Bakgrund: Vården ska utgå ifrån att varje patient kräver individanpassad vård. Patientens behov och berättelse är grunden för en delaktighet i vården. Arbetet kring patienten ska ske personcentrerat vilket ställer krav på arbetssätt och vårdens utformning. Syfte: Syftet var att beskriva patientens upplevelse av bedsiderapportering mellan arbetsskift inom akutsjukvård. Metod: En allmän litteraturöversikt användes som metod. Databaserna CINAHL och PubMed användes för att söka artiklar. Även manuella sökningar gjordes för att besvara syftet. Totalt valdes 15 artiklar ut till resultatet. Dessa artiklar granskades genom Sophiahemmets modifierad granskningsmall för att klassificera och kvalitetsbedöma artiklarna. En integrerad analys genomfördes av artiklarna som därefter kodades och sammanställdes. Återkommande innehåll i resultat artiklarna skapade teman som sedan summerades till ett resultat. Resultat: Artiklarna som presenteras i resultatet är publicerade under åren 2011-2020. Resultatet delades in i två teman med fyra respektive tre underkategorier. Patientens upplevelse av bedsiderapportering och patientens upplevelse av information vid bedsiderapportering. Under dessa teman skapades fem kategorier, att bli lyssnad på, den deltagande rollen, sekretess, den personliga kontakten och vikten av tillräcklig information. Slutsats: Slutsatsen var att bedsiderapportering inom akutsjukvården är viktig för att patienten ska uppmuntras vara delaktig i vården. Patienter kände sig sedda och får en bättre följsamhet till vården. Det bidrar till en ökad patientsäkerhet och mindre vårdskador. Mer forskning behövs inom svensk sjukvård för att säkerställa resultatet av patientens upplevelse kring bedsiderapportering och effekterna de i längden ger på vården och arbetssätt Background: The healthcare must be based on each patient requiring individual care. The patient's needs and background is the basis for a participation in healthcare. It should be person-centered which requires a good working method and care design. Purpose: The purpose was to describe the patients experience of bedside report between work shifts in emergency care. Method: A general literature review was used as the method. The databases CINAHL and PubMed were used for the article searches. Also, manual searches were performed to answer the purpose. A total of 15 articles were selected for the result. These articles were reviewed by Sophiahemmets modified the review template to classify and quality assess the articles. An integrated analysis was conducted of the articles which were encoded and summarized. Recurring content in the result articles made themes that created the results. Results: The articles presented in the results are published during the years 2011-2020. The result was divided into two themes with four and three subcategories respectively. The patient's experience of participation in bedside reporting and the patient's experience of information in bedside reporting. Under these themes were five categories, to be listened to, the participatory role, secrecy, the personal contact, and information. Conclusion: The conclusion was that bedside reporting in emergency care is important for the patient to be able to participate in. Patients feel more respected and have better compliance. This contributes to increased patient safety and decreases the risk for medical errors. However, more research is needed in Swedish hospitals in the area to be able to optimize the results of bedside reporting from the patients' perspective and investigate the long-term effects of the care and work routines. Bakgrund: En fungerande kommunikation inom hälso- och sjukvård är viktigt för att förebygga misstag som potentiellt kan leda till en vårdskada. Kommunikation förekommer i olika situationer inom vården och däribland vid rapportering mellan sjuksköterskor. Det finns olika rapporteringsmodeller att tillämpa och en av dem är bedsiderapportering. Bedsiderapportering innebär att patienten får närvara under rapporteringen och således har patienten möjlighet att fylla i med saker som sjuksköterskorna glömmer bort eller blir fel. Patienternas uppfattning är att bedsiderapportering förbättrade den individuella vården och att de i större utsträckning kunde fråga och få ytterligare information.Syfte: Syftet är att beskriva sjuksköterskans upplevelse att arbeta med bedsiderapportering.Metod: En litteraturöversikt vars resultat är baserat på 16 vetenskapliga artiklar som är av kvalitativ, kvantitativ och mixad metod. Artiklarna är publicerade mellan 2011–2021.Resultat: Enligt sjuksköterskorna leder bedsiderapportering till att patienterna blir mer involverade i sin vård och patientsäkerheten ökar. Detta medför att sjuksköterskorna känner att de behöver mer tid att förbereda sig för att gå in till patienter och sjuksköterskorna menade att detta bidrar till att bedsiderapportering blir en tidskrävande rapporteringsmodell.Konklusion: Bedsiderapportering har en positiv effekt på sjuksköterskors vårdrelation