The EAST-AFNET 4 trial has brought a major paradigm shift in the treatment of atrial fibrillation (AF). The results of the trial have confirmed the findings of previous observational studies that have demonstrated that in patients with recently diagnosed AF, early rhythm control improves prognosis compared to usual care (mostly aimed at rate control). The sub-analyses have shown that the observed benefit is dependent on the presence of sinus rhythm. Thus, in patients with recently diagnosed AF, the treatment should aim at restoration and maintenance of sinus rhythm. This applies to patients with both paroxysmal and persistent AF regardless of the presence of AF symptoms or heart failure. It is conceivable that the next guidelines from the European Society of Cardiology will no lon‑ ger be focused on the treatment of symptomatic AF, but rather support rhythm control strategy for most patients with this arrhythmia. Aim: The exact relationship between the gene mutation of hematopoietic cells (clonal hematopoiesis of indeterminate potential; CHIP) and the clinical manifestation has not yet been clearly described. The aim of this academic multicentric clinical study is to investigate the association between the presence of CHIP in peripheral blood and the incidence of ischemic stroke, and at the same time, to demonstrate whether CHIP contributes to the etiopathogenesis of atherosclerosis. This could lead to the identification of a new potential risk factor in the diagnosis of ischemic stroke predisposition. Methodology: This is a multicenter prospective study involving 14 primary and comprehensive stroke centers in the Czech Republic. The total number of enrolled patients aged ≥ 70 years is expected to be 1,000. The selected surgical centers will collaborate to provide 50 atherosclerotic plaque samples. Monitored parameters include personal and pharmacological history, laboratory parameters, neurological parameters, and results of selected imaging examinations. The expected outcome will be the definition of specific allelic variants and their frequencies as risk factors for the development of atherosclerosis and subsequent manifestation of ischemic stroke. The goal of the study focused on post-stroke patients was to identify whether the patients suffered from sequelae, required the care provided by other persons, and which services they used within the home care. Methods: The design of the study was quantitative. The field study used the method of a standardized face-to-face interview. The sample consisted of 416 post-stroke patients, 207 (49.8%) men and 209 (50.2%) women. Results: In the respondents, the most frequent persisting post-stroke problems included disturbed coordination of mobility (57.2%), weakness in or motility loss of upper or lower limbs (47.1%) and speech problems (43.5%). The persisting symptoms significantly correlate with age < 0.001 and occupation < 0.001. 43.0% post-stroke patients said that their condition always or often demanded a care or another person; in some cases, this care was demanded by more than a quarter of respondents. The Home Care Agency services are used by less than ½ of (47.6%) respondents. The utilization of these services significantly correlates with age < 0.001, occupation < 0.001 and the time that has passed since stroke < 0.001. The most frequently used post-stroke services include the services provided by physiotherapists and social workers, the least demanded services are those provided by psychiatrists and psychologists. Conclusion: Post-stroke patients have particularly to rely on the home care nurses help or the help offered by selected professionals the patients have been informed on. The study showed that this was not sufficient since it did not outreach into the aftercare. Therefore, one of the goals of our project is to develop a program for a lifelong education of nurses taking care of stroke patients, and to propose an increase of competences for nurses in the aftercare of stroke patients. Stroke is one of the most common causes of permanent disability and death in economically developed countries. Ischemic strokes and transient ischemic attacks account for 85% of all these events. The paper deals with two the main causes of central thromboembolism: non-valvular atrial fibrillation and paradoxical embolization with evidence of patent foramen ovale as well as the possibilities of catheterization treatment in their prophylaxis. Aim: Based on an annual questionnaire, an analysis of the techniques used in endovascular recanalization of large cerebral artery occlusions in the treatment of acute ischemic stroke (AIS) in 2021 was performed. Method: In 2022, all centres of highly specialised cerebrovascular care and 2 primary centres in the Czech Republic were surveyed. The structured questionnaire in the section on endovascular procedures for AIS focused on the number of patients thrombectomized, the proportion of procedures where technically only a stent retriever was used versus a combination of stent retriever and aspiration catheter, and the number of procedures where only an aspiration catheter was used. The numbers of acutely implanted extra- and intracranial stents in these procedures and the percentage of balloon guiding catheters used were also queried. Results: Mechanical thrombectomy was performed in 1439 patients in 2021, of whom between 70 and 147 patients were treated in 13 comprehensive cerebrovascular centres, and 41 and 49 patients were treated in two ict centres. The ratio of stentretriever: stentretriever + aspiration catheter: aspiration catheter combination techniques used was approximately 0,8: 1: 0,6. A total