Background: Despite comprehensive study, the aetiology of stroke is not identified in 35% of cases. Aims: We conducted a study to assess the diagnostic capacity of N-terminal pro-B-type natriuretic peptide (NT-proBNP) in the identification of ischaemic stroke of cardioembolic origin. The secondary purpose of the study was to evaluate the prognostic value of NT-proBNP for predicting 90-day all-cause mortality. Methods: We designed a prospective observational study including patients hospitalised due to stroke between March 2019 and March 2020. Blood samples were collected on admission to the emergency department and serum NT-proBNP levels were determined. Statistical analysis was performed using a bivariate logistic regression model and receiver operating characteristic (ROC) and Kaplan–Meier curves. Statistical significance was established at p 499 pg/mL was the optimum value for diagnosing cardioembolic ischaemic stroke (sensitivity, 82%; specificity, 80%). Moreover, plasma NT-proBNP levels > 499 pg/mL were independently associated with cardioembolic stroke (OR: 9.881; p = .001). Finally, NT-proBNP > 1500 pg/mL was useful for predicting 90-day mortality (sensitivity, 70%; specificity, 93%). Conclusions: NT-proBNP was independently associated with cardioembolic stroke and should be quantified in blood tests within 24 h of stroke onset. High plasma levels (>499 pg/mL) may indicate an underlying cardioembolic cause, which should be further studied, while NT-proBNP >1500 pg/mL was associated with increased 90-day mortality. Resumen: Introducción: A pesar de un completo y exhaustivo estudio, la etiología de un 35% de los ictus isquémicos es desconocida. Objetivos: Evaluar la capacidad de diagnóstico del NT-proBNP en la identificación del ictus isquémico de origen cardioembólico. El objetivo secundario fue evaluar el valor pronóstico del NT-proBNP como predictor de mortalidad de cualquier causa tras 90 días del ictus. Métodos: Diseñamos un estudio observacional prospectivo, incluyendo pacientes hospitalizados por ictus. El periodo de reclutamiento fue de marzo de 2019 a marzo de 2020. Se obtuvieron muestras de sangre a su admisión en Urgencias y se midieron los niveles plasmáticos de NT-proBNP. El análisis estadístico fue realizado mediante un modelo bivariado de regresión logística, curva de la característica operativa del receptor (curvas ROC) y curvas Kaplan-Meier. Se consideraron significativas diferencias con p < 0,05. Resultados: Fueron incluidos 207 pacientes con un primer episodio de ictus isquémico agudo. Los niveles de NT-proBNP fueron significativamente mayores (p < 0,001) en el grupo de ictus isquémico cardioembólico (2.069 ± 488,5). Las curvas ROC mostraron que un NT-proBNP > 499 pg/mL es el valor óptimo para el diagnóstico de ictus isquémico cardioembólico (sensibilidad 82%, especificidad 80%). Además, los niveles plasmáticos de NT-proBNP > 499 pg/mL estaban asociados de forma independiente con el ictus isquémico cardioembólico (OR = 9.881, p = 0,001). Finalmente, un valor de NT-proBNP > 1.500 pg/mL fue útil para predecir la mortalidad de cualquier causa 90 días tras el ictus isquémico (sensibilidad = 70%, especificidad = 93%). Conclusiones: El valor de NT-proBNP estaba asociado independientemente al ictus isquémico de origen cardioembólico y debería ser considerado su análisis en las primeras 24 h desde el inicio del ictus. Niveles elevados (NT-proBNP > 499 pg/mL) pueden indicar un origen cardioembólico subyacente, el cual debería ser estudiado con mayor detalle, mientras que niveles de NT-proBNP > 1.500 pg/mL se asociaron con un incremento de la mortalidad a los 90 días del ictus.

El Síndrome de Sjögren (SS) es una enfermedad autoinmune de carácter sistémico, que afecta principalmente al sistema glandular exocrino, generando un funcionamiento anormal de las glándulas lacrimales y salivales. Objetivo: proporcionar una actualización sobre la identificación de nuevos biomarcadores y mecanismos moleculares implicados en la fisiopatogénesis del SS. Método: Revisión narrativa de la literatura en diferentes bases de datos, mediante la búsqueda de términos descritos incluidos en los tesauros MESH y DeCs, para artículos publicados a partir del año 2018. Resultados: presentamos evidencia que destaca la identificación de nuevos biomarcadores y mecanismos implicados en la fisiopatogénesis del SS, describiendo las vías de: linfocitos B, catepsina S, cistatina C, quimioquina C-X3-C modificada de ligando 1, quimiocina regulada por activación del timo, células T, proteína morfogenética ósea 6, estimulación del receptor de oxitocina, receptor de zinc, calponina-3. Conclusión: los avances en la tecnología facilita el análisis detallado de la genética y fisiopatogénesis del SS, impulsando el desarrollo de terapias específicas. La búsqueda de biomarcadores no invasivos responde a las limitaciones de los métodos existentes y la invasividad de las biopsias salivales, prometiendo mejoras diagnósticas y terapéuticas

