Viral respiratory infections play a role in about half of COPD exacerbations. Although the prevalence of bacterial superinfection is estimated 10– 40% in admitted AE-COPD patients with an influenza infection, the majority is treated with antibiotics. Current national and international guidelines provide limited guidance regarding antibiotic use in hospitalized patients with an AE-COPD with proven viral respiratory pathogens.Study Goal: We aimed to investigate antibiotic prescription in hospitalized patients with a COPD exacerbation and an influenza- or RS virus infection.Patients and methods: We performed a retrospective cohort study in patients admitted with an AE-COPD and influenza- or RS virus infection. We compared clinical characteristics of patients with and without antibiotic treatment on admission and estimated adequacy of antibiotic prescriptions.Results: We included 134 patients. Seventy-nine (59%) received antibiotics on admission. Chest X-ray infiltrates and plasma CRP level (≥ 50 mg/L) were correlated with the prescription of antibiotics. Outcomes, such as number of hospitalized days and mortality, were not significantly different between the groups with and without antibiotic treatment. Antibiotic treatment was considered “probably adequate” in 52/79 (65.8%) patients; “not necessary” in 12/79 patients (15.2%) and “probably not necessary” in another 15/79 patients (19.0%).Conclusion: Prescription of antibiotics in hospitalized COPD patients is common practice despite a proven viral infection on admission. A significant antibiotic reduction of 34.2% in these patients seems feasible. Future guidelines should include recommendations regarding antibiotic stewardship in hospitalized patients with AE-COPD with a proven viral respiratory infection.Keywords: COPD, exacerbation, influenza virus, antibiotic treatment, RS virus

Abstract

The science of mountainous hydrology spans the atmosphere through the bedrock and inherently crosses physical and disciplinary boundaries: land–atmosphere interactions in complex terrain enhance clouds and precipitation, while watersheds retain and release water over a large range of spatial and temporal scales. Limited observations in complex terrain challenge efforts to improve predictive models of the hydrology in the face of rapid changes. The Upper Colorado River exemplifies these challenges, especially with ongoing mismatches between precipitation, snowpack, and discharge. Consequently, the U.S. Department of Energy’s (DOE) Atmospheric Radiation Measurement (ARM) user facility has deployed an observatory to the East River Watershed near Crested Butte, Colorado, between September 2021 and June 2023 to measure the main atmospheric drivers of water resources, including precipitation, clouds, winds, aerosols, radiation, temperature, and humidity. This effort, called the Surface Atmosphere Integrated Field Laboratory (SAIL), is also working in tandem with DOE-sponsored surface and subsurface hydrologists and other federal, state, and local partners. SAIL data can be benchmarks for model development by producing a wide range of observational information on precipitation and its associated processes, including those processes that impact snowpack sublimation and redistribution, aerosol direct radiative effects in the atmosphere and in the snowpack, aerosol impacts on clouds and precipitation, and processes controlling surface fluxes of energy and mass. Preliminary data from SAIL’s first year showcase the rich information content in SAIL’s many datastreams and support testing hypotheses that will ultimately improve scientific understanding and predictability of Upper Colorado River hydrology in 2023 and beyond.

7. Developing a predictive model for low-temperature Laval nozzles with applications in chemical kinetics.

Abstract

Laval nozzles are used in the CRESU ("Cinétique de Réaction en Écoulement Supersonique Uniforme") method to generate a collimated low temperature (5–200 K), low pressure (30–500 Pa), high Mach number (1 < M < 20) supersonic jet. Laval nozzles have been designed using the Method of Characteristics (MOC) since the development of CRESU, which is an analytical method that assumes inviscid, isentropic flow, and is routinely used to design nozzle profiles for a particular gas and temperature with a uniform shock free exit. This study aims to provide a robust computational framework to overcome the limitations of the MOC while also providing recommendations on the numerical model setup required to model a low-temperature supersonic jet. It also discusses the blockage effects when using the Pitot tube method for flow characterization, the influence of inlet turbulence and reservoir size. Numerical results are validated using two different experimental apparatuses from research groups at the University of Leeds and the University of Birmingham. Finally, a MATLAB framework was developed and has been provided as an open source toolbox to allow any user to perform computational fluid dynamics on any Laval nozzle, with the ability to change nozzle geometry, operating conditions and bath gas. The toolbox has been rigorously tested against many benchmark cases, which shows that steady-state Reynolds-averaged Navier–Stokes with the k-omega-shear stress transport turbulence model can be used to accurately predict global quantities, such as average temperature in the stable region of the supersonic jet. Abstract

