Introduction: The aim of the study was to assess health behaviours and analyse selected risk factors for disease among patients diagnosed with chronic lower limb ischaemia. Material and methods: The study included 100 patients with chronic lower limb ischaemia according to the Fontaine classification in stages IIb to IV. The study used the diagnostic survey method using Juczynski's Health Behaviour Inventory and a proprietary survey questionnaire, as well as analysis of medical records. Results: In the study, 54% of the subjects had low levels of health behaviours. A statistical analysis revealed a correlation between health behaviours and certain risk factors such as the number of cigarettes smoked (p = 0.0357), hypertension (p = 0.0398), hypercholesterolaemia (p = 0.0446), age (p = 0.0015), and gender (p = 0.0021). The study group had the main risk factors for chronic ischaemic disease of the lower limbs, which include gender, age, hypertension, type 2 diabetes mellitus, smoking, lipid metabolism disorders, overweight and obesity, and history of atherosclerotic diseases. According to Fontaine's classification, diagnosed degree III ischaemia prevailed among the subjects (44%), slightly fewer subjects (33%) had degree IV, while the fewest subjects (23%) were diagnosed with degree IIb ischaemia. Conclusions: Health behaviours of patients with chronic lower limb ischaemia were at a low level overall; the lowest in the categories of health practices and proper eating habits. A burden of risk factors for chronic lower limb ischaemia was detected in the study group of patients. [ABSTRACT FROM AUTHOR]

8. Risk factors of the depressive and anxiety disorders development among patients with Adolescent Idiopathic Scoliosis and possible therapeutic interventions.

Introduction: Adolescent idiopathic scoliosis is a common type of scoliosis among adolescents. A significant problem is the coexistence of depressive and anxiety disorders. However, the causes of this condition are not fully recognized. Thus, it seems reasonable to search for possible risk factors that may trigger mental disorder occurrence. Material and methods: This is a literature narrative review concentrating on the influence of the adolescent idiopathic scoliosis on the development of depression and anxiety disorders among adolescents, as well as possible therapeutic interventions. Google Scholar, Medline, PubMed and Science Direct databases were searched throughout January 2024 for relevant original and review articles from 2000 to 2024, using the following keywords: "adolescent idiopathic scoliosis", "anxiety", "body image", "bracing", "chronic disease", "depression", "mental disorders", "pain", "peer relationship", "physical activity", "pulmonary function", "sleep disorders", "surgery", "treatment". Manuscripts in other language than Polish or English were excluded from the search. To ensure the appropriate quality of this review, the Scale for the Assessment of Narrative Review Articles was used. Results: Chronic pain, sleep disorders, pulmonary function impairment, perception of the disease as a chronic somatic disease, disturbed body image, lack of social interactions, physical activity limitation and applied methods of treatment may lead to the depressive and anxiety disorders development in the adolescent idiopathic scoliosis course. Conclusions: Biological and psychosocial factors contribute to the pathogenesis of depressive and anxiety disorders among patients with adolescent idiopathic scoliosis. Their identification should help create a comprehensive plan of treatment, which may effectively maximize therapeutic outcomes in the context of both scoliosis and mental disorders. Introduction and objective: In recent years, there has been an increase in the prevalence of mood disorders in the developmental population. Their atypical nature, symptoms masking the diagnosis, and a high risk of auto-aggressive behaviours are also noticeable. The aim of the study was to determine the prevalence of mood disorders in the population of psychiatrically hospitalised adolescents. Materials and methods: The research material was an anonymous database of 1,311 patients aged 13-18 years, treated for 10 years (2006-2015) in a stationary youth ward. A group of patients with mood disorders was selected from the study population. Results: Mood/affective disorders were diagnosed in 13.5% of hospitalised adolescents. The mean age of patients with this diagnosis was 16 years. A statistically significant relationship was observed between the diagnosis of mood disorders and a history of suicide attempts (in >50%), self-injury (in >54%), learning difficulties, truancy, and repeating a grade. Conclusions: It was found that a history of suicide attempts, learning difficulties, truancy, repeating a grade, and a history of self-injury were risk factors for affective disorders in the study population. Introduction: The presence of delirium is associated with decreased cognitive function, long-term hospitalization, more severe health problems, and higher mortality rates. The aim of the study was to assess the occurrence of postoperative delirium and its main risk factors based on available research studies. Material and methods: The source of data used to achieve the purpose of the work were 78 original works on the subject of postoperative delirium. The analysed material concerned full-text works published in the years 2013-2023. Results: Patients after cardiac, orthopaedic, abdominal, thoracic, vascular, neurosurgical, and urological procedures were qualified for postoperative delirium testing. In total, in all studies, the study group consisted of 29,359 patients. Postoperative delirium syndrome occurred in 4720 of these patients (17%). It developed in 1/5 of patients after vascular and neurosurgical operations. It mainly concerned patients over 65 years of age. It was determined that the risk factors predisposing to the development of this complication accounted for 70% and the triggering factors for 30%. Various scales were used to diagnose delirium: DSM-IV, DSM-V, CAM, CAM-ICU, MDAS. The CAM scale was most frequently used (31%). Conclusions: Postoperative delirium occurred mainly in people over 65 years of age. The highest rate of delirium was observed after vascular and neurosurgical operations. The review shows that the greatest impact on the occurrence of delirium after procedures had predisposing factors, i.e. advanced age, comorbidities (diabetes, hypertension, lung disease, chronic renal failure, depression), cognitive, visual, and auditory function disorders, and dementia. In the diagnosis of postoperative delirium, the CAM scale was most often used. [ABSTRACT FROM AUTHOR]