till patienter. Sjuksköterskorna ansåg att bedsiderapportering medför en högre patientsäkerhet och att patienternas deltagande vid rapportering är positiv. Sjuksköterskorna ansåg att ett modifierat tillvägagångssätt som bestod av både bedsiderapportering och traditionell rapportering vore det ultimata sättet att föra rapport. Background: Functional communication in health care is important to prevent mistakes that can potentially lead to a care injury. Communication occurs in different situations in health care, including when reporting between nurses. There are different reporting models to apply and one of them is bedside reporting. Bedside reporting means that the patient can be present during the reporting between the nurses in the ward. The patient has the opportunity to fill in with things that the nurses forget or go wrong. The patients' perception of bedside reporting is that it improved individual care and that they could ask and receive further information to a greater extent.Aim: The aim was to describe the nurse's experience of working with bedside reporting.Method: A literature review whose results were based on 16 scientific articles that are of a qualitative, quantitative and mixed method. The articles are published between 2011–2021.Results: According to the nurses, bedside reporting leads to patients becoming more involved in their care and patient safety increasing. Bedside reporting is a person-centered reporting model that increases the nurses' commitment to their work. However, there are obstacles to conducting bedside reporting, the nurses experience that it is difficult to maintain confidentiality towards patients when the reporting takes place inside patient rooms. This means that the nurses feel that they need more time to prepare to go into patients and the nurses said that this contributes to bedside reporting becoming a time-consuming reporting model.Conclusion: Bedside reporting has a positive effect on nurses' care relationship with patients. The nurses considered that bedside reporting entails higher patient safety and that patients' participation in reporting is positive. The nurses considered that a modified approach that consisted of both bedside reporting and traditional reporting would be the ultimate way to report.

Bedsiderapportering är ett relativt nytt rapporteringssystem i Sverige, som innebär att överrapporteringen mellan förmiddagssjuksköterskan och kvällssjuksköterskan sker inne hos patienten. Syftet med studien var att undersöka patienternas upplevelse av rapporteringsmodellen samt belysa eventuella förbättringsområden. För att besvara syftet utfördes semi-strukturerade intervjuer på åtta patienter som var inneliggande på en avdelning där bedsiderapportering tillämpades. Resultatet visade att patienterna är positivt inställda till rapporteringsmodellen, då det ökar patienternas delaktighet och ger ett ökat förtroende till personalen. Dock ansåg informanterna att sekretessen var bristande, då majoriteten låg på flerbäddssalar. Trots detta var bedsiderapportering något som samtliga intervjupersoner tyckte skulle införas på fler avdelningar på sjukhuset. Bedside report is a relatively new reporting system in Sweden, which imply that the shift report between the nurse who has been working the morning shift and the nurse who shall work the evening shift, appears in the patient's room. The aim of this study was to examine patients experience of the report model and illuminate any potential area of improvement. To answer the aim of the study we conducted semi-structured interviews on eight patients who were admitted at a division of the hospital which used bedside report. The result showed that the patients had a positive attitude to the shift report model, because it increases the participation and gives a gained trust to the nursing staff. However did the participants consider the confidentiality was inadequate, since the majority laid on multi-bed rooms. Despite this was bedside report something that all of the participants thought should be applied to other divisions at the hospital.

Vid varje skiftbyte sker överrapportering från en sjuksköterska till en annan, överrapporteringarna saknar ofta patientmedverkan. En rapporteringsmodell är bedsiderapportering vilket innebär att rapporteringen sker i patientens närvaro. Syfte : Syftet var att beskriva patientens delaktighet i vården vid bedsiderapportering. Metod : En litteraturöversikt. Resultat : Bedsiderapportering fick patienterna att känna sig delaktiga i vården, vilket delvis berodde på att de kände sig som värdefulla deltagare. Patienterna betraktades i första hand som en person. Både patienter och sjuksköterskor ansåg att rapporteringssättet var avgörande för patientens delaktighet i vården. De flesta patienter uppskattade att få vara med i rapporteringen och få viktig information som de ansåg sig ha rätt till. However, not all patients wanted to participate and the reporting method were important for the level of participation.