The science guiding the EUREC4A campaign and its measurements is presented. EUREC4A comprised roughly 5 weeks of measurements in the downstream winter trades of the North Atlantic - eastward and southeastward of Barbados. Through its ability to characterize processes operating across a wide range of scales, EUREC4A marked a turning point in our ability to observationally study factors influencing clouds in the trades, how they will respond to warming, and their link to other components of the earth system, such as upper-ocean processes or the life cycle of particulate matter. This characterization was made possible by thousands (2500) of sondes distributed to measure circulations on meso- (200 km) and larger (500 km) scales, roughly 400 h of flight time by four heavily instrumented research aircraft; four global-class research vessels; an advanced ground-based cloud observatory; scores of autonomous observing platforms operating in the upper ocean (nearly 10 000 profiles), lower atmosphere (continuous profiling), and along the air-sea interface; a network of water stable isotopologue measurements; targeted tasking of satellite remote sensing; and modeling with a new generation of weather and climate models. In addition to providing an outline of the novel measurements and their composition into a unified and coordinated campaign, the six distinct scientific facets that EUREC4A explored - from North Brazil Current rings to turbulence-induced clustering of cloud droplets and its influence on warm-rain formation - are presented along with an overview of EUREC4A's outreach activities, environmental impact, and guidelines for scientific practice. Abstract

With the Arctic rapidly changing, the needs to observe, understand, and model the changes are essential. To support these needs, an annual cycle of observations of atmospheric properties, processes, and interactions were made while drifting with the sea ice across the central Arctic during the Multidisciplinary drifting Observatory for the Study of Arctic Climate (MOSAiC) expedition from October 2019 to September 2020. An international team designed and implemented the comprehensive program to document and characterize all aspects of the Arctic atmospheric system in unprecedented detail, using a variety of approaches, and across multiple scales. These measurements were coordinated with other observational teams to explore cross- cutting and coupled interactions with the Arctic Ocean, sea ice, and ecosystem through a variety of physical and biogeochemical processes. This overview outlines the breadth and complexity of the atmospheric research program, which was organized into 4 subgroups: atmospheric state, clouds and precipitation, gases and aerosols, and energy budgets. Atmospheric variability over the annual cycle revealed important influences from a persistent large-scale winter circulation pattern, leading to some storms with pressure and winds that were outside the interquartile range of past conditions suggested by long-term reanalysis. Similarly, the MOSAiC location was warmer and wetter in summer than the reanalysis climatology, in part due to its close proximity to the sea ice edge.The comprehensiveness of the observational program for characterizing and analyzing atmospheric phenomena is demonstrated via a winter case study examining air mass transitions and a summer case study examining vertical atmospheric evolution. Overall, the MOSAiC atmospheric program successfully met its objectives and was the most comprehensive atmospheric measurement program to date conducted over the Arctic sea ice. Abstract

With the Arctic rapidly changing, the needs to observe, understand, and model the changes are essential. To support these needs, an annual cycle of observations of atmospheric properties, processes, and interactions were made while drifting with the sea ice across the central Arctic during the Multidisciplinary drifting Observatory for the Study of Arctic Climate (MOSAiC) expedition from October 2019 to September 2020. An international team designed and implemented the comprehensive program to document and characterize all aspects of the Arctic atmospheric system in unprecedented detail, using a variety of approaches, and across multiple scales. These measurements were coordinated with other observational teams to explore cross- cutting and coupled interactions with the Arctic Ocean, sea ice, and ecosystem through a variety of physical and biogeochemical processes. This overview outlines the breadth and complexity of the atmospheric research program, which was organized into 4 subgroups: atmospheric state, clouds and precipitation, gases and aerosols, and energy budgets. Atmospheric variability over the annual cycle revealed important influences from a persistent large-scale winter circulation pattern, leading to some storms with pressure and winds that were outside the interquartile range of past conditions suggested by long-term reanalysis. Similarly, the MOSAiC location was warmer and wetter in summer than the reanalysis climatology, in part due to its close proximity to the sea ice edge.The comprehensiveness of the observational program for characterizing and analyzing atmospheric phenomena is demonstrated via a winter case study examining air mass transitions and a summer case study examining vertical atmospheric evolution. Overall, the MOSAiC atmospheric program successfully met its objectives and was the most comprehensive atmospheric measurement program to date conducted over the Arctic sea ice. 22. Antibiotic Prescriptions in Hospitalized Patients with an Exacerbation COPD and a Proven Influenza or RS Virus Infection