Persistent postoperative pain is defined as a chronic pathological pain, which arises as a result of a surgical procedure and persists beyond normal tissue healing time, whose intensity and location cannot be explained by the presence of other causes, such as relapse, infection, or recurrence in the case of cancer. It usually develops after limb amputations and surgical procedures involving the trunk (thoracotomy, breast surgery, reduction of pelvic fractures, prostatectomy, and cardiac surgery). It is affected by a number of pre-, intra- and post-surgical factors, which are discussed in detail in this paper. The most effective persistent postoperative pain management strategy is to prevent it. The key measures aimed at reducing the risk of persistent postoperative pain include: the recognition of predisposing factors, proper identification of high-risk patients and taking individual therapeutic measures accordingly, including surgery eligibility criteria that include a careful risk/benefit assessment, appropriate surgical techniques (minimally invasive techniques, nerve identification and protection) minimising the risk of intraoperative nerve damage and proper anaesthesiology care of the patient during the intra- and postoperative period as well as effective postoperative pain relief. Where possible, the underlying condition should be addressed (e.g. decompression or repair of an injured nerve). In other patients, symptomatic treatment is preferred. Since in most cases persistent postoperative pain is neuropathic in nature, it should be treated using appropriate neuropathic pain treatment algorithms combined with psychotherapy and rehabilitation. Delirium is a serious problem in critically ill patients. Its pathomechanism has been poorly recognized, and treatment is mainly based on clinical symptoms. This article presents a problem of delirium as well as its epidemiology, risk factors, pathomechanisms and frequently used diagnostic methods. The authors postulate to prevent delirium at the beginning of treatment including prevention of the most important risk factors. Introduction: The aim of the study was: to assess the severity of Secondary Traumatic Stress (STS), to identify ST's predictors, and to assess the model of relationships between predictors of Secondary Traumatic Stress in psychotherapists working during the pandemic in Poland. Materials and methods: The research group consisted of 153 psychotherapists (130 women, 85%) aged 25-66 (M = 39.39; SD = 9.65). The following tools were used to measure the variables: Coping Inventory for Stressful Situations (CISS), Perceived Stress Scale (PSS-10), Fatigue Severity Scale (FSS), FCV-19S Fear of COVID-19 Scale (FCV-19S), Secondary Traumatic Stress Scale (STSS), Personal Inquiry, Assessment of working conditions during a pandemic. Discussion: It has been shown that 9.8% of psychotherapists working during the pandemic experienced high or very highintensity of symptoms of Secondary Traumatic Stress. The predictors of STS turned out to be: task-induced stress during the pandemic, emotion-focused stress-coping style, and fear of COVID-19. The proposed model of moderated moderation, which assumes that COVID-19 anxiety that intensifies the emotional style of coping with stress in specialists, has an influence on the relationship between occupational tasks stress during a pandemic and the severity of symptoms of Secondary Traumatic Stress, has not been confirmed. Therefore, there is a need for further exploration of the issue. Conclusions: Research shows an increase in STSD among Polish psychotherapists during covid as well as risk factors intensifying this diagnosis. [ABSTRACT FROM AUTHOR]

The problem of obesity not only affects people working in various sectors and performing various professions but also affects the work environment. Blue-collar workers, even though their work involves physical exertion, may also be exposed to problems with maintaining proper body weight. The work environment can directly influence employee's lifestyle behaviours. An active lifestyle related to professional work that is not adapted to the employee's capabilities often leads to fatigue and may be the reason for reluctance to engage in recreational activities. Performing physical work may also lead to various types of health problems that contribute to limiting physical activity, both in professional and recreational sense. Excess work and responsibilities often force employees to limit their daily meals to a minimum. Additionally, certain factors present in the work environment may also increase the risk of developing obesity. 20. The incidence of schizophrenia in the context of growing up in urbanised vs. green areas - a narrative review.

