Resumen: La pandemia de influenza A(H1N1) de 2009 representó un acontecimiento crucial en el siglo XXI al ser identificada como la primera pandemia del presente siglo, recordando una vez más la vulnerabilidad de las poblaciones ante los virus respiratorios. Este artículo busca explorar, las lecciones aprendidas de la pandemia de Influenza A H1N1, especialmente relevantes en el actual panorama epidemiológico. Se analizan las características del virus, su patogénesis, los síntomas clínicos y los factores de riesgo asociados. Adicionalmente, se examina la importancia de la terapia antiviral precoz, los posibles efectos adversos ligados al uso de corticosteroides y las complicaciones infecciosas no virales, como la neumonía necrotizante por Staphylococcus aureus y el síndrome de shock tóxico estreptocócico. Esta revisión pretende ser un recurso para comprender y abordar, desde la prevención hasta el tratamiento, las neumonías causadas por influenza y sus complicaciones. Abstract: The influenza A(H1N1) pandemic of 2009 marked a pivotal event in the 21st century, as it was recognized as the first pandemic of that era, once again highlighting the vulnerability of populations to respiratory viruses. This article aims to explore the lessons learned from the Influenza A H1N1 pandemic, particularly pertinent to the current epidemiological landscape. Viral characteristics, pathogenesis, clinical symptoms, and associated risk factors were analyzed. Additionally, the significance of early antiviral therapy, potential adverse effects linked to corticosteroid use, and non-viral infectious complications, such as necrotizing pneumonia caused by Staphylococcus aureus and streptococcal toxic shock syndrome, were examined. This review aspires to be a valuable resource for understanding and addressing influenza-induced pneumonia and its complications from prevention to treatment.

- Abstract
BACKGROUND: Cholangitis is a fibroinflammatory disease, almost always due to obstructive jaundice, which is difficult to differentiate from primary sclerosing cholangitis by imaging; its diagnosis is based on elevated serum IgG4 concentrations and histopathological data. OBJECTIVE: To describe the concept and current classification of SC-IgG4 according to different associations, as well as the pathogenic, epidemiological, clinical and therapeutic aspects. METHODOLOGY: Bibliographic search in the databases GoogleScholar, PubMed, Medline LILACS and Scopus, articles in English and Spanish were included. RESULTS: From the bibliographic review, 60 articles were obtained that met the inclusion criteria, of which 13 were discarded due to several reasons (letters to the editor, case reports and duplicate reviews). To date there are no unified criteria in relation to the diagnosis, classification and management of SC-IgG4. Similarities and differences in relation to the diagnostic criteria of SC-IgG4 between the Endoscopy Society of Japan and the Gastroenterology Societies of Europe and the United States were found. A significant proportion of SC-IgG4 cases present high levels of IgG4 in the blood and usually respond satisfactorily to corticosteroids. CONCLUSIONS: IgG4-related sclerosing cholangitis is a biliary manifestation of the disease related to IgG4, it is very difficult to diagnose and is often confused with cholangiocarcinoma and other types of cholangitis. IgG4-related sclerosing cholangitis has a very good prognosis and responds satisfactorily to corticosteroids. [ABSTRACT FROM AUTHOR]

Introducción: El asma es un trastorno inflamatorio crónico de las vías respiratorias, es tratada con corticoesteroides inhalatorios, que actúan sobre las vías respiratorias provocando efectos adversos sobre la salud bucodental. Objetivo: Evidenciar las afecciones bucales que se relacionan con el uso de corticosteroides para concientizar sobre las repercusiones de los mismos en la cavidad oral. Reporte de caso: Paciente de 4 años presenta lesión en el dorso de la lengua forma de placa blanquecina, refiere molestias ligeras, sensibilidad y ardor. La madre refiere que padece de asma y utiliza Salbutamol 100 mcg y Budesonida 200 mcg vía inhalatoria bucofaringea. Se retira la placa blanquecina quedando una zona eritematosa en el sector de la línea media de la lengua, rojiza, lisa y brillante con depapilación así también se evidencian lesiones cariosas en los incisivos primarios superiores. Se establece como diagnóstico candidiasis pseudomembranosa. Se indica enjuague con 5mL de nistatina 100.000 UI/mL por un minuto. A los 15 días, refiere gran disminución de las molestias y del ardor pero con persistencia de zonas lisas, brillantes y depapiladas, esta vez ubicadas enel sector anterior de la lengua, se indica continuar con el enjuague 1 vez al día antes de dormir durante 2 días más. A dos meses de haber iniciado el tratamiento se observa ausencia de candidiasis y se establece como diagnóstico secundario glositis migratoria benigna. Conclusión: El uso de corticoides repercute en la cavidad oral haciendo que el niño sea más propenso a padecer ciertas patologías orales como candidiasis y caries dental.

Corticosteroids are potent anti-inflammatories and immunomodulators used to treat various clinical conditions, including several that require chronic use of these drugs, placing patients at risk of complications and adverse effects. In this context, the objective of this study was to review the scientific literature on the harmful effects and consequences resulting from prolonged use or high doses of corticosteroids. For this purpose, a bibliographical search was carried out in the MEDLINE, LILACS and SCIENCE DIRECT databases and original articles and case reports published between 2017 and 2023 were selected for in-depth reading, with seven articles being included in the present review. The results indicate that there is an accumulation of damage related to corticosteroids over time of use, in addition to the association of these drugs with various side effects. The importance of the responsibility of health professionals is highlighted in prescribing corticosteroids only when strictly necessary, in a cautious manner, as well as monitoring patients under chronic use of these drugs. Long COVID is a term used to describe the long-terms effects of COVID-19 infection that continue for weeks or months after the patient has recovered from COVID-19. Long COVID is defined by the persistence of symptoms beyond 12 weeks from the onset of the disease. Corticosteroids are part of the treatment in this period with good results in controlling the disease; however, it is a predisposing factor for the development of avascular necrosis. We present a clinic case of a young man of 39 years old with diagnosis of avascular necrosis in his left hip, before the administration of corticosteroids for the treatment of COVID-19. There is a lack of consensus about the dosage and duration of steroids required to develop avascular necrosis. Some authors have reported that cumulative dose of 2,000 mg prednisone (or its equivalent) was required for avascular necrosis development. For patients with advanced avascular necrosis stages total hip arthroplasty is an attractive option with excellent outcomes in terms of pain relief and survivorship. [ABSTRACT FROM AUTHOR]

