Introduction and objective: Historically, tracheostomy was the first curative treatment of obstructive sleep apnea (OSA). However, it causes important sequela that make it unacceptable nowadays. When obesity is severe, with a BMI>35, bariatric surgery has become the best and most common treatment for OSA. Synthesis: We present s summary of tracheostomy and bariatric surgery in OSA. Conclusions: Nowadays, tracheostomy has only minimal indications in OSA patients. In patients with severe OSA (AHI>30) and obesity with BMI>35, surgical treatment for OSA is bariatric surgery when patients do not tolerate CPAP. [ABSTRACT FROM AUTHOR]

Introduction and objective: Skeletal surgery for obstructive sleep apnea includes maxillomandibular advancement, with or without genioglossus advance, and maxillary expansion. Synthesis: We present a summary of techniques and results of maxillofacial surgery in obstructive sleep apnea. Conclusions: As upper airway improvement after bone surgery is stable and long-lasting, its effectiveness in reducing the apnea-hypopnea index is high in the long term, enabling this surgery to be included in recent consensus. Introduction and objective: Obstructive sleep apnea (OSA) is a multifactorial etiology disease that leads to the collapse of the upper airway. Hypertrophy of lymphoid tissues contributes to airway collapsibility. Conceptual review of adenotonsillectomy in the adult patient with OSA. Synthesis: In the presence of grade ≥3 tonsillar hypertrophy, tonsillectomy is the first-line treatment. The different available adenotonsillectomy techniques are suitable for treating patients with OSA. The integrity of the palatoglossus and palatopharyngeus muscles must be respected. In addition to healing and surgical success, tonsillectomy offers benefits such as reducing necessary pressure applied to CPAP and consequently increasing its tolerance. Conclusions: The removal of hypertrophic lymphoid tissues in adult patients with OSA is essential. La AOS es un trastorno que ocurre mientras la persona duerme, consiste en que el flujo de aire se detiene (apnea) o disminuye (hipoapnea) debido a que las vías respiratorias superiores se obstruyen. Este trastorno puede repercutir en problemas odontológicos y además puede ser tratado con dispositivos orales. Objetivo: Determinar la relación de la apnea obstructiva del sueño con la odontología. Materiales y métodos: Revisión sistemática, de tipo transversal, con un diseño descriptivo no experimental de corte retrospectivo con un universo n=1602 y muestra de n=33 artículos científicos. Resultados: Luego de la revisión de los 33 artículos se encontró que: no hay evidencia suficiente que confirme un vínculo causal entre el bruxismo del sueño y la AOS, se pueden desarrollar lesiones dentales secundarias a la AOS, 62,5% publicaciones de estudios de casos indicaron que existe una asociación entre la AOS, la maloclusión predominante en pacientes con AOS es la clase II división I en un 94,1%, 9 publicaciones describen la eficacia de los dispositivos orales como tratamiento de la AOS. Conclusión: Existe una relación entre la apnea obstructiva del sueño y la odontología, ya que este trastorno se asocia con patologías orofaciales además, el odontólogo puede diagnosticar y ayudar en la terapia de la AOS

Resumen: La apnea obstructiva del sueño (AOS) es una enfermedad frecuente que produce síntomas diurnos, deterioro en la calidad de vida, mayor riesgo de morbi-mortalidad cardio y cerebrovascular, deterioro neuro cognitivo y mayor accidentabilidad de tránsito. La terapia con presión positiva continua en la vía aérea (CPAP) es altamente eficaz y ha sido el tratamiento de elección en los pacientes con enfermedad grave y aquellos con enfermedad moderada con síntomas diurnos y/o comorbilidad cardiovascular, sin embargo, la CPAP tiene limitaciones en términos de aceptación y adherencia al tratamiento. Existen diversas opciones de tratamiento, como la pérdida de peso, el uso de dispositivos orales, medidas posturales, cirugía y la terapia miofuncional. Estos tratamientos personalizados han demostrado buenos resultados, especialmente en pacientes con AOS leve a moderada o en aquellos que buscan alternativas efectivas al tratamiento convencional. Se reconoce cada vez más que la AOS es un trastorno multifactorial con diversas presentaciones clínicas, fisiopatología y comorbilidades. Una mejor comprensión de la heterogeneidad de los pacientes permitirá enfoques de tratamiento más personalizados y garantizará la efectividad de las intervenciones mediante una medicina avanzada y de precisión. Esta revisión proporciona una descripción de los enfoques actuales para el tratamiento no quirúrgico de la AOS leve a moderada en pacientes sin indicación de tratamiento con CPAP o que rechazan dicha opción, además de proponer un algoritmo terapéutico para orientar el manejo de estos pacientes. Abstract: Obstructive sleep apnea (OSA) is a common disease that causes daytime symptoms, decreased quality of life, increased risk of cardio and cerebrovascular morbidity and mortality, neurocognitive impairments, and higher traffic accident rates. Continuous positive airway pressure therapy (CPAP) is highly effective and has been the treatment of choice for patients with severe disease and those with moderate disease accompanied by daytime symptoms and/or cardiovascular comorbidity. However, CPAP has limitations in terms of acceptance and treatment adherence. There are various treatment options available, such as weight loss, the use of oral devices, positional devices, surgery and emerging therapies like myofunctional therapy. These personalized treatments have shown promising results, particularly in patients with mild to moderate OSA or those seeking an effective alternative to standard treatment. It is increasingly recognized that OSA is a disorder with diverse risk factors, clinical presentations, pathophysiology, and comorbidities. A better understanding of patient heterogeneity will allow for more personalized treatment approaches and ensure the effectiveness of interventions through advanced and precision medicine. This review provides an overview of current approaches to non-surgical treatment of mild to moderate OSA in patients without an indication for CPAP treatment or who reject that option. Additionally, it proposes a therapeutic algorithm to guide the management of these patients.

