- Abstract
Obejective: To explore the effect of fluid resuscitation guided by B-ultrasound measurement of inferior vena cava variability on blood lactic acid levels, pro-inflammatory factors, and immune function in patients with traumatic hemorrhagic shock. Methods: 108 patients with traumatic hemorrhagic shock who were admitted to the Affiliated Hospital of Panzhihua University from April 2020 to February 2023 were selected, according to the different methods of fluid resuscitation, the patients were divided into control group and observation group, with 54 cases each. The control group was treated with traditional restricted fluid resuscitation, and the observation group was treated with B-ultrasonography to guide fluid resuscitation. Clinical indicators, blood lactic acid levels, pro-inflammatory factors, immune function and multiple organ dysfunction syndrome (MODS), acute respiratory distress syndrome (ARDS) and mortality were compared in the two groups. Results: The infusion volume and total blood transfusion volume in the observation group were less than those in the control group, and the hospitalization time in the intensive care unit (ICU) was shorter than that in the control group (P<0.05). After treatment, the blood lactic acid levels of the two groups decreased, and the observation group was lower than the control group during the same period (P<0.05). After treatment, the tumor necrosis factor (TNF)-α, interleukin (IL) -1β and interleukin (IL)-6 levels in the two groups decreased, and the observation group was lower than the control group during the same period (P<0.05). After treatment, CD3+, CD4+, CD4+ /CD8+ in the two groups increased, and CD8+ decreased, and the improvement range in the observation group was greater than that in the control group during the same period (P<0.05). There were no significant differences in MODS, ARDS and mortality between the two groups (P>0.05). Conclusion: Patients with traumatic hemorrhagic shock can receive fluid resuscitation guided by the variability of the vena cava measured by ultrasound, and their pro-inflammatory factors, blood lactic acid levels, and immune function can be significantly improved, which helps to promote patient recovery.