9. Evaluation of the first 5 years of a glutaric aciduria type I neonatal screening programme in Asturias

Introduction: . Neonatal screening of glutaric aciduria type 1 (GA-1) has brought radical changes in the course and outcomes of this disease. This study analyses the outcomes of the first 5 years (2015–2019) of the AGA1 neonatal screening programme in our autonomous community. Material: . We conducted an observational, descriptive and retrospective study. All neonates born between January 1, 2015 and December 31, 2019 that participated in the neonatal screening programme were included in the study. The glutarylcarnitine (C5DC) concentration in dry blood spot samples was measured by means of tandem mass spectrometry applying a cut-off point of 0.25 µmol/L. Results: . A total of 30 120 newborns underwent screening. We found differences in the C5DC concentration based on gestational age, type of feeding and hours of life at sample collection. These differences were not relevant for screening purposes. There were no differences between neonates with weights smaller and greater than 1500 g. Screening identified 2 affected patients and there were 3 false positives. There were no false negatives. The diagnosis was confirmed by genetic testing. Patients have been in treatment since diagnosis and have not developed encephalopathic crises in the first 4 years of life. Conclusions: . Screening allowed early diagnosis of two cases of GA-1 in the first 5 years since its introduction in our autonomous community. Although there were differences in C5DC levels based on gestational age, type of feeding and hours of life at blood extraction, they were not relevant for screening. Resumen: Introducción: El cribado neonatal de la aciduria glutárica tipo 1(AG-1) ha supuesto un cambio radical en la evolución y debut de la enfermedad. En este estudio se analizan los resultados de los primeros cinco años (2015–2019) del programa de cribado neonatal (PCN) de AG-1 en nuestra comunidad. Material: Se diseñó un estudio observacional, descriptivo y retrospectivo. La población estaba formada por neonatos participantes en el PCN nacidos entre el 1 de enero de 2015 y el 31 de diciembre de 2019. La determinación de glutarilcarnitina (C5DC) en sangre impregnada en papel se realizó mediante espectrometría de masas en tándem con punto de corte en 0,25 µmol/L. Resultados: El cribado se llevó a cabo en 30120 recién nacidos. En el análisis del marcador C5DC se encontraron diferencias en la concentración dependiendo de la edad gestacional, del tipo de alimentación y de las horas de vida a la extracción. Estas diferencias no fueron relevantes a efectos del cribado. No se observaron diferencias entre menores y mayores de 1500 g. Se identificaron 2 casos afectos y 3 falsos positivos. No se detectaron falsos negativos. Los diagnósticos fueron confirmados mediante estudio genético. Los pacientes reciben tratamiento desde el diagnóstico y no han presentado crisis encefalopáticas los primeros cuatro años de vida. Conclusiones: El cribado ha permitido el diagnóstico precoz de dos casos de AG-1 en los cinco primeros años desde su instauración en nuestra comunidad. Aunque existen diferencias en los niveles de C5DC en función de edad gestacional, del tipo de alimentación y de las horas de vida a la extracción, estas no tuvieron trascendencia para el cribado.

11. Predictores de la respuesta al tratamiento en pacientes con nefritis lúpica asociada con microangiopatía trombótica.

- Abstract
OBJECTIVE: To gather the biomarkers identified in the current literature capable of predicting the response to treatment of patients with lupus nephritis associated with thrombotic microangiopathy. MATERIALS AND METHODS: Retrospective study based on a bibliographic search of articles published in the last five years and registered in the databases PubMed, Science Direct, Clinical Key, Google Scholar, Embase, Lilacs and Scielo with the terms MesH lupus nephritis, thrombotic microangiopathy and systemic lupus erythematosus, as well as the key terms treatment and predictors. Studies with outcomes of interest were selected regardless of study type. RESULTS: Several biomarkers are currently useful in lupus nephritis; among them proteinuria continues to be the strongest biomarker for predicting long-term renal outcome. Many new molecules have proven to be useful in the diagnosis and prognosis of this disease, such as monocyte chemoattractant protein and gelastin-associated lipocalin, among others. Finally, some biomarkers such as VCAM-1, T-CD8 lymphocyte count and CD11b protein decreased with clinical remission. CONCLUSIONS: Lupus nephritis is a frequent complication of systemic lupus erythematosus that associated with thrombotic microangiopathy worsens the prognosis of clinical outcome of the patient, likewise, several diagnostic methods and biomarkers have been identified that can contribute to early and successful intervention in patients with this disease. - Abstract
Thyroid cancer (TC) is the first malignant tumor in endocrine glands, and it is estimated that by 2030 it will be in the top five cancers worldwide. In Chile, TC was recently included as pathology No. 82 with Explicit Health Guarantees, which implies allocating significant public and private resources to the treatment of patients with a cancer that is on the rise. There are groups of young patients with small tumors that present a more aggressive clinical behavior, where decision-making could be advanced. Specifically, a group of patients under 55 years of age with tumors less than 2 cm in size but with regional metastases, that lack indication for radioiodine therapy and could require complementary treatment or present a worse evolution. Clinical and molecular tools to guide appropriate treatment in patients with lymph node metastases are limited and have not been updated. There are tumorigenicity and prognostic factors, such as EpithelialMesenchymal Transition (EMT) and Cancer Stem Cells (CSC) markers that have been incorporated into the study of other tumors and recently in thyroid cancer. Studies linking EMT and CSC with TC currently point to molecular and genetic description, with few reports clinically correlating these findings (particularly in subgroups with particular characteristics of aggressiveness) and proposing them as tumorigenicity and prognosis markers. The description of these biomarkers in the described population could facilitate decision-making regarding follow-up, surgical therapy, and radioiodine therapy. [ABSTRACT FROM AUTHOR]