Resumen: Actualmente, la apnea obstructiva del sueño (AOS) se define como un índice de apena hipopnea (IAH) de 5 eventos por hora. Este índice se basa en la frecuencia de los eventos respiratorios en función del tiempo. El IAH se usa como índice para predecir severidad de la enfermedad y para definir intervenciones terapéuticas. Sin embargo, el IAH no logra captar la heterogeneidad sustancial existente en la AOS (patrones de hipoxemia, respuesta simpática y diferencias de género). En los últimos años, se han descrito múltiples métricas derivadas del análisis de las señales del oxímetro de pulso, de la frecuencia cardíaca, electroencefalograma y señales de ventilación. Este artículo se enfocará en aquellas métricas validadas a través de estudios externos y de mayor proyección clínica. Abstract: Currently, obstructive sleep apnea (OSA) is defined as having an apnea-hypopnea index (AHI) of 5 events per hour. The AHI is based on the frequency of respiratory events over time and is used to predict disease severity and guide therapeutic interventions. However, the AHI fails to capture the substantial heterogeneity present in OSA, including patterns of hypoxemia, sympathetic response, and gender differences. In recent years, multiple metrics derived from pulse oximeter signals, heart rate, electroencephalogram, and ventilation signals have been described. This article will focus on those metrics that have been validated through external studies and possess greater clinical relevance.

Objective: To determine the clinical-epidemiological characteristics, diagnostic feasibility of home respiratory polygraphy and treatment of patients with suspected obstructive sleep apnea (OSA) at cardiovascular risk. Methods: An observational, cross-sectional, descriptive study was conducted in patients seen in a cardiology outpatient service with suspected OSA, from January 2015 to December 2019. The information was obtained from medical records, and a descriptive statistical analysis was applied to this information. Results: 138 files were reviewed; only 8% of the home respiratory polygraphs were discarded, because they did not meet the required quality standards. It was demonstrated that 89% suffered from OSA, 60% moderate to severe; in men after 50 years of age. The main cardiovascular risk factors was hypertension (89%). The most prevalent heart disease was hypertension (52%). Cardiovascular pharmacological treatment was improved in 82% of the cases. Cardiac rehabilitation in 30%, noninvasive mechanical ventilation 41%, fixed modality 33%, and self-adjustable 9%, all with telemetry. Conclusions: The prevalence and severity of OSA is higher in the presence of risk or established cardiovascular disease. In the presence of clinical suspicion, it is feasible to confirm the diagnosis with home respiratory poligrafy due to the level of precision and the lower infrastructure required. Greater involvement of the cardiologist in the diagnosis and treatment of this disorder is necessary due to the significant risk of cardiovascular disease it represents. [ABSTRACT FROM AUTHOR]

Background: This preliminary study examined the mediating role of illness representations on health-related quality of life (HRQoL) between adherent and poorly adherent obstructive sleep apnea patients (OSA) to the automatic positive airway pressure (APAP) therapy. Method: A total of 185 patients were assessed on determinants of APAP treatment, illness representations, family coping, and self-efficacy, at T1 (prior to APAP treatment) and T2 (1 to 2 months with APAP treatment). Results: Regarding the determinants of APAP, adherent patients showed higher self-efficacy, outcome expectations, and decisional balance index, compared to poorly adherent patients. Adherent patients also showed higher family coping and HRQoL, but less threatening cognitive representations compared to poorly adherent patients. Illness cognitive and emotional representations mediated the relationship between self-efficacy/family coping and HRQoL, in adherent patients. Only illness cognitive representations mediated the relationship between self-efficacy and HRQoL in poorly adherent patients. Conclusions: The results highlight the importance of illness representations during OSA treatment in the promotion of adherence to APAP. [ABSTRACT FROM AUTHOR]

Introducción: La apnea obstructiva del sueño, es un problema de salud que no siempre es reconocido y con frecuencia no se diagnostica. Objetivo: Describir algunos elementos epidemiológicos y clínicos, útiles para comprender la apnea obstructiva del sueño. Métodos: Se realizó una revisión descriptiva, sobre la apnea obstructiva del sueño, en bases de datos como Pubmed, Scielo y Google Académico. Fueron incluidos artículos de revisión, investigación y páginas Web, con menos de cinco años de publicados y se excluyeron los que no estuvieran en idioma español, portugués o inglés; se referenciaron 30 artículos. Conclusiones: La apnea obstructiva del sueño es un trastorno frecuente, máxime en hombres, en personas obesas y con otros factores de riesgo asociados. Se caracteriza por hipoxia crónica intermitente, fragmentación del sueño e inflamación sistémica. Aumenta el riesgo de desarrollar comorbilidades y provocar depresión, deterioro cognitivo, disfunción sexual y lesiones o muerte por accidentes resultantes de la hipersomnolencia.

