RESUMEN Introducción. La Organización Mundial de la Salud considera la violencia de género como un grave problema de salud pública. Una de cada cuatro mujeres entre 15 y 49 años ha sufrido violencia de género a lo largo de su vida desde los 15 años, esto conlleva graves consecuencias para la salud no solo de las mujeres sino también de sus hijas e hijos. Desde la Organización Sanitaria Integrada Bilbao-Basurto y la Universidad del País Vasco existe el compromiso de dar respuesta a este importante problema a través de la implementación de la Guía de Buenas Prácticas: El maltrato contra las mujeres: cribado, identificación y respuesta inicial. Objetivo. Reflexionar acerca del proceso de implementación en los ámbitos asistencial y académico de la guía de buenas prácticas clínicas sobre “El maltrato contra las mujeres de la Registered Nurses’ Association of Ontario RNAO: cribado, identificación y respuesta inicial. Temas de reflexión. En este artículo se abordarán dos temas de reflexión: 1) Descripción de la metodología de implementación. 2) Descripción de cada una de las fases, tanto en el ámbito clínico como académico. Conclusiones. La Guía de Buenas prácticas “El maltrato contra las mujeres de la RNAO: cribado, identificación y respuesta inicial”, presenta una herramienta útil para la implementación de buenas prácticas en los sistemas sanitarios que ayudan a la lucha contra la violencia de género. Las enfermeras y otros profesionales de la salud se encuentran en una posición privilegiada para ayudar a las mujeres que sufren abuso/violencia de su pareja íntima. The World Health Organization considers gender-based violence to be a severe public health problem. One in four women between 15 and 49 years of age has suffered gender-based violence in her lifetime since the age of 15. It has severe consequences for the health not only of women but also of their children. The Bilbao-Basurto Integrated Health Organization and the University of the Basque Country are committed to responding to this critical problem through the implementation of the Good Practice Guide: Abuse against Women: Screening, identification, and initial response. Objective. To reflect on the process of implementation of the good clinical practice guide on “Abuse against women in the RNAO: screening, identification and initial response”. Reflection topics. This article will address two topics for reflection: 1) Description of the implementation methodology. 2) Description of each of the phases, both clinically and academically. Conclusions. The RNAO Good Practice Guide: “RNAO Woman Abuse: Screening, identification and initial response”, presents a useful tool for implementing exemplary practices in health systems that help in the fight against gender-based violence. Nurses and other health professionals are in a privileged position to assist women in the fight agains t gender-based violence. Ela tem sérias consequências para a saúde não apenas das mulheres, mas também de seus filhos. A Organização Integrada de Saúde de Bilbao-Basurto e a Universidade do País Basco estão comprometidas em responder a esse importante problema por meio da implementação do Guia de Boas Práticas: Abuso contra mulheres: triagem, identificação e resposta inicial. Objetivo. Refletir sobre o processo de implementação do guia de boas práticas clínicas sobre “Abuso de mulheres na RNAO: triagem, identificação e resposta inicial”. Temas para reflexão. Este artigo abordará dois tópicos para reflexão: 1) Descrição da metodologia de implementação. 2) Descrição de cada uma das fases, tanto em ambientes clínicos quanto acadêmicos. Conclusões. Objetivo. Medir el efecto de la pandemia de COVID-19 en la implementación de un programa de detección oportuna, intervención breve y referencia para tratamiento por consumo de alcohol en centros de salud del primer nivel de atención de la Ciudad de México. Métodos. Se recopilaron los datos en 18 centros de salud de la Ciudad de México entre 2019 y 2021, como parte de un estudio más amplio. Participaron 287 profesionales de la salud, quienes midieron el consumo de alcohol de sus pacientes con la prueba AUDIT-C. Se analizaron aspectos demográficos y patrones de consumo de los pacientes, así como las acciones de atención (detección, intervención breve, entrega de folletos, monitoreo y referencia) en tres momentos: antes de la pandemia de COVID-19, durante el confinamiento y luego del confinamiento. Se realizaron pruebas paramétricas y no paramétricas para identificar la relación y las diferencias entre las variables en los tres períodos. Resultados. Se identificaron 9090 personas con consumo de alcohol; el porcentaje de detección del consumo tuvo fluctuaciones en los tres períodos estudiados (26%, 53% y 37%, respectivamente). El consumo de riesgo medio y alto fue más frecuente entre hombres jóvenes y con mayor escolaridad durante y después del confinamiento. En el período posconfinamiento aumentaron las acciones de monitoreo y referencia. Conclusiones. La medición del impacto de la pandemia en la implementación de un programa de detección oportuna, asesoramiento breve y referencia para el tratamiento del consumo de alcohol en centros de salud ayuda a elaborar políticas en salud por edad, género, escolaridad y tipo de consumo para el primer nivel de atención.