Abstract

Background: COPD exacerbations (AE-COPD) add up to over 200,000 hospitalization days annually in the Netherlands. Viral respiratory infections play a role in about half of COPD exacerbations. Although the prevalence of bacterial superinfection is estimated 10–40% in admitted AE-COPD patients with an influenza infection, the majority is treated with antibiotics. Current national and international guidelines provide limited guidance regarding antibiotic use in hospitalized patients with an AE-COPD with proven viral respiratory pathogens. Study Goal: We aimed to investigate antibiotic prescription in hospitalized patients with a COPD exacerbation and an influenza-or RS virus infection. Patients and methods: We performed a retrospective cohort study in patients admitted with an AE-COPD and influenza-or RS virus infection. We compared clinical characteristics of patients with and without antibiotic treatment on admission and estimated adequacy of antibiotic prescriptions. Results: We included 134 patients. Seventy-nine (59%) received antibiotics on admission. Chest X-ray infiltrates and plasma CRP level (≥50 mg/L) were correlated with the prescription of antibiotics. Outcomes, such as number of hospitalized days and mortality, were not significantly different between the groups with and without antibiotic treatment. Antibiotic treatment was considered “probably adequate” in 52/79 (65.8%) patients; “not necessary” in 12/79 patients (15.2%) and “probably not necessary” in another 15/79 patients (19.0%). Conclusion: Prescription of antibiotics in hospitalized COPD patients is common practice despite a proven viral infection on admission. A significant antibiotic reduction of 34.2% in these patients seems feasible. Future guidelines should include recommendations regarding antibiotic stewardship in hospitalized patients with AE-COPD with a proven viral respiratory infection.

Abstract

Objective: Standard surgical treatment of advanced-stage ovarian carcinoma with electrosurgery cannot always result in complete cytoreductive surgery (CRS), especially when many small metastases are found on the mesentery and intestinal surface. We investigated whether adjuvant use of a neutral argon plasma device can help increase the complete cytoreduction rate. Patients and Methods: 327 patients with FIGO stage IIIB–IV epithelial ovarian cancer (EOC) who underwent primary or interval CRS were randomized to either surgery with neutral argon plasma (PlasmaJet) (intervention) or without PlasmaJet (control group). The primary outcome was the percentage of complete CRS. The secondary outcomes were duration of surgery, blood loss, number of bowel resections and colostomies, hospitalization, 30-day morbidity, and quality of life (QoL). Results: Complete CRS was achieved in 119 patients (75.8%) in the intervention group and 115 patients (67.6%) in the control group (risk difference (RD) 8.2%, 95% confidence interval (CI) –0.021 to 0.181; P = 0.131). In a per-protocol analysis excluding patients with unresectable disease, complete CRS was obtained in 85.6% in the intervention group and 71.5% in the control group (RD 14.1%, 95% CI 0.042 to 0.235; P = 0.005). Patient-reported QoL at 6 months after surgery differed between groups in favor of PlasmaJet surgery (95% CI 0.455–8.350; P = 0.029). Other secondary outcomes did not differ significantly. Conclusions: Adjuvant use of PlasmaJet during CRS for advanced-stage ovarian cancer resulted in a significantly higher proportion of complete CRS in patients with resectable disease and higher QoL at 6 months after surgery. Abstract

Profiles of eddy momentum flux divergence are calculated as the residual in the momentum budget constructed from airborne circular dropsonde arrays ((Formula presented.) 220 km) for 13 days during the EUREC (Formula presented.) A/ATOMIC field campaign. The observed dynamical forcing averaged over all flights agrees broadly with European Centre for Medium-Range Weather Forecasts (ECMWF) Integrated Forecasting System (IFS) forecasts. In the direction of the flow, a mean flux divergence (friction) exists over a 1.5-km deep Ekman layer, and a mean flux convergence (acceleration) is present near cloud tops. The friction is countergradient between 1 and 1.5 km, where vertical wind shear exceeds the observed thermal wind. From the frictional profile, a 10-m momentum flux of (Formula presented.) 0.1 N (Formula presented.) m (Formula presented.) is derived, in line with Saildrone turbulence measurements. A momentum flux divergence in the cross-wind direction is pronounced near the surface and acts to veer the wind, opposing the friction-induced cross-isobaric wind turning. Weaker friction and upper-level acceleration of easterly flow are observed when stronger winds and more vigorous convection prevail. Turbulence measurements on board the SAFIRE ATR-42 aircraft and the Uncrewed Aircraft System (UAS) RAAVEN reveal pronounced spatial variability of momentum fluxes, with a non-negligible contribution of mesoscales (5–30 km). The findings highlight the nontrivial impact of turbulence, convection, and mesoscale flows in the presence of diverse cloud fields on the depth and strength of the frictional layer., Atmospheric Remote Sensing

