This study evaluated the prevalence and characteristics of advanced practice providers, including nurse practitioners and physician assistants, who provide care related to burns in the United States from 2013 to 2022 using national claims data. Our analysis was a retrospective, large cohort study evaluating debridement, wound care, and office-based services provided by advanced practice providers from 2013 to 2022 using the Medicare Provider Utilization and Payment Data Public Use Files from the CMS. The reported provider type and billing codes were used to identify healthcare professionals providing burn care. Trends over the study period and available data about care provided were analyzed. From 2013 to 2022, burn care providers included 6918 (43.3%) surgeons, 4264 (26.7%) other physicians, and 4783 (30.0%) advanced practice providers. Overall, the number of advanced practice providers who billed for burn care increased by 250.4% from 714 in 2013 to 2502 in 2022. Compared with physicians, advanced practice providers were more likely to be female and provide care in micropolitan areas with less than 50,000 people. Based on these trends, we project that the number of advanced practice providers providing services related to burns will increase by 56.7% from 2502 in 2022 to 3921 (95% CI, 3541–4303; R 2 = 0.97) in 2030. With a growing need for burn care and predicted shortages in the surgeon workforce, the scope of practice and integration of advanced practice providers merits further discussion and evaluation. Abstract Background Allied health assistants (AHAs) support allied health professionals (AHPs) to meet workforce demands in modern healthcare systems. In an Australian context historically, AHAs have been sub-optimally utilised. Prior research has identified that AHAs and AHPs working in health, disability and aged care sectors, and the Vocational Education and Training (VET) industry, may benefit from access to resources to support the optimisation of the AHA workforce. As a part of a Victorian department of health funded project, several resources were developed in line with workforce recommendations for use in each of the above sectors. Recommendations and resources covered the broad areas of pre-employment training, workforce planning and governance, consumer-centred therapy and supports, recruitment and induction and workplace training and development. This study aimed to evaluate the engagement with these newly designed resources to support optimisation of AHAs in the Victorian context. Methods Semi-structured interviews were conducted to evaluate engagement with resources, from the perspective of AHAs, AHPs and allied health leaders (AHLs) in the health, aged care or disability sectors, and educators and managers of allied health assistance training. Thematic analysis was conducted using team-based framework analysis. Results Thematic framework analysis of the interview data identified four themes; Why participants accessed the resources; How participants engaged with the resources; What (if any) changes in practice occurred as a result of engaging with the resources in a participant’s local context, How did participants envision the resources being utilised for AHA workforce optimisation in the future. Responses were mapped to the AHA workforce career pathway at the career preparation, career development and career trajectory tiers. Conclusions Appetite for AHA workforce development and optimal utilisation is evident across Victoria, Australia. Readily accessible resources that inform AHA role and scope of practice, delegation practice, or improve the ability for an AHA to state their own development needs, were identified as useful by participants. The potential for these resources to assist in the optimal utilisation and development of AHA workforces across the career continuum differs according to the role, sector and geographical location of the resource user. Further study is needed to investigate the transferability of these resources to national and global contexts.

