Cardiovascular magnetic resonance has emerged as a very helpful tool for the interventional cardiologists not only in the assessment and treatment of coronary artery disease, but also in the evaluation of various structural cardiac diseases. The main pulse sequences are standardised, acquired during short breath-holds, and include steady-state free precession cines, dynamic myocardial first-pass perfusion imaging during contrast injection, and late enhancement imaging for the identification of myocardial substrates. Less than 30-minute CMR studies are now available for the most common clinical indications. More recently, T1 and T2 parametric myocardial maps are promising for detailed myocardial tissue characterisation (edema, replacement fibrosis, diffuse interstitial fibrosis). La communication interauriculaire (CIA) constitue la seconde cardiopathie congénitale la plus importante chez l'enfant, elle représente 6 à 8% des malformations congénitales cardiaques de l'enfant. Toutefois, de nombreuses questions demeurent ouvertes concernant cette pathologie. Ainsi notre objectif était de rapporter la prévalence hospitalière des CIA mais surtout d'en décrire les aspects cliniques, paracliniques et thérapeutiques. Pour y parvenir nous avons réalisé une étude rétrospective descriptive. Les données ont été recueillies grâce à un questionnaire puis saisies et analysées sur Sphinx (V5). La prévalence hospitalière était de 2 pour 1000. L'âge moyen était de 37mois et le sex-ratio de 0,75. Il n'y a eu aucun cas diagnostiqué en anténatal. Une consanguinité parentale était retrouvée dans 30% des cas. Les infections respiratoires étaient présentes dans 24% des cas. Une cardiomégalie était retrouvée dans 35 cas avec une hyper vascularisation pour 63%. L'échographie notait une prédominance du type ostium secundum; la CIA était large dans 63% des cas. La sténose pulmonaire représentait l'atteinte cardiaque associée la plus importante. L'HTAP était présente dans 63% des cas. Les diurétiques étaient très utilisés et seuls 7 enfants avaient bénéficié d'une cure chirurgicale. L'évolution était favorable chez 39 patients soit 79%. Le diagnostic précoce des CIA doit être amélioré ainsi que la prise en charge chirurgicale des formes avec retentissement du nourrisson.

Interauricular communication (IAC) is the second most important congenital heart disease in children. It accounts for 6-8% of congenital cardiac malformations in children. However, many questions are still open about this pathology. Thus, this study aims to report the prevalence rate of IAC in hospital but especially to describe its clinical, paraclinical and therapeutic aspects. For this purpose, we conducted a retrospective descriptive study. Data were collected by means of a questionnaire and then entered and analyzed in Sphinx (V5). The prevalence in Hospital was 2x1000. The average age of patients was 37 months, the sex-ratio was 0,75. No prenatal diagnosis was made. Parental consanguinity was detected in 30% of cases. Respiratory infections were found in 24% of cases. Cardiomegaly was found in 35 cases with hypervascularization in 63% of cases. Ultrasound showed a predominance of ostium secundum; wide IAC was found in 63% of cases. Pulmonary stenosis was the most important associated heart attack. Pulmonary arterial hypertension was found in 63% of the cases. Treatment was based on diuretics, which were largely used and only 7 children underwent surgery. Evolution was favorable in 39 patients reflecting a rate of 79%. Early diagnosis of IAC should be improved as well as surgical treatment of IAC affecting infants. [ABSTRACT FROM AUTHOR]

Résumé L’embolie paradoxale doit être suspectée devant une pathologie thromboembolique associée à un shunt anatomique droit–gauche. Les autres étiologies cardioemboliques doivent être éliminées surtout pour la pathologie concernée la plus fréquente qui est l’accident vasculaire cérébral. La communication interauriculaire, le foramen ovale perméable et les malformations artérioveineuses pulmonaires peuvent être responsables d’un shunt droit–gauche. La responsabilité d’un foramen ovale perméable lors de la survenue d’un accident vasculaire cérébral peut être évaluée par un score. La prévention secondaire neurologique de l’embolie paradoxale repose sur des traitements antithrombotiques et/ou interventionnels. Leur place respective commence à être mieux définie grâce à des études randomisées récentes. Paradoxical embolism should be suspected in front of a clinical phenomenon of thromboembolism associated with an anatomical right-to-left shunt. Others potential cardiac sources of thromboembolism must be ruled out. Strokes constitute the most frequent clinical manifestations of paradoxical embolism. Right-to-left left shunts are in connection with intracardiac defects (atrial septal defect and patent foramen ovale) or pulmonary arteriovenous malformations. The probability that a discovered PFO is stroke-related can be evaluated by a score. Therapeutic approaches for secondary prevention of recurrent stroke include antithrombotic and/or percutaneous treatments. Nous rapportons une observation d'un nourrisson de 5 mois présentant une transposition des gros vaisseaux associée aux communications interventriculaire et interauriculaire. Il est né à terme sans aucun facteur de risque retrouvé dans les antécédents maternels. Le diagnostic est posé, grâce à une échocardiographie, à 5 mois après sa naissance lors de la survenue d'une cyanose et d'un malaise anoxique. Une prise en charge symptomatique a permis de stabiliser l'état du patient mais suite à l'absence d'un traitement chirurgical, il est décédé à domicile 3 semaines après sa sortie de l'hôpital. Dans les pays en développement, le diagnostic de la transposition des gros vaisseaux est souvent fait en période postnatale et son pronostic reste fatal par manque des centres médico-chirurgicaux spécialisés.

