Blood cysts are rarely found during an echocardiogram in adults. They are benign lesions most often seen on the cardiac valves, mainly on the atrial side of the anterior mitral leaflet (AML). They occasionally cause symptoms and complications. Here, we report a patient with non-ST-elevation myocardial infarction, who on echocardiogram was found to have an AML mass suggestive of mitral valve blood cyst; the cyst was seen on the ventricular aspect of the AML. Cardiac blood cysts (BCs) are rare benign cardiac masses. This study aimed to summarize the clinical and echocardiographic characteristics and clinical outcomes of patients with BCs. This retrospective study enrolled patients diagnosed with BC between 2009 and 2021 at a single center in China. Their clinical and echocardiographic characteristics, management, and outcomes at follow-up are summarized. The study population comprised 26 patients with BCs with a mean age of 40 ± 19 years (median, 42 years; range, 27–56 years). The cohort was predominantly male (16, 62%). Twenty-two (81%) patients were symptomatic: dyspnea or exertional dyspnea (11, 42%) and palpitations (6, 23%) were the most frequent symptoms. Most cysts appeared as single round, oval, or lobulated anechoic masses, measuring 4–30 mm (mean, 18 mm). Half of the BCs (13, 50%) were pedunculated and mobile, and the others swung with the movement of the valve. The mitral valve (13, 50%) was the most common site of occurrence, followed by the tricuspid valve (6, 23%), right atrium (4, 15%), and aortic valve (3, 12%). Contrast echocardiography, which was performed for two patients, revealed a closed cyst without bubbles. Five cases were complicated by left ventricular outflow tract obstruction and six by valve prolapse. Eighteen patients underwent surgery, and one received antiplatelet therapy. Cardiovascular or cerebrovascular events did not occur in any patient during the follow-up period. Cardiac BCs are most commonly attached to the atrioventricular valve. Echocardiography is considered to be the most useful imaging modality for assessing BC. The management of patients with BC should be individualized based on the symptoms and associated complications. Cardiac blood cysts are benign tumors, usually congenital malformations, found on the endocardium, particularly along the closing lines of the heart valves. Blood cysts of the heart are commonly reported at postmortem findings in infants and they are very rare in adults. We report a case of a 39 year old patient, who was incidentally discovered during echocardiography having a blood cyst attached to the ventricular face of the anterior mitral valve and to the mitral chordae. During surgery, the cystic mass was resected. Mitral valvuloplasty was successfully performed and the patient had an uneventful recovery. Blood cysts are rarely reported, thus there is no consensus or guidelines for the optimal management of the asymptomatic cases. Although our patient was asymptomatic and the cyst did not interfere with the cardiac function, together with the heart team, we chose the surgical resection of the cardiac mass in order to prevent possible complications.

Blood cysts are rare cardiac tumors, often involve the atrioventricular valves. Blood cysts are usually small and tend to be clinically silent. Here, we report a case of blood cyst adhered to the junction of the pulmonary valve and the right ventricular outflow, causing severe outflow obstruction. This case was initially misdiagnosed as pulmonary valve stenosis by transthoracic echocardiography (TTE) and even underwent percutaneous balloon pulmonary valvuloplasty (PBPV). As the right ventricle‐to‐pulmonary artery peak gradient did not decrease after PBPV procedure, the boy underwent TTE again and the diagnosis was corrected as blood cyst. Cardiac blood cysts are very rare in adults. Their etiology, pathogenesis, and optimal management remain unclear. We report a case of a large left atrial cystic mass with clinical and imaging characteristics of a blood cyst that vanished between chest CT angiogram and trans-esophageal echocardiogram examinations done 2 days apart, without embolic phenomena. This patient likely had a blood cyst that spontaneously ruptured. This case is unusual since we did not find a case published in the English literature of a spontaneously ruptured blood cyst. Intracardiac blood cysts are rare congenital malformations most commonly located in the endocardium of semilunar or atrioventricular valves. They are predominantly seen in infants, especially below 2 months of age, and disappear spontaneously in the first 6 months of life making the detection of blood cysts a rare finding in the adult population. Although most patients are asymptomatic, they may occasionally present with obstruction, valvular dysfunction, or embolism. Blood cysts in the heart are often incidentally found during post-mortem examinations in infants, and usually do not cause clinical symptoms. They almost always regress spontaneously, and only a few cases of embolization or valvular dysfunction have been reported previously. We report a 62-year-old man who presented with chest discomfort and dyspnea on exertion for several years, and was found to have a dynamic murmur in left ventricular outflow tract (LVOT) on physical examination. His echocardiogram demonstrated two large blood cysts attached to the subvalvular apparatus of mitral valve, causing significant LVOT obstruction and mild to moderate mitral regurgitation. Cardiac blood cysts are confirmed by autopsies in stillbirths but rarely detected in adults. Although cardiac blood cysts are common in newborns, they disappear within a few months after birth. These cysts are blood‐filled diverticula surrounded by endothelium. Their possible etiology is invaginations of atrial endothelium into the stroma of atrioventricular valves. Most blood cysts originate from the left side of the heart and are located on the atrioventricular valves. Complication is followed in very little cases, such as embolic stroke, valvular dysfunction, and ventricular outflow system obstruction. Intracardiac blood cysts are thin-walled cysts, lined by flattened, cobblestone-shaped epithelium, and filled with nonorganized blood. During autopsy, they are found on cardiac valves in approximately 50% of infants below the age of 2 months and are rarely found after the age of 2 years. We report a case of giant blood cyst originating from the anterior mitral valve leaflet, which was discovered during a two-dimensional echocardiography examination performed for investigation of a systolic murmur in a 34-year-old healthy man. The female neonate, 27 days, 53 cm, 3450 g, was found dead in early morning hours. Baby was healthy, well nourished, and not neglected, up to the day before when she started to cough and scheduled for next-day regular pediatrician visit. Due to unexpected death, the autopsy was performed. Multiple oval, blood cysts, up to 0.5 cm, were found on the free valvular margins of the mitral valve, tricuspid valve, and aortic valve. The blood cysts were unilocular, filled with blood, and lined with flattened endothelial cells. The surrounding stroma was slightly edematous but without myxoid changes. [ABSTRACT FROM AUTHOR]

Purpose: To report a case of orbital cholesterol granuloma and discuss the orbital findings seen in this entity., Observation: A 38-year-old male presented with an 8-month history of progressive left upper lid ptosis and hypoglobus. Clinical examination was significant for 3 mm of hypoglobus and restricted supraduction in the left eye. Contrasted computed tomography imaging revealed a well-circumscribed lesion in the superotemporal orbit causing extensive bone erosion that appeared to arise from the lacrimal gland. An incisional biopsy was performed, and histopathological evaluation demonstrated fibrovascular tissue surrounding a mixture of histiocytes and cholesterol clefts, consistent with a cholesterol granuloma., Conclusions and Importance: Orbital cholesterol granulomas are rare lesions that are predominantly found in the superotemporal orbit. These lesions can be associated with marked bony changes in the superotemporal fossa that can be mistaken for a lacrimal gland neoplasm; however, bony erosion is a hallmark of this lesion and should be considered on the differential diagnosis of any lacrimal gland mass with extensive bony erosion.

Blood cysts are benign, congenital tumours of the heart endothelium, found most commonly on heart valves. Adult cases are rare, since most spontaneously regress with age. However, reports of symptomatic cases with embolic phenomena and valve dysfunction have been described. We present a case of a previously healthy 44-year-old woman with no cardiovascular risk factors who developed acute myocardial infarction caused by coronary artery embolism from a blood cyst of the anterior mitral valve leaflet.