A service evaluation was conducted to explore staff perception of the role of assistant psychologists (APs) working in a Crisis Resolution and Home Treatment Team (CRHTT). Four APs were employed on a pilot scheme and embedded in four CRHTTs across Hywel Dda University Health Board (HDUHB). Using a mixed-methods survey, data were collected from staff working with these CRHTTs (N = 26). Quantitative results reported that the participants perceived the contribution of the AP as 'very valuable' to both the team and its patients. Qualitative results identified two overarching themes (Additional Value and Additional Expectations) and five themes (with subthemes): (1) A Stepping Stone; (2) A Different Perspective; (3) More is Needed; (4) Clarifying the Remit; and (5) The Impact on Discharge. Integrative analysis indicated that there is a need to improve staff understanding of the role and its remit. Different sins from the view point of the amount of villainy are not equal and have the degrees of severity and weakness. It is possible that other people including natural or legal ones facilitate the preliminaries of committing an unlawful action to perform a crime and provide the base of committing an illegal act that is called “crime assistant” and the committed act is referred to as “crime assistance”. Some jurists consider that the conclusion of the occurrence by mubāshir (perpetrator) (the occurrence of assisted crime) is a condition of title verity of assistance of crime, and from the view point of other jurists, this condition is not considered to be valid. The present research has been performed through descriptive-analytical method and by referring to jurisprudence books and jurists’ opinions along with justification, explanation, reasoning and inference. The finding of the present research includes total opinions of jurists and existing settlement of disputes in “conditionality crime assistance occurrence”. Therefore, considering the severity and weakness of the amount of sin indecency and it’s negative effect on psychological security of society, particularly in the cyberspace, the judicial courts are allowed and authorized, considering two criteria of intention of people and their knowledge and attention on the possibility of the occurrence of crime in the real world and or (internal or external) preventive factors of crime occurrence in the real world, in some cases, to consider the assistance of crime as the subject to crime assistance occurrence and in relation to some other crimes and legal and lawful sins do not consider the occurrence of assisted crime as criterion. This paper examines the potential integration of Large Language Models (LLMs) with the Systems Modeling Language version 2 (SysML v2), proposing a novel methodology for systems engineering by capitalizing on the enhanced readability and human‐friendly syntax of SysML v2. Given the emergent sophistication of LLMs and the coincidental development of SysML v2—‐an endeavor that presents a pivot toward naturally articulated model interaction—‐we explore the possibilities and implications of such an intersection. Our investigation posits that LLMs can serve not only as an interpretive layer, allowing for the syntactically simplified manipulation of system models, but also as a catalyst for a knowledge‐driven design approach. We highlight the efficiencies gained by deploying LLMs for SysML v2 interactions, which reduce the dependency on technical expertise traditionally needed for API navigation and model management. Through case studies and analysis, we demonstrate that the conversational engagement with system models facilitated by LLMs can lead to a democratized and accelerated design process. However, this advent is tempered by a critical awareness of potential pitfalls, such as automation bias and overreliance on automated systems—‐underscoring the need for continued human oversight and the examination of ethical considerations. Emphasizing the chance of SysML v2 being inherently English‐based and the parallel maturation of LLMs, this paper suggests that the collaborative utilization of these concurrent advancements may offer an opportune fusion, potentially revolutionizing the way systems are modeled and managed. Future work involves the empirical validation of these approaches and a deeper investigation into interoperability with existing and future systems engineering ecosystems. These difficulties have led to increased interest in workforce redesign, and the upskilling of existing staff in all areas of healthcare, including within the Allied Health Professions (AHP). Clinical support staff are a key component of workforce redesign, yet little has been documented on the utilization of this workforce across the wide range of professions that collectively form the AHP workforce. Existing research is also unclear due to the variety of titles used to describe them (eg, allied health assistants, therapy assistants, etc). This study aimed to review how Support Workers and Assistant Practitioners (SWAPs) are utilized within the AHP professions. Electronic databases (MEDLINE, CINAHL complete, Scopus, and Google Scholar) were searched to find English Language primary research articles that explored the deployment of clinical support staff within Allied Health. Following the scoping review methodology, data from each study were analyzed in terms of design, key findings, and implications. A quality assessment was also completed. Thirty-nine articles met the eligibility criteria. Studies were undertaken in Australia, UK, and USA, and covered a range of AHPs and methodological approaches. Most articles employed qualitative methods, with highly variable research quality identified. Key findings were that cost-effectiveness of this workforce has not been formally evaluated in any setting or AHP discipline, and that support workers are a largely underutilized staff group potentially due to inconsistencies in their deployment and scope of practice, and the lack of a clear career pathway. Rigorous, quantitative, and mixed methods research into the deployment and impact of this staff group is needed in order to gain a clearer understanding of how they are optimally utilized across the different AHP disciplines.Keywords: allied health personnel, workforce, scope of practice, assistant, support worker

