2. Experimental Substantiation of Autoplasma Application as a Haemostatic Agent in Endoscopic Operations in the Digestive Tract.

In endosurgery of the digestive tract, 'cellular technologies' are gaining popularity, including the use of blood components or blood cells with a haemostatic purpose. In this regard, the objective of the study is evaluation of the effectiveness of bleeding control and safety of resection of the hollow organs of the digestive tract in the experimental trauma models of the abdominal organs in laboratory animals. The study was conducted in 20 mature male Chinchilla rabbits with the mean body mass of 2450 ± 210 g. All animals were divided into four experimental groups: the control group (without bleeding control); the group that received the infiltration of the wall of a hollow organ with saline; the group in which bleeding was controlled by argon plasma coagulation (APC), and the group wherein animals underwent haemostasis with autoplasma. In the control group and the physiological saline group, no statistical difference was observed in the time of bleeding control. Autoplasma, due to preventive local administration, is more effective than APC and has a high haemostatic potential. Autoplasma has been found to be highly effective and safe for bleeding control in the gastrointestinal tract in the experimental model, which has paved the way for new possibilities for operations of various scales, including endoscopic operations. Modern and glance at the methods of diagnosis and treatment of patients with odontogenic inflammatory diseases of the maxillofacial area and neck was and remains one of the pressing problems that are frequent complications that lead to disorders in the tooth-jaw system, and can be a direct life-threatening ill. Purposeofthestudy. Toincreasetheeffectiveness of treatment of phlegmon of maxillofacial area and neck due to the active evaluation of the dynamics of the inflammatory process comparing a specific patient with standard indicatorschanges in the limits of edema of infiltration. Materials and methods. We carried out diagnostics and complex treatment of 120 patients with odontogenic phlegmons, who were in hospital treatment. The patients were divided into 2 groups: group I – 60 patients, treatment of which was carried out by the traditional method. Group II – 60 patients, modern treatment methods regional administration of attibiotics, vacuum drainage of purulent wound and plasmolifting at the stage of reparation, were added to their standard therapy. Conclusions: The proposed method is simple, reliable and does not require expensive equipment and makes it possible to predict the development of the disease only by clinical measurements of the volume of inflammation.

4. The platelet-rich plasma influence on the inflammatory process in proliferation stage

One of the known methods of treatment is biomesotherapy, in which the patient’s own biomaterials are used for injection. It ensures complete biocompatibility and practically eliminates the possibility of beginning the allergic reactions. One of the options for mesotherapy is the using of Plasmolifting TM (plasmolifting) – commercially patented title of PRP-therapy (Platelet Rich Plasma), which involves the using of an injection form of platelet-rich plasma. Purpose of the study. Increase the efficiency treatment phlegmon of the maxillofacial area and neck with the help of impact of autoplasma rich in platelets on the proliferative phase. Materials and methods. We carried out the diagnosis and complex treatment of 80 patients with odontogenic phlegmons who were hospitalized in the Department ofMaxillofacial Department. surgical and therapeutic dentistry State Institution «Zaporizhia Medical Academy of post-graduate education Ministry of Health of Ukraine». The patients were divided into 2 groups: 1st group – 25 patients, their treatment was carried out by the traditional method and 2nd group – 55 patients among whom platelet-rich plasma (PRP) was added to the treatment at the repair stage. Conclusions. Treatment in this way accelerated the course of the healing process due to the additional reparative and proliferative effects of such therapy.

5. CLINICAL AND MORPHOLOGICAL ASPECTS OF THE APPLICATION OF PLATELET GROWTH FACTORS IN PATIENTS WITH PILONIDAL CYST WITH ABSCESS