Material y métodos: Mediante un estudio observacional tipo caso-control, se incluyeron pacientes portadores de asma de ambos sexos y un grupo control pareado por sexo y edad. Se consignaron datos demográficos, características de la enfermedad, datos relativos al control del asma y espirometría, comorbilidades y riesgo de apnea de sueño medido por el cuestionario STOP-BANG durante una entrevista estructurada. Resultados: Se incluyeron 132 individuos en el grupo caso y 132 en el grupo control. Entre los asmáticos, se consignaron 38,63% de formas no controladas y 9% severas. También se constató mayor frecuencia de hipertensión arterial, obesidad, rinitis alérgica y trastornos de la memoria. Riesgo mayor para AOS, aunque no significativo, se consignó en el grupo de casos. En el subgrupo de asmáticos con obesidad o en adultos mayores, la diferencia fue estadísticamente significativa- Discusión: Aun disponiendo de medicamentos preventivos, la tasa de control de los asmáticos en esta muestra es llamativa y merece estudios sistemáticos. Es extremadamente importante tener en cuenta algunas comorbilidades para el manejo de este grupo de pacientes, incluyendo la estratificación de riesgo para AOS.

21. The Value of Implementing a Digital Approach in the Obstructive Sleep Apnoea Patient Pathway: A Spanish Example

Introduction: Continuous positive airway pressure (CPAP) is the gold standard therapy for obstructive sleep apnoea (OSA). However, non-adherence is common and costly. The COVID-19 pandemic required the use of novel solutions to ensure service provision and quality of care. This retrospective analysis determined the impact and value of a digital versus standard pathway for the management of OSA in Spain. Material and methods: A time-driven activity-based costing approach was applied to OSA management over 1 year using a standard or digital pathway. The standard pathway included face-to-face appointments at the time of diagnosis, then after 1–3 months and every 6 months thereafter. The digital pathway had fewer face-to-face appointments and utilised telemonitoring. A cost analysis was performed to determine the per-patient cost per healthcare professional (HCP) for a digital pathway for therapy implementation and follow-up compared with the standard pathway. Results: Compared with the standard pathway, the digital pathway decreased the waiting list time from 18 to 2 months, the overall pathway time from 12 to 6 months, HCP cost per patient from €95 to €85, and number of hospital appointments per patient from 6 to 3.1. Furthermore, CPAP device usage improved from 5.7 to 6.3 h/night and the proportion of individuals defined as adherent increased from 79% to 91%. Conclusions: Implementation of digital processes using available technology reduced HCP time and costs, and improved adherence to CPAP in people with OSA. Greater utilisation of a digital pathway could improve access to therapy, allow personalised patient management, and facilitate better clinical outcomes. Resumen: Introducción: La presión positiva continua en la vía aérea (CPAP) es el tratamiento de referencia para la apnea obstructiva del sueño (AOS). Sin embargo, su incumplimiento es frecuente y costoso. La pandemia de COVID-19 requirió el uso de soluciones novedosas para garantizar la prestación de servicios y la calidad de la atención. Este análisis retrospectivo determinó el impacto y el valor de un pathway digital frente a un pathway estándar para el manejo de la AOS en España. Material y métodos: Se aplicó el Time-DrivenActivityBasedCosting al tratamiento de la AOS durante 1 año utilizando el pathway estándar o digital. El pathway estándar incluía citas presenciales en el momento del diagnóstico, después de 1 a 3 meses y posteriormente cada 6 meses. el pathway digital tenía menos citas presenciales y utilizaba la telemonitorización. Se realizó un análisis de costes para determinar el coste por paciente y profesional sanitario (HCP) del pathway digital para la implementación y el seguimiento de la terapia en comparación con el pathway estándar. Resultados: En comparación con el pathway estándar, el pathway digital redujo el tiempo de la lista de espera de 18 a 2 meses, el tiempo total del pathway del paciente de 12 a 6 meses, el coste del HCP por paciente de 95 a 85 euros, y el número de citas hospitalarias por paciente de 6 a 3,1. Además, el uso del dispositivo de CPAP mejoró de 5,7 a 6,3 h/noche y la proporción de pacientes definidos como adherentes aumentó del 79% al 91%. Conclusiones: La implementación de procesos digitales utilizando la tecnología disponible redujo el tiempo y los costes del HCP y mejoró la adherencia a la CPAP en personas con AOS. Una mayor utilización de un pathway digital podría mejorar el acceso a la terapia, permitir una gestión personalizada del paciente y facilitar mejores resultados clínicos.