Överrapportering mellan vårdpersonal som arbetar skift utförs vanligtvis bakom, stängda dörrar på en expedition. Samhället idag kräver att vårdarbetet ska vara mer förebyggande och personcentrerat från att tidigare varit sjukdomscentrerad. Detta förutsätter en ökad delaktighet hos personen som vårdas där bland annat personberättelsen är en viktig del i informationsutbytet. Bedsiderapportering har införts för att öka den äldres deltagande och innebär att den dagliga rapporteringen sker tillsammans med den äldre personen vid hens sängkant. Syftet med studien var att beskriva sjuksköterskornas erfarenheter av bedsiderapport vid vård av äldre. En fokusgruppsstudie genomfördes där sjuksköterskor på en medicinavdelning i södra Sverige medverkade. Totalt genomfördes tre fokusintervjuer med totalt tio sjuksköterskor, som analyserades med kvalitativ induktiv innehållsanalys. Resultatet visar att sjuksköterskorna uppskattar grundtanken med överrapportering med bedside, men att det har förekommit problem med brist på förståelse från ledningen samt avsaknad av rutiner för vissa moment under bedsiderapporteringen. Fördelar som har setts är att patienter har varit mer delaktiga och flera sjuksköterskor har upplevt att det har varit skönt att möta personerna med dåligt allmäntillstånd redan under rapporteringen. Problem som uppkommit vid bedsiderapportering har varit när patienten har varit nedsatt kognitivt eller kommunikationsproblem uppstått. Vidare studier bör göras hur de äldre upplever bedsiderapportering. Over-reporting between healthcare personnel has previously been carried out behind closed doors at an expedition. Society today requires that health work should be more preventative and person-centred from previously disease-centered. This calls for an increased participation of the person being cared for, where they’re personal story is an vital part in the exchange of information. Bedside reporting has been introduced to increase the participation of older people and means that the daily reporting takes place together with the elderly person at his/her bedside. The aim of the study was to describe the nurse’s experiences of bedside reporting in the care of elderly. A focus group study was conducted where ten nurses on a medical department in southern Sweden participated. In total three interviews were conducted which were analyzed with qualitative inductive content analysis. The results shows that the nurses appreciate the basic idea of over-reporting of bedside, but that there have been problems during the implementation. Benefits that have been seen is that patients have been more involved and several nurses have experienced that it is good to meet the patients with poor general condition already during bedside reporting. Problems that arose while conducting bedside reporting where when the patient had impaired cognition or communications problem occurred. Further studies should be made on how older people experience bedside reporting.

Bakgrund: Överrapportering mellan sjuksköterskor vid skiftbyte är en stor och viktig del av sjuksköterskans arbete, och dess innehåll påverkar det resterade arbetsskiftets sysslor. Rapporteringen kan ske på flertalet sätt, exempelvis traditionell rapportering som sker avsides från patienten, eller så kallad bedsiderapportering som sker i patientsalen tillsammans med patienten. Syfte: Denna litteraturstudies syfte var att undersöka evidensen för vilket av de två mest frekvent använda rapporteringssätten som är att föredra: traditionell rapportering eller bedsiderapportering. Vid jämförelsen mellan de båda fanns både sjuksköterskans uppfattning samt patientens uppfattning i åtanke. Metod: En litteraturöversikt utfördes. 12 originalartiklar valdes från databaser för vetenskapliga artiklar, CINAHL och Pubmed. Dessa lästes, analyserades och ledde fram till framställandet av fem resultatkategorier. Hildegard E. Peplau och hennes relationsteori kring omvårdnad bidrog som teoretiskt ramverk. Resultat: Fem teman identifierades ur studiens artiklar: främjande av relation mellan patient och sjuksköterska, ökad vårdsäkerhet för patienterna, involvering av patienten i vården, tidsåtgång för rapportprocessen samt påverkan på patientsekretessen. Resultatet visar att bedsiderapportering föredras av både patient och sjuksköterska, dock gärna en modifierad sådan där känslig information delges utom hörhåll för att bevara sekretessen. Slutsats: Bedsiderapportering föredras av både sjuksköterskor och patienter och är ett steg i rätt riktning mot en patientcentrerad vård. Trots att studier gjorts som bevisar detta är bedsiderapportering inte standard inom vården, något som bör ändras genom fortsatta studier för en säkrare och mer delaktig vård för patienter och ett bättre och effektivare arbetssätt för sjuksköterskor. Background: Change of shift report between nurses is a big and important part of a nurse’s tasks, and its content and quality affects the coming shift’s tasks. The change of shift report can be carried out in multiple ways, for example traditional report which is conducted away from the patient, or bedside report which is located in the patient’s room with the patient. Purpose: The purpose of this literature review was to investigate the evidence for which of the two most commonly used methods of report is preferable: traditional report or bedside report. The comparison between the two methods of reporting addressed both the nurse’s opinion as well as the patient’s. Methods: The method used was literature review. 12 original articles were selected from databases for scientific articles, CINAHL and Pubmed. They were read and analyzed and resulted in five themes of results. Hildegard E. Peplau and her theory about relations in caring contributed as a theoretical framework. Results: Five themes were identified in the articles of the review: promoting of relationship between patient and nurse, safer care for patients, involving patient in care, time used for report and impact on patient’s confidentiality. The results showed that bedside report is preferred by both the patient and the nurse, preferably a modified version where sensitive information is handled away from the patient to keep the patient’s confidentiality. Conclusion: Bedside report is preferred by both nurses and patients and is a step towards a more patient centered care. Despite previous studies having shown this same result, bedside report is not yet implemented as standard report within the health care system. The conclusion of this review is that this should change and continuous studies should be conducted with an aim of safer care for patients with more patient participation, as well as better and more efficient way of work for nurses.