Abstract

Despite the importance of surface energy budgets (SEBs) for land-climate interactions in the Arctic, uncertainties in their prediction persist. In situ observational data of SEB components - useful for research and model validation - are collected at relatively few sites across the terrestrial Arctic, and not all available datasets are readily interoperable. Furthermore, the terrestrial Arctic consists of a diversity of vegetation types, which are generally not well represented in land surface schemes of current Earth system models. This dataset describes the environmental conditions for 64 tundra and glacier sites (>=60°N latitude) across the Arctic, for which in situ measurements of surface energy budget components were harmonized (see Oehri et al. 2022). These environmental conditions are (proxies of) potential drivers of SEB-components and could therefore be called SEB-drivers. The associated environmental conditions, include the vegetation types graminoid tundra, prostrate dwarf-shrub tundra, erect-shrub tundra, wetland complexes, barren complexes (≤ 40% horizontal plant cover), boreal peat bogs and glacier. These land surface types (apart from boreal peat bogs) correspond to the main classification units of the Circumpolar Arctic Vegetation Map (CAVM, Raynolds et al. 2019). For each site, additional climatic and biophysical variables are available, including cloud cover, snow cover duration, permafrost characteristics, climatic conditions and topographic conditions.

Despite the importance of surface energy budgets (SEBs) for land-climate interactions in the Arctic, uncertainties in their prediction persist. In situ observational data of SEB components - useful for research and model validation - are collected at relatively few sites across the terrestrial Arctic, and not all available datasets are readily interoperable. Furthermore, the terrestrial Arctic consists of a diversity of vegetation types, which are generally not well represented in land surface schemes of current Earth system models. This dataset comprises harmonized, standardized and aggregated in-situ observations of surface energy budget components measured at 64 sites on vegetated and glaciated sites north of 60° latitude, in the time period from 1994 till 2021. The surface energy budget components include net radiation, sensible heat flux, latent heat flux, ground heat flux, net shortwave radiation, net longwave radiation, surface temperature and albedo, which were aggregated to daily mean, minimum and maximum values from hourly and half-hourly measurements. Data were retrieved from the monitoring networks FLUXNET, AmeriFlux, AON, GC-Net and PROMICE.

Abstract

Introduction/Background* The most important prognostic facttor for the survival of advanced-stage ovarian cancer is the completeness of cytoreductive surgery (CRS). Therefore, an intraoperative technique to detect microscopic tumours would be of great value. The aim of this pilot study is to assess feasibility of near infrared (NIR) hyperspectral imaging (HSI) for the detection of malignant ovarian cancer, using ex vivo tissue samples collected during CRS. Methodology In this pilot-study, patients with proven or suspected ovarian cancer planned for CRS were enrolled in the study. Hyperspectral images with 25 spectral bands were acquired from the resected tissues in the wavelength range of 665-975 nm. All hyperspectral data were processed by image calibration and min-max normalisation, glare removal, feature selection and linear support vector machine (SVM) classifier training. The performance of the classification was evaluated by leave-one-out cross-validation. Result(s)* Ten patients who underwent cytoreductive surgery for advanced-stage epithelial ovarian cancer (EOC) were included in the study, from which 26 tissue samples were imaged, with a total of 26.446 data points that were matched to histopathology. Samples included tissue of the ovaries, fallopian tubes, uterus, omentum and/or part of the intestines. Overall, HSI combined with the SVM classifier was capable to discriminate tumour tissue from non-tumour tissue with a sensitivity of 0.81, specificity of 0.75, area under the curve of 0.83, and Matthew’s correlation coefficient of 0.41. Conclusion* This pilot study shows that hyperspectral imaging is a promising technique to discriminate ovarian carcinomas from the surrounding tissue. Hyperspectral imaging can scan a whole area, is fast, non-contact, non-invasive and can be used inside the operation room.