The neurodevelopmental theory of schizophrenia describes multiple risk factors of this serious mental disorder. These include genetic factors that generally have biological roots, as well as factors related to the upbringing in a specific environment during childhood and adolescence of a person who will develop schizophrenia in adulthood. The neurodevelopmental hypothesis also implies the existence of protective factors. The paper discusses selected environmental risk factors and protective factors of schizophrenia in the context of ecopsychiatry. Urbanicity has for decades been a factor indicated as one that increases the risk of schizophrenia. Contemporary research helps understand how growing up in an urban environment affects brain development, with a particular focus on grey matter volume changes in the dorsolateral prefrontal cortex and the perigenual anterior cingulate cortex. The impact of air pollution as a risk factor for schizophrenia is also the subject of discussions. Exposure to outdoor air pollutants containing particulate matter and ultrafine particulate matter, gases, organic compounds, and metals can lead to neurotoxicity and neuropathology. Air pollution can affect the brain through such mechanisms as inflammation and oxidative stress. On contrary, higher exposure to green spaces has been suggested to reduce the incidence of schizophrenia. Some research indicates a dose-response association between the level of exposure to green space in childhood and a lower risk of developing schizophrenia in the future. The presented review of selected publications indicates that exposure to a certain environment in childhood and adolescence modulates the risk of schizophrenia. Environments heavily transformed by human activity (urbanisation) and characterised by a high level of chemical contamination (toxins, air pollution, smog) are a risk factor for schizophrenia. Protective factors, on the other hand, include direct contact with the natural environment from childhood through adolescence to adulthood. [ABSTRACT FROM AUTHOR]

21. Risk factors of ischaemic stroke in patients with atrial fibrillation.

Introduction: Atrial fibrillation (AF) is a common arrhythmia and a major risk factor for stroke. However, this risk depends on the coexistence of other factors that predispose to stroke and AF. Aim of the research: We evaluated which of the common clinical factors were most strongly associated with stroke in patients with AF compared to patients with sinus rhythm. Material and methods: In this retrospective, observational, single-centre study we analysed patients with acute ischaemic stroke admitted to the neurology centre between 1 January 2013 and 30 April 2015, inclusive. Patients were divided into groups with and without AF. Multivariate logistic regression analysis was used to identify predictors of stroke in the AF group. Results: A total of 2339 ischaemic stroke patients were included in the study (mean age: 73.26 ±12.38 years, 51% male). Of these, 29.1% had AF. Patients with stroke and AF were significantly older (p < 0.001), were more often female (p < 0.001), and had higher rates of hypertension (p < 0.001), coronary heart disease (p < 0.001), and heart failure (p < 0.001). Multivariate logistic regression analyses identified older age (OR = 1.043, p < 0.001), female gender (OR = 1.389, p = 0.001), heart failure (OR = 2.467, p < 0.001), and coronary heart disease (OR = 1.618, p < 0.001) as independent factors that increased the risk of AF-associated stroke. Conclusions: Patients with stroke and AF had additional risk factors coexisting with arrhythmia. The detection of other potential causes of stroke in patients with AF is important to allow modification of some of them and to establish effective treatment to improve outcomes in this group of patients. Frontal fibrosing alopecia is a variant of lichen planus, leading to permanent hair loss from the frontal hairline. It occurs mainly in postmenopausal women, however, the role of hormonal factors in the pathogenesis of this type of alopecia has not been determined. The coexistence of frontal fibrosing alopecia and autoimmune diseases is observed, e.g., hypothyroidism, rheumatoid arthritis, vitiligo, polymyositis, systemic lupus erythematosus and discoid lupus erythematosus. Considering the possible role of genetic factors, research is underway to identify human leukocyte antigen (HLA) polymorphisms. One of the most studied factors is sunscreen, widely used in cosmetic products. The data available so far in this regard are controversial. Factors raising significant doubts include: the retrospective nature of the research, basing the collected data on the self-assessment of respondents and the lack of specification of the ingredients of these products. In the light of current data, sunscreen components cannot be considered as a factor playing a role in the pathogenesis of frontal fibrosing alopecia. Overestimation of the role of sunscreens in hair loss could lead to unjustified avoidance of their use and contribute to increased incidence of skin cancer. [ABSTRACT FROM AUTHOR]