Introducción: la epilepsia es un desorden caracterizado por la predisposición a generar crisis epilépticas, mientras que el síndrome de apnea del sueño (SAOS) ha sido reconocido como un desorden crónico de colapso intermitente de la vía aérea que genera hipoxia recurrente. En este trabajo se aplicó la escala de trastornos del sueño (Sleep Apnea Scale of the Sleep Disorders Questionnaire SA-SDQ), previamente validada en inglés para pacientes con epilepsia, a fin de determinar su capacidad para detectar apnea de sueño en nuestra población. Materiales y métodos: en una primera etapa se realizó la adaptación transcultural de la escala SA-SDQ en castellano, provista por los autores, al español colombiano. Luego se recopiló la información de los pacientes en quienes se realizó polisomnografía entre mayo y agosto del 2022 y se determinó el valor de corte para diagnosticar SAOS con la escala SA-SDQ. Resultados: cuarenta pacientes pudieron realizarse la polisomnografía, de los cuales 30 (75?%) tuvieron índices de apnea-hipopnea superiores a 5, lo que indica SAOS. El área bajo la curva fue 0,790 y la puntuación SA-SDQ de 21 proporcionó una sensibilidad del 73,3?% (IC 53,83-87,02?%) y una especificidad del 80?% (IC 44,2-96,5?%). La consistencia interna fue aceptable (? = 0,713). Conclusiones: la escala SA-SDQ es un instrumento útil para tamizar SAOS en la población colombiana que padece epilepsia. Nuestros resultados indican que los puntos de corte sugeridos anteriormente (29-36 para hombres y 26-32 para mujeres) pueden ser demasiado altos para nuestra población. Sugerimos un punto de corte de 21 para ambos.

La cánula nasal de alto flujo (CNAF) es una modalidad ventilatoria no invasiva segura y efectiva, usada ampliamente en patología respiratoria aguda en adultos y niños. Objetivo: presentar casos clínicos pediátricos que utilizaron CNAF por tiempo prolongado por problemas respiratorios crónicos. Descripción de casos clínicos, revisión de fichas clínicas de 5 pacientes que utilizaron CNAF por más de 1 mes, entre los años 2017-2020 en el Complejo Asistencial Dr. Sótero del Río. Aprobado por Comité de Ética. Resultados: 5 pacientes varones de mediana 61 (44 a 212) días de edad al inicio del uso de CNAF. Diagnóstico de base: displasia broncopulmonar (2/5), síndrome de Treacher Collins (1/5), síndrome de cimitarra con hipoplasia pulmonar derecha (1/5) y traqueobroncomalacia severa (1/5). Todos requirieron previamente uso de ventilación invasiva o no invasiva con mediana de 59 (4 a 78) días. A todos se les realizó broncoscopia, saturometría contínua o poligrafía para diagnóstico y titulación de CNAF y oxígeno. Todos mejoraron clínicamente, la SpO2 y el número de apneas. Dos pacientes se enviaron a domicilio con uso de Airvo2 nocturno. La mediana de uso de CNAF fue 165 (34 a 445) días. Conclusiones: el uso prolongado de CNAF es útil en pacientes pediátricos seleccionados, bien tolerado y factible de utilizar en domicilio.