Introdução: O uso da infiltração no aparelho locomotor para benefício de pacientes portadores de doenças reumáticas remonta à década de 1950. Os corticosteroides são os fármacos mais usados nos procedimentos de infiltração intra e periarticulares. Seus efeitos se devem às suas propriedades anti-inflamatórias, antiproliferativas e de atrofia de sinóvia. Este protocolo foi desenvolvido para melhor padronização das infiltrações intra e periarticulares às cegas com uso de corticosteroides no Hospital Universitário Walter Cantídio da Universidade Federal do Ceará. A padronização das técnicas objetiva homogeneizar resultados de segurança e eficácia de tais procedimentos. Metodologia: Neste protocolo são discutidas as recomendações gerais sobre infiltrações articulares, indicações, contraindicações, técnicas de realização de procedimentos e os cuidados pós-infiltração. Resultados: Médicos assistentes e médicos residentes do Hospital Universitário Walter Cantídio passaram a seguir as instruções deste protocolo contribuindo para a padronização de terapias intra e periarticulares no hospital. Conclusão: As infiltrações com corticosteroides no aparelho locomotor podem beneficiar pacientes com certas condições reumáticas. A padronização das técnicas traz resultados de segurança e eficácia mais homogêneos.

11. A case of anaphylaxis induced by intravenous hydrocortisone.

Our case focuses on a 34-year-old woman, pregnant, with a history of allergic rhinitis treated with antihistamines as needed. She had no documented history of medication allergies. At 38 weeks she went into labor. Three days after delivery, she received intravenous hydrocortisone for postpartum pain management. During administration, the patient developed generalized urticaria, dysphonia, and dyspnea. She was brought to the emergency room and received intramuscular adrenaline and antihistamines with clinical improvement. Prick tests for methylprednisolone succinate 10 mg/ml and hydrocortisone succinate 10 mg/mL and 100 mg/ml were positive. In challenge testing, the patient tolerated dexamethasone and budesonide. A diagnosis of anaphylactic reaction to group A corticosteroids was made. The patient was instructed to avoid hydrocortisone, methylprednisolone, and prednisolone. Although rare, corticosteroid allergy has very important therapeutic consequences; therefore it is necessary to be alert and to offer a safe alternative to these patients demonstrating tolerance to other corticosteroids. 13. Non-surgical treatment of aseptic olecranon bursitis: A systematic review.

Olecranon bursitis (OB), characterized by inflammation and fluid collection in the olecranon bursa is a commonly encountered out-patient condition. The data is heterogeneous regarding a stepwise and standardized approach to aseptic OB treatment and the efficacy of intra-bursal corticosteroid injections (CSI). The objective of this review is to systematically evaluate the non-surgical treatment options for aseptic OB. This systematic review was conducted in accordance with PRISMA recommendations. The English and non-English literature search was performed in 5 medical databases to identify studies evaluating the treatment of OB. All included studies were evaluated for risk of bias (RoB) using the revised Cochrane RoB tool for randomized control trials (RCTs) and the Newcastle-Ottawa Scale (NOS) for case–control and cohort studies. For the final analyses, 2 RCTs and 2 observational studies were included. The RoB for the RCTs was high and both failed to demonstrate a significant difference in terms of the resolution of OB and bursal tenderness among various invasive and non-invasive treatment options. Corticosteroid injection (CSI) was associated with a significant decline in the duration of symptoms. However, it was associated with a higher number of complications including bursal infection and skin atrophy. Based on the available data, it appears that the clinical resolution of aseptic OB can occur with conservative methods if implemented earlier in the disease course. Although CSI is more effective than other treatments, it should be reserved for refractory cases because of a higher complication rate. [ABSTRACT FROM AUTHOR]

BACKGROUND: Meningitis due to Listeria monocytogenes is a severe and potentially mortal disease. CLINICAL CASE: A 62-year-old female patient, who suffered from a rheumatological disease which was treated with corticosteroids and disease-modifying drugs, that induced a chronic immunosuppressive state. The patient was admitted to our hospital due to agitation, headache and fever, for which a study of cerebrospinal fluid was carried out, where fine growth of Listeria monocytogenes sensitive to ampicillin, gentamicin, trimethoprim and imipenem was observed. The patient was treated with a scheme of ampicillin with gentamicin for 21 days, obtaining a satisfactory clinical evolution with negative cerebrospinal fluid cultures. She remained in the internal medicine service for 29 days and was discharged without neurological sequelae with biochemical, microbiological and radiological normalization. CONCLUSIONS: The adoption of preventive measures by health personnel against this infectious agent is suggested since the impact on our environment is unknown. [ABSTRACT FROM AUTHOR]

Sweet syndrome or acute febrile neutrophilic dermatosis is a non-infectious inflammatory skin reaction characterized by the association of acute fever, tender skin lesions localized on the face, neck, trunk and upper extremities, neutrophilia with histologically a diffuse dense neutrophilic infiltrate in the dermis. It is most often idiopathic but it can be associated with various diseases, caused by drugs or reveal neoplasia. We report a case of Sweet’s syndrome in a man who presented with a palmoplantar pustulosis associated with an articular symptomatology mimicking a SAPHO syndrome. [ABSTRACT FROM AUTHOR]