Introducción y objetivo: La contaminación por metales pesados, y en particular por cromo hexavalente, es un tema de preocupación mundial debido a la alta toxicidad y persistencia ambiental que exhibe este metal. La fitorremediación surge como una tecnología económica para la recuperación de ambientes contaminados por metales. Las macrófitas acuáticas, como Salvinia mínima, son excelentes candidatas como especies remediadoras de cuerpos de agua. Para optimizar su utilización es importante la identificación de biomarcadores eficientes y confiables. El objetivo del presente trabajo fue realizar un análisis temporal de distintos parámetros bioquímicos para identificar tanto biomarcadores a la exposición a Cr(VI) como el momento más apropiado para su análisis en plantas de S. minima bajo condiciones de laboratorio. M&M: Se cultivaron ejemplares de S. mínima en presencia y ausencia de cromo hexavalente, durante siete días, en condiciones controladas de luz y temperatura. Se tomaron muestras de tejidos y de la solución de cultivo a los 0, 2, 4 y 7 días. Se cuantificaron los niveles de H2O2, malondialdehído, fenoles solubles e insolubles y sacarosa, en pinnas flotantes y raquis sumergido. En la solución de tratamiento se determinó Cr(VI) remanente. Todas las determinaciones se realizaron espectrofotométricamente. Resultados: Los resultados permitieron identificar, entre los parámetros seleccionados, biomarcadores tempranos (malondialdehído), de mediano plazo (sacarosa y fenoles insolubles) y tardíos (peróxido de hidrógeno). Conclusiones: Se analizan y discuten las implicancias fisiológicas de las variaciones observadas.

Extracellular vesicles (EVs) are small vesicles released by cells that facilitate cell signaling. They are categorized based on their biogenesis and size. In the context of the central nervous system (CNS), EVs have been extensively studied for their role in both normal physiological functions and diseases like Alzheimer's disease (AD). AD is a neurodegenerative disorder characterized by cognitive decline and neuronal death. EVs have emerged as potential biomarkers for AD due to their involvement in disease progression. Specifically, EVs derived from neurons, astrocytes, and neuron precursor cells exhibit changes in quantity and composition in AD. Neuron-derived EVs have been found to contain key proteins associated with AD pathology, such as amyloid beta (Aß) and tau. Increased levels of Aß in neuron-derived EVs isolated from the plasma have been observed in individuals with AD and mild cognitive impairment, suggesting their potential as early biomarkers. However, the analysis of tau in neuron-derived EVs is still inconclusive. In addition to Aß and tau, neuron-derived EVs also carry other proteins linked to AD, including synaptic proteins. These findings indicate that EVs could serve as biomarkers for AD, particularly for early diagnosis and disease monitoring. However, further research is required to validate their use and explore potential therapeutic applications. To summarize, EVs are small vesicles involved in cell signaling within the CNS. They hold promise as biomarkers for AD, potentially enabling early diagnosis and monitoring of disease progression. Ongoing research aims to refine their use as biomarkers and uncover additional therapeutic applications.