of 114 extracranial stents (per centre ranging from 0-19) and 19 intracranial stents (per centre ranging from 0-6) were implanted acutely. Overall, a stent was acutely implanted in 11.8% of patients. Balloon catheters were used in 55% of the procedures. Conclusion: The surveyed parameters show that the number of patients treated endovascularly for iCMP in the Czech Republic in 2021 slightly decreased (to 99.3% in 2020), the number of procedures in complex centres differed by up to two times. Stroke and ischemic heart diseases are some of the most common causes of death in the world. The HOBIT project was created to increase awareness of these diseases and increase the number of people who are able to recognize them and respond correctly. The project is aimed mainly at pupils in primary schools and multi-year grammar schools, but the general public can also join in. Aim: The authors analysed numbers of endovascular neurointerventional procedures in Czech Republic comprehensive neurovascular centres in the years from 2013 to 2020. Methods: The questionnaire was sent to all tertiary and two secondary neurovascular centres in Czech Republic. Numbers of each specified procedure were required. On basis of the 3 most frequently performed procedures (endovascular therapy of acute ischemic stroke, extra- and intracranial stenoses, and intracranial aneurysms) the eight-year trends of each procedure frequency, and total numbers of all neurovascular procedures (i.e. carotid dissections, intracranial stenoses, cerebral and spinal arteriovenous shunts, skull base tumors, epistaxis, vasospasms after subarachnoid hemorrhage) were evaluated. Results: Frequency of the mechanic thrombectomy increased from 417 procedures in the year 2013 to 1468 procedures in the year 2019. There was yearly increase of the number of procedures by 132 in 2017, by 154 in 2018, by 148 in 2019. In 2020 number of procedures for ischemic stroke decreased by 19. There was stagnation in numbers of carotid angioplasties, and treated cerebral aneurysms during the studied period. Conclusion: There were significant differences revealed among cerebrovascular centres regarding numbers of treated patiens for acute ischemic stroke, extra- and intracranial stenoses, and cerebral aneurysms. Aim: Mechanical thrombectomy (MT) is the standard treatment for stroke due to the occlusion of a large endovascularly accessible artery. The topic discussed is the permanent presence of the endovascular team in a 24/7 mode in the center to reduce delays until the beginning of MT. Materials and methods: The set of 355 patients who underwent MT for large artery occlusion in the Comprehensive Stroke Center (CSC) in the years 2018–2019 was analyzed retrospectively. The data were confronted with theoretical assumptions according to the calendar share of working hours and the total annual time fund. Results: The period of institutional emergency service is approximately 76% of the time of the year according to the calendar data in the Czech Republic. This value was chosen as the most pessimistic value for the number of patients potentially admitted in the absence of the endovascular team (with a concomitant 100% admission of patients by primary transport to the CSC). 171 were treated in the CSC in 2018 and 184 in 2019. At the time of the institutional emergency service, 227 (63.9 %) patients were admitted. However, only 59 (16.6%) patients were admitted in the absence of the endovascular team at the time of the institutional emergency service. In the next steps, this number could be reduced to below 5%. Conclusion: Even with the impossibility of establishing the presence of an endovascular team in the center in a 24/7 mode, it is possible to reduce the number of patients admitted during the absence of the team to below 5%. Stroke is one of the most frequent acute events in neurology and it is associated with severe disability and high mortality. Advances in current diagnostic and therapeutic guidelines reflect the advances in a multidisciplinary approach. Malnutrition or a risk of malnutrition in stroke patients is common and systematic nutrition screening at admission is important. Acute illness and hospitalization can contribute to worsening of nutritional status. Appropriate timing of nutritional support, early capture and rehabilitation of speech and swallowing disorders together with physiotherapy may reduce incidence of complications, improve patient’s clinical outcome, prognosis and quality of life. The presented summary is in agreement with consensus of the European Society for Clinical Nutrition and Metabolism and recommended best practice expert consensus in the Czech Republic. Aim: The basis of the paradigm of cognitive-motor interference is the belonging of executive and motor functions. The conditions of a normal day lead to the need to perform several tasks simultaneously. Optimal functional adjustment after stroke should be eff ective in a combination of cognitive and movement training. The basic idea of the neurorehabilitation approach is primarily seen in the potentiation of neuroplasticity. However, so far, no obvious benefi t of cognitive-motor interference has been demonstrated due to the relatively small amount of available data. The aim of the systematic review was to demonstrate the eff ect of cognitive-motor interference on the improvement of gait and postural stability in patients after stroke. Methods: First, we formulated a review question and specifi ed criteria for the inclusion and exclusion of primary studies into a systematic review. We created a systematic search strategy and a three-step systematic search of published and