Toxic epidermal necrolysis is a serious dermatological entity associated with multiple organ failure and high mortality. The most frequent etiology is related to the use of drugs. It begins with a prodromal picture, followed by a rash consisting of erythematous or erythematous-violaceous macules and plaques that progress to multiple blisters, leaving extensive denuded areas and mucosal involvement. It requires multidisciplinary management in the intensive care unit that includes the services of Dermatology, Internal Medicine, Critical Medicine, Plastic Surgery, Emergencies and Anesthesiology. The management of this disease is based on life support measures and targeted pharmacological treatment. Avoiding complications is a priority to improve survival. [ABSTRACT FROM AUTHOR]

RESUMO Objetivo: Analisar e comparar as características de pacientes críticos com a COVID-19, a abordagem clínica e os resultados entre os períodos de pico e de platô na primeira onda pandêmica em Portugal. Métodos: Este foi um estudo de coorte multicêntrico ambispectivo, que incluiu pacientes consecutivos com a forma grave da COVID-19 entre março e agosto de 2020 de 16 unidades de terapia intensiva portuguesas. Definiram-se as semanas 10 - 16 e 17 - 34 como os períodos de pico e platô. Resultados: Incluíram-se 541 pacientes adultos com mediana de idade de 65 [57 - 74] anos, a maioria do sexo masculino (71,2%). Não houve diferenças significativas na mediana de idade (p = 0,3), no Simplified Acute Physiology Score II (40 versus 39; p = 0,8), na pressão parcial de oxigênio/fração inspirada de oxigênio (139 versus 136; p = 0,6), na terapia com antibióticos na admissão (57% versus 64%; p = 0,2) ou na mortalidade aos 28 dias (24,4% versus 22,8%; p = 0,7) entre o período de pico e platô. Durante o período de pico, os pacientes tiveram menos comorbidades (1 [0 - 3] versus 2 [0 - 5]; p = 0,002); fizeram mais uso de vasopressores (47% versus 36%; p < 0,001) e ventilação mecânica invasiva na admissão (58,1% versus 49,2%; p < 0,001), e tiveram mais prescrição de hidroxicloroquina (59% versus 10%; p < 0,001), lopinavir/ritonavir (41% versus 10%; p < 0,001) e posição prona (45% versus 36%; p = 0,04). Entretanto, durante o platô, observou-se maior uso de cânulas nasais de alto fluxo (5% versus 16%; p < 0,001) na admissão, remdesivir (0,3% versus 15%; p < 0,001) e corticosteroides (29% versus 52%; p < 0,001), além de menor tempo de internação na unidade de terapia intensiva (12 versus 8 dias; p < 0,001). Conclusão: Houve mudanças significativas nas comorbidades dos pacientes, nos tratamentos da unidade de terapia intensiva e no tempo de internação entre os períodos de pico e platô na primeira onda da COVID-19.

Introducción: el Síndrome Inflamatorio Multisistémico pediátrico (PIMS) asociado a Covid-19 está considerado como una complicación potencialmente mortal, post-COVID-19, que se caracteriza por un estado pro inflamatorio, con afectación a varios sistemas de órganos, de gravedad variable. La mayoría de la literatura publicada al respecto es de origen anglosajón, por lo que se hizo necesario investigar la literatura reportada en lengua hispana y lusófona, en América Latina. Metodos: Se desarrolló un estudio observacional, descriptivo. Se tomó como fuentes de información artículos científicos de investigaciones realizadas con la población pediátrica de países de habla hispana y lusófona, con diseño analítico, descriptivo, de series de casos. Se eliminaron revisiones sistemáticas, reportes de caso y artículos de pago. Se tomaron como resultados de interés las manifestaciones clínicas y de laboratorio; así como el tratamiento del PIMS. Resultados: Se incluyeron 15 investigaciones realizadas en países de América Latina.; con una población total de 305 pacientes en edad pediátrica con Síndrome Inflamatorio Multisistémico asociado a Covid 19 (PIMS). Manifestaciones clínicas: fiebre (80 % al 100 %), dolor abdominal (36%-100%), diarreas (39 %-100 %), vómitos (40%-100 %), exantema (47%-100 %), inyección conjuntival (27%-100%), shock (10%-100%), enfermedad Kawasaki-like (59%-100%), manifestaciones neurológicas (25%-60%), cardiovasculares: derrame pericárdico (17%-40%), disfunción ventricular (30%-50%). Manifestaciones analíticas: leucocitosis, linfopenia, trombocitopenia; incremento de reactantes de fase aguda (proteína C reactiva, procalcitonina, ferritina sérica), incremento de dímero D y fibrinógeno. Tratamiento general: tratamiento del choque, estabilizando hemodinámica, algoritmo ABC (A: manejo de la vía aérea; B: ventilación; C: circulación. Tratamiento específico: inmunoglobulinas intravenosas y/o corticoides; antibioticoterapia empírica, ácido acetil salicílico, a dosis antiinflamatoria o antiagregante, y heparinas de bajo peso molecular. Manejo de las complicaciones. Conclusiones: En los países de habla hispana y lusófona de América Latina, la presentación clínica, analítica y el manejo de los pacientes pediátricos con síndrome inflamatorio multisistémico asociado a Covid-19; no difieren de las descripciones en la literatura anglosajona.