La diabetes tipo II es una enfermedad crónica creciente, que se asocia con el riesgo de daño renal, una complicación que puede llevar a la insuficiencia renal. Por ende, la detección temprana y el monitoreo del daño renal es fundamental en este tipo de pacientes. En este contexto, la cistatina C es un marcador endógeno confiable para evaluar la función renal, puesto que, su concentración no está influenciada por factores externos a diferencia de otros marcadores endógenos. El objetivo es analizar la cistatina C como marcador de daño renal en pacientes con diabetes mellitus tipo II. Se realizó una investigación exhaustiva, en las diferentes bases de datos, para identificar estudios relacionados al tema de interés. Se empleó el método PRISMA. Los estudios revisados demostraron una asociación entre la concentración elevada de la cistatina C con un riesgo de daño renal en pacientes con diabetes. Además, la cistatina C sugiere ser un marcador más sensible que la creatinina para detectar problemas renales en etapas tempranas. Se concluyó que la cistatina C es un marcador prometedor para la evaluación de daño renal en pacientes con diabetes tipo II, por su sensibilidad para detectar cambios tempranos y determinar la progresión de la enfermedad. Timely detection and understanding of causes for population decline are essential for effective wildlife management and conservation. Assessing trends in population size has been the standard approach, but we propose that monitoring population health could prove more effective. We collated data from 7 bottlenose dolphin (Tursiops truncatus) populations in the southeastern United States to develop a method for estimating survival probability based on a suite of health measures identified by experts as indices for inflammatory, metabolic, pulmonary, and neuroendocrine systems. We used logistic regression to implement the veterinary expert system for outcome prediction (VESOP) within a Bayesian analysis framework. We fitted parameters with records from 5 of the sites that had a robust network of responders to marine mammal strandings and frequent photographic identification surveys that documented definitive survival outcomes. We also conducted capture–mark–recapture (CMR) analyses of photographic identification data to obtain separate estimates of population survival rates for comparison with VESOP survival estimates. The VESOP analyses showed that multiple measures of health, particularly markers of inflammation, were predictive of 1‐ and 2‐year individual survival. The highest mortality risk 1 year following health assessment related to low alkaline phosphatase (odds ratio [OR] = 10.2 [95% CI: 3.41–26.8]), whereas 2‐year mortality was most influenced by elevated globulin (OR = 9.60 [95% CI: 3.88–22.4]); both are markers of inflammation. The VESOP model predicted population‐level survival rates that correlated with estimated survival rates from CMR analyses for the same populations (1‐year Pearson's r = 0.99, p = 1.52 × 10–5; 2‐year r = 0.94, p = 0.001). Although our proposed approach will not detect acute mortality threats that are largely independent of animal health, such as harmful algal blooms, it can be used to detect chronic health conditions that increase mortality risk. Random sampling of the population is important and advancement in remote sampling methods could facilitate more random selection of subjects, obtainment of larger sample sizes, and extension of the approach to other wildlife species. 24. Comparación de biomarcadores inflamatorios plasmáticos en pacientes con SIM-PedS versus infecciones potencialmente graves en pacientes pediátricos.