Résumé: Le syndrome de Lutembacher associe une communication interauriculaire (CIA) congénitale et un rétrécissement mitral (RM). Il est rarement associé à un retour veineux pulmonaire anormal (RVPA) partiel qui dilate encore plus l’oreillette droite. Cette condition relève d’un traitement chirurgical de commissurotomie mitrale ou de remplacement valvulaire mitral, fermeture de la communication interauriculaire avec correction du retour veineux. La dilatation mitrale percutanée préalable à la chirurgie peut être une alternative thérapeutique si le rétrécissement mitral est serré et l’état anatomique valvulaire favorable. Les auteurs rapportent le cas d’un patient âgé de 24ans ayant un rétrécissement mitral serré en rythme sinusal avec une communication interauriculaire type sinus venosus et un retour veineux pulmonaire anormal partiel. Le diagnostic a été fait à l’âge de 17ans à l’occasion d’une dyspnée. Il a bénéficié en février 2003 d’une dilatation mitrale percutanée de sauvetage au décours d’un œdème aigu du poumon. La surface mitrale est passée de 0,7 cm2 à 1,6 cm2. Devant l’amélioration clinique ultérieure, le patient a refusé la chirurgie réparatrice et a été perdu de vue. Sept ans plus tard (août 2010), il a été repris pour une deuxième dilatation mitrale de sauvetage en raison d’une resténose mitrale très serrée (surface mitrale à 0,8 cm2), en subœdème pulmonaire avec une hypertension artérielle pulmonaire échocardiographique à 68mmHg. Le trans-septal s’est compliqué d’une fausse route à partir de l’oreillette droite, vers la cavité péricardique. Le patient a été opéré en urgence et a alors bénéficié d’un remplacement valvulaire mitral par prothèse mécanique, d’une fermeture de la CIA type sinus venosus par un patch de PTFE et correction du retour veineux. Les suites opératoires ont été simples et l’échocardiographie postopératoire met en évidence un bon profil de la prothèse, l’absence de shunt résiduel et une diminution de la pression artérielle pulmonaire qui est passée de 68 à 40mmHg. En conclusion, la dilatation mitrale percutanée peut être un geste d’attente de la chirurgie réparatrice de cette pathologie ou un traitement d’urgence de sa valvulopathie, moyennant un suivi régulier dans l’attente d’une chirurgie rapide et dans les meilleures conditions. Abstract: Introduction. – Lutembacher syndrome refers to the rare combination of congenital atrial septal defect and acquired mitral stenosis. This condition is usually treated surgically by mitral valve operation with concomitant closure of the atrial septal defect. Materials and methods. – Between 1993 and 2003, 4 patients with congenital Lutembacher syndrome had percutaneous mitral commissurotomy without closure of the atrial septal defect at our institution. The 4 patients were very symptomatic with right-sided heart failure signs and NYHA functional class III-IV. Results. – The procedure was carried out successfully for the four patients. Mitral valve area increased from 0,87 to 1,97 cm2 at mean; left atrial pressure decreased from 28,2 to 12,7 mmHg and the mean valve mitral gradient was reduced from 15,5 to 3,9 mmHg. Functional and clinical improvement was observed in all the cases. During a mean follow up of 55±29 months, our 4 patients remain pauci symptomatic under medical treatment. Conclusion. – The percutaneous treatment of the Lutembacher syndrome is currently a possible alternative to the surgery among patients having an anatomy favourable to the procedure. Lutembacher's syndrome refers to the rare combination of congenital atrial septal defect and acquired mitral stenosis. It is rarely associated to partial anomalous pulmonary venous connection. This condition is treated surgically by mitral commissurotomy or mitral valve operation with concomitant closure of the atrial septal defect with correction of the abnormal pulmonary venous connection. Percutaneous mitral commissurotomy before surgery can be a therapeutic alternative when mitral valve stenosis is severe and valve anatomy is favourable. The authors bring back the case of a 24 years old man having mitral stenosis in sinus rhythm associated to sinus venosus septal defect and partial anomalous pulmonary venous connection. The diagnosis was made for the age of 17 years old on the occasion of dyspnea. He benefited in February 2003 of rescue percutaneous mitral commissurotomy because of pulmonary oedema. Mitral valve area increased from 0.7 cm(2) to 1.6 cm(2). The patient was clinically approved, so that he refused surgery and was lost sight. Seven years later (August 2010) he was taken back for a second rescue percutaneous mitral commissurotomy because of a very severe mitral stenosis (mitral valve area was 0.8cm(2)), in pulmonary oedema with echocardiographic evaluated pulmonary hypertension at 68mmHg. The trans-septal complicated of a false road from the right atrium, towards the pericardic cavity. The patient was operated as the matter of urgency, and benefited from a mitral valve replacement by mechanical prosthesis, of closure of sinus venosus septal defect by PTFE patch and correction of abnormal pulmonary venous connection. Operating suites were simple, and the postoperative echocardiography concludes to a good prosthesis profile, the absence of residual shunt and a decrease of pulmonary artery blood pressure from 68 to 40mmHg. In conclusion, percutaneous mitral commissurotomy may be a waiting procedure for surgery of this disease or emergency treatment of it's valve anomaly, with regular monitoring while awaiting surgery faster and in better conditions., (Copyright © 2012.