Objective. To measure the impact of the COVID-19 pandemic on the implementation of a program for timely detection, brief intervention, and referral for treatment of alcohol consumption in health centers at the first level of care in Mexico City. Methods. The data were collected at 18 health centers in Mexico City between 2019 and 2021, as part of a larger study. A total of 287 participating health professionals measured their patients' alcohol consumption using the AUDIT-C test. The patients' demographic aspects and consumption patterns were analyzed, as well as care actions (detection, brief intervention, provision of leaflets, monitoring, and referral) at three points in time: before the COVID-19 pandemic, during confinement, and after confinement. Parametric and nonparametric tests were performed to identify the relationship and differences between the variables at the three points. Results. A total of 9090 people who consumed alcohol were identified; detection of consumption fluctuated in the three periods studied (26%, 53%, and 37%, respectively). Medium- and high-risk consumption was more frequent among young men with higher schooling during and after confinement. In the post-confinement period, monitoring and referral actions increased. Conclusions. Measuring the impact of the pandemic on the implementation of a program for timely detection of alcohol consumption, brief intervention, and referral for treatment in health centers helps to develop health policies by age, gender, schooling, and type of consumption at the first level of care. This study sought to investigate the Teaching-Service-Community Integration (IESC) from the perception of Primary Health Care (PHC) workers. The research has an exploratory, observational, descriptive, qualitative and quantitative approach, carried out by declaring. Eighty-six professionals participated, mostly Community Health Agents (60%), of which 95% (n = 80) consider that the presence of students in the Units collaborated to carry out the work, 48% (n = 39) They identify that the institutions are of Higher Education (HEI) together with managers/workers/representatives of the Unit that defines as actions. Difficulties for IESC: lack of infrastructure in health units, inadequacy of timetables and transport and communication failure between the Teaching Institution and Service. The understanding of the IESC was more restricted to an "exchange of knowledge", however 99% said that this articulation favors the construction of an important practice for the training of those who are in the services as well as for the students of the educational institution. Introducción. La mayor parte de las desigualdades en salud observadas están relacionadas con desigualdades de otros planos de la vida social. Estudios de pequeñas áreas nos permiten visibilizar estos gradientes al descender el análisis a escalas más pequeñas. El objetivo de este estudio es relacionar la distribución espacial de enfermedad cardiovascular según condiciones de vida y lugar de atención. Materiales y Métodos. Se caracterizó la ciudad de Bariloche en base a las condiciones de vida de sus vecindarios seleccionando indicadores mediante un análisis factorial. Se ubicó espa¬cialmente a la población atendida por diabetes e hipertensión arterial en los centros de salud. Se relevaron los registros médicos de esta población para analizar sus problemas de salud. Se analizó la distribución de cada dimensión y se calcularon las proporciones de las personas atendidas según condiciones de vida y lugar de atención. Resultados. Casi la mitad (47,6%) de la población vive en vecindarios con condiciones de vida deficitarias y críticas. El 63,2% de las personas atendidas en centros de salud viven en vecindarios con peores condiciones de vida. Y la identificación de un gradiente positivo: ante peores condiciones de vida, mayor es la proporción de enfermedad cardiovascular. Discusión. En el espacio urbano se entrelazan condiciones de vida con problemas de salud, configurándose distintas posibilidades de desarrollarlos. Al interior de una ciudad, pertenecer a determinada área implica la posibilidad/imposibilidad de acceder a niveles relativos de satisfacción de necesidades y presentar determinados problemas de salud. Permite identificar grupos y zonas críticas podría ser un insumo para el diseño de políticas públicas.

The aim of this essay is to analyze plays as a central element in the practices that construct micropolitics within the social institutions of the State. The main concepts addressed are: plays, practices, and micropolitics. The analysis focuses on institutions within social fields, emphasizing material size. The hypothesis posits that the size of the organization is inversely proportional to the development of plays within the institutions. This discussion takes place in a context marked by a strong detachment from the public and the state, exacerbating profound social inequalities, nihilism, and aporophobia, alongside a crisis of legitimacy of public institutions in the face of the advance of non-democratic ideas in democratically elected governments in several countries in Latin America and other continents., El objetivo de este ensayo es analizar el juego como un elemento central en las prácticas que construyen micropolíticas en las instituciones sociales del Estado. Los principales conceptos que se trabajan son: juego, prácticas y micropolíticas. El análisis se recorta a las instituciones de los campos sociales haciendo énfasis en el tamaño material. La hipótesis es que el tamaño de la organización es inversamente proporcional al desarrollo del juego en las instituciones. Esta discusión se da en un contexto marcado por un fuerte desapego a lo público y a lo estatal, lo cual no hace más que agravar las profundas desigualdades sociales, el nihilismo y la aporofobia, con una crisis de legitimidad de las instituciones públicas frente al avance de ideas no democráticas en gobiernos elegidos democráticamente en varios países de América Latina y de otros continentes.