Introduction/Background*The aim of surgery for advanced-stage ovarian cancer is complete cytoreductive surgery, because this is the most important independent prognostic factor for prolonged survival. Yet this can be difficult to achieve when there are micrometastases on the mesenterium or intestines. The PlasmaJet® Surgical device is an instrument to remove these micrometastases, but little is known about the depth of tissue damage compared to electrocoagulation devices. Methodology A prospective study was performed to determine the histological depth of thermal damage in tissues in a series of 106 histological slides of 17 advanced-stage ovarian cancer patients to compare thermal damage depth after use of the PlasmaJet® Surgical device with electrocoagulation devices. Depending on which tissue types were resected during complete cytoreductive surgery, samples were collected from reproductive organs (uterus, ovaries), intestines (ileum, colon, rectum) and omentum, mesenterium and peritoneum. A systematic review of the literature on tissue damage with the PlasmaJet® Surgical device was carried out. Abstract

A viscoelastic turbulence model in a fully-developed drag reducing channel flow is improved, with turbulent eddies modelled under a k–ε representation, along with polymeric solutions described by the finitely extensible nonlinear elastic-Peterlin (FENE-P) constitutive model. The model performance is evaluated against a wide variety of direct numerical simulation data, described by different combinations of rheological parameters, which is able to predict all drag reduction (low, intermediate and high) regimes with good accuracy. Three main contributions are proposed: one with a simplified viscoelastic closure for the NLTij term (which accounts for the interactions between the fluctuating components of the conformation tensor and the velocity gradient tensor), by removing additional damping functions and reducing complexity compared with previous models; second through a reformulation for the closure of the viscoelastic destruction term, Eτp, which removes all friction velocity dependence; lastly by an improved modified damping function capable of predicting the reduction in the eddy viscosity and thus accurately capturing the turbulent kinetic energy throughout the channel. The main advantage is the capacity to predict all flow fields for low, intermediate and high friction Reynolds numbers, up to high drag reduction without friction velocity dependence.

Abstract

Deze bijdrage bestaat uit een beschrijving van de herziene 'Richtlijn Scheiding en problemen van jeugdigen voor jeugdhulp en jeugdbescherming'. Het artikel beoogt daarmee kinder- en jeugdpsychotherapeuten en andere specialisten in de jeugdhulp te wijzen op het bestaan van de richtlijn, de weg te wijzen in deze herziene versie en informatie te geven over de belangrijkste wijzigingen. In het kort wordt aangegeven wat het algemene doel van een richtlijn is, hoe deze richtlijn tot stand kwam en wat de aanleiding was voor de herziening. Vervolgens wordt ingegaan op de opbouw en inhoud van de richtlijn, waarbij de prevalentie en relevantie worden beschreven en enkele belangrijke wijzigingen worden toegelicht. Deze betreffen met name: (1) complexe scheiding: definitie, beschermende en risicofactoren en gevolgen voor kinderen, inclusief contactbreuk; (2) zorg- en omgangsregelingen: het belang van contact met beide ouders, waarbij overnachting van jonge kinderen bij beide ouders speciale aandacht krijgt; (3) ontwikkelingen op het gebied van interventies en signaleringsinstrumenten en (4) samenwerking met jeugdigen, ouders en het netwerk, waarbij vooral de ondersteuning en participatie van jeugdigen worden uitgelicht en het belang van tijdige opschaling wordt benadrukt. Abstract

Atmospheric measurements from the Arctic Summer Cloud Ocean Study (ASCOS) are used to evaluate the performance of three atmospheric reanalyses (European Centre for Medium Range Weather Forecasting (ECMWF)- Interim reanalysis, National Center for Environmental Prediction (NCEP)-National Center for Atmospheric Research (NCAR) reanalysis, and NCEP-DOE (Department of Energy) reanalysis) and two global climate models (CAM5 (Community Atmosphere Model 5) and NASA GISS (Goddard Institute for Space Studies) ModelE2) in simulation of the high Arctic environment. Quantities analyzed include near surface meteorological variables such as temperature, pressure, humidity and winds, surface-based estimates of cloud and precipitation properties, the surface energy budget, and lower atmospheric temperature structure. In general, the models perform well in simulating large-scale dynamical quantities such as pressure and winds. Near-surface temperature and lower atmospheric stability, along with surface energy budget terms, are not as well represented due largely to errors in simulation of cloud occurrence, phase and altitude. Additionally, a development version of CAM5, which features improved handling of cloud macro physics, has demonstrated to improve simulation of cloud properties and liquid water amount. The ASCOS period additionally provides an excellent example of the benefits gained by evaluating individual budget terms, rather than simply evaluating the net end product, with large compensating errors between individual surface energy budget terms that result in the best net energy budget.
