Bullous pemphigoid (BP) is an autoimmune blistering disease resulting from autoantibodies directed against BP180 and/or BP230 proteins. BP is traditionally diagnosed based on clinical features, histologic assessment of cutaneous biopsies, direct (DIF) and indirect immunofluorescence (IIF) studies, and/or enzyme-linked immunoassay (ELISA) analysis. We here report, 6 elderly patients who presented with multiple tense bullae, clinically mimicking BP in the absence of detectable autoantibodies by DIF, IIF, and ELISA for BP-180, BP-230, and type VII collagen autoantibodies. All patients in this case series had histological overlap between BP and eczema, potentially representing a unique form of bullous eczema. Patient demographics are listed in Table I along with clinical images (Fig 1). Patients were evaluated in the Autoimmune Blistering Disease Clinic at Emory University. All patients had at least 2 negative DIFs on 2 separate occasions. Only 1 patient was subject to systemic immune suppression at the time of biopsy, and no history of diabetes, peripheral edema, or arthropod assault was noted. Spider bites are a common emergency department complaint, accounting for ~21% of noncanine bites, injuries, and stings. Military service members, more so than most civilian personnel, are exposed to conditions that place them at increased risk for envenomation, stings, and bites. We present the case of an active duty service member working in a relatively austere environment who presented to a level 1 trauma center with a lesion consistent with a bite from a brown recluse spider, or Loxosceles reclusa.

Skin disease is a common medical complaint in travellers returning from abroad. Frequent causes of travel-related skin disease include arthropod assault and infestation, viral exanthems and marine envenomation. It is important that primary care physicians and dermatologists are familiar with travel-related illnesses and their cutaneous manifestations, as well as how to approach their evaluation and management. Individuals planning to travel should be advised of precautions they can take to avoid contact with potentially hazardous organisms.

Key Clinical Message Dermatologic reactions are commonly encountered in clinical practice. Providers must be aware of both the common and uncommon etiologies leading to these eruptions, particularly in patients with underlying malignancies. Establishing the appropriate etiology directs treatment of these conditions, which may be therapy directed at the malignancy itself.

Wasp sting is a relatively common arthropod assault. This usually results in pain and mild allergic reactions, but sometimes may cause severe systemic reaction and multiorgan dysfunction including rhabdomyolysis, hemolysis, coagulopathy, hepatic, renal and cardiac complications. Along with several other pathomechanisms, rhabdomyolysis is a distinguished cause of acute kidney injury (AKI) in patients with wasp sting. We herein report a case in which the patient developed rhabdomyolysis followed by AKI due to multiple wasp stings. The offending wasp was brought to the hospital and the species was confirmed by a zoologist (Vespa affinis).

In pediatric patients, determining the culprit insect in arthropod assaults can be challenging. The patient's history may be vague, the causative insect may not be readily associated with the bites, and the clinical appearance of bites can be variable. Six pediatric patients from the Penn State Milton S. Hershey Medical Center dermatology offices with bed bug bites were identified. All had bites involving the face, trunk, and extremities. Five patients demonstrated papules on one upper eyelid associated with erythema and edema. One patient had papules on both upper eyelids. When an arthropod assault is suspected, the "eyelid sign," i.e., bites involving the upper eyelid associated with erythema and edema, may point to bed bugs.

Wasp sting is a relatively common arthropod assault, but is sometimes fatal because of anaphylaxis. Rhabdomyolysis is a serious condition, with destruction of striated muscles, and can be induced by various causes such as drugs, heart attacks, CRASH syndrome, and viper bites. Mass envenomation by multiple wasp stings can also cause rhabdomyolysis followed by acute renal failure, although it is extremely rare. We herein report a case who had an anaphylaxis-like reaction and rhabdomyolysis due to multiple wasp stings.

Background: A scanning microscopic clue to the diagnosis of arthropod assault reactions is a wedge-shaped inflammatory infiltrate. However, to describe an inflammatory infiltrate as wedge-shaped or not involves a high degree of subjectivity. Methods: We studied hematoxylin and eosin-stained sections of 137 biopsies of arthropod assault reactions for epidermal and dermal changes and for the composition, density and depth of the inflammatory infiltrate. Results: We found a wedge-shaped inflammatory component in only 10.2% of the cases. A much more common feature is an alteration of the interstitial tissue present in 85.4% of the biopsies. It consisted of a narrowing of the spaces between the collagen bundles, which was readily observable on scanning magnification. On higher magnification, a loosely textured basophilic material was often noted within the dermis. Conclusions: The hitherto often emphasized wedge-shaped configuration of the inflammatory component of arthropod assault reactions is not of great diagnostic value. The altered interstitial tissue is easily recognizable by its diminished interstitial spaces at low power magnification and can serve as a scanning magnification clue to the diagnosis of arthropod assault reactions.