16. Intervención farmacéutica en la optimización del uso de antimicrobianos restringidos mediante capacitación en áreas de unidosis del Hospital de Especialidades José Carrasco Arteaga de la ciudad de Cuenca

Background: Antimicrobial resistance is a growing health threat and, unless action is taken, the burden of deaths from this cause could reach 10 million per year by 2050. Objective: Optimize the use of restricted antimicrobials through training in single-dose areas of the José Carrasco Arteaga Specialty Hospital in Cuenca. Method: Prospective and intervention study. The work was carried out between February and April 2023; and reports on restricted antimicrobials were implemented, both in prescription and therapeutics, through a pharmaceutical training intervention for single-dose pharmacists, which sought to improve knowledge and skills in this matter and incorporate the pharmacist into the Antimicrobial Optimization Program (PROA). In both the pre-intervention and post-intervention phases, professionals filled out a knowledge evaluation questionnaire, as well as the respective records. Results: The pharmaceutical report obtained most frequently during the pre-intervention phase was a request for restrictive authorization (42.2%), and in the post-intervention phase days of treatment (69%). After the evaluation of the talk, an improvement of one point was observed, with an excellent rating. In reporting effectiveness, a rate of 41.3% was reached in the pre-intervention stage and 58.7% in the post-intervention stage. The economic benefit for the Hospital in the postintervention phase was $2,344.90 US dollars. Conclusions: These results favor the role of the pharmacist in the PROA team, since probably in the future he will be in charge of providing data on the consumption of restricted use antimicrobials and reporting indicators periodically.

Objetivo: analizar las repercusiones que tiene sobre la atención en salud mental la implantación de Protocolos de Conducta Asistencial en una Unidad Básica de Salud de Juiz de Fora (Minas Gerais - Brasil). Método: estudio histórico-social, realizado en 2018, cuyas fuentes directas son los documentos escritos y testimonios orales analizados según los presupuestos de la Reforma Psiquiátrica. Resultados: se presentan las categorías: a) Reorganización de la atención en salud mental a través de la implantación de Protocolos de Conducta; b) Proceso de derivación y contraderivación entre la Unidad Básica de Salud y los servicios de atención especializada en salud mental; c) Transformación de la atención en salud mental. Conclusión: los protocolos organizaron la red de atención en salud mental, apoyando la formación multidisciplinaria, instaurando la Atención Primaria de la Salud como puerta de entrada para la recepción, el tratamiento y seguimiento de la salud mental del territorio.

Objective. To measure the impact of the COVID-19 pandemic on the implementation of a program for timely detection, brief intervention, and referral for treatment of alcohol consumption in health centers at the first level of care in Mexico City. Methods. The data were collected at 18 health centers in Mexico City between 2019 and 2021, as part of a larger study. A total of 287 participating health professionals measured their patients' alcohol consumption using the AUDIT-C test. The patients' demographic aspects and consumption patterns were analyzed, as well as care actions (detection, brief intervention, provision of leaflets, monitoring, and referral) at three points in time: before the COVID-19 pandemic, during confinement, and after confinement. Parametric and nonparametric tests were performed to identify the relationship and differences between the variables at the three points. Results. A total of 9090 people who consumed alcohol were identified; detection of consumption fluctuated in the three periods studied (26%, 53%, and 37%, respectively). Medium- and high-risk consumption was more frequent among young men with higher schooling during and after confinement. In the post-confinement period, monitoring and referral actions increased. Conclusions. Measuring the impact of the pandemic on the implementation of a program for timely detection of alcohol consumption, brief intervention, and referral for treatment in health centers helps to develop health policies by age, gender, schooling, and type of consumption at the first level of care. The main objective of this work has focused on extending the existing literature on the analysis of total factor productivity of the Health Care Centers (HCC) of the Community of Madrid (Spain) in the period 2015-2017, with two important contributions; the application and comparison of the Malmquist indices with two robust methodologies such as the Hicks-Moorsteen and Färe Primont indices which, except for error, have not been previously used in this sector and the regional scope, which has been scarcely analyzed. The main results reveal decreases in HC productivity due to the three proposed methodologies. These decreases range between 4.90% and 0.25% for the Färe-Primont and Hicks-Moorsteen indices respectively between 2015/17 in average terms. Convergence analyses confirm that this takes place in the case of the Malmquist and Hicks-Moorsteen proposals. || El objetivo principal de este trabajo se ha enfocado en extender la literatura existente sobre el análisis de la productividad total de los factores, de los Centros Sanitarios de la Comunidad de Madrid en el periodo 2015-2017, con dos importantes contribuciones, la aplicación y comparación de los índices de Malmquist con dos metodologías robustas, los índices de Hicks-Moorsteen y Färe Primont que salvo error no han sido previamente utilizadas en este sector y el ámbito regional, que ha sido escasamente analizado. Los principales resultados revelan disminuciones de la productividad de los CS por las tres metodologías propuestas. Estas disminuciones oscilan entre 4.90% y 0.25% para los índices de Färe-Primont y de Hicks-Moorsteen respectivamente entre 2015/17 en términos medios. Los análisis de convergencia confirman que esta tiene lugar en el caso de las propuestas de Malmquist y Hicks-Moorsteen.