Bakgrund: Dagens omvårdnadsforskning lyfter fram patienten som den viktigaste men samtidigt den mest outnyttjade resursen i hälso- och sjukvården. Patienterna måste göras mer delaktiga i sin egen vård och behandling. Historiskt har överrapportering mellan sjuksköterskor skett på sjuksköterskeexpeditionen. I bedsiderapportering sker kommunikationen mellan vårdpersonal och patient, vilket gör informationsöverföringen effektiv och säker. Syfte: Att beskriva patientens upplevelse av delaktighet vid bedsiderapport. Metod: Studien är en litteraturöversikt baserad på kvalitativa och kvantitativa vetenskapliga artiklar. Resultat: Resultatet redovisas utifrån fyra teman: att bli bemött som en person, att förstå och bli förstådd, att bidra till sin egen vård och att känna sig trygg. Det är viktigt att patienten erkänns som en person för att känna sig delaktig och därmed kunna bidra till sin egen vård. När patienten förstår och gör sig förstådd upplevs delaktighet. En bra bedsiderapport skapar trygghet. Slutsats: När sjuksköterskan blir medveten om fördelarna, tar tillvara på möjligheterna med patientens deltagande och samtidigt tar hänsyn till sekretess, språkbruk och patientens personella förutsättningar, blir bedsiderapportering betydelsefull för ökad delaktighet och inflytande för patienterna. Background : Today nursing science brings forward the patient as the most important, but also the most neglected, resource in healthcare. The patients must be invited to participate in their own care and treatment. Historically, nursing handover has been carried out at the nurses’ expedition. In bedside reporting, care personnel communicate directly with the patient, which makes information exchange more efficient and safer. Purpose: To describe the patient’s experiences of participation in bedside report. Method: The study is a literature review based on quantitative and qualitative articles. Results: The results are presented based on four themes: to be met as a person, to understand and to make oneself understood, to contribute to one’s own care and to feel secure. It’s important that patients are recognized as persons to make them feel that they participate, thereby being able to contribute to their own care. When patients understand and make themselves understood, they feel participation. A good bedsiderapport creates security. Conclusion: Successful bedside report requires that the nurse becomes aware of the benefits and takes advantage of the opportunities that patient participation signifies. Bedside report is important to increase patients’ participation in care.

Med dagens krav i lagar, författningar, föreskrifter och med patienters samt anhörigas tydliga förväntningar på sjukvården kan informationen och delaktigheten gällande vård, behandling klart förbättras inklusive säkerställas med nya rapporteringssystem. Syftet med denna studie var att undersöka föräldrar och barns uppfattning angående information och delaktighet gällande barnets omvårdnad och behandling under vårdtiden på barnonkologen på ett universitetssjukhus i centrala Sverige. För att ta reda på detta utfördes en för– och eftermätning med frågeformulär till barn och föräldrar i samband med att ett nytt skiftrapporteringssystem- Bedside Handover implementerades. Inne på patientsalen tillsammans med barn och föräldrar rapporterade avgående sjuksköterska till nästkommande skifts personal enligt en rapporteringsmall. Detta innebar att alla parter fick vetskap om planerad vård och behandling samt att det förbättrade informationsflödet inklusive delaktigheten för föräldrar och barn gällande barnets omvårdnad och behandling. Samtliga frågeställningar förbättrades efter implementeringen hos både föräldrar och barn. Störst var förbättringen hos barnen. Detta rapporteringssystem sätter patienten i fokus och förstålig kommunikation används. Missförstånd, frustration och irritation kan undvikas och delaktighet uppmuntras. Nyckelord: bedside rapport, föräldrar, barn, information, delaktighet, pilotstudie With today’s requirements of laws, decrees, regulations and with patients as well as relatives expectations regarding, both information about and participation in treatment and care decisions, force the health care system to improve and secure this with new reporting systems. The purpose of this study was to investigate the parents and children’s view on the information and participation concerning the child’s care and treatment at the paediatric oncology ward in Sweden. The results were assessed by a pre- and post questionnaire answered by children and parents in connection to implementation of a new reporting system- Bedside Handover was implemented. In the patientroom with child and parents present, the outgoing nurse reported to the incoming ward staff through a checklist. This meant that all parties had knowledge of planned care, treatment, which improved the flow of information, including the participation of parents and child concerning the child’s care and treatment. All issues were improved for both parents and children after the implementation. The greatest improvement was shown in the children. This reporting system puts the patient in focus, and understandable communication is used. Misunderstandings, frustration and irritation can be avoided and encouragement in participation is lifted.