BACKGROUND: Erosive pustular dermatosis is a neutrophilic dermatosis of chronic course, characterized by the appearance of pustules, erosions and scabs. Although it typically affects the scalp, it can appear in any cutaneous territory. The diagnostic-therapeutic management of this disease is a real challenge when it affects special areas. CLINICAL CASE: A 45-year-old male patient went under Mohs micrographic surgery for a dermatofibrosarcoma protuberans and he presented a surgical wound on right shoulder with an unfavourable healing. After months of treatment with a split-thickness skin graft, punch grafting and negative wound pressure therapy, the diagnosis of erosive pustular dermatosis secondary to negative wound pressure therapy and the practice of skin grafts was reached. A spectacular relieve of the wound was observed, and complete epithelialization was achieved in two weeks after the withdrawal of negative wound pressure therapy and the application of topical corticosteroids. CONCLUSIONS: We report to our knowledge the first case of erosive pustular dermatosis secondary to negative wound pressure therapy and skin graft. Being aware of erosive pustular dermatosis when treating a post-surgical wound may help to promptly detect this dermatological condition and might help to carry out an early alternative therapy to avoid more aggressive procedures. Urticarial vasculitis is considered a systemic vasculitis mediated by the deposition of immunocomplexes; clinically it is characterized by the appearance of wheal-like skin lesions that are usually painful or burning, persistent and may leave residual hyperpigmentation. In its more complex forms it can be associated with multiple organ involvement. We present the case of a 39-year-old female with a history of Systemic Lupus Erythematosus Erythematosus of twelve years of evolution, who presented cutaneous involvement, with persistent wheals and episodes of angioedema Biopsy of the lesions was compatible with leukocytoclastic vasculitis. Multiple therapeutic options were used, from systemic corticosteroids to immunosuppressive agents and even biologic therapy with a first cycle of Rituximab without satisfactory response In the face of failed therapies and refractory symptoms, a second cycle of Rituximab was offered, maintaining methotrexate in conjunction, with favorable results so far. We present a case of an 87-year-old nonsmoker female who recovered after infection by SARS-CoV2 and was readmitted two weeks later due to respiratory sepsis. Radiological imaging showed a significant radiological worsening with extensive areas of bronchopneumonia and ground-glass opacities suggestive of organizing pneumonia. Empirical treatment with meropenem 1g/8h was started; however, clinical worsening persisted with tachypnea and desaturation requiring heated high-flow nasal cannula oxygen therapy, with poor response. Methicillin-resistant Staphylococcus aureus was isolated both in nare screening and sputum, and 16S rRNA polymerase chain reaction in induced sputum was positive for P. jirovecii. Serum (1-3)-beta-D-glucan was normal, and blood cultures were sterile. Antibiotic therapy was adjusted with intravenous linezolid 600mg/12h and trimethoprim-sulfamethoxazole 1600/320mg/6h, plus methylprednisolone 40mg/day. Unfortunately, the patient had no response to optimized treatment and finally died. Opportunistic and resistant microorganisms superinfections should be aware by clinicians in SARS-CoV2 infection, even more, when corticosteroids are widely used.

El penfigoide es una enfermedad autoinmune con autoanticuerpos contra la membrana basal de los epitelios. El penfigoide cicatricial muestra su principal localización en la boca, ojos y otras mucosas siendo en este cuadro escasas las lesiones cutáneas. En muchos casos las lesiones orales son su primer signo clínico, permitiendo un diagnóstico precoz y la localización de las ampollas es el elemento clínico determinante para el diagnóstico de enfermedades ampollosas autoinmunitarias. El caso reporta paciente femenino de 39 años, que acude al área de Patología bucal de la Facultad de Odontología de la Universidad de Carabobo. En el examen clínico se observaron lesiones tipo ampollas en: mucosa yugal (izquierda y derecha), área retromolar, encía marginal en arcada inferior por vestibular y en la cara interna del labio inferior, algunas de ellas con contenido purulento y hemorrágico. Las lesiones son dolorosas a la palpación, ocasionando dificultades funcionales en consecuencia. Es importante destacar que son lesiones recurrentes, las cuales han sido tratadas anteriormente. Posterior a los exámenes clínicos e histopatológicos se diagnostica Penfigoide de las membranas mucosas, y se planifica tratamiento local y sistémico con corticosteroides, el objetivo es mostrar la experiencia clínica alcanzada luego de la realización del tratamiento quirúrgico y terapéutico.

Keloids and hypertrophic scars occur in 30-90% of patients. The two entities have different clinical and histochemical characteristics and unfortunately still represent a great challenge for clinicians due to the lack of effective treatments. Current advances reveal new preventive and therapeutic options that offer hope for managing this widespread, chronic and debilitating problem. Case report: We report the efficacy of the combination of fractionated carbon dioxide (FCO2) laser and intra-lesional 5-fluorouracil (5-FU) for the treatment of a mid-thoracic keloid scar resistant to intra-lesional infiltration steroids and pressotherapy. Discussion: The appearance of cortisone dermatitis at the site of corticosteroid infiltration makes it necessary to seek an effective therapeutic alternative to minimize the risk of recurrence and resistance without hesitating to combine several therapeutic means such as laser, 5-FU or Bleomycin and cryosurgery… Conclusion: Effort is needed to choose the treatment with maximum results. Objective: To analyze and compare COVID-19 patient characteristics, clinical management and outcomes between the peak and plateau periods of the first pandemic wave in Portugal. Methods: This was a multicentric ambispective cohort study including consecutive severe COVID-19 patients between March and August 2020 from 16 Portuguese intensive care units. The peak and plateau periods, respectively, weeks 10 - 16 and 17 - 34, were defined. Results: Five hundred forty-one adult patients with a median age of 65 [57 - 74] years, mostly male (71.2%), were included. There were no significant differences in median age (p = 0.3), Simplified Acute Physiology Score II (40 versus 39; p = 0.8), partial arterial oxygen pressure/fraction of inspired oxygen ratio (139 versus 136; p = 0.6), antibiotic therapy (57% versus 64%; p = 0.2) at admission, or 28-day mortality (24.4% versus 22.8%; p = 0.7) between the peak and plateau periods. During the peak period, patients had fewer comorbidities (1 [0 - 3] versus 2 [0 - 5]; p = 0.002) and presented a higher use of vasopressors (47% versus 36%; p < 0.001) and invasive mechanical ventilation (58.1 versus 49.2%; p < 0.001) at admission, prone positioning (45% versus 36%; p = 0.04), and hydroxychloroquine (59% versus 10%; p < 0.001) and lopinavir/ritonavir (41% versus 10%; p < 0.001) prescriptions. However, a greater use of high-flow nasal cannulas (5% versus 16%, p < 0.001) on admission, remdesivir (0.3% versus 15%; p < 0.001) and corticosteroid (29% versus 52%, p < 0.001) therapy, and a shorter ICU length of stay (12 days versus 8, p < 0.001) were observed during the plateau. Conclusion: There were significant changes in patient comorbidities, intensive care unit therapies and length of stay between the peak and plateau periods of the first COVID-19 wave. 32. Systematic Review of Systemic Corticosteroids for Treatment of Organizing Pneumonia