Dementia affects a significant portion of the population of elderly people and thus has become one of the most important health problems. At the same time, people with dementia are more likely to be also affected by concomitant diseases. Cardiovascular factors seem to be of particular importance. It has been shown that problems regarding blood pressure as well as lipid and carbohydrate metabolism play a crucial role for the rate of cognitive deterioration in elderly individuals in both vascular cognitive impairments and primary degenerative impairments (e.g., Alzheimer's disease). A clear relationship is observed between vascular pathology and degenerative processes in the brain. The period of life in which the exposure to cardiovascular factors occurs seems to be key, and these relationships are best documented in middle age. With aging, their importance as factors accelerating the progression of cognitive impairments seems to diminish, especially in Alzheimer-type dementia. Research on the importance of comorbidity in dementia processes may be crucial for the development of prevention and therapy programs for dementia. [ABSTRACT FROM AUTHOR]

Introduction: The reality is full of new phenomena, and the existing ones are transformed. An example is the phenomenon of social maladjustment. There is an increase in the number of criminal acts and acts proving demoralization committed by children and adolescents. The situation forces us to take radical measures, preceded by an in-depth analysis of the mechanisms and causes of the problem. The analysis shows the diversification of the demoralization phenomenon and, most often, the accumulation of risk factors for the emergence of the problem of social maladjustment, with the scarcity of the so-called protective factors. People with disabilities are in a particularly disadvantaged situation, often with a low degree of adaptation, exposed to unfavorable social situations. The greater the danger, the less favorable the environmental conditions - family and lack of support. Research Aim: The aim of the research is to get to know the situation of a boy experiencing hearing impairment and manifesting a particularly intense, relatively permanent form of social maladjustment and the determinants of this situation. The diagnosis will be used to develop a proposal of actions supporting the improvement of the juvenile's functioning. Method: The research is qualitative research using the case-by-case method. The technique of interview, obserwation technique and document analysis was used. The data was obtained through the use of an interview questionnaire, observation sweet and analysis of OZSS opinions. Results: The results of the diagnosis indicate numerous and varied determinants of the demoralization of a young person with hearing impairment. Impaired efficiency causes limitations in functioning, exposes them to a low degree of adaptation, and in the case of an unfavorable family situation and the lack of adequate support, it led to social maladjustment. Conclusion: The analysis of the juvenile's situation is the basis for drawing conclusions about the need to undertake decisive actions of a social rehabilitation, revalidation and preventive nature. Comprehensive actions are able to change the functioning of a person whose biography contains unfavorable factors.

Background: Up to date, the assessment of adherence to the treatment of children with asthma in Kazakhstan has not been carried out yet. Objective: The study aimed to evaluate the level of therapeutic adherence of paediatric patients with asthma in urban and rural areas of Kazakhstan. In addition, the validity and reliability of a structured scale for assessing adherence in patients with asthma was also scrutinized. Methods: A prospective study of verified cases of bronchial asthma of 518 children in the dispensary control in the Children's City Clinical Hospital N2 (Almaty, Kazakhstan) was conducted in the period from 2018 to 2020. Children with asthma were sub-divided into 2 groups: urban and rural residents. The demographic and clinical characteristics, external and social factors of the patients were analysed. Patient adherence to treatment was assessed using the Lebanese Medication Adherence Scale-14(LMAS-14). The reliability was tested using the measure of internal consistency (Cronbach's alpha). Data were collected after the discharge from the hospital (T1) and after 6 months (T2). Results: Urban and rural patients made up 81.3% vs. 18.7% respectively. Children aged 5-12 years accounted for 70.6%. Male gender was predominant in all groups (p=0.73). Better financial situation, education were noted by 90.5% and 95.5% of urban parents (p=0.001). The influence of external factors (dampness, coal heating and possession of pets) was higher in rural areas (p=0.001). The results of study based on the LMAS-14 questionnaire (T1 period) showed no significant differences. However, in the T2 period, the adherence of urban children was higher than rural ones (p=0.001). The Cronbach's a was 0.606 for the total score. According to the results of a survey after 18 months, a significant relationship was found between the levels of adherence and the frequency of asthma attacks (p=0.001). Conclusion: In 2018-2020, a low therapeutic adherence of children with asthma, especially in a rural area, was detected in the territory of the Republic of Kazakhstan. Our findings indicate the necessity to develop effective measures to regulate and increase adherence to treatment of patients, especially those living in rural areas. [ABSTRACT FROM AUTHOR]