A raíz de la pandemia del COVID–19 la tecnología en los procesos de capacitación tuvo un aceleramiento significativo por tanto surge la problemática para determinar si los ámbitos tecnológicos y pedagógicos han tenido un impacto favorable en la capacitación virtual. El objetivo de la investigación fue realizar un análisis sobre la implementación de marcos de referencia educativos y tecnológicos para el proceso de capacitación. La investigación fue con un enfoque cuantitativo de tipo exploratoria y descriptiva, la población y muestra fueron los colaboradores de servicios de salud del Estado de Veracruz. Los resultados muestran que 70% de los trabajadores consideran los cursos virtuales como excelentes o buenos, la prueba de proporción muestral obtuvo que al menos el 90% de los colaboradores desean continuar tomando cursos virtuales. Se concluye que el método y la tecnología apropiada para la impartición de cursos ha sido bien aceptado por lo que debe continuar.

23. Desnutrición en pacientes oncológicos hospitalizados en el Centro Médico Docente La Trinidad en agosto-septiembre 2017

Todos los individuos con deficiencias de macro y micronutrientes necesarios para el mantenimiento y la reparación de tejidos cursan con desnutrición. El reconocimiento y tratamiento precoz de desnutrición en los centros de salud es un tema de cuidado principal. Los pacientes hospitalizados cursan con aumento del metabolismo basal y producción de energía para favorecer la curación. La desnutrición hospitalaria es un problema importante a nivel mundial, ya que se ha reportado que 50% de los pacientes hospitalizados presentan algún tipo de déficit nutricional. Esto se traduce en aumento de la morbi-mortalidad principalmente por aumento de las complicaciones. En el paciente oncológico hospitalizado se conjugan múltiples factores por los que puede aparecer desnutrición, entre ellos escaso aporte de nutrientes, alteración de la digestión y aumento de las necesidades. Se ha reportado 40-80% de malnutrición en los pacientes con cáncer, por lo que es importante diagnosticar y en caso de ser necesario, solicitar intervención nutricional en estos pacientes con el fin de disminuir la morbimortalidad y mejorar la calidad de atención. El presente trabajo de investigación buscó determinar el estado nutricional de los pacientes hospitalizados en el Centro Médico Docente La Trinidad en el período de Agosto-Septiembre 2017 utilizando la Valoración Global Subjetiva Generada por el Paciente como método diagnóstico. Comprobando las hipótesis, se encontró malnutrición moderada a severa (o riesgo de padecerla) en 87% de los pacientes estudiados en una muestra de 15 pacientes. Así mismo, se determinó que el 100% de la muestra ameritaba intervención nutricional o farmacológica por su oncólogo y un especialista en nutrición.

26. Satisfaction, compassion fatigue and associated factors in primary care nurses.

Objective: Verifying satisfaction and fatigue due to compassion and its associated factors in nurses of the Basic Health Unit. Method: A descriptive and cross-sectional research developed with 101 nurses from 40 Basic Health Units in a municipality of Parana. Data were collected between November 2019 and February 2020 through a questionnaire of sociodemographic characterization, occupational and life habits and the Professional Quality of Life Scale that assesses Satisfaction and Fatigue by Compassion. The associated factors were obtained by logistic regression models. Results: Having a good interpersonal relationship decreased the chances of low satisfaction by compassion (p=0.025) and burnout (p=0.049). Being recognized at work had a significantly lower probability of low compassion satisfaction (p=0.040). Conclusion: Good interpersonal relationships were associated with compassion satisfaction and burnout. Feeling recognized for the work done was also associated with satisfaction for compassion. Most nurses, even with high levels of satisfaction due to compassion, feel tired, which leads to reinforce the need for greater attention to the work developed by nurses in the Basic Health Unit by managers. More than half of medical interns do not feel safe in Health Centers (HS), since there are records of violence mainly towards social service female medical interns (SSFMI) in HS. The objective of the study was to identify the type of violence exercised in a sample of 60 women between 22 and 24 years old. The survey helped to identify four risk situations: insecurity (30%), life in danger (50%), attempted break-in (20%) and attempted sexual harassment (30%). Results also show six different types of violence: psychological (42.17 ± 11.54), physical (94.86 ± 7.07), economic (83.45 ± 5.24), sexual (27.38 ± 3.53), work (53.13 ± 36.05) and educational (43.77 ± 23.49). Violent assaults are evident in the SSFMI, suggesting that the HS do not have appropriate security measures to protect the safety of the SSFMI. [ABSTRACT FROM AUTHOR]