Introduction: Regardless corticosteroids are recommended for the treatment of organizing pneumonia there is limited evidence supporting this practice. Thus, we performed a systematic review of the literature on systemic corticosteroid treatment for organizing pneumonia. Methods: A search was implemented in the PubMed database (Medline) for articles published in the last 20 years. Those studies with incomplete or insufficient data and case reports were excluded. We collected data including: demographics, clinical data, diagnostic procedures, aetiology, treatment regimen (drug, posology, duration, response) and evolution. Results: A total of 135 publications were selected and finally 13 studies with 849 patients were included in the review: 12 retrospective observational studies and a single prospective observational study. Most of the patients were started on treatment with systemic corticosteroids – a total of 627 (30–100% depending on the series), but there was a great heterogeneity regarding drug, doses and duration. On those that started treatment, 226 (36%) presented a relapse of the disease during follow-up. Only one study provided information regarding treatment side-effects. Conclusion: The findings of this systematic review show the low quality data supporting the use of corticosteroids for the treatment of organizing pneumonia. This highlights a need to undertake appropriately designed studies to investigate which is the most appropriate treatment regimen that trades off benefits and risks of prolonged corticosteroid administration. Resumen: Introducción: Aunque los corticosteroides están recomendados para tratar la neumonía organizada, hay pocos datos que respalden esta práctica, por lo cual efectuamos una revisión sistemática de la bibliografía sobre el tratamiento con corticosteroides sistémicos para la neumonía organizada. Métodos: Se hizo una búsqueda en la base de datos PubMed (Medline) de artículos publicados en los últimos 20 años. Se descartaron los estudios con datos y casos clínicos incompletos o insuficientes. Los datos que recabamos abarcaron: datos demográficos, datos clínicos, técnicas diagnósticas, etiología, pauta terapéutica (fármaco, posología, duración, respuesta) y evolución. Resultados: Se eligieron 135 publicaciones en total y se incorporaron finalmente a la revisión 13 estudios con 849 pacientes: 12 estudios observacionales retrospectivos y un solo estudio observacional prospectivo. La mayor parte de los pacientes habían comenzado el tratamiento con corticosteroides sistémicos, un total de 627 (30%-100% en función de la serie), pero la duración, las dosis y el fármaco manifestaron una gran heterogeneidad. Entre los que habían empezado el tratamiento, 226 (36%) presentaron una recidiva de la enfermedad durante el seguimiento. Solo en un estudio se ofreció información sobre los efectos adversos del tratamiento. Conclusión: Los resultados de esta revisión ponen de manifiesto la escasa calidad de los datos sobre el tratamiento de la neumonía organizada con corticosteroides. Este hecho destaca la necesidad de emprender estudios diseñados correctamente para investigar la pauta terapéutica más adecuada que compense los riesgos y beneficios de la administración prolongada de corticosteroides.