Los biomarcadores inflamatorios se han utilizado para el diagnóstico y tratamiento del síndrome inflamatorio multisistémico en niños (SIM-PedS). Nuestro objetivo fue determinar cómo se comportan estos biomarcadores inflamatorios en pacientes con síndrome febril orientados en principio como infección bacteriana potencialmente grave y comparar los hallazgos clínicos y de laboratorio con los casos SIM-PedS. Estudio de cohorte observacional ambispectivo unicéntrico (junio de 2020-febrero de 2022). Analizamos la demografía, los síntomas clínicos y los hallazgos de laboratorio en casos SIM-PedS y en casos de síndrome febril de otras etiologías infecciosas de pacientes menores de 15 años con ingreso hospitalario. Incluimos a 54 pacientes con sospecha analítica de infección bacteriana y a 20 pacientes con SIM-PedS para el análisis. La fiebre (100%), los hallazgos gastrointestinales (80%) y mucocutáneos (35%) fueron más frecuentes en los pacientes con SIM-PedS, también la hipotensión (36,8%) y la taquicardia (55%). Los hallazgos de laboratorio mostraron niveles significativamente elevados de pro-BNP (70%), ferritina (35%), dímeros D (80%) así como linfopenia (55%) y trombocitopenia (27,8%) en los casos de SIM-PedS. Los valores de IL-6 fueron elevados en pacientes sin SIM-PedS (92,6%). En el manejo de pacientes con SIM-PedS, la monitorización dinámica de pro-BNP, ferritina, dímero D, linfocitos y plaquetas podría ser útil para evaluar efectivamente el progreso de la enfermedad en las primeras fases. Los valores de IL-6 pueden elevarse de forma significativa en pacientes con síndrome febril de otras etiologías, así como los dímeros D. El uso de diversos biomarcadores de laboratorio podría ayudar a determinar precozmente la evolución de los pacientes con síndrome febril. Inflammatory biomarkers have been used for the diagnosis and management of multisystemic inflammatory syndrome in children (MIS-C). We aimed to compare the clinical and laboratory findings of MIS-C cases versus other febrile cases cataloged as potentially suspected bacterial infection (non-MIS-C). Unicentric ambispective observational cohort study (June 2020 to February 2022). We analyzed demographics, clinical symptoms and laboratory findings in MIS-C cases and in non-MIS-C cases with febrile processes of patients under 15 years of age admitted to hospital. We enrolled 54 patients with potential suspected bacterial infection and 20 patients with MIS-C for analysis. Fever (100%), gastrointestinal (80%) and mucocutaneous findings (35%) were common in MIS-C patients, also hypotension (36.8%) and tachycardia (55%). Laboratory findings showed significantly elevated proBNP (70%), ferritin (35%), D-dimer (80%) and lymphopenia (55%) and thrombocytopenia (27.8%) in MIS-C cases. IL-6 values were high in non-MIS-C patients (92.6%). In the management of MIS-C patients, the dynamic monitoring of proBNP, ferritin, D-dimer, lymphocytes and platelets could be helpful to pediatricians to effectively evaluate the progress of MIS-C in the early phases, not IL-6 values. The applicability of the IL-6 level as a prognostic biomarker in MIS-C patients may require closer discussion. In addition, the optimal laboratory markers, as stated in our study, can help establish a biomarkers model to early distinguish the MIS-C versus non-MIS-C in patients who are admitted to febrile syndrome. Resumo Fundamento A regurgitação valvar pulmonar é uma importante complicação de longo prazo em pacientes com tetralogia de Fallot (TF). Objetivo O presente estudo tem como objetivo investigar os efeitos do implante valvar pulmonar (IVP) na anatomia e função do ventrículo direito (VD) e na evolução em longo prazo da prótese implantada em posição pulmonar. Métodos Uma análise de coorte retrospectiva e unicêntrica foi realizada em 56 pacientes consecutivos com TF submetidos a IVP. O estudo incluiu pacientes de ambos os gêneros, com idade ≥ 12 anos e compreendeu avaliação de dados clínicos e cirúrgicos, ressonância magnética cardiovascular pré e pós-operatória e dados ecocardiográficos obtidos mais de 1 ano após IVP. Resultados Após o IVP, houve uma diminuição significativa do volume sistólico final do VD indexado pela área de superfície corpórea (ASC), de 89 mL/ASC para 69 mL/ASC (p < 0,001) e do volume diastólico final indexado do VD, de 157 mL/ASC para 116 mL/ASC (p < 0,001). Além disso, houve aumento da fração de ejeção corrigida do VD [ FEVDc = fluxo pulmonar ajustado (fluxo pulmonar anterógrado − fluxo regurgitante) / volume diastólico final do VD ] de 23% para 35% (p < 0,001) e da fração de ejeção do ventrículo esquerdo de 58% para 60% (p = 0,008). No entanto, foi observado um aumento progressivo no gradiente de pico da válvula pulmonar ao longo do tempo, com 25% dos pacientes apresentando um gradiente superior a 60 mmHg. Próteses menores (tamanhos 19 a 23) foram associadas a um risco 4,3 vezes maior de gradiente > 60 mmHg em comparação com próteses maiores (tamanhos 25 a 27; p = 0,029; intervalo de confiança: 1,18 a 17,8). Conclusão Conforme esperado, o IVP demonstrou melhorias nos volumes e na função do VD. O acompanhamento e a vigilância a longo prazo são cruciais para avaliar a durabilidade da prótese e detectar potenciais complicações. O dimensionamento adequado das próteses é essencial para melhorar a longevidade da prótese.

Resumo Fundamento Embora tenha havido melhorias significativas no tratamento da insuficiência cardíaca (IC) nas últimas décadas, seu prognóstico permanece desfavorável. Embora existam muitos biomarcadores que podem ajudar a prever o prognóstico de pacientes com IC, há necessidade de biomarcadores mais simples, menos dispendiosos e mais facilmente disponíveis. Objetivo Avaliar o valor preditivo do valor pan-imune-inflamatório (PIV, do inglês pan-immune-inflammation value) em pacientes com IC agudamente descompensada. Métodos Analisamos 409 pacientes com IC com fração de ejeção reduzida internados por IC aguda descompensada. Os pacientes foram divididos em 3 grupos de acordo com os tercis de PIV: tercil 1 (PIV < 357,25), tercil 2 (PIV ≥ 357,25 e < 834,55) e tercil 3 (PIV ≥ 834,55). Foram considerados estatisticamente significativos valores de p < 0,05. Curvas de Kaplan-Meier e modelos de regressão de riscos proporcionais de Cox foram utilizados para avaliar a associação entre PIV e mortalidade por todas as causas. O desfecho primário foi mortalidade por todas as causas em 5 anos, e o desfecho secundário compreendeu a mortalidade por todas as causas intra-hospitalar em 30 dias, em 180 dias e em 1 ano Resultados Mostramos que valores mais elevados de PIV estavam associados a desfechos primários e secundários. A curva de Kaplan-Meier mostrou que pacientes com valores mais elevados de PIV apresentaram risco aumentado de mortalidade por todas as causas em curto e longo prazo (log-rank p < 0,001). Na análise multivariada, o PIV foi identificado como um preditor independente de mortalidade por todas as causas em longo prazo em pacientes com IC aguda descompensada, e observamos um aumento de 1,96 vezes no risco de um evento (razão de chances: 1,96; intervalo de confiança de 95%: 1,330 a 2,908; p = 0,001). Conclusões Nosso estudo mostrou que o novo biomarcador PIV pode ser usado como preditor de prognóstico em pacientes com IC aguda descompensada.