Background: Obstructive sleep apnea is a highly prevalent disorder, characterized by recurrent events of upper airway obstruction during sleep and associated with recurrent cycles of desaturation and re-oxygenation, sympathetic hyperactivity, and intra-thoracic pressure fluctuations, resulting in fragmentation of sleep and subsequent daytime fatigue with excessive sleepiness. Obstructive sleep apnea-induced bilateral tonic–clonic seizures are unheard of. We aimed to report 3 patients with previously undiagnosed obstructive sleep apnea who presented to the emergency department with new onset bilateral tonic–clonic seizure without any evidential neurological or metabolic cause. Methods: Patient data were obtained from medical records from the Department of Internal Medicine, IPGMER and SSKM Hospital, Kolkata, and Belle Vue Clinic, Kolkata, India. Results: Three male patients (67, 58, and 44 years old) presented with bilateral tonic–clonic seizure disorder without any underlying cause of seizures after rigorous investigations except for moderate to severe obstructive sleep apnea on polysomnography. All 2 patients were seizure-free after being treated with levetiracetam, chronic continuous positive airway pressure therapy in 2, and only continuous positive airway pressure in the other. The patients remained seizure-free on continuous positive airway pressure, even when levetiracetam was withdrawn, suggesting obstructive sleep apnea's causality in their new-onset acute seizures. Conclusion: Although further investigation is required to clarify this association, underlying obstructive sleep apnea should be ruled out in patients with a first-ever bilateral tonic–clonic seizure. Whether or not continuous positive airway pressure alone could effectively treat hypoxia and deranged cortical excitability, which may lead to seizures in cases with long-standing obstructive sleep apnea, is yet to be explored. Resumen: Introducción: La apnea obstructiva del sueño es una enfermedad con una alta prevalencia que se caracteriza por episodios recurrentes de obstrucción de las vías respiratorias altas durante el sueño, lo que conlleva ciclos repetidos de hipoxia y reoxigenación, hiperactividad simpática y fluctuaciones en la presión intratorácica. Todos estos procesos dan lugar a una fragmentación del sueño, lo que provoca fatiga diurna y somnolencia excesiva. Las crisis tónico-clónicas bilaterales inducidas por apnea obstructiva del sueño son poco conocidas. Presentamos los casos de tres pacientes con apnea obstructiva del sueño sin diagnosticar previamente que acudieron a urgencias por crisis tónico-clónicas de nueva aparición sin causa neurológica o metabólica aparente Métodos: Los datos de nuestros pacientes se recogieron de los historiales médicos del servicio de Medicina Interna del Institute of Post-Graduate Medical Education and Research and Seth Sukhlal Karnani Memorial Hospital y de la Belle Vue Clinic, ambos en Kolkata (India). Resultados: Tres pacientes varones de 67, 58 y 44 años de edad presentaron convulsiones tónico-clónicas bilaterales sin causa identificada tras examen riguroso, exceptuando una apnea obstructiva del sueño de gravedad moderada a grave observada en la polisomnografía. Los tres pacientes recibieron tratamiento con levetiracetam durante el ingreso; al alta, se pautó tratamiento crónico con presión positiva continua de las vías respiratorias más levetiracetam en dos pacientes, y en el tercero solo presión positiva continua de las vías respiratorias. Ninguno presentó nuevas crisis tras la retirada de levetiracetam, lo que sugiere que la causa de las convulsiones era la apnea obstructiva del sueño. Conclusión: Aunque es necesario realizar más estudios para aclarar esta asociación, debemos descartar la apnea obstructiva del sueño en pacientes con crisis tónico-clónicas bilaterales de nueva aparición. Queda aún por determinar si la presión positiva continua podría tratar de forma efectiva la hipoxia y las alteraciones en la excitabilidad cortical, que podrían provocar crisis en casos de apnea obstructiva del sueño de larga evolución.

28. Cumplimiento del tratamiento con CPAP en pacientes con apneas obstructivas del sueño durante la pandemia COVID-19 en dos hospitales de Capital Federal

Introducción: La terapia con CPAP es el tratamiento de primera línea para la apnea del sueño y su eficacia depende de la adherencia. El control y seguimiento de forma presencial fue limitado debido a que nuestro país se vio inmerso en aislamiento social debido a la pandemia por COVID-19 a partir de marzo del 2020. Con el objeto de evaluar si se mantuvo la adherencia de la CPAP en pacientes con apnea obstructiva del sueño (AOS), en dos hospitales durante la pandemia COVID-19 y compararlo con la situación prepandemia sobre una base de control histórico de la ciudad de Buenos Aires. Métodos: Estudio observacional y retrospectivo basado en datos de recolección sistemática de adherencia a la CPAP e índice de apnea-hipopnea (IAH) residual. Para la comparación, se utilizó como referencia un control histórico correspondiente al periodo especular (mayo a diciembre de cada año entre el 2016 y 2019). Se incluyeron pacientes de más de 18 años de edad, con AOS con terapia con CPAP de más de 30 días de tratamiento. Se excluyeron pacientes con otras patologías respiratorias crónicas que requerían de terapias de ventilación (Bi-level, servo ventilación, ventilación con volumen asegurado). Resultados: Fueron evaluados 151 pacientes prepandemia y 127 del periodo pandemia respectivamente. Hombres 98 (65%) vs. 50 (60.3%) p: 0.9, edad de 65.4± 11.9 vs 63.6 ± 12.6 p: 0.22, índice de masa corporal 31.5 ± 5.0 vs. 31.2 ± 5.3 kg/m2 p: 0.6, respectivamente En ambos centros, el tratamiento más utilizado fue CPAP fijo; 90 (59.6%) vs. 96 (75.6%) p: 0.005. Existió un aumento en el cumplimiento del mismo en comparación al periodo prepandémico en minutos/noche [341.4 IC 95% 292.4 - 340.6 vs. 274.3 IC 95% 208.5 - 267.4, p: 0.001] y reducción del IAH residual [3.3 IC 95% 2.0 - 3.05 vs. 6.3 IC 95% 2.6 - 4.3 p: 0.006]. Conclusiones: En el periodo de pandemia COVID-19 se observó mayor adherencia al tratamiento con CPAP en pacientes con apneas del sueño.