This restrospective cohort study of Medicare payment data found increases in the number of advanced practice providers who bill for plastic and reconstructive surgery care. The growth in advanced practice providers is expected to continue in plastic surgery, and further investigation is needed to effectively integrate these providers into academic centers. This document aspires to pioneer an innovative system designed to support individuals facing blindness or visual impairment by integrating Radio Frequency, Identification (RFID) technology with objects and face recognition via image processing. The key objective of the proposed system is enhancing independence and maintaining security of blind person as they navigate their environment. Household utensils and various items within a residence or building are equipped with passive RFID tags. Each of these RFID tags is associated with a different audio file, individually recorded for every object. These audio files are stored on a centralized server. Furthermore, the system maintains a dataset encompassing unidentified faces, assist blind individuals to recognize unknown person through audio feedback. The data is sent to the server by the RFID reader, which reads the tags. This server conducts the database search for the particular ID and plays the audio file. Additionally, the system captures and stores the facial data of unknown persons in the database. When the person reappears in front of blind person the system employs the facial recognition by crossreferencing the information within the database to facilitate face identification. Data generation in clinical settings is ongoing and perpetually increasing. Artificial intelligence (AI) software may help detect data-related errors or facilitate process management. The aim of the present study was to test the extent to which the frequently encountered pre-analytical, analytical, and postanalytical errors in clinical laboratories, and likely clinical diagnoses can be detected through the use of a chatbot. A total of 20 case scenarios, 20 multiple-choice, and 20 direct questions related to errors observed in pre-analytical, analytical, and postanalytical processes were developed in English. Difficulty assessment was performed for the 60 questions. Responses by 4 chatbots to the questions were scored in a blinded manner by 3 independent laboratory experts for accuracy, usefulness, and completeness. According to Chi-squared test, accuracy score of ChatGPT-3.5 (54.4 %) was significantly lower than CopyAI (86.7 %) (p=0.0269) and ChatGPT v4.0. (88.9 %) (p=0.0168), respectively in cases. In direct questions, there was no significant difference between ChatGPT-3.5 (67.8 %) and WriteSonic (69.4 %), ChatGPT v4.0. (78.9 %) and CopyAI (73.9 %) (p=0.914, p=0.433 and p=0.675, respectively) accuracy scores. CopyAI (90.6 %) presented significantly better performance compared to ChatGPT-3.5 (62.2 %) (p=0.036) in multiple choice questions. These applications presented considerable performance to find out the cases and reply to questions. In the future, the use of AI applications is likely to increase in clinical settings if trained and validated by technical and medical experts within a structural framework. [ABSTRACT FROM AUTHOR]

In 2021, 44 states and the District of Columbia operated 63 school/center-based preschool programs serving over 1.3 million children. The vast majority of state-funded preschool programs require two adults in each classroom: typically, a lead teacher and an assistant teacher, with an expectation for team teaching. This paper examines the trends and variations in state policies that address the qualifications requirements, compensation, and professional development supports for preschool lead and assistant teachers in state-funded preschool programs that provide a foundation for supporting a culture of collective success and potential quality of the classroom.

Subject of the Research: The creation of an intelligent assistance system for people with visual impairments. Nowadays, the task of developing effective intelligent assistance systems that allow people with vision problems to achieve maximum independence is important and relevant, as existing systems have a number of drawbacks, such as limited autonomy, limited integration with other devices and systems, limited analysis of dynamic obstacles, and limited user feedback capabilities, which in most cases are restricted to voice guidance. Objective: The objective of this work is to create a generalized functional model of an intelligent assistance system for people with visual impairments, which has enhanced autonomy, integration with other devices and systems, the ability to analyze dynamic objects and predict their movement trajectory, and provide diverse feedback to the user. Tasks: To achieve the set objective, the following tasks were accomplished: a generalized functional model of the proposed intelligent assistance system for people with visual impairments was created; the functional dependencies of the components of the developed model were substantiated; a review of the basic modules of the proposed system model was conducted. Methods: The methods used include functional modeling methods and system analysis methods. Results: The following results were obtained: a functional model of an intelligent assistance system for people with visual impairments was proposed. This system surpasses existing analogs in a number of functional capabilities: detection of static and dynamic obstacles with prediction of dynamic obstacles’ movement trajectory, the ability to operate in various conditions (indoors, outdoors, in light or dark, in different weather conditions), support for integration with other systems and devices, and a high level of autonomy. Conclusions: The developed system model has enhanced autonomy, integration with other devices and systems, the ability to analyze dynamic objects and predict their movement trajectory, and provides diverse feedback to the user.

ChatGPT is an artificial intelligence model that has the potential to revolutionize the field of endoscopy. It can rapidly summarize medical records, assist with diagnosis, provide patient communication support, and even understand endoscopic images. However, there are limitations, including the risk of inaccurate or inappropriate responses, privacy and security issues, and the potential to limit doctors' thinking. Further research and improvements are needed to ensure ChatGPT's safe use in the medical field. Overall, the potential benefits of ChatGPT in endoscopy are vast, and it has the ability to greatly improve the efficiency of diagnosis and treatment.