unpublished primary studies in six databases. We searched for experimental and observational studies in the range from January 1, 2003 to December 31, 2019 in the English language only. Results: We found a total of 58 studies and removed 20 duplicates. Based on a two-phase evaluation of the relevance and critical evaluation of the methodological quality of the studies using the standardized evaluation tool Standard Quality Assessment Criteria for Evaluating a Primary Research Paper, 5 meta-analyses and randomized controlled trials were included. Conclusion: Cognitive-motor interference training brings benefi ts in locomotor and postural strategies in patients after stroke. Imaging methods play a crucial role in stroke diagnostics. Computed tomography (CT) remains the most used imaging method in stroke management. MRI is important in diagnoses of stroke in vertebrobasilar circulation, in patients with unknown stroke symptom onset and in diagnostics of stroke mimics. Non-contrast CT is compulsory in all patients with suspected acute stroke to exclude other pathologies (e.g. hemorrhage, tumor) and to assess the age and the extent of ischemic changes. CT angiography is required in all patients indicated for mechanical thrombectomy to detect and localize the arterial occlusion. The main goal of perfusion analysis is to identify the ischemic core and distinguish it from ischemic penumbra. To have a reliable interpretation of ischemic changes on CT, it is important to set up correct postprocessing. Nowadays, multiple software packages are available which enable automatic analysis of ASPECTS, collateral status and perfusion. The main benefits of automatic postprocessing are a decrease in variability of the assessment and faster triage of patients indicated for mechanical thrombectomy. [ABSTRACT FROM AUTHOR]

This diploma thesis deals with the issue of robotic-assisted therapy, namely with the mechanical exoskeleton robotic device Armeo Spring, and it is focused on an acute stroke population. The aim of this study is to evaluate the efficiency of a robotic-assisted therapy using Armeo Spring device and to compare it with the efficiency of a common individual occupational therapy focused on improving motor function of upper extremity of patients with the acute stroke. There were 19 patients involved in the research, who were hospitalized in early rehabilitation acute beds. They were selected on the basis of entry criteria and further divided into two groups. An experimental group included 10 participants (n=10), a control group included 9 participants (n=9). The patients in the experimental group using Armeo Spring device underwent four to five 30-minutes therapies per week for three-week period, the control group underwent the same amount of individual occupational therapies focused on improving motor function of a paretic upper limp. Each participant, evaluated before and after the intervention, underwent the total number of 12 therapy sessions. In the face of the determined hypothesis, following methods were used to evaluate the sample of patients: Function Independence Measure (FIM) for evaluating rate of independence, Jamar Dynamometer (JD) for evaluating a handgrip, Fugl-Meyer Assessment and modified Frenchay Arm Test (mFAT) evaluating motor function of paretic upper extremity. The study shows statistically significant improvement (p<0,05) of several parameters in both groups of patients. In the experimental group, there was statistically significant improvement in the FIM, the Fugl-Meyer Assessment, the mFAT, and in some pitches in JD measurement. However, when comparing the two groups, there was no statistically significant difference. The improvement with higher significant rate (p<0,1) was observed in just one parameter, which is the second pitch of JD measure. In conclusion, the study has shown that the use of robotic-assisted therapy via Armeo Spring device in the acute phase is effective in the above-mentioned items, however, it does not substitute the presence of a therapist and it is not significantly more effective than common occupational therapy focused on improving motor function of upper extremity, either. Stroke is a very serious and common disease with a high mortality. It is important for patients to start rehabilitation as soon as possible to ensure the use of time-limited brain plasticity. Early discharge from the hospital allows stroke patients homecoming to continue rehabilitation in familiar surroundings according to the patient's choice. The responsibility for continued rehabilitation is in the hands of patients and their families and the main aim is to adjust the everyday life, reach the maximum of self-sufficiency and return to work if it is possible. However, discharge from the hospital is sometimes critical for older patients because of lower functional abilities in activities and participations. Some tasks and activities should be difficult for these patients, sometimes appears higher risks, motor, cognitive and facial problems and fear of failure. It is very important for patients to increase their awareness of available services, the possibilities of ensuring the safety home environment and the availability of the necessary services. For the time and financial reasons, it is almost impossible to provide a home visit for all patients by an occupational therapist to evaluate the home environment, or even propose its treatments and equipment with compensatory aids. For this reason, occupational therapists in the Czech Republic try to increase the awareness of patients before their discharge from hospital. To raise awareness of patients, a "Summary of