Introduction: There are few studies that associate anemia with complementary feeding practices in cities of the provinces of Peru.Objective: To determine the characteristics of complementary feeding practices, sociodemographic characteristics and their association with anemia in children between 6 and 12 months of age from Cusco-Peru between August – December, 2017.Material and Methods: A cross-sectional and analytical study based on the data from 360 mothers and children was carried out. The complementary feeding practices were evaluated through a semi-structured questionnaire, and their association with the variable of interest "anemia" was calculated. The crude (cPR) and adjusted (aPR) prevalence ratios were estimated using linear and generalized Poisson regression models and log link function, with their respective 95 % confidence intervals (95 % CI). P-values Results: The prevalence of anemia was 68,3 %. Associated factors were: living in poverty (aPR 1.27, p= 0.002), receiving an inadequate daily diversity of foods (aPR 1.13, p= 0.038); receiving an inadequate daily amount of food (aPR 1.15, p= 0.036) and supplementation with porridge (aPR 1.51, p= 0.002). Iron supplementation (aPR 0.33, p Conclusions: Living in poverty has been associated with anemia as well as some complementary feeding practices. These findings could be modified through proper follow-up in home visits and appropriate assignment of personnel for this activity.

Foundation: Having a triage system is essential for emergency systems, given the high demand for assistance and the need to offer quality medical care. Objective: to describe the system of triage developed at the Cienfuegos Province between 1988 and 2016.Methods: research in systems and health services, mixed in methods and techniques, which included the 24 emergency services of the territory. Indicators in the records of the health institutions were included and the results of other research conducted during the period, which also measured them. Results: atriage system was designed with three coding levels (red, yellow and green) according to vital risk. The red code was similar for the entire healthcare network. Main indicators: average time of arrival-triage 2.5 minutes; duration of the triage Conclusion: The triage system in Cienfuegos is structured, is useful and has been effective mainly for the most serious adult patients.

Este texto propone la construcción de una nueva hegemonía institucional de configuración rizomática basada en los centros de salud y en relación directa con una red hospitalaria de complejidad creciente al servicio de estos centros de salud y no a la inversa. Propone equipos de trabajadores para esos centros con anclaje territorial, que desarrollen prácticas humanizadas centradas en el paso de la atención al cuidado en cada territorio, respetando las singularidades e idiosincrasia de los habitantes y sus culturas. En primer lugar, se realiza una breve síntesis histórica del devenir de los centros de salud y de los hospitales. En segundo lugar, se realizan propuestas técnicas para reformular los modelos de atención y organización en las instituciones del campo de la salud y propuestas tecno-políticas para la nueva hegemonía institucional, que se desarrolla en tres dimensiones: organización, políticas y procesos de atención y cuidado. Por último, se identifican obstáculos en la construcción de esta nueva hegemonía.

Introduction: diabetes mellitus has become a serious health problem, due to its high prevalence, in addition to the high degree of mortality and disability it generates. Objective: to evaluate the economic burden of diabetes mellitus in the Diabetic Care Center of Pinar del Río, during 2016. Method: cross-sectional descriptive study through the cost approach of the disease, working with a target group of (U = 8623), where all patients attending the aforementioned institution were included, the cost reports for diabetes mellitus issued by the unit were also used, the descriptive statistics was applied. Results: the direct and indirect costs of diabetes mellitus in the Diabetic Care Center of Pinar del Río in 2016 were not individually reflected, which made us to infer that the general assessment of the economic burden of diabetes mellitus could lead to the non-implementation of the approved budget for 2016 in the Diabetic Care Center. Conclusions: based on the results of this investigation, it is necessary to reflect on the economic need to separate the costs of diabetes mellitus type 1 and 2 in the analysis of the budget, taking into account that they do not have the same levels of expenses in their treatment, as well as direct and indirect costs, to obtain real costs in the care of patients with the disease.