Skin diseases remains a significant medical problem. It is associated with increasing prevalence, morbidity, and relapses, as well as development of complications, which has a negative impact on the patient’s quality of life. The approach to the diagnosis and treatment of dermatological diseases has changed significantly with the development of information technology. Algorithms of artificial intelligence are developed to assist dermatologists with the diagnosis of diseases. They may facilitate early diagnosis and reduce the number of appointments, reduce the waiting time for specialist consultations and the correct distribution of patient referrals to doctors of other specialties. Sports medicine, an essential branch of orthopedics, focuses on preserving, restoring, improving, and rebuilding the function of the human motor system. As a thriving interdisciplinary field, sports medicine attracts not only the interest of the orthopedic community, but also artificial intelligence (AI). In this study, our team summarized the potential applications of GPT-4 in sports medicine including diagnostic imaging, exercise prescription, medical supervision, surgery treatment, sports nutrition, and science research. Objectives: This study aimed to reveal the communication skills of family medicine residents who are new to the profession and to provide formative feedback to residents in the early stages of their education. Materials and Methods: 222 family medicine assistants studying in hospitals affiliated with the University of Health Sciences within the borders of Ankara province participated in the research online. The sociodemographic characteristics of the residents participating in the study were accepted as independent variables, and the Turkish version of the Health Professionals Communication Skills Scale (HP-CSS-TR) was considered as the dependent variable. Results: The mean age of the participants was 22.13±2.25 years. The assistant seniority level of the participants was 1.77±1.39 years. 80.63% of the participants reported having problems with the patients and their relatives. 99.55% stated that they encountered difficult patients or their relatives. In our study, family medicine clinical assistants' communication, empathy, respect and social skills were high. Senior participants had better communication levels (p=0.011). Conclusion: In our study, we found that family medicine residents have high communication skills, but they expressed that they have difficulties communicating with patients and wanted to receive communication training to improve their communication skills and cope with difficult patients. Giving feedback by measuring the communication skills of young assistants who are new to the profession should be included in our assistant training practice as a standard first-step assessment of the profession. The emergence of artificial intelligence (AI) offers unprecedented opportunities for joint arthroplasty surgery. Notably, on 14th March, 2023, the OpenAI company officially launched its latest version GPT-4, which once again become the focus of discussion on social media. Although more than 200 articles have reported the potential role of ChatGPT/GPT-4 in various areas, there are no studies that discussed the potential of GPT-4 as an AI-powered virtual assistant for surgeons specialized in joint arthroplasty. In this study, we summarized the five major roles of GPT-4 including scientific research, disease diagnosis, treatment options, preoperative planning, intraoperative support, and postoperative rehabilitation for arthroplasty doctors. The advent of artificial intelligence (AI), particularly ChatGPT/GPT-4, has led to advancements in various fields, including healthcare. This study explores the prospective role of ChatGPT/GPT-4 in various facets of spinal surgical practice, especially in supporting spinal surgeons during the perioperative management of endoscopic spinal surgery for patients with lumbar disc herniation. The AI-driven chatbot can facilitate communication between spinal surgeons, patients, and their relatives, streamline the collection and analysis of patient data, and contribute to the surgical planning process. Furthermore, ChatGPT/GPT-4 may enhance intraoperative support by providing real-time surgical navigation information and physiological parameter monitoring, as well as aiding in postoperative rehabilitation guidance. However, the appropriate and supervised use of ChatGPT/GPT-4 is essential, considering the potential risks associated with data security and privacy. Currently, most of the powerlifting competitions are conducted without any digital systems, which will be responsible for the fairness of evaluation of participants. Since the evaluation of participants is carried out by judges, the following problems follow: Errors when checking the correctness of the exercise A large number of judges are needed to conduct competitions Occurrence of errors when logging competitions (entering all results manually) To solve this problem, we have decided to create the prototype of a digital referee system designed to analyze the correctness of the athlete's performance of the Chest Press exercise on the MB Barbell Chest Press / Bench Press simulator, based on the readings of a three-axis accelerometer provided by MB Barbell. The developed prototype allows to fix the initial and final positions of the athlete's hands, with the calculation of the movement of the hands performing the exercises. Before performing the working approach, calibration is performed, consisting of an arbitrary number of repetitions on a simulator with an empty weight. Calibration determines the extreme positions of the hands when performing an exercise on this simulator for this athlete. All calibration measurement results are entered into a special calibration table, according to which, in the future, the average calibration value of the amplitude of the athlete's arm movement is calculated. Robot-assisted surgery (RAS) has been widely accepted in the clinical routine around the world with a focus on urology, gynecology and general surgery. Even transoral robotic surgery (TORS) is applied on a regular basis in some specialized medical centers. However, the anatomical constraints and the delicate procedures at the anterior skull base prevent the introduction of RAS until today. We have performed an extended literature research on robot-assisted skull base surgery and added our own pre-clinical experiences to the list of publications. The overview includes available systems, pre-clinical and clinical applications, specific limitations, remaining challenges and an outlook into future developments.