The article is a review. This article, which is aligned with theme of the journal, refers to research on the risk factors of gambling disorders in children and adolescents and was based on an analysis of foundational data from a systematic review of English-language scholarly journals over the years 2013-2021. Although research into the aetiology and pathogenesis of eating disorders has been ongoing for many years, it has not yet been possible to identify all the factors responsible for their development. Current research does not focus on a single risk factor, but looks for correlations between them. This article presents selected aspects of the pathogenesis and risk factors of specific eating disorders. Family and individual factors, social and cultural factors, and biological factors are described. Particular attention has been paid to the aspect of parenting styles and behaviours exhibited, e.g. excessive punishment and emotional coldness. The coexistence of personality disorders with specific and non-specific eating disorders is also described. The social aspect highlights the development of social media and their impact on self-esteem, body dissatisfaction, and promotion of pathological thinness. Cultural factors include progressive "westernisation" and the rapid increase in the prevalence of eating disorders in the areas of the world where they did not previously exist. Among biological factors, publications on appetite neuromodulators, genetic factors, gut microbiota disorders, and the so-called brain-gut axis were analysed. Modern science is not as yet able to answer the question of which of the mentioned factors has the greatest impact on the development of eating disorders, however, ongoing research offers hope for effective treatment of these diseases in the future. In this article out lines the koncept of resilience chich broadly relates to the adaptability of person, family or en tire communities in the face of adversity or traumatic events. The genesis of the conceptof resilience was pointed out. Norman Garmezy, Emmy Werner and Michael Rutter were pointed out as precusors to resilience research and a synthetic description of their research was also made. The considerations taken in this article are intended to show the complexity of the resilience construct Istel and the concepts related to it – positive adaptation, risk and protective factors. Cognitive possibilities and limitations in the use of the resilience koncept were also analyzed, emphasizing the Reed to establish uniform theoretical and empirical frameworks.

W artykule zaprezentowano polską adaptację narzędzia Teachers’ Resilience Scale (M. Platsidou, A. Danilidou). Skala służy do diagnozowania resilience wśród nauczycieli. W artykule zawarto informacje dotyczące procedury adaptacyjnej – opracowania polskiej wersji językowej i sprawdzenia właściwości psychometrycznych skali. Badania walidacyjne zostały przeprowadzone wśród 317 nauczycieli. Podstawowa struktura skali zbliżona jest do wersji oryginalnej - eksploracyjna i konfirmacyjna analiza czynnikowa potwierdziły jej czteroczynnikową strukturę (Kompetencje osobiste, Spójność rodziny, Kompetencje społeczne i wsparcie współpracowników, Znaczenie duchowości). Uzyskane dane empiryczne wskazują na satysfakcjonującą rzetelność, stabilność i trafność prezentowanego narzędzia.

Hand hygiene continues to be the most important part of the strategy to combat healthcare associated infections and the first step in reducing them. Hand hygiene procedures are fully performed by less than half of healthcare workers and research confirms a low compliance coefficient for hygiene (at the level of 1.4–6). The list of factors influencing the adherence to hand hygiene routine protocols (as per WHO guidelines) includes factors related to poor hand hygiene practices, poor hand hygiene practices reported by staff and additional perceived obstacles to good hand hygiene. The following are the barriers and reasons why healthcare workers do not sufficiently adhere to the correct technique of washing and disinfecting hands: forgetting to perform this procedure, excessive workload, haste during routine care activities, lack of a pattern of behavior among other employees and superiors, lack of habits, as well as a shortage of disinfectants and disposable towels in patient rooms, or irritation of the skin of the hands caused by the disinfectant. To sum up, it can be concluded that hand hygiene is and will be an issue that requires continuous action and permanent education of healthcare personnel and recipients of healthcare services. The list of factors causing poor hand hygiene is quite long. It includes, among others, the level of knowledge and habits of medical personnel and requires constant updating, as in the place of confirmed and verified reasons for low rates, newer ones which are specific for the current situation, will appear. - Abstract
Surgical site infections (SSI) are some of the most common and serious postoperative complications. SSI may lead to longer hospitalization time, higher costs of treatment, and even death. The relevant components of the prevention of surgical site infections are: knowledge of SSI risk factors, the course of a surgical procedure and the hospital environment, as well as effective surveillance based on epidemiological indicators. Such knowledge may reduce this risk to a minimum. Activities undertaken as part of epidemiological surveillance of the operated site should include clear and consistent procedures that will allow for optimal preparation of the patient for surgery and reduce the risk of SSI at every stage of hospital care.