El diagnóstico de Cardiomiopatía Cirrótica está basado en la presencia de cirrosis hepática avanzada con alteraciones de la función cardíaca sin cardiopatía pre-existente, pero en gran parte de su evolución natural ésta es subclínica. Por ello son imprescindibles los estudios complementarios no invasivos para confirmar el diagnóstico y su rol pronóstico en pacientes con o sin trasplante hepático. En esta segunda parte revisamos los métodos de diagnóstico desde el ECG hasta las imágenes de resonancia magnética cardíaca.

En la actualidad se cuenta con diversos enfoques y medidas terapéuticas para un paciente con cáncer gástrico. La gastrectomía es uno de los procedimientos quirúrgicos más utilizado en la práctica clínica para su tratamiento. Luego de ser realizado este procedimiento, los pacientes pueden presentar complicaciones postquirúrgicas, las cuales son un reto en el ámbito clínico, ya que ponen en riesgo la salud y la vida del paciente. Se ha venido investigando la correlación entre los biomarcadores, el estadio inflamatorio y su importancia para un hallazgo temprano de complicaciones en diversas patologías o procedimientos. En el caso de la gastrectomía, esta relación inflamación/biomarcadores es positiva, ampliando así el panorama para poder estar un paso adelante y prevenir, en mayor parte, un deterioro postquirúrgico del paciente. En este escrito se puede apreciar cómo se relacionan los niveles de determinados biomarcadores y su relación con las complicaciones que ponen en riesgo la efectividad y el éxito de la cirugía.

Introducción: El cáncer de cabeza y cuello es la sexta forma más común de cáncer humano; constituye un reto, para la odontología reducir la mortalidad y la morbilidad por su causa, mediante el desarrollo de herramientas diagnósticas que lo detecten en sus primeras etapas. Objetivo: Describir el papel de los biomarcadores salivales como herramienta diagnóstica para la detección temprana de cáncer bucal. Desarrollo: Los biomarcadores salivales proporcionan información adicional a la obtenida en el estudio histopatológico de este tipo de cáncer y pueden constituir una herramienta de análisis no invasivo y de bajo costo en su diagnóstico precoz. Conclusiones: A pesar de que hasta la fecha no existe un biomarcador ideal para el diagnóstico temprano del cáncer bucal, los investigadores declaran un futuro prometedor en el uso de la saliva como fuente de biomarcadores para estas enfermedades, debido en gran parte, al contacto directo de la saliva con la mucosa oral y el avance tecnológico en el campo de la medicina molecular.

30. Neurodevelopmental outcomes in congenital heart disease: Usefulness of biomarkers of brain injury

Introduction: At present, neurodevelopmental abnormalities are the most frequent type of complication in school-aged children with congenital heart disease (CHD). We analysed the incidence of acute neurologic events (ANEs) in patients with operated CHD and the usefulness of neuromarkers for the prediction of neurodevelopment outcomes. Methods: Prospective observational study in infants with a prenatal diagnosis of CHD who underwent cardiac surgery in the first year of life. We assessed the following variables: (1) serum biomarkers of brain injury (S100B, neuron-specific enolase) in cord blood and preoperative blood samples; (2) clinical and laboratory data from the immediate postnatal and perioperative periods; (3) treatments and complications; (4) neurodevelopment (Bayley-III scale) at age 2 years. Results: the study included 84 infants with a prenatal diagnosis of CHD who underwent cardiac surgery in the first year of life. Seventeen had univentricular heart, 20 left ventricular outflow obstruction and 10 genetic syndromes. The postoperative mortality was 5.9% (5/84) and 10.7% (9/84) patients experienced ANEs. The mean overall Bayley-III scores were within the normal range, but 31% of patients had abnormal scores in the cognitive, motor or language domains. Patients with genetic syndromes, ANEs and univentricular heart had poorer neurodevelopmental outcomes. Elevation of S100B in the immediate postoperative period was associated with poorer scores. Conclusions: children with a history of cardiac surgery for CHD in the first year of life are at risk of adverse neurodevelopmental outcomes. Patients with genetic syndromes, ANEs or univentricular heart had poorer outcomes. Postoperative ANEs may contribute to poorer outcomes. Elevation of S100B levels in the postoperative period was associated with poorer neurodevelopmental outcomes at 2 years. Studies with larger samples and longer follow-ups are needed to define the role of these biomarkers of brain injury in the prediction of neurodevelopmental outcomes in patients who undergo surgery for management of CHD. Resumen: Introducción: En la actualidad, las alteraciones del neurodesarrollo son la complicación más frecuente en pacientes con cardiopatía congénita (CC) en edad escolar. Analizamos la incidencia de eventos neurológicos agudos (ENA) en pacientes con CC sometidos a cirugía cardiaca y la utilidad de los neuromarcadores para predecir el neurodesarrollo. Métodos: Estudio prospectivo observacional en recién nacidos (RN) con CC diagnosticada prenatalmente sometidos a cirugía el primer año de vida. Se evaluaron: 1) biomarcadores sanguíneos de lesión cerebral (S100B, enolasa neuronal específica) en sangre de cordón y periquirúrgicos; 2) datos clínicos y analíticos perinatales y periquirúrgicos; 3) tratamientos y complicaciones; 4) neurodesarrollo (Escala Bayley III) a los dos años. Resultados: Se incluyeron 84 RN con CC de diagnóstico fetal, confirmada postnatalmente, sometidos a cirugía cardiaca en el primer año de vida. 17 pacientes tenían corazón univentricular, 20 pacientes obstrucción izquierda y 10 síndromes genéticos. Fallecieron en el periodo postquirúrgico 5 pacientes (5,9%) y 9 pacientes presentaron ENA (10,7%). Las puntuaciones medias en el test de Bayley III fueron normales pero el 31% tuvieron alteración cognitiva, motora o en el lenguaje. Los pacientes con síndromes genéticos, ENA y CC univentriculares tuvieron peor neurodesarrollo. La elevación de S100B en el postoperatorio inmediato se correlacionó con peores puntuaciones. Conclusiones: Los pacientes con CC sometidos a cirugía tienen mayor riesgo de sufrir alteraciones del neurodesarrollo. Los pacientes con síndromes genéticos o corazones univentriculares presentan peores resultados. Presentar ENA postquirúrgico puede contribuir a peores resultados. Niveles de s100B elevados en el postoperatorio se correlacionan con peores resultados en los tests de neurodesarrollo a los dos años. Series con más pacientes y con seguimiento a largo plazo nos ayudarán a definir el papel de estos biomarcadores de lesión cerebral en la predicción del neurodesarrollo en pacientes sometidos a cirugía de CC.

31. Biomarkers of Oxidative Stress in Huntington's Disease and Other Neurological Disorders: a Comparative Study

Foundation: biomarkers of oxidative stress in Huntington's disease could predict the course of the disease and evaluate new treatments, but their nonspecific nature seems to prevent the identification of any useful marker. Clarifying similarities and differences of this phenomenon and its behavior with clinical characteristics may be essential. Objective: compare biomarkers of oxidative stress between patients with Huntington's disease and other neurological disorders. Methods: an analytical, retrospective and case-control study was carried out (Huntington's disease, amyotrophic lateral sclerosis, spinocerebellar ataxia type 2 and ischemic stroke: acute and chronic stage). Demographic and clinical variables and markers of oxidative damage (malonildialdehyde, advanced protein oxidation products) and antioxidants (superoxide dismutase: catalase: glutathione peroxidase, plasma antioxidant capacity) were collected. Results: there were significant differences in malonyldialdehyde in Huntington's disease compared to the control (p=0.02), but not with the rest of the groups. The enzyme superoxide dismutase in Huntington's disease was statistically lower compared to amyotrophic lateral sclerosis, although for catalase it was higher in relation to the rest of the patients. FRAP in Huntington's disease was significantly lower versus amyotrophic lateral sclerosis and acute ischemic stroke. Advanced products of protein oxidation were directly correlated with the biological and onset ages of Huntington's disease. Motor activity in amyotrophic lateral sclerosis and neurological deficit in acute ischemic stroke were correlated with malonyldialdehyde and glutathione peroxidase, respectively. Conclusions: huntington's disease seems to show specific characteristics in its antioxidant system. Protein oxidation could be related to the accumulation of mutated huntingtin over time.

Precision medicine has revolutionized the field of neuroimmunology, with innovative approaches that characterize diseases based on their biology, deeper understanding of the factors leading to heterogeneity within the same disease, development of targeted therapies, and strategies to tailor therapies to each patient. This review explores the impact of precision medicine on various neuroimmunological conditions, including multiple sclerosis (MS), neuromyelitis optica spectrum disorder (NMOSD), myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), optic neuritis, autoimmune encephalitis, and immune-mediated neuropathies. We discuss advances in disease subtyping, recognition of novel entities, promising biomarkers, and the development of more selective monoclonal antibodies and cutting-edge synthetic cell-based immunotherapies in neuroimmunological disorders. In addition, we analyze the challenges related to affordability and equity in the implementation of these emerging technologies, especially in situations with limited resources.