Purpose: This study sought to explore whether the experience level of the first assistant surgeon influences perioperative organ injuries (ureteral, bladder, and intestinal injuries) in patients undergoing total laparoscopic hysterectomy (TLH) for benign diseases. We defined an experienced surgeon as a surgeon certified by the Skill Qualification Committee of the Japan Society of Gynecologic and Obstetric Endoscopy and Minimally Invasive Therapy or a surgeon with equivalent surgical skills. Methods: We reviewed our surgical registry database of TLH for benign indications between 2014 and 2020 and only selected cases performed by an experienced primary surgeon. Patients were divided into two groups based on the experience level of the first assistant. Inverse probability of treatment weighting by propensity score, which was adjusted for patient and procedure characteristics, was used to examine differences in perioperative organ injuries according to the experience level of the first assistant. Results: Among 1682 patients who underwent TLH, 18 organ injuries were found (0.83%). In the propensity score inverse probability of treatment weighting models, less experience of the first assistant had no significant impact on the occurrence of perioperative organ injuries (p = 0.348). Conclusion: In TLH for benign indications at our hospital, given an experienced primary surgeon, the inclusion of a less experienced first assistant does not negatively affect the occurrence of perioperative organ injuries. Background: Medical assistants in particular are at a high risk of acquiring mental illnesses as postgraduate students. The purpose of this research was to characterize postgraduate students' quality of life and the prevalence of psychiatric and personality disorders. Methods: This cross-sectional study was performed on 499 postgraduate students in 2019. Participants completed four questionnaires including demographic information questionnaire, the 36-item short-form 36-t-quality questionnaire (SF-36), the Millon 3 multi-axis clinical test (MCMI-III), the 53-item short-form short-term psychological symptoms questionnaire (BSI-53). One-way analysis of variance and statistical analysis was performed in SPSS software and causal structural model was performed in PLS software. Results: The quality of life of students was moderate. In physical subcomponents, women's quality of life scores was higher than men, and conversely, in terms of psychological subcomponents, men's quality of life scores was higher than women. The causal structural model showed that income was a positive predictor of quality of life (B=0313, p<0.001), but female gender (B=-0.503, p<0.001) and age (B=-0.101, p<0.001) were negative predictors. About 30% of postgraduate students/residents suffer from psychological symptoms, and about 14% have overt mental disorders or personality disorders. Conclusion: Gender, age, residency year, area of study, and income are individual traits of postgraduate students and residents that have a significant impact on quality of life, psychological symptoms, mental disorders, and personality disorders. Introduction: The three-arm approach is mainly selected, despite the multiple robotic arms in da Vinci Xi. This type of surgical setup may provide less autonomy to the console surgeon and result in greater dependence on the bedside surgical assistant. Therefore, the 4th arm is used instead of the assist port, which is why we developed "pure" robot simple hysterectomy (PRSH) as a novel surgical technique, in which all ports are operated by robotic arms., Materials and Surgical Technique: After pneumoperitoneum was established, trocars were inserted under visual control: three 8 mm robotic ports on the same horizontal line spaced 8 cm apart at the level of the endoscope port. The 2nd arm was used to insert the endoscope, and the fenestrated bipolar forceps in the 1st arm and Maryland bipolar forceps in the 3rd arm were operated using the double bipolar method. In this technique, the uterine manipulator is not used because the Cadiere forceps in the 4th arm manipulate the uterus. For suturing, the 3rd arm was equipped with a SutureCut needle driver from Maryland bipolar forceps, which enabled suturing and thread cutting. Suction and intra-abdominal transport of the needle was introduced into the abdominal cavity by pulling out the instrument in the 3rd arm. As by Article 24 of the Law No 140/2022 the legislator imposed on the National Authority for the Protection of the Rights of Persons with Disabilities and the Superior Council of the Magistracy obligations aimed at assessing the impact of this normative act, accompanied by possible proposals for its improvement, it is obvious that the legislator itself did not exclude that his legislative work does not constitute the masterpiece in the field. In this context, the