OBJECTIVE. The objective of this study is to investigate the surgical management of pilonidal cyst with abscess and to improve short-term and long-term outcomes.MATERIAL AND METHODS. The initial sample consisted of 353 patients operated for pilonidal cyst with abscess. The first group included 105 patients, receiving treatment with the excision of pilonidal cyst and the suturing in staggered rows. The second group consisted of 136 patients with the excision of pilonidal cyst using a unique patented technique. The third group of 112 patients underwent an identical operation to that of the second group, but with additional platelet growth factors (PGF) to improve the wound healing.RESULTS. The current study found that in comparison to the traditional technique, the optimal surgical treatment of pilonidal cyst with abscess was the original one-staged surgical technique, based on an economical excision, which led to an improvement in sickness leave of up to 27 days and in long-term outcomes (2 % of long-term complications). Another important finding was that the application of platelet growth factors (PGF) in the complex treatment of patients with pilonidal cyst with abscesses reduces the recovery time by up to 21 days and resulted in 30 % lower postoperative pain scores (Visual Analog Scale for Pain).CONCLUSION. In general, this finding shows that the original one-stage excision of pilonidal cyst with abscess with the application of PGF and with no suturing reduces disability time without increasing the number of early or late postoperative complications.

Improvement of methods of complex treatment of periodontium diseases continues to be an urgent problem of dentistry and requires an interdisciplinary approach. The severity of thecourse of periodontitis and the severity of destructive phenomena dictates the need to use osteoplastic agents to restore damaged periodontium tissues. It has been proven that the efficiency of membrane technology is significantly increased when membranes and substitute materials are used together. The positive effect of the combined use of Platelet Rich Plasma (PRP — autogenous growth factors), osteoplastic materials, and the method of Guided Tissue Regeneration (GTR) has been confirmed. To increase the efficiency of reconstructive treatment of patients with generalized periodontitis through the combined use of alloplastic, Platelet Rich Plasma, and the method of Guided Tissue Regeneration. Method of testing: clinical, X-ray. For the period 2018–2019, 34patients with chronic generalized periodontal atrophy of moderate severity were under observation. All patients underwent professional oral hygiene, laser therapy, and antibacterial therapy with rovamycin. 6patients in complex therapy underwent surgical interventions: osteogingivoplasty with biocomposite materials using GTR auto-allogenic membranes. Bone defects were filled with biocomposite material: hydroxylappatite granules (Polistom, RF) in combination with PRP, on top of an alto plasma membrane isolated from PRP and collagen membrane (Polistom, RF). The immediate and long-term periods of treatment were quite favorable. Control X-ray examination after 12 months showed restoration of a bone defect with an organotypic structure.

Improvement of methods of treatment of odontogenic cysts of the jaws continues to be an urgent problem in surgical dentistry and maxillofacial surgery. Currently, the most promising is the use of multicomponent osteoplastic materials in order to optimize reparative osteogenesis after cystectomy for extensive jaw cysts. The aim of the study is to improve the method of cystectomy in the surgical treatment of large cysts of the upper jaw with osteoplasty of the bone defect with biocomposite materials in combination with the method of directed tissue regeneration. Clinical, radiological, histological. During the period 2015-2019, 54 patients with odontogenic cysts of the jaws were under observation, of which 12 were diagnosed as extensive. After cystectomy, the bone cavity was filled with biocomposite material: hydroxylappatite granules (Polistom, RF) in combination with PRP, on top of an autoplasma membrane isolated from PRP and collagen membrane. The immediate and long-term periods of treatment were quite favorable. Control X-ray examination after 12 months showed complete recovery of the bone defect with an organotypic structure in all patients.

В статье говорится о сравнении эффективности применения «гиалуронатов» и аутоплазмы при остеоартрите коленных суставов у пациентов молодого возраста. В исследовании принимали участие пациенты возраста 16-28 лет с остеоартритом коленных суставов различных стадий, которым внутрисуставно вводились гиалуронаты или\и аутоплазма, обогащенная тромбоцитами. Далее проводилась клинико-инструментальная оценка (по шкале боли ВАШ и ультразвуковая диагностика). В ходе исследования нами не было выявлено значимого различия эффективности курсового лечения между «гиалуронатами» и аутоплазмой, обогащенной тромбоцитами. Однако, в случае раннего остеоартрита у молодых пациентов, обезболивающее действие наблюдалось в первые сутки после инъекции аутоплазмы. Также применение аутоплазмы оказалось экономически более выгодным для пациентов, The article deals with the comparison of the effectiveness of the use of "hyaluronic acid" and autoplasma in osteoarthritis of the knee joints in young patients.The study involved patients aged 16-28 years with osteoarthritis of the knee joints of various stages, who were intra-articularly injected with hyaluronates or\and autoplasm enriched with platelets. Next, a clinical and instrumental assessment was performed (according to the VAS pain scale and ultrasound diagnostics). During the study, we did not find a significant difference in the effectiveness of the course of treatment between "hyaluronates" and platelet-rich autolasma. However, in the case of early osteoarthritis in young patients, the analgesic effect was observed in the first day after the injection of autoplasma. The use of autoplasma also proved to be more cost-effective for patients.