Objective: to evaluate the public vaccination rooms of the municipality of Aracaju according to the National Immunization Program. Methodology: evaluative, cross-sectional study conducted in 42 public health units in 2018 through semi-structured interviews with approaches related to structure, process. Results: all rooms stored immunobiologicals in household refrigerators and more than 60% did not maintain the room's adequate temperature; 45% had decorative objects on their walls, 50% of the daily syringes and needles were not properly packed and 73% did not treat vaccines composed with living microorganisms before disposal and 69% did not use the Cards controls as a way of monitoring to perform active search. Conclusion: It is concluded that the vaccination rooms of the municipality presents a regular general index and there is a growing need of the municipality to institute the practice of evaluation in the service for better planning and monitoring of activities. Objectives: Evaluate access and accessibility to Basic Health Units in different regions of Brazil, characterizing them according to organizational and infrastructure components. Method: Evaluative Research through a cross-sectional study with a quantitative approach, based on data from the third cycle of the National Program for Improving Access and Quality in Primary Care Results: Most Basic Health Units (84.4%) have fixed opening hours; units of the North region revealed the most variable hours (72.0%); on average, 46.3% of the units maintain activities at lunchtime, with the Midwest (24.2%) and South (29.5%) regions having lowest percentage of units operating during lunch hours; 87.1% presented all environments with signage, facilitating access of users; few units had handrails in non-level locations (19.6%) and tactile flooring for access to premises (24.1%); 21.7% presents a structure that differs from that established by the Ministry of Health. There were regional disparities in the findings found nationwide regarding access and accessibility. Objective: to analyze the difficulties of those responsible for children under one year of age in adhering to the childcare program. Method: descriptive, exploratory study with a quantitative approach, conducted from interviews with 40 parents of children under the age of one who underwent childcare consultations in nine Family Health Strategies. For data collection, a semistructured questionnaire was used. Data were analyzed by the percentage of the agreement calculation, considered satisfactory above 80% and the content validity index. Results: it was evidenced that 80% of the guardians know that the childcare consultation is to monitor the growth and child development, 60% that the consultation must occur monthly and 47% until the age of ten. Among the difficulties in adhering to childcare, the planning of the teams stood out, with limitations for scheduling and holding consultations. Conclusion: to overcome this scenario, some strategies stand out, such as, implementation of groups, continuing / permanent education with the team, reorganization of the schedule of activities of the health unit and production of educational booklets on childcare. Objective: analyse the situation of the spe-cialty of family and community nursing in Castilla y León.Methodology: A longitudinal study was car-ried out, analysing secondary data through the transparency portal of Health Castilla y León, the calls for elective tests for the specialty of the Spanish Ministry of Health and the orders pub-lished in Castilla y León in relation to specialized health training. Data were extracted from the above sources and statistical analysis was carried out. Results: only 0,69% of the nurses working at that time occupied positions in the category un-der study; if extrapolated to the Primary Health Care setting, 2,87% of the total occupy a posi-tion categorized as a specialist nurse. The trend in the number of vacancies has been increas-ing in line with the vacancies offered in general throughout the country. The year that account-ed for a higher percentage of vacancies offered with respect to the rest of Spain was 2019. The provinces that have most notably increased the number of accreditations since 2011 are Burgos, Soria and Valladolid, while Segovia decreased its accreditation capacity. With an estimated annu-al expenditure per resident of € 29.948,66, only the 4% of residents have obtained a long-term contract after their training period. Conclusions: It is urgent the creation and unblocking of specific exchanges for specialist nurses in family and community nursing as well as the priority in transfers and the recruitment of nurses with this degree in Primary Health Care., Objetivo: analizar la situación de la especiali-dad de enfermería familiar y comunitaria en Cas-tilla y León. Metodología: Se realizó un estudio longitu-dinal, de análisis de datos secundarios a través del portal de transparencia de Sanidad Castilla y León, las convocatorias de las pruebas selectivas para la especialidad del Ministerio de Sanidad Español y las órdenes publicadas en Castilla y León en relación con la Formación Sanitaria Es-pecializada. Se extrajeron los datos de las fuen-tes citadas y se realizó el análisis estadístico de los mismos.Resultados: únicamente el 0,69% de las en-fermeras que trabajaban ese momento ocupa-ban plazas de la categoría en estudio, si se extra-pola al ámbito de Atención Primaria ocupan una plaza categorizada como enfermero especialista el 2,87% del total. La tendencia de la oferta de plazas ha ido en aumento en sintonía con las pla-zas ofertadas en general en todo el país, el año que supuso un mayor porcentaje de plazas ofer-tadas con respecto al resto de España fue 2019. Las provincias que desde el año 2011 aumenta-ron de forma más notoria el número de acredita-ciones serían Burgos, Soria y Valladolid mientras que Segovia ha visto mermada su capacidad de acreditación. Se estima un gasto por residente anual de 29.948,66€, se ha fidelizado al 4% de residentes. Conclusión: urge la creación y desbloqueo de bolsas específicas de enfermera especialista en enfermería familiar y comunitaria, así como la prioridad en los traslados y en la contratación de enfermeras con esta titulación en Atención Pri-maria.