L’ivermectine, un agent antiparasitaire, n’est pas recommandé pour la prophylaxie ou le traitement de la maladie à coronavirus 2019 (COVID-19). L’utilisation inappropriée de l’ivermectine pour le traitement de la COVID-19 peut en diminuer la disponibilité pour les patients atteints d’infections parasitaires graves qui pourraient en bénéficier et aggraver la pénurie actuelle de ce médicament au Canada. Cependant, les patients atteints de COVID-19 qui sont des candidats pour recevoir des thérapies immunomodulatrices (e.g. corticostéroïdes et inhibiteurs de l’interleukine-6) peuvent être exposés au risque de syndrome d’hyperinfection et de la maladie disséminée à Strongyloides stercoralis. Ces complications peuvent être graves, voire mortelles. Il est important de reconnaître et de dépister les patients qui présentent des risques de strongyloïdose, car ces patients peuvent avoir besoin d’un traitement à l’ivermectine pour éviter le risque d’un syndrome d’hyperinfection et d’une maladie disséminée, qui est souvent mortelle. - Abstract
Spinal cord compression syndrome is defined as the symptoms and signs produced by compression of the spinal cord or nerve roots that form the cauda equina due to any lesion related to underlying neoplastic disease. It is the second most frequent neurological complication in cancer patients after brain metastases. It is estimated that it affects approximately 5% of cancer patients. It occurs mainly as a complication in patients affected by lung, breast, prostate cancer and multiple myeloma. Spinal cord compression syndrome is an oncological emergency in which early diagnosis and treatment largely determine the patient's functional prognosis. Its symptoms generally develop gradually over hours, days, or weeks, which varies depending on the biological behavior of the primary tumor. Magnetic resonance imaging is considered the imaging study of choice for the diagnosis of spinal cord compression syndrome. Treatment includes corticosteroids, radiation therapy, surgery, and chemotherapy. A search for original articles, systematic and narrative reviews was performed in the PubMed and Scielo database. From the review of 45 selected articles, a description of the pathophysiology and clinical manifestations of this syndrome was made. The hiccup or singulto can be considered a banal symptomatology but it can become a very disabling factor for patients. The relationship between oral corticosteroids and the appearance of hiccups is well known, but the relationship between the two by peridural procedure is much less described, with only six cases published to date. We present the case of a young patient without basic treatment, in whom the only risk factor for the onset of this symptomatology was the performance of an epidural-caudal technique with betamethasone as treatment for left lumbociatalgia. Objective: To estimate the rate of the use of antenatal corticosteroids (ANC) among pregnant women and to identify the conditions associated with their non-use in Brazil. Methods: Secondary data analysis from "Birth in Brazil", a national hospital-based survey carried out in 2011-2012 on childbirth and birth. The sample was characterized regarding maternal age, marital status and maternal education, parity, mode of delivery and place of residence. The association of ANC use with gestational age and type of delivery was analyzed. The studied maternal complications were the presence of hypertension, preeclampsia/eclampsia, and pyelonephritis, infection by the HIV virus or acquired immune deficiency syndrome. Results: 2,623 pregnant women with less than 37 weeks of gestational age were identified, and, of these, 835 (31.8%) received ANC. The frequency of ANC use was higher among women with gestational ages between 26-34 weeks (481 cases; 48.73%). In pregnancies with less than 37 weeks, the use of ANC was 23.9% in spontaneous deliveries, 20.6% in induced deliveries and 43.8% among those who did not go into labor. The variables vaginal delivery (OR 2.5; 95%CI 1.8-3.4) and living in the countryside were associated with not using ANC, and the occurrence of preeclampsia/eclampsia (OR 1.8; 95%CI 1.2-2.9) was associated with the use of ANC. Conclusions: The use of ANC among Brazilian pregnant women was low. Interventions to increase its use are necessary and can contribute to reduce neonatal mortality and morbidity. ANC should be promoted in pregnancies of less than 37 weeks, especially in cases of vaginal delivery and for those living in the countryside. [ABSTRACT FROM AUTHOR]

LR has identifiable etiology and can be triggered by systemic drug exposure or any other agents. LR occurs in all racial groups and is predominantly observed in young adults aged 20– 40 years, but can occur in any age group. Typically, the clinical manifestation of LR is an itchy rash of small (± 3– 5 mm) shiny, raised, reddish-purple papules. The rash appears suddenly and commonly affects the anterior surface of the arm and hand and the back part of the body. We report a rare case of LR to antihistamines (cetirizine, loratadine) and corticosteroid (methylprednisolone): an adult female 32-year-old patient, with a chief complaint of burning sensation, a few small rashes and itching on the anterior surface of the arm after eating seafood and eggs 7 days prior to admission. From the detailed and chronologic anamnesis of her dietary habits, routine use of drugs and oral hygiene, and from the histopathological results, we concluded the patient has lichenoid reaction caused by drugs (antihistamines and corticosteroids).Keywords: lichenoid reaction, hypersensitivity, antihistamines, corticosteroids

We present a case of an 87-year-old nonsmoker female who recovered after infection by SARS-CoV-2 and was readmitted two weeks later due to respiratory sepsis. Radiological imaging showed a significant radiological worsening with extensive areas of bronchopneumonia and ground-glass opacities suggestive of organizing pneumonia. Empirical treatment with meropenem 1g/8h was started; however, clinical worsening persisted with tachypnea and desaturation requiring heated high-flow nasal cannula oxygen therapy, with poor response. Methicillin-resistant Staphylococcus aureus was isolated both in nasal screening swab and sputum, and RNA polymerase chain reaction in induced sputum was positive for P. jirovecii. Serum (1-3)-beta-D-glucan was normal, and blood cultures were sterile. Antibiotic therapy was adjusted with intravenous linezolid 600mg/12h and trimethoprim-sulfamethoxazole 320/1600mg/6h, plus methylprednisolone 40mg/day. Unfortunately, the patient had no response to optimized treatment and finally died. Clinicians should be aware of opportunistic and resistant microorganisms superinfections in relation to SARS-CoV-2 infection, even more, when corticosteroids are widely used. [ABSTRACT FROM AUTHOR]