Внутриматочные синехии (ВМС) по-прежнему представляют собой большую медико-социальную проблему. Прогноз в отношении репродуктивной функции и качества дальнейшей жизни при гинекологических операциях во многом определяется возможным спаечным процессом. Пусковым механизмом формирования ВМС является травма базального слоя эндометрия, которая может быть вызвана различными факторами, включая внутриматочные вмешательства. В настоящее время «золотым стандартом» диагностики и лечения ВМС является гистероскопия (ГС) и их разделение. Для профилактики образования спаек традиционно используются заместительная гормонотерапия с целью ускоренной пролиферации нормального эндометрия, специальные внутриматочные устройства или введение баллонных катетеров, противоспаечные барьеры. Однако остаётся высокий риск рецидива, а при наступении беременности развитие её осложнений. Применение аутоплазмы, содержащей тромбоциты, в лечении синехий полости матки способствует снижению риска рецидива ВМС после их разделения, восстановлению менструальной и репродуктивной функций.The problem of intrauterine synechia (IUS) is still a major medical and social problem. However, there remains a high risk of relapse, and when pregnancy occurs, the development of its complications. 12. Эффективность лечения краевой рецессии десны с применением инъекций обогащенной тромбоцитами плазмы

Заболевания пародонта являются распространенной проблемой современной стоматологии. Очень часто данная патология сопровождается убылью мягких тканей пародонта. Прогноз течения рецессии не всегда является благоприятным, так как посредством имеющихся методов не удается достичь полной регенерации тканей. Поэтому возникает интерес к использованию аутоплазмы, для коррекции утраченных тканей. Действуя на все звенья регенерации тканей, она обеспечивает усиление трофики и тканевого анаболизма. Цель исследования: изучить эффективность лечения посредством аутотрансплантации плазмы, богатой тромбоцитами. Для исследования было отобрано 47 пациентов без соматической патологии, которые тщательно обследовались. Исследуемые получали по 3 инъекции плазмы богатой тромбоцитами с промежутками 7-10 дней, обследование пациентов проводили через 1, 3, 6 и 9 месяцев. Оценивали ширину прикрепления, биотип и состояние микрогемоциркуляции. Посредством инъекций удалось получить значительный прирост тканей, существенно улучшились показатели сосудистого русла тканей пародонта, достигнуты изменения биотипа десны. Полученные результаты клинических и функциональных исследований позволяют утверждать об эффективности использования инъекций аутоплазмы с целью лечения рецессии десны различной этиологии. Однако стоит отметить, что эффект является непостоянным и возникает необходимость проведения повторных курсов., Periodontal diseases are wide spread problem in modern stomatology. Very often this pathology is following by waste of soft periodontal tissues. The forecast of the recession is not always favorable, since the available methods do not allow the complete regeneration of tissues. Therefore, there is interest in the use of autoplasma for the correction of lost tissues. Influencing on all elements of regeneration the use of autoplasma provides improving of trophic and tissue anabolism. The research purpose is to study the effectiveness of treatment by autotransplantation of plasma rich in platelets. For this research 47 patients without somatic pathology were selected and scrupulously examined. The patients got by 3 injections of platelet-rich plasma with the interval 7-10 days. The patients were examined after 1, 3, 6 and 9 months. An attachment width, a biotype and a microhemocirculation status were evaluated. It was possible to obtain a significant increase in tissue by injection. The parameters of the vascular bed of the periodontal tissues have significantly improved, the changes in the gingival biotype of the gum have been achieved. The obtained results of clinical and functional studies allow to confirming about using autoplasma injections for the treatment gingival recession different etiology. However, it should be noted that the effect is not permanent and there is a need for repeated courses.