For the proper treatment and management of these patients, it is necessary for the dentist to have adequate training. In this review article we present an updated summary of the scientific evidence on the association of bruxism and apnea, as well as management by the dentist with mandibular advancement devices. Objetivo: Determinar la variación de la calidad de vida en pacientes con Síndrome de Apnea Hipopnea Obstructiva del Sueño leve moderado, luego del tratamiento con Dispositivo de Avance Mandibular a una altitud de 3259 msnm. Métodos: Estudio prospectivo, cuasi-experimental. Se incluyeron sujetos con índice de Disturbio Respiratorio (IDR) entre 5 y 30 /hora (medido con poligrafía respiratoria), sin tratamientos previos, que presentaban condiciones para el uso del DAM. Se valoró calidad de vida con el FOSQ (Functional Outcomes Sleep Questionnaire) antes y después de 45 días de tratamiento con DAM. Resultados: 26 completaron el estudio (20 varones). La media (DE) de edad fue: 43 (9.6) años, IMC: 29 (8.6) kg/m2 e IDR: 19 (7.6) /h. Patients and methods: A prospective study in children (3–14 years), referred to the “Multidisciplinary Sleep Unit” due to suspected SAHS, between 1 November 2015 and 1 August 2017. RESUMEN Objetivos: Identificar cómo afecta a la calidad de vida de las personas de edad avanzada padecer apnea obstructiva del sueño. Metodología: Revisión sistemática sobre los diferentes estudios que traten la apnea del sueño en personas de edad avanzada, y cómo afecta esta a su calidad de vida. Esta revisión incluye casos y controles, estudios de cohortes, ensayos clínicos y estudios observacionales publicados desde 2014 hasta 2019. Resultados: Se revisaron cerca de 400 artículos, de los cuales solo 16 formaron parte de la revisión, tras cumplir con exactitud todos los criterios de inclusión establecidos. Existe una gran disparidad entre los resultados encontrados; aun así, la calidad de vida de las personas de edad avanzada con apnea obstructiva del sueño mejora con el uso de la CPAP (presión positiva continua en la vía aérea). Conclusiones: Son necesarias más investigaciones que estudien el síndrome de apnea obstructiva del sueño en personas de edad avanzada; la mayoría de los estudios encontrados se centran en población infantil o adulta. Entre los pocos estudios que se centran en estudiar la calidad de vida de las personas de edad avanzada cuando padecen apnea obstructiva del sueño se encuentra disparidad en sus resultados, no llegando a ninguna conclusión firme. Este es un tema prioritario de salud, debido al envejecimiento de la población y a la cada vez mayor prevalencia de este síndrome.

La apnea obstructiva del sue˜no (AOS) es un trastorno prevalente asociado con un aumento de la mortalidad. Este estudio tiene como objetivo caracterizar la carga de hipoxemia mediante el procesamiento autom´atico de se˜nales de oximetr´ıa nocturna en pacientes con AOS y analizar su relaci´on con la mortalidad. Se utilizaron datos de 420 pacientes con sospecha de AOS, empleando t´ecnicas avanzadas de aprendizaje autom´atico para crear modelos predictivos. La metodolog´ıa incluy´o el preprocesamiento de se˜nales de oximetr´ıa, la selecci´on de variables relevantes y la aplicaci´on de modelos de aprendizaje autom´atico, destacando Random Forest. Los resultados mostraron que los ´ındices de carga de hipoxemia son robustos predictores de mortalidad, superando a las m´etricas tradicionales como el ´ındice de Apnea-Hipopnea. En conclusi ´on, la carga de hipoxemia medida autom´aticamente proporciona una herramienta m´as precisa para predecir la mortalidad en pacientes con AOS., Grado en Ingeniería Biomédica

40. An explainable deep-learning architecture for pediatric sleep apnea identification from overnight airflow and oximetry signals

- Abstract
Producción Científica, Deep-learning algorithms have been proposed to analyze overnight airflow (AF) and oximetry (SpO2) signals to simplify the diagnosis of pediatric obstructive sleep apnea (OSA), but current algorithms are hardly interpretable. Explainable artificial intelligence (XAI) algorithms can clarify the models-derived predictions on these signals, enhancing their diagnostic trustworthiness. Here, we assess an explainable architecture that combines convolutional and recurrent neural networks (CNN + RNN) to detect pediatric OSA and its severity. AF and SpO2 were obtained from the Childhood Adenotonsillectomy Trial (CHAT) public database (n = 1,638) and a proprietary database (n = 974). These signals were arranged in 30-min segments and processed by the CNN + RNN architecture to derive the number of apneic events per segment. The apnea-hypopnea index (AHI) was computed from the CNN + RNN-derived estimates and grouped into four OSA severity levels. The Gradient-weighted Class Activation Mapping (Grad-CAM) XAI algorithm was used to identify and interpret novel OSA-related patterns of interest. The AHI regression reached very high agreement (intraclass correlation coefficient > 0.9), while OSA severity classification achieved 4-class accuracies 74.51% and 62.31%, and 4-class Cohen’s Kappa 0.6231 and 0.4495, in CHAT and the private datasets, respectively. All diagnostic accuracies on increasing AHI cutoffs (1, 5 and 10 events/h) surpassed 84%. The Grad-CAM heatmaps revealed that the model focuses on sudden AF cessations and SpO2 drops to detect apneas and hypopneas with desaturations, and often discards patterns of hypopneas linked to arousals. 42. Inmunoglobulinas y complemento como indicadores de gravedad en niños con síndrome de apnea-hipopnea obstructiva del sueño.

