Comma and corkscrew hairs are trichoscopic markers of tinea capitis. Although comma hairs have been reported in both black and white patients with tinea capitis, corkscrew hairs were previously hypothesized to manifest exclusively in patients with curly or African hair types. However, we report a significant number of comma and corkscrew hairs in a Hispanic female patient with naturally straight hair and Trichophyton tonsurans tinea capitis. Thus, dermoscopy is a rapid, noninvasive, and cost-effective tool for evaluating tinea capitis and should be used in combination with culture in all patients regardless of hair texture or race.

The culprit species differ across the continents; for instance, Microsporum canis prevails in Europe, whereas Trichophyton tonsurans dominates in North America. Oral medication is always* the main required remedy as TC does not respond well to topical therapy unaided. However, in some countries, griseofulvin is no longer available. Fungal culture should be included in the study of persistent, scalp lesions and trichoscopy can offer a speedy diagnosis by its characteristic findings. Scalp dermoscopy or “trichoscopy” represents a valuable, noninvasive technique for the evaluation of patients with hair loss due to TC. It is simple, quick, and easy to perform.

Scurvy is a rare condition characterized by a deficiency in dietary vitamin C. Historically a disease taught in the context of long ocean voyages with limited vitamin intake, it is now rare in developed nations. The classical physical exam findings include gingival bleeding, perifollicular hemorrhages, and corkscrew hairs. We discuss the case of a 15-year-old female with scurvy whose initial presentation suggested more common diagnoses seen in the emergency department setting. Her course was complicated by a prior history of anorexia nervosa and a restrictive diet that lacked necessary vitamins. Once the patient's dietary habits were identified, a detailed physical exam revealed the characteristic findings. Dermoscopy is a method of growing significance in the diagnoses of dermatological pigmented skin diseases. However, in my case, mycology culture was negative and successful treatment was given on the basis of trichoscopy and wood lamp examination. I hereby describe a young boy with tinea capitis, multiple "comma hairs" and "zigzag hair" and a subtle additional feature "Morse code-like hair" when intensification was applied. Tinea capitis is a frequent dermatophytosis of childhood. Clinically, it can be divided into its dry or inflammatory presentation, the former accounts for up to 90% of all cases. Diagnosis is made by microscopic examination of tonsured hair with potassium hydroxide (KOH), in which parasitation of the hair can be observed, or by isolation of the dermatophyte in Sabouraud agar. The dermoscopic examination of the hair and scalp is known as trichoscopy. It has proven to be very useful in the diagnosis and screening of some hair disorders, among them, infectious diseases such as tinea capitis. The presence of certain trichoscopic features allows identifying and differentiating between these entities. The most commonly reported trichoscopic features in tinea capitis patients have been coma hairs and corkscrew hairs, which have been described as markers of this particular disease. A 17-year-old girl presented with multiple perifollicular hemorrhagic lesions on legs for 2 weeks. There was history of intermittent gum bleeding for last 2 months. Clinical examination revealed follicular keratosis and broken twisted hair (corkscrew hair), in addition to hemorrhagic lesions. Perifollicular hemorrhagic lesions and corkscrew hair are virtually pathognomonic for scurvy. When specifically asked, mother revealed that the girl was on self-imposed diet with almost no citrus fruits. Based on history and clinical findings, diagnosis of scurvy was made. The response to vitamin C supplementation was very good and all the lesions disappeared within one month. Scurvy has been reported in increasing numbers in recent literature; hence, recognition of scurvy is essential to avoid many serious complications including internal bleeding and even, death.
