Abstract

Introduction: With a surge in the prevalence of coronavirus disease-2019 (COVID-19) in Beijing starting in October 2022, hospitalisation rates increased markedly. This study aimed to evaluate factors associated with in-hospital mortality in patients with COVID-19. Methods: Using data from hospitalised patients, sex-based differences in clinical characteristics, in-hospital management, and in-hospital mortality among patients diagnosed with COVID-19 were evaluated. Predictive factors associated with mortality in 1,091 patients admitted to the Beijing Anzhen Hospital (Beijing, China) for COVID-19 between October 2022 and January 2023 were also evaluated. Results: Data from 1,091 patients hospitalised with COVID-19 were included in the analysis. In-hospital mortality rates for male and female patients were 14.9% and 10.4%, respectively. Multifactorial logistic analysis indicated that lymphocyte percentage (LYM%) (odds ratio [OR] 0.863, 95% confidence interval [CI] 0.805–0.925; p < 0.001), uric acid (OR 1.004, 95% CI: 1.002–1.006; p = 0.001), and high-sensitivity C-reactive protein (OR 1.094, 95% CI: 1.012–1.183; p = 0.024) levels were independently associated with COVID-19-related in-hospital mortality. Among female patients, multifactorial analysis revealed that LYM% (OR 0.856, 95% CI: 0.796–0.920; p < 0.001), older age (OR 1.061, 95% CI: 1.020–1.103; p = 0.003), obesity (OR 2.590, 95% CI: 1.131–5.931; p = 0.024), and a high high-sensitivity troponin I level (OR 2.602, 95% CI: 1.157–5.853; p = 0.021) were risk factors for in-hospital mortality. Receiver operating characteristic (ROC) curve analysis, including area under the ROC curve, showed that the efficacy of LYM% in predicting in-hospital death was 0.800 (sensitivity, 63.2%; specificity, 83.2%) in male patients and 0.815 (sensitivity, 87.5%; specificity, 64.4%) in female patients. Conclusion: LYM% is a consistent predictor of in-hospital mortality for both sexes. Older age and markers of systemic inflammation, myocardial injury, and metabolic dysregulation are also associated with a high mortality risk. Abstract

Introduction: We assessed whether the social media-based (WeChat) intervention integrated with follow-up care could improve adherence to drugs, lifestyle changes and clinical risk markers in patients undergoing coronary artery bypass graft (CABG) in China.Methods: We randomized patients at hospital discharge following CABG to intervention group or control care in China. The intervention is a structured programme of cardiac health education, medication reminders and cardiologist-based follow-up service using WeChat platform. The control group maintains a routine practice pattern. The primary outcome is adherence to cardioprotective medications measured for 12 months after discharge. We also evaluated the lifestyle modifications and clinical risk markers at 12 months.Results: A total of 164 participants completed the trial for analysis. The intervention group had significantly greater adherence to statins use 98.6% vs. 75.0% (p < 0.01), beta-blockers 93.4% vs. 69.3% (p < 0.01) and aspirin 98.8% vs. 87.8% (p < 0.001). The intervention group had significantly greater adherence to regular physical activity (64.2% vs. 48.2%; p < 0.039). Furthermore, intervention versus standard group at 12 months had significantly lower mean systolic blood pressure and low-density lipoprotein cholesterol (p < 0.05).Discussion: A WeChat-based intervention strategy in post-CABG patients improved adherence to medications, including statin, aspirin and beta-blockers, and regular physical activity and resulted in an improvement in systolic blood pressure and low-density lipoprotein cholesterol level. Abstract

Background To simplify the assessment of the recording and control of coronary heart disease risk factors in different countries and regions. Design The SUrvey of Risk Factors (SURF) is an international clinical audit. Methods Data on consecutive patients with established coronary heart disease from countries in Europe, Asia and the Middle East were collected on a one-page collection sheet or electronically during routine clinic visits. Information on demographics, diagnostic category, risk factors, physical and laboratory measurements, and medications were included and key variables summarized in a Cardiovascular Health Index Score. Results Coronary heart disease patients (N = 10,186; 29% women) were enrolled from 79 centres in 11 countries. Recording of risk factors varied considerably: smoking was recorded in over 98% of subjects, while about 20% lacked data on laboratory measurements relevant to cardiovascular disease risk. Sixteen per cent of participants reported smoking, 29% were obese, and 46% had abdominal obesity. Sixty per cent of participants had blood pressure <140/90 mmHg (140/80 mmHg for diabetics), 48% had HbA1c<7%, 30% had low-density lipoprotein <1.8 mmol/l and 17% had a good cardiovascular health index score. There were substantial regional variations. Less than 3% of patients attended cardiac rehabilitation in Asia or the Middle East, compared with 45% in Europe. In Asia, 15% of patients had low-density lipoprotein cholesterol <1.8 mmol/l compared with 33% in Europe and 36% in the Middle East. Variations in medications were noted, with lower use of statins in Asia. Conclusions SURF proved to be practical in daily practice. Abstract

Objectives To analyse the type of blood pressure, blood pressure classification and pulse pressure among patients with uncontrolled hypertension and their relations with ischemic stroke Methods A total of 3909 uncontrolled hypertensive patients aged 35 years and above were identified from screening in the patients pool of community health stations/centres and tertiary Anzhen Hospital from March to December 2012, of whom 144 had ischemic stroke. Results There were 1932 (49.4%) males and 1977 (50.6%) females. Most (61.3%) patients had the lowest degree of blood pressure classification. The percentages of patients with uncontrolled diastolic pressure only, uncontrolled systolic pressure only and both uncontrolled were 18.5%, 33.8% and 47.7%, respectively. More female patients have both systolic and diastolic pressures uncontrolled (40.27% compared to 27.4%), while fewer females having only systolic pressure uncontrolled (13.9% compared to 23.9%). The likelihood for patients below 55 in developing uncontrolled systolic pressure was low (6.5% in age group of 35-44, 17.9% in age group of 45-54). The likelihood of patients above 65 in developing uncontrolled diastolic pressure was even lower (6.7%). After multivariable adjustment for age, gender, smoke, drink, BMI, dyslipidemia and exercise, the likelihood of developing ischemic stroke is 50% lower in patients with only uncontrolled diastolic pressure than patients with both diastolic and systolic pressure uncontrolled. The OR is 0.493 (95% CI:0.256∼0.947). The risk of stroke in individuals with diabetes was one point five times higher than individuals without diabetes OR = 2.534 (95% CI:1.726∼3.720). There was no significant association between blood pressure classification and risk of stroke and between pause pressures and risk of stoke. Conclusions Systolic pressure was not controlled in most patients with uncontrolled blood pressure. The risk of stroke increased significantly in patients with uncontrolled systolic pressure, of higher age and with diabetes.