Background: Steroids are the mainstream drugs of immunosuppressive regimen in renal transplantation. They are successfully used on induction, maintenance and rejection treatment. Due to complications caused by steroids, treatments are switched to immunosuppressive agents. Graft dysfunction risk caused by reduced total immunosuppression disturbs clinicians very often. We documented the differences among patients by means of clinical presentation and PRA/DSA levels between patients who are using steroids and patients that were prescribed for steroid-free regimen. Methods: 82 individuals who did not use steroid and 52 patients on steroid treatment were included with similar rates of age, sex, primary renal disease, dialysis type, posttransplant follow-up duration and donor type. Pre and posttransplant PRA, DSA levels, posttransplant and current graft function and comorbidities were evaluated. Results: Individuals who do not use steroids were found to have a lower posttransplant creatinine level and glomerular filtration rate (GFR) compared to steroid users. Posttransplant and current spot urinary protein/creatinine rates were also lower in the steroid-free group. However DM, BKVN and induction therapy rates were higher in the steroid-free group. PRA and DSA levels were similar in both groups. On the other hand, posttransplant PRA-I levels were significantly higher in those with less steroid use time. Conclusions: Although steroid free regimens usually worry the clinicians, they can be preferred in patients with low immunological risk for rejection to avoid its side effects such as uncontrolled diabetes, obesity, musculoskeletal problems and cataracts. Introdução: A síndrome de Churg Strauss (SCS) é uma doença autoimune de etiologia indeterminada. Seu diagnóstico é difícil, não somente pela raridade, mas também pela sobreposição clínica e anatomopatológica que pode haver entre diferentes vasculites; por este motivo, pode ter sua prevalência subestimada. Manifestações sistêmicas incluem mialgia, febre, perda de peso, artralgia, alterações cutâneas, neuropatia periférica, envolvimento pulmonar, de trato gastrintestinal e cardiomiopatia. Comumente são utilizados glicocorticoides para combater os efeitos da doença, entretanto, apresentam efeitos colaterais importantes, como obesidade centrípeta, face em lua cheia, giba ou corcunda de búfalo, osteoporose, fraqueza, miopatia, atrofia muscular; necrose asséptica da cabeça do fêmur e úmero, além da depressão do sistema imunológico. Objetivo: Relatar um caso singular de uma paciente diagnosticada com SCS por 10 anos, que realiza tratamento com corticosteroides e treinamento físico para combater os efeitos colaterais do tratamento farmacológico. Relato de caso: Para combater os efeitos colaterais desse tratamento, foi adotado o treinamento de força (TF) e corrida. A paciente realizou TF três a cinco vezes por semana durante todo o período de tratamento até o momento. A carga utilizada em seus treinamentos variou entre 50 a 70% de 1 Repetição Máxima, de acordo com a periodização, além de corridas duas vezes por semana com percepção subjetiva de esforço entre 4 e 6. Regularmente foram realizados exames de sangue para análise de eosinófilos, densitometria óssea e avaliações antropométricas. O treinamento mostrou indícios de ser útil na manutenção da composição corporal e da densidade mineral óssea (DMO). Conclusão: O TF e o treinamento aeróbio são estratégias importantes no combate aos efeitos da SCS e do tratamento com corticosteroides.

Background & Objective: Ulcerative colitis (UC) is an inflammatory bowel disease, usually begins in the rectum and can involve the entire colon. Damage done by this condition can lead to several intestinal and extraintestinal manifestations (EIMs). Due to malnutrition, malabsorption and medications used in UC patients, bone mineral loss seems to happen at a higher pace, compared to healthy population. In this study, we aim to investigate bone mineral density (BMD) and its related biomarkers like Vitamin K, Vitamin D and Calcium in patients with UC. Method: One hundred and one documented UC patients with at least one year of treatment entered our study. BMD was measured with dual X-ray absorptiometry. Lab tests were done to examine Vitamin K, Vitamin D, Calcium and Magnesium levels in patients’ serum. Results: Among 101 patients, 32 (31.7%) were men and 69 (68.3%) were women. The mean age of participants was 32.15 ± 9.46 years. The mean duration of the disease was 6.24 ± 3.30 years. Based on the femoral T-score index, 75 (74.3%) patients had normal femoral density, 24 (23.8%) were osteopenic and (2%) 2 patients had osteoporosis. Bone loss significantly correlated with age and disease. Patients with lower BMI tend to have lower BMD values. The data analysis revealed lower levels of Vitamin K, Vitamin D and Calcium in UC patients significantly decreased bone density indexes, except for lumbar T-score which did not showed a clear relationship with serum Vitamin K. Also, the cumulative dose of prednisolone significantly reduced all BMD indicators, especially in patients with longer disease durations. Conclusion: BMD in UC patients seems to decrease at a higher pace and it's associated with lower levels of Vitamin K, Vitamin D and Calcium in serum as well as corticosteroid consumption. Close follow-ups, dietary supplements and corticosteroid usage limitations could be beneficial for UC patients and decrease the risk of developing osteoporosis. Resumen: Antecedentes y objetivo: La colitis ulcerosa (CU) es una enfermedad inflamatoria intestinal, generalmente comienza en el recto y puede afectar todo el colon. El daño causado por esta condición puede conducir a varias manifestaciones intestinales y extraintestinales (EIM). Debido a la desnutrición, la malabsorción y los medicamentos utilizados en los pacientes con CU, la pérdida de minerales óseos parece ocurrir a un ritmo más rápido, en comparación con la población sana. En este estudio, nuestro objetivo es investigar la densidad mineral ósea (DMO) y sus biomarcadores relacionados como la vitamina K, la vitamina D y el calcio en pacientes con CU. Método: Ciento uno pacientes con CU documentados con al menos un año de tratamiento ingresaron a nuestro estudio. La DMO se midió con absorciometría de rayos X dual. Se realizaron pruebas de laboratorio para examinar los niveles de vitamina K, vitamina D, calcio y magnesio en el suero de los pacientes. Resultados: Entre 101 pacientes, 32 (31,7%) eran varones y 69 (68,3%) eran mujeres. La edad media de los participantes fue de 32,15 ± 9,46 años. La duración media de la enfermedad fue de 6,24 ± 3,30 años. Según el índice de puntuación T femoral, 75 (74,3%) pacientes tenían densidad femoral normal, 24 (23,8%) pacientes eran osteopénicos y 2 (2%) pacientes tenían osteoporosis. La pérdida ósea se correlacionó significativamente con la edad y la enfermedad. Los pacientes con un IMC más bajo tienden a tener valores de DMO más bajos. El análisis de datos reveló niveles más bajos de vitamina K, vitamina D y calcio en pacientes con CU que disminuyeron significativamente los índices de densidad ósea, excepto para la puntuación T lumbar que no mostró una relación clara con la vitamina K sérica. Además, la dosis acumulada de prednisolona redujo significativamente todos los indicadores de DMO, especialmente en pacientes con enfermedad de mayor duración. Conclusión: La DMO en pacientes con CU parece disminuir a un ritmo más rápido y está asociada con niveles más bajos de vitamina K, vitamina D y calcio en suero, así como con el consumo de corticosteroides. Los seguimientos minuciosos, los suplementos dietéticos y las limitaciones en el uso de corticosteroides podrían ser beneficiosos para los pacientes con CU y disminuir el riesgo de desarrollar osteoporosis.