In recent decades, there have been significant advances in the diagnosis of diffuse gliomas, driven by the integration of novel technologies. These advancements have deepened our understanding of tumor oncogenesis, enabling a more refined stratification of the biological behavior of these neoplasms. This progress culminated in the fifth edition of the WHO classification of central nervous system (CNS) tumors in 2021. This comprehensive review article aims to elucidate these advances within a multidisciplinary framework, contextualized within the backdrop of the new classification. This article will explore morphologic pathology and molecular/genetics techniques (immunohistochemistry, genetic sequencing, and methylation profiling), which are pivotal in diagnosis, besides the correlation of structural neuroimaging radiophenotypes to pathology and genetics. It briefly reviews the usefulness of tractography and functional neuroimaging in surgical planning. Additionally, the article addresses the value of other functional imaging techniques such as perfusion MRI, spectroscopy, and nuclear medicine in distinguishing tumor progression from treatment-related changes. Furthermore, it discusses the advantages of evolving diagnostic techniques in classifying these tumors, as well as their limitations in terms of availability and utilization. Moreover, the expanding domains of data processing, artificial intelligence, radiomics, and radiogenomics hold great promise and may soon exert a substantial influence on glioma diagnosis. These innovative technologies have the potential to revolutionize our approach to these tumors. Ultimately, this review underscores the fundamental importance of multidisciplinary collaboration in employing recent diagnostic advancements, thereby hoping to translate them into improved quality of life and extended survival for glioma patients.

Objective. To analyze the accuracy of cell ratios in the diagnosis of pediatric acute appendicitis while introducing a new one --the derived neutrophil-to-lymphocyte ratio (dNLR). Materials and methods. An observational, retrospective study of patients aged 0-15 years old diagnosed with acute appendicitis (AA) and with non-surgical abdominal pain (AP) treated in our institution from 2021 to 2022 was carried out. The neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), platelet- to-lymphocyte ratio (PLR), and dNLR were compared between groups. Results. 98 AA patients (30% of whom were female; age: 10 ± 3.3 years) and 97 AP patients (53% of whom were male; age: 9.3 ± 3.7 years) were included. NLR, MLR, PLR, and dNLR values were higher in AA patients than in AP patients: 9.6 IQR (interquartile range) 9.5 vs. 3.3 IQR 5.3: p < 0.0001; 0.7 IQR 0.6 vs. 0.46 IQR 0.7: p < 0.023; 199.8 IQR 163.9 vs. 134.0 IQR 129.2: p < 0.0001; and 5.29 IQR 3.9 vs. 2.39 IQR 2.7; p < 0.0001, respectively. Sensitivity, specificity, positive-negative predictive value, area under the ROC curve, and dNLR cut-off point for AA diagnosis were 70%, 78%, 77-72%, 0.811, and 3.98, respectively. Conclusions. Cell ratios are useful and cost-effective inflammatory parameters in the diagnosis of pediatric acute appendicitis. The results of this study suggest dNLR has the greatest clinical accuracy. - Abstract
The rational use of antimicrobials promotes lower risk for the patient and the development of bacterial resistance. Thus, the objective was to evaluate the use of procalcitonin (PCT) as an auxiliary test in monitoring antimicrobial treatment for patients with respiratory infection (RI) or sepsis, admitted to Intensive Care Units (ICU). The data were collected from electronic medical records. The 235 selected patients were divided based on adherence to the institutional protocol that guides the request for PCT, into group 1 (PCT in accordance with the protocol) (n=33); group 2 (PCT not in accordance with the protocol) (n=25) and group 3 (PCT not requested) (n=177). Patients in group 1 had a shorter average duration of antibiotic therapy than the other groups. The length of hospitalization and ICU stay was shorter in group 3 compared to group 2. There was a statistical difference in outcomes; there was a higher frequency of deaths than discharges in groups 2 and 3, unlike group 1, where discharges outnumbered deaths. Discontinuation of treatment based on a PCT protocol leads to a reduction in antimicrobial exposure in patients with lower respiratory tract infections or sepsis, with no negative impact on mortality. Resumo Fundamento: O software ablation index (AI) permitiu melhorar os resultados da ablação de fibrilação atrial (FA), mas as taxas de recorrência permanecem significativas. Biomarcadores séricos específicos têm sido associados a essa recorrência. Objetivos: Avaliar se certos biomarcadores podem ser utilizados (individualmente ou combinados) para predizer a recorrência de FA pós ablação guiada pelo AI. Métodos: Estudo multicêntrico, observacional, prospectivo de pacientes consecutivos, encaminhados para ablação de FA de janeiro de 2018 a março de 2021. Hemoglobina, peptídeo natriurético cerebral (BNP), proteína C reativa, troponina I ultrassensível, clearance de creatinina, Hormônio Tireoestimulante (TSH), e Tiroxina livre (T4) foram avaliados quanto à capacidade de prever a recorrência de arritmias durante o acompanhamento. Valores de p 1,8 μUI/mL [HR=1,82 (IC95%, 1,89-2,80), p=0,006] foi um preditor independente de recorrência. Avaliando-se a combinação de TSH, FT4 e BNP, a adição de cada valor “anormal” foi associada a uma menor sobrevida livre de recorrência (87,1% se nenhum vs. 83,5% se um vs. 75,1% se dois vs. 43,3% se três biomarcadores, p