Introduction: Both Occupational Therapy and primary care focus their interventions on the client’s individual needs and preferences and establish close collaboration with them and their environment. Within the Andalusian public health system, Occupational Therapy is included in the support devices, but the care structure is not clear. The aim of this study is to explore the need and importance of Occupational Therapy at the primary care level. Methods: During 2021 October, five occupational therapists contracted by the Granada-Metropolitan Health District for COVID-19 follow-up functions carried out external work related to the discipline’s own functions in five health centers in the province. Data were collected through the Canadian Occupational Performance Measure and a self-developed survey. Conclusion: There is a board theoretical basis for the benefits that Occupational Therapy can bring to primary care. The combination can help the Andalusian public health system to improve outcomes in prevention, rehabilitation and cost reduction., Introducción: tanto la terapia ocupacional como la atención primaria centran sus intervenciones en las necesidades y preferencias individuales de la persona, y establecen una estrecha colaboración con ésta y con su entorno. Dentro del sistema sanitario público andaluz se contempla la figura de la terapia ocupacional en los dispositivos de apoyo, pero no está clara su estructura asistencial. El objetivo de este trabajo es profundizar en la necesidad e importancia de la figura de la terapia ocupacional en niveles asistenciales primarios. Métodos: durante el mes de octubre de 2021, cinco terapeutas ocupacionales contratados por el Distrito Sanitario Granada-Metropolitano para funciones de rastreo COVID-19, llevamos a cabo un trabajo externo relacionado con funciones propias de la disciplina en cinco centros de salud de la provincia. Recogimos datos a través de la Medida Canadiense del Desempeño Ocupacional y de una encuesta de elaboración propia. Conclusión: existe una base teórica muy amplia que sustenta los beneficios que puede aportar la terapia ocupacional a la atención primaria. La combinación entre ambas puede ayudar al sistema sanitario público andaluz a mejorar resultados en prevención, rehabilitación y reducción de costes.

44. Barreras de Accesibilidad para Personas con Discapacidad Física en Servicios de Salud del Cantón Rocafuerte, Ecuador.

The present research study was carried out with the objective of evaluating the accessibility barriers for people with physical disabilities in the health services of the Rocafuerte canton, Ecuador; To achieve compliance with it, a mixed approach methodology is used, with a non-experimental design, using the descriptive method, under an analytical research level. Applying as an instrument the observation guide to the seven health centers of the Rocafuerte county, and an interview with thirty people with disabilities who receive care in these health centers. Among the main findings obtained, it can be observed the poor compliance with the regulations that govern accessibility as a right towards people with physical disabilities in the country. The "Frutillo" health center is the institution with the highest accessibility barrier, which completely disables efficient access for people with physical disabilities to areas such as bathrooms, ramps, or stairs. In this way, it is recommended to carry out the necessary actions to provide adequate accessibility to patients with physical disabilities who are affected by the barriers presented by these institutions., El presente estudio de investigación se llevó a cabo con el objetivo de evaluar las barreras de accesibilidad para las personas con discapacidad física en los servicios de salud del cantón Rocafuerte, Ecuador; para alcanzar el cumplimiento de este, se emplea una metodología de enfoque mixto, con diseño no experimental, utilizando el método descriptivo, bajo un nivel de investigación analítico. Aplicando como instrumento la guía de observación a los siete centros de salud del catón de Rocafuerte, y una entrevista a treinta personas con discapacidad que reciben atención en estos centros de salud. Entre los principales hallazgos obtenidos se pueden observar el escaso cumplimiento de las normativas que rigen la accesibilidad como un derecho hacía las personas con discapacidad física en el país. Siendo el centro de salud “Frutillo” la institución con mayor barrera de accesibilidad, lo que inhabilitan en su totalidad, el acceso eficiente a las personas con discapacidad física a áreas como baños, rampa, o escalera. De esta forma se recomienda ejecutar las acciones necesarias para brindar una accesibilidad adecuada a los pacientes con discapacidad física que se ven afectados por las barreras que presentan estas instituciones.