A Vasculite por imunoglobulina A (IgAV) é conhecida por ser uma vasculite sistêmica de pequenos vasos. Apesar desta condição ser capaz de se estabelecer em qualquer faixa etária, o perfil epidemiológico desta doença afeta, majoritariamente, a população pediátrica situada entre 2 e 10 anos de idade. Nesse sentido, este artigo descritivo, tem como objetivo relatar o raro caso de uma paciente adulta que evoluiu com a IgAV, apresentação de forma cutânea, sendo tratada com corticosteroide. Para tanto, após consentimento livre e esclarecido da paciente foi aprovado pelo Comitê de Ética em Pesquisa da Universidade de Rio Verde (UniRV – GO), cujo Certificado de Apresentação para Apreciação Ética é 36621920.0.0000.507. Logo, descobriu-se que a paciente evoluiu com clínica compatível para IgAV de provável acometimento cutâneo pelas lesões em forma de púrpura e pelo envolvimento renal, apresentando boa resposta terapêutica com a corticoterapia em doses não imunossupressoras e após 1 ano e 3 meses de follow-up houve remissão do quadro sem novos episódios. Encontra-se na literatura alguns relatos sobre a boa resposta terapêutica desta forma clínica com a corticoterapia em doses não imunossupressoras, no entanto ensaios clínicos são necessários.

50. ANÁLISE COMPARATIVA DO EFEITO DO ESTRESSE INDUZIDO POR HORMÔNIO NA HEMOSTASE EM CYPRINUS CARPIOE OREOCHROMIS NILOTICUS.

Background: The mortality of freshwater fish due to stress during various production manipulations is a severe problem, which requires a thorough understanding of the basic mechanisms involved, including the hemostasis system. Therefore, on the application level, the study of blood clotting can perform fish coagulopathies diagnostics and develop practical preventive and therapeutic anticoagulation methods for fish farming. Aim: The goal of this research was a comparative assessment of the reaction of some hemostasis parameters of two commercial fish species, carp Cyprinus carpio and tilapia Oreochromis niloticus, to the stress of different duration induced by corticosteroids. Methods: The fishes were divided into three groups: chronically stressed (induction by betamethasone), acutely stressed (induction by dexamethasone), and control animals with blood taken from the caudal hemal canal before hormone treatment(by dexamethasone and betamethasone), then 7 and 21 days after. Results and Discussion: Changes in the following parameters were studied: thrombin time, prothrombin time, activated partial thromboplastin time, the concentration of fibrinogen, soluble fibrin monomer complexes, antithrombin. It was found that both hormone-induced stress and handling stress associated with blood sampling strongly increased blood clotting ability in carps (prothrombin time decrease by 78,5-86,1%, fibrinogen increase by 12,7-100%, thrombin time decrease by 83,4-85%, and antithrombin III decreases by 15,3-21,7%), while in tilapias, acceleration of blood clotting by intrinsic and extrinsic pathways were recorded by the end of the experiment only in fishes with imitation of chronic stress (prothrombin time decrease by 76,8%, activated partial thromboplastin time decrease by 20,0%, and 2,3 multiplying soluble fibrin monomer complexes). Conclusions: It was concluded that the adaptive mechanisms of the tilapia (Oreochromis niloticus) organism allowed the clotting function to recover in most cases by the end of the experiment in all groups of fish, in contrast to carps (Cyprinus carpio). Смертность пресноводных рыб в результате стресса при различных манипуляциях на производстве представляет серьезную проблему, что требует глубокого понимания основных задействованных механизмов, в том числе системы гемостаза. На прикладном уровне проведение исследования свертывания крови имеет потенциал для диагностики коагулопатий рыб и разработки практических профилактических и терапевтических противосвертывающих методов для рыбоводства. Цель: Целью этого исследования была сравнительная оценка реакции некоторых параметров гемостаза двух видов промысловых рыб - карпа Cyprinuscarpio и тиляпии Oreochromisniloticus в условиях экспериментальной модели стресса различной продолжительности, индуцированного кортикостероидами. Методы: Рыбы были подразделены на три группы: рыбы с хроническим стрессом (индукция бетаметазоном), острым стрессом (индукция дексаметазоном) и контрольные животные, кровь у которых брали из хвостового гемального канала до обработки гормонами, и далее через 7 и 21 день после. Результаты и Обсуждение: Было исследовано изменение следующих параметров коагулограммы: тромбиновое время, протромбиновое время, активированное частичное тромбопластиновое время, концентрацию фибриногена, растворимых фибрин-мономерных комплексов, антитромбина. Было установлено, что как индуцированный гормонами стресс, так и хэндлинг-стресс, связанным с забором крови, сильно увеличивают свертывающую способность крови у карпов (сокращение протромбинового времени на 78,5-86,1%, увеличение фибриногена на 12,7-100%, сокращение тромбопластинового времени на 83,4-85% и антитромбинаIIIна 15,3-21,7%), тогда как у тиляпий ускорение свертывания крови по внутреннему и внешнему путям к концу эксперимента было зафиксировано только у рыб с имитацией хронического стресса (снижение протромбинового времени на 76,8%, снижение активированного частичного тромбопластинового времени на 20,0%, увеличение фибрин-мономерных комплексов в 2,3 раза). Заключение: Можно отметить, что адаптивные механизмы организма тиляпий позволили свертывающей функции в большинстве случаев восстановиться к концу эксперимента у всех групп рыб, в отличие от карпов.