OBJECTIVE: To determine if the use of immunoglobulins and complement can be used as an indicator of severity in patients with obstructive sleep apnea-hypopnea syndrome. MATERIALS AND METHODS: Cross-sectional, observational, analytical study, which included patients aged 2 to 12 years, of both sexes and with a diagnosis of obstructive sleep apnea-hypopnea syndrome by polysomnography, treated from 2017 to 2020 in the Otorhinolaryngology Division of the General Hospital Dr. Manuel Gea González, Mexico City. The correlation analysis of the variables of interest was established by the Spearman Rho correlation test. 95% confidence interval (95% CI) was used as preci- sion measures. RESULTS: There were included 23 patients. Spearman's Rho nonparametric correlation was 0.53 for IgA, -0.1 for IgM, 0.38 for IgG, 0.31 for C3 and 0.02 for C4. The Pearson correlation test showed IgA of 0.64, IgM of -0.11, IgG of 0.17, C3 of 0.25 and C4 of -0.24. CONCLUSIONS: Immunoglobulin A is positively correlated with the severity and index of obstructive sleep apnea-hypopnea; however, immunoglobulin and complement values cannot be used as an indicator of severity in pediatric patients with obstructive sleep apnea-hypopnea syndrome. Introducción: el objetivo de este estudio es develar si la técnica Neuromuscular Funcional (NMF) puede ser considerada como opción de tratamiento en el Síndrome de Apnea Obstructiva de Sueño (SAOS) y si se complementa con la Terapia Miofuncional Orofacial o hace parte de ella. Métodos: la fenomenología trascendental de Husserl es el enfoque metodológico que se adoptó para este estudio, siguiendo la ruta para su desarrollo: epojé, reducción trascendental fenomenológica y síntesis. El contenido real se obtuvo mediante dos entrevistas semiestructuradas, aplicadas a un informante clave que contó con los criterios de inclusión presupuestados. Resultados: en la primera etapa se identificaron 10 noesis y 266 noemas, que fueron relacionados entre sí; en la fase de reducción trascendental las noesis se redujeron a 5 y los noemas a 14. Análisis y discusión: el Método Chiavaro como fenómeno se decanta en las dimensiones: (a) Enfoque Sistémico, (b) Técnica Neuromuscular Funcional y (c) Técnica Respiratoria. La prioridad del fenómeno NMF es la explicación de la alteración dentro del enfoque sistémico a partir de la lógica Función-Estructura-Función, de esta manera el SAOS se considera una manifestación de esas relaciones y como tal se le cataloga como adaptación o como función en disfunción. Conclusiones: la Técnica NMF hace parte de la neurorrehabilitación mientras que la TMO para intervenir el SAOS se inscribe en la fisiología del ejercicio.

44. Inmunoterapia con alérgenos de ácaros en pacientes con rinitis alérgica y apnea obstructiva del sueño

Para confirmar la presencia de apnea obstructiva del sueño (OSA) en rinitis alérgica (RA), se realizó poligrafía cardiorrespiratoria (CRP) y para tratar ambas condiciones, inmunoterapia alérgeno específica (ITAE). El diseño fue descriptivo en el universo de pacientes del servicio de Alergología del policlínico “Previsora” en Camagüey, Cuba, desde diciembre 2018 a marzo 2020. Se seleccionaron 326 pacientes en orden consecutivo de asistencia a la consulta, teniendo en cuenta los criterios: ≥5 años con RA, sospecha de OSA y prueba cutánea con ácaros: Dermatophagoides pteronyssinus, Dermatophagoides siboney y Blomia tropicalis, producidos en: “Centro Nacional de Biopreparados” de Cuba. Todos recibieron ASIT por ≥10 meses y se realizó CRP antes y después de la ASIT. La CRP se realizó utilizando el marcaje automático de eventos de ApneaLink AirTM (Resmed Corp., RFA), validado para estudiar los trastornos del sueño en el hogar. Los resultados de la prueba cutánea y de CRP se evaluaron antes y después de la ASIT; también la eficacia de la ASIT según criterios de pacientes y profesionales. De la muestra, 152 fueron del sexo femenino y 174 del sexo masculino para un 46,6% y 53,4% respectivamente; aquellos con habón de 5 a 6 mm fueron los más representados (p=0,04). Hubo descenso en los niveles de gravedad de OSA después de la ASIT (p=0,025). En la valoración de la eficacia de la ASIT, hubo un número significativo de mejorados (p=0,012). La CRP proporciona el diagnóstico de OSA en RA, y la ASIT cambia el curso de ambas condiciones.