[Resumen] Avanzada la segunda mitad del siglo XX, surge a nivel mundial un nuevo equipamiento sanitario de pequeña escala, puramente contemporáneo, que cubre las necesidades de la recién llegada atención primaria: el centro de salud. A lo largo del paisaje gallego se construye una red de equipamientos públicos al servicio de la atención primaria de gran calidad arquitectónica promovidos por el Servizo Galego de Saúde de la Consellería de Sanidade de la Xunta de Galicia. Un estudio crítico de estos centros puede evidenciar la evolución de la arquitectura contemporánea en la Comunidad Autónoma de Galicia. Para ello, se analiza la carga contextual, la carga formal de la arquitectura experimental y el valor sostenible como razón fundamental de proyecto de las obras seleccionadas y construidas entre 1989 y 2019, desde la creación del Servicio Galego de Saúde hasta la pandemia por COVID-19., [Resumo] A finais da segunda metade do século XX, xurde en todo o mundo un novo equipamento sanitario de pequena escala, puramente contemporáneo, que cubre as necesidades da recén chegaga atención primaria: o centro de saúde. Along the Galician scenery, a web of public equipments devoted to the primary healthcare of great architectural quality are built promoted by the Galician Healthcare Service of the Xunta de Galicia’s Healthcare Office (Servizo Galego de Saúde de la Consellería de Sanidade de la Xunta de Galicia). A critical analysis of these facilities can reveal the evolution of contemporary architecture in the Autonomous Community of Galicia. In order to do so, it is analysed the contextual burden, the formal burden of experimental architecture and the sustainable value as fundamental project’s reason for the selected constructions built between 1989 and 2019, since the creation of the Galician Healthcare Service until the COVID-19 pandemic.

En la actualidad, es evidente la estigmatización y la falta de lugares para pacientes con discapacidad mental en el área metropolitana de Bucaramanga. Tomando como referencia la pandemia ocasionada por el COVID-19 (2020-2021), que ha afectado y sigue afectando la salud física y mental de la población, se hace notoria la necesidad de diseñar espacios que ayuden con tratamientos psicosociales a las personas con enfermedades mentales. Lamentablemente, en el área metropolitana de Bucaramanga hay una deficiencia en este tipo de centros, los cuales se encuentran concentrados en una única zona (Bucaramanga). Por esta razón, se ha diseñado un centro de ayuda psicosocial ubicado en el municipio de Floridablanca, como el punto más cercano para las personas de Piedecuesta y Girón. El proyecto busca, a través del diseño de los espacios interiores y exteriores, colaborar en la recuperación de los pacientes, brindando ayuda psicosocial mediante los espacios, la funcionalidad, el color y los distintos ambientes que garanticen una constante relación entre el paciente y su núcleo cercano. Currently, the stigmatization and lack of places for patients with mental disabilities in the metropolitan area of Bucaramanga is evident. Taking as a reference the COVID-19 pandemic (2020-2021), which has affected and continues to affect the physical and mental health of the population, the need to design spaces that help with psychosocial treatments for people with mental illnesses is noticeable. Unfortunately, in the metropolitan area of Bucaramanga, there is a deficiency in this type of center, which is concentrated in a single zone (Bucaramanga). For this reason, a psychosocial support center has been designed in the municipality of Floridablanca as the closest point for people from Piedecuesta and Girón. The project aims, through the design of interior and exterior spaces, to collaborate in the recovery of patients, providing psychosocial support through spaces, functionality, color, and different environments that guarantee a constant relationship between the patient and their close circle. Objetivo. determinar los riesgos psicosociales del personal asistencial de una clínica privada de tercer nivel de la ciudad de Cartagena de Indias. Método. se realizó un estudio descriptivo de corte transversal, la población objeto de estudio fue de cincuenta y siete trabajadores del área asistencial, se utilizó la batería de instrumentos para la evaluación de factores de riesgo psicosocial diseñada por el Ministerio de la Protección Social de Colombia. Resultados. el promedio de edad es de 34.2 años, 52% pertenece al nivel educativo técnico, con un predominio del género femenino. El estado civil predominante con un 46% es soltero, 44% viven en estrato 2. La mayoría (74%) de los trabajadores considera que su jefe brinda siempre o casi la posibilidad de tener contacto con él. Siempre tienen buena relación y apoyo de sus compañeros, 61% considera que no tienen control sobre el tiempo y cantidad de trabajo que realizan. El pago del salario no es puntual y los trabajadores tienen muy pocas posibilidades de ascenso y además sienten que la empresa muy pocas veces se preocupa por su bienestar. La prevalencia de nivel más alto de riesgo correspondió a un 76% por exceso de carga laboral, por alta exigencia de esfuerzo mental un 75% y está relacionado con el poco control sobre el trabajo en un 57%. Los trabajadores se exponen a jornadas laborales extensas, poco tiempo de descanso, cansancio y falta de sueño por jornadas nocturnas, escasa interacción con la familia y el contacto constante con situaciones de extremo sufrimiento y dolor.