Recommendations for patients and their families" was written to provide stroke patients and their families with a clear set of information that is important for increasing self-sufficiency in the home environment. Aim: Accord ing to literature sources, cognitive dysfunction is one of the most com mon strokeinduced disorders. Despite the high number of cases of cognitive disorders after stroke, treatment options are still rather limited. The aim of this study was to as sess the effi ciency of cognitive train ing in individuals after stroke, in particular computer-based cognitive rehabilitation (CBCR) program mes. Methods: The analysis included studies where intervention of a CBCR program me was applied to subjects after stroke. We searched PubMed (Medline), Cochrane Database, and EBSCO for publications between January 2007 and July 2016. All the included studies were published in English. Results: Ten studies met the inclusion criteria. The included studies consisted of nine randomised control led trials and one randomised pilot study. All the studies targeted general or domain-specifi c cognitive functions. The majority of the included studies resulted in the improvement of the as ses sed functional outcome measures. Conclusion: The overview conducted by the authors of this article al lows us to claim that CBCR program mes may help to improve cognitive functions in subjects after stroke. [ABSTRACT FROM AUTHOR]

Aim: This study aims to investigate the eff ects of a combined ther apy compris ing a combination of computer-based cognitive rehabilitation with traditional rehabilitation techniques on cognitive functions in post-stroke patients suff er ing from a mild-to-moderate degree of cognitive impairment and to compare the results with a group of patients not receiv ing such ther apy. Methods: 33 post-stroke patients fulfi l l ing exclusion/ inclusion criteria were al located accord ing to travel distance from the treatment centre to either the treatment (N = 19) or control group (N = 14). Cognitive rehabilitation was performed in 60-min-long ses sions held twice a week for 12 weeks. Mini-Mental State Examination (MMSE) and Addenbrook Cognitive Examination - Revised (ACE-R) tests were performed at the begin n ing of the treatment, retests were performed approximately 16 weeks later and the results, includ ing ACE-R subscores as secondary endpoints, were analysed. Results: In the treatment group, statistical ly signifi cant improvement (p < 0.05) was detected in MMSE, ACE-R, and in ACE-R subtests Memory, Verbal fl uency and Language, while only the Memory subtest recorded statistical ly signifi cant improvement in the control group. However, due to the small number of patients, we only present the results as trends indicat ing that a study on a larger cohort is needed. Hence, a sample size for a future study required for proper as ses sment of the eff ects of combined approach cognitive rehabilitation was calculated, the result ing group size is 334 patients. Conclusions: A combination of computer-based rehabilitation and traditional rehabilitation techniques in patients suff er ing from mild-to-moderate cognitive impairment as a result of stroke led to a statistical ly signifi cant improvement in MMSE and ACE-R tests and in ACE-R Memory, Verbal fl uency and Language subtests. Aim: The aim of this pilot study was to verify therapeutical eff ect of periodical functional electrical stimulation of peroneal nerve dur ing 4 weeks of intensive outpatient ther apy on speed and performance of gait in post-stroke patients, suff er ing from spastic leg paresis. Methods: 14 subjects were enrolled into this pilot study (10 male, 4 female), aged 54,4 ± 9,4. The subjects were selected from patients suff er ing from chronic phase of stroke, spactic leg paresis and drop foot syndrome. Functional electrical stimulation was performed in total for 4 weeks (5 days weekly) as intensive ambulatory gait train ing with WalkAide neurostimulator. Emory Functional Ambulation Profi le and Two Minute Walk Test were performed for evaluation of changes in speed of gait on diff erent terrains and changes in the walking performance, respectively. Results: In this pilot study, average improvement of Emory Functional Ambulation Profi le score of 11% was observed as well as average improvement of Two Minute Walk Test of 9%. Conclusion: Positive therapeutic eff ect of periodical ambulatory functional electrical stimulation of peroneal nerve on gait speed and gait performance in post-stroke patients was observed in this limited pilot study. The article describes the importance of telerehabilitation and provides a description of its application in practice. As a practical example of a feasible and affordable telerehabilitation approach in the domestic context, we present the case report of a patient with whom we have used, as a telerehabilitation intervention, the interactive Home balance system. The system was used for balance training, utilizing a stabilometric platform and visual feedback as well as the Skype application for online video calls. The overall benefits, including sustainability of the effect of the therapy, were objectively verified using posturographic examination and standardized tests. Following therapy, the patient measured improvement in the Berg Balance Scale, Mini-BESTest and Functional Independence Measure (FIM) standardized tests and in posturographic examination. The improvement remained partially sustained even after four weeks without therapy. Based on a structured questionnaire completed by the patient, the positive subjective impression of the patient was also ascertained. [ABSTRACT FROM AUTHOR]