Introduction and Objectives: Obstructive sleep apnea (OSA) is a prevalent condition among electable to bariatric surgery obese patients, often remaining underdiagnosed, thereby increasing surgical risk. The main purpose was to determine prevalence of OSA among candidates for bariatric surgery and to assess the rate of underdiagnosis of this condition. Additionally, the study aimed to evaluate the specific performance of three sleep questionnaires and scales (Excessive Daytime Sleepiness Scale (EDSS), Epworth Sleepiness Scale (ESS), and STOP-Bang) in these patients., Methods: A longitudinal, prospective, single-cohort study, with consecutive sampling including patients aged 18-65 years with obesity grade II (body mass index (BMI) ≥ 35 kg/m
2 ) and hypertension, type 2 diabetes, metabolic syndrome or OSA or obesity grade III or IV (BMI ≥ 40 kg/m2 ) elective for bariatric surgery. Patients were evaluated at the Otorhinolaryngology department with an anamnesis regarding OSA including the administration of three sleep questionnaires (EDSS, ESS, and STOP-Bang), followed by cardiorespiratory polygraphy (CRP) for sleep evaluation., Results: 124 patients were included in this study. While 74.2% of the sample exhibited OSA on CRP, only 28.2% had a prior diagnosis. The STOP-Bang questionnaire demonstrated the highest sensitivity (93.3%) for detecting moderate to severe OSA, although with low specificity (33.8%). EDSS and ESS did not show a significant association with the presence of OSA., Conclusions: OSA screening is crucial in candidates for bariatric surgery due to its high prevalence and low diagnosis rate. The STOP-Bang questionnaire may serve as a useful tool for identifying patients at risk of moderate to severe OSA and optimizing sleep assessments. 46. Does the number of loops affect the results of barbed pharyngoplasties? A retrospective comparison in two techniques.

Introduction: The use of barbed sutures for pharyngoplasty techniques is a new trend in sleep apnea surgery, but little is known about its short-term results depending on the different techniques. The purpose of this study was to analyse the surgical results in two different centres using barbed sutures with different barbed pharyngoplasty techniques., Material and Methods: This is a multicenter retrospective study of patients with obstructive sleep apnea (OSA) undergoing surgery, all of whom underwent pharyngoplasty with barbed suture using a classic or modified technique, which may or may not be associated with other surgical techniques. A univariate and multivariate statistical analysis were performed to assess the relationship of these surgical techniques with surgical success and with different descriptive variables., Results: The final sample size was 126 patients. A surgical success rate of 39.7% was obtained for a postoperative AHI < 10/h and a 51.6% success rate according to Sher's criteria. A greater proportion of success was observed in the modified barbed pharyngoplasty technique that was not maintained after the multivariate analysis, as with other factors such as age, ODI or the presence of nasal surgery., Conclusion: Non-resective pharyngoplasty with barbed sutures is an effective technique for the treatment of OSA in selected patients, with both the classic technique and the modified alternatives having good postoperative results, with no differences observed in relation to surgical success between both. 48. Trastornos del sueño en personas mayores. ¿Cómo duermen las personas mayores de la Ciudad de México?

Background: Good sleep is essential for physical and mental health throughout life. However, there are few reports describing the sleep problems experienced by communitydwelling older people. Objective: To describe the prevalence of sleep disorders and their relationship with medical conditions in communitydwelling older people. Material and methods: Cross-sectional study with 1678 older people (60 years and over) from the Mexican Institute of Social Security (IMSS) in Mexico City. Sleep duration, sleep quality, daytime sleepiness, insomnia and risk of obstructive sleep apnea were evaluated, alongside with sociodemographic variables and medical conditions. Results: Participants self-reported an average sleep duration of 6.04 (± 1.5) hours per night, approximately half had poor sleep quality (49.1%), and 2 out of 10 experienced daytime sleepiness (19.1%); one third had insomnia (30.8%) and risk of obstructive sleep apnea (27.5%). A pattern of higher prevalence of sleep problems was observed in older adults with depression, with cognitive impairment, with difficulties in basic activities of daily living and in those taking medications for the nervous system. Conclusions: This study showed that older people sleep few hours, have low sleep quality, and have a high prevalence of sleep disorders. The identification and treatment of sleep disordes in older people should be a priority in the IMSS. [ABSTRACT FROM AUTHOR]

49. STOP-Bang as a predictor of obstructive sleep apnea-hypopnea syndrome in outpatients.

The STOP-Bang has been promoted as a valuable tool for identifying obstructive sleep apnea-hypopnea syndrome (OSAHS) in medical and surgical patients. However, its performance in Colombian samples is unknown. The objective of this study was to determine the clinimetric performance of the STOP-Bang index versus the study of polysomnography in patients from Santa Marta, Colombia. An accuracy study of diagnostic tests with a test-based approach was designed. Seven hundred sixty-two adults referred for polysomnography to evaluate OSAHS were included in the research. They were aged between 18 and 94 years old (mean=47.2, SD=13.4), 63.3% were men, and 46.5% were classified as obese. The STOPBang performance was compared against the best reference criterion, the hypopnea/apnea index determined by polysomnography. The diagnosis of OSAHS was confirmed in 461 (60.5%) and corroborated in 301 (39.5%). The area under the curve was 0.70 (95%CI 0.66-0.74), and the best cut-off point was 4, with a sensitivity of 79.2%, a specificity of 53.5%, the positive predictive value of 72.2%, the negative predictive value of 62.6%, positive likelihood ratio was 1.70, negative likelihood ratio was 0.39; OR=4.08 (CI95% 2.99-5.56) and Cohen's kappa of 0.33. As conclusions, performance indicators show that STOP-Bang has little utility as a predictor of OSAHS in the evaluated population. This indicator should be abandoned. [ABSTRACT FROM AUTHOR]