present approach is intended to continue to emphasize, by the doctrine, some shortcomings of the Law No 140/2022 and to already propose a genuine reform in the matter. In the spirit of respecting the right to dignity of the persons with severe intellectual and psychosocial disabilities, a right that requires that a person who has acquired by law a full capacity of exercise should not be subjected to the degrading treatment of losing, totally or partially, this capacity, in the following we propose to be introduced, in the matter of capacity of exercise, the institutions of assisted capacity of exercise and of supervised capacity of exercise. Consequently, it required either the replacement of the institution of guardianship with the existing one, that of the personal assistant, as the case may be, of the professional personal assistant, or a specialization in the matter of guardianship, for such persons, of the sort of administrative guardianship. Abstract Background Allied health assistants (AHAs) support allied health professionals (AHPs) to meet workforce demands in modern healthcare systems. Previous studies have indicated that AHAs may be underutilised in some contexts. This study aims to identify factors contributing to the effective utilisation of AHAs across health, aged care and disability sectors and possible pathway elements that may optimise AHA careers in Victoria. Methods Using an interpretive description approach data collection included a workforce survey and semi structured interviews (individual and group). Data analysis included descriptive statistics, independent t-tests and thematic analysis. Participants included allied health assistants, allied health professionals and allied health leaders in the health, aged care or disability sectors; educators, managers or student of allied health assistance training; and consumers of Victorian health, disability or aged care services. Results The literature scan identified numerous potential barriers to and enablers of AHA workforce utilisation. A total of 727 participants completed the survey consisting of AHAs (n = 284), AHPs & allied health leaders (n = 443). Thirteen group and 25 individual interviews were conducted with a total of 119 participants. Thematic analysis of the interview data identified four interrelated factors (system, training, individual and workplace) in pre-employment training and workplace environments. These factors were reported to contribute to effective utilisation of the AHA workforce across health, aged care and disability sectors. Study findings were also used to create a conceptual diagram of potential AHA career pathway elements. Conclusion This study identified pre-employment and workplace factors which may contribute to the optimal utilisation of the AHA workforce across Victorian health, aged care and disability sectors. Further study is needed to investigate the transferability of these findings to national and global contexts, and testing of the conceptual model.

This article provides a reflection upon empowerment through spirituality. It presents the way in which the updating process of a spiritual guidebook was taken forward in Finland. The original guidebook, which was meant to be used in hospitals by healthcare professionals and chaplains, was published in 1996. At one point, it became evident that the original guidebook no longer correlated with the changing realities of Finnish society. At the time that the guidebook was originally published, Finnish society was rather homogenous and Christianity dominated the discourse. Nowadays, society has become more pluralistic and the discourse has become more enriched. The Nordic healthcare system increasingly includes people from diverse religious groups. The project to update the guidebook, which ran from 2014 to 2020, aimed to give social and healthcare professionals working in a variety of settings an easy way to update their knowledge on providing spiritual care. The primary objective of the project was to renew and update the guidebook to better match the contemporary spiritual landscape of Finland. The project had several stages. In the first phase, master's degree students conducted thesis research to produce new information and evidence for the process. In the second phase, data from the two theses were synthesized and analyzed, and the results were used to update the guidebook. The revised guidebook was ready to go to press in March 2020. In this article, we provide insights into the project, and reflect on our own understanding of the role of spirituality in our society and in healthcare based on the work of one of the master's theses. The master's thesis discovers relationships between disabled individuals and their assistants, and suggests changes to the guidebook through widening the horizon of spirituality in the caring relationship.
