesia Care Unit (PACU) and identify outcomes related to care safety., Methods: Scoping review guided by recommendations of the JBI Manual and the Preferred Reporting Items for Systematic Reviews and Meta-Analysis extension for Scoping Reviews (PRISMA-ScR) guide. The Population, Concept, and Context (PCC) framework was used to develop the research question and consolidate inclusion and exclusion criteria in databases consulted without date parameters. The review protocol was registered in the Open Science Framework., Results: The systematic search identified 182 records. After reading their titles and abstracts, 49 studies were read in full. The final sample consisted of 17 texts published between 2013 and 2024. The main elements evaluated in the implementation of interventions to guide patient transfer to the PACU related to patient safety were as follows: increase in the number of transferred information; higher rates of professional satisfaction with teamwork; presence of surgical professionals; questions from the receiver and adequacy to the instrument. Also was identified a pause to monitor the patient before the transfer and reduction of omitting information, interruptions, distractions, task errors, hypoxemia, nausea, vomiting and excessive pain., Conclusions: The variables evaluated after the interventions to promote a structured patient transfer to the PACU have the potential to guide the development of quality assessment indicators and contribute to the promotion of a safety culture in institutions., Impact: Considering the constant increase in complexity and demand for perioperative care, the investigation of patient transfer processes to the PACU can provide support for the management of surgical practices and contribute to improving patient safety and the experience of the health professionals involved., Patient or Public Contribution: No patient or public contribution. Objective: The aim of this study was to analyze the potential association between nursing workload and patient safety in the medical and surgical inpatient units of a teaching hospital., Method: a mixed method strategy (sequential explanatory design)., Results: the initial quantitative stage of the study suggest that increases in the number of patients assigned to each nursing team lead to increased rates of bed-related falls, central line-associated bloodstream infections, nursing staff turnover, and absenteeism. During the subsequent qualitative stage of the research, the nursing team stressed medication administration, bed baths, and patient transport as the aspects of care that have the greatest impact on workload and pose the greatest hazards to patient, provider, and environment safety., Conclusions: The findings demonstrated significant associations between nursing workload and patient safety. We observed that nursing staff with fewer patients presented best results of care-related and management-related patient safety indicators. In addition, the tenets of ecological and restorative thinking contributed to the understanding of some of the aspects in this intricate relationship from the standpoint of nursing providers. They also promoted a participatory approach in this study.

- Abstract
This exploratory-descriptive, qualitative study was performed with the objective to analyze the social representation of the process of choosing leaders among nursing workers of a university hospital. The information was obtained through free word association and open interviews. For the purposes of this study, thematic content analysis was performed, guided by the Social Representations Theory. Through this focus the following category emerged: division of the nursing team - the input and output of the process of choosing leaders. The category consists of a crystallization of the social representation of the subject, anchored in elements related to leadership, impartiality, change, knowledge, trust and humanization. The idea of leadership is highlighted as inducing a movement in the division of the team as it is associated with the process of choosing leaders for an environment of communication, suitable for integrating the team and recognizing potential leaders.

Shift reporting is an important tool for organizing and planning nursing care. The purpose of this study was to describe how shift reporting is conducted in some wards of a teaching hospital in Porto Alegre, Rio Grande do Sul, Brazil, and to identify the nurses' knowledge on how to systematize this activity. Two data collection methods were used in this qualitative study: observation of shift report; and a questionnaire answered by nurses. Data were submitted to thematic content analysis. In all wards, shift report happened during a nurses' meeting, when information was exchanged, although nurse knew other form of shift reporting. Several positive and negative factors affect shift reporting, and the nursing staff should try to make this activity as easy, systematic, and effective as possible.

This article is an observational study that analyzes the performance of venous punctures and nasogastric and nasointestinal tube placement in pediatric patients, performed by nurses in a pediatric ward. This exploratory descriptive study was carried out at a teaching hospital in Porto Alegre, Rio Grande do Sul, Brazil, and its purpose is to identify frequency and time spent in the performance of these procedures, as well as to provide data for the management of nursing human resources. Two nurses were selected intentionally and an observation structured record was used to collect the data. The average time spent in the performance of venous punctures was 17.62 minutes, and tube placement, 10.8 minutes. The outcomes provided an understanding of the processes complexity, whose elements are related to the nurse's work process, as well as to the other professionals involved, the family and the child who underwent the procedures.

- Abstract
This case study discusses the application of a model to determine the number of nurses in the surgical unit in a general hospital of Porto Alegre, Rio Grande do Sul, Brazil. The obtained results were compared with literature findings. Results of calculations identifying the annual capacity of the unit and the annual capacity of nursing hours are presented. The results obtained by applying this model can allow higher precision when planning the number of nurses needed in this unit for nursing care during pre- and post-surgery, thereby contributing to improve productivity.

- Abstract
This case study aims at presenting the follow-up of a patient who underwent allogeneic bone marrow transplantation and the experience of the nursing team of the Centro de Transplante de Medula Ossea (Bone Marrow Transplantation Center) at the Hospital de Clínicas de Porto Alegre, Brazil, during the development of the systematization of the nurse practice emphasizing the period of elaboration of the nursing diagnosis. Twenty-six diagnosis established in this case are discussed and put together in the format of a table to allow the reader to have a view of their initial and ending periods, during the in-patient treatment